The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. During early 2004, Fortescue Metals Group (FMG) began an aggressive exploration and drilling program in the Chichester Range, targeting iron enrichment developed in the Nammuldi Member at the base of the Marra Mamba Iron Formation. The timely recognition by FMG of distinct zones of (hypogene) microplaty hematite (mplH) enrichment overprinted by extensive supergene mineralisation led to an early reappraisal of the geological history of the area and characterisation of the possible structural and stratigraphic controls on mineralisation. Regionally, the Chichester Range dips gently at less than 5¦ to the south, however, detailed mapping shows that this regional limb is overprinted by low amplitude, long wavelength, north/south to northeast/southwest trending folds. The locations of major structures in the Marra Mamba Iron Formation, combined with spectral analysis of known mineralisation, allowed extrapolation and prioritisation of exploration target areas. The delineation under cover and understanding of these structures was enhanced by detailed airborne magnetic data. Recognition that supergene mineralisation was associated with a palaeotopography that continued significantly under cover to the south under sediments of the Fortescue River valley led to the discovery of extensive supergene mineralisation together with additional mplH mineralisation. A model for the landscape evolution and preservation of enriched Nammuldi Member was generated in order to help communicate both the complexities involved with targeting drill holes beneath cover and the potential limits to the lateral extents of mineralisation.

The iron ore producer Rana Gruber AS is situated in northern Norway just below the Arctic Circle, and has been producing hematite and magnetite concentrates for more than 60 years. Since the late 1990s, the major part of the production comes from a sublevel stoping underground operation after many years of open pit mining. Despite moderate mining depth, the operations have experienced considerable rock stress problems due to quite high tectonic, horizontal stresses. To manage the problems, comprehensive rock mechanics investigations have been carried out by SINTEF, a major contract research organisation. This has included in situ rock stress measurements, laboratory investigations and numerical modelling. Based on this and practical experience, satisfactory design of vertical and horizontal pillars was achieved. At depth, the ore width increases from 30–40?m to more than 100?m. This, together with a major planned increase in production, called for a change of the mining method. After thorough investigations and considerations, it was decided to change to sublevel caving, very much like the LKAB system. The development of the new system has been closely followed up by rock mechanics measures, including 3D numerical modelling and comprehensive monitoring of deformations and rock stress changes. To achieve a satisfactory caving of the hanging wall, all vertical and horizontal pillars in the sublevel stoping area had to be removed. This has been a challenging task, and the development of the caving has been slow until recently. Observation from attending many conferences around the world reveals that many people are constantly shifting from the word ‘error’ to the word ‘uncertainty’ without a clear vision of what the subtle difference is. The same applies to a large swath of the scientific literature, in which these two concepts are unfortunately often used synonymously, which is a scientific flaw of the first order. In practice, as demonstrated in the Theory of Sampling (TOS), there are both sampling errors and sampling uncertainties. Sampling errors can easily be preventively minimised, while sampling uncertainty for a preselected sampling protocol is inevitable. P M Gy’s work, supported by G Matheron’s critical reviews, demonstrates beyond any possible doubt that sampling uncertainty cannot be achieved if sampling errors are not preventively minimised by applying very stringent rules that are too often ignored by sampling practitioners. A clarification, in line with the TOS, is presented to mitigate the confusion created by circles favouring the use of uncertainty versus the use of the word error. Flinders Mines is an iron ore explorer and developer with a tenement package located in the Hamersley Ranges region of Western Australia, approximately 70 km north of Tom Price. Exploration over the past five years has identified a Global Resource in excess of 900 Mt at a grade of 55.2 per cent Fe (+50 per cent Fe cut-off). This resource is spread across multiple deposits and has been divided into a number of mineralisation type domains. These domains are comprised of transported hematite rich detrital iron deposits (DID) and goethite dominant channel iron deposits (CID) as well as in situ bedded iron deposits (BID). The BID at PIOP is low in alumina and silica with a high calcined iron content making it an attractive exploration target. BID exploration techniques include a combination of mapping, structural analysis, downhole gamma logging and 3D geological modelling. Relationships identified between existing BID mineralisation, geology and structure has aided in targeting additional mineralisation across the tenement and highlighted other previously untested areas that warrant further investigation.CITATION:Tylkowski, L N, Corlis, N J, McDonald, G D, Petts, A E, Jennings, M F and Thomas, B J, 2013. The Musselwhite mine, located in northwestern Ontario, Canada, has been an underground narrow-vein gold producer since the spring of 1997. As an exploration project at the feasibility stage, much effort was undertaken to generate a geostatistical resource model that would integrate the orebodyÆs geological features, as well as the controls on mineralisation. Today as an operating mine producing over 200 000 ounces of gold per year, reconciliation studies of the tonnes and grades treated at the mill, with the feasibility model, for longer time periods, and with the grade control models, for shorter time periods, show results that are in good agreement. It was clearly demonstrated with the reconciliation to production, that the key to good reconciliation remains in the proper selection and application of the geological resource estimation method. The application of a grade interpolation technique capable of capturing threedimensionally pertinent geological features, as well as accounting for spatial grade continuity, proved to be successful in the predictability of tonnage and grade at the Musselwhite mine. Along with good mine practices, this approach, which was carried through to the operation, has produced excellent reconciliation results. The ongoing reconciliation studies at Musselwhite allow for a constant revision of the grade estimation strategy. In return, this optimisation process adds value by raising the accuracy level of production expectations. As this is an evolving practice at the mine site, further implementation developments are recommended to integrate these reconciliation procedures on a more regular basis.

Arsenic-containing waste was dissolved into molten slags and the leaching behaviour of the quenched arsenic-doped slags (containing up to 2.3 wt per cent arsenic) was ascertained to evaluate the potential of this method as a means for arsenic disposal. The leaching behaviour was evaluated using acetic and sulphuric/nitric acid based solutions.From short-term (18 hour) leach tests, the amounts of arsenic in the leach solutions were found generally to decrease with an increase in the Fe(III)/Fe(total) ratio of the slag or an increase in pH of the leach solution. The amount of arsenic in solution increased with an increase in the slag basicity, the slag arsenic concentration or the surface area of the slag exposed to the leach solution. From longer term leach tests (up to 40 weeks) in the pH range 5 to 8, the arsenic remaining in the leach solution was believed to be controlled by sorption processes. The sorbent was an iron oxide-rich material which had the capacity to lower the arsenic concentration in the leach bottles to well below 1 ppm in some cases. At a leach solution pH over 9, the arsenic concentration in the solution increased with time, although the rate of solubilisation was much slower due to the high pH.Slags have the potential to stabilise arsenic and prevent it entering the environment provided attention is paid to the slag chemical composition and oxygen potential.

Stresses around mine openings at depth can cause the surrounding rock to fail, releasing stored strain energy. When this happens, the rock may literally explode or `burst' into the opening without warning. As mining progresses deeper into the earth, the possibility of seismic events and rock bursts increases. Researchers at the US Bureau of Mines (USBM) are currently studying seismic activity in three underground hard-rock mines in the United States: the Homestake Mine, Lead, SD; the Lucky Friday Mine, Mullan, ID; and the Sunshine Mine, Kellogg, ID. Waveforms from seismic events are recorded by personal-computer-based hardware and software. This information is archived and transferred from the mine sites to the USBM via modem communication links. Then, using USBM-developed software, the events are analysed to determine various parameters, including magnitude, frequency, and location. In addition, researchers are modelling the deep levels of the Homestake Mine using a finite-element code. These models generate stress and displacement values for a given loading condition. By studying material properties, in situ stresses, and seismic activity along with actual scope sequencing, researchers can make correlations between stresses induced by mining and the frequency and magnitude of seismic events. Information on seismic events coupled with results from finite-element analyses have increased understanding of rock mass behavior and the mechanisms that may lead to rock bursts. Optimisation of slope sequencing designs as a result of these studies could reduce rock burst hazards.

A discussion on simulation tools to aid the development of robotics platforms is presented. The focus of the work is towards mining robotics platforms where operations are typically in unstructured, unpredictable environments and a high degree of dependability in their operations is required. It is argued that a test-based development approach leads to more robust solutions and faster development cycles than traditional single-track field-only development and that the key enabling technology for this development paradigm is the judicious use of simulation tools. The future direction of simulation for use in online applications such as active planning and as a means to assist tele-operation in high latency communication applications is also brief y considered together with the requirements for a useful simulation environment in terms of the simulator architecture. Finally, a case study is presented on the application of the discussed methodologies to an unmanned ground vehicle (UGV) for earth-moving tasks, which is being developed in an accelerated time frame with a transient development team.

Mining is a fundamental activity on which modern society and the world's economy are entirely dependent. Mining investors, large and small, invest on an international scale, frequently being active in several countries.   Prior to committing investment to new countries, the prudent investor will consider the risk involved by analysing a number of physical, financial, administrative and political parameters that impact on the potential financial returns it is seeking. This paper considers these risk factors in relationship to New Zealand and six competitor countries. The conclusion is that for reasons largely within the control of our politicians, New Zealand is a high risk country for mining investors.

A testwork program aimed to improve the assaying of underground face samples established that for an Accelerated Cyanidation Extraction using the LeachWELLTM 60X catalyst, followed by an Aqua Regia finish (to quantify non-cyanide-extractable gold): ò Assay results (including the aqua regia finish) from a full set of samples from one underground face can be returned within four hours. ò The reagent dose to make the method simple and 'bullet-proof' is 8 gm LeachWELLTM 60X and 20 gm NaCN per 200 gm sample (30 per cent solids). Pulverising is to 90 per cent passing 75 micron (compared to the standard of 90 per cent passing 150 micron), and cyanidation is completed within one hour - however both might be further optimised. ò The LeachWELLTM/Aqua Regia results demonstrate that Fire Assays suffer from a significant nugget effect which makes the Fire Assay data relatively unreliable. ò Original costs, specific to Bannockburn (viz including an Aqua Regia finish), handling approximately 20 000 to 25 000 per annum is estimated to be $5.40, ie about one-third less than fire assaying. Optimising is further reducing these costs. It is estimated that the LeachWELLT"'/Aqua Regia assay procedure has reduced the probable error limits of any given assay of this ore from the range 0.74x to 1.36x for Fire Assay, to an appreciably smaller range of 0.90x to 1.11x. Thus for a cut-off grade of 1.60 g Au/t, samples with Fire Assays ranging up to 2.18 g Au/t have 25 per cent chance of being below cut-off, while samples with Fire Assays as low as 1.18 g Au/t have an equal chance of actually exceeding cut-off grade. This 1 g Au/t range in uncertainty is generally unacceptable for many practicalgrade/tonnage relationships. By contrast, using LeachWELLTM + Aqua Regia assays, the estimated corresponding limits are 1.44 g Au/t and 1.78 g Auk respectively - a far more tolerable 0.34 g Au/t range of uncertainty. Subsequent data has demonstrated that sufficient accuracy for grade control is achieved by Accelerated Cyanidation alone, further halving the cost of assays.

With the launch of Immersive Technologies' next generation of Advanced Equipment Simulators for the mining industry, correlating and influencing safety, productivity and maintenance with operator competency improvements is now made even more effective. In early 2008, Immersive assembled a project team to investigate the potential linkages between operator competency and performance. Advanced Equipment Simulator Users with a range of sophistication were interviewed and consulted. The result was new and enhanced 'value realisation' programs to directly assist mine sites to deliver the expected return on investment (ROI) from simulators and operator competency development. In just over one year, 29 mine sites around the world have benefited from 34 completed programs. Case studies have been compiled to demonstrate the impact of the implementation of this approach, including at the following operations: Anglo Coal - Callide (Australia), Freeport McMoRAN - Grasburg (Indonesia), Equinox Minerals - Lumwana (Zambia), Peabody Energy - Cabllo (USA), and Xstrata - Lomas Bayas (Chile). Significant performance improvements have been achieved including: reductions of 47 per cent in gear shifting and 37 per cent in over speed errors, improvements of 68 per cent in park break errors and 12 per cent in spotting time, and increase of 54 per cent in passing emergency fire handling competency tests. This presentation draws on these case studies to clearly demonstrate the linkage between improved operator competency through the implementation of training programs tailored around Advanced Equipment Simulators and production performace measured by a range of standard metrics at mines around the world.

Mineralogy and coproponents Sino Iron, International Minerals and Austeel are in the process of developing four world class magnetite iron ore mines with multiple open pits along a 25 km strike length of Brockman Iron Formation to the south of Cape Preston on the West Pilbara coast. The close proximity of the proposed pits raises some challenging project approvals and environmental management issues relating to the impacts of dewatering at individual projects and the cumulative impacts of progressive development on local and regional groundwater resources, which includes a large fresh water aquifer system in the Fortescue River alluvium.This paper presents an outline of the hydro-environmental impact assessment process adopted and the key result that the cumulative impacts of dewatering at all four projects on the local brackish groundwater system in the basement rocks is only marginally more than the impact of any one project on its own, and that the impact on the Fortescue River alluvium aquifer is negligible. The paper also presents an outline of the proposed integrated water management approach that will involve coordinated monitoring, ongoing impact assessment and management, and reporting programs for all projects.

The Culampajß auriferous district is located in the northern sector of the eastern edge of the homonymous hill, Catamarca Province, Argentina. Tajo Largo mine is situated in the southern flank of Las Minas gulch at an approximate height of 3.600 m above sea level. In the studied area main outcrops are orthogneiss rocks of Chango Real Formation. The ore mineral is native gold with a lesser amount of pyrite, chalcopyrite, haematite, limonite, malachite, azurite and covellite. These ore mineral assemblages are disseminated in quartz veins filling tension fractures in the host orthogneiss.Several types of inclusions have been recognised, including three phase H2O-CO2liq-CO2vap inclusions, polyphase inclusions with H2O-CO2 and one or more solid phases, and two phase aqueous inclusions. In the three phase H2O-CO2liq-CO2vap inclusions the Tmclath is about 1¦C and the CO2 liquid-vapour homogenisation occurs at 26 - 28¦C, to liquid phase. The homogenisation temperature of H2O-CO2 occurs at 365 - 380¦C in the aqueous phase. This set of data defined a solution with a salinity of 14, 4 wt per cent NaCl equivalent and a density of 0.84 g/cm3. In the two phase aqueous inclusions the melting temperature (Tm) of -5.4¦C and the eutectic point (Te) in -38¦C indicates a salinity of about 7.5 wt per cent NaCl equivalent, with the probable presence of Ca2+. Density of fluid is about 0.83 g/cm3. Homogenisation temperature ranges between 260¦ and 280¦C. The pressure, temperature and salinity of fluids in the polyphase inclusions suggest an early fluid of high density and high salinity. The three-phase fluid inclusions may be the result of an unmixing-phase separation of post magmatic hydrothermal fluid due to pressure decrease along fractures. It is suggested that the deposit was formed during the late magmatic hydrothermal stage genetically linked to granitic rocks and can be comparable with the intrusion-related gold system of Baker (2003).

PIP is regularly engaged in businesses where Greenfield Capital Projects are under- delivering on the expected business case. In most of these projects, we can identify that rigorous review has been carried out on the design and financial cost elements and substantial reporting is maintained to Boards on progress versus plan of project completion and cost. However, in all sites of this nature that we have worked at, the planning, design and execution of the operational elements or æwiringÆ of the organisation have fallen short and resulted in significant extra cost or opportunity cost (lost tonnage or quality) during start-up. When compared to the amount of planning and resources that have been poured into the capital side, the ledger on the æwiringÆ side is typically fairly sparse. This presentation outlines the importance of running a disciplined, rigorous process to prepare the human and wiring side of a start-up to ensure rapid start-ups and put the site on the pathway to high performance. It gives an outline of the rigorous process PIP has used with projects to ensure excellent start-ups.

The mining industry is facing economic pressures to treat more challenging and lower grade ore types, as well as pressure relating to environmental impact and social license to operate. Preconcentration through bulk ore sorting has the potential to benefit all these areas by enabling mining operations to divert material of low economic value prior to processing, thereby achieving higher feed grades and recovery, while reducing the processing plant and tailings dam footprint and capital required throughout the life of mine. The effectiveness of bulk ore sorting depends on the heterogeneity of material at the measurement location, as well as the accuracy and precision of the measurement. An understanding of ore heterogeneity can be assessed using drill core data, which provides metre by metre elemental assays. This paper reviews such data and assesses the grade distributions over different scales, which suggests that sufficient heterogeneity exists at mining scales to allow the potential for bulk ore sorting. Prompt Gamma Neutron Activation Analysis (PGNAA) technology has proven to be a successful measurement technique within many industries, including mining. PGNAA was first developed in the early 1980’s for the measurement of the elemental composition of bulk materials on a conveyor belt. Today, Pulsed Fast Thermal Neutron Activation (PFTNA) technology is also applied in elemental analysis of bulk materials and has some advantages for bulk ore sorting. In order to enable effective bulk ore sorting, however, a PGNAA/PFTNA analyser must be designed to provide a representative bulk ore analysis with the necessary accuracy and precision. Examples are presented to provide an understanding of the factors affecting PGNAA/PFTNA performance in relation to effective bulk ore sorting.

The Porgera gold project is now in its seventh year of operation as one of the world's prime gold mining projects in one of the most underdeveloped and remote regions of the world. The unique gold deposit was development in stages to its present size of operation with an underground mine of 2000tpd, an open pit operation moving 210,000tpd and a complex metallurgical plant treating an average of 16,500tpd of ore. During the past years the operation had to address an array of issues relating to the "development" of the region and this is still ongoing. Development means different things to different people and though the parties that signed the different agreements relating to the Mining Development Contract were aware of this, none has real- ised the extent to which this is true. The developing company (a joint venture between Placer Pacific as the Manager, Goldfields Porgera, Highlands Gold and the Government of PNG), provided funds for risky exploration and the capital for development of the project. Its shareholders ex- pect an economic and viable mine operation from the deposit to ensure them returns on their investment. The local people gave up their land, their isolated way of living and accepted to be out of their comfort zone for some time in anticipation of a better lifestyle and economic benefits in support of that lifestyle. The PNG Government has a leg in both camps and expects both outcomes at the same time. The Government is also the regulator and expects compliance on top of that. If development in general is to be successful, all parties must substantially satisfy all expectations. It is evident that the outcome of some elements of development are reasonably defined (i.e. develop a viable mine to exploit the deposit), whilst other elements are moving targets (i.e. a better lifestyle). The seeds for conflict are thus abundantly present and to resolve them requires the will of all parties in all areas to come to a solution. This path to problem solving is many times torturous as each party comes from a different culture and background and has a different understanding of the parameters in the equation.

This presentation discusses a water filtration technology termed Fabric Media that is somewhat novel to the Australian market place and reports several recent mining industry installations. Like many novel household cleaning products, the Fabric Media technology applies the science of active washable micro fibres as the means of capturing contaminant solid particles. The simple Fabric Media water filtration product offers several attractive attributes when compared to the more traditional media bed filtration processes such as sand filters, and barrier filters such as micro membranes. A micro fibre based filtration media enables both superior adsorption for the contaminant and wash ability. Stronger adsorption capture enables a much higher flux and greater solids loading density for the filtration task, meaning that lower cost and smaller footprint equipment is available. For Fabric Media water filters, a textile weave is selected that minimises the flow backpressure for a given particle size separation. Thus for an equivalent filtration performance this provides a much lower operating energy requirement, generally between 50 and 80 per cent less than for a sand filter. A Fabric Media is not abrasive to itself or to the filter tank. The filtration performance remains constant and predictable. The equipment life is much longer and the asset value of the new filtration equipment should maintain throughout the project life. Fibres are extremely mechanically robust and chemically inert. Fabric Media survives repetitive exposure to a rapid mechanical washing event providing high wash efficiency with minimum equipment off-line time for the backwash. Dislodging captured contaminants by a high-speed mechanical agitation enables a minimum off-line time during backwash events. The vigorous backwash destroys adhering flocculant and eliminates the formation of mud balls, which can permanently foul sand filters. The mechanical agitation wash of the Fabric Media water filter ensures complete removal of grease and biofilm, which can adhere and block a membrane surface. In comparison to sand filters and micro filtration membranes, the Fabric Media product is applicable to a significantly wider range of solids concentrations without permanent damage. The technology is very forgiving to excursion in plant control and is therefore considered a very operator-friendly concept.

This paper describes the mining methods and strategy used to extract the 5 Orebody crown pillar at the Isa Lead Mine over the past five years. The bulk of the crown pillar was extracted by the use of sublevel open stope method. An uphole bench stope was mined to scavenge the remaining ore. Geomechanical issues associated with the crown pillar extraction are described. Stope sizes were gradually increased on a stope by stope basis. This has allowed the maximising of ore recovery while relevant risks were managed. The principles in designing ore rib pillars against various types of fill materials are also described.
