Safety departments have traditionally been held responsible for the safety performance of their division or company. This approach leads to a lack of ownership of safety issues by the larger organization. An alternative paradigm is significantly more effective: the members of the work force (managers as well as workers) are fully responsible and accountable for safe production. The safety department is not extinct, however. Its role has changed, to focus on training, litigation, and external relationships such as community relations. This approach was implemented at BHP Copper's Superior mine and mill in Arizona. By obtaining the genuine commitment of the work force to accident-free production, the mill's MSHA incident rate immediately dropped from 7.4 to zero. The mill also ran one year with no MSHA citations. When this philosophy was implemented at the divisional level, the division's incident rate decreased from 9.5 to 1.1,an 88-percent reduction in six months.

The technology for using limestone or lime to scrub SO2 from power-plant flue gas has significant implications to the limestone industry. A brief status report on these processes will be made, possible process requirements on the limestone properties will be reviewed, and projections on the buildup of demand with respect to time will be reviewed. Disposal problems inherent with the use of these throwaway processes will also be summarized. Given a viable technology, relations among (1) distance of power plants from low- and high-sulfur coal and limestone deposits, (2) modes of handling and transporting immense tonnages of bulk material-coal and/or limestone, and (3) regional air-quality standards are some of the factors which will ultimately determine the demand for limestone. A discussion of these factors will be presented, with the intent of identifying geographic regions where significant increases in production can reasonably be expected.

"INTRODUCTION The Butte district of Montana is well-known as one of the biggest historic producers of copper in the world. It is also a classic example of a porphyry Cu-Mo system that has been overprinted by zoned, polymetallic (Cu-Zn-Pb-Ag) lode mineralization. After copper, silver was the next most important metal mined from Butte, with over 700 million ounces of total Ag production between 1880 and the present day. Although many papers have been written on the mineralogy of the early porphyry Cu-Mo mineralization and the high-grade Cu-rich lodes in the center of the district, relatively little information exists on the mineralogy of silver in Butte. The purpose of this paper is to summarize past and present silver production trends for the district, and to combine previously published work with new research to examine the mineralogy of silver in the Butte mining district. BUTTE HISTORIC SILVER PRODUCTION According to a recent USGS compilation1, Butte is one of the most Ag-rich porphyry deposits in the world, both in terms of average Ag grade (8.6 g/ton) and total production + reserves (Fig. 1). It is also the 2nd biggest Ag-producing mining district in the U.S. (after the Couer d’Alene district, Idaho). Annual silver and copper production data for the Butte district, compiled from various sources2,3,4, are summarized in Figure 2. Peak production of Ag and Cu was in 1917. While Ag production gradually declined in the ensuing decades, Cu production remained strong until the years leading up to the closure of the Berkeley Pit in 1982. In 1984, mining of the nearby Continental Pit resumed under a different operator, and continues to the present day. The Continental mine produced an average of 35,000 metric tons Cu and 575,000 oz Ag per year between 1984 and 20044. These figures do not take into account a period of mine closure in 2001-2003 due to an unexpected hike in utility prices. The long-term decline in Ag/Cu ratio of ore mined in Butte (Figure 3) is due to several factors, including: 1) changes in metal prices; 2) a downwards decrease in Ag/Cu ratio of most of the lodes with depth below surface; and 3) a shift in mining methods from selective underground mining of high-grade veins to open-pit mining of the bulk rock."

The objectives of this paper are: to review the current status of chloride leach processes for sulfide concentrates, to make an evaluation as to the applicability of chloride leaching for replacing pyrometallurgical processes and to comment on recent advances in construction materials. The paper does not delve in detail on process chemistry. Sufficient references are presented on each process to allow the interested researcher to obtain process chemistry details. GENERAL The fact that chloride oxidates can solubilize base metals from sulfide concentrates has been known since the 1880's. The work of Hoepfner in 1902 was particularly accurate for its time with regard to the chemistry of chloride solutions. Recent work has dealt with the understanding of chloride extraction thermodynamics by modelling the behavior of metal chloride complexes. In the last several years several excellent review articles and symposiums have been issued covering chloride chemistry (1 thru 6).

The largest hard rock TBM ever to bore in the USA, an 11.6 m diameter Robbins Main Beam TBM, recently underwent a planned in-tunnel diameter change to a more compact 9.9 meters. The first-of-itskind conversion process for the Main Beam TBM was undertaken 2.8 km into the bore and was not done inside a shaft or pre-excavated portal. This paper will detail the unique dual-diameter machine designed for the Mill Creek Drainage Relief Tunnel in Dallas, Texas, USA, machine performance, and successful size conversion process that took place in early 2021—a process that utilized the four C’s of construction to enormous benefit: Communication, Cooperation, Collaboration and Coordination.

During the past fifteen years considerable progress has been made in the technology of slope design. It is possible to design rational slope angles if suitable data on geology, hydrology, and rock properties are available. It is of course preferable to conduct a full scale slope design study, but there are those situations where a full scale study is not feasible because of time and budget constraints. In this paper we will treat two situations: (1) choosing slope angles for a preliminary feasibility study at the exploration stage; and (2) dealing with a slope failure in an operating pit. In these instances slope angles must often be chosen on the basis of available or readily obtained data. In an exploration program all too often a feasibility study is initiates just a few months prior to the decision deadline to drop the property or to exercise an expensive purchase option. If all that is available at this point are a few cross sections at odd angles showing ore and waste contacts and a dog-earred map of the surface geology there may not be time to collect and organize enough information for design of slope angles. On the other hand if during the drilling program geologic cross sections are constructed and the structural data is presented in tabular form, rational slope angles can be determined within the allotted time. Certain types of data can be collected only at specific times during a project. Two prime examples are logging fracture intensity and collecting samples for rock testing before the core is split. It is particularly disheartening to ask to see the core and to be shown a tattered cardboard box filled with plastic bags of sample pulps.

There is general agreement among those involved in occupational and public health that work-related diseases are under-diagnosed and under-reported. Highmark is particularly well positioned to carry out industry health analysis as it provides medical insurance for approximately three quarters of the insured population of Western Pennsylvania including some of the areas largest employers. The purpose of this study is to gain a more accurate understanding of the mining industry rates of respiratory diseases that maybe occupationally related and identify those inflated respiratory disease and hearing loss rates. Conditions examined included COPD, asthma, and pneumoconiosis.

Fly ash as an additive for oil well cement was introduced to the oil industry by the Halliburton Company in 1949. Since its early usage in the Illinois area of the U. S., the total amount of fly ash pumped into wells has exceeded 30 million sacks, The advantages of using fly ash-portland cement mixtures in wells are fundamentally the same as for concrete except the conditions of placement and curing are variable and the nomenclature of properties are expressed somewhat differently. In some instances deep wells have been cemented with mixtures of fly ash and hydrated lime together with an activator which functions as a catalyst for setting. This fly ash-lime formula has many advantages for high temperature-pressure conditions found in wells.

A limestone mine in Ohio has had roof instability problems that have led to massive roof falls extending to the surface. This paper describes a collaborative study of these events undertaken by the National Institute for Occupational Safety and Health and the mine owners. The study focuses on the roles that weak, moisture-sensitive floor and weak bands in pillars play in a general instability that led to roof falls at this mine. The NIOSH Stone Mine Pillar Design software (S-Pillar) does not include analysis for weak floor or weak bands and recommends that when such issues arise they should be investigated further using a more advanced analysis. It is important to recognize that without the impact of the floor or weak bands, the calculated inherent stability factor for these pillars is within the recommended design range. Therefore, to further investigate the observed instability occurring on a large scale at the Ohio mine, FLAC3D numerical models were employed to help determine the failure mechanism. Roof-to-floor convergence monitors were also installed in the areas predicted to have further instability. This case study will provide important information to limestone mine operators regarding the impact of weak floor causing the potential for roof collapse, pillar failure, and subsequent subsidence of the ground surface. This recent case demonstrates that weak floor can be a significant factor in pillar stability and should be assessed during the design stage of the mine.

"Steel Fibre Reinforced Concrete (SFRC) Segmental Tunnel Linings have successfully been used for over 20 years in the UK and have been adopted in a number of high profile projects such as High Speed 1, Crossrail, Thames Tideway, as well as High Speed 2. Tunnel linings and especially SFRC design are not covered in the Eurocodes. This shortage of SFRC tunnel design rules seems to be partially addressed by several international publications. However, in the UK, the ‘design-assisted by testing’ (DAT) approach has also been adopted. The Paper discusses the UK’s best design practice and focuses on the DAT methodology as applied in the case of the 6.9km (4.3miles), 7.8m (25.6 feet) ID Lining for the £635million Lee Tunnel Project. The design approach, the full scale testing at the Building Research Establishment (BRE), and the construction innovations are presented. This experience will be transferred to the £4billion Thames Tideway Tunnel.INTRODUCTIONSteel Fibre Reinforced Concrete (SFRC) has been used for Tunnel segments for more than 20 years. The first application of SFRC in the UK was for the Heathrow Baggage tunnel (1994). Since then significant size tunnels have been designed and constructed, such as the Jubilee Line (1999) High Speed 1 (CTRL, 2006), Crossrail (2014), Lee Tunnel (2014) and the scheduled 25km Thames Tideway (‘Super Sewer’) and High Speed 2 rail tunnels. However one the main obstacles in achieving streamlined design and improving efficiency are the lack of appropriate design guidelines for SFRC tunnel segments. This issue will – to some extent – be addressed by a number of current documents on tunnel FRC design either being produced by BSi (PAS8810, expected 2016), ACI (544.R) and ITAtech (Design Guideline PCFRC, 2015) in an effort to advance the design of SFRC and take into consideration the recent developments in Steel and Macro-Synthetic FRC technology."

INTRODUCTION Production of chrysotile asbestos from the northern Appalachians has been going on for 100 years and, on the basis of known reserves and the present rates of mining, appears assured of continued production for at least another 25 to 30 years. In spite of this, mining companies and government agencies are showing an increasing interest in exploring for chrysotile asbestos throughout this region and elsewhere in the world, implying that increases in the rate of consumption are envisioned. This chapter describes the geologic and tectonic settings surrounding these important chrysotile deposits, with emphasis on those in southern Quebec that yield about 90% of the total production from the northern Appala- chians. Detailed descriptions of a few individual deposits are also given, along with exploration guidelines aimed at facilitating the discovery of additional chrysotile deposits. One section of this chapter deals with the origin and mode of formation of the chrysotile deposits in the light of modern day concepts of plate tectonics. The objectives of this chapter are to give a brief description of the geologic and tectonic settings of the ultramafic rocks in which the numerous chrysotile deposits of the northern Appalachians are found; to describe a few of the individual deposits; to give a few exploration guidelines for those interested in exploring for new asbestos deposits in similar geologic environments; and to propose a new model for asbestos genesis in light of the recently developed theories of global tectonics. As this chapter is based almost entirely on field and laboratory observations, rather than on the study of experimental laboratory models, most of our deductions and suggestions are empirical in nature and may be interpreted in more than one way. The interpretations we have selected seem to offer, we feel, the best explanation of observed geological phenomena (Lamarche and Wicks, 1975). This region had a production capability in 1979 of close to p.5 Mt of chrysotile asbestos with 92% of this tonnage being derived from the deposits of the Eastern Townships of Quebec. With known resources, which now stand at between 36 and 40 Mt of fibers, this production capability seems assured for the next few decades. In spite of all known reserves of chrysotile asbestos, which now stand at between 36 and 40 Mt of fiber in this region alone, mining companies and government agencies are still showing a good deal of interest in exploring for new sources throughout the world. GEOLOGICAL SETTING The asbestos deposits at Baie Verte in eastern Newfoundland, Thetford Mines, Black Lake, Asbestos in southern Quebec, and Belvidere Mountain in Vermont, all occur in partly serpentinized ultramafic rocks that are part of the Lower Paleozoic ophiolite complexes of the northern Appalachians [Cady, Albee, Chidester, 1963, Chidester, Albee, Cady, 1978, Lamarche, 1972,1973, Laurent, 1973,1975, Williams, Hibbard, Bursnall, 1977, Norman and Strong, 1975). The deposits to the northeast of Thetford Mines, on the other hand, are found in a more highly serpentinized ultramafic body known as the Pennington dike, whose spatial, genetic, and chronological links with the ophiolite masses proper have not as yet been adequately resolved. The Appalachian ophiolites are part of an ultramafic belt (or belts), that extends from northwestern Newfoundland, through Quebec and Vermont. Fig. 1, and southwestward as far as the

Safety management can best be described as how each company deals with the control of hazards (human or mechanical) to reduce costly accidents and employee injury caused by accidents. Properly used, effective safety management can mean the difference between being a highly productive, cost-efficient operation or one that "walks the fence," that is less productive with higher product costs. At Utah Power & Light Co's mining division, effective safety management has been used as tool to achieve this difference and remains as the number one priority to maintaining that difference.

The Karst asbestos deposit, discovered seventy years ago, is 32 miles south of Bozeman in southwestern Montana. The deposit is about one-half mile west of the Gallatin River which flows in a deep V-shaped valley flanked on either side by rugged mountains. The Madison range is west of the river and the Gallatin range lies to the east. A busy all-weather highway, U.S. No. 191, follows the river and is the major north-south route between Bozeman and the Gallatin valley to the north and Idaho and the west entrance of Yellowstone National Park to the south. The asbestos deposit is a large body of serpentinized peridotite in which anthophy11ite.occurs, both as distinct veins and as a disseminated mineral in the peridotite mass. The known exposures of the asbestos-bearing rock occur on a steen south slope at an "elevation of about 7000 feet, an elevation 'which is about 1200 feet above the Gallatin River. Over the years, a number of small companies have attempted to exploit the Karst asbestos deposit without continuing success. The present operator, Galaxy Mining Corporation, has attempted a systematic approach to the problem by utilizing geologic and engineering surveys to determine the economic and technical feasibility of a mine and plant to produce a finished asbestos pro-duct. The results of this work indicate a high probability of success for the project. This paper presents an analytical technique for prediction of the damaged rock zone (DRZ) around a circular drift subjected to an anisotropic stress field (analytical solution exists only for the case of hydrostatic stress field). The method can readily be adapted to micro or personal computers. Stress concentrations around the drift are calculated at the centers of small rock elements, using the analytical solutions for a hole in an infinite plate. Potential failure of a rock element is assumed to be governed by the Coulomb failure criterion. When the failure condition is exceeded for an element, the excess shear stress is transferred to the neighboring elements. Progressive failure is allowed to occur until convergence is obtained. The final configuration of the damaged rock zone so obtained can be used to assess rock support requirements, as well as modified modulus and permeability of the DRZ.

Production scheduling of an open-pit mine is one of the most important steps in open-pit mining. The production schedule of a mine defines the distribution of cash flows and as a result directly influences the net present value (NPV) and overall economic feasibility of a project. Although the need to plan and operate a mine with an optimum production schedule has been obvious for some time, the methods used to obtain such a schedule have not been. The current practice for doing is to do it by hand on the basis of trial and error analysis. In an attempt to improve the way production scheduling is currently done, a new computer based production scheduling algorithm is developed. This paper describes the fundamentals of this newly developed heuristic scheduling algorithm and its application on a gold deposit. The results coming from a case study show that the schedules based on the new method provides improved cash flows compare to a manually done production schedules.

As part of a $2.2 Billion bridge project that will link Indiana and Kentucky, an interchange project northeast of Louisville will require the mining of a 6-lane tunnel system about 2,000 ft long in order to cross beneath the historic Drumanard Estate. This paper will discuss preliminary engineering that has been undertaken to compare two-bore and three-bore layouts, the planning of an exploratory tunnel facility, and the potential for including a test chamber to investigate the ability of high capacity roadheaders to mine the 22,000 psi limestones.

The recent seismic events have shown that earthquakes have an immense destructive power. Chile was hit in February 2010 with a seismic intensity of 8.8 on the Richter scale, and had loss of about 700 killed persons, while Haiti was hit in January 2010 with 7.0 killing about 220?000 people. The event in Haiti showed that the seismic events can lead to a disaster if no preventive measures are taken. On the other side, the seismic event in Chile has hit a well-prepared country with moderate consequences. The earthquake with 6.6 Richter in Japan in March 2011, have further shown how severe the consequences can be if critical equipment is involved. Fortunately, the equipment in a mine has not the potential hazard as a nuclear power plant. Anyway, we are diligent and it is obvious that the seismic risk may lead to a real disaster even if the probability is small. The GMD is an important component in the grinding circuit of a mine. This paper maps out earthquake-prone areas including expected earthquake magnitude calculated according to relevant standards, focusing mainly on the building code IBC 2009. An overview is provided on how this standard is applied to mining equipment to minimize damage and aid large mills in operation to successfully withstand seismic effects. The differences in stresses and forces on the GMD and foundation are shown based on a turning and stopped mill simulation during an earthquake. The results demonstrate the need to maintain active and operating earthquake detection systems. Aside from detecting an earthquake?s magnitude, the sensor information can also be used to reduce the harmful effects that an earthquake has on mining equipment. As follows the earthquake requirements are defined. The last part of this paper explains how to fullfill these requirements.

The link between artificial ground freezing and the mining industry stretches back more than 140 years, and across an ocean. Patented by the German scientist F.H. Poetsch in 1863, the technique is reported to have first been used circa 1862 in the coal mining valleys of South Wales. In the United States, the first ground freezing application, to a depth of 30 m (100 ft), occurred in 1888 for the Chapin Mine Co. in Iron Mountain, MO. In England in 1901, the Poetsch method was used for the first time to successfully sink two shafts through 24 to 27 m (80 to 90 ft) of wet sand and boulder clay at the Washington Glebe Colliery, near Sunderland. British mining engineer T.H. Cockin, in his 1905 Class-Book of Practical Coal-Mining, noted that, ?This method of sinking through thick beds of very wet quicksand has met with great success; it is one of the recognized methods of sinking on the Continent.? Mine shaft freezing soon became relatively common in Europe where the project could bear the cost of a major refrigeration plant construction. However, it was not until 1952 that the second North American ground freezing project was completed to facilitate the sinking of a 4.5-m- (15-ft-) diameter, 233-m- (765-ft-) deep shaft at a Potash Company of America (PCA) mine in Carlsbad, NM. Nowadays, ground freezing is used worldwide for both mining and civil applications. In small mine shafts of 3 to 6 m (10 to 20 ft) in diameter, excavations have been carried out to depths of more than 820 m (2,700 ft) within the protection of unbraced or unlined frozen walls. In fact, for deep mines, no better method has yet been established for sinking production shafts through deep, water-bearing ground. Major deposits of coal, potash and salt would still be inaccessible if not for artificial ground freezing. There are several advantages of ground freezing unique to the construction of shafts. Proper instrumentation can provide complete assurance of the integrity of the freeze to full depth prior to excavation. The frozen wall allows construction to be scheduled without evaluating the in-shaft time necessary to probe ahead, place additional sup-ports or deal with ground water. The freeze can be implemented perfectly through the soil/rock interface, which is often the most difficult geology in which to create a ground water cutoff by other methods. Public Law 91-190 - January I, 1970 states that, "The purposes of this Act are: To declare a national policy which will encourage productive and enjoyable harmony between man and his environment; to promote efforts which will prevent or eliminate damage to the environment and biosphere and stimulate the health and welfare of man; to enrich the understanding of the ecological systems and natural resources important to the Nation; and to establish a Council on Environmental Duality." The administrative aspects of this National Environmental Policy Act of 1969 have in essence established the requirement that all agencies of the Federal government assess the consequence of their actions with an Environmental Impact Statement. Furthermore, five states have already enacted environmental legislation, and others will surely follow. The Environmental Services Project Director for Dames and Moore has stated in the June issue of that company's publication, "Elements," that, "The nuclear power industry has perhaps been most affected to date by the Act [NEPAL but recent court decisions, legislative trends and general public concern will accelerate the spread of the Act's impact on other industries. Mining and petroleum are already involved in environmental affairs." There can be little doubt that within a very short time all environmental aspects of the minerals industry will be strictly regulated and monitored by some agency of Federal and/or state government. Indeed, it is probable that any future effort to extract natural resources will be contingent upon a favorable review of an Environmental Impact Statement. It is also quite possible that continuation of present operations will have to be justified on the basis of proof of an acceptable environmental impact. It is the objective of this paper to present some ways in which remote sensing technology can be applied toward an efficient collection of environmental data necessary for the preparation of an Environmental Impact Statement.

Cyanide is a significant contributor to the cost of producing gold. Any reduction in consumption that can be achieved through more reliable measurement and/or more efficient management of dosing rates can have a significant financial impact. An automated analyser, by its mechanical nature, is not prone to the variations of human operators, particularly notice­able from shift to shift. Reducing the standard deviation of the measurement (ie, reducing noise ), the automated measurement will give the metallurgist a truer picture of the underlying patterns of usage. The combination of automated analysis with on-line operation and a control loop provides timely information that is efficiently utilised in the management of the plant. Until the release of the Chemtronics On-Line Cyanide Analyser (OTA3000) about 12 months ago, the Australian experience with automated analysers (especially on-line) had been substantially negative. Since its release, the Chemtronics instrument has outperformed both manual titrations and other automated instruments, gaining an enviable reputation for accuracy and reliability in the process. The instrument is built around the technique ofpotentiomet­ric titrations, in which the potential of an indicator electrode is measured as a function of the volume oftitrant that is added to the sample. As the cyanide in the sample is depleted, the potential on the indicator electrode changes rapidly, producing a typical sigmoidal curve. The OTA3000 takes the first differ­ential of the curve, identifies the apex of the peak as the endpoint and reports the result in parts per million of sodium cyanide. The OTA3000 accepts the peak as valid only if it exceeds a certain minimum height. That minimum height is a programma­ble parameter and is used to optimise the relationship between sensitivity and the rejection of spurious peaks. The instrument will track cyanide trends through both positive and negative slopes of any magnitude, but it will provide a warning if it the slope becomes excessive, not because it affects the instrument but because it means that something unusual is occurring in the tanks. There are a limited number of conditions under which the instrument will produce a result of zero. In these cases the OTA3000 holds any output lines at the last valid result while it repeats the titration. This feature was incorporated for reasons of stability in a feedback loop.
