The third edition of the Underground Construction Association of SME?s (UCA) Cut-ting Edge series of conferences focused on a challenge every underground construction and tunneling project faces: controlling ground water. The conference, held in New York City in November, is a partnership between the UCA and The North American Tunneling Journal. It featured more than 20 technical presentations by professionals from around the world. Each described specific challenges unique to their projects and the solutions employed. More than 175 professionals attended the two-day conference that included an exhibition featuring 17 companies. In addition to the technical program, the Cutting Edge conference featured a site visit to the $10.4 billion East Side Access Project. Sponsors of the meeting included Soil Freeze, Traylor Bros., Kiewit, Skanska, Moretrench, Aecom and The Robbins Co.

During 1963 more than 200 million tons of coal was consumed by the public utility industry. Ash residual, from the burning of this coal has caused disposal problems. In order to prevent air pollution, dust collection systems have been installed to suppress the fine ash emitted from boilers before it is discharged into the atmosphere. The collected ash, called fly ash, must be removed from the dust collection facilities and disposed in some manner, usually at a substantial cost to the utility. Many years ago fly ash disposal was not a serious problem because the demand for power was relatively low and coarse coal was burned on stoker type traveling grate machines. The low burning rates utilized on these grates did not produce large quantities of air-borne coal ash while the mechanics of this system of burning resulted in a clinkered residue of cinders. Both of these conditions resulted in the production of only small quantities of fly ash. As the demand for electricity increased, a method of burning pulverized coal was developed and was introduced to the public utility industry in 1918 at Milwaukee, Wisconsin.1 This method of firing was considered a significant advance in steam electric generation as it (1) permitted the use of coals that could not be economically fired by other methods and (2) has generally resulted in improved efficiency, greater generating capacity and lower costs.2 The utilization of larger generating units using pulverized coal substantially increased the quantity of fly ash which resulted in a' more severe disposal problem.

INTRODUCTION Legend has it that when Isembard Kingdom Brunel was confronted with the cost and consequences of one of his bridges having failed, he responded cheerfully that the event had saved investors and taxpayers a fortune because it prevented him from repeating the same design at several other locations. To meet the demands of rejuvenating and expanding the transportation system in an era of industrial growth Brunel and his contemporaries introduced bold designs and many innovations. Great advances in design and construction techniques resulted, yet these too had their practical limits as the story tells. In the gold mining industry today we face the same challenge of expansion, yet designers must respond mindful of economic restraints. Is innovation the answer? It has two meanings; to bring in novelties and to make changes. Distinguishing between improvement and novelties, or simply recognizing the benefits and limitations of new ideas over proven technology is part of good design. This paper will illustrate the practical aspects of selecting appropriate solutions to a design problem by examining a variety of examples common throughout the industry. Since a variety of engineering disciplines contribute to the design of a gold processing plant, and each has some old wisdom to impart or new development to boast this paper is divided into the different areas of expertise involved.

"The majority of tunnels for civil engineering applications are now being constructed using some form of mechanical excavation. Beginning in the 1960s with rock tunnel boring machines (TBM), the tunneling industry has introduced both earth pressure balance (EPB) and slurry pressure balance (SPB) soil machines; mixed soil and rock machines and a variety of different mechanical devices for the construction of small diameter tunnels. Over time, these machines have become more powerful and more adaptable to a wider variety of ground conditions; so much so that tunnels are now being constructed in ground conditions and in the vicinity of third-party impacts that would have been considered beyond the state of the art just 10 years ago. Figure 1 is an example of a mixed ground crossover TBM used successfully at Mexico City’s Túnel Emisor Poniente II.All of the above is highly advantageous for the tunneling industry, but it has also placed a much higher level of risk on the performance characteristics of the tunneling machines, on the contractors operating those TBMs and on the manufacturers of those machines. Most of the risks for a tunneling project are associated with creating the space inside of which the finished facility will be constructed. In order to create that space, the tunnel contractor must make many decisions about the best way to excavate the ground, the best way to control the ground at the face of excavation and the preferred method for supporting the ground around the tunnel in a manner that is safe for the workers and stable with respect to all of the overlying and adjacent existing structures. If it is proposed to use some form of TBM in order to build the tunnel, then the TBM becomes central to all three of these activities and becomes an integral part of managing the risks associated with these activities."

"BACKGROUND As of December 2015, there are roughly 475 road tunnels within the United States according to the most recent national tunnel inventory data compiled by the Federal Highway Administration (FHWA); whereas, there are more than 615,000 highway bridges on public roads. The FHWA estimates that tunnels represent nearly 100 miles of Interstates, State routes, and local routes. Tunnels are part of the national transportation infrastructure and represent a significant financial investment for the owner and, hence, the tax payers. Tunnels, that are not properly maintained, usually require more extensive repairs and consume more resources. Tunnels must provide safe, reliable, and effective transportation even as these structures age and deteriorate. Furthermore, tunnels generally remain in service for extended periods of time due to their very high construction costs. For example, many tunnels were constructed decades ago during two historic periods of highway system expansion; the first period correlated with the era of public works programs linked to the Great Depression of the 1930-40s; and the second period of expansion occurred with the building of the interstate highway system in the 1950-60s. As such, these tunnels are fast approaching 60 to 80 years old. Congress enacted the Moving Ahead for Progress in the 21st Century Act (MAP-21), which was signed by the President on July 6, 2012. This legislation followed a fatal tunnel incident on July 10, 2006 and a National Transportation Safety Board (NTSB) report1 that noted the importance of conducting regular tunnel inspections. MAP-21 authorized the Federal Highway Administration (FHWA) to develop a mandatory inspection program for all tunnels on all public roadways (Bergeson and Ernst 2015): To help safeguard tunnel users and to ensure reliable levels of service, the FHWA developed the National Tunnel Inspection Program (NTIP), which is comprised of the National Tunnel Inspection Standards (NTIS), the Tunnel Operations Maintenance Inspection and Evaluation (TOMIE) Manual, the Specifications for National Tunnel Inventory (SNTI), and the Manual for Bridge Evaluation (MBE). The data collected from the program will be stored in a National Tunnel Inventory (NTI) database. A comprehensive and refresher training program was also developed to help qualify and certify key tunnel inspection personnel to include the program manager and team leader."

This paper outlines the MOVIE project, which is dedicated to the development of a digital twin for an underground laboratory at the research and education mine “Forschungs- und Lehrbergwerk Reiche Zeche” in Freiberg (Germany). Data from induced rock deformations and temperature fluctuations is collected using fiber-optic sensors. The objective of the project is to establish a virtual laboratory for real-time analysis of the interactions between geology, geomechanics, geometry and ventilation. This will be achieved by integrating models from these disciplines with artificial intelligence, creating a sophisticated hybrid model. The insights gained will enhance safety and efficiency in mining through improved digital planning options, ultimately contributing to more effective resource management and operational decision-making in different mining environments.

Column flotation cells continue to gain acceptance in the coal industry for the treatment of high-ash streams containing clay slimes. Unfortunately, some industrial installations have failed to meet expectations due to improper scale-up, incorrect operation and poor ancillary circuit design. To overcome these problems, case studies have been conducted to fully define the capabilities of column cells for coal applications. These data have been used to develop generic engineering guidelines for column circuit design and operation. An economic analysis has also been conducted to illustrate the importance of key capital cost considerations. These analyses indicate that column cells are significantly more expensive than conventional machines, although this difference can be greatly reduced through the installation of very large-diameter columns.

"Mine refuge shelters are designed to protect mine workers from hazardous environmental conditions after a mine disaster, but high temperature and humidity levels inside these shelters may pose a significant safety risk. Due to the safety risks associated with human testing, detailed thermal models of shelters and human occupants have been developed using TAITherm simulation software. These models are used to predict the temperature and humidity within the shelter resulting from metabolic heating, evaporation due to sweat and respiration, moisture and heat from the shelter’s carbon dioxide scrubbing system, and heat transfer within the shelter. A detailed human thermoregulation model simulates the sensible and latent heat generated by the shelter occupants, and predicts the occupant’s body core temperature and moisture lost due to sweating and respiration. This information is used to assess the safety of the shelter. In this paper we will review technical details of the human thermal model, validation comparing the human model to human subject test data, and results of a mine shelter analysis. INTRODUCTION Portable mine refuge chambers are used in US coal mines to provide protection to mine workers from hazardous conditions following a traumatic event. Although these shelters are designed to supply clean air and sustenance for up to 96 hours, prolonged exposure to the mine shelter environment itself can pose a substantial risk to human safety. Heat generated by the shelter occupants combined with heat from the carbon dioxide (CO2) scrubbing system can raise the shelter interior temperatures and relative humidity to dangerous levels that can lead to heat stress in humans. Unfortunately, the thermal burden associated with mine shelter exposure is not well characterized due to potential hazards associated with underground human subject testing. In order to assess the thermal burden on people within a mine shelter environment, a detailed thermal model was created to accurately predict the temperatures and humidity within the shelter, as well as the impact of these environmental conditions on human occupants. The model consists of a mobile inflatable tent-type mine shelter, 23 humans, and also includes the surrounding mine environment. The thermal simulations were performed using ThermoAnalytics' validated commercial heat transfer prediction code, TAITherm (v12.0). Additionally, TAITherm's Human Thermal Module (HTM) provided the capability to simultaneously model the thermo-physiology of 23 unique humans contained within the mine shelter. The human thermal models accounted for the thermal interaction with the environment and also provided a core temperature response prediction, which can be used to evaluate survivability in heat stress situations [1]."

The San Gabriel Valley contains some of the richest and most extensive sand and gravel deposits in California. Operating quarries within the San Gabriel area mine more than 15 million tons of high quality aggregate per year. This represents about 45 percent of the total mined in the greater Los Angeles area, one of the nation's largest aggregate markets and producing regions. Popular opposition to continued aggregate production in the San Gabriel Valley is high, especially in the city of Irwindale where 40 per-cent of the city's property is made up of "excavated holes," some 150-200 feet deep. Reclamation of 70 years of mining is now an almost impossible task. Zoning regulations have the effect of depleting these vast reserves rapidly by changing the definition of total reserves to include only "legally minable reserves." All but two or three of the aggregate mines now operating will be depleted or zoned out of business by the year 2000. Some studies indicate that there will be no aggregate mining operations by 1995. Recent studies show a drastic change in land use patterns within this valuable mineral resource area over the past 20 years. The San Gabriel Valley is a classic example of the lack of adequate regional planning with regard to urban development in MRA's and represents a "too little, too late" effort by local government and industry.

The goal of the project reported in this paper was to make recommendations for changes in roof bolting procedures and bolter design that would reduce roof bolter accident rates. To that end the project identified the major hazards associated with each task a roof bolter operator per-forms, normalized task accident frequencies. by the time spent on each task, reviewed equipment design features and operating procedures related to the identified hazards and developed recommendations for equipment and procedural changes that would reduce, if not eliminate, the hazards. Tunnel boring machines (TBMs) have been used for the construction of various tunnels for mining projects for the purpose of access, conveyance of ore and waste, drainage, exploration, water supply and water diversion. Several mining projects have seen the successful and economic beneficial use of TBMs, and there is an increasing awareness of the benefits of TBMs for mining projects. Key technical considerations for the use of TBMs for the construction of tunnels for mining projects include geological issues (rock type, rock alteration, rock strength, rock abrasivity, durability, ground water in flows), depth of cover and the potential for overstressing/rockbursts, site access and terrain, portal locations, TBM constraints, minimum tunnel size, tunnel support requirements, contractor and labor experience, and project schedule demands. TBMs offer key project schedule benefits with high rates of progress, which is a unique economic advantage recognized for mining projects where timing is of the essence in order to advance mining development and achieve early startup for operations. A thorough evaluation of the anticipated ground conditions should be performed as part of the decision to adopt a TBM for a mining project. Minimum specifications for TBMs are necessary in order to prevent the incorrect type of TBM from being used. Appropriate geotechnical investigations are necessary in order to provide key information on the anticipated subsurface conditions for careful planning and design of the proposed tunnel alignment. The overall site and geological conditions associated with some projects may suggest that it is not appropriate to use TBMs for some mining projects due to a perceived higher risk. In September 1992, mining in Mexico was opened again to foreign investment by President Salinas-de Gortari, after thirty years of "Mexicanization" of mining industry which limited foreign participation up to a 49 % (or 34 % in case of national reserves) on a Mexican mining corporation. During said period mining in Mexico was controlled by four or five big mining companies and besides mining production was focused mainly on silver, lead, zinc, copper and fluorspar. Suddenly in middle of a big crisis on Mexican mining industry due to low prices of minerals in the world and also in a period on which it seems that mining industry in the United States and Canada has been condemned to die for environmental reasons, it appears a new Mexican Mining Law to permit foreigners to participate in mining in Mexico' trough Mexican mining corporations that they may control up to 100 %

"This paper will explain how the Contractor and Owner avoided a contractual dispute involving characterization, impacts, and final remedy for challenging ground conditions encountered during drill and blast excavation of the starter tunnel required for launch of the tunnel boring machine (TBM). During excavation more than 600 feet underground, the Contractor encountered a large complex fault zone subject to more than 14 bar of water pressure. Successive ground inflows from the fault complex over a six-month period presented significant technical and partnership challenges for the team and ultimately delayed the project by nearly one year. This paper will discuss how the Contractor, Owner, and their respective consultants worked together to address these challenges, make decisions, and ultimately move the project to successful completion. PROJECT BACKGROUND AND NEED Seven states of the western United States of America rely heavily on the Colorado River and its tributaries for their water supply. The primary source of water for the Colorado River is winter snow that falls in the mountains ringing the Colorado River basin, including the Rocky Mountains, the Wind River Mountains, and the Uinta Mountains. Flows in the Colorado River vary significantly from year to year, depending on the amount of snow and rain deposited in the basin. During the 1980s and 1990s, snowfall was relatively abundant and Colorado River flows were high on average. Beginning in the year 2000 and continuing to the present day, snowfall has been well below documented historical averages and Colorado River flows have diminished. In fact, a trend toward diminishing flows is evident during the period of the historical record. This variability in Colorado River flows and the trend to lower flows is represented in the following Figure 1."

The LA Effluent Outfall tunnel is a 7-mile long, 18' internal diameter, secondary treated effluent discharge tunnel currently under construction by Dragados USA for the Los Angeles County Sanitation District No. 2 (LACSD). The purpose of this tunnel is to add redundancy and increased capacity for discharge of secondary treated effluent from the LACSD’s Joint Water Pollution Control Plant, located in Wilmington, California within Southern Los Angeles County. Current discharge from this facility flows through two existing tunnels of 8' and 12' horseshoe profile, constructed in 1937 and 1958, respectively. When the new tunnel is complete, it will add an additional capacity of 670MGD in peak wet weather flow conditions. Geology of the roughly 7 mile long tunnel was divided into six reaches by the contract documents. Tunnel Reaches (TR) 1 to 3, comprising 19,550' will be excavated in alluvial sands of the San Pedro and Lakewood formations, with cover ranging from 45 to 90 feet. In this reach the tunnel intersects the seismically active Palos Verdes fault Zone. TR 4 is a 1,600' transitional zone between alluvium and the rock formations of the Palos Verdes (PV) Hills, comprised mostly of Malaga Mudstone. TR’s 5 and 6 total 15,591', and are within the rock formations of the PV Hills with overburden ranging from 315' to 475'. The primary constituent of the PV Hills is Altamira

While the mood at the 2013 annual joint meeting between the Pittsburgh Coal Mining Institute of America (127th Annual Meeting) and SME Pittsburgh Section (68th Annual Off-the-Record Meeting) in Canonsburg, PA was subdued following a rocky year in the coal industry, attendance was down only slightly, to 300, and overall, the meeting?s focus was optimistic about the future of coal. The conference began by addressing one of the central challenges of coal mining head-on with a keynote address on the status of safety in the mining industry. Noting that two more U.S. miners had died in 2013 than by the same date in 2012, the National Mining Association?s (NMA) Senior Vice President of Regulatory Affairs Bruce Watzman made a plea for a new approach to safety. He described the NMA CORESafety initiative, a program that promotes proactive safety measures versus a compliance-driven model. CORESafety, described in more detail on pp. 29 of this issue, focuses on developing successful ways to change company cultures of safety and behavior. Other industries that have adopted this approach have seen dramatic drops in fatalities, including the Australian mining industry, which has seen its annual fatality rate drop to nearly zero even while the number of people employed in the industry has soared. The rest of the morning?s sessions explored what is arguably the industry?s biggest challenge right now ? maintaining the social license to operate.

Stoppings are necessary ventilation control devices for effective air distribution underground. However, one of the major problems underground mining has is that every stopping is a potential leaking source. Since air always flows from a point of high pressure to one of lower pressure, and the total pressure at every point in intake airways is higher than that in the return airways, air will always tend to leak from intake to return (Tien, 1996). In many mines, less than half of the air entering the mine actually gets to the working areas; the rest of it simply short-circuits directly to return airways through leaky stoppings instead (Timko and Thimons, 1983). Leakage through a stopping occurs because of pressure drops across the stopping and available leaky path in stoppings. Empirical equation has been widely used to characterize the relation the amount of leakage and variables that affect leakage. However the use of this equation requires accurate information for constants which are not always readily available. It is the attempt of this paper to derive an equation similar to the Atkinson Equation that can readily be applied to characterize pressure-leakage relationship for any given stopping.

Amphiboles are hydrated mineral silicates five of which occur in asbestiform habits as asbestos grunerite (amosite) asbestos, riebeckite (crocidolite) asbestos, anthophyllite asbestos, tremolite asbestos and actinolite asbestos] and non-asbestiform habits (grunerite, riebeckite, anthophyllite, tremolite and actinolite). The asbestiform varieties are characterized by long, thin fibers while non-asbestiform varieties such as cleavage fragments form short fibers with larger widths. The U.S. regulatory method for counting asbestos fibers (aspect ratio [>]3:1, length [>]5 µm) does not distinguish between asbestos and cleavage fragments. The method biases toward increased counts of non-asbestiform cleavage fragments compared to long, thin asbestos fibers. One consequence of this regulatory approach is that workers can be erroneously classified as exposed to concentrations of asbestos (asbestiform amphiboles) above the U.S. 0.1 f/mL exposure standard when in fact they are not exposed to asbestos at all but non-asbestiform amphibole cleavage fragments. Another consequence is that the known carcinogenic effects of asbestos may be falsely attributed to non-asbestiform amphibole cleavage fragments of the same mineral. The purpose of this review is to assess whether amphibole cleavage fragments pose the same risk of lung cancer and mesothelioma characteristic of amphibole asbestos fibers. We identified three groups of workers exposed to non-asbestiform amphiboles: two groups exposed to grunerite (Homestake gold miners and taconite miners) and one group exposed to industrial talc containing non-asbestiform tremolite and anthophyllite in St. Lawrence County, NY. In addition to assessing strength of association and exposure?response trends in the non-asbestiform amphibole cohorts, comparisons were also made with cohorts exposed to the asbestiform counterpart (positive control) and cohorts exposed to the mineral (e.g. talc) that does not contain amphiboles (negative controls). The cohorts exposed to non-asbestiform amphiboles had no excesses of lung cancer or mesothelioma. Similar results were observed in the negative control groups, in stark contrast to the excess risks of asbestos-related disease found in the asbestos cohorts. The only possible exception is the twofold increased risk of lung cancer where exposure was to industrial talc containing cleavage fragments of tremolite and anthophyllite. However, this risk is not considered attributable to the talc or amphibole cleavage fragments for several reasons. A similar increased risk of lung cancer was found in Vermont talc workers, studied in the same time period. Their exposure was to relatively pure talc. There was no relationship between lung cancer mortality and exposure measured as mg/m3 years and years worked. A case?control study reported that all the lung cancer cases were smokers (or former smokers) and attributed the excess to smoking. There were two mesothelioma cases among the NY State talc workers exposed to cleavage fragments of tremolite and anthophyllite, but talc is not a plausible cause because of too short latency and potential for previous asbestos exposure. The positive controls of tremolite asbestos and anthophyllite asbestos exposed workers showed excess risks of both lung cancer and mesothelioma and positive exposure? response trends. St. Lawrence, NY talc does not produce mesotheliomas in animals while amphibole asbestos does. In sum, the weight of evidence fully supports a conclusion that non-asbestiform amphiboles do not increase the risk of lung cancer or mesothelioma. ©2008 Published by Elsevier Inc. Asset Maintenance Readiness (AMR) provides a tailored reliability and maintenance strategy specific to operational needs. AMR delivers reliability and maintenance foundations for the plant and equipment before start-up. Having a dependable reliability and maintenance strategy before start-up is critical, particularly in those first weeks and months during ramp up to stable production. The Asset Maintenance Readiness is the integration of Reliability and Maintenance Engineering analysis/studies at the earliest stages of the new Production System concept and design and during the Capital Project CapEx phases; adding this analysis to the Capital Project CAPEX cost, will have an impact of 2% to 3% of total project cost, but will deliver a good compendium of benefits far from the AMR costs, and those results will be shown upon commissioning and during the life cycle of the plant.

Following on from many years of research aimed at improving the effectiveness of truck haulage operations in mines operated by BHP Minerals, BHP Research developed a concept for an Innovative Large Mining Truck, (ILMT) which has the potential to significantly improve the cost effectiveness of rear dump mining trucks. The ILMT concept reduces the empty vehicle weight dramatically, greatly reduces unequal rear tyre loading and improves ride and handling characteristics. The concept has been taken to the "proof of concept" prototype truck stage with the cooperative efforts of BHP Coal, BHP Research, Liebherr Mining Truck, Inc., Liebherr-Australia and Birrana Engineering. Testing of the prototype truck commenced at a coal mine in Queensland, Australia during May 1996. Preliminary results from the testing program are very encouraging. Expectations are being met and in some cases exceeded.
