All flyrock incidents have the potential to result in injuries or fatalities that can result in loss of company reputation, license to operate with clients, and bear the exposure to high cost liability. From 2007 to 2009, one NA Quarry and Construction Business experienced a series of flyrock incidents. In the Spring of 2009, this business declared that the frequency and severity of Flyrock Incidents had reached an unacceptable level. A Flyrock Elimination Program was launched.

In the modern world of demolition safety is paramount and unnecessary or unquantifiable risks are not accepted (FasterCapital, 2024) and all risks must be kept As Low As Reasonably Practicable (ALARP) (BSI, 2017). This principle has led to a huge reduction in deaths in the last 50 years and the statistics continue to improve although this is starting to plateau (Beal, 2007). Under normal circumstances, this is now simply part of the normal way work is done and systems are in place to ensure both safety and economic profits are assured. What happens when the environment is less controlled and risks are unknown, such as after a natural disaster, war, an accident or a stand-up from a failed demolition attempt? A dangerous structure cannot responsibly be left alone with a fence around it so some action must be taken, but how can this be done safely and responsibly? An incident in Didcot, England in which half of a power station’s boiler house being prepared for an explosive demolition collapsed suddenly, killing four men in 2016 provides a useful case study of how technology, creativity and planning can be used to conduct complex demolition fully remotely to avoid any further risk to life.

Accident data obtained from the Mine Safety and Health Administration (MSHA) were analyzed by the Bureau of Mines to determine the most frequent causes of blasting accidents in U.S. mining and to examine trends in blasting accidents. This accident analysis is an update of a similar blasting accident study conducted by the Bureau and published in 1983. The analysis shows that there has been a recent reduction in mine blasting accidents: The annual average number of nonfatal injuries has decreased from 107 for 1978-81 to 46 for 1982-85. The annual average number of fatal injuries was reduced from 11.3 to 5.0 during that time.

Dynamite, diamond wire and hydraulic excavators overcame the most impressive building in town, working in very little spaces and on a geotechnically difficult site; moreover, a study on the dynamics and the effects of the impact of the structure, an analysis of the presence of potentially explosive dusts and an extensive vibrometric monitoring supported the design and the development of the operations.

This paper describes the technology and methods deployed in the continuous autonomous remote monitoring of cracks in interior and exterior walls of a residence near a limestone quarry. The objective is to quantitatively compare crack response due to blast-induced ground motion to that induced by diurnal temperature changes, weather fronts, occupant activity, and other phenomena. The remote monitoring and communication technology developed to obtain and disseminate crack response data for this study may provide the means to mitigate the effect of public concerns about potential impacts of ground vibrations inherent in construction and production of raw materials; it is critical to address these concerns because subsequent litigation may result in delays and increased costs.

The resolving capacity of the thermodynamic work-principal model has been seriously refined over its twelve year history, so a review of the innovations, wrong turns and their rectification is warranted to help future researchers. The work-principal model rests upon the four laws of thermodynamics that drive the universe and relatively few other assumptions: like the removing and imposing of constraints on the quasi-potential to yield trapped minimum key-states that represent the wanted process. The formative research was concerned with the role of ingredient changes, the removal of booster influence and the quench temperature with which to reckon toxic fumes. The transfer of reaction-zone energy as work upon the surrounding rock/water/fixture was not fully rationalized until the notion of reversed-inertial pressure was formulated and determined (for water) with underwater trinitrotoluene (TNT) shots. The rock-water-jelly model incorporated that term into its dynamic pressure formula as a way to represent rock blasts or underwater shots, while the zone of reaction gases remained trapped. The real irreversible thermodynamic process, which yielded the total expansion work, was later complemented with a wholly reversible imaginary process, to form a closed thermodynamic loop, with a remarkable consequence – the determination of the theoretical heave and hence, the undissipated-shock by difference from the total work. With the wholly developed model, it was recently possible to quantify the time retardation of reaction chemistry for non-molecular (mining) explosives, as compared to the rather negligible time-retardation found for the molecular (military) explosives in underwater tests.

A strong inverse relationship between rock face movement and stemming ejection is presented. Previously unpublished data from blast control plug research and development involving high speed video analysis, which was conducted between 1986 and 1988, is presented. Face and stemming ejection velocities were calculated for tests utilizing the same bench blasting configuration comprising; a 4 ft high bench in dolomite, 3 ft burden and single 1.75 in. holes loaded with 2 ft of small diameter cartridge emulsion. The singular difference between these tests was the stemming configuration. Varying lengths of stemming were used with the presence or absence of differing experimental configurations of blast control plug. The effectiveness of this plug has been published previously.

Blasting liability problems are frequently compounded by a company's inability to effectively communicate with concerned property owners and the lack of adequate complaint documentation. Often, by the time the insurance carrier becomes involved in a blasting claim, the situation has deteriorated to the point that a solution agreeable to both parties is virtually impossible to reach. The insurance carrier, wishing to minimize their lose potential may decide to pay on a somewhat questionable blasting claim rather than be drawn into lengthy civil proceedings that would ultimately end up in front of a jury with little chance for a successful outcome.

This paper presents recommendations for blast design to prevent damage due to ground motion from blasting on adjacent concrete structures of underground operations. It includes a case study describing a problem regarding an open pit mine expansion and asks whether production blasting will damage concrete shaft structures due to blast stress waves and blast-induced ground motion.
