The identification of Critical Risks, their associated Critical Controls, and the implementation of processes to ensure their ongoing management can be difficult and resource intensive. Answering the following Critical Risk Management related questions can be challenging: • What is a Critical Risk? • What is a Bow-tie Analysis and when should it be used? • What is Critical Control? • How is the effectiveness of a Critical Control assured, and how is this measured? • Who is responsible for managing Critical Controls? • What are the implications if a Critical Control is not effective? It appears that in the mining industry’s efforts to implement the Critical Control Management Process outlined by the International Council on Mining and Metals (ICMM), instead of embedding an integrated, well understood, value adding process, we have instead created a resource hungry monster that is poorly understood by numerous layers of the organisation.

Six years ago it was my responsibility to give the presidential address, and now that privilege has come my way again. On that occasion we were gathered atWhyalla and I took the opportunity of putting before members some observations on the problems we then faced in Australia, and some gains we had been able to chalk up during those early postwar years. You may recall that we were then just beginning to emerge from the difficulties of postwar reconstructions - of fuel and power shortages, and a grossly inadequate manpower for the tasks facing us. The expansion of manufacturing industry in Australia greatly stimulated by wartime activities, and by the postwar developments of many institutions - ran ahead of the supporting services and of the supply of labour to do thejobs. For many industries this unfortunate state of affairs had meant operating at rates well below their installed capacities and therefore levels of output insufficient for Australia's rapidly growing needs. When I last spoke to you these problems were still with us, though their intensity was beginning to lessen as a result of vigorous efforts by industry and governments to reduce them. But I must confess that my thoughts on that occasion were full of those problems and of the effect they were having on the iron and steel industry of this country-of the coal shortage, the labour shortage, and the many other difficulties we were then faced with - and all at a time when the stepping up of our postwar expansion programme was of vital concern to all of us.What a different picture it is six years later! The profitability of individual ore blocks can be classed as clearly profitable, clearly not profitable, and those in between which are by nature quite sensitive to their major cost and revenue drivers. It is often the case that the most effort goes into the least profitable ore blocks to ensure that these will in fact not generate a loss. In these cases a cut-off grade policy is often not sufficiently live to separate individual sources given the volatility of financial markets and individual circumstances of some sources particularly in remnant operations such as Mount Isa MinesÆ Lead Mine. Standard sensitivity analysis can determine which factors stope profitability is most sensitive to. However, this does not usually indicate the likelihood of any input change, nor the combined effect of multiple inputs changing. The author attempted to modify the Lead Stream financial model that determines cash margin by inserting probability distribution functions and then using Monte Carlo simulations to determine the probability of any particular ore block making a profit. This effectively demonstrates the combined effect of varying a number of inputs simultaneously to provide a probability of making a profit. The limitations of this analysis lie in the accuracy that individual probability distributions can be specified. Attempts to generate valid functions from first principles have proved difficult for a number of reasons and these are discussed along with some practical alternatives.

Rock mass strength and brittleness generally have a significant impact on both cavability and fragmentation. Variability in these properties across a panel or block footprint is of particular interest because stronger units have the potential to significantly impact cave shape. This paper describes how FLAC3D™ was used to examine the impact of variability in rock mass properties on predicted cave performance to aid in assessing potential risks associated with the proposed design.A case study orebody was subdivided into a number of domains to reflect the heterogeneity of the rock mass at the scale of a proposed inclined cave. The results suggest that the presence of stronger units has the potential to impact cave shape significantly in the southern half of the cave footprint and could increase the potential for cave stalling at shallow depths. If this unit is stronger and less brittle than expected, an overhang is predicted to develop in the southern half of the inclined cave at depth. The effective hydraulic radius is significantly reduced due to the overhang in the south, and the cave could eventually stall at shallower depths in the north.CITATION:Garza-Cruz, T V and Pierce, M E, 2016. Energy Resources of Australia Ltd - Ranger Mine is located about 255 kilometres east of Darwin in the Alligator Rivers Region of the Northern Territory. The Ranger project is surrounded by Kakadu National Park which is on the UNESCO World Heritage list. . Ranger Mine operates the largest uranium mine and processing plant in Australia. It is designed to produce 3000 tonnes of uranium oxide per year, but presently is producing a little less than one half that figure due to the depressed uranium market. The safety performance of the employees and Ranger as a whole continues to improve. The lost time injuries (LTI) have significantly reduced from 74 in 1985 to five in 1994. The total days lost due to LTIs have gone from 1543 to 14. These significant improvements have reflected directly on workers' compensation premiums in which a four-fold reduction has occurred. It is estimated that the actual monetary saving to the company is about $1.3 million per year and directly reports to the profit of the company. There is also an obvious and significant reduction in the less quantifiable costs such as the suffering of the injured employees and cost to society. The paper will review the various initiatives introduced and assess their effectiveness. Performance improvement is primarily dependent on the commitment, introduction of various initiatives and the subsequent change in workplace culture.

Commencing with early experiences in iron ore beneficiation made at Beeshoek Mine, the paper follows through the process of extensive mineralogical and metallurgical investigations carried out during feasibility study. The paper will discuss the design process and logic followed over the last years, the challenges and decisions the design team faced in order to draw the conclusions that ultimately resulted in today’s value add approach at Assmang’s Khumani Operations.As integral part of the benefi ciation concept, a focus is set on the jigs, explaining the plant concepts, planned expansions and their contribution to the overall value add approach at the mine.The case study is intended to describe the outcomes of the design versus the actual achieved and the further value add projects to be implemented. As for future prospect, the incorporation of Wet High Intensity Magnetic Separation (WHIMS) demonstrates Assmang Khumani’s pursuit to maximise productivity and overall recovery.DMS was evaluated as an alternative to jigging within a comparative test work study, but in conclusion the option had to be excluded due to its inability to achieve the high density cut point required to achieve the sales specifi cations. In contrast, the investigation on producing an additional middlings fraction on the lumpy ore jigs prove to be an adequate option for further improvements of overall mass yield. With further liberation, a saleable product will be achieved by secondary beneficiation on Fines Jigs and WHIMS.Concluding, jigs are considered the most viable and cost effective benefi ciation process for off-grade ore +0.8 - 32 mm at Beeshoek as well as at Khumani, while WHIMS is the process under consideration for super fines recovery.

Most mines in the Bundamba, North Ipswich, and Rosewood districts of the West Moreton coalfield of south eastern Queensland have installed washing plants during the last five years. Many problems were encountered in their installation, including the small size of washing plant required in many cases, the economical minimum ash content under present and future market requirements, production from a one-product washbox of large and small coal of quality to satisfy both markets, measures to avoid adding unnecessary moisture to the washed slack coals, and excessive middlings in some coals or soft clay in others.INTRODUCTIONIt was fortunate for Brisbane, which was to become the capital of a new State, Queensland, in 1859, and grow into a city of over half a million people by the time of its centenary, that coal was found in abundance almost on its doorstep near Ipswich, the first mine being opened at Redbank in 1843. This was followed by the opening of many more mines in that district and, some 40 years later, in the nearby Rosewood district where high-volatile gas coal was produced (Fig. 1). These discoveries ensured a supply of reasonably priced coal (currently about 85/- per ton delivered) in Brisbane, but the quality is inferior to that of other coals mined in Queensland beyond economic range of Brisbane.Various factors led to an increase in the average ash content of the West Moreton coals during and after the war. Between 1940 and 1952 the gasworks coals rose in average ash from 13 8 per cent to 23·9 per cent and the powerhouse coals from 17·1 per cent to 27·1 per cent. This drift was arrested by the installation of 28 screening and picking plants from 1952 onward, but it became obvious that mechanical cleaning of the coals was essential due to the competitive market for coal and the worsening quality of reserves.

In the flotation of platinum group minerals pyroxene is a major gangue mineral. This study shows using zeta potential and ToF-SIMS analyses that when pyroxene is exposed to copper, nickel and/or calcium ions in solution at alkaline pHs a surface reaction occurs between the hydroxy species, in the case of copper and nickel, or divalent calcium ions and the pyroxene surface which influences the floatability of the pyroxene. Following exposure to copper and nickel ions at pH 9 addition of SIBX resulted in the pyroxene floating readily. It is also shown, in the case of copper, that the activating effect of the adsorbed copper hydroxy species can be reduced by the addition of complexing amines prior to SIBX. Proposals are made to explain these observations.

Factors leading to the necessity for some individual testwork in solving most specific hydraulic transportation problems are listed and a description is given of a method used in determining critical flow conditions. An outline is given of the basic theory of the purpose of the testwork, and a means of applying test results for varied pipeline diamet.er and material conditions follows. Engineering and economic factors are applied to the solution of a specific hydraulic transportation problem and the use of the test results in plant practice is illustrated. The advantages of connecting pumps in series in centralised pumping stations rather than individually along the pipeline are stressed. Some notes are added on the future possibilities and economies of post pump injection of solids into high pressure pipelines.THE HYDRAULIC TRANSPORTATION OF SANDSAs well as all the classical variables involved in a pipeline installation, one must consider, when solids are to be transported, the effects of such variables as:(i) Proportion of solids in the pulp.(ii) The average specific gravity of the pulp.(iii) Specific gravity of the solid material.(iv) Solid particle average sizing and possibly shape.(v) Solids particle grading.(vi) Pipeline diameter for effects other than classically hydraulic.(vii) Pipeline material.(viii) Fluid viscosity.(ix) Pump characteristics.(x) Abrasive or corrosive properties of the pulp.Not surprisingly, the application of published data to the hydraulic transportation of a specific material is difficult if not well nigh impossible. The Mathews et al (1981) stability graph was developed to provide guidance in the determination of stope sizes to control dilution in bulk mining of large orebodies. Longhole and sublevel open stoping (LHOS and SLOS) are the lower end in scale of underground mass mining with panel and block caving (PBC) at the higher end. Currently, the stability states of stopes in the stability graph are described qualitatively as stable, unstable or cave. The alternative quantitative option to the qualitative stability graph is the Equivalent Linear Overbreak Slough (ELOS) stability graph. The ELOS stability graph only applies to narrow vein orebodies. It is easy for the Mathews et al (1981) and the ELOS stability graphs to be applied outside the limits of their databases. What is currently referred to as ‘stable’ in the present stability graphs has diverse interpretations at different mines depending on their acceptable dilution values. In 2014, a research program was conceptually conceived at the UNSW Australia School of Mining Engineering to develop a quantitative dilution-based stability graph independent of orebody width to address some industry concerns on the use of the stability graphs. A dilution-based stability graph provides the mining engineer the flexibility to determine open stope sizes based on what dilution amounts are acceptable. The graph also unifies both the Mathews et al (1981) and ELOS stability graphs for wide and narrow vein orebodies, thus, eliminating their potential misapplications. To date, the concept is proven with case studies from metalliferous underground mining operations located across Australia. The literature pertaining to the electrochemical reaction at the anode during electrolytic reduction of alumina by the Hall-Heroult process is not in universal llgreement about the gaseous product. The possibility of electrochemical formation of CO at the anode was examined using existing data. Although the thermochemistry favours co-evolution of CO and CO2 with CO as the major product, the mechanism of the reaction shows this is unlikely. Since the origin of CO can be more logically explained by non-electrochemical mechanisims, it is concluded that CO2 is the only product from the electrolytic reaction at all current densities.

Cliffs Natural Resources Pty Ltd - Asia Pacific Iron Ore's (formerly Portman Iron Ore Limited) K deposit (Dowd's Hill), at its Koolyanobbing operation in Western Australia, hosts enriched magnetite mineralisation below the presently mined haematite-goethite ore zone. Two distinct northwest-southeast elongated, highly magnetic anomalous bodies that dip to the north-east appear to be sourced by this deeper mineralisation. Magnetic modelling indicates significant remanent magnetisation with Qratios around 1.5, directed shallow down. The north-west anomaly is overlain by a ridge of uneconomic banded iron formation (BIF) and the present waste dump, whereas the south-east body was partly exposed during ongoing mining of overlying haematite-goethite ore. Sections of the magnetite mineralisation in this south-east anomaly were intersected previously by diamond drill holes. Review of the historic cores identified potential magnetite enrichment, with grades and mineralogy that might be amenable to beneficiation. A first-phase reverse circulation drilling program was designed to further delineate the trend and extent of the two anomalies. The following magnetite mineralisation assemblages are recognised from cores, reverse circulation (RC) drill cuttings and pit mapping of the south-east magnetic body: higher-grade magnetite-haematite ¦ goethite and magnetite ¦ pyrite and lower-grade, recrystallised magnetite-quartz (BIF), magnetite-carbonate, magnetite-talc and minor magnetite-bearing mafic schist. Disseminated specularite is locally present in theassemblages. Talc-schist and massive pyrite occur along the footwall contact. Magnetic susceptibility measured from drill chips within the magnetite-enriched zones reveals an apparent susceptibility values range of 0.2 to 1.3 SI. Preliminary results from Satmagan and Davis tube recovery analysis shows that the magnetite content in these mineralised zones correlate well with existing geophysical data and magnetic susceptibility measurements.

Base metal sulphide mineralisation on the northern end of Great Barrier Island is spatially associated with hypabyssal intermediate to acidic intrusives, in transgressive structures in indurated, complexly deformed greywackes and argillites. Copper predominates in the mineralisation, with minor accompanying lead and zinc and also some gold and silver. However, in some cases lead or zinc may be the dominant constituent. The mineralisation is generally characterised by only narrow envelopes of weak alteration (propyliticassemblages dominated by chlorite and epidote) and with only minor amounts of quartz accompanying the sulphides, which form a cement in zones of hydraulic fracturing. There are two distinct structural settings to the mineralisation: at MinerÆs Head, in a structurally complex situation in a shell-like zone of crackling in hornfelsed greywacke, in association with both north- and east-striking fractures and rubble breccias associated with the apical part of a large intrusive; and in very narrow, but very strike-persistent, 1inear steeply-dipping fracture zones, exposed on the north coast and inland areas near Tataweka.

The Huntly West Mine has undergone significant changes in the last two years, which will affect the remainder of the mine's life. The paper describes the evolvement of plans and an overview of the technical considerations made in planning for and implementing the first New Zealand longwall operation. The technical investigations involve the longwall equipment, exploration, ventilation and strata control. The focus on these areas was to ensure that unacceptable project risks were addressed. Implementation of longwall mining at West Mine is planned in three phases, Development, Panel One extraction trial, and Full Longwall operation.   At present the development phase is complete, longwall installation is in progress and production start-up is planned for the end of August, 1991.

Waste waters from mine sites may be screened for all constituents of significance as a first step in establishing water quality criteria for protection of aquatic ecosystems. Inductively-coupled plasma mass spectrometry (ICP-MS) offers the capability to screen water samples and digest solutions for virtually all elements in the periodic table. This technique is therefore able to identify elements whose concentrations are elevated in waste waters and may be rarely present in background surface or ground waters. Such elements may be present in waters at only a few ¦g/L. ICP-MS has sufficient sensitivity to detect sub-¦g/L or even ng/L levels of many elements. Waste waters from most mining activities in the Northern Territory have been examined by ICP-MS. The concentration data provide the basis of a contaminant data base. The data can show specific tracers in the ore or mineral itself, subsequently found in waste water. An example is the presence of rhenium in uranium ore. Such trace elements may be present in soluble or insoluble chemical forms and are more useful as indicators when soluble and exemplifying conservative chemical behaviour. The approach described, based on ICP-MS screening of mine site waste waters, together with general physico-chemical variables, provides the most comprehensive means yet available of selecting variables for monitoring programs and ensures that key elements and/or their chemical species are not excluded from monitoring programs.

A risk-robust development of a heavy mineral resource requires an assessment of the geological uncertainty and spatial variability of the key factors impacting the mining and processing operation. Attributes of interest are the total heavy mineral grade, the slime content and the amount of oversized particles. The modelling of a mineral sand deposit presents two main challenges. Firstly, the exact location of the geological boundaries are obscure due to the complex interaction of the deposit formation processes. Secondly, the properties of interest exhibit a rather complex transitional behaviour across the so-called soft boundaries. Traditionally, the modelling exercise is restricted to a consecutive stochastic simulation of distinct geological facies and orebody properties. This results in different zones separated by hard boundaries, ignoring the typical transitional behaviour.This contribution presents an alternative approach which models the entire sand body as a single domain while accounting for the complex trends present. Error curves are used to infer covariance model parameters without the need of a prior removal of the trend. The principle is based on minimising the differences between empirical and theoretical errors resulting from two methods of prediction calculated for different rings of influence. The added value results from the unambiguous decomposition of the spatial variation into a deterministic trend and a stochastic residual. The trend model neither overfits nor underfits the data. The calculated residuals are thus unbiased and the uncertainty can be correctly assessed.The effort results in a characterisation of the resource uncertainty which provides a more realistic image of the spatial variability of the key properties in and across the geological domains of an existing heavy mineral sand deposit.CITATION:Wambeke, T and Benndorf, J, 2014. The use of stable isotopic measurements as a technique for elucidating the origin and history of the water in hydrothermal fluids is relatively new. Recept studies have emphasized the importance of meteoric water in the formation of many hydrothermal ore deposits including many epithermal gold-base metal deposits which occur in faulted volcanic terrane. Other vein-type gold ores are believed to have formed from fluids of magmatic or metamorphic origin and exhibit characteristic d18O values. Preliminary oxygen and carbon isotopic data for some important gold deposits of eastern Australia have emphasized the diverse origins of their ore fluids.The Mount Morgan gold-copper orebody is shown to be of the volcanogenic massive sulphide type with ore fluids predominantly of sea water origin. The Charters Towers fissure veins formed during hydrothermal activity associated with intrusion of Lolworth Igneous Complex equivalents. Oxygen isotopic studies of the quartz reefs at Stawell and Gympie have been inconclusive. The ore fluids have a d18O value of about 7 per mil and may be of magmatic or metamorphic derivation.

The successful development of industrial minerals projects requires an integrated, holistic understanding of the metallurgical and product performance test work in the context of the geological setting of the deposit and applied to mining, processing and marketing of saleable products. The product specifications provide the essential starting point for geometallurgical studies. A sound understanding of the desired product is needed to design and interpret a program of test work. Examples are provided from recent studies on bentonite and graphite deposits.For bentonite projects, the naïve testing of only the cation exchange capacity (CEC) as a measure of purity can lead to unexpected outcomes when the deposit is mined. The CEC attribute may remain the same, while other important properties such as pH and swelling index change markedly with depth, due to weathering and oxidation, leading to disappointing production outcomes.For graphite projects it is not enough to report only total graphitic carbon content (TGC). Knowledge is required of the in situ flake size and textures in addition to purity and flake size distribution of extracted graphite, which may all vary from one domain to another across the deposit. Such geometallurgical attributes of the ore will impact on the ‘basket price’ used to estimate the value of marketable parcels of expected product.Disclosure of such information must be considered part of fully informing the market. As with all new project developments, consideration of the JORC (2012) and VALMIN (2015) Codes requires disclosure of all material information. This includes the geometallurgical characterisation, ie those aspects of the mineralogy that impact on resource evaluation, ore reserve definition and project valuation.CITATION:Shaw, W J and Scogings, A J, 2017. Except for uranium search, conventional oil well logging techniques are relatively little-used in mining exploration. The main reasons for this are cost and an apparent lack of awareness of the potential use of the methods in the search for ore. However, relatively inexpensive portable units are currently available for performing the simpler electrical and radiometric logs in diamond drill holes.The emphasis in oil exploration logging programmes is largely on the estimation of porosity. In most mineral work, however, porosity is not very significant, and factors such as mineral conductivity and radioactivity assume an important role.The Bureau of Mineral Resources (BMR) during a programme to explore for uranium and associated base metals in the area east of Rum Jungle, N.T., carried out electrical and radiometric logging of diamond drill holes as a routine part of their investigations. Case histories are described in which the logs were used to aid in defining stratigraphic sequences and transgressive structures in base-metal prospects. Some surface geophysicalanomalies were explained by the logging: induced polarization and electromagnetic anomalies were related to black slate; and self-potential anomalies were related to oxidation of pyrite black slate, or apparent electrokineticeffects in the black slate. The logs provided an objective appraisal of the rock units penetrated and thereby helped to differentiate lithologies.Borehole logging, including newer techniques such as downhole analysis, could be done in geological and geophysical investigations in other areas for a relatively small cost with a considerable potential gain of information.
