There has been an unprecedented rise in the fatality rate in the Australian mining industry over the last few years. The factors behind this rise are not clear; however, it is noted that the increase in fatalities has occurred during a fall in commodity prices.This paper examines prevention methods and systems to deal with hazards that have the potential to result in either single or multiple fatalities. Managing the risks associated with mine ventilation is used to illustrate the principals behind the approach.The paper also touches on the broad science of human error and proposes a multifaceted approach to controlling and preventing fatal risks.A summary of the steps involved in developing an effective principal hazard management plan is provided.CITATION:Clough, A J, 2015. The Yanacocha district hosts the largest and most significant acid-sulphate, high-sulphidation deposits in Per· with total production, reserves and resources of over 27 million ounces of gold. Several other smaller high-sulphidation systems are also recognised in the Cajamarca region including La Zanja, Sipßn, Tantahuatay, and San Nicolas. These each have reserves/resources of between 640 000 and 800 000 ounces of gold. Gold deposits in the Yanacocha district are hosted in an extensive volcanic field. Flow domes have intruded precursor pyroclastic rocks and flows that are interpreted as part of an earlier intermediate multi-vent volcanic complex. The larger, contiguous Yanacocha Norte - Sur and Encajon gold deposits are spatially associated with late-stage diatreme breccias. At La Zanja extensive acid-sulphate alteration is associated with the margins of the small San Pedro caldera (6 +4.5 km), nested within the larger, less well-defined La Zanja caldera (19 +24 km). The inner caldera is resurgent with abundant magmatism expressed as several flow dome complexes located on the caldera rim, and shallow level intrusive stocks. Gold mineralisation is associated with a WNW-trending structural corridor that transects the southern margin of the San Pedro caldera. On the eastern side of the La Zanja caldera there are a series of younger andesitic eruptive centres, some associated with acid-sulphate alteration and gold mineralisation. The Sipßn deposit occurs in one such centre with structurally-controlled gold associated with a NE-trending silicified structure developed within a sequence of andesitic tuffs. In conclusion, although a number of high-sulphidation deposits are recognised in northern Per·, their settings and styles of mineralisation are all quite different. Smaller deposits generally have strong and obvious structural controls. In the larger Yanacocha deposits the structural controls are less obvious, and mineralisation is disseminated in porous or strongly fractured and brecciated silica. Erosional levels are probably a significant factor, with disseminated mineralisation at higher levels, and structurally-controlled mineralisation in more deeply eroded deposits.

The McPhee Creek bedded iron deposit is a recent discovery made by Giralia Resources NL (Giralia) in the northern Pilbara of Western Australia. The deposit lies 30 km north of the town of Nullagine along the southeastern margin of the East Pilbara Granite-Greenstone Terrane (EPGGT).The McPhee Creek area is located in the Kelly Greenstone Belt in the southeastern part of the EPGGT. The Archaean aged greenstone belt consists of volcanic and sedimentary sequences including the Warrawoona Group and the Gorge Creek Group. The McPhee Creek iron ore deposit lies within banded iron formation (BIF) rocks of the Gorge Creek Group. The structure of the McPhee Creek area is dominated by a northeast trending, upright synform (Sandy Creek Syncline) and associated folding that is truncated by northeast trending faults in the east. Iron ore mineralisation is predominantly stratabound and follows the dip of the bedding – however, in some areas, particularly in the southwest of the deposit, mapped steep to moderate dips of bedding planes are at odds with underlying fl at mineralised zones in drill sections. Zones within the deposit that contain substantial thickness of iron ore correspond to areas of complex folding suggesting structural control and iron enrichment in fold hinges. Petrographic analyses confirm mineralisation is goethite dominant and the deposit is strongly supergene enriched.Giralia initially identified iron ore potential in the McPhee Creek area through the presence of a small known channel iron deposit (CID) mesa. Giralia began exploration in 2008 by re-drilling a portion of the CID (subsequently named Crescent Moon) and reported a small low-grade Inferred Mineral Resource. Target generation in the wider area utilising Geological Society of Western Australia maps and Ikonos satellite images identifi ed strongly ferruginous outcrops associated with major structures along an 8 km long range located 2.5 km to the west north-west of the Crescent Moon CID deposit. In late July 2008, a total of 62 rock chip samples were collected during a helicopter supported sampling program conducted at McPhee Creek. Samples taken from haematite-goethite enriched BIF and canga zones outcropping intermittently over the 8 km length of the western and central areas of the prominent range assayed up to 63 per cent Fe. These zones represent the discovery outcrops and surface expression of the McPhee Creek main range iron ore deposit.Follow up sampling, geological mapping, bulldozing of access tracks and ramps and reverse circulation (RC) drilling commenced in September 2009. A maiden Inferred Mineral Resource was announced by Giralia to the Australasian Securities Exchange (ASX) on 15 December 2009.The current Indicated and Inferred Mineral Resource at December 2010 stands at 260 Mt grading 56.2 per cent Fe, 0.12 per cent P, 6.7 per cent SiO2, 2.3 per cent Al2O3 and 9.5 per cent LOI at a 50 per cent Fe cut-off. McPhee Creek is one of the largest iron deposits found in Archaean BIF in the northern Pilbara.

There is often a difference in opinion about the allocation of risk in the establishment of mine closure bonds or financial assurances. From the State’s perspective, protection is needed to ensure that the broader community is not left with the liability of properly closing a mine in the event of a default by the mine operator. Such a default might occur where an operator decides to abandon a site, perform incomplete rehabilitation, becomes insolvent or claims limited liability. On the other hand, mine operators who can demonstrate a strong financial position plus a record of social and environmental commitment and performance, argue that there should be some recognition of this in setting a bond that is less than the full cost of early closure. They argue that a mine rehabilitation bond based on the full cost of early closure is overly conservative and ignores the actual risk of the operator being in default. The gold salting fraud at Busang on the island of Kalimantan, Indonesia was exposed in early 1997 and resulted from the independent drilling, sampling and assaying of PT Freeport Indonesia which reported assays from the South East Zone with negligible concentrations of gold. Since then, a number of legal actions directed at former Bre-X directors, consulting firms and financial or broking institutions are at various stages and are based on numerous allegations surrounding the supposed existence of obvious and numerous warning signs or æred flagsÆ. These include: the absence of references to visible gold in the drill logs; the alluvial nature of the gold grains; the high gravity recoveries; the invalid comparison with Kelian; the absence of outside scrutiny and the lack of external review/audit and surface depletion of gold. The focus on the red flags has been at the expense of the numerous positive indications or ægreen flagsÆ. These include the morphology and chemistry of the gold grains, the gravity testwork, the amalgamation results, the role of external consultants, the results of a review by one of North AmericaÆs leading minerals auditing groups, the behaviour of the gold at surface and the presence of references to visible gold in the drill logs. The flawed nature of the discussion of the technical facts surrounding Busang has prevented a meaningful and informed discussion about what actually happened and has, therefore, prevented the formulation of meaningful remedies for the future.

HORWOOD wrote: In view of the difficulty of obtaining accurate results when using plumb lines in shafts, owing to the effect of currents of air, the writer, over M years ago, devised a method of transferring oearings from the surface to, the underground workings independently of plumb lines.An attachment was procured for the theodolite consisting of a 45° prism enabling a view of a fine line stretched across the shaft at the surface to be observed from the bottom of the shaft. The instrument was set up in such a position that.a view of the portion of the surface line could be obtained as close as possible to the shaft timbers.By candle light signals an assistant at the surface moved his line so that the portion close to the side or end of the shaft coincided with the cross wires of the theodolite. The theodolite was then swung to enable the crosswires to traverse the surface line. It was a simple matter to transfer the bearing of the surface line to points in the drive extending from the shaft. Similarly the relation of the bearing of the surface line to the shaft, with which a connection was intended, was easily obtained.The success of this method was demonstrated at Broken Hill in making connections many hundreds of feet from the shafts from which these were driven.

Geotechnical practice combines the site engineering geological model with engineering design and operations to control risk. The responsibility of the geotechnical professional is to present relevant engineering geological information to an engineer to effectively impact an outcome. Return is enhanced by understanding expected operating and capital cost due to ground conditions.   Risk is effectively communicated by a successful geotechnical practitioner. In a mining environment the need is to assess ground conditions accurately and quickly, and to effectively communicate expected ground response due to mining. A good engineering geological model includes a description of geotechnical engineering parameters to design assuming reasonable expected soil, rock and water conditions. Pit slopes are steepened, stope spans widened and waste foundations and land forms are constructed all under geotechnical control.   The responsibility of the geotechnical professional is to collect, assess and disseminate relative engineering geological information. The geotechnical professional is a person advising on ground related engineering, and generally operates within geology and engineering end members.   Return is better assessed on an understanding of ground cost. In operations the goal is to meet production at lower cost. In evaluation, regional and exploration geology, mine design and expected operational factors influence expected cost and project return. Return allows companies to meet shareholder expectations, attract equity to develop and add value to the community.   This paper presents a discussion with some examples to present an approach and design philosophy for geotechnical practice in the mining industry.

The locality from where these specimens come is distant about 12 miles due south from the township of Bingara New South Wales, and is known locally as Carney's Reef, Barrack's Creek, Top Bingara. The formation traverses the western side of a serpentine spur from the main range, which runs through the country from the other side of Tamworth to Inverell, in the New England District. It has a due north and south trend, with an easterly underlay slightly out of the vertical. The formation has been opened up at different points for a distance of over two miles, and each place showing gold in the stone, and no place more than three to four chlins apart, the whole distance along the line of Reef being leased. The formation varies in thickness, the smallest being about 1 foot and the widest about 10 feet. The following is a list of properties, and description of work done on them at present. First south, and about one and a half miles from Hanion Brothers' old BalIarat claim due north, Court and patty hold a tenacre lease. The formation outcrops near the south boundary, and gold is obtainable in the stone. In the centre of the lease a costeen pit has cut the capping, which shows gold, and about two chains from the north boundary another one cuts the capping, also showing gold. The reef averages, where exposed on this property, 3 feet wide.

The objective, of the Diesel Emissions Evaluation Program (DEEP)-sponsored project at IncoÆs Stobie mine, was to conduct a long-term field evaluation of selected diesel particulate filter (DPF) systems, available to the underground mining industry. Nine state-of-the-art DPF systems were retrofitted to heavy-duty and light-duty vehicles and were subjected to extensive long-term in-mine evaluation. Periodic efficiency tests were conducted at various stages of the study (in 2001, 2002 and 2004) to establish in-use efficiencies and durabilities of the tested DPF systems. During efficiency tests, the vehicles/engines were operated over several steady-state operating conditions. Various instruments were used to measure particulate and gaseous emissions upstream and downstream of the filter systems. The results were used to assess the effects of the filter systems on the concentrations of diesel particulate matter (DPM), nitric oxide, nitrogen dioxide and carbon monoxide in the vehicle exhaust. Experience with the operational issues related to deployment of the filter systems on underground mining vehicles, coupled with the assessment of the filtration efficiencies, were the primary objectives of the study. The variety of filtration systems and regeneration concepts used in this study offered the opportunity to investigate the advantages and disadvantages of the various types. Some of the major issues studied were criteria for selecting the filter media, means of regeneration, the long-term operational reliability, and the occurrence of unwanted secondary emissions.

Water is a key resource input to copper heap leach operations in both quality and quantity. A number of recent developments in analysis of the technology have shown that water consumption can be optimised. The leach consumption has been generally considered in terms of minimisation of the evaporative loss by changing the application method (drippers or wobblers). Modelling of the irrigation effects on leach rate has shown that the poorer surface coverage from drippers has an adverse effect on the leach extraction rate. This is particularly evident in the early part of the leach. The use of different irrigation systems during alternate parts of the leach cycle can have benefits in minimising the water consumption without an unacceptable outcome from extended extraction times. By combining the leach rate effect with the evaporative loss estimates, an economic decision matrix can be generated to allow the operator to select an appropriate leach irrigation strategy, combining both irrigation delivery and irrigation rate. In the SX-EW plant, the total quantity of EW bleed and SX scrub stage bleed (both for iron control) can be reduced to an absolute minimum. The economic minimum may be different depending on the cost of producing the high-quality water used to replace the bleed streams. Adjustment of the SX process, in particular use of multiple entrainment control steps, can provide significant reductions in the total bleed and make up requirements.

By the year 2000, China was the sixth largest gold producer in the world, with about one-quarter of total production being derived from refractory or semi-refractory deposits. Although bacterial oxidation plants had been previously tried in China using local technology, these plants had not been commercially successful. The Laizhou Gold Enterprise (Tarzan BioGold Co Ltd) located in Shandong Province, undertook to modernise their gold processing facility in 2000 with the introduction of Bacterial Oxidation (BACOX) Technology. In 1997 BacTech Australia and Mintek of South Africa had formed a technology partnership, combining forces in the provision of bioleach technology. Under a cooperation agreement, BacTech/Mintek were chosen as the preferred technology providers for this first application of commercially proven bacterial oxidation technology within China. The paper describes the successful operation of this plant over the last three years. The events of project implementation are described, from the metallurgical testing of concentrate; through to the commissioning at the design throughput of 100 tonnes per day resulting in 80 000 ounces (2.5 tonnes) per year of refined gold. Process aspects of the operation encountered over the last three years are discussed which illustrate the robustness of the operation. The success of the bioleach facility has led to an advancement of plans to double plant capacity and some of the innovations, which may be employed in this expansion phase, are discussed.

At moderate depths this question is of relatively small importance, but at depths over 2,000 feet it becomes a matter of some consequence. The rate of increase of temperature with the depth varies in different localities, but as a rough average we may take it to be about 1° Fahr. for every 50 feet of depth. The variation of temperature of the earth's surface from summer to winter penetrates to a comparatively shallow depth. At 50 feet the variation becomes practically insensible, and the temperature of the strata will be about the mean temperature at the given locality, from 60° to 75° F. in this country or the comfort of men working in the mines it is desirable to maintain a temperature of about 70° F. in the workings, or approximately the temperature of the strata near the surface.In order to obtain some idea of the magnitude of the undertaking involved in cooling a mine, I have examined the effect of placing in a long drive an apparatus by means of which we can abstract heat at a uniform rate from the drive, keeping, thetemperature nearly the same at all points of the drive at any given time.The lowering of temperature 70 F. at any distance feet from the centre of the drive (treated as a cylinder of the same sectional area as the actual drive), after an interval of time days from the commencement of the operation is given by the formula...

The Luzon arc extends for about 900 km from Taiwan to the island of Mindoro (central Philippines); in this area late Tertiary to Recent volcanism is considered to be the result of eastward subduction along the Manila Trench. The arc is composed of five segments, all of which have individual characteristics with respect to tectonic setting and magma chemistry (Defant et al., 1989). From the north to the south these are (Fig. 1): (1) the eastern Coast Ranges of Taiwan and the islands Lan-Hsu and Lu-Tao (Taiwan Segment), (2) the islands off the north coast of Luzon (Babuyan Segment), (3) the Northern Luzon Segment, (4) the Bataan Segment, and (5) the Mindoro Segment. Defant et al. (1988) suggest that the Bataan and Mindoro segments of the arc, both of which are characterized by different isotopic compositions and levels of large ion lithophile element (LILE) enrichment in the lavas, are separated by a 'pull-apart' zone, the Macolod Corridor.We are currently studying the mineralogy and petrochemistry of Late-Miocene to Pliocene volcanism in the Northern Luzon Segment (NLS), which comprises the geographic area of the Cordillera Central. This area has so far received limited attention, inspite of its wealth of porphyry copper, skarn and epithermal gold deposits. In this contribution we report the first results of our study of igneous rocks from the Mankayan Mineral District (MMD) which is located in the centre of the arc segment under consideration.

Fluid circulation occurs in oceanic crust of all ages, at the seafloor spreading axis, on mid-ocean ridge flanks, in the ocean basins, and in subduction zones. In the first three settings the fluid originates as seawater. At the axis and on the ridge flanks, the circulating seawater produces a major chemical exchange between the oceans and the crust. Based on heat budget constraints, between 10 and 40% of the river flux of Mg can be taken up during high-temperature alteration of the basaltic crust along the mid-ocean ridge axis. Most of the hydrothermal -heat loss, however, occurs on the mid-ocean ridge flanks, where the temperatures are lower and the seawater flux correspondingly larger. The estimated heat loss on the flanks is so large that upwelling must occur over a large fraction (5-30%) of the seafloor less than 65 Ma in age, at seepage velocities on the order of 10 to 100 cm/yr. The circulating seawater need lose less than 1 to 2% of its Mg content in order to solve the Mg mass balance for the oceans. Fluids in subduction zones are commonly less saline than seawater and are rich in methane and other light hydrocarbons. These fluids may originate by dewatering of the downgoing slab. If so, they may close the geochemical cycle of H2O that began with hydration of the basaltic crust at the ridge axis.

Ore in pillars constitutes a significant proportion of the ore reserve at PasmincoÆs Broken Hill Mine. Many of these pillars are narrow (5 -6 m) and high (16 û 35 m) vertical blocks. High-grade ore has been left between primary horizontal cut and fill stopes. These stopes were mined from the late-1960s until the early-1980s. Fill material is a variety of hydraulically stowed sand, sand-cement, and remnant timber support. Many pillar cut-throughs were made between the old stopes. In order to gain access for the current pillar recovery operations, it has been necessary to develop pillar drill drives through, and adjacent to, the hydraulic filled stopes. Whilst long hole recovery of vertical pillars is not new, some notable successes using innovative ideas have been achieved during 1997. These innovations include:The use of steel mesh reinforcing in shotcrete when mining through, or adjacent to, hydraulic fill. Construction of steel sets to support vent rises and haulage ways encountered in the cut-throughs. The use of ground movement monitoring equipment. Survey lasers with chain droppers and convergence monitors were installed in the 18 Pillar to detect any movement of the pillar block and decrease risk to personnel. The development of a three-dimensional computer model of primary stoping, using Vulcan software, to assist in the design of pillars. Maximisation of recovery using different drilling and firing practices.

Arsenic-containing waste was dissolved into molten slags and the leaching behaviour of the quenched arsenic-doped slags (containing up to 2.3 wt per cent arsenic) was ascertained to evaluate the potential of this method as a means for arsenic disposal. The leaching behaviour was evaluated using acetic and sulphuric/nitric acid based solutions.From short-term (18 hour) leach tests, the amounts of arsenic in the leach solutions were found generally to decrease with an increase in the Fe(III)/Fe(total) ratio of the slag or an increase in pH of the leach solution. The amount of arsenic in solution increased with an increase in the slag basicity, the slag arsenic concentration or the surface area of the slag exposed to the leach solution. From longer term leach tests (up to 40 weeks) in the pH range 5 to 8, the arsenic remaining in the leach solution was believed to be controlled by sorption processes. The sorbent was an iron oxide-rich material which had the capacity to lower the arsenic concentration in the leach bottles to well below 1 ppm in some cases. At a leach solution pH over 9, the arsenic concentration in the solution increased with time, although the rate of solubilisation was much slower due to the high pH.Slags have the potential to stabilise arsenic and prevent it entering the environment provided attention is paid to the slag chemical composition and oxygen potential.

Fluid inclusions and sulfur isotopic compositions were studied in a possible extension of the Toyoha polymetallic vein deposit. The objective of this work was to gain a better understanding of the ore depositional conditions, sources of ore constituents, and distribution of mineralizing fluid in the region. Fluid inclusions in sphalerite and quartz show that most homogenization temperatures range between 190¦ and 290¦C. Most salinities range from 0.0 to 3.4 wt per cent NaCl equiv. Sulfur isotopic compositions of ore minerals (pyrite, sphalerite and galena), determined by a conventional technique, range between 1.3 and 7.7 per mil. The mineralizing fluids were characterized by relatively low and various temperatures and low to moderate salinities compared to those at the southeast region of the main ore bodies (Izumo and Shinano veins) where rare elements such as tin, tungsten and indium occurred. Obtained temperatures and salinities were attributed to the mixing of hydrothermal fluids with groundwater. Sulfur isotopic data suggest sulfide sulfur was of magmatic origin. Isotopic temperatures, estimated from the isotopic fractionation factor between sphalerite and coexisting galena, are between 195¦ and 322¦C. The temperatures are only consistent with fluid inclusion homogenization temperatures in sphalerite or coexisting quartz when sphalerite was a honey-yellow coarse crystal lacking conspicuous colored growth bands. This suggests the detailed classification of mineral pairs growth textures will be necessary for precise in situ measurements to obtain reasonable eothermometric results.

There are many critical nodes along the mine value chain, from orebody to mineral product, and inter-disciplinary input is required to analyse the variability and uncertainty at each node in order to identify and mitigate areas of risk. Mining differs from many other industries in that the variability in the product is dictated largely by the inherent nature of the input material (the orebody). The true characteristics, including the variability of the orebody, are never known exactly and are instead based on estimates derived from sample data. A further difference is that bottom-line gains are largely controlled by world commodity prices, which can vary with time, rather than by local supply and demand issues. Methods for identifying, reporting and responding to this inherently variable raw material are discussed. These include analytical and reporting methods, equipment and software, risk analysis and reality checks (eg reconciliation). Sampling results, mineral resource and ore reserve estimates and economic assumptions are a function of the variability of the orebody. Risk quantification should be expressed as a level of confidence, which takes into account the scale or period over which the risk is being assessed (life-of-mine, annual or shorter production periods) and should convey the likelihood, severity and consequence of occurrence of a given event. The future generation of mining specialists needs to understand the entire mine value chain to better manage risks and maximise mine dollar-value.

A summary of the more important work by previous workers is followed by brief descriptions of the rock formations and regional structure. Type examples of the various forms of mineralization are described and the conclusion is drawn that there has been one main period of ore formation, namely, during the "Viti Revolution" in the mid or late Miocene.INTRODUCTIONThe Crown Colony of Fiji is comprised of a group of some 250 islands situated between latitude 160 sand 21 0 S and longitudes 1770 E and 1780 w: By far the greater, part of the total area, 7,083 sq. miles, is contained within the two main islands, Viti Levu 4,053 sq. miles, and Vanua Levu 2,128 sq. miles, and it is to these that most geologists have directed their studies. The most detailed work has been in the Tavua Basin, north coast of Viti Levu, centering about the mining town of Vatukoula and the geology of this area forms the subject of a separate paper by Blatchford (this volume p. 13).The present contribution summarizes geological knowledge of Fiji, at the time of writing (Feb. 1952), with particular reference to the known mineralIzation, but excluding the Tavua Basin. The results presented here show quite definitely that in the system investigated the intensity of dispersion is enhanced by switching the flow entry from bottom to sides of the packed trough. Since there is no reason to suppose that this would not be true generally then it is not surprising that the blast furnace dispersion data of Standish and Polthier lie above the data for packed columns with bottom fluid entry. In fact, on the basis of the present results, blast furnace dispersion results can never equal standard packed column behaviour but must always be above these. The result that the intensity of dispersion in the trough model is higher closer to the side ports than away from them is at variance with that of Standish and Polthier blast furnace results. Aside from the obvious structural differences between these two systems, it is thought that the presence of raceways and the tortuous path of the gas flow in the furnace may provide an answer. Clearly this is an important difference in the behaviour of the two systems and further investigations designed to clarify the position would have significant practical value.
