Why discuss techniques of accident prod lot on? Would it not be more useful to discuss means to reduce accidents? This discussion will answer these questions by suggesting ways in which effective accident prediction techniques improve the ability to foresee and prevent accidents. Accident Prediction Process The secret to accident prediction is to utilize the tendency for history to repeat itself. The same circumstances and the same human behavior that produced accidents in the past tend to produce accidents in the present and the future. The ability to predict accident occurence in the future by projecting from the past is a statistical process. In a bookmaker's jargon this means "playing the odds" or "the percentages". A horse or a sports team may be beaten on any given day, but by skillfully assessing the factors of past performance and assigning odds, many clever bettors make money in the long run.

Focus is on the sustainable use of the natural deposits in order to stay competitive. For this reason a raw material policy, both national and international, is necessary to ensure access to new mineable deposits which are still available in Germany. Further on, knowing market conditions is a must for sustainable development. At least sustainability and resource efficiency in potash and rock salt mining are achieved by constantly developing new technologies as well as optimizing the organization and improving qualification of workers."

Gold processing methods have evolved to such an extent that low grade and complex ores once considered uneconomic are now becoming viable. "1. THE VICTORIA STATION TRANSPORT HUB Victoria Underground station is part of the Victoria transport hub, which comprises one of London’s main train termini and a major bus station. At the Underground station, the District Line running in the older shallow brick tunnels dating back to 1868 cross the Victoria Line running in the lower tunnels, which were opened in 1969. After Waterloo, Oxford Circus and King’s Cross/St. Pancras Tube stations, Victoria is the most frequented station on the London Underground network and is used by over 80 million passengers per year. Capacity at Victoria Tube station has been inadequate for many years with passenger numbers steadily rising. Overcrowding is a constant challenge and the station has to be closed regularly during morning and evening peaks until sufficient trains pass through to clear the platforms. For this reason an extension has been debated since the 1990s. After intensive preliminary investigations and feasibility studies covering many variants, which included a large-scale jet grouting test to determine the applicability of this technology as a protective measure for the tunnelling works, the project was officially started with the Victoria Station Transport and Works Act Order (Secretary of State, 2009) effective on the 18th of September 2009. Following a competitive tender process a joint venture of Taylor Woodrow and BAM Nuttall were awarded the contract by London Underground, with their design being carried out by Mott MacDonald Ltd. Ensuring the busy station remained open to the public was a key requirement of the Victoria Station Upgrade. Jet grouting works, which were executed from ground surface, were also carried out while the station was fully operational 2. OVERVIEW The Victoria Station Upgrade aims to increase the capacity of the Underground station significantly and one of its major design objectives is to optimise the transfer between the Underground lines. In order to achieve this, the following new structures are currently under construction: •,A new North Ticket Hall •,Extension of the existing South Ticket Hall •,Nine new escalators •,Connecting tunnels between the North and the South Ticket Hall •,8 new lifts providing step free access to all platforms •,An improved connection between the rail/bus terminus and the Underground station As the elevation and geometry of the existing structures cannot be changed, the new structures need to be adapted to the existing structures (see Figure 1)."

The history of gold and silver analyses shows an interesting duality. On one hand, classical chemical and instrumental methods have evolved to meet changing needs, but on the other hand, the age-old and somewhat empirical fire assay has remained a popular tool. Inasmuch as gold and silver generally have been present in ores at low concentra¬tions, methods have been challenged by a need for sensitivity and selectivity coupled with reasonable assay time. A progressive increase in the importance of low grade materials as ore sources has led to the need for improved methods. Assays of these materials are required for process control and accounting in operating plants as well as for guiding research and development activities. The analytical response to these needs is evidenced by numerous reviews published through 1977-¬e.g., cf Kolthoff and Elving (1966), Beamish and Van Loon (1966, 1972, and 1977), Young (1971) , Mallett (1970), and Sen Gupta (1973). In this paper we present a brief sketch of the history and status of various gold and silver methods. A more detailed summary of atomic absorption spectroscopy (AAS) is Given, as this method has gained in popularity more than other assay methods. AAS methods currently in use are summarized and discussed briefly. This paper is not meant to be a detailed review, as are the above mentioned publications. Rather, we borrow from those works and more recent papers to report trends in gold and silver analyses (particularly in AAS). Also, we discuss some selected poten¬tial problems, and problems that have been experienced by this laboratory.

A big issue in crushing and grinding operations in raw materials handling is high energy consumption and low breakage effectiveness. According to the Legendre [7], only 10% of energy input is directly consumed on the feed material breakage, while the remaining portion is dissipated in the form of heat and noise [8]. In the industry of limestone powder production, the grinding operations constitute integral part of technological circuits, and the energy consumption in operations of industrial comminution consume as high as 2% of world energy [14]. High-pressure grinding is the technology with proved energy benefits both in ore processing circuits and in limestone grinding. The promising results concerned mostly the achieved energy savings and good results of material breakage were achieved and demonstrated in numerous investigations [1,3,12]. However, it appears that a proper selection of operational parameters of the press devise, determination the feed material characteristics and adjustment of process course san be of a key-significance in improving an effectiveness the HPGR operation. Operational parameters of devices and process course should be verified towards the characteristics of the feed material, which mainly applies to the particle size and shape of the feed material and its moisture, and also mineralization in case of ores [2,4,9]. There are various investigations concerning an impact of the operational pressing force on comminution results which mostly refer to the determination of energy consumption and crushing ratio [5,6,10,13]. Investigations on the moisture content are less common, however their results suggest that, in general, the HPGR feed with higher moisture shows the tendency to higher comminution intensity, while the energy consumption increases together with increasing of feed material moisture. Investigations on olivine sand feed [11] showed that crushing ratio decreases for higher moisture, especially when the material is crushed at higher pressing force. HPGR devices, however, shouldn’t be regarded as a unique and perfect solution for comminution of each type of material. But they indeed may bring technological and economic benefits, comparing to conventional crushing and grinding machines, resulting in more favourable breakage of material particles and lower consumption of energy per unit mass of processed feed material.

Three years ago, the New Britannia Mine started a project to improve productivity of its cyanidation plant. Results indicated that gold dissolution was strongly influenced by sulphide minerals and that the slower gold dissolution rate had a detrimental effect on overall gold recovery. The leaching strategy had to be modified to address this issue. Changes to the leaching circuit proved to be very successful. The first action was to introduce lead nitrate in the leaching circuit. The strategy of addition of lead nitrate was upgraded in three steps. An on-line free cyanide analyser was installed and the strategy of cyanide addition automated. The enhancement of the leaching kinetics have resulted: an 4% increase of gold recovery, from 87.8% in 1996 to 91.9% in 1999, which represents a reduction of 0.17 git Au in the leach residue; an average of 1330 additional extracted gold ounces per year from 1997 to 1999; a 21 % reduction in the cyanide consumption; the shut down of the cyanide destruction plant; and finally a contribution to the 27% reduction in the overall production cost per tonne of ore.($ 3 per ton). The significant increase of the throughput was instrumental in the reduction of the processing cost. In 1999, the improvement of gold recovery represented an additional gross revenue of $c""l.9 Million. The project is discussed in details in this paper.

"The mining environment presents challenging and complex situations for the application of medical care. The combination of remote locations, difficult access and longer response times may create an increase in morbidity and mortality in the event of serious accidents. The U.S Mine Safety and Health Administration (MSHA) has only minimal requirements of first aid training for mine workers. For mine rescue team members, MSHA requires medical training at the Emergency Medical Responder (EMR) level, as only prescribed by mine rescue contest rules. This paper examines, based on mine accident statistical information, whether a higher level of medical training, for example at the Emergency Medical Technician (EMT) or at the Advanced Life Support (ALS) level can better address injuries most commonly disabling or leading to the death of miners in the United States. BACKGROUND Prehospital emergency medical care for the sick and injured has evolved considerably from first inception. Ambulances used to be staffed by persons with little training and equipment and prehospital care was focused largely on the rapid transport of the injured person to a hospital and. The modern prehospital care model was initially developed in military applications, where medics were responsible for some treatment on the battlefield and rapid transport to the care of a physician. In 1965, the National Academy of Sciences and National Research Council Committees on Trauma and Shock published Accidental Death and Disability: The Neglected Disease of Modern Society (National Research Council Committees on Trauma and Shock, 1966). This white paper laid out the consequences of the insufficiency of the Emergency Medical System (EMS) in the United States at that time. According to this paper, in 1965, 107,000 people were killed from accidental injury, with another 400,000 injured with permanent disability. The authors estimated that approximately 50% of ambulance services were provided by local morticians that were not capable of rendering advanced life support. The paper states that, based on experiences in the Vietnam war, seriously wounded soldiers had a better chance of survival in the battle field compared to traffic accident victims with similar traumatic injuries because the military was capable of providing first aid and efficient transportation to emergency care facilities. The white paper states that death rates from battle injuries declined significantly from 8% in World War I to 2% in Vietnam. The publication of this white paper is credited for the broad evolution and rapid advance of the civilian EMS system after 1965. It eventually lead to the creation of the National Highway Transportation Safety Administration (NHTSA) National Standard Curricula (NSC) for emergency medical care in 1985. Training, and therefore required skill and knowledge for prehospital emergency care has continued to grow to reach the current standard."

The Atlantic City iron mine is located in Fremont County in central Wyoming, near the historic gold mining camps of Atlantic City and South Pass City. The mine lies on the southeast flank of the Wind River Mountains at an elevation of about 2,470 m (8,100 ft) above sea level (Fig. 1). An 81-h M2 (200-acre) tailings impoundment at the Atlantic City taconite operation has been a source of blowing dust. The site qualified for reclamation under Wyoming&apos;s Abandoned Mine Land (AML) reclamation program. The ultimate reclamation construction incorporated commercially available organic amendments and fertilizers into a 300-mm- (12-in.-) thick cap of a sterile clay loam cover material. The water level was lowered in an adjacent flooded mine pit that was feeding uncontrolled seeps into the tailings basin. A wide tailings pond spillway was built that allows flood control while minimizing seasonal water-level fluctuations in the pond. Rock berms and rock snow fences were built for wind-erosion control and soil-moisture preservation. Future work includes tree planting in the protective wind/ snow shadows of rock berms and rock snow fences and plugging a decant pipe. US Steel opened the Atlantic City iron mine in August 1962. It closed in 1983. Taconite iron ore was mined by surface methods. The ore was magnetically separated from the waste. The magnetic concentrate was mixed with bentonite, formed into pellets and roasted to produce a blast furnace feed stock for US Steel&apos;s blast furnaces in Geneva, UT. The pellets were hauled by rail to the Geneva Iron Works. The Atlantic City Mine produced three types of wastes. These included waste rock overburden from mining, a coarse-grained (sand to small-gravel sized), nonmagnetic reject rock termed "cobbs" and fine-grained tailings ranging from 600 to 75 µm (28 to <200 mesh). The mining/mineral processing industry constantly faces the challenges of handling tons of material in a variety of processes such as crushing, milling, drying, belt transfer and loadout. These processes result in the production of high volumes of dust. A variety of dust detection and collection equipment is needed to properly manage these emissions to remain compliant with stringent air quality and safety requirements. Dust monitoring and bag leak detection can provide ways to reduce air pollutants, make work environments safer and remain compliant with requirements from the U.S. Environmental Protection Agency (EPA), U.S. Mine Safety and Health Administration (MSHA) and U.S. Occupational Safety and Health Administration (OSHA). There are several approaches to monitoring particulate emissions from dust collection equipment. These can include visual inspections, differential pressure gauges or bag leak detection systems. Bag leak detection and monitoring systems can be optical, light based or electrostatic/triboelectric. Recent developments have advanced the efficiency and reliability of electrostatic/triboelectric style monitors. New technological improvements have been developed that are based upon the principal of unifying dc (impaction) and ac (induction) signals and incorporating other proprietary circuit innovations. This higher signal resolution technology results in a more robust and stable measurement for detecting the presence of particulate in a dust collector emission duct or a flow process pipe.

Of all the fossil fuels produced in the U. S., coal is by far the most abundant. U. S. reserves amount to an estimated 1.5 trillion tons, 2,500 times the 600 million tons produced in 1970. Although it would not be possible to recover all of this coal economically with current mining technology, a significant portion would be available given the proper circumstances. With existing technology, approximately one-eighth of these reserves is accessible, an amount which would last for several hundred years at the 1970 level of production. On the face of it, this substantial reserve should go a long way in meeting the Nation&apos;s energy needs. But coal offers special problems. As additional coal is mined and then burned, there are increased costs to the Nation&apos;s land, water, skies, and health. Hence, coal poses a dilemma because of its dual status as one of our most abundant domestic fuels and as one of our major sources of environmental degradation. Against the benefits that would be chalked up in the areas of national security, balance of trade, and economic well-being from the increased use of coal, we must enter on the debit side of the ledger the increased environmental cost. This paper reviews the use of U.S. fossil fuel, the drawbacks of coal as an energy source, the means being developed to overcome these drawbacks, and the background behind the present sulfur emission regulations relative to coal combustion. It also discusses in detail the Federal Environmental Protection Agency&apos;s (EPA&apos;s) current policies to ensure the continued use of coal as a major source of energy for fossil-fuel fired power plants.

Smart defense: that?s what the industry needs to play for the next four years, according to panelists at the 118th Annual Meeting of the Northwest Mining Association (NWMA), which met December 2-7 in Spokane, WA. The conference was well attended, with approximately 2,200 attendees, 244 booths in the two packed exhibition halls, and a rich offering of short courses, field trips, social events and technical sessions. Speakers at the packed legislative session articulated many of the frustrations felt by members of the mining community after the U.S. elections in November, and offered words of advice. Matthew Ellsworth, the new Government Affairs hire at NWMA, stated it best: ?Elections are like Christmas: you find out who?s naughty and nice ? and it?s always going to come again.?

The trend towards proportionately rising volumes of surface mining through- out the world and especially in the United States has led some observers to predict that the future of underground mining is quite limited. Certainly, the simple extrapolation of current trends1 gives evidence of a compelling sort for the hypothesis that technological progress is proceeding at a faster pace in surface than in underground mining. This paper attempts to examine the basis for these predictions by searching the historical record on differential rates of productivity as these are observed in iron ore and coal mining. In recent years economists have discussed "technology gaps" among industries or segments of the same industry, by which they mean that mechanisms for invention and its diffusion are faulty or retarded in such a way that feasible cost-cutting innovations are not applied or developed, even though technicians regard them as practical.2

This paper presents an engineering approach to solve a complex ground control problem at reduced cost, allowing the mine to be operated safely and economically. The mine has experienced serious and complicated cutter roof problem in the entries. This cutler roof problem has delayed the advance of the entry development considerably and cost of these entries is very high due to the requirement of very heavy artificial supports. The analysis includes assessment of the cause of cutter roof and step by step action taken to improve the entry condition through appropriate longwall panel entry design and roof support system. The approach for solving the problem was two-fold: 1) to apply yield pillar concept in order to reduce the effects of in-situ horizontal stress in the roof strata and 2) to find an effective way to reinforce the roof strata and to support the fractured roof and avoid caving.

"Feldspar may represent 60% of the earth’s crust, but the mineral does not exist in a pure state. It is generally associated with quartz, iron containing minerals such as mica, and titanium containing minerals such as rutile. Feldspar itself can even exist in different lattices from those containing sodium to various divalent ions. As such, its beneficiation has become a process that is quite complicated, when compared to the beneficiation of other minerals. It is this complexity that has made feldspar beneficiation dependent on tailor made reagents to successfully yield the industry specified feldspar products. This paper reviews the mineralogical variations found from two feldspar mines and the development of collectors to specifically target a Fe+Ti concentration of < 0.05%, while maintaining a feldspar recovery greater than 90%.INTRODUCTION Feldspar is a class of aluminum silicates that represents the most abundant mineral on earth (Vidyadhar, 2002). This class of mineral can further be defined by the types or concentrations of cations present in the crystal lattice, MAlSi3O8 (M = K, orthoclase and microcline; M = Na, Albite; M = Ca, Anorthite) (Heyes, 2012). Feldspar is most commonly mined and processed for either the alumina used in glass making or for the alkalis used in ceramics. The alumina is known to improve product hardness, durability, and resistance to chemical corrosion. The alkalis act as flux, which means they not only lower the melting temperature of the mineral mixture, but also assist in connecting the other components present in ceramics. It was stated in 2009 that the world production of feldspar was in excess of 18.3 million tons produced, with the majority coming from China, Italy, and Turkey. Raw feldspar is typically found with quartz, iron containing minerals such as mica, titanium containing minerals such as rutile, and other feldspar minerals (Celik, 2001). Quartz is stated as having a negative effect on the melting point of the feldspar, whilst the iron and titanium based mineral contaminants are known to have negative effects on color and quality of the product produced; these mineral contaminants must therefore be separated prior to application (Barns, 1997 & Celik, 2001). Mineral flotation and magnetic separation are the most common means of feldspar beneficiation. However, conditions vary greatly from mineral contaminant to mineral contaminant (Table 1). Some literature suggests feldspar can be floated with an amine, but this generally yields lower than acceptable recoveries and often still requires the magnetic separation of iron based minerals to achieve the desired grade (Amarante, 1997 & Heyes, 2012). Feldspar flotation can be further improved with the addition of hydrofluoric acid (HF) as a silica depressant and feldspar surface modifier, but this still requires the removal of some of the other minerals first. Rutile can be floated with an anionic fatty acid collector at a pH range of 7-8 or with a petroleum sulfonate at a pH < 4 (Celik, 2001), while mica can be floated with a petroleum sulfonate or an amine collector at a pH of 3.5."

The worldwide budget for non fuel mineral exploration was expected to increase by 27 percent in 2003 from the 2002 budget, according to the Metals Economics Group (MEG) of Halifax, Nova Scotia. The increase comes after five years of declining spending for mineral exploration. For the first time in several years, all regional exploration budget estimates for 2003 as defined by MEG would increase from similar 2002 estimates. However, MEG included 193 more companies in its 2003 survey than in 2002. As with previous MEG survey reports, estimates reflect anticipated expenditures rather than actual dollars spent. Caution, therefore, must be taken when comparing 2002 and2003 estimates or evaluating the magnitude of regional changes from year to year. Sample variations and currency exchange rates may influence year-to-year variations of MEG data. Even so, it is likely that the general climate of improved metal prices in 2003 encouraged companies that had previously left the mineral exploration sector or reduced their exploration activity to once again focus on minerals exploration or increase exploration activity and budgets.

Exposure to whole-body vibration has been recognised as an occupational hazard in the mining industry, especially in the operations of earthmoving equipment. Managing whole-body vibration exposures requires periodic measurement of exposure levels to identify priority areas for implementing controls. However, due to different approaches to collecting exposure values, significant variabilities lie in the results. This systematic review is the first attempt to analyse whole-body assessment methods in the mining industry. The aim was to identify how whole-body vibration has been assessed, including the sources of variabilities to improve future research. The PRISMA methodology was adopted for the review and a total of 152 peer-reviewed journal articles were identified. However, only 24 were included in the review, following the application of some inclusion criteria. Descriptive and thematic analyses were performed on the 24 selected articles. Results indicate that whole-body vibration has been assessed as a function of either equipment characteristics, activity undertaken, operator characteristics, measurement approach or the assessment standards. Due to the multifactorial and dynamic nature of wholebody vibration exposures, the variabilities in results are due to the interrelationships between the risk factors of whole-body vibration and the differences in the sample sizes. To identify the sources of variabilities, a comprehensive assessment of all the risk factors, including equipment characteristics, road conditions, operator characteristics, and activity undertaken, is highly warranted. Finally, research gaps and directions for future research have also been discussed.

As resources are depleted in coal mines, isolated support pillars are often mined. Cemented paste backfill has been used to provide support during the mining of these pillars due to its safe and green mining characteristics. However, traditional cemented paste backfill mining has some shortcomings, such as poor adaptability to geological structure, leakage of paste during filling, imperfect roof-contact in partially backfilled areas, and insufficient roof strata support before the backfill reaches its design strength. This paper presents the results of a case study where L-shaped interval cemented paste backfill technology has been safely used to mine an isolated coal panel beneath existing surface structures. This backfilling method has the advantages of preventing paste leakage, improving the backfilled ratio, reducing surface subsidence, and strongly adapting to geological structure. During this field application, the setting pressure of the backfill shield reached 20 MPa, the compressive strength of the backfilling body reached 2.0 MPa, and the backfilled ratio reached 93.7%. Observational ground monitoring showed that roof-to-floor convergence in adjacent roadways ranged from 132 to 167 mm. No obvious fractures or separations in the overlying strata were observed by a borescope. The maximum surface subsidence was 342 mm, the subsidence factor was 0.136, and the damage to surface buildings was within Grade I (slight damage). This case study clearly demonstrates that the L-shaped interval cemented paste backfill method can be safely used to mine the isolated coal panel beneath surface buildings.

Despite technological advancements and organizational adjustments, lost time accidents are major issues in occupational safety. However, there is very limited work that focuses on variables influencing days lost as a result of occupational accidents. In this study, decision tree and artificial neural network methods were used as machine learning techniques to investigate the impact of factors on accident lost day duration. Degree of injury, worker age, and worker activity were found to be the top three variables impacting loss of time from work. It was also identified that the mining method, location, and nature of injury had a moderate influence on duration lost due to occupational accidents. However, worker experience and ore type did not have any significant impact on the duration, which is an unexpected result. These results confirmed that some accident factors that might have a large influence on the number of mine accidents can be less critical when it comes to accident lost day duration.

Metallurgists are trained to seek synergisms and to optimize trade-offs in the selection of processes and flowsheets. The subject of my paper and the focus of this entire conference is on synergisms involving more than just metallurgical recovery, but encompassing mining and environmental management too. Operators have always known that by recycling tailings water they could recover some cyanide and lime values or reduce the amount of new flotation reagent additions. The trade-offs have changed at many US mines today where concern about avian mortality necessitates that water in the tailings pond be detoxified. This is just one example of how selection of processes and technology today requires integration of environmental management. MUDDER & GOLDSTONE (1989) showed how cyanide can be recovered from tailings slurries by an acidification­volatilization-reabsorption (A VR) process which was installed in New Zealand, and which in 1992 is being commissioned also at Nerco DeLamar Silver Mine as described by OMOFOMA & HAMPDEN (1992). FRICKER (1990) and FRICKER & OWERS (1992) show how using A VR to recover cyanide from pregnant pulps ahead of gold recovery improves the yield of recoverable cyanide while improving the performance of the gold recovery step and detoxifying the tailings. This is an excellent example of the synergisms that can be achieved ,y applying a "systems approach."
