Many organizations are realizing that continual improvement in health and safety relies on addressing the potential for various forms of human error. Of these, violations are often the most frequent form associated with accidents. These occur when an individual, or individuals, deliberately contravenes established and known safety rules. They have been associated with up to 90% of accidents and incidents in some industries and yet rarely receive the systematic attention they deserve in the promotion of safety. This paper presents underlying motives which cause violations. These can vary significantly from mine to mine and from job to job. Managers need some measure of attitudes to determine the underlying motives behind violations if they are to determine effective strategies for promoting safe behaviour. Questionnaires form the starting point to address attitudes and the motives behind rule violations. The choice of the most suitable questionnaire, however, will vary from organization to organization. Guidelines on the choice of questionnaire are offered.

The closing remarks of a paper which I presented at the annual AIME meeting at Dallas, Texas during February of 1963; contained a forecast of increased activity in trackless underground ore mining which finalized that presentation. At that time, HAD I been peering through my brightest rose colored glasses, I could not have foreseen the unprecedented rapidity of acceleration of underground ore mining methods which has since ensued. From Death Valley, California diagonally to St. Johns, New Foundland; from Australia to Africa to Sweden to France and Germany; from South America to Southern Mexico to Northern Saskatchewan and Northern Manitoba the expansion of trackless underground ore mining methods has probably progressed at a faster rate than any other revolutionary' change in the history of underground ore mining.

Mining has been recognized among the most hazardous industries in the United States, despite the significant reduction in injury and fatality rates over the past century. This study focuses on the ground-fall incidents classified in the Mine Safety and Health Administration (MSHA) database as “fall of roof, back, or brow from in-place” and “fall of face, rib, pillar, side, or highwall.” The main objective of this study is to conduct a comprehensive statistical analysis of the reported ground-fall incidents from 2010 through 2019 including both coal and metal/non-metal mines. Since most of the ground-fall injuries and fatalities occurred in underground coal mines and were attributed to falls of roof and rib, an additional analysis was directed to evaluate roof- and rib-fall incident trends in underground coal mines associated with the mining method, coal-seam thickness, mine size, and the seasonal effects.

The National Institute for Occupational Safety and Health (NIOSH), in partnership with the Mine Safety and Health Administration (MSHA), is conducting research to test, evaluate, improve or modify coal mine fire-fighting strategies and methodologies through large-scale tests. Since wood is the second most abundant fuel available during a coal mine fire, it was decided that series of large-scale wood crib fire tests were needed to measure the products of combustion and to observe the capability of commonly available direct application fire suppression equipment, namely, fire extinguishers, water and gas-enhanced foam. This paper discusses the wood crib fire tests, provides insight into the products of combustion and describes observations made during the application of the fire suppression agents.

Conventional ground support in accordance with latest state-of-the-art considers proper preparation of geotechnical investigation, structural design, observation during construction and fair risk sharing documents. Depending on the ground, there are different means and methods, reflecting different experiences. The paper is presenting ground support concepts, as mostly used in Europe during the past three decades. It discusses the use of anchors as well as the use of steel arches respectively combinations. It also discusses different applications of plain shotcrete, respectively fibre reinforced shotcrete. As excavation sequence is a major input to create the ground arch, the paper discusses also criteria to create closed ring conditions. Finally deformation criteria will be presented with case histories in order to control stability of the structure.

The global tunneling industry has many challenges, both technically and financially, with the latter likely to be a significant feature over the next few years. Many industries have ad-opted standard practices in order to overcome specific technical challenges, from which standard out-comes are anticipated. In tunneling, however, every project is unique so standardized practice is not easy. This requires original thinking and being open to adopting new techniques and technologies as they emerge. Sprayed waterproofing is a hot industry topic and a paper recently presented at the North American Tunneling conference not only discussed this methodology for achieving a dry tunnel but also how it can reduce build costs and construction time. Despite their being relatively few ?universal truths? throughout the tunneling industry, there is one opinion that appears to be shared by most in the sector, ?all tunnels leak.?

Minproc Engineers Inc. (Minproc) recently completed the construction and start-up of a process facility to treat 14,000 t/d of gold-silver ore at Kori Kollo, Bolivia. The comminution circuit consists of crushing and SAG-ball mill grinding. Breakage characteristics of the ore were determined and a circuit model developed. This enabled variations of the circuit to be simulated, and evaluated in terms of predicted performance to provide the criteria for final engineering design. Subsequently, analysis of the operating circuit provided data for circuit model-fitting. The validity of the original model and circuit performance predictions are discussed.

In spite of a global economy that continues to seesaw and a U.S. Congress that is at odds with the president and themselves, cities around the world are still spending billions on their infrastructure. And that bodes well for the underground construction and tunneling industry and the professionals who work in it. In June, 1,405 of those professionals gathered in San Francisco, CA for the Rapid Excavation and Tunneling Conference (RETC). In addition to the first rate technical programming, attendees were able to see first-hand the latest in tunneling technology from 127 exhibitors who occupied 152 booths. Two short courses preceded the conference and two field trips were offered following the conference.

Chemical mining is the in situ extraction of metals from ores located within the confines of a mine (broken or fractured ore, stope fill, caved material, ores in permeable zones) or in dumps, prepared ore heaps, slag heaps, and tailing ponds on the surface. These materials represent an enormous, untapped, potential source of all types of metals. The field of chemical mining, now in its infancy, encompasses the preparation of ore for subsequent in-place leaching, the flow of solutions and ionic species through rock masses and within rock pores, the leaching of minerals with inexpensive and regenerable lixiviants under prevailing conditions of the in-place environment, the generation and regeneration of such solutions, and the recovery of metals or metal compounds from the metal-bearing liquors. It is not inconceivable that eventually our ore reserves will consist largely of low-grade, refractory, and inaccessible new deposits and low-grade zones near previously worked deposits, caved and gob-filled stopes, waste dumps, tailing ponds, and slag-heaps. Chemical mining promises economic recovery from these types of deposits. Before it can come of age, however, a much better understanding is needed concerning its chemical and physical aspects.

Decision theory is the formal theory of the logic of decisions in view of the decision maker's objectives, preferences, I beliefs, and available information, It usually deals with weighty economic problems involving irrevocable (or costly to reverse) decisions. Capital investment planning problems constitute an instance par excellence for application of the decision theory concepts. Decision theory as it is presently conceived has evolved in recent years. At first it concentrated on decisions in the face of total lack of information and on games with intelligent adversaries. The results of these efforts proved theoretically interesting, but practically of little applicability. In the last few years, stress has shifted to the introduction of subjective probabilities and preferences, and the results have been eminently successful in applications both in government and with- in business firms in the resolution of complex problems.

The following is extracted from a report by the author to the International Valuation Standards Committee (IVSC), with minor modifications for the purpose of publication. The author represented the IVSC at the annual session of the Ad Hoc Group of Experts on Harmonization of Fossil Energy and Mineral Resources Terminology. This is a body organized by the United Nations Economic Commission for Europe (UNECE). This second session, held in Geneva, Switzerland, achieved an important agreement for advancement toward uniform global standards for the classification of minerals and petroleum reserves and resources. Attendees discussed the possibility of enhancing the United Nations Framework Classification for Energy and Mineral Resources (UNFC) for implementation globally as the classification standard for use in financial reports under the International Financial Reporting Standards (IFRS). Presently, there are between 100 and 200 mineral and petroleum resource classification (or definition) systems throughout the world — a similar number to what there are countries.

"To more rigorously address tunneling risks to above-ground structures, vulnerability evaluation of all structures along a tunnel route is required. This multi-block area along the route can be considered a district. To fully assess each structure within a tunnel’s zone of influence, a multi-block or district-level model may provide new insights as to risk evaluation and mitigation strategies. However populating such a model with the existing geometry of the built environment poses a major challenge as measured drawings are not readily available for all structures along a tunnel’s route. Cost-effective population of such a model could arguably involve remote sensing data in the form of laser scanning or photogramme-try. However even for unreinforced masonry structures, where external, above-ground geometries can be captured, without a prohibitively expensive building-by-building, in person survey many factors would remain unknown. To consider these uncertainties in an automatic way, a performance assess-ment framework is proposed. Such a framework allows a more rigorous, initial, risk quantification than is currently possible within the simple empirical models generally being used in industry when tunneling risk is initially assessed. This paper introduces (within the allowable space limits of this format) consider-ations for auto-population and application of a district-level, tunnel-risk screening tool. INTRODUCTION Challenging topography, intense urbanization and infrastructure development, combined with limited land availability and a growing awareness of environmental issues have lead to increasing usage of un-derground spaces to control aboveground traffic and provide essential services in metropolitan areas. Tunneling, however, is not without its own risks, especially with respect to aboveground structures whether it be widespread minor damage (Burland et al. 2001) or the more notable, although less fre-quent, large-scale collapses such as the 2003 Shanghai (Allianz 2015) or 2004 Singapore (Magnus et al. 2005) events."

The soils included 65 m of rock not exceeding an unconfined compressive strength (UCS) of 200 kgf/cm2, 130 meters of soft clay, silt, and sand not exceeding 30standard penetration test SPT blows per 300 mm, and 220 m of rock ranging between an UCS of 400-600 kgf/cm2. The river crossing, utilizing 1778 mm OD x 19 mm wall x600 mm steel casing pipe without intermediate jacking stations, was part of a seven kilometer liquefied natural gas (LNG) transmission line in South Korea. This 440-m, 1,442-ft, river crossing was microtunneled through three different ground conditions and completed in just over three months. The crossing was achieved with an average of 25 m of ground cover and 15 m of water cover. The Kwang Yang River Crossing was part of a 7 km liquefied natural gas pipeline project for POSCO, a Korean steel producer. The microtunneling machine was custom built for this project. The microtunneling machine selected for this project was tooled with 7 button bits and 2 roller bits. None of the cutter bits were replaced during the drive. Inside the machine, before the spoils laden slurry was pump back to the surface, it would pass through an independent cone crusher. The crusher is placed in the slurry flow to make sure that none of the spoils are so large as to stop the return flow of the slurry. An additional requirement was for the inclusion of a compressed air lock. The air lock was required in the event that the cutter tools required replacement. The cutters were back loaded so that the workers would have access to change the cutters in the event of cutter failure or wear out. No intermediate jacking stations were installed for the drive. The soils included 65 meters of rock not exceeding an unconfined compressive strength (UCS) of 200 kgf/cm2 with production averaging between 7 and 20 mm/minute. The second zone contained 130 meters of soft clay, silt, and sand not exceeding 30 standard penetration test (SPT) blows per 300 mm with production averaging between 80 and 180 mm/minute. The third zone contained 220 m of rock ranging between an UCS of 400-600 kgf/cm2 with production averaging between 7 and20 mm/minute. The steel casing pipe was 1778 mm OD x 19 mm wall x 6000 mm long. The project profile, see Figure 1, shows the soil conditions. The hand written numbers locate just above the pipeline are the production dates. The circled numbers are the 10 pipe joint increments. As you are all well aware, the minerals processing industry and process industries in general are now heavily involved in reassessing their process flowsheets. The topic of concern is the little arrow in one corner of the drawing at the end of the process flow that usually says "to waste." A really startling statement might have said "to waste disposal area," which implied that there was some concern, thought, or measures taken to deal with the waste. Since 1965, the year of the Water Quality Act, many laws have been passed and agencies formed dealing with the multi-faceted area of pollution. One of these laws, the Air Quality Control Act of 1967, was the initiating force in the development of Calcilox additive to help solve the problem of sludge disposal resulting from the scrubbing of SO2 bearing gases. The intent of this paper is to focus in with a wide-angle view on a particular type of waste stream, generally an aqueous, inorganic slurry, and present the reader with an alternate approach to their disposal via Calcilox additive stabilization. Each waste stream is unique and will require different techniques to meet the design objectives. Calcilox additive is not intended to be a panacea for every slurry disposal problem, but, rather a viable alternative that should be investigated before a final waste disposal approach is chosen. This paper will summarize the development of Calcilox additive; briefly discuss stabilization; and most importantly, discuss some applications to develop a feeling for Calcilox additive stabilization. Actual dollar costs will not be presented, for each waste stream differs and the ultimate disposal mode chosen greatly influences the costs.

A vital aspect of environmental impact assessment for surface mining operations is the determination of potential effects on water resources. This topic has been the subject of much recent concern, controversy and legislation. As increasing data become available regarding the types of hydrologic effects that may be anticipated, methods for safe guarding our water resources can be refined. In view of our current energy situation, the need to develop coal re- sources in a timely and economic manner is apparent. The challenge to the mining industry is to develop these resources in a manner that is also compatible with the beneficial use of water resources in mining regions. For this reason, thorough evaluation of the hydrology of potential mining areas in consideration of proposed mining processes and relevant legal criteria is essential. While pre-mining hydrologic investigations often suggest generalized potential effects, a more reliable method for evaluating specific potential effects is to establish and maintain a hydrologic monitoring program. Such a program must be carefully designed to suit the specific project and its location. This paper describes an approach for selection of criteria for designing a monitoring program for a proposed coal surface mining operation and gasification plant in southwest North Dakota. Woodward-Clyde Consultants (WCC), recently prepared an environ- mental impact report for the ANG Coal Gasification Company (ANG), a subsidiary of the American Natural Gas Company. The mining will be carried out by The North American Coal Corporation (NACCO). Preparation of the report provided the opportunity for the development of a sensitive and economical monitoring program which would function as a baseline hydrological data gathering network as well as act an an early detection system for hydrologic effects of the proposed mining.

In this paper, a methodology for the reliability assessment of geogrid-reinforced unpaved roadways is presented, by taking into account the variability of the subgrade?s undrained shear strength. Analyses are performed using the Point Estimate Method and the Monte Carlo simulations technique. The undrained shear strength is represented by its correlation with the California Bearing Ratio and it is modeled as a random variable following a long-normal distribution. Analyses are performed for different types of reinforcement. The study provides a quantitative assessment of the variation of the system?s probability of failure with this parameter. Overall, the approach suggested in this study may be considered as a complementary approach on the traditional concepts of global or partial safety factors.

North American Coal is a subsidiary of NACCO Industries. Originally founded as Cleveland and Western Coal in 1913, North American Coal took its present name in1925. The Department of Mining Engineering, University of Utah, has developed a thermodynamic-based mine ventilation simulator capable of predicting or analyzing the climatic conditions in hot underground environments. This compressible flow model, besides analyzing an operationally and legally-constrained network for mass and pressure balance, also takes into account various existing and anticipated sources of heat. Presently, three major sources of heat have been incorporated in the model. These include autocompression, wall-rock heat, and heat from equipment. A unique feature of this simulator which makes it distinctly different from its existing predecessors is the mathematical technique utilized to solve the network problem. Advantage has been taken of newer, more sophisticated mathematical and programming techniques to accomplish accelerated convergence and network solution generation. This paper describes in detail the development of the simulator including the mathematical relationships and the techniques used to predict the initial and final thermodynamic states of the mine air after its interaction with heat sources. The algorithm for solution to the network, computer model description, and the input data and program execution requirements are presented. A simple hypothetical network is used as a case study to illustrate the application of this simulator.

Geologically simple deposits can be modeled quickly and easily with inexpensive gridding and mapping software. Hundreds of mines around the world use RockWorks99 and Surfer 7 for generating contour maps, producing log plots and cross sections, and performing other useful geological calculations. These programs are used by many mining operations as their only modeling and graphical software or they are used in conjunction with larger systems. The investment in learning time is low and the programs produce reasonable quality results with standard computing equipment. RockWorks99 and Surfer 7 provide a good level of capability while maintaining high quality technical integrity. Both programs are used for many applications in mining, oil and gas, environmental and other fields that require analysis of spatial data.
