"Abstract-The Amalgamated Kirkland gold deposit is located at Kirkland Lake, Ontario, within the Archean Abitibi Greenstone belt of the Canadian Shield. It lies on a fault or "break", termed the Cyprus Break, which is a splay off a regional structure termed the Cyprus-Battle Mountain Break. The Cyprus Break merges with the Cyprus - Battle Mountain Break above the -250 m level, as it was not encountered in shallow drilling or surface exploration. These structures lie 1.5 km south of the 727 000 kg (23 million ounce) Kirkland Lake Main Break and 0.5 km north of the 872 000 kg (30 million ounce) Larder Lake Break.The deposit is hosted by alkalic Timiskaming Group volcanic and sedimentary rocks which are overthrust by older Larder Lake Group tholeiitic and komatiitic flows along the Larder Lake Break. Timiskaming syenitic intrusives, similar to the host rocks for the Kirkland Lake Main Break gold deposit, intrude both suites.The Amalgamated Kirkland deposit consists of lode-style gold mineralization which is mainly hosted by altered and pyritized Timiskaming trachytic volcanics which wedge out or thin at depth, between two sedimentary units. The intersection of the Cyprus - Battle Mountain and Cyprus Breaks, in conjunction with the westward plunge of the volcanic wedge, appears to control the westerly plunge of the Amalgamated Kirkland deposit at approximately 50º.Mineralization is characterized by blue-gray brecciated and "wormy" quartz-ankerite veins which contain up to 10% fine-grained pyrite and, in decreasing order of abundance, minor galena, chalcopyrite, sphalerite, molybdenite and visible gold. The sulfides and gold commonly occur along fractures and wallrock inclusions within the veins. Native gold occurs as fine pinpoints distributed in 1-mm to 5-mm size clusters of up to ten or more grains.The auriferous veins occur within a quartz-ankerite-sericite-pyrite alteration assemblage which is enveloped by a broader zone of ankerite-sericite±hematite±quartz-pyrite alteration up to 60 m wide. This latter assemblage overprints the regional greenschist-facies chlorite-calcite assemblage.The Amalgamated Kirkland property has had a long history of exploration activity beginning with the first discovery of mineralization in 1911 on the Hunton Shaft claim located in the northeast corner of the property. Since that time various prospecting, mapping, trenching, geophysical surveys and diamond drilling programs have concentrated on either carbonate zones along the Larder Lake Break or quartz veins along the northern side of the property in association with complex trachyte flows and syenite dikes.Battle Mountain Canada Inc. located the Cyprus - Battle Mountain Break through a prospecting, trenching and geophysical program in 1989. Drilling by Battle Mountain and Cyprus Canada under the surface showing, coupled with several revisions to the structural interpretation of the Cyprus - Battle Mountain and Cyprus Breaks, resulted in delineation of the Amalgamated Kirkland deposit. Location of an auriferous structure through basic surface exploration within 1 km of a world-class orebody demonstrates both the subtle nature of Archean gold deposits and the potential for finding new mineralization in heavily prospected camps."

"The 2010/11 Canterbury earthquakes triggered many mass movements in the Port Hills including rockfalls, rock and debris avalanches, slides and slumps and associated cliff-top cracking. The most abundant mass movements with the highest risk to people and buildings were rockfalls and rock/debris avalanches. Over 100 residential homes were impacted by landslides, leading to the evacuation of several hundred residents.Volumes of rock leaving several of the larger cliffs during the earthquake sequence were determined from terrestrial laser scan change models. There were no seismically instrumented cliff sites and there was some distance between the cliffs and nearest strong-motion sites. Therefore, we synthesised free-field rock-outcrop seismograms by employing a stochastic approach controlled by source models and regional parameters derived using spectral inversion of the extensive strong motion data set. Relationships between volumes leaving cliffs during the earthquakes and site peak ground acceleration (PGA), peak ground velocity (PGV) and Arias intensity were compared for different sites. Multiple linear regression was used to analyse the variables that best predict the volumes of debris that fell from the slopes during the main earthquakes. The best correlation between the volume of debris falling per square metre of slope face and the seismic forcing parameters was for vertical PGV. The results from the multiple linear regression incorporating slope height, inclination, PGV horizontal and PGV vertical, improved the statistical relationship.Field data and results from the 2D seismic site response assessment indicate that the following factors affect dynamic performance of the modelled cliffs: 1) cliff geology—mainly material modulus, shear strength and shear wave velocity; 2) slope geometry—ridge-scale versus site-scale effects; and 3) the temporal aspects of the earthquake shaking (i.e., single acceleration peaks of large amplitude versus multiple peaks of smaller amplitude). Model results show that amplification of shaking does not increase linearly with increasing height, but instead reflects changes in the cliff geology where material strength, modulus and shear-wave velocity contrasts lead to acceleration contrasts.These factors show that the use of well documented case histories provide the basis for more certainty in seismic landslide assessments compared to those that are only empirically based."

"Answers to some of the problems ofRecovery and Metallurgical AccountingIn Gold and Silver Hydrometallurgical PlantsThe start-up over the last few years of new hydrometallurgical plants for the recovery of gold and silver has brought to the surface again a number of problems. Most of the problems have existed for some time and they manifest themselves mainly in two ways. Firstly, the plant results do not agree with the Metallurgical Test work figures and secondly, the Metallurgical Accounting figures do not balance.The plant results not agreeing with testwork figures are mainly due to:a) The testwork samples were not representative of the eventual plantfeed;b) Misinterpretation or misunderstanding of testwork;c) Plant losses;d) Plant design not allowing testwork results to be achieved;e) Operating deficiencies.a} The testwork samples being dissimilar to the plant feed occurs on a very high proportion of projects. The effects of this can beccr.ie exaggerated because the upper levels of the mining company often regard the original testwork figures as sacred and therefore base their judgement of current plant performance on these figures.Frequently the test work samples represent only a minor part of the orebody or alternatively a single composite of the whole ore body is used for the testwork long before it has been determined that all of the potentially different ore zones behave in the same manner. These errors can be compensated for by repeating the ""testwork tests 11 on current plant feed on a routine basis and using these tests to determine whether or not the plant is operating satisfactorily."

"Ventilation in process buildings is key to proper mill function. Correctly engineered systems to evacuate surplus thermal energy from process areas allow optimized rates of air change, thus assuring the good functioning for the lifecycle of the process while demonstrating conformity with occupational hygiene standards. Computational fluid dynamic simulations, coupled with traditional calculations and physical modeling, can demonstrate to end users and assist engineers in the optimization of mill ventilation, reducing total installation costs while assuring that the proper solution is in place for the complete lifecycle of the process equipment. Results have been fully implemented on a number of steel mill and aluminium smelter applications and we continue to study, optimize, and innovate as new tools become available to provide the best possible engineered solutions for particular applications. INTRODUCTIONThe objective of this research project is the development and implementation of modern approaches and computational fluid dynamics (CFD) simulation in the engineering of green ventilation of hot process buildings. This will assure the predictive performance and improve the quality and ease of construction of industrial gravity ventilation equipment, along with the adjunct hot process buildings, while lowering the overall project costs and time to completion.In order for a hot process building, such as a steel mill, to properly function, heat must be extracted from within. This is accomplished by having air inlets at the bottom of the building and outlets at its apex. The reason for this is to maximize the potential chimney effect. When using stack (buoyancy) effect ventilation, the process heat becomes the driver. Therefore, increasing this distance (height) improves the ability of the heated airstream to utilize basic gravity for acceleration of the heat/air flow, allowing a regular number of air changes per hour while ensuring that the process environment remains weather resistant under normal operating conditions.This process has been understood since the time of King Charles I of England, who decreed in 1600 that buildings should have 10 foot ceilings and that windows should be made taller rather than wider to allow better smoke extraction.Figure 1 is a painting of the Bethlehem Steel Works in the 1860s, where the concept of air inlets and penthouses on the roofs for air outlet was already being put into practice. Figure 2 is an early photograph of the Pittsburgh Steel Works."

"This paper explores the history of AC static variable frequency control drive systems on open cast mining machinery, in particular electric mining shovels and electric walking draglines, over the past ten years. It critiques the peformance of these drive systems from a productivity and operating cost standpoint and evaluates the over-all success of the original design concepts as they were envisaged in the 1970s. The paper details the state-of-the-art as it exists today as well as design improvements contemplated for the immediate future.IntroductionConsidering the fact that the very first successful application of an alternating current electrical distribution system occurred at the Gold King gold mine near Ames, Colorado, U.S.A., in the year 1891, it seems somewhat ironic that almost 100 years later, AC excavator drive systems are just now becoming commonplace in the mining industry.If one were to read the biography of Lucien L. Nunn, the first person to successfully install a commercially viable alternating current power distribution system, one would quickly visualize a parallelism between his fight to achieve acceptance of his new lower cost technology and that of those commissioned with the belief that AC static variable frequency control drive systems are not only a better performing, lower cost drive system for mining excavators today, but moreover, the drive systems of the future in our industry.This paper will deal with the evolution and incorporation of AC static drive systems on open cast mining excavators. It will trace the history of the development of these drive systems in the mining industry, the current state-of-the-art concerning these systems, and what the future holds for these excavators in our industry."

"IntroductionIn respect to The Human Dimension by M. Phyllis Bray, this author's impression is that research on long distance commuting (LDC) is long overdue. This type of mining is not new to many workers, especially those from areas such as the Maritimes, who have had to look for work away from their families for at least a decade. Perhaps to the industry the over-all set-up is new, but to workers who have faced mine shut-downs in their areas finding a job was the main objective and if the security of long-term employment was evident, then the families might join them later. This was the situation with which the writer' s family was faced.LDC LifestyleBeing the wife of an underground miner, the author has personally experienced this lifestyle for almost 10 years, and has never experienced accommodation in a company town. The first five years of marriage were spent in a small rural municipality within 50 miles of the worksite, but the town had industry separate from mining and was not affected by the mine shut-down.Since her LDC lifestyle began her family has been pretty much city-based, out-of-province commuters with two children and a third to arrive some years later. She therefore lived a fairly normal married life with a husband home daily. The author has experienced life with a husband who worked away for five-day stretches and then drove long distances to be home weekends or the portion of time left over from driving and fatigue. She has done the 3D-days in/l4-days out routine, and for approximately the past four years has been existing on a two-weeks in/two-weeks out ritual .The author does not agree that the ""bottom line is that mining families go where the jobs are"" . The miners go where the jobs are and the families fend for themselves with the assistance of the pay cheque making it home each pay period.Perhaps because there are many personal frustrations with this type of lifestyle, perceptions may be inaccurate. Having only dealt with underground operations, there may be gross differences between the pressures and demands of other forms of mining."

The rougher flotation stage is used to reject liberated gangue at a coarse size. Base and precious metals due to their high specific gravity tend to recycle in the grinding circuit and report to the flotation circuit at a finer size than the gangue from which it is being separated. Generally liberation levels of greater than 50% are achieved in roughing and thus a portion of the feed can be immediately upgraded to final concentrate. The proportion increasing as the amount of liberated mineral in the feed increases. Scalping out concentrate from the rougher can reduce the size of the subsequent regrind and cleaner circuits. Examples of alternative process design for circuits demonstrates that you don’t need large volumes to effectively recover all of the minerals – you need smart flotation. Several case studies will be presented where circuits can be significantly reduced in size by designing smarter – not larger. This paper describes case studies to test the amenability of the Jameson Cell to produce final concentrate from rougher feed. Metallurgical performance and plant design implications are discussed. Froth flotation has been used extensively for mineral concentration for over 100 years. Although the flotation cell design has changed, it has generally simply increased in capacity rather than increase in efficiency. Cell capacities of over 600 m3 are now being used in rougher flotation particularly for the lower grade ores where the amount of surface area for flotation is deemed less significant than the required volume. The plant design volume in rougher flotation comes from laboratory batch flotation tests and/or pilot plants. The volume required for the plant is calculated based on the laboratory residence time multiplied by a scale- up factor. Examples of laboratory to plant scale-up design are given in Dunne et al. (2010 part 2). The factors are usually between 2 and 3. A laboratory residence time of 10 minutes would equate to a plant design residence time of 25 minutes using a 2.5 factor. The volume of the required flotation cells can then be calculated using the design tonnage and volumetric throughput. This scale-up factor takes into account the differences in performance between batch flotation efficiency and full scale plant. The differences are likely related to inefficiencies of bubble particle contact in large cells as well as froth recovery, which is essentially complete in a batch cell. Flotation cells, such as the Jameson Cells, are smarter designs that improve bubble/particle collection, bubble/pulp separation efficiency and operate with high froth recovery rather than simply getting bigger. The Jameson Cell was designed on the principal of improving the collection efficiency of fine particles by separating the contact zone from the separation zone and having the bubbles generated by the plunging jet shearing and entraining air. This means the method of bubble generation is not impacted by the wear of mechanical equipment as it the case in other flotation devices. Jameson Cells make bubbles less than 600 micron in size under most operating conditions (Ding, 2016).

In September, 1953, seismographs were installed in the Crowsnest Pass area of Alberta and British Columbia to study the 'bumps' which were occurring in the coal mines at Fernie and Coleman, and to investigate the possibility that they might be related to minor earthquakes in the area. During the initial phase of the work, three seismographs were operated at widely separated stations, two in the region of coal mines at Coal Creek and Coleman and one at Turner Valley, distant from any mining operations. Some earthquakes were recorded at all three stations and, in addition, many small disturbances were recorded by the seismographs located near the mines, especially -by the instrument at Goal Greek. A closer network of radio-linked seismographs was then in-stalled near Fernie, the records of which definitely established that the small disturbances were occurring in the mine workings, both active and inactive. No relationship was established between bumps and local earth-quakes in the area. This work was initiated as a -co-operative effort with the Fuels Di-vision of the Mines Branch, Department of Mines and Technical Surveys, which has undertaken a study into the phenomena of ground stress relief in underground mine workings.

INTRODUCTION Preparation of this article, at the request of the Committee on papers, has been undertaken in the hope that it may stir up helpful discussion and constructive criticism. The Canadian mining industry is in the dumps and there is no use trying to deny the fact; nor does it help to say that other industries are in as bad or a worse condition. Let us face the situation honestly and see just how serious it is. The entire financial and industrial world is in a state of depression. The gravity of the case is no doubt exaggerated by contrast with the wave of optimism and false prosperity which pre-ceded it. The prevailing gloom and pessimism are just as irrational as was the former super-optimism. As far as we mining engineers are concerned, the causes are quite beyond our control, and it is the effect upon our own industry which interests us most. This effect is an accumulation of mineral products with consequent depression in price to a point where profits have almost, if not quite, disappeared. Whether this accumulation of stocks is the result of over-production or under-consumption is difficult to say, but it would seem likely that there is a combination of the two. The introduction of selective flotation has brought about a large increase in the production of zinc, a moderate increase in the production of lead, and a comparatively small increase in copper. It has turned relatively small mines into great producers, as in the case of the Sullivan, revived expiring mines like La Cananea, and brought-in new ones like Flin Flon

"11.1. IntroductionThe recovery of platinum-group elements (POE) from ores is a subject which has received almost no mention in the literature. Edwards (1975) refers to the process used for refining of the POE, throughout the Western World, as a so-called ""conventional"" process .... ""not because of the simplicity or convenience of the operations, but more probably because of the inertia of established processes and the aura of secrecy with which the major platinum refiners have surrounded this process."" There is no doubt in this writer's mind that the latter is also applicable to the dearth of knowledge of the mineralogy, geochemistry and ore deposits of PGE and that the poor recoveries, mainly attributed to the mineral beneficiation stages of concentration, are also affected by this traditional secrecy. Fortunately, this attitude is changing, due no doubt to the advantages that accrue from more open research which may be critically assessed in the literature in the traditional manner. Examples of this more open approach to recoveries and recovery problems are represented by Newman (1973), who gave some typical recovery percentages for Merensky Reef ores, and the recent encouragement and cooperation on mill samples from the Sudbury area by lnco Metals Company (cf. Cabri, 1981a).In general, platinum-group elements are recovered from an ore by the use of a physical concentration method, such as gravity separation or flotation, followed by further upgrading using further physical, wet chemical or pyrometallurgical techniques. The final process of refining in which the individual platinum-group elements are produced in pure form is outside the scope of this chapter.Platinum-group element deposits naturally fall into different categories according to the recovery process best suited for them, viz. those amenable to gravity separation, those amenable to flotation, and those where platinum-group elements are by-products of base metal sulphide, chromite and miscellaneous other deposits."

In 1920 the writer presented some notes, embodying his experience with storage-battery locomotives as main under-ground haulage-motors at Brazeau collieries. (1) Since that date these motors have stood up well to the test of maximum output and heavy overloads, having accomplished all that was expected of them. The notes presented in 1920 gave a brief description of the conditions under which these motors were working at Brazeau collieries, also the cost, per ton mile, of operating them. The cost per ton mile has since then been appreciably reduced, by working the locomotives nearly to capacity and under more ideal haulage conditions, on both shifts. It is not the intention of the writer to repeat what was written in 1920, but rather to give members the benefit of his experience in operating different types or designs of locomotives; to draw their attention to the drawbacks of some designs, and to the improvements made in some of the more recent storage-battery locomotives put on the market, as regards efficiency, upkeep, and safety. Because of their flexibility storage-battery locomotives have advantages over any other type of haulage, where grades are suitable to their use. The number in use in the mines in the United States has increased enormously during the past few years, and this method of haulage is, at the present time, attracting the attention of British and Continental mining engineers.

The HNIA field is part of the Tinrhert project which consists of developing 17 fields containing essentially gas with a few small rings of oil. It is considered as one of the main fields in this project in terms of its area and the number of geological structures that has formed it. It is a wide anticline of Cambro-Ordovician age, orientated NNE-SSW and located in the South-eastern part of Illizi basin. This structure extends from Alrar-center in the North, to the North of In Amenas in the South. The area is 1350 km2. The main reservoirs forming HNIA field are the Devonian (F2, F3 and F4), Siluro-Devonian (F6) and Cambro-Ordovician. These latest are formed by 4 sandy units alternated by layers of shale and shaly silt termed from the base to the top as follow: Unit II, Unit III-3U, Unit IV-2L and Unit IV3; these units are the subject of this study. The HNIA field is essentially gas, up to day this field is at the stage of appraisal and evaluation in order to establish a development plan; therefore the main target of this study is to contribute to this frame work. The following objectives are: ?To build a structural model of the study area. ?To integrate all the sedimentological, structural and petrophysical data in order to build a 3D geological model

"Blast design is becoming more and more important as industrial limitations change in regard to economical and physical restraints. Certainly there are ways to alter the nature of the blast by using different explosives, various hole sizes and modifying the timing and charging details. However, it is not possible to alter the fabric of the rock mass. Therefore, it is necessary to learn more about the rock mass and its role in the blast design, and the resulting fragmentation. The major determinant in the degree of fragmentation, or lack of it, is proposed to be the inherent fragmentation of the in situ rock mass. A practical method of quantifying the discontinuities existing in the rock mass is presented and discussed. Likewise, the effects of inherent fragmentation on the resulting muckpile are reviewed. Finally, a fragmentation model combining the calculated inherent fragmentation and the Kuzram model is shown using a sample blast.IntroductionIn blasting, discontinuity characteristics (Fig. 1) control the method by which the rock mass will behave. Research carried out by Piteau (1970) showed a number of characteristics important in engineering design and, as proposed, in blasting processess. This paper gives a brief outline of the mining and milling methods which will be used on the Stirling zinc-lead-copper ores by Stirling Mines, Limited, who have for the past two years been developing the Stirling mine in Richmond county, Nova Scotia, and who will bring the mine into production in November, 1929. No attempt has been made to give the paper a detailed technical character, for the main reason chat, since base-metal mining is something new in Nova Scotia, it is felt that a general description would be preferable before this Society, whose membership is mainly made up of chose engaged in the coal mining and associated industries. Furthermore, as both mining and milling operations will be governed to a certain degree by the experience gained after production commences, there are many points which cannot be definitely stated now. GEOLOGY Before discussing the mining methods, it is desirable to present a general picture of the geology and occurrence of the ore-bodies. The surface rocks in the vicinity of the mine consist of rhyolites of .pre-Cambrian age. These volcanic flows outcrop in very few places, usually along' the beds of creeks or on the shores of the numerous small lakes. Consequently, a detailed geological map is not available. The exploration work carried out to date has revealed a sheared zone extending through the volcanic flows for a distance of over 1,000 feet. Along this zone, there has been a replacement of the rhyolites by a carbonate rock for widths varying from 50 to 230 feet, and it is in this carbonate formation chat the mineralization at a later date took place.

"This paper provides background information on the expansion programs of Rio Algom Limited at Elliot Lake. Included are details on the Quirke expansion program, the Panel mine rehabilitation program, including the selection procedure for the tailings containment area, the Stanleigh mine rehabilitation program, housing, government regulations and the Environmental Assessment Board hearings. Introduction After 2Y2 years of public hearings, 4 years of arbritrating differences of opinion between our engineers and government licencing authorities, interspersed with research into radium removal and development of the LAMIX process and carrying out the work associated with some 24 control orders issued to us by the MOE, it is very difficult to compress all of the experiences and frustrations into a single paper. The highlights, as they relate to the expansion programs of Rio Algorn Limited and its affiliate, Preston Mines Limited, will be covered. Denison Mines Limited, of course, is the other uranium producer in Elliot Lake. They are currently in the process of expanding from 7,000 to 11,000 TPD by the second half of 1980 and then to 15,000 TPD by the mid 1980's. In giving this overview, the following seven items will be discussed: • The basis for Rio Algom's and Preston's expansion plans. • The Quirke Expansion. • The Panel Rehabilitation. • The Stanleigh Rehabilitation. • Housing Programs. • Government Licencing Requirements. • Environmental Assessment Board Hearings."

"Following the depletion of nickel sulfide ores, laterites are considered as one of the main sources for producing nickel and cobalt. One of the primary steps in pyrometallurgical extraction of nickel from laterites involves partial reduction of the ore at temperatures below 1000°C. Studying the kinetics of laterite reduction provides valuable information on reducibility of nickel laterites using coal as a reductant. The present study investigates the reduction behavior of a nickel laterite by thermal gravimetric analysis and differential scanning calorimetry. Non-isothermal reduction tests and X-ray diffraction analysis were performed to find the main reactions that occur during the process. The kinetic parameters were calculated by Coats-Redfern method. The general model of simple reaction f(a)=(1-a)n was used for calculating kinetic parameters.INTRODUCTION Nickel (Ni) naturally occurs in the form of sulfide and laterite ores. Laterite ores are mainly available in tropical regions. Weathering phenomena during millions of years has led to the formation of several layers with specific mineralogy (Butt & Cluzel, 2013). The three main layers downward are limonite, smectite, and saprolite (Crundwell et al., 2011). Although laterites are the most available nickel ore resource (70%), their share in the overall Ni production is limited to 40% (Crundwell et al., 2011). The main reasons for not materializing the full potential of laterites are as follows (Ray et al., 1985): •,Laterite ores contain high water content (free and combined) which in turn results in high energy consumption of the extraction process. •,As Ni is present in the form of solid solution in laterites, physical upgrading methods such as flotation are not effective in increasing the Ni content before the extraction process. •,Extracting Ni from sulfide ores involve exothermic reactions which are beneficial due to decreasing the overall energy requirement of the process, however there is not efficient release of energy in the process of extracting Ni from laterite ores."

"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

"ArcelorMittal Mines Canada operates hydraulic classifiers to reduce the silica content of an iron oxide concentrate from 4.5% to less than 1.2% SiO2. Sampling campaigns were conducted to develop a preliminary model for the hydraulic classifier. Results show that iron oxide and quartz are respectively concentrated in the coarse and fine size fractions of the classifier feed. The mineral partition curves in the classifier and the size distribution of the minerals in the feed are used to simulate the classifier operation in order to identify the source of variability observed in the daily plant production data. Results indicate that most of the variation is probably caused by unsteady operating conditions of the classifiers.INTRODUCTION Hydraulic classifiers are a class of gravity separation devices (Wills, 1992) used to classify particles according to their size and specific gravity. Hydraulic classifiers also known as density separators (Hearn, 2002) combine hindered settling and upward flow of water to selectively entrain light particles to an overflow stream. Denser particles settle to an underflow stream. Hydraulic classifiers are used for various purposes ranging from cleaning of coal (Das, Sarkar, & Mehrotra, 2009), recovery of chromite from plant tailings (Kumar, Tripathy, & Rao, 2009) to upgrading iron oxide concentrate (Hearn, 2002). ArcelorMittal Mines Canada operates several hydraulic classifiers to upgrade an iron oxide concentrate by removing fine quartz and other light minerals from hematite that is the main iron oxide mineral. A project was initiated to analyze the operation of the classifier in an attempt to identify the source of the variability in the production results and to assess the potential of increasing iron recovery in the classifier. One possible disturbance for the classifier operation is the change in the classifier feed composition and size distribution, while the other considered disturbance is the operation of the classifier. The paper is divided into five sections. The first section summarizes the ore processing sequence for the production of iron oxide pellets to identify the position of the hydraulic classifier in the process. The second section describes the hydraulic classifier. The third section discusses the sampling campaigns conducted on the hydraulic classifier. The method used to process the data from sampling, the results of the data processing and simulation of the classifier are discussed in the remaining sections."

In this paper we describe a mathematical model of the magnetic field and magnetohydrodynamic processes simulation in aluminum reduction cells developed in St. Petersburg State Polytechnic University, Russia. The model includes: (a) calculation of the currents in the busbar system and inside the aluminum cell construction elements including cathode and anode structures, aluminum and electrolyte layers; (b) magnetic field simulation using the integral equation method; ( c) investigation of the magnetohydrodynamic characteristics on the basis of a shallow-water approach. The developed model was used to investigate operational characteristics of the different industrial aluminum cells. Calculated magnetic field distributions and MHD stability parameters were compared with the available experimental information; the comparison demonstrated a reasonably good agreement.

We have been in business 129 years and like most of the coal mining companies in the United States, since 1969 the majority of our safety improvement efforts had been driven by compliance with the various versions of the Federal Mine Safety and Health Act and the supporting federal regulations. In 2004 Peabody Energy management came to the understanding that in order to achieve our goal of zero incidents, that focus must change. This paper will describe the elements of the journey we began at that point to make the transition to a behavior and systems-based approach to safety management and our efforts to win the minds and hearts of our employees. This journey is undertaken with the goal of making safety truly a way of life for all Peabody Energy employees.
