Methods for increasing compliance to medical and health regimens were reviewed along with similar research in industrial and mine safety. Principles abstracted from both areas suggest that compliance with health and safety practices is improved when: (1) Workers are cooperatively involved with management in setting safety goals and planning for their achievement, (2) the program has the support of workers, supervisors, and management, (3) specific tasks responsible for injuries are targeted, (4) company organizational policy, educational, and behavioral programs work together toward the same goals, (5) workers and supervisors are frequently informed about their degree of correct task performance, and (6) programs are monitored by the frequency of correct performance of targeted tasks, (the means to lowered accident rates) rather than by lowered accident and injury rates themselves (the goals for safety programs.) Safety programs that incorporate these principles increase persons' adherence to specified health and safety practices and subsequently decrease injury rates. It is recommended that: (1) a group of experts be convened to study the application of these principles to coal mine safety programs, and (2) a short course for safety trainers be developed to demonstrate how these principles can be used to strengthen existing mine safety programs.

This report was written in an effort to provide better control measures for low back pain (LBP) and low back disability in the mining industry. There are numerous factors associated with development of LBP, many of which can be effectively controlled while some cannot. Better job design is promoted as the best method of reducing cases of LBP and can also reduce the disability (i.e., lost time from work) associated with LBP when it happens. The report draws attention to what is currently known about LBP, what the causes are thought to be, and discusses recent back injury trends in the mining industry. Research describing unique physical demands in mining, such as the capabilities and limitations of working in awkward postures, is also presented. Methods that can be used to prevent initial LBP episodes are provided, including facilities design and layout for materials and supplies, use of mechanical-assist devices, improved design of lifting tasks, and better seat design. Methods of reducing the disability associated with LBP (including workplace design, proactive return-to-work efforts, communication, and management commitment) are also discussed. The report concludes that control of LBP and disability in mining requires a comprehensive approach to limit the repetitive loading that can occur on the low back due to manual materials-handling tasks and exposure to whole-body vibration (WBV). Specific recommendations include the following: ? Successful LBP prevention efforts require a proactive program that has strong management commitment and incorporates employee involvement. ? More efficient supply handling systems and use of mechanical-assist devices can greatly reduce exposure to hazardous lifting tasks. ? Lifting tasks should be designed to minimize low back stress. Tools to evaluate and redesign lifting tasks are presented. ? Improved seat design can reduce exposure to WBV and improve posture, leading to reduced LBP risk. ? The disability associated with LBP can be reduced. Getting the worker back on the job as quickly as possible is in the interest of everyone involved.

To aid in winning the recent world war, the Bureau of Mines conducted a National Fuel Efficiency Program, directed to commercial plants throughout the Nation, to save greatly needed fuel, a basic resource vital to industrial production. In planning this program it was realized that to accomplish a satisfactory reduction in fuel consumption in any plant, it would be necessary to reach, enthuse, and usually educate to some extent the actual operators of the various types of plant equipment. A major part of the programs consisted, therefore, of visits to plants and work with the operators, including a review with them of their operating conditions and of educational and guiding literature pertinent to the operating problems involved. Through a Nation-wide organization, local volunteer engineers served in making visits to local plants. Each cooperating plant appointed an employee as the "waste chaser," who headed the work of saving fuel in that plant. The educational literature was designed to interest the operator, as well as serve as a guide in eliminating waste of fuel. Volumes have been written on many subjects relating to the saving of fuel. Writings of the treatise type contain considerable good information but do not have wide appeal and probably will be read only by a few studious operators. The problem is not one of getting clown on paper everything that the experts know and would like to have the operator know, but rather to write something that will actually interest the operator and stimulate him to action. Past experience led to the adoption, therefore, of the "quiz-sheet" type of writing, using a quiz sheet of not more than two pages for any principal item. The results obtained during the National Fuel Efficiency Program in many types of plants further demonstrated that the short quiz sheet is satisfactorily appropriate and does accomplish results. In safety work a study of past accidents is of value in determining means of reducing future accidents of the same character. This is particularly true with reference to coal-mine explosions. Data from publications of the United States Bureau of Mines and from published reports of the State inspector of coal mines of Colorado are presented in this circular in the belief that they will be of value to operators by indicating methods of preventing future coal-mine explosions. The tables included are complete only with regard to major disasters. The Bureau of Mines considers as a major disaster one in which five 'or more men are killed. However, this circular also includes a considerable number of minor disasters, Various explosions in the coal mines of Colorado from 1883 to 1932, inclusive, are listed with information available as to their probable cause, number of men killed and measures which, if they had been in use at that time, might have prevented them. John McNeil, first Colorado State coal-mine inspector, on page 43 of his first report for the period ended July 31, 1884, made the following recommendations for the prevention of explosions in the coal industry: True causes could be traced either to insufficient ventilation, carelessness and lack of rigid discipline in carrying on energetic ventilation and other precautions needed to be enforced in a mine giving off explosive gases. *** When any quantity of fine coal dust is observed in an entry, air course, or room it should be made damp and be removed from the mine, and thus beep every place as free from dust as possible. *** A system of bratticing close up to the face of each and every place in the mine from crosscut last made (whether gas should just then show its presence or not) which should not be more than 50 or 75 feet according to the nature of the mine.

A series of studies are described that identify critical health and safety skills needed by underground coal miners for coping with mine emergencies. First aid and self-rescue and escape were selected as two broad performance domains that include many subordinate skills needed for preventing, recognizing, and coping with mine emergencies. The performance domains and their objectives were used to direct the development of simulation exercises that permit miners to experience mine emergencies in the safety of a training room. The exercises are based upon research with similar simulations for the training of flight crews and medical personnel in information gathering, judgment, and decision making skills. Exercise content is drawn from actual mine accident and disaster case studies. The exercises incorporate recent principles of cognitive psychology and instructional design. Initial studies in annual refresher training classes suggest the exercises are stimulating for miners and their instructors, easy to use and read, sufficiently complex and authentic to challenge experienced miners, and useful devices for both teaching and testing critical problem solving skills required for preventing and minimizing emergencies.

Definite progress has been made in accident prevention in Illinois during the past few years; the State Department of Mines and Minerals is receiving excellent cooperation from coal companies and employees, the companies are making greater effort to maintain their mines in good condi- tion, and the employees are eliminating more unsafe practices. As a result, some coal companies have made commendable safety records that prove that coal can be mined with a comparatively low accident rate. However, there is need for further improvement, as the accident rate of the State as a whole is much higher than it should be. During the 55-year period 1882-1936, in- clusive, reports of the State Department of Mines and Minerals show a total of 7,005 (an average of 127.36 per year) men killed in and around the coal mines. Of this number, 6,425 (91.7 percent) were killed underground, 346 (4.9 percent) in the shaft, and 234 (3.3 percent) on the surface. Of the fatalities underground, 3,300 (51.3 percent) were killed by falls of roof, sides, or face coal, 1,167 (18.1 percent) by haulage, 639 (9.9 percent) by handling explosives, 579 (9 percent) by explosions, 151 (2.3 percent) by electricity, and 589 (9.1 percent) by other means. Of the fatalities in the shaft, 171 men (46.5 percent) were killed by falling down the shaft, 14 (41.6 percent) by the cage, and 31 (8.9 percent) otherwise. On the surface, 113 (48.2 percent) of the fatalities were due to railroad cars, 51 (21.7 percent) to machinery, and 70 (29.9 percent) to other causes. The fact that 7,005 lives (an average of 127.36 per year) were lost during 1882-1936 is sufficient evidence that in the past too little interest has been taken in safety by either the management or workers. A realization of this deplorable loss of life should act as an incentive to the industry to make greater effort to prevent accidents in the future. After an accident occurs, the facts pertaining to it become history, and anything that may be done afterward cannot change in any way the damage that has already been done, such as loss of life, personal injury, and destruction of property. The main benefit that can be derived from an acci- dent is that suitable procedure may be adopted and some lesson may be learned to aid in preventing a similar accident in the future. This U.S. Bureau of Mines report identifies the types of serious accidents that occur most frequently at small underground coal mines and describes the strategies that could help prevent these accidents. A wide variety of methods for improving safety are suggested, including improvements in the design of equipment, work procedures, work schedules, safety programs, and emergency response plans, as well as techniques for diagnosing the potential hazards associated with new technologies and work procedures. Some of the papers in this volume focus on preventing specific types of mining accidents--ones associated with materials handling (primarily back injuries), equipment maintenance, improper machine guarding, and ground failure during retreat mining. Although the recommendations in this volume are heavily influenced by research performed at underground coal mines in the Appalachian coalfields, most of the papers contain advice that is equally pertinent to almost any type of mine.

It tells how to use the exercise, presents the objectives, the master answer sheet, the scoring key, and discussion notes to be used following the exercise. The next section summarizes results from field tests of the exercise, and reports the miners' evaluation of the activity. The last part of this document is three appendices. Appendix A is the exercise problem booklet. This booklet can be duplicated locally. The booklets are reusable. One is needed for every person in the classroom. Appendix B is the answer sheet. Now, he has squared around enough so that you and Red, the miner helper, can set some posts on the corner. The two of you set posts along the left inby rib of #5 entry while the shuttle car operator waits. You are in a tight spot when Mo decides to start mining. You must decide where the best place is to stand for safety and to be out of the way.

Research workers from the Bureau of Mines and industry met with other government and industry representatives at the Mont Chateau Lodge, Morgantown, W. Va., on May 30-31, 1973, to discuss the current status of methane control in eastern U.S. coal mines. The opening remarks, the eight technical presentations, and the discussions of the members of a government-industry panel are published here for the benefit of those concerned with the control of methane in coal mines. In addition, an introduction has been added to present the background of work in this field.

Safety is fostered by this company in consideration of both the humanitarian and economic aspects; the officials, take pride in promoting the welfare of employees, and the management has found that the safest way is the most eco¬nomical and that unless an organization is operated economically it can not continue to exist successfully. In planning and operating any safety, organization, terry things must be taken into consideration because an Enterprise can not be operated solely for safety any more than it can for any other single part of the operations. All work must be planned and executed for the common interest.

In the Unfederated Malay States, with the exception of Johore, the rights of suzerainty, protection, and administration were transferred by giam to the British Government in 1909. In the States of Kelantan, Trengganu, Kedah, and Perlis, the rulers are assisted by State Councils and by British advisers appointed by the British Government.3 With respect to Johore, the largest, richest, and most highly organized of the Malay States not in the Federation, the treaty of December 11, 1885, with Great Britain, defined the relations of the State to the British Govern- ment and allowed the Maharaja to assume the title of "Sultan"; an amendment to this treaty (May 12, 1914) provided that the Sultan should be advised by a British officer, the General Adviser. ...... The political organization of the States is reflected in the mining legis- lation, Johore being the only one having a comprehensive code. During 1998-99, groundfall incidents resulted in 27 fatalities and were responsible for over 70% of all deaths in U.S. underground coal mines. To obtain a better understanding of where and why these incidents occurred, a comprehensive analysis of ground fall injuries and fatalities was conducted. The first portion of the study examined various factors associated with roof and rib fall injuries and reportable roof fall noninjuries that occurred during 1995-98. The study found that the room-and-pillar mining method has twice the groundfall incident rate than the longwall method. Mine locations with high groundfall rates seem to correlate to regions where there is a higher concentration of problematic coalbeds. For example, the Illinois Basin has very high groundfall rates, which can be traced back to several key coalbeds-Kentucky No. 13, Herrin/No. 6/Kentucky No. 11, and Springfield No. 5/Kentucky No. 9. High rib fall rates were found in mines located in thick seams. Groundfall rates were found to be 30% to 40% higher during the months of July through September, possibly due to high humidity that may cause the shale mine roof to deteriorate. The second part of the study examined the root causes of failure by reviewing all groundfall fatality reports for 1996-99. Primary and secondary hazard factors were assigned to each groundfall incident. The primaryf actors resulting in these groundfall fatalities were pillar extraction, traveling under unsupported roof, skin failure, construction, longwall faces, intersections, and geologic discontinuities. Defining prominent ground control incident trends and hazards will identify areas where additional study is needed and where innovative solutions need to be developed to reduce these severe occupational hazards.

The chapter will also help if your dust control problem doesn’t fit any of the circumstances described in later chapters. ] This chapter describes the three major control methods used to reduce airborne dust in tunnels and underground mines: ventilation, water, and dust collectors. It also describes methods to reduce the generation of dust, so less has to be removed from the mine air. The ventilation methods provide the best use of air in the vicinity of workers and in the vicinity of dust sources. In this sense, the methods described are local ventilation methods. Most of the emphasis is on so-called displacement ventilation because it is the most effective dust control technique available. The section on water sprays outlines the dual role of sprays—wetting and airborne capture—and describes why wetting is more important. It also corrects some of the misconceptions about spray effectiveness and describes circumstances where sprays can actually increase the dust exposure of workers. The section on dust collectors outlines the circumstances under which dust collectors can be expected to function effectively. It also describes some common design and maintenance problems. The final section describes how to reduce the amount of dust that gets into the air in the first place, since once the dust is airborne it is always harder to control.

Problem: Incident investigation reports do not usually contain enough information to aid in studying boom arm vertical speed for roof bolting machines to determine the impact that appendage speed had on an operator's risk of experiencing a contact. Laboratory experiments with human subjects are also not feasible because of safety and ethical issues. Method Researchers successfully developed a three-dimensional computer model that uses virtual human simulation software as the primary means to gather contact data when the boom arm touches the operator's hand, arm, head, or leg. Results: Data analysis of roof bolter simulations shows that the speed of the boom arm is the most important factor in determining the risk of an operator making contact. Regardless of other variables, contact incidents were always greater when the bolter arm was moving up, greater on the hand, and greater for the boom arm part of the machine. The reason why the subject experiences more contacts when the boom arm is moving up rather than down is that more risky behaviors occur during drilling and bolting when the boom arm is ascending. Based on the data collected, boom speeds greater than 13 in/sec result in a substantial increase in risk to the roof bolter operator of making contact. Speeds less than or equal to 13 in/sec are associated with a more modest relative risk of making contact, which represents a decrease in potential hazard. Impact on Industry: The use of such information can be quite helpful in making recommendations to machine design and task procedures to reduce the likelihood that roof bolter operators will experience injury due to contact with a moving roof bolting machine's boom arm.

Several significant problems associated with employee turnover among underground miners are described in this Bureau of Mines report. Although a vast amount of research has been conducted on turnover among employees in nonmining industries, there has never been an attempt to empirically investigate the causes of turnover among underground miners. This report presents the results of empirical research on employee turnover in other industries and describes and critiques the research methods that have been used to investigate turnover. Using the findings on employee turnover in other industries and known data about miners and the mining industry, a conceptual model of the factors that cause turnover among miners was generated. This model, which is expected to provide a basis for later empirical studies, is presented. Several recommendations for controlling miner turnover are also presented.

Groundfall accidents have been the leading cause of fatalities in the underground coal mining industry for many years. Statistics from the Mine Safety and Health Administration (MSHA) indicate that approximately half of the victims of these fatal accidents were in an area where no devices had been installed to support the mine roof. This report was prepared by the U.S. Bureau of Mines to provide guidance to operators of underground mines concerning the use of techniques that may help to keep miners from going under unsupported mine roof. The report is divided into two major sections. The first section reviews the findings from several areas of empirical research pertaining to strategies for convincing employees to avoid unsafe acts and/or adopt self-protective behaviors. The strategies are (1) incentives and feedback, (2) disciplinary actions, (3) training (primarily fear communications and behavioral modeling), and (4) nondirective techniques. This literature review has a broad focus and should be helpful in making decisions about how to encourage a wide variety of types of self-protective behaviors. The second major section of the report presents an analysis of each strategy's potential for keeping miners away from unsupported roof.

Like many experienced workers, remote control continuous miner operators perform their everyday jobs making choices and decisions that they may not consciously think about. While performing tasks, they are continuously processing feedback and cues which guide them for the next move. Initial training teaches the operators the tasks necessary to operate the equipment and perform the job. However, with experience they are better able to deal with hazardous conditions and non routine situations. To better understand what potential cues and feedback help to guide the operators, a questionnaire was developed to learn what factors affect decisions. Operator responses address issues related to specific tasks, equipment operation, visibility, lighting, and communication issues. The information collected is useful to understand what cues experienced operators use to make task performance decisions. Results will be used to develop interventions and training for safe and effective operator performance.

The XVIth International Conference on Coal Mine Safety Research was divided into eight half-day technical sessions. In accord with the established practice of these conferences, at each session, presentation of the papers was followed by a discussion period. Questions and comments submitted in writing to the session chairman were answered or discussed by the authors of the relevant papers or by their representatives. These discussions were tape-recorded in the original language of the speaker and subsequently translated into English when necessary. Questions to which no answer was available at the time and a few comments that exceeded the scope of the session have been omitted.
