The Los Angeles Metro Red Line has installed high density polyethylene (HDPE) as a gas and water barrier around all subsurface structures. The paper reviews the geological setting and resulting need for the barrier. HDPE installation practices are outlined, as are the risks to the barrier, from normal placement methods. Testing and inspection methods used to assure a quality installation are described, and also the field tag method for recording deficiencies and their repair. Specifications and installation changes that will be incorporated into subsequent contracts to improve the quality of the installation are high lighted. The cause of the fire that destroyed part of a tunnel is reviewed as are the revised precautions put in place to prevent a recurrence.

It was recently proposed to use the pastefill technology as a surface tailings management method. Surface paste is an economic and safe storage method that respects current environmental regulations. There are many problems related to accidents that occur in conventional tailings damps. In this study, the applicability of the surface paste disposal (SPD) method for a Pb-Zn mine in western Turkey was examined. Density was 3.084 and specific surface area was 0.899 m2/g. The mill tailings have very low plasticity and show characteristics of clays and silts in terms of the Unified Soil Classification System. The particle size distribution of the tailings was 39.2% particles finer than 20 µm. The results of this study showed that the physical and chemical properties of mill tailings make them suitable for SPD.

Advances in conveyor technology over the past decade have enabled the design and erection of very long overland belt conveyors. Such conveyors usually require head and tail drives to lower the belt tension and belt strength required. This paper provides an overview of the problems encountered in controlling the head and tail drives on such conveyors and some of the solutions developed by CDI for several such projects.

The Cannon mine, located in north-central Washington is mining gold hosted in silicified Eocene-age arkosic sediments arrayed along a regional strike-slip fault system. Branches of the Eagle Creek fault have broken the Chumstick Formation into blocks and wedges, and have caused intense deformation of stratigraphy in the mine. Mineralization at the Cannon mine (B-Reef) and O-Reef (L-O mine) is a bulk silicification with high grade cross-cutting veins containing principally electrum, silver sulfosalts and pyrite in a gangue of quartz, adularia and calcite. Orebodies have very irregular shapes, but tend to follow stratigraphy and high-angle pre-mineral fault breccias and splays developed adjacent to the major faults. Both B-Reef and O-Reef occur at significant bends in the regional trend of major fault strands. Mean gold grades decrease upward from near sea level to the 300 m elevation at the Cannon mine. A new discovery, the A-Reef is hosted by a moderately dipping structure which is at a low angle to bedding. Fluid inclusion studies of B-Reef ore zones indicate dilute ore-bearing fluids boiled at temperatures between 185 and 2850 C at a depth of 1 km. Many ore zones are bounded by sheared mudstone and claystone and a much broader halo of argillic alteration comprising illite, smectite and sericite. Trace and major element analysis by ICP-ES for 33 elements, LECO for S, Hg by CVAA and gold by AA-FA shows dispersion patterns hundreds of meters outward from the known orebodies in the Cannon mine. Geometric mean comparisons, hierarchical R-mode cluster analysis and single element spatial plots show that Hg, As, Sb, S and K/Al compose a suite of elements which are significantly dispersed away from, and increase toward mineralization. Ba, Ca, Na, Mn, V and Zr decrease toward mineralization. The suite is more selective than gold with respect to known mineralization within the exploration area.

The occlusion of unliberated gold particles in silicates and sulfides in crushed/milled ore dictates the recovery that can be expected. The grade of this occluded gold in cyanide leach residues shows a consistent, general relationship to the head grade of the ore, irrespective of mineralogy or geographical location. The residue grade almost invariably increases with the head grade, but not in proportion to it. This phenomenon appears to stem from the nature of the geological events originally responsible for creating the ore deposits. A statistically-based technique for determining the true head grade-tail grade relationship from gold plant operating data is described. The effect is illustrated with results from several worldwide gold milling operations analyzed over the last 35 years.

Organized efforts toward the prevention of injuries and occupational diseases have been of comparatively recent origin in the mining industry, dating generally from the beginning of this century. Mining-and particularly underground mining- is more hazardous in many respects than most other major industries, and continual and increasing attention has been given to its health and safety aspects. The importance of the physical well-being of those engaged in mining was recognized by Agricola, who, after describing the hazards encountered in 16th century mining operations, stated that "Some of these evils, as well as certain other things, are the reason why pits are occasionally abandoned." In the past 50 yr particularly, mine managements and labor organizations, separately or collectively, have mounted productive efforts toward the prevention of accidents and industry-associated diseases and illnesses. A statistical study of this period indicates that increasing awareness coupled with the endeavors of all concerned agencies has resulted in decreasing frequency of occurrences of injuries and illnesses. In recent years particularly the mining industry has used the services of safety engineers, industrial hygienists, inspectors, ventilation engineers and other specializing in locating and correcting deficiencies affecting the welfare of employees. Concurrently, the labor movement has incorporated into its organization individuals working in the fields of health and safety, many devoting full time to these responsibilities. Others serve in behalf of their fellow workmen as on-the-job safety representatives. Most authorities agree that accidents are caused by (1) unsafe acts, (2) unsafe conditions and, exceptionally, (3) Acts of God. The "unsafe act" often is cited as the prime cause. but many accidents, and attendant injuries, are the result of a blend of "unsafe acts" and "unsafe conditions," with each chargeable in varying proportions. It therefore is necessary to consider both the physical elements of the mine plant and the training, mental attributes and actions of all personnel involved. Analysis of an accident, if based on hasty reconnaissance and unduly hurried questioning, seldom will disclose more than surface causes and consequently will result in inadequate measures to prevent repetition. Mining involves unavoidable and calculated risks, as in all facets of life. It is most important then that men employed in mining be schooled in the hazards and develop fully the ability to evaluate and minimize risks. The environment, of underground mining often is harsh and strange compared to the more natural surface conditions man has learned to cope with as a part of established life patterns. The miner. therefore, must adapt to unusual situations and conditions and must develop a mental attitude permitting acceptance of change. 3.1-INDUSTRY-AFFILIATED SAFETY AGENCIES L. S. "Health and Safety problems can make a mineral deposit or a portion of one, an operating plant, or finished product unprofitable or worthless. Therefore they need to be reviewed when reserves are established and yearly thereafter until the deposit is closed down. Health and Safety regulations serve as a guide, but I have always done more frequent checks, when adding new products with different specifications or during times of bad weather and crisis. Water quality and quantity are always in question. Health and Safety are quality of life issues which are at or near the top of priorities for communities, companies, employees, and government bodies. Georgia dropped from one to three in U. S. for business development due to “Quality of Life” issues. Remember crystalline silica is present in nearly all sedimentary and acidic rocks and basic igneous rocks are the perceived home of asbestos. These health issues, any other safety and health issues or perceived issues need to be resolved initially and any time they come up during life of deposit. When handled properly good safety and health programs add to the profitability of a deposit, not to the costs. Daily safety checks in operating plants and mines, and visual check of all drilling core with an eye for changes are a key to successful programs. Both plants I turned around had terrible safety records, too much airborne dust, and maintenance problems resulting in low productivity. In almost every case cost reduction was tied to eliminating maintenance and spillage problems which caused health problems and increased possibility of accidents. Implementation of these safety and health programs around 1970 starting prior to government programs, working with new MSA/MSHA regulations, working to improve the regulations as a president of a state mining organization, and maintain profits was a challenge, but very rewarding. Reducing time to make repairs and work simplification gleaned from observations and employee recommendations learned from making multiple daily inspections resulted in years without lost time accidents at two plants, reduced maintenance and 33-75 % improvement in cost per ton at A. P. Green Refractories Co. Eufaula Calcining Plant 1972-77 and Oil-Dri Ochlocknee plant and mine 1979-82. The Ochlocknee mine worked 1981-2007 without a LTA. The author participated in 16 acquisitions and new plant design projects. All of above reduced costs allowed mining deposits with higher overburden to be used and considered. Thereby increasing reserves considered in all operations."

Mining in China dates back at least 4,000 years. Today, with no less than 103,000 metal/nonmetal mines and between 40,000 to 80,000 coal mines, China’s mining industry is undoubtedly the largest in the world. Despite improvements in recent decades, seemingly endless mining accidents continue to make the news. Official coal mining deaths were 6,702 in 2003, and 6,027 miners died in 2004; actual figures may be much higher. Added to these were the 2,699 deaths in the metal and nonmetal industries. Most of these accidents were due to CH4 (methane gas) explosions and fires as a result of inadequate mine ventilation de-sign or practices. A good portion of these accidents occur in small, privately run and often technically illegal mines. During the past 10 years, the Chinese government has attempted to crack down on small and often technically illegal mines without much success. The number of accidents and associated fatalities has not significantly reduced. However, there appears to be a new urgency in China’s efforts to step up mine safety in the country. After its worst reported mine disaster in more than a half century, the government quickly paid reparations to families, suspended high-level officials in charge and announced additonal reforms. One of those was the elevation of the central government’s work safety office to cabinet-level status. There has been increased transparency in reporting and investigation of mine accidents and disasters in recent years. There has also been an increase in public criticism by labor leaders concerning the neglect of mine workersafety.1 This article examines the causes of China’s many mining accidents, current legal and enforcement structures, respective mining health and safety related laws and regulations, recent mining industry developments and the latest effort to curtail small mining operations.

INTRODUCTION The accomplishment of a well engineered design, and the realization of efficient plant operation cannot be achieved without weaving a web of safety consciousness from the beginning of the project. When the plant becomes operational, and throughout its life, responsibility safety will rest with the client or operator. Recognition of this understanding does not reduce the importance of the engineering and construction contractor's role, but merely puts the responsibility where it belongs. A project which has had little or no operator input during the early stages of design is almost certain to have serious work place safety, health, and environmental problems in spite of the fact that all MSHA and OSHA design requirements were met and the engineering was performed using accepted practices. DEFINITIONS The author has a personal preference in safety definitions that begins and ends with simplicity. If the safety criteria, program, and rules are confusing or complex, the results will soon become obvious. Confusing and complex are the key elements in the avoidance of responsibility and if punitive is added to the list, a brew exists which breeds accidents, illness, and poor performance. A safely designed and operated plant is one that a worker can journey to each day; leaving that person and his or her family with the above average expectation that the person will return healthy, emotionally intact, and hopefully a little wealthier. Longwall mining technological improvements have provided significant increases in productivity over the last 15 years. Longwall productivity is anticipated to continue to escalate with expanded application of automated equipment. Automation technology that will likely be adopted over the next 5 years is memory cut with shearer initiation of shield movement supervised by a couple of face workers. Operations using this technology will most likely relocate many production workers from the face to the outby gate entries and position several face workers outby the shearer, exposing workers to different health and safety risks. These changing risks are primarily related to worker relocation and expected increased production levels. Workers outby the face will most likely have less dust and noise exposure, but will be exposed to other safety hazards associated with outby equipment movement, materials handling, and gate entry ground movements. The face workers will most likely be exposed to higher respirable dust and noise levels generated by the equipment at elevated production rates. Methane gas emission is also expected to increase with faster mining rates. These operational changes and the control technologies needed to reduce redefined longwall worker hazards at semiautonomous longwall operations require examination to maintain the health and safety of mine workers.

Within the last year, the Nation has been shocked by two major tunneling disasters. Almost exactly a year ago 17 men died in the San Fernando Tunnel in an explosion attributed to gas. A few months later another 21 men died in an explosion laid to the same cause at Port Huron, Michigan. The analysis of the causes of these accidents is still continuing. While it appears that they differ substantially from each other in their specifics, they share the common trait that, like most industrial accidents, they might well have been prevented. Gas explosions in tunneling are neither new nor particularly unusual. Forty years ago, Assistant Director Jarrett was hired as a safety engineer on the Hetch Hetchy Aqueduct in California after an explosion that killed 12 men. Prior to 1946, in California alone, the Bureau of Mines had recorded 27 gas explosions incidental to hard rock mining and tunneling. The purpose of this paper is to review the status of the principal regulations affecting tunneling and to indicate some ways in which safer and more healthful conditions can be obtained. The Bureau of Mines was established by an Act of Congress of May 16, 1910. Since that time the Bureau has been making investigations and compiling and publishing health and safety data relating to mining and to tunneling. If any benefit is to come from an anaylsis of tunneling accidents, it must be the knowledge needed to prevent their repetition. The Bureau of Mines has also had informal discussions with the Department of Labor regarding the possibility of the Bureau's assuming that agency's responsibility for tunnel inspections.

Many companies and institutions worldwide have committed to a continuous effort to minimize such occurrences. In this study, the health and safety data pertaining to the Greek mining and quarrying industry for the last 3 decades are presented and critically evaluated with respect to number of workers, annual production, etc. Appropriate indices are also calculated regarding the incidence and frequency of accidents. Health and safety issues specific to mining in Greek are also discussed.

At Kerr-McGee's Galatia Mine in southern Illinois, the need for safe and efficient longwall moves prompted a detailed planning effort. Galatia Mine operates two longwalls on separate coal seams with different geologic conditions. An overview of the longwall move planning as related to manpower training, roof control, ventilation, transportation, equipment recovery and setup will be discussed. The impressive safety record achieved during each of the longwall moves at Galatia Mine verifies that health and safety planning does play a major role.

Regional characteristics in the coal resource base have resulted in application of different mining methods across Canada. Longwall methods predominate in eastern Canada, whereas open pit, hydraulic and room-and-pillar methods are utilized in the west. Both provincial and federal agencies have regulatory responsibilities and both also share in the development of R&D programs. This paper outlines the scale and scope of the various agencies involved with health and safety R&D in our expanding coal mining industry. Specific R&D activities in ventilation, dust control, explosion hazards, tunnel boring, strata control and truck braking, which form part of the national program, are outlined.

Communication was the buzzword for health and safety products at the 2008 MINExpo, and it is still important in 2012. Products unveiled following the Miner Act in 2006 have been upgraded and improved. Saving lives and reducing injuries through better leadership, planning and practice is the goal of every mine professional.

In order to investigate production decline in Tarkwa Goldfields Ltd., located in a tropical rain forest, amongst other areas, a health survey was conducted with the aim to see 1. The health status of the workers. 2. Which parameters carry the most information about the health situation and which of them can be applied easily during screening programmes. 3. What steps could be taken to improve the situation. In all 249 workers were investigated. A non-invasive method involving taking of body weight and arterial blood pressure was used. All subjects were required to answer a questionaire. Emphasis was placed on the history of malaria and diarrhoea within the last six months. Both management and workers co-operated fully. Another set of 110 workers from a neighbouring tyre factory were also investigated for control. Among the underground workers, the percentage body weight loss during a shift increased with decreasing body weight. Dehydration levels could be as much as 4 to 5% of body weight. This could be attributed to the fact that workers took insufficient or no water during the shift. The health status of the mine workers was generally poor and in our opinion malnutrition combined with malaria severely threatens the health and well-being of the workers. Hypertension is also a problem. An improvement in production is intimately linked with the well-being of the workers.

INTRODUCTION An evaluation of the radiological health risks from uranium mining and milling requires the determination of several key parameters. As shown in Figure 1, the first step is the estimation of the radioactive source term. The radionuclides in the ore consist of the 238U decay series nuclides shown in Figure 2, the 235U series nuclides, and 40K. The 238U decay series isotopes dominate the source term, with the 235U series isotopes causing less concern and 40K remaining at background levels. [ ] Specifically, the risks from uranium mining and milling are mainly associated with 230Th, 226Ra, 222Rn, and its short-lived daughters. These radioisotopes are mainly alpha-particle emitters; hence, some dispersion mechanism is necessary to bring them into intimate contact with man so that they can be inhaled or ingested For water pathways, the leaching of the isotopes from [ ] the solid ore or tailings provides a measure of the amount of contaminant initially available to the environment. For the air pathway, dust-creation mechanisms such as blasting, crushing or general resuspension processes dominate for 226Ra and 230Th. However, 222Rn, being a chemically inert gas generated from the decay of 226Ra, can migrate through the ore, overburden and tailings over considerable distances. As shown in Figure 1, the next step in the pathway to man is environmental dilution and delay. For the water transport mechanism, the delay in the isotopes reaching an accessible water supply may be considerable (several thousand years or more), but for airborne nuclides the dispersion in the atmosphere is rapid and constitutes the main mitigating factor. Considerable ventilation is provided in underground mines to dilute and remove radon and its daughters in order to keep their concentrations low and thereby minimize their inhalation by miners. The rate of uptake by both the workers and the general public is often difficult to ascertain. It involves breathing rates, rates of water consumption and other poorly-defined parameters. Since 1978, the Colorado Department of Health has become involved in specific investigations of possible radiation hazards among nonminers in Colorado communities. In each instance, the improper disposal of mill tailings has precipitated concerns and allegations of radiation hazards. This presentation is a brief summary of the findings, to date, of 4 such investigations involving tailings disposal problems in Canon City, Denver, Durango and Grand Junction, Colorado. Canon City In the summer of 1979, allegations of excessive cancer incidence were made at public hearings concerning an application for a Radioactive Materials License submitted to the Colorado Department of Health by the Cotter Corporation for a uranium mill at Canon City. Canon City is the site of a uranium mill operated since 1958 by Commonwealth Edison. Local residents accumulated figures and calculated rates suggesting a 2-fold excess in total cancer mortality. These rates were not adjusted or standardized for demographic varibles. A review of cancer mortality data and computation of age standardized rates for years 1950 through 1977 showed that Canon City's cancer mortality rates were within expected limits, actually lower than rates for the State of Colorado or the United States. The first slide shows rates for every fifth year, 1950 through 1975. Further, an analysis of Canon City cancer incidence data from the Colorado Central Cancer Registry revealed that 1979 incidence rates were not significantly different from Colorado rates, with the exception of prostate cancer. The next slide shows age standardized incidence rates for lung, colon, breast and prostate cancer. Data were also reviewed for leukemia, myeloma, lymphoma and cancers of the thyroid stomach, uterine certix, ovary, kidney, bladder-and brain, however, small numbers of cases prevented meaningful rate calculation. 1980 data are presently being analyzed. None of the data yet examined support allegations of radiation-associated cancer in Canon City. Denver In February of 1979, the Department of Health became aware of a number of radium mill tailing deposits in the Denver metropolitan area, remnants of the radium milling industry of the early 1900's. Several of the deposits were situated so as to possibly contribute significantly to radiation exposure of a small number of people over a period of several decades. The Department developed protocols for radiation surveys, dosage estimates and, for a small number of persons, body burdens determinations and peripheral blood lymphocyte cytogenetic studies. The results of this investigation suggested no measurable biomedical impact as a result of the radium deposits. Durango In October of 1979, a physician residing in Durango, Colorado released information from a preliminary analysis of lung cancer data suggesting an incidence rate several times expected in that city. The data were presented at a meeting of a citizen's group concerned about the possible health hazards of 2 uranium mill tailing deposits located in the south end of the city. Since cancer incidence data generally do not exist in most of western Colorado, a team of epidemiologists was dispatched to Durango to work with local physicians and hospitals in conducting an epidemiologic study of selected cancers for the period 1969 through 1978. After case-finding and record abstracting were completed, it was determined that sufficient data were available for study of only 3 sites: lung cancer, breast cancer and leukemia. The next slide shows age standardized incidence rates for these 3 sites for 1969-1978 for Durango and the State of Colorado. There are no significant differences in these rates. The data did show a geographic peculiarity with regards to the relative proportion of tumor types near the tailings deposits as opposed to away from the deposits, however, this finding is based upon very small numbers of cases and may represent only the random excursion of rates based upon small observations. This investigation suggests that if a carcinogenic hazard is present, it is too small to be detected by the study method employed. Grand Junction The Department of Health has been involved in the Grand Junction mill tailings problem for several years. In 1966, the Department issued an order terminating the practice of free public access to a 55-acre pile of uranium mill tailings. Prior to this order, an estimated 300,000 tons of material was removed. Of this amount, an estimated 50,000 tons was presumably used in residential and commercial construction. Despite many allegations of cancer and birth defects excesses in Grant Junction since the mid 1960s, the first epidemiologic study of cancer was conducted in the spring of 1977. Data from the Colorado Central Cancer Registry were analyzed and reported to the Executive Director of the Department of Health in June of that year. The findings of the first and preliminary study were an unexplained excess of acute leukemia and chronic myelocytic leukemia. The excess was based upon small numbers but was present across all age groups. No increase in chronic lymphocytic leukemia was evident.

This study focuses on the amosite mining region in South Africa and associated health effects, compared to other mined asbestos fiber types. Historically, dust and fiber levels were high in the amosite mills and mines, and many miners and members of the surrounding communities were exposed to the fibers. Research has shown that amosite produces both benign and malignant disease. Nevertheless, the mesotheliomagenic potential of amosite is several fold lower than crocidolite. The risk of disease associated with amosite exposure is difficult to quantify. Reasons for this include the scarcity of available information, including fiber measurements, and case ascertainment, as well as the juxtaposition of the amosite and crocidolite asbestos seams in South Africa. © 2007 Elsevier Inc.
