Contemporary approaches to safety management appear to be failing short of meeting its mark in improving mine safety. This is evidenced by the high workers compensation, high incidence rates and fatalities. Evidence from high-risk and complex organisations points towards safety culture as being important in improving site safety. In more recent years resilience engineering has been touted as a new and innovative way of managing safety. This paper reviews and synthesises previous literature on safety culture and resilience engineering. It then highlights methods that can be used to measure safety culture and resilience engineering, and explores similarities and differences between complex organisations and gold mining to identify opportunities for more innovative approaches to improving safety in gold mining operations through safety culture and resilience engineering.

Australia as a leading producer of mineral sands has also occupied a leading position in many areas of research'in the processing and applications of these minerals. Research in upgrading ilmenite to produce synthetic rutile has resulted in the develop- ment of many processes, e.g,., Becher, Murso, Chlorine Technology, and ICI processes. Studies on the reducibility of ilmenites, aqueous leaching of reduced ilmenites, Fe-Ti-O system, leachability of natural and treated ilmenites in hydrochloric acid and selective chlorination of iron from ilmenites have been carried out in order to develop and improve the efficiency of the above processes. While research on the processing of zircon sand to produce high purity (99.99% ZrO2 zirconia powder and zirconium chemicals started only recently, research by CSIRO in the early 1970s led to the discovery of the phenomenon of transformation toughening in partially stabilized zirconia (PSZ) ceramics. Australia, with very significant resources of lanthanide-containing ores, pro- duces substantial quantities of monazite each year but research in processing and appli- cations of rare earths was neglected in recent years in this country. However, research to improve processes for the recovery of rare earths is now under way and studies on the production and testing of Nd-B-Fe magnets which are 1.5-2 times stronger than the strongest magnets (Sm-Co) currently available, have also commenced.' An overview of the research in the pro- cessing and applications of mineral sands during the last two decades in Australia and its implications for the industry are pre- sented in the paper.

The Mine granite at the Ardlethan tinfield is dated by Rb-Sr on a muscovite concentrate at not less than 417 ± 2.5 Ma (2s), consistent with the intrusion of this granitic body into folded Late Ordovician sediments. A combination of Rb-Sr and K-Ar evidence suggests a slightly younger age, 41O ± 2.5 Ma, both for the neighbouring Ardlethan Granite and for the hydrothermal alteration of the Mine granite, with which the tin is associated.On this basis, the somewhat high 206Pb/204Pb of two galena samples from within the hydrothermal alteration zone is best explained by an increase in U/Pb of order 10 per cent during a geochemical event in the Late Precambrian, in the rocks which eventually yielded the Pb to the mineralising fluid. This earlier event probably increased Th/U in these rocks as well, by as much as 5 per cent. If these reconstructions are valid, the lower-than-average U/Pb implied for the original Precambrian source rock bespeaks a mantle component of marginally greater proportion than for "average orogene".

Northern Australia leads in world-class mineral deposits' is a bold statement. First there must be a starting point. Land north of the Tropic of Capricorn (23¦ 26' 5") will be regarded as Northern Australia covering nearly 40 per cent of the land surface of Australia. As opposed to land south of the Tropic of Capricorn, it should be regarded as Tropical Northern Australia and for the sake of brevity, (TNA) Refer Figure 1. Apart from the tyranny of distance from centres of power and population in the temperate zone, it suffered in the past from the belief that the tropics are unsuited for European workers and from the lack of infrastructure. Outstanding mineral deposits of iron, base metals, gold and diamonds, discovered and developed in the past three decades, provided the substance for the title. There are many world-class mines producing and there are mineral deposits neutralised through Government policy. TNA can maintain its lead in world-class mineral deposits provided Government policy will stop partitioning the land and decreasing that available for exploration. Let's look at what we have that is world-class. In Western TNA iron ore deposits at Paraburdoo, Mt Tom Price, Marandoo, gold at Telfer, diamonds at Argyle. In central TNA uranium at Ranger and Jabiluka, lead-zinc-silver at McArthur River, gold at the Tanami, manganese at Groote Eylandt and bauxite at Gove. In eastern TNA Mt Isa, Hilton, Century zinc-lead, bauxite at Weipa, gold at Kidston, Mt Leyshon, magnesite at Yamba and coal mines in the Bowen Basin. Those mines or deposits stand out yet are only an indication of the numerous lesser mines and of mineral deposits depending for development on market demand and/or changes in Government policy, eg 'The Three Mines' policy. Very large areas of mineral rich structures in the western, central and eastern parts of TNA are concealed below flat lying comparatively young unmineralised sediments. The Great Artisan basin sediments in eastern TNA can and do host oil shale and non metallic minerals. Nearly 40 per cent of western TNA is covered by the Great Sandy Desert. The Canning Track, with a series of 54 wells, was constructed by surveyor Alfred Canning in 1908 - 1910 with great difficulty. It extends 1700 kilometres from Halls Creek in the north to the gold town of Wiluna in the south, through the Great and Little Sandy Deserts. Gold explorers are following favourable structures extending to the north east below the sediments and sand hills covering the track. Half of central TNA's mineral structures are concealed by the Tanami desert and the Cretaceous sediments covering the pastoral regions of the Barkly Tableland and the Daly Waters to Top Springs pastoral areas. The Georgina Basin in the south eastern section is part of the concealment.

A mining companyÆs approach to managing the risk of theft of explosives and initiating devices should be underpinned by an organisation-wide policy developed with internal and external consultation. It should be based on the principles of sound proactive risk management, planning, monitoring and remedial action. This paper aims to provide mining (and construction)organisations with the rationale and many of the tools they need to control explosive security risks. As with all legislative compliance, they are best managed by incorporating the statutory and legal requirements into company policies, management systems and plans, operating procedures and culture. This is the approach that has been adopted in this paper. Trends we should expect: more prescriptive legislation and stricter government regulations; enhanced enforcement and more prosecution; more sophisticated detection systems, eg airports; additional compliance costs; additional vigilance on record keeping; improved risk awareness and risk communication; wider use of explosives policies, management systems and plans; and additional security staff with investigative skills.It should be noted that mining companies will be required to play their part in reducing the likelihood of explosives, intended for mining purposes, being wrongfully used to threaten, hold to ransom or to injure individuals or members of the public or to produce property damage. This will be done by putting in place safeguards to reduce the risk of theft of explosives while they are under their control. On that basis, this is a business risk assessment rather than a hazard risk assessment. A conclusion of this paper is that mining companies can and should play their part in reducing the supply of explosive materials that can then be used for illegal activities. In fact, mining companies have always had this duty, but now the stakes are higher û æcomplacency no longerÆ.

THE Yallourn chimney has aroused interest, as it is, for the time being, the tallest structural erection in Australia. Its height was not decided on, however, with the object of breaking records, but is purely incidental to the purpose which it serves. Theboiler plant at the briquetting works (first instalment) consists of five Babcock and Wilcox land-type water-tube boilers, each of 4510 sq ft. heating su:r£ace and 225 sq. ft. grate area in the form of step grates. Four boilers will normally produce 73,000 lb. of steam per hour at a gauge pressure of 265 lb. and a temperature of 6500 F., the fifth boiler being a reserve. Each boiler will consume about 100 tons of Yallourn coal per 24 hours. Provision is made for future installation of economizers, should it be found desirable. Designing for this condition, and allowing for a waste gas temperature of 3500 F. and an air temperature exceeding 1000 it was found necessary to have a draft of about 1.3 in. (water gauge) at the base of the chimney, and to deal with about 2000 cub. ft. of gases per second. Estimates were made of the cost of providing the necessary draught by means of fans and by means of a tall chill).ney. The annual charges of the two methods differed only slightly, and the tall chimney wasselected as being the more general practice in briquetting factories.Estimates were prepared for brick and concrete chiIlllleys. Yallourn bricks at that time were hand-made, and, while very suitable for domestic work, were not capable of withstanding high compressive stresses. The chimney design to suit these bricks thus required to be on very ample lines, resulting in high cost. Machine-made bricks from Melbourne would cost about £5 10s. per thousand on the site, with extra for radials, so that high cost again resulted. On the score...

"Wet-weather or 24×7 wet-trafficability is a commendable design objective for unpaved mine haul roads, but practically often difficult to achieve, due both to the road sheeting material selected, and also its breakdown under the action of traffic and environment. Accidents allied to wet-trafficability issues, either due to rain or often more commonly, over-watering of mine roads, is increasing and especially the loss of control or uncontrolled vehicle movements categorisation. The dominant system failure is that once vehicle control is lost, control cannot be regained.The friction supply, or ‘skid resistance’ of a mine haul road sheeting is critical to the safe operation of the road and data has to be collated concerning the variation of friction supply with sheeting material engineering or indicator properties. In the light of the paucity of data existing in this field, this paper presents the contributory research that informs the development of improved and validated unpaved mine road friction supply estimates, based on a number of mine site evaluations across a range of sheeting material types and characteristic parameters, leading to improved predictive models of friction supply.Results from this preliminary review will enable operators to assess their road sheeting materials, improve and validate unpaved mine road friction supply estimates. The Nokomai valley is located within the pumpellyite/actinolite facies of the Otago Schist, a regional scale metamorphic belt in Southern New Zealand. The Otago Schist is dominated by psammitic and pelitic greyschist with more subordinate greenschist, quartzite, marble, and ultramafics. Mining in the Nokomai, began in the 1860s, with sluicing of terrace gravels in the lower valley. Good gold is reported to have been won from unconformities within very weathered gravels lower in the terrace sequences, and particularly from within channels which dissect these older deposits. Gold was recovered from these terraces in the main Nokomai Valley, and two major tributaries (Victoria Gully and Bullock Head Creek). In the period 1885 - 1887 further gold was discovered in basal gravels beneath the Nokomai flats when the Hunter brothers sank two shafts and excavated a connecting drive to discover a lead 1.2 - 1.5 m thick. The Golden Lion Sluicing Company began mining in 1896 using an hydraulic elevator on the very 'tight' 1.8 - 2.4 m of basal gravels. This operation was followed by the Nokomai Gold Sluicing Company whose average return grade was >485 mglm3 until mining ceased in 1943 (Wopereis; 1988, 1989). A quartz vein, Commissioners Reef, was worked for small production at some time during the 1920 - 1930 period, until fault termination of the vein ceased production.

On 15 August 1998 the first section of the PUTRA LRT system was opened to the public as a free service prior to official commencement of revenue service on 1 September 1998. It is a remarkable achievement to open a highly advanced medium capacity urban railway scheme in four years after the outline concept began. Even more remarkable is the implementation of the next stage which is the underground section planned to be operational in mid-1999. The decision to build the first underground railway in any major urban centre is a technical challenge. The PUTRA LRT System can be likened to Hong Kong when the initial MTR system began in the 1970s and Singapore MRT in the 1980s. Prior to the PUTRA railway very little bored tunnelling had been undertaken in Kuala Lumpur. A few shallow sewer tunnels had been built but the main experience of tunnelling in Malaysia had been for water, hydroelectric and road tunnels usually through granite formations. The ground conditions in Kuala Lumpur were known mainly from experience of piled foundations and occasional deep excavations. The suitability and likely difficulties to be experienced during bored tunnelling could only be theoretically predicted. In both Hong Kong and Singapore, government owned corporations were set up to implement the scheme, but in Kuala Lumpur the whole project has been undertaken by a private company PUTRA, a wholly owned subsidiary of Renong Berhad.

The Western Australian gold industry has approximately 100 operating plahts, mainly CIP/CIL. The nature of the State's rapidly expanding gold industry gave rise to concerns over management of the  cyanide hazard, in that most plants are remote from major population centres, have a workforce with a high tum over and use highly saline process water. Approximately 4500 workers in the industry have the potential to be exposed to cyanide in the course of their normal work.   This paper outlines the risk management strategy developed by Government and industry to monitor atmospheric HCN gas in the work environment and the role of continuing education and training along with regular site inspections by Department of Mines Inspectors.

The process of longwall coal extraction coal causes fractures in the overlying and underlying strata and these fractures become pathways for gas released from adjacent coal seams to flow into the mine workings and contaminate the ventilation air. If the rate of gas emission exceeds the diluting capacity of the ventilation air, the gas concentration will increase and exceed the statutory limit resulting in production delays.All potential gas sources within the planned mining area, including coal seams located above and below the working seam, should be identified and sufficient gas data collected and used to determine the specific gas emission from each gas source. Gas reservoir and emission modelling is recommended to determine specific gas emission and changes in gas emission from individual sources over the planned mining area. Accurate gas reservoir and emission modelling provides the information required to accurately design gas drainage programs to effectively manage gas emissions and minimise the risk of ‘gas-outs’.Given the increasing occurrence of incidents of gas concentration in ventilation air exceeding the 2.0 per cent statutory limit and changes to mine plans to avoid ‘difficult-to-drain’ areas, it is apparent that many operations are not effectively utilising gas reservoir and emission modelling to identify high gas emission areas and are not completing sufficient gas drainage in advance of planned mining.The process of gas reservoir and emission modelling to support gas drainage design to control tailgate gas concentration and fugitive emissions is presented and discussed.CITATION:Black, D J, 2017. Industrial minerals contribute about 50% of the NZ$1.2 billion generated annually by the minerals industry in New Zealand. The value is supplemented by a further NZ$500 million of minerals imported to supply our vital agricultural and industrial activity. As globalisation of the world's economy precedes, sources of minerals are being processed by fewer companies giving an ever-increasing control over global markets. If only to protect its primary production New Zealand needs to aggressively develop its mineral resources. The successful development of a deposit requires identification of the preferred market. While low-value aggregates dominate New Zealand industrial mineral production it is more specialised minerals supplied to the manufacturing and industrial sectors that provide more value per tonne. Minerals that have highly desirable intrinsic properties, or through value-added processing provide desirable properties, have the ability to fill niche markets both locally and overseas. Research and development towards enhancement or creation of desirable mineral properties could benefit New Zealand with both increased mineral potential and export of technological information. But, it is nearly always the market not the resource that dictates the viability of an industrial mineral and, with the globalisation of industrial mineral markets resulting in fewer but larger companies, any supplier must be ready to explore niche markets in order to compete on a more global basis.

Territory Iron has seen fit to revive the old Frances Creek iron ore mine some 30 years after it ceased operations in AustraliaÆs Northern Territory. The key to this revival has been the changes that have happened since market forces, infrastructure limitations and a cyclone combined to force its closure. As it breathes life into this old mine, the Company is serious about its responsibilities to its shareholders, to its workers, to local and regional communities and to the environment. This paper addresses how Territory Iron has approached technical, operational and regulatory issues that apply in Australia. Some insight may be gained into the myriad of tasks that mining companies face to revitalise an old mine. These tasks vary from employing the latest exploration tools, best practice drilling, sampling and modelling of orebodies, optimisation of pit designs and schedules, maximising product value by blending, optimising infrastructure usage and minimising capital and operating costs. All of this has to take cognisance of commercial, environmental and other issues including Native Title.

As more than a few mines gradually sink under the weight of emails, whiteboards, notice boards and ever more complex spreadsheets, the holy grail of effective short-term interval planning and control in underground mines is as far away as ever. Despite the mining industry investing ever greater amounts in planning and Enterprise Resource Planning systems, short-term plans become ever more complex and rigid. What the industry desperately needs are not more ‘disembodied voices down the speaking tubes’, but flexible and effective planning systems that encourage people to make reasonable commitments to each other so that everyone can work as a coordinated team to complete the work required each shift.One solution to the challenge has emerged. This paper describes the ‘Fewzion’ shift planning and short interval control system which has been developed at Anglo American’s Moranbah North and Glencore’s Oaky Creek coalmines in Central Queensland. Fewzion has drawn on management theory embodied in Management Operating Systems, Lean and Theory of Constraints approaches to continuous improvement to play a key role in both mines achieving record breaking tonnages.In addition to explaining the Fewzion approach, a case study is presented describing how Anglo American’s Moranbah North improved production by 39 per cent in 2014/2015 and won ‘Coalmine of the Year’ two out of the last four years and how Glencore’s Oaky Creek sustainably improved monthly development meters by 34 per cent within two months of implementing Fewzion.CITATION:Moynagh, P and Britton, G, 2017. I have headed this paper with a question and for the impatient among you ûthe answer is no we have not run out of developments but, and it is a big æbutÆ, we need to be innovative and ahead of the game or ænoÆ will be æyesÆ before we know it.Why do I believe that our future lies in innovation? Thirty years ago, knowledge doubled every 14 years. It is now doubling every seven years. In 1965, Gordon Moore, one of the founders of the chip maker Intel, made an observation that growth in memory chip performance is doubled every 18 months which became known as MooreÆs Law. Now there is fibre law, where the transmission line capacity doubles every nine months and storage capacity, under disc law, doubles every 12 months. What does this mean? Well, firstly there is a massive rise in information flow and availability with no limits in growth for at least the next ten years. Secondly, unlike our grandfathers, we are most unlikely to be employed by the same firm for our working career - we may not even have the choice to stay with the same company. In a phenomenon Richard Foster and Sarah Kaplan (2001), borrowing from Schumpeter, termed æcreative destructionÆ, companies are unable to sustain market-beating levels of performance for more than ten to 15 years. In the 1920s the turnover rate of companies in the United States was 1.5 per cent, so a company could employ almost three generations from the one family. A company had a half life of 60 years. In other words, you could well be in the same working environment as your grandparents. Today the churn rate is estimated to be eight to ten per cent. A company has a half life of five to six years. So for large companies, as an employer or employee or as a contractor, your working environment has a 50 per cent chance of changing in five years. Innovation is the driver of this rapid rate of change. Innovation is not for the faint hearted (Cutler, 2001) but it is the core for success in the global economy. The rate of change in our world is not going to decrease. We must learn, not only to manage change, but to enjoy the process of changing û to ride the tsunami of the knowledge economy.

Site specific geotechnical data, deposit characteristics and method of mining are key factors when assessing an appropriate slope design philosophy. Mining geotechnics should be viewed as an iterative design process linked closely with mine planning, development and operational safety constraints. This case study presents geotechnical investigations for slope design at Macraes Flat Gold Mine using kinematic, probabilistic and economic analyses tempered with knowledge of site specific conditions. The recommended designs not only reduce early waste stripping tonnages but also provide data to optimise final walls.

The first steps for in situ resource utilisation (ISRU) on target bodies such as the Moon, Mars and near-Earth asteroids (NEA), and even comets, involve the same sequence of steps as in the terrestrial mining of resources. First exploration including prospecting must occur, and then the resource must be acquired through excavation methods if it is of value. Subsequently a load-haul-dump sequence of events occurs, followed by processing of the resource in an ISRU plant, to produce useful commodities.While these technologies and related supporting operations are mature in terrestrial applications, they will be different in space since the environment and indigenous materials are different than on Earth. In addition, the equipment must be highly automated, since for the majority of the production cycle time, there will be no humans present to assist or intervene. This space mining equipment must withstand a harsh environment which includes vacuum, radical temperature swing cycles, highly abrasive lofted dust, electrostatic effects, van der Waals forces effects, galactic cosmic radiation, solar particle events, high thermal gradients when spanning sunlight terminators, steep slopes into craters / lava tubes and cryogenic temperatures as low as 40 K in permanently shadowed regions. In addition the equipment must be tele-operated from Earth or a local base where the crew is sheltered. If the tele-operation occurs from Earth then significant communications latency effects mandate the use of autonomous control systems in the mining equipment. While this is an extremely challenging engineering design scenario, it is also an opportunity, since the technologies developed in this endeavour could be used in the next generations of terrestrial mining equipment, in order to mine deeper, safer, more economical and with a higher degree of flexibility. New space technologies could precipitate new mining solutions here on Earth.The NASA Kennedy Space Center (KSC) Swamp Works is an innovation environment and methodology, with associated laboratories that use lean development methods and creativity-enhancing processes to invent and develop new solutions for space exploration. This paper will discuss the Swamp Works approach to developing space mining and resource extraction systems and the vision of space development it serves. The ultimate goal of the Swamp Works is to expand human civilisation into the solar system via the use of local resources utilisation. The purpose of this paper is to provide an illustration of the Resource Management Act processes at their best. Many of you will be aware of the Martha Mine situated in the middle of Waihi Township in Hauraki District. Underground mining ceased on Martha Hill in the 1950s. A new mining licence was issued in the late 1980s and since that date open cast mining has continued at the site. The Hauraki District Council notified its proposed plan in October 1994. Prior to that date numerous discussion papers and draft sections of the plan had been disseminated in the community. Considerable consultation was undertaken with all interest groups. As an indication of the success of that consultation process only 17 references to the Environment Court were lodged following the making of decisions on submissions and further submissions. Those references, which included a number from the Minerals Industry, were all resolved by consent. Out of the reference and consent order process EMMA (Extended Martha Mineral Area) was born.

Mining companies are moving towards automated haul trucks and other equipment. One reason for this is that if automated trucks can operate in a standard pattern over and over again without variation, then human error is reduced, if not eliminated.The challenge for mining production leaders/managers is to ensure operators follow standard operating procedures for load and dump operations, with each operator doing the same thing, every time. However, operators are only human and inconsistent performance is rife at every mine site. The finger may be pointed at the operators for not doing as instructed. The clarity and variability of trainer instructions may be considered causal to the problem. Herein lies the problem. Almost all operators believe they are following the procedure very closely; moreover, they all demonstrate a strong desire to conform to instruction.Using video analysis and feedback, each operator can be shown where and by how much they differ from the benchmark set by their company. Operators, often for the first time, can see definitively what they are doing, compared to industry best practice.The technology utilised by real-time video (RTV) crews measures time, distance and angles. It can overlay the video of the operator at the load site, with video of the perfect pass. Operators are amazed when they see how near to, or how far they are from best practice.The footage and data gathered by RTV assists operators in understanding exactly what adjustments are required to increase proficiency. It also assists load and dump superintendents to identify where in the process they are losing time.
