Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

Mine face ignition due to high levels of methane is one of the most common fire incidents in underground coal mines. In continuous miner headings, depending on the magnitude of the ignition or resulting fire, auxiliary ventilation controls may be damaged or dislodged, affecting the ventilation into the area. A computational study was conducted to investigate the effects of different levels of damage to an exhausting ventilation curtain in a continuous miner heading, and the impact on ventilation to the face. The computational fluid dynamics software Fire Dynamics Simulator (FDS) Version 6.0 was used to predict the conditions that develop due to a 200-kW fire in the face following a methane ignition with different levels of assumed ventilation curtain damage. Smoke layer depth, temperature and mass flow inby the last open crosscut were used to evaluate the impact of no damage to the curtain, partially dislodged curtain, and fully removed curtain. As more of the ventilation curtain was damaged, smoke depth and temperature outby the fire increased. With the full removal of the curtain, the smoke accumulated from the last open crosscut to the face, with the ventilation completely bypassing the continuous miner region.

Rail access to Ontario International Airport (ONT) has long been a goal of San Bernardino County Transportation Authority (SBCTA) to increase public transit options to ONT and provide incoming ONT passengers improved access to San Bernardino County and the greater Southern California region. This paper describes the planning for the Design-Build-Operate-Maintain (DBOM) delivery of an emerging technology tunnel connection between the Rancho Cucamonga Metrolink station and ONT. The paper discusses key technical considerations, challenges, and opportunities; conceptual configuration development; and construction planning and establishment of the footprint for the underground connection.

Heat-related illnesses (HRI) are relatively common in both hot surface and underground mining operations. When workers are exposed to extreme heat or strenuous work in a hot environment, they become prone to heat stress. Heat strain is the result of the body’s response to external and internal heat stress. It is therefore vital for the conditions leading to heat strain be detected and treated in a timely manner. Heat-related illnesses are manifested by exhaustion and heat stroke. The predicted heat strain (PHS) [ISO 7933 (2004)] model has been developed to predict the health condition of the worker in terms of core body temperature and water loss. The PHS model tested in this study is based on eight physical parameters that are measured at different intervals during a work shift. They include air temperature, humidity, radiation, air velocity, metabolic rate, clothing insulation, posture, and acclimatization. The model predictions are then compared with a direct physiological measurement, such as core body temperature. We present the results of an extensive study that monitored and predicted body’s response to heat stress under different environmental and working conditions. The PHS model provided reliable results in most instances in comparison with other prediction methods currently in use in the field.

Trust in AI systems and the nature and amount of information about the system can influence decision-making. It is, therefore, crucial to understand how AI-powered monitoring systems (e.g., gas monitoring systems) influence miners’ trust and decision-making in emergency evacuation. This work focused on measuring the differences between how participants respond to a simulated underground mine emergency evacuation situation when warnings are represented as coming from a human or AI-based monitoring system. We also manipulated the amount of information participants received about the system, yielding a 2×2 between-participants survey design. The participants received an alert message about rising gas levels in a mine and provided a response on how they would react and reported their perceived safety. We also asked questions to assess their trust in, preference for AI over human-based gas monitoring systems, and whether they are willing to delegate the duties of underground gas monitoring to AI systems. The experiment results show the amount of information had a significant influence on miners’ trust and decision-making. The safety perception of participants based on age, number of children and ethnicity was significantly different from one category to another. This work provides valuable insights as the mining industry deploys AI systems to aid mine safety.

Advanced measurement and verification (M&V) is a process where the impact of an energy initiative is determined in real time. Data prediction models can be used to predict and compare energy consumption with a predetermined baseline. Deeplevel mines can benefit from advanced M&V as the impact of operational changes can be identified in real time. Ultimately, this can ensure that informed decisions are made regarding energy management based on the impact and severity of the occurrence. Previous studies on advanced M&V do not account for operational changes to the model’s baseline. These changes can significantly impact the performance of both the mine and related activities. The aim of this study is to propose a method that can accurately predict the energy consumption of a deep-level mine regardless of operational changes. To achieve this objective, nine different data predictive modelling techniques were considered. The proposed method is based on the Moving Average (MA) modelling technique due to its simplicity, flexibility, and rapid development time. The coefficient of variation for the root mean square error (CV(RMSE)) was used to statistically analyse the modelling error. If the predictions exhibit an unsatisfactory CV(RMSE), the model dynamically adjusts to reduce the prediction error. The model was developed to adapt to operational changes in scenarios where the standard model becomes irrelevant. The method developed in this study was able to predict the future energy consumption of deep-level mines with an accuracy of 99%, improving the mine’s energy and finance management.

The general theory for the description of seismic sources is briefly discussed. The effect of excavations on seismic radiation is considered. Expressions for seismic source mechanisms in terms of the geometrical and mechanical parameters of source processes are provided for the following cases: fault slip and shear failure in a confined environment, dynamic stress fracturing around a tunnel, rock fall, landslide and sudden subsidence. A classification of seismic events in mines based on source mechanisms is introduced. The classification separates blast-, slip- and crush-type events. Various approaches of analysis of source mechanisms involving forward modelling and inversion are discussed. The following examples demonstrating the utility of source mechanisms in mining are presented: forensic analysis of damaging seismic events, assessment of the increase in the depth of failure for dynamic stress fracturing around tunnels and its duration, examination of the rockmass deformation regime, assessment of deformation around tunnels using crush-type events, identification and parameterisation of weak geological structures, stress inversion from slip-type events. Finally, the requirements for seismic data to ensure high quality source mechanism solutions are briefly reviewed and future challenges in the interpretation and utilisation of seismic source mechanisms in mines are speculated on.

Wet kata cooling power (WKCP) as measured by a kata thermometer is a well-known heat stress index used in the mining industry. Although, over the years, several researchers proposed different heat indices and indicated limitations of the kata thermometer, WKCP is used in the South African gold mining industry due to its simplicity and rapidity of assessment. This paper presents a comparative analysis of the newly developed empirical expression for the WKCP and some of the more commonly used empirical models. For this purpose, WKCP measurements are obtained based on experiments conducted in a low-speed wind tunnel. Based on a regression technique, the analysis presented aims to assess the disagreement between the empirical model predictions and actual measurements. The correlation is also presented between WKCP and the well-established thermoregulation models using parameters from the low-speed wind tunnel experiments, including dry-bulb temperature (Td), wet-bulb temperature (Tw), barometric pressure (P), and wind speed (v). It is found that the proposed empirical model for the WKCP represents the bioclimatic interaction quite well under the specific parameters that defines the harsh thermal environment and air condition, such as metabolic activity, clothing of the person, and the velocity of the ventilating air in which the person is working. The statistical analyses performed show that the empirical model proposed for the WKCP produces satisfactory estimates of the thermal stress experienced under lower air velocities, light clothing, and higher metabolic activity rates.

The Upper Llagas Creek Flood Protection Project in Northern California encompasses portions of the communities of Morgan Hill and Gilroy, and unincorporated areas of Santa Clara County. The project provides for flood protection improvements along Llagas Creek and two of its tributaries, West Little Llagas Creek and East Little Llagas Creek. Llagas Creek is one of the tributaries of the Pajaro River and drains a watershed of approximately 104 square miles. The river system has a history of flooding the watershed communities, and the project’s goal is to provide 100-year flood protection by creating a bypass infrastructure, including channels, cut-and-cover culverts, and a tunnel. The project has been divided into several phases. This paper describes Phase 2A, which consists of approximately 2,150 linear feet of lined horseshoe-shaped bypass tunnel measuring 14 feet wide by 12 feet high. The bypass tunnel was sequentially excavated using a combination of roadheader and controlled blasting through colluvium and weathered to fresh greenstone. The tunnel was mined near residences, requiring strict noise and vibration controls. Phase 2A also includes the construction of twin bypass concrete culverts, portal shoring, and dewatering systems. It features the use of a Design Review Board and partnering for project delivery. This case study discusses the bypass tunnel design, progress of tunnel construction, plans to complete the work, construction risks, and the software and procedural tools being used to manage the project, including ProjectWise construction management software.

A significant news item appearing in a coal mining journal in November 1969 stated simply that a Caterpillar D-333-CNA engine had received Bureau of Mines Schedule 31 approval. This item occurring at the time a new mining property in Martin County of East Kentucky was being prospected and prepared for mining, resulted in the manage-ment of Martin County Coal Corporation asking the question, "Why not use diesel equipment in our new operation?" Martin County Coal Corporation is part of the A. T. Massey Coal Company, Incorporated organization. The property being developed is on a 17,500 acre leasehold obtained from Pocahontas Land Corporation and lies in the Wolf Creek area of Martin County. The Norfolk and Western Railway Company built a 24-mile railroad into this undeveloped section of Kentucky to tap a huge reserve of steam coal. Duke Power Company is the customer who will take all of the production from Martin County Coal Corporation mines which ultimately is projected to be 2,000,000 tons annually. Four coal seams, all above drainage, are included in the lease. The Middle Coalburg and Stockton are being deep mined now and it is in those mines that the diesel haulage equipment was introduced. This paper will endeavor to trace the steps loading to the introduction of the first diesel-powered Wagner Mining Equipment Company ScoopTram in a coal mine in America. It will further elaborate on the features which make this unit a very versatile and useful utility unit as well as the safety features which are incorporated in the machine. "INTRODUCTION This paper addresses the prevalence of poor public reporting practices of Coal Exploration Results, Coal Resources and Coal Reserves in southern Africa. Currently, the reporting of coal tonnage without supporting coal qualities or “saleable” tonnes and quality is a poor reporting practice that occurs far too often. Not all coal is equal – therefore it is extremely important that investors or potential investors are provided with the full details of both run-of-mine (“ROM”) and Saleable coal tonnage, their associated qualities and other material and relevant information. Competent or Qualified Persons should note that publicly reported information should be sufficient to enable an informed reader to make a reasonable and balanced assessment of the significance of the information provide. “Companies are encouraged to provide information in their Public Reports, which is as comprehensive as possible” (CRIRSCO, 2013): too often this isn’t the case and material matters are either not reported or inadequately disclosed. According to the South African National Standard (SANS) 10320:2014 draft document - “tonnage and coal quality that will be available for sale, either in the raw ROM state (raw Saleable) at a specified moisture content, or after beneficiation of the ROM coal by processing operations to produce materials at specified qualities, size ranges and moisture contents. The assessments must demonstrate that at the time of reporting, the marketing of the products is reasonably justified. The basis of the predicted practical product yield to achieve the Saleable Coal Reserve must be stated. The moisture basis of reporting shall be stated.” As an example to why coal qualities are so important to the public, the requirements of South Africa’s electric public utility (Eskom) power stations are highlighted. The Kendal power station, commissioned in 1988, operates at a Calorific Value (“CV”) range of 19 to 20MJ/kg, an Ash content between 29 and 34% and Volatiles between 20 to 21%. Other power stations, such as Kriel or Hendrina, may require different qualities such as a CV range of 22 to 23MJ/kg, an Ash content between 20 and 28% and Volatiles between 22 to 23%. Thus, in terms of domestic saleable coal, it isn’t just about reporting the coal qualities but also informing the investor as to which power station the coal is intended to be sold as “boilers are designed to burn particular qualities of coal” and “it is not always possible to transfer coal between stations because of different quality requirements at each station” (Chetty, 2010). Sulphur content is also another important quality with the ideal coal content having a quality of less than 1% Sulphur."

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Most of the reported coal spontaneous combustion events occur in surface and underground mines where low rank coals (i.e., subbituminous and lower for this particular studies) are extracted indicating their high propensities for spontaneous combustion. Low rank coals have high moisture content (15 to 45%) and are capable of absorbing more moisture in dried condition. However, it is hard to induce the initial stage of self-heating process for the low rank coal using the current laboratory R70 testing method — a method widely used in the coal industry. Like a sponge, dry low rank coals can absorb water vapor in the air while the heat contained in the moist air is transferred to coal and served as the required activation energy to initiate the coal self-heating process. In order to prove this theorem and quantify the effects, the R70 adiabatic oxidation testing method has been improved to introduce a controlled moist flow into the coal reaction vessel. Tests using the new method and its setup on low rank coals showed that the initial self-heating process of coal sample can be greatly accelerated by the added water vapor — impossible with the regular R70 method. Once the self-heating is sustained, the removal of air moisture can further accelerate the process.

The advantages of this approach over the unit process control will be outlined. Some insight will be given into the risks associated with this method. Furthermore, it presents a comprehensive analysis of the historical development of deep-sea mining technology and equipment, offering valuable insights into this field. By examining the current issues and obstacles faced in deep-sea mineral extraction, this study concludes that the future of deep-sea mining lies in the direction of automation, intelligence, and standardization. Additionally, it identifies the key technological advancements that need to be achieved in order to overcome the challenges associated with deep-sea mining.

The Southern Powder River Basin (SPRB) now commands 30% of the US utility, coal market due to the region's low mine production costs, competitive rail rates and low sulfur content. In 1997, production exceeded 240 Mt (265 million st). This was an in¬crease of 104 Mt (115 million st) since 1989 and 4.5 Mt (5 million st) above 1996. Continued growth is expected as the second phase of the Clean Air Act Amendments (CAAA) approaches. This growth will be the greatest in the southern part of the Basin where coal quality is higher. Coal characteristics SPRB coal is classified as subbituminous due to its low heating value and high moisture content. The coal is characterized by low sulfur and ash contents, low ash fusion temperatures and high reactivities. In 1997, the average heating content was 8,593 Btu/lb, with an average sulfur and ash content of 0.79 lbs S02/million [ ] Btu and 5.13%, respectively. The distinguishing characteristic of SPRB coal is its low sulfur content. Most SPRB coal contains less than 1.2 lbs S02/million Btu. This is the approximate definition of "compliance" coal for Phase II of the CA AA. Much of the coal, particularly in the southern part of the Basin, is referred to as "super-compliance" coal due to SO2 contents of between 0.4 and 0.7 lbs SO2/million Btu. For process analysis, BRGM is developing and commercializing three packages: ? sampling software: ECHANT 2.0 for Windows ? material balance calculation: BILCO 2.0 for Windows ? steady-state simulator for plant design, adaptation and optimization: USIM PAC 2.1 for Windows This paper deals in a first part with the main features of these three packages. In a second part, advanced models are presented for SAG/ AG mills and flotation columns. An industrial case of application is shown for each of them.

"Una de las enfermedades más extendidas en la industria minera es la pérdida de audición inducida por el ruido. La exposición de los mineros a niveles de ruido superiores a los niveles de exposición permisibles resulta en la pérdida de audición de aproximadamente el 80% de los mineros del carbón a la edad de jubilarse. En este contexto, el Instituto Nacional para la Seguridad y Salud Ocupacional (NIOSH, por sus siglas en inglés) está realizando investigaciones para desarrollar controles de ruido aplicados a los sistemas de minado de tajeos largos. Este método de minado es usado para extraer la mitad de la producción nacional de carbón subterránea. A partir de las mediciones en campo, se determinó en investigaciones anteriores que los componentes dominantes que generan sonido en el cizallador de un sistema de tajeos largos son los dos tambores de corte utilizados para extraer el carbón. Debido a las dimensiones y complejidad de los sistemas de minado de tajeos largos, el proyecto NIOSH desarrolló un modelo de elementos finitos validado del tambor de corte para utilizarlo en la predicción del sonido generado por este debido a las fuerzas de excitación experimentadas en las operaciones subterráneas. Este mismo modelo se utilizará para desarrollar controles de ingeniería para el ruido del tambor. Las fuerzas de excitación surgen de la interacción de la broca de corte y el carbón, y se transmiten al tambor de corte a través de los sujetadores de brocas. Debido a las condiciones adversas en el frente y la falta de instrumentación aprobada para uso subterráneo, las fuerzas operativas de corte de carbón no han sido evaluadas anteriormente. Para este fin, NIOSH, en colaboración con la Universidad Tecnológica de Michigan, desarrolló una broca instrumentada autónoma con seguridad intrínseca para medir estas fuerzas. Este artículo describe el desarrollo de la broca instrumentada y la medición de las fuerzas operativas de corte de carbón en tres minas. Las mediciones revelaron que las fuerzas tienen un espectro plano, algo característico de la naturaleza del impacto de estas fuerzas, hasta alrededor de 100 Hz. Por encima de esta frecuencia "cutoff", la magnitud de la fuerza disminuye a una razón proporcional al cuadrado de la inversa de la frecuencia."
