During 1996 and 1997, Oregon experienced flooding on a scale not seen in many years. Some gravel pits located near rivers experienced erosion and, in several cases, breaches between the gravel pit and the river. Initial evaluation of several sites that experienced breaches indicates that impacts to the river system are complex and vary significantly from site to site. The impacts to the adjacent river system may be viewed as both positive and negative. Several floodplain-mining sites in Oregon located in areas of historic river flow have reverted to active riverbed. In many cases, state and federal fisheries biologists and university researchers see the resulting increase in channel complexity and habitat as favorable. The Oregon Department of Geology and Mineral Industries -Mined Land Reclamation Program (DOGAMI-MLR) is performing additional studies of the impacts to the off-channel areas that resulted from the 1996 floods. More data is needed to make sound floodplain decisions regarding mining.

The City of Toronto’s Coxwell Bypass Tunnel is currently being constructed by North Tunnel Constructors ULC, a joint venture of Jay Dee Canada, C&M McNally Tunnel Constructors and Michels Canada. This project includes construction of approximately 10.5 km of 6.3 meter finished diameter rock tunnel, five 20 meter diameter storage shafts and eleven tunnel connection drop shafts, along with associated deaeration and adit tunnels. This paper describes progress on the project to date and reviews the risk mitigation measures utilized on the project to move the project forward through tough geological conditions during shaft excavation and tunneling phase of the project.

Throughout the mining industry, pneumoconiosis is one of the most severe occupational health problems being faced. The goal of completely eliminating pneumoconiosis as an occupational health hazard was underscored by the Federal Government with its enactment of Public Law 91-173 (Federal Coal Mine Health and Safety Act of 1969). Under this law, permissible dust level standards will be placed at 2 mg/m3 in mine atmospheres by December 30, 1972. The mining industry is thus faced with the tremendous technical challenge of achieving this level. Two basic questions can be raised with regard to this legislation. First, does a maximum dust concentration of 2 mg/m3 prevent the occurrence of pneumoconiosis? Secondly, can this dust standard be achieved using presently available methods and equipment? A precise answer to the first question has not yet been developed by the medical profession. Studies are continuing on the origin, prevention, and therapy of pneumoconiosis, and permissible dust levels could become even lower as medical evidence accumulates. In the constant search for ways and means of improving the quality of coke and thus making its use at the blast-fornace more satisfactory and economical, it has become increasingly evident that the present laboratory tests do not show the full picture of a coke's value as a blast-furnace fuel. This fact was recognized by Dr. A. C. Pieldner thirty-five years ago: (Ref. 1) here is perhaps no phase of fuel technicology which has aroused more discussion or regarding which there is more difference of opinion than the properties of coke which make it most suitable for blast-furnaces and other uses." Dr. Fieldner followed this assertion with a statement to the effect that men who write on this subject today (1923) have, because of conflicting experimental evidence brought forth in recent years, far less positive opinions on the matter than did writers of some years past. One is led to wonder if these statements are not just as true now as they were when first made.

This is a shortened version of the paper ?The Methane-Air Explosion Hazard Within Coal Mine Gobs published in the 2013 volume of SME Transactions (Brune, 2013). This paper analyzes the explosion hazard resulting from the formation of methane-air mixtures in the mined-out gobs of underground longwall coal mines. Evidence from numerous mine explosions suggests that explosive methane zones can form within longwall gobs. Explosions and fires resulting from these methane accumulations have led to severe and fatal injuries to the miners, including the explosion of the Upper Big Branch mine in West Virginia, where 29 miners lost their lives. The paper also reviews current research on numerical modeling of gas flows in longwall gobs and outlines how targeted injection of nitrogen into gobs can effectively reduce the volume of such explosive methane zones. The paper concludes that comprehensive monitoring of gas compositions along the accessible fringes of the gobs along with inertization, improved ventilation schemes and further research are necessary to control this explosion hazard and make mining operations safe.

Forecasting demand is an art as well as a science, and much personal judgment is required. National forecasts made for sand and gravel, crushed stone, and/or aggregate may be misleading or unhelpful locally. This is because of significant variation in demand within different geographic and/or political areas in the United States, and/or the use of an inadequate data base and forecasting methodology. The best forecasts may be made for local or regional market areas, irrespective of geographic or political boundaries, provided a proper data base is available. The Bureau of Mines compiles detailed statistics on commodities sold or used in the construction industries and, therefore, is in an advantageous position to make national aggregate demand forecasts.

Diamond mining is one of the two main mining activities in Namibia and diamond mining operations are rapidly moving from onshore to offshore deposits. This article provides a comprehensive overview of the current and future situation of the Namibian mining industry, with a special focus on the tasks and new technical developments for the successful excavation of beach and marine diamond deposits. The different deposits and the adequate applied mining technologies and methods are shown in examples.

Project engineering and development is a process of optimisation of capital, operating and financial costs, and extraction rates in the light of current and forecast economic conditions for the sale of the product. The aim of the process is to maximize financial returns to the sponsors (project owners) within many often conflicting constraints. Thus, the sponsor's engineering and financial staff have a common goal, with performance measured in financial terms. Consequently, each group must be aware of, and responsive to, the constraints imposed upon the other. For financial staff, the increasing size and complexity, and escalating capital costs of new ventures have required them to seek finance in larger amounts, and by non-traditional methods. In addition, senior management are requiring indications of the availability of finance as a pre-requisite to a decision to commence project development activities.

The discussion will relate only to the southern half of the Silver Cliff volcanic center and differences in geologic interpretation, which are largely structural, will be pointed out as they arise. Only the manganese/silver zone of the Kate Area will be discussed in detail.

The Newell Creek Dam Inlet/Outlet Replacement Project is located approximately 11 miles north of Santa Cruz, California along Highway 9 in the San Lorenzo Valley. This project will replace the existing inlet/outlet works at the Loch Lomond Reservoir by constructing a tunnel using conventional excavation methods while simultaneously drilling three intake shafts to intercept the tunnel from within the city’s water supply. This project will improve the system’s overall operational efficiency and performance, and provide a safe and reliable facility for the City of Santa Cruz’s drinking water supply. This paper discusses the overall project configuration, construction observations, and challenges during construction, including tunnel excavating through the marine sedimentary siltstone of the Monterey Formation, probing and pre-excavation grouting, and the tools utilized to manage the project.

Mine face ignition due to high levels of methane is one of the most common fire incidents in underground coal mines. In continuous miner headings, depending on the magnitude of the ignition or resulting fire, auxiliary ventilation controls may be damaged or dislodged, affecting the ventilation into the area. A computational study was conducted to investigate the effects of different levels of damage to an exhausting ventilation curtain in a continuous miner heading, and the impact on ventilation to the face. The computational fluid dynamics software Fire Dynamics Simulator (FDS) Version 6.0 was used to predict the conditions that develop due to a 200-kW fire in the face following a methane ignition with different levels of assumed ventilation curtain damage. Smoke layer depth, temperature and mass flow inby the last open crosscut were used to evaluate the impact of no damage to the curtain, partially dislodged curtain, and fully removed curtain. As more of the ventilation curtain was damaged, smoke depth and temperature outby the fire increased. With the full removal of the curtain, the smoke accumulated from the last open crosscut to the face, with the ventilation completely bypassing the continuous miner region.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

"Tunnel safety is evolving as science, engineering and technology make advances. Applicable safety codes and standards simply do not cover all the potentials of design, construction and operation. This paper reviews a series of tunnel projects code gaps in Seattle USA that were uncovered during the design and construction of a new highway tunnel, conversion of bus tunnels to combined road and rail tunnels, new rail tunnels using TBM and NATM, conversion of a road tunnel to light rail, and retrofit of an existing road tunnel to add lanes and improve safety systems. The perspectives of the ‘fire code official’ (Authority Having Jurisdiction) on methods to identify and resolve gaps in safety code and standard regulations, while garnering the consensus of the owner, designer, and contractors are presented. Additionally, the emergency responder command perspective on safe tunnel design, tunnel rescue during construction and operational safety is explained TUNNEL SAFETY EVOLUTION The underground has become nearly the only option to provide essential transportation corridors to continue our economic growth in key global areas. Tunnels are not built on a whim. Often significant portion of a regions wealth are invested to meet current, and hopefully future transportation demands. There are more transportation tunnels being built now than at any time in the planets history. The location of these tunnels is often selected for us based upon regional demand. Most commonly under densely populated urban centers and between populations’ centers to bypass geologic conditions like mountains and water bodies. Tunnels are being built in unforgiving soils, between tectonic plates, in extremes environmental conditions and often, under the very heart of the highest density population centers on the planet. All of these locations can bring with it conditions that challenge our common assumptions about safety. These extraordinary locations and soils create risks for both those constructing the tunnel, and in design engineering to meet the ‘minimum’ requirements at the lowest costs. There is lengthy history of above ground safety. Most countries and major local jurisdictions have adopted, or created safety requirements for the construction of above ground buildings. Through decades of use, these safety requirements have evolved, changed and been expanded to cover a wide gamut of actual and potential safety risks. However, the use of the underground for tunnel transportation corridors has grown exponentially with limited attention to safety requirements."

Rio Tinto is one of the world's largest mining companies, involved in every stage of metal and mineral production, producing aluminium, copper, diamonds, coal, iron ore, uranium, gold and industrial minerals. Operations are in more than 50 countries in multiple continents and vary in location between large urban centres, regional industrial complexes and remote regions. A risk-based strategy was developed by Rio Tinto to manage its air quality impacts on a global scale. The strategy consists of three objectives ? improving air quality performance, developing industry leading practices and engaging and influencing in air issues. This paper discusses three major successes achieved this approach. Firstly, how air shed resources are managed in constrained air sheds. Secondly, how financial modelling is used to value air quality constraints, and thirdly, how a rigorous and comprehensive e-learning course is used to improve the overall level of air management knowledge of Rio Tinto?s environment managers.

"Hydrocyclone separators are widely used in various industrial applications in the oil and mining industries to sort, classify and separate solid particles or liquid droplets within liquid suspensions, which are considered to be multi-phase systems. Numerous valuable studies have been conducted in recent years to investigate the flow fields inside hydrocyclones. However, much of the information regarding the performance of cyclones in the literature has limitations, based in some part on the respective current-available theoretical models, and much of it cannot be considered as completely applicable to most real-world applications; many of the studies investigated the flow fields within extremely simplified hydraulic designs that are not representative of the complex geometries or large sizes which are typical in industry. Therefore, in this study, the two phase flow system inside the actual hydraulic geometry of a milling circuit hydrocyclone was explored with the aid of both computational and experimental techniques (Particle Image Velocimetry). In this study, the flow field with an air core has been investigated; in essence, the research was a two-phase problem, which caused some challenges on both the computational and experimental sides. Two turbulence models were used in the numerical calculations: the Reynolds Stress Model and Large Eddy Simulation. The computational studies are mainly focused on the prediction of the dimensions and shape of the air core. Different section planes of the hydrocyclone were selected as planes of interest and then were divided into several fields of view (FOV) for PIV measurements. Two dimensional experimental velocity vector maps were obtained in each of the fields of view. The computational results were validated globally using pressure and flow rate readings at the boundaries and locally by comparison to the PIV velocity vector maps and profiles. INTRODUCTION A hydrocyclone is a device used to separate a solid-liquid, liquid-liquid, solid-gas, or liquid-gas suspensions into at least two separate density or particle size components. Hence, the flow field of the hydrocyclone is generally considered a multi-phases flow system. Application of hydrocyclones have been used in numerous industries including oil, drilling, metal working, and mineral processing since 1891, when E. Bretney obtained the first patent for the predecessor design of all contemporary hydrocyclones. A modern hydrocyclone typically consists a cylindrical section at the top where the liquid is being fed tangentially, and a conical chamber attached to the bottom of the cylindrical section, as shown in Figure 1. The characteristics of the feed stream that is injected into the cylindrical chamber play a significant role in hydrocyclone operation. The inlet is tangential relative to the cylindrical chamber and hence causes a centripetal force in the flow field which is considered as the primary separation mechanism."

Products unveiled following the Miner Act in 2006 have been upgraded and improved. This paper reviews a study conducted for Australian Coal Research Limited (ACARP) to assist Anglo-American operations with better understanding of coal burst mechanics and control based on U.S. practical experience, investigations, and inspections in three major U.S. coalfields over the past 45 years. The study examined both nonviolent and burst-prone case studies to identify contributing risk factors and addressed failure mechanisms for major U.S. coalfields in Utah, Colorado, and Kentucky, which have diverse characteristics. To investigate the mechanics of coal burst, a multipronged approach is used. First, field measurements in three mines from East Mountain, Utah, are complemented with finite-difference stress analyses to address the importance of horizontal stress in coal pillar mechanics of violent failure. Second, the computational procedures for estimating seismicity resulting from slip along geological discontinuities are reviewed, as these measurements point to seismicity being the trigger mechanism for violent failure of marginally stable structures. This joint-slip seismicity mechanism is in agreement with the more recentimproved re-examination of mining-induced seismicity (MIS) data from the Crandall Canyon Mine, Utah by University of Utah (UOU). Third, a hybrid statistical-analytical methodology is applied for identifying significant factors affecting coal burst. It utilizes data from 30 case studies including those from Utah, Colorado, Kentucky, and other eastern U.S. mines. The statistical approach reinforces field measurements and points to strata rigidity, joint spacings, and horizontal stress field as significant factors affecting damage resulting from coal bursts. This provides practical capabilities for identifying operations of higher risk using a rigidity-cavability variable. Overall, the study provides valuable insights into the mechanics and risk factors associated with coal bursts in major U.S. coalfields. By utilizing a multipronged approach that incorporates both field measurements and statistical-analytical calculations, the study offers practical capabilities for identifying operations of higher risk and improving coal burst control.

"The International Tunnelling and Underground Space Association (ITA) held its 42nd General Assembly along with the 2016 World Tunnel Congress (WTC) April 22-28 in San Francisco, CA. The event was co-organized by the Underground Construction Association of SME (UCA). This was the first time the event has been held in the United States in 20 years.As cities demonstrate strong needs in tunnels and underground spaces for public transportation, subway stations, hydroelectric infrastructures and other underground infrastructure needs, the WTC 2016 provided the 2,319 attendees from around the world a unique occasion to discover the latest state-of-the art innovations in urban planning and underground construction projects in the United States and worldwide.The Congress, held in lieu of the UCA’s North American Tunneling Conference, included an exhibit that attracted 226 exhibitors in 298 booths. The exhibit had on display the latest in technical equipment and services from some of the largest and best companies in the international tunneling industry. The exhibit hall was also the place where participants were able to interact with the authors of the 168 posters and receive answers to various topics directly concerning civil engineers: innovative procurement processes, cost and risk management, contractual methods, BIM technologies and examples of innovative underground infrastructures built worldwide. And the technical program included 353 technical presentations – 185 podium and 168 posters.What is ITA?ITA-AITES is a nonprofit, nongovernmental organization that promotes greater use of underground space as a key instrument in sustainable development. This paper will present the results of an analysis of several abandoned coal mine projects and includes field data for over a dozen different reclamation projects recently completed in the state of West . Virginia. The analysis was performed with the objective of summarizing and or classification of the environmental consequences with reference to the characteristics of the mining facilities (surface, underground, etc.). The analysis also addressed the itemized cost of remediation with respect to procedures, environmental concerns and time. The results of the analysis are that each abandoned coal mine site in West Virginia have/had many unique features and in most cases include a combination of environmental concerns that complicate comparisons. The study also revealed that the abandoned mine land program for mines abandoned prior to 1977 that is being financed by a tax on the production of coal, has produced many benefits for the state of West Virginia. This grant program administered by the Office of Surface Mining, has resulted in a high quality methodology of remediation that has been successful for many years in greatly minimizing and/or eliminating environmental concerns from abandoned coal mines.

The management of mine tailings poses a complex range of potential risks, including physical risks, such as the risk of catastrophic failure of a tailings facility and chemical risks such as the risk of long-term leaching of contaminants into the downstream environment. If these risks materialize, they can lead to a wide range of impacts on people and the environment, including human fatalities and severe impacts on downstream ecosystems, and the costs of recovery borne by industry, communities, governments, and society as a whole can be immense. Tailings facility failures in Canada (2014), Brazil (2015 and 2019), South Africa (2022), and in other locations around the world underscore how high the stakes are when mines produce and manage tailings (WISE Uranium Project 2019). Understandably, many people do not trust the mining industry to manage tailings responsibly. As an industry, it is our responsibility to manage tailings in a manner that is responsible and strives to minimize harm associated with tailings management, including effectively managing both the physical and chemical risks associated with tailings. Standards have an important role to play in responsible tailings management by setting expectations for standards of care and providing a means to measure and report on performance. In jurisdictions with strong legal requirements for tailings management, standards play a complementary role, pushing performance improvements in areas that may be difficult or impossible to address in legal requirements. In jurisdictions where legal requirements are lagging, standards may represent the de facto leading practice and may be the primary tool to help ensure responsible tailings management. Implementation of standards can help companies demonstrate that tailings are being responsibly managed and help build trust and credibility. Most importantly though, standards are important tools to help ensure that the risks associated with tailings are reduced, and that remaining risks are effectively managed. However, we must recognize that conformance with standards can have a down-side – the proverbial two-edged sword. Having standards is vital but there is a risk that implementing standards becomes a “box checking” exercise, with ongoing conformance leading to complacency. We must not allow ourselves to fall into the trap of believing that, just because we are complying with applicable standards and laws, we are therefore doing our best, and that “it can’t happen to us.” In addition, if conformance becomes the goal and the standard does not have mechanisms to drive continual improvement, then once the standard is met, there may be no incentive to improve. The point of complying with standards is not to pass the test. The point is to manage tailings responsibly, prevent catastrophic failures, and minimize harm. The point is to do better, because we must do better. To do that, we need to strive not for conformance, but for excellence in tailings management, driven by continual improvement and a quest to do better.
