Disasters are often tragic outcomes of high-risk technologies such as mines. In Australia, more mines are being developed every day, and the risk of disasters is ever increasing. No matter how effective our conventional safety devices are, there is a form of accident that is inevitable. These relate to accidents that result from æinteracting failuresÆ in a way that could not be foreseen by the designers. In so-called ætightly coupled production systemsÆ (processes that happen very fast, such as on a high producing mine) the risk is even higher and our risk controls mostly introduce some sort of a technological fix. While we are at the one hand attempting to control the risk, we are also introducing another level of complexity. Are we really controlling the risk? This is one of the fundamental questions that will be addressed in this paper: Do we have the ability within the mining industry and other industries to effectively prevent these catastrophes, or are we, on the contrary, faced with an increasing risk as a result of increasing complexities of our technology, management systems and practices. This paper will contend that perceived improvement in risk control is an illusion of activity, and that the likelihood of mining catastrophes may be exponentially increasing. Although the paper focuses on the mining sector many aspects may be applicable across a range of industries. This paper will further contend that we are applying the right solutions to the wrong problems. Our focus is technological and procedural, while our problem is one of production cultures ripe for error and failure.

Based on the stiffness theory, the stiffness difference and energy supply of surrounding rocks are important inducing factors for strain burst. However, due to the lack of multi-level low stiffness testing machine, there are few experimental studies on rock failure mechanisms under the influence of loading stiffness. This paper introduced a self-developed rock testing system with changeable stiffness. The main testing machine of this system is a combined structure of inner and outer frame. The change in the loading stiffness is achieved by using a stiffness servo control system to control the energy accumulation in the inner frame. The tests of the sandstone specimens under three different loading stiffnesses in the testing system showed that the loading stiffness did not significantly affect either the uniaxial compressive strength or the Young’s modulus of the rock. However, the post-peak stress-strain curve of the rock became smoother and steeper when the loading stiffness was decreased, and the stress drop rate increased. It is shown that the stress drop rate had a power function correlation with the loading stiffness. After the peak load, the inner frame of the testing system rebounded several times at high speeds. The magnitude of the instantaneous rebound speed and the rebound duration increased with the decrease of loading stiffness. Both the mean rebound velocity and the total rebound deformation had power function correlations with the loading stiffness. The loading stiffness has limited effect on the energy storage and energy dissipation characteristics of the specimen, while the released energy of the testing machine increases as the loading stiffness decreases, which is the key factor affecting the failure mode of the specimen. According to the test results, a rock burst mechanical model considering the stiffness is established, and the energy supply mechanism of rock burst is quantitatively explained.

The extensive literature on sulfide flotation indicates that pyrite poorly floats under alkaline condition. Xanthate concentration has a positive effect on pyrite flotation, but above a pH of 11, pyrite seems to be strongly depressed, independently of the concentration in xanthate. In this investigation, aeration and xanthate concentration effects were studied for pyrite flotation conditioned at pH 11 with lime. Results presented in this paper include surface infrared analyses and pulp chemistry measurements which have showed that aeration had a dual effect on pyrite flotation. It induced pyrite surface activation by oxygen, and oxidation of ferrous iron to ferric hydroxides or oxyhydroxides species. It also favoured dissolution of CO2, leading to calcium carbonate precipitation and adsorption onto pyrite surface. Both CO2 dissolution and ferrous oxidation generated protons and decreased pulp pH. Pyrite flotation was observed to be effective with a statistical monolayer of xanthate that could be adsorbed as mixture of dixanthogen and metal-xanthate complex. Non-floated fractions showed high amounts of carbonate, and presence of hydroxyl iron sulfate (Fen(OH)m(SO4)p, xH2O) which have inhibited the hydrophobicity effect of adsorbed xanthate. Environmental desulfurisation could be improved by aeration control causing pyrite oxidation and pH decrease to reach optimal flotation condition with a relatively low xanthate concentration.

Cooling tower design has not advanced significantly in the last decade. Recognising this, Outotec has developed an innovative horizontal outflow cooling tower with increased performance compared to conventional design cooling towers.Outotec® Cooling Towers operate today in applications such as spent electrolyte cooling in zinc plants and for gypsum removal before copper solvent extraction. In such applications the use of this latest design has enabled increased production and lower maintenance costs in the downstream operations.In actual operations, the cooling performance has been confirmed at start-up, and has demonstrated that the towers operate in accordance with the CFD modelling. In the last three years these towers have proven themselves in commercial operation to operate with high efficiency and availability.This case study will outline comparative data, collected over two years at a site where both Outotec and conventional cooling towers are operating. The data shows that Outotec cooling towers require less than half the maintenance time of conventional cooling towers, when adjusted to a total solution flow basis. A close partnership approach with the site has allowed continual development, ensuring high availability and high performance.Experience with previous cooling tower installations guided the development of maintenance philosophies and procedures; this resulted in a design where maintenance intensive components were accessible from the top of the cooling tower. THE exceptional rainfall characteristic of the West Coast of Tasmania, in conjunction with the rugged topography of the district, has always suggested the utilization of water-power, and from the earliest days of the Mount Lyell Company theeventual installation of a hydro-electric power scheme, making use of some near-lying lake, has constantly been kept in view. At the period of the inception of the company, however, the time was not ripe for such an undertaking, and it is only oflate that the various important considerations involved have sanctioned incun;ing the heavy expenditure necessary, with the positive assurance of permanently reaping the benefits expected. The West Coast of Tasmania contains many lakes of varioussites; situated at high levels, and, although the catchments of these lakes are not very extensive, still, the excessive rainfall is so constant over long periods that its regularity may be relied on as offsetting the limited catchments. The explanation of the abnormal rainfall is simply the circumstance that the West Coast Range forms the first high land barrier which is met by the moisture-laden westerly and north-westerly winds blowing off the Southern Ocean. By the time the clouds strike the mountains they ride very low, consequently the elevation of the range, the peaks of which do not exceed 3000 to 4000 ft. above the sea, is sufficient to cause a copious precipitation. Lake Margaret, which is situated five miles north of the reduction works, was first recognized as a possible source of waterpower by Mr. Hugh Ohrisp, a surveyor, who visited it about 1893-4. Magnesium-rich olivine is used as an additive in LKABÆs iron ore pellets in order to improve the high-temperature properties of the finished products. Olivine is only added to the pellets that are manufactured for ironmaking in the blast furnace process. The olivine comes from Minelco/LKABÆs mine at Seqi in Greenland. This dunite complex of Archean age contains about 95 - 98 per cent olivine. The grain size is typically 1 - 3 mm but much larger euhedral olivine crystals are common in the porphyritic type. The serpentinisation is absent or weak. The most common minor minerals are chromite, chlorite, talc, phlogopite, amphibole and pyroxene. The crushed olivine is transported to LKABÆs pelletising plants in Northern Sweden. After grinding, small amounts of olivine are mixed with the magnetite concentrate. The concentrate is rolled into green balls, about 10 - 14 mm in diameter. The moist green balls are transferred into the pelletising machine where the maximum temperature is about 1250¦C. The residence time in the pelletising machine is about one hour. Olivine is used in order to get pellets with better high-temperature properties. This paper deals with the olivine alteration during the pelletising process and the behaviour of magnesium following the decomposition of magnetite. The reactions are quite complex in the pellets because the physico-chemical conditions vary considerably in the process from one process step to another, and in the pellet bed from the top to the bottom. Some olivine particles in the finished product show very slight alteration, some particles are completely decomposed. The formation of the iron-rich coronas around the olivine particles is the most typical alteration. In the finished pellets the magnesium is mainly distributed between the following phases: unaltered olivine, coronas, Mg/Fe-ferrite and silicate slag.

Tin-polymetallic vein type deposits are one of the major source of metal producers in the Circum Pacific Belt. This type of deposit is of great importance for the modem electronics and medical industries because of their high content of rare metals (especially indium and bismuth). We have studied two representatives, Tosham of India (Cu-In-Bi-Sn-W-Ag-Au), and Mount Pleasant of Canada (Zn-Cu-In-Bi-Sn-W), using proton and electron microprobes in order to delineate the distribution of medical/high-tech rare metals. The electron microprobe study showed that major indium carrier in such type of deposit are roquesite (CulnSZ), sphalerite, stannite and a Cu-Zn-Fe-In-Sn-S mineral; and that indium is incorporated in sulfides by coupled substitution like Cu + In = 2(Zn + Fe). The proton microprobe showed that indium at trace levels is also present in chalcopyrite and pyrite. In addition, zinc, copper, arsenic, antimony, selenium, tin, and bismuth are common in chalcopyrite and pyrite. Arsenopyrite contains nickel, copper, zinc, silver, tin, antimony and bismuth. In chalcopyrite and pyrite, zinc, arsenic, indium, bismuth and lead are higher in Mount Pleasant ore, but silver is higher at Tosham. Also thallium and gold were found only in Tosham pyrite. The Tosham deposit is genetically related to S-type magma, while the Mount Pleasant deposit is related to A-type granite. Tosham ore contains zinc-bearing biotites that coexist with chalcopyrite in ore bodies but Mount Pleasant ore lacks such biotite. We propose that the difference in elemental abundance between the two deposits are caused by the different character of source magma and different mineral associations of the ore body.

To enable this, large tonnages are put through the blast furnace, and, at the same time, the quality of the iron must be under definite control.The modern blast furnace enables these objectives to be attained. The description whieh follows sets out the general details of construction of such furnaces, and the operation at thc Broken Hill Proprietary Company's blast furnaces at Newcastle, New South Wales.The occurence of iron in many forms is so well known as to need no description. Modern civilisation may be said to be built on a foundation of iron, and, today, the production of iron and steel has beeome one of the world's industries.A short historical introduction will serve to show the evolution of the processes and apparatus since iron production assumed importance.There is no definite date fixed as to when iron was first made, or by what means it was first discovered. Perhaps prehistoric man threw iron bearing stone on to his fire, and these became reduced, Ieaving a soft metal behind, or more probably the first iron was of meteoric origin.There is evidence, however, to prove that it has been known for a period of 6000 years. A Joint Venture between Union Oil Development Corporation and Geoscan Proprietary Limited carried out an Airborne Multispectral Scanner Survey over approximately 15,000 square kilometres in the Charters towers area of northeastern Queensland during late 1985 and in September, 1986. The survey, using the Geoscan Mark I scanner, consisted of 50 runs each having a swath width of 5 kilometres. Twelve or thirteen spectral bands were recorded, 5 in the VNIR, 4 in the SWIR and 3 or 4 in the TIR. The results of the survey were interpreted by interactive image processing with the aim of identifying those minerals or their weathering products which are characteristically associated with hydrothermally emplaced gold mineralisation in this part of Queensland. Spectral bands in the VNIR were used to identify and discriminate the iron hydroxides, whilst the SWIR bands were used to outline the clay and carbonate rich areas. The TIR aided in identifying areas of silicification.

Artificial ground support has developed into a refined science over the past 20 years. From reactive support systems such as timber props and steel supports, to active systems such as roof bolts and cable anchors, ground support technology has now developed efficient systems that work well in many instances. However this paper demonstrates that there is still a long way to go to develop optimum ground support systems. Some conventional rock bolts and cable anchors emphasise their tensile capacity only, without considering other factors such as their shear strength or shear modulus, or if the support member can transfer all of its support capacity to the surrounding rock. Examples are given of the use of ground anchors with high tensile and shear capacity in underground coal mining as well as examples of slope failures in open pits where the cable bolts have remained intact because they have been unable to transfer their full capacity to the surrounding weak rock mass. For underground coal mines, speed of installation is also of primary concern, but it must also have high tensile and shear capacity both in terms of strength and stiffness. Ground support systems of the future must therefore address these problems. An innovative new rock bolt is discussed which goes some way towards solving these problems. This new rock bolt is screwed into the rock and thus has a direct mechanical interlock with the rock. Initial field results show that it has an extremely high anchor capacity and can be fully installed in approximately 30 seconds. It has considerable capacity to improve the productivity of the coal mining industry.

Mispah form (FAO: Lithosol) soil contaminated with >380 000 mg kg-1 creosote was co-composted with sewage sludge and poultry manure for a period of 19 months. The soil was mixed in a ratio of 1:1 (v/v) with wood chips to improve aeration. The soil-wood chips mixture was then mixed in a ratio of four soil-wood chips mixture: one compost material (sewage sludge or poultry manure). Compost heaps were set up in triplicate on wood palettes overlaid with double layers of nylon straw bags. Control experiments, which contained the contaminated soil-wood chips mixture without sewage sludge or poultry manure were set up in triplicate. Moisture, temperature, pH, ash content, C:N ratio of the compost and the creosote concentration of the soil was monitored monthly. The concentration of selected creosote components in the soil was also determined at the end of the incubation period. Temperature was observed to rise to about 60¦C in the poultry manure and about 58¦C in the sewage sludge within two months of incubation, while temperature in the control fluctuated between 15¦C and 35¦C throughout the incubation period. Creosote concentration in the control was reduced by 17 per cent at the end of the incubation period while concentration of creosote in the sewage sludge and poultry manure compost was reduced by about 99 per cent. The percentage rate of creosote reduction in the poultry manure compost was initially lower than observed in the sewage sludge compost. However, the reduction rate became similar towards the end of the treatment period. The concentrations of selected creosote components were reduced by about 100 per cent within the same period. Respiration of compost microorganisms and counts of microbial populations showed that microbial activity correlated with reduction in creosote concentration.

Vatukoula, the Fijian equivalent of gold rock, is the name of the community embracing the gold mines owned and operated by Emperor Gold Mining Company Ltd., Loloma (Fiji) Gold Mines, N.L., and' Dolphin Mines Ltd. It is situated in the Tavua Goldfields approximately five miles from the northern coast of Viti Levu, the largest island of the Colony of Fiji.The climate is sub-tropical, there being a definite wet and dry season. The wet season is somewhat variable in length but usually extends from December to March, during which time most of the rainfall for the year is recorded.Although the average annual rainfall is 71 in., more than twice this amount has been recorded. Precipitation at the rate of 5 in. per hour is not uncommon and 15 in. of rain has been registered in 24 hours. The mining methods adopted, therefore, had to suit these wet conditions.The occurrence of gold in the Tavua area has been known since 1872, but it was not until 1932 that discoveries were made which led to the formation of the present operating companies. In 1934 the Emperor Gold Mining Company Ltd. commenced operations, followed in 1935 by the Loloma (Fiji) Gold Mines, N.L., and later by the Fiji Mines Development Ltd., now known as the Dolphin Mines Ltd. A significant resource of uranium has been discovered within a buried palaeochannel located in the south western corner of the Officer Basin of Western Australia by PNC Exploration (Australia) Pty. Ltd. The mineral- isation is redox controlled and is hosted by Eocene organic rich paludal sediments. Prob- able insitu reserves are 13,000 t of contained uranium with an average ore grade of 0.12% at a cutoff grade of 0.03% U. The Mulga Rock palaeochannel has been traced for at least 100 km beneath the Quaternary surficial cover. The main channel is horseshoe shaped with the channel arms vary- ing from 5 to 15 km in width. The western arm connects to the Lake Raeside-Ponton Creek drainage system, whilst the eastern arm'is open to the south. The headwaters of the palaeo- channel are thought to have been the Lake Minigwal drainage system, prior to its capture by the Lake Rason system. Differential.erosion of the basement rocks during the Late Cretac- eous to Early Tertiary strongly influenced channel shape and resulted in the main channel being confined to a Permian,mudstone filled valley. Upwards of 100 m of fluviatile sediments accumulated within the palaeovalley during a period of subsidence in the Early Tertiary. Three broad stratigraphic units have been re- cognised within the channel: 1. An upper unit of fluviatile sands with interbedded lacustrine sediments, which have subsequently been wholly oxidised and variably silicified and ferruginised. 2. A middle unit of lacustrine to paludal sediments, which host the main mineral- isation. 3. A basal unit of fluviatile gravels and sand. The presence of excellently preserved Nothofagidites in organic rich paludal sediments from the middle unit, suggest that unit was laid down in a non-marine, fluviatile, humid environ- ment during the Middle Eocene.

Supergene lateritic iron ores are uncommon when compared with the vast tonnages of BIF-hosted microplaty haematite ores of the world or with the dominant supergene mimetic martite-goethite ores of the Hamersleys. In Australia and to a lesser extent in India and South America, mimetic replacement of gangue minerals by iron hydroxyoxides is a major factor in forming supergene iron ore from banded iron-formation (BIF) and results in the superb preservation of all of the original BIF features. This includes primary banding down to the finest inclusions, as a result of pseudomorphing of the BIF gangue at depth by goethite. The enrichment as a whole is an absolute accumulation of Fe at depth, dissolved from the surface BIF, and a removal of the gangue elements without loss of the original BIF mineral textures. Thus the ores grow upward to meet the downward destruction of the surface BIF by erosion. By contrast the supergene lateritic ores including the 'blue dust ores' are residual ores resulting from the relative accumulation of Fe. In the supergene lateritic ores derived from BIF, removal of the gangue is accompanied by repetitive solution and reprecipitation by secondary haematite and goethite. The final product of lateritisation of BIF and BIF-derived ores is typically complex, due to successive overprinting of earlier stages of alteration during the process, and because of the different precursors. This progressive lateritic modification tends to produce a varied surface complex including massive, vermiform, and botryoidal, or nodular aluminous/siliceous goethite and haematite, variously known as ferricrete, duricrust, canga, or 'hydrated zone hardcap'. The significant difference in these ores to the supergene mimetic martite-goethite ores is that they grow downward, with the total destruction of any of the parent BIF or BIF-ore textures.

Basicity(aO2−) has been known to affect the physicochemical properties of molten slags in the metallurgical processes. However, because of the impossibility of direct measurement of single-ion activity, various indirect measures such as capacity, redox equilibrium of transition metal ions, activity of basic oxides, and (theoretical) optical basicity have been proposed; but these basicity measures have been restricted within slag systems and the interaction between measuring elements and slag components has affected the quantitative estimation of slag basicity. Recently, since it was reported that solubility of noble metals in molten slags could be an indirect measure of basicity, the influence of slag basicity on the solubility of platinum and silver has been reported. The reaction mechanism of platinum dissolution into basic slags has been reported, whilst that in acidic slags has not (Nakamura and Sano, 1997; Sano and Nakamura, 1997; Nakamura et al, 1998). Richardson reported that the solubility of metals such as cupper, gold, silver, and lead in the CaO-SiO2-Al2O3 slag was presumably dependent on the atomic radius and the accommodation ability of the silicate network (Richardson, 1974). Swinbourne and You measured silver solubility in the Na2O-SiO2 slag over a wide range of oxygen partial pressure and suggested that silver would be present as both Ago and Ag0.5 in the slags (Swinbourne and You, 1999). For brevity, these previous works discussed the dissolution mechanism of silver into the slags based on the existence of silver as a neutral or molecular species. However, because the almost chemical reactions occurring in the slag are affected by the basicity, the reaction formula with O2- ion is required based on the ionic characteristics of the slag such as desulphurisation and dephosphorisation reactions as given in Equations 1 and 2, respectively.

Gas explosion in underground coal workings can pose a serious threat to the safety of mine workers and the entire mine operations. Gas explosion is a very complex thermal-chain reaction in a confined underground environment. Based on combustion and explosion theory, three dimensional (3D) computational models were developed to simulate mine gas explosion flame and overpressure wave propagation in a single entry gateroad in underground coalmines. Six different explosion scenarios were modelled and compared with large scale experimental results and AutoReaGas simulations. Model validations show that the use of shear-stress transport (SST) k-? model together with eddy-dissipation model (EDM) produces good agreement with both experimental and AutoReagas simulation results. Modelling results demonstrated that the changes of temperature versus time can be divided into four stages, ie the initial condition stage, and periods of warm-up, active burning, and burnt-out. Assuming a limited volume of methane is involved in the explosion, the maximum temperature decreases as the distance increases from the explosion point, with the highest temperature reaching about 2460 K. Modelling results show that the evolution of the explosion flame shape experiences four typical stages, ie spherical flame, finger-shape flame, flame with the skirt touching the side walls (ribs) and tulip flame, which are similar to the pattern of a duct explosion. The pressure in the gateroad remains the same as the initial value before the arrival of the explosion wave and then increases rapidly to the maximum value. Ore handling improvements at Mount Isa Mines Limited were necessary to keep abreast of technological advances in all phases of sublevel stoping practice.In the early 1930s an efficient ore handling system was in operation.However in 1936 excessive water encountered in deepening the hoisting shaft necessitated the sinking of a second hoisting shaft. Financial difficulties prevented abandoning the original shaft and surface ore handling and crushing facilities, so ore was transferred underground and underground crushing deferred. It was not until 1951 Jhat multilevel crushing permitted increased efficiency in the handling of ore from stopes. Ore was then hoisted in one shaft and transferred on the surface by conveyor belt to a new crushing plant.In 1956 a planned production increase from 5,000 to 11,000 tons per mining day made possible further improvements in ore handling. Larger equipment was installed and existing equipment modified to capacitate the handling of larger sized ore at increased daily tonnages.INTRODUCTIONThe intensive exploratory programme commenced in 1954 disclosed large reserves of both lead and copper ores and in 1956 an expansion programme was initiated to increase production from 5,000 to 11,000 tons per mining day. The increased tonnage would result in decreased costs per ton and further cost reductions would result from increased mechanisation underground. The combined effect would permit larger tonnages of lower grade ores to be included in ore reserves. Every mine is situated within a water catchment – a catchment with potentially many other land uses. A mine may take up a relatively small area in that catchment, but its operations have the potential to affect the whole catchment and long after that mine has ceased to operate. Therefore, the residents of any given catchment have the right to feel secure in the knowledge that their catchment, and indeed the wider environment, will not be degraded by mining activities. Much conflict can arise when this confidence does not exist.But, imagine if a community not only felt confident that their catchment would not be degraded by mining activities, but was also confident that their catchment would be left in a better and continuously improving condition – with improving water quality, biodiversity and soil - and a strengthening, more resilient economy. INCREASE OF CATASTROPHE FREQUENCY AND SEVERITYIn the last couple of decades the frequency of natural catastrophes has increased dramatically. This realization results from the statistical analysis of the comprehensive loss data that we have been collecting constantly over the last 30 years and have evaluated in connec-tion with the preparation of our "World Map of Natural Hazards", which is now in its second edition (Munich Re, 1988). Each month brings data on between 30 and 50 new loss events. Fortunately, however, only a few of these have to be added to the list of major natural catastrophes, that is only those involv-ing damage that considerably exceeds what the country or region concerned can handle itself so that substantial na-tional or international aid is required. That is generally the case if the number of dead runs into the hundreds or thousands, if tens or hundreds of thousands of people are rendered home-less and if the total economic loss is in the region of US$ 100 million. These major catastrophes, about which we have fairly comprehensive data even in the case of those that occurred farther back in the past, numbered only 14 in the 1960s but 70 in the 1980s; they there-fore increased over a period of 30 years by a factor of 5. The trend is also very clear when we consider the total economic loss, which - adjusted for in-flation - increased during the same period from an average of US$ 3.7 bil-lion per year to US$ 11.4 billion, i.e. by a factor of 3.1. The insured losses, finally, increased by as much as a fac-tor of 4.8.

Existing production facilities are constantly looking at ways to increase capacity. Many recent solutions have focused on the implementation of capital and infrastructure projects. It is accepted that capital expansions are frequently necessary for production gains; however, in many cases existing assets have the potential to deliver substantially better throughput than they are currently achieving. By focusing on automation of production control systems, production reporting and business processes substantial gains can be achieved utilising existing assets. This potential can be realised quickly and has been proven across several sites and industries. In the Pilbara region of Western Australia throughput gains of between two per cent and 25 per cent (translating to millions of tonnes per annum) have been achieved from existing assets using the methodologies discussed. The improvement process involves evaluating the entire production chain from the time the ore arrives at the plant to the time it is dispatched. A technical evaluation of the performance of the control system and plant automation interfaces to upstream and downstream processes is conducted. The relationship of these processes to the business systems and the human interface to the systems is reviewed to ensure a complete picture of the system is understood. Technical evaluation targets include: individual equipment operation, the engineering level of control systems, feed controls, etc; managing recirculating loads, surge capacities, dynamic inventories as well as management of in-feed systems and integration into yard and out-feed systems; and communication flow between control and other systems, data alignment between rail and shipping schedules, yard systems, sales systems and maintenance systems including such fundamentals as truck dispatch links to plant control. In parallel with the technical evaluation, business system evaluation takes place, including analysis of the operational systems, methods and personnel.
