When it comes to processing industries, speed matters. More precisely, the faster/greater the throughput of finished product out the door, the more quickly money can flow back into the corporate coffers. In this quest for greater output, no processor can afford any weak links in the chain. Especially in the case of bulk solids operations, any one piece of equipment that slows the process down jeopardizes the profitability of the entire plant. However, one clay processor uncorked a pre-existing bottleneck by replacing its outdated separating equipment with a new design that immediately improved output.

The precious metal mining industry in Nevada handles and generates a certain amount of hazardous materials and waste on a regular basis. These products range from processing material additives to wastes from laboratory testing, heap leaching, and hydrocarbon treatment and disposal. The volume of materials and wastes is dependent upon the amount of ore processed. Handling, treatment and disposal is dependent on the type of material handled and the waste generated. Owing to the health and safety concerns and due to the existing regulations that control handling and disposal of hazardous materials and wastes, the mining industry is obligated to comply with all applicable requirements for proper handling of these products. Plus, in an effort to minimize any impacts from this effort, the industry is continually looking for alternative methods to accomplish these requirements and to also minimize costs. This paper will discuss the general, and in some cases the specific conditions, by which the industry is coping with the issue and efforts to minimize any impacts from handling, treatment and disposal for hazardous materials and wastes. The subject of compliance with the Toxic Release Inventory (TRI) requirements and bio-treatment of wastes and other technologies will be discussed.

"In April 2016, MSHA began requiring the use of continuous personal dust monitors (cPDMs) to monitor and measure respirable mine dust exposures to underground coal miners. After the cPDM’s implementation, mine operators discovered that it interfered with proximity detection systems (PDSs), thus exposing miners to potential striking and pinning hazards from continuous mining machines. NIOSH was sought out by MSHA and mining industry stakeholders to determine how the cPDM and PDS interact with each other. Accordingly, NIOSH investigated existing standards, developed test protocols, designed experiments, and conducted lab evaluations. Some interferences were observed to be caused by electromagnetic interference (EMI) from the cPDM. Results showed that there was no significant interference when the cPDM and the miner-wearable component of the PDS were separated by distances of 6 inches or greater. In this study, the cPDM and PAD needed to be at least 6 inches apart in order for them to be used simultaneously and reduce interference potential. INTRODUCTION Underground coal miners are exposed to a variety of hazards on a daily basis such as coal dust exposure, high noise levels, roof and rib falls, the potential for fires and explosions, and operating and working with heavy machinery. One of the hazardous jobs is operating or working nearby a continuous mining machine (CMM). According to Mine Safety and Health Administration (MSHA) statistics, 40 miners have been fatally struck or pinned by a CMM since 1984. In an effort to prevent future striking and pinning fatalities from occurring, proximity detection systems have been developed and are required on all operating CMMs in underground coal mines, with the exception of full-face CMMs, by 2018 (1,2). Proximity detection systems (PDSs) are designed to alarm miners and stop machine motion in order to protect miners from being struck, pinned or crushed by CMMs(3). Currently, MSHA-approved PDSs, installed on CMMs, are based on the principle of magnetic flux density (B-field) (4,5). The system generates a magnetic field around a CMM and determines the relative distance a miner is from the CMM based on a detected magnetic flux density. A PDS typically consists of multiple magnetic field generators mounted at different places around the CMM, and personal alarm devices (PADs), which are worn by the miners to detect the magnetic flux density. When a miner, wearing a PAD, gets closer to the machine, the PAD detects a stronger magnetic field from the generators and detects a weaker magnetic field when a miner moves away from the machine. The magnetic field generators installed on CMMs typically produce modulated magnetic wave signals at frequencies between 10 kHz and 120 kHz. The magnetic fields are measured by three small magnetic coil antennas mounted on orthogonal axes inside of the PAD worn by the miner. According to Faraday’s Law, the changing magnetic field induces a voltage in each coil antenna, and the voltage values are transmitted from the PAD back to a controller on the CMM wirelessly, typically at a frequency between 400 MHz and 2.5 GHz. These values are then used to determine the distance between the miner and the generators of the machine. Typically this information is used to determine when a miner wearing a PAD is in a warning zone or stop zone which would trigger different alarms and actions such as slowing the machine down or stopping it."

"Float coal dust is produced by various mining methods, carried by ventilating air and deposited on the floor, roof, and ribs of mine airways. If deposited float dust is re-entrained during a methane explosion, without sufficient inert rock dust quantities, this float coal dust can propagate an explosion throughout mining entries. Consequently, controlling float coal dust is of critical interest to mining operations. Rock dusting—adding inert material to airway surfaces—is the main control technique currently utilized by the coal mining industry to reduce the float coal dust explosion hazard. To assist the industry in reducing this hazard, NIOSH’s Pittsburgh Mining Research Division (PMRD) initiated a project to investigate methods and technologies to reduce float coal dust in underground coal mines through prevention, capture, and suppression prior to deposition. Field characterization studies were performed to determine quantitatively the sources, types, and amounts of dust produced during various coal mining processes. The operations chosen for study were a continuous miner section, a longwall section, and a coal handling facility. For each of these operations, the primary dust sources were confirmed to be the continuous mining machine, longwall shearer, and conveyor belt transfer points, respectively. Respirable and total airborne float dust samples were collected and analyzed for each operation and the ratio of total airborne float coal dust to respirable dust was calculated. During the continuous mining process, the ratio of total airborne float coal dust to respirable dust ranged from 10.3 to 13.8. The ratios measured on the longwall face were between 18.5 and 21.5. The total airborne float coal dust to respirable dust ratio observed during belt transport ranged between 7.5 and 21.8. INTRODUCTION During the extraction of coal, various dusts are produced that can be classified into two categories of interest: respirable and float coal dust. Respirable dust (diameter 10.0 = µm) has the capability to penetrate beyond the terminal bronchioles and become deeply embedded into the human lungs, leading towards respiratory disease complications (Potts et al., 1990). Float coal dust is defined as coal dust having a diameter less than 74 µm (Harris et al., 2009). During the mining process, float coal dust can be deposited on the floor, roof, and ribs of mine airways. With the proper mixture of methane gas (between 5% and 15%) and an ignition source, a methane gas explosion can occur (Barker and Humphreys, 1996). The resulting explosion can develop a pressure wave which lifts the deposited float coal dust material into the air, propagating a disastrous coal mine explosion (Harris et al., 2010). The current technique to safeguard against the propagation of a coal mine explosion is to treat the mine airways with incombustible rock dust (typically limestone), maintaining an 80% total incombustible content of the deposited material. NIOSH initiated a project to investigate control technologies that could be aimed at reducing the amount of float coal dust allowed to deposit in the mine airways using three approaches: prevention, capture, and suppression prior to deposition. Reducing the float coal dust being generated during the mining processes and capturing or suppressing airborne float coal dust prior to deposition would in turn increase the efficiency of the applied rock dust used to prevent mine explosions."

"Paste Technology has some advantages comparing to other surface disposal methods. With this method, it is possible to pump high concentrated solid materials to very long distances without segregation problem. However, it is known from Literature that pumpability of materials is completely related to rheological properties of the materials. For this reason, in this study, the rheological parameters (e.g. yield stress and viscosity values) of the paste material prepared from Pb-Zn tailings at various solid ratios (60-85%) were obtained in order to determine the pumpability of the paste material. Additionally, slump tests, which are usually used in mining industry, was carried out. Finally, regression analysis was made between the slump values and rheological parameters of the paste material to find a correlation between two independent parameters. The results showed that there is a great correlation between the slump values and the yield stress of the material. On the other hand, there was no correlation with viscosity: ty=430.28-16.877Sv (R2= 0,94; ty: Yield Stress (Pa), Sv: Slump Value (cm)). Based on the results obtained from this study, it can be clearly concluded that the slump values obtained from a simple test can be used to predict yield stress values of paste materials. INTRODUCTION Recently, surface disposal of mining tailings has become one of the biggest problems in mining industry in terms of environmental issues. Therefore, several researches have been focused on this subject in order to find a best solution or method for this problem. These studies showed that paste technology has some advantages compared to other conventional methods. Tailings thickened to the extent show no segregation of particles during transportation, and relatively small amount of settling occur post-deposition which are often called “paste” (Cincilla et al. 1997). Paste tailing technology has two applications in the industrial area as underground paste fill and surface paste disposal. Paste tailing technology was first started to be used in Grund mine, Germany in the late 1970s (Grice, 1998). Process tailing material, water and - if needed - cement materials, which are the components of the paste, must be blended to obtain a homogenous mixture suitable for pumpability. Consistency behavior of the paste material is determined using a conventional sedimentation cone concrete test (slump). Meanwhile, the slump value of the paste must be between 200 and 250 mm for surface applications (Newman et al., 2001). Additionally, in the case of underground paste system, the optimum slump value must be between 150 and 200 mm. On the other hand, the water content and density of paste at certain slump value change depending on the amount of fine particles in the tailing sample. While moisture of the material will be high, its density will be low because of surface area of particles which increases with decrease in particle size (Meggyes and Debreczeni, 2006). For pumpability of the paste to long distances, slump value of the paste must be good enough to be accepted for use of surface paste disposal. Paste material has a yield stress because they show non-Newtonian behavior and generally is classified as a Bingham plastic fluid. (Cooke, 2008; Theriault, 2003). In addition, the appropriate viscosity value of the paste along with the yield stress must be between 100 and 250 Pa (Bascetin et al., 2013; Bascetin et al., 2014; Bascetin et al., 2015)."

System simulation is an increasingly popular technique for analyzing and improving existing as well as newly designed underground mining operations. An engineering firm that specializes in simulation helped a major metals producer build a long-range planning model of an existing underground mine. Subsequently the team adapted this model to assess technology alternatives in the design of a new mine. The two models differ in the level of detail used for modeling the development process. This paper discusses the differences in approach between the models along with the experiences gained and challenges faced during the construction and use of the models.

The Rasmussen Ridge phosphate mine is owned by Agrium CPO and operated by a unit of Washington Group International. The mine, located in southeastern Idaho, received the Idaho Inter-Agency Reclamation Award from the Idaho Department of Lands. The phosphate mine, located northeast of Soda Springs, was honored for “Excellence in Ongoing Operations, Soft Rock Mines over 75Acres.” Charles Ross is Agrium CPO’s plant manager. He said, “We are honored to receive this award. This recognizes the commitment and dedication of our employees toward the protection of the environment. I am really proud of the people that work at the mine.”

Introduction A number of processes for deriving more convenient liquid and gaseous fuels from coal are commercially available or in final stages of development. Generally, synfuels from coal processes have efficiencies ranging from 50 to 70%. Considerable engineering design and development work are necessary to bring forth commercial plants using the various technologies-but technological breakthroughs are not expected or needed. Gasification While a variety of solids handling, gas contacting, and gas quality upgrading techniques may be employed in the various gasification processes, the primary goal is the same-to convert a sulfur-containing solid to an essentially sulfur-free gaseous product. The gaseous product may be a mixture of hydrogen and carbon monoxide with nitrogen. Definitions are not rigorous, but high-Btu gas is generally over 900 Btu/scf(33 534 kJ/m3) and low-Btu gas is generally below 350 Btu/scf(13 041 kJ/m3). Medium or intermediate gas falls between these values. High-Btu production is more difficult, as it requires additional upgrading of a low- or intermediate-Btu gas stream to produce a commercially acceptable product. The carbon/ hydrogen ratio is adjusted either by rejecting a portion of the carbon, increasing the hydrogen, or a combination of both. The chemical processing steps include production processing gas containing carbon oxides and hydrogen by the reaction of carbon and steam (gasification); reaction of hydrogen and coal to form methane (a hydrogasification process); reaction of hydrogen and carbon monoxide to produce methane (methanation); and the removal of impurities. The gasification processes share a number of common techniques. The coal received at the plant site must be further cleaned and crushed or ground before it can enter the gasifier. Conventional cleaning methods are used to remove extraneous materials such as metal, rocks, etc. For some processes the coal must be finely ground so it may be fluidized or entrained in a flowing gas stream. For other processes crushing and sizing only are required. Agglomerating coals may be pretreated by heating, mild oxidation with air, or other procedures to assure smooth gasifier operation and to avoid reactor plugging. Nonagglomerating coals Such as lignite and subbituminous coal normally do not require pretreatment. Feeding a solid material such as coal to a higher pressure reactor is a difficult task. Dry feeders using a series of pressurized lock hoppers, or wet systems in which a slurry is pumped into the reactor, are used. The gasification step is the heart of the process. Here, the solid feed material is contacted with synthesis gas (carbon monoxide and hydrogen). The coal is devolatilized and a portion of the fixed carbon converted to methane. The gas-solids contacting methods used include fixed beds, fluidized beds, moving beds, entrained beds, and free-fall systems. The gasification process, overall, is endothermic and the heat required to maintain reaction rates is usually supplied by burning a portion of the coal feed. External heat sources such as electric resistance heating, molten salt or solid heat carriers, and the introduction of a hot hydrogen stream also have been tested. Following the gasification step the particulate materials and heavy hydrocarbons contained in the reactor effluent are removed by a water quench and solids separation system. If low-Btu gas or synthesis gas is the desired product, the raw gas is scrubbed to remove carbon dioxide and sulfur compounds and then is used directly. Discussion by A.E Agnew Strachan's paper summarizes a wealth of data on water-storage dam "incidents" published by USCOLD and ASCE in 1988, together with a review of tailings-dam incidents published by USCOLD in 1994. The data is summarized by text and 11 histograms. The histograms present a useful compilation of these data, although the patterns within the histogram blocks did not survive the Mining Engineering printing process - so their usefulness is impaired. Unfortunately, the text discussion is too cursory and does not highlight the significant relationships among the various causes of the incidents. Four paragraphs of text and Fig. 1 were devoted to water-storage dams, but the discussion is too sketchy, and Fig. 1 is unclear. Important statistics that could be uncovered are: •the total number of dam incidents tabulated were 500+, •earth embankments constituted 73% of those in operation and •incidents related to earth embankments were 75%, of which dam failures accounted for 24%, major repairs accounted for 42% and accidents accounted for 34%. Strachan concluded that "the majority of the dam failures are associated with overtopping (p. 49), and that the majority of incidents are associated with spillway and other facility structures" - although this is difficult to discern from Fig. 1. Confusion regarding Strachan's use of the terms "dam failures" and "incidents" is compounded by his use of "failures" and "accidents" in one paragraph of his Introduction section followed by his use of "incidents" in the next paragraph. Getting to the meat of Strachan's important paper, the tailings-dam incident survey (USCOLD, 1994), the connection between Figs 2 through 11 seems obscure. First, in the four paragraphs of introductory material, one learns that "incidents consisted of failures, accidents, seepage and other incidents of performance that did not meet design criteria" (p. 50). One does learn the helpful caveat that "structures not related to mill tailings were not included in the review" - so that the Buffalo Creek (West Virginia in 1972) and Aberfan (Wales in 1966) coal-mine failures, together with the ongoing metal-mine remediation work at Summitville (Colorado) do not muddy the waters of his paper. The all-too-brief discussion of the data shown in Figs. 3 through 11 (185 dams in USCOLD plus 26 dams in UNEP) reveals the following: •Figure 3: 57% of the incidents (failures and accidents) occurred with dams less than 20-m (66-ft) in height. •Figure 4: 63% of the incidents were due to failures alone (subset of Fig. 3). •Figure 5: Active dams had 82% of the accidents and 93% of the failures. •Figure 6: Types of dams (upstream, water-retention, downstream, centerline and "unknown") separated according to incidents. •Figure 7: Breaks out the incidents in Fig. 6 by failures, accidents and ground water. •Figure 8: Breaks out incidents by type (overtopping, slope stability, earthquake, foundation, seepage, structural, erosion, mine subsidence and "unknown"). •Figure 9: Separates Fig. 8 data into active and inactive dams. •Figure 10: Separates Fig. 9 data into failures and accidents. •Figure 11: Same as Fig. 8, but broken out by tailings-dam type (Fig. 6). The iron and steel industry continues to operate at depressed production levels due to the low level of demand for steel and foundry products. Crude steel production in the Western world dropped by 13.7% from 459.3 Gt (506 billion st) in 1981 to 396.4 Gt (437 billion st). The reduction was particularly severe in North America where the industry suffered a loss of 37.3% from 123.6 to 77.5 Gt (136 to 85 billion st). This reduced level of production severely affected iron ore and agglomerates output. Substantial curtailment was made in both sintering and pelletizing plants to meet reduced demand. The pressure on these plant operators shifted from quantity to quality. With energy cost becoming an ever increasing part of production costs, conservation was a major area of emphasis. Iron Making Sinter has continued to be the favored burden for blast furnaces overseas. As hot metal production declined and excess sintering capacity developed, many of the overseas operators that have been using pellets found it advantageous on hot metal production cost to increase the ratio of sinter at the expense of pellets. This was the case despite the depressed price level of pellets in the world market. In North America where pellets have been the predominant burden material-mainly because it is necessary to pelletize the fine grain iron ores available domestically-there was also increased interest in sinter usage. Bethlehem Steel Corporation's 13.6-m (44.5-ft) furnace at Sparrows Point experimented with a burden with more than 70% of sinter during the second half of the year and indicated some favorable results as compared to its previous 50% super flux sinter and 50% acid pellet burden. The blast furnace operators continued to press for improved burden quality. In addition to the emphasis on the importance of burden chemistry and quality, uniformity, much emphasis was placed on improving reducibilitiy and high temperature metallurgical qualities. These burden properties were recognized to hold further promise in reducing fuels rates. Sintering With reduced consumption caused by the cutback in hot metal production, many sinter plants sat idle while others operated at greatly reduced rates. The deemphasis on production in many plants allowed them to experiment with various means to reduce operating costs. The most notable action was the introduction of low cost South American pellet feed fines into the sinter ore mix. These feed fines, which are finer than 150 µm (100 mesh) ore, were previously thought to be too fine for sintering. However, when the full production rate is not required the use of up to 40% in the ore mix was reported to be successful with the aid of burnt lime and adequate premixing and granulation of the sinter mix before sintering. The emphasis on quality and tighter cost control also has resulted in a further breakthrough in sintering technology. It was recognized that good feed preparation-including selection of compatible ores, good blending, good pseudo-particle formation, additive size control increased bed depth, and closed control of the sintering bed heat pattern-could allow the production of high quality sinters with much lower heat input and FeO level. Bed depth of more than 600 mm (23.6 in.) was practiced in a number of operations. Total heat input of better than 1.3 GJ/t (290,300 kcal per st) has been achieved. Introduction In response to industry needs and requests, Joy Mining Machinery is aggressively evolving a "game changing" system of underground development mining machinery. This entry-development system (EDS) project is concentrated on providing clean, protected and ergonomic operator areas and a quantum leap in productivity. To meet these goals, a virtually continuous and automated mining system is needed. Operators must be able to efficiently and ergonomically perform required functions from a protected environment. Health-related issues, such as respirable dust and noise, must also be addressed. A new concept system of integrated face equipment, from the continuous miner to and including advancing the section belt and services, is needed. Phase I of the project was aimed at developing the bolting technology required. Through acquisition of Cram Australia and joint development efforts with Canyon Fuel Co., Joy now has this capability. The test platform, known as the Joy Multibolter, has successfully completed its trials at Canyon Fuel's SUFCo Mine (Fig. 1). These tests validated the bolting technology and operator protection and ergonomic issues. Phase II includes the development and testing of an integrated mining machine (IMM) capable of advancing a heading 1.5 m (5 ft) while simultaneously cutting a fullface width (single pass) and installing a complete bolt pattern. The IMM, unveiled at MINExpo 2000, incorporates the bolting technology of the Multibolter with automated full-face cutting. This machine was designed to meet Joy's stringent health, operator protection and 300m/d (1,000-ft/d) productivity goals. Phases III through V will include development of processes and integrated haulage, ventilation, power, material handling and section services advancing equipment. Processes and equipment developed will be designed for seamless integration into a complete entry development system. The focus of this paper is the new continuous miner (IMM) development, proposed testing and expectations for the EDS. The IMM is scheduled for commissioning and trials beginning in April 2001 at the SUFCo Mine. SUFCo Mine Canyon Fuel Co. is jointly owned by Arch Western Resources and the Itochu Corp. of Japan. It owns and operates three coal mines in Utah: the SUFCo Mine in Sevier County, the Skyline Mine in Carbon and Emery Counties and the Dougout Canyon Mine in Carbon County. Incorporating state-of-the-art-mining technology and advanced mine management practices, the SUFCo Mine is one of the most efficient underground coal mines in the world. The SUFCo Mine is located approximately 50-km (30 miles) northeast of Salina, UT, in the Wasatch Plateau portion of the Uinta coal basin. The mine property consists of 9,900 hm2 (24,470 acres) with approximately 118 Mt (130 million st) of recoverable reserves. Public utilities and industrial concerns purchase approximately 95% of the coal mined at SUFCo through short- or long-term contracts. Coal reserves are extracted by longwall mining, and the panels are developed using continuous miners. The longwall face at SUFCo Mine is 283-m (930-ft) wide, and the seam height ranges from 2.4 to 4 m (8 to 13 ft). The two development sections are in full operation using Joy 12CM12 continuous miners. Two super longwall panels, under development in the northeast portion of the lease block, are 4,270-m (14,000-ft) long with a 283-m (930-ft) wide longwall face. INTRODUCTION Uranium production in Saskatchewan commenced in 1953 at the Eldorado Nuclear Mine in the Beaverlodge area of North Saskatchewan, followed over the next few years by nineteen other smaller mining operations. Production rose from 453 600 to 2 721 600 kg per annum by 1958. Over supply and the closure of the U.S. market to Canadian uranium led to an equally rapid decline to an annual production which varied from 453 600 to 907 200 kg during the years 1964 to 1976. As the market strengthened there was a resurgence of the Saskatchewan industry and production from one underground and two open pit mines reached 2 809 000 kg in 1980 for a return of 232 000 000 dollars (Table 1). [ ] Future output will be increased by a potential 4 536 000 kg from the Key Lake mine now under construction and scheduled to commence production in 1983. Three other projects are in an advanced stage of planning and are currently in the regulatory review process. Several other prospects are being evaluated. Figure 1 shows the location of uranium mines in Saskatchewan and table 2 illustrates the present status of uranium mines and deposits in Saskatchewan. At present, 1400 workers are employed directly by the uranium mining industry. Saskatchewan has diverse mining conditions ranging from the underground mine of Eldorado Nuclear Limited 1870 m deep with a low grade ore deposit averaging from 0.17-0.20% with extraction by carbonate leaching through an open pit operated by Gulf Minerals with a moderate depth of 110 m and 0.37-0.5% grade to the shallow open pit of Cluff Mining 30 m deep with a very high average ore grade of 7.0%. Extraction at both open pits utilizes acid leach extraction processes, the latter after gravimetric separation. Key Lake mining has commenced development of the next mine to come into production which will have two open pits to a depth of 80 and 175 m with an average 3% grade to be followed by the deep open pit of Canada Wide Mines Ltd. to a depth of 205 m with average grade of 1.5%. One existing mine will transfer its operation from the present open pit to another of slightly higher average grade. From the Saskatchewan point of view, these different operating conditions present varied problems associated with occupational health and safety and concomitant solutions to reducing worker exposure to radiation. REGULATION OF URANIUM MINES In 1946, the Government of Canada enacted the Atomic Energy Control Act which established the Atomic Energy Control Board (AECB) and vested in that board powers to regulate all aspects of uranium mining. In the early years, the AECB relied heavily on provincial occupational health and safety agencies for the surveillance of uranium mining and milling operations, particularly with regard to "conventional" occupational health and safety, the AECB having at the time no specific regulations covering the mining industry. There has been considerable dialogue between federal and provincial governments since, traditionally, occupational health and safety is a provincial jurisdiction. Saskatchewan is strongly opposed to the intrusion of the federal government into the provincial domain, particularly as there is little justification for a federal presence in occupational health and safety in uranium mining and milling. The province does, however, accept the need for federal involvement in certain aspects of the regulation of nuclear industry, for example, international sales, research and security of reserves for domestic use.

In 1998, the Concentrator Modeling Center was established within the Coleraine Minerals Research Lab to develop models for simulation of iron ore processing plants, and to assist plant operators in improving performance of their aging plants. The short-term objective was to develop necessary models for reliable simulation using conventional techniques, while the long-term objective was set as liberation-based modeling and simulation of these plants. The short-term objective was achieved in 2002, after the development of models for magnetic separators, hydroseparators and fine screens. A project funded the US Department of Energy was carried out to demonstrate the capability in improving iron ore processing at the Mittal plant. As a result of this study, a 10% improvement in throughput at the plant was obtained, and simulation became a desired tool for plant operators. A significant step in the long-term objective was reached in 2005 with the development of a liberation model. This was followed by incorporation of models capable of processing liberation data into the software. By early 2007, the Center had the capability of performing liberation-based simulation of entire plants.

Small-mine and contractor Fatal Incidence Rates have been a problem for over 20 years in the coal industry, and the rates persist as unacceptably high. This paper briefly retraces historical information and data on the continuing problem and recounts the many unsuccessful efforts and initiatives to address the problems effectively. Next the results of a review of international literature on addressing the problems are presented. After focusing on a method to pinpoint the trouble spots more specifically through a nested data analysis, suggestions are provided on how the problem may potentially be addressed more systematically and effectively in the U.S.

When modeling predictive pit lake chemistry, groundwater chemistry is integrated with the affected portions of the wall rock and chemistry of the lake in a complex series of mixing. Fractions of the solutions impinge upon fresh, unaltered or fractured and oxidized zones of the wall containing sulfide materials and/or soluble metal/metalloid salts. Changing conditions over the short term (biannually) and long term (decades) can drive reactions allowing for dissolution or precipitation of acid rock drainage and metal leaching (ARD/ML) constituents. Local zonal variations can result in a portion of the pit releasing materials into solution; whereas other portions of the lake might favor the formation of colloids or precipitants. The proportions and character of groundwater can drive the chemistry of the pit lake provided sufficient buffering capacity is available to temper the generation of ARD in the system. For sulfide-rich systems, reduction of oxygen by rapid inundation and consumption by sulfides is key to reducing the ARD/ML. This paper presents several models of a sulfide-rich wall rock system using a range of groundwater chemistries to show these constraints.

Coal mine ground control is the science of the application of rock mechanics principles to coal mining operations. More acceptance of ground control techniques by the US coal mining industry has been seen in the 1980s. It has been demonstrated many times that a proper ground control design is key to a successful coal mining operation. However, numerous in-mine applications of various ground control techniques by the author indicate that cur¬rent techniques need improvement. The biggest problem in coal mine ground control is that miners are dealing with materials of rapidly varying properties and occurrence. And only the exposed surface can be identified positively. Thus, in attempting to solve the ground control problems, many unknown factors are assumed. Some of these problems deriving from such assumptions are discussed here. Coal pillars There are two types of coal pillars - barrier and chain pillars - depending on the purpose. Barrier pillars are those located between panels or sections or mines. Their objective is to isolate the mined-out effects of one from the other. Chain pillars are characteristic of room-and-pillar mining. They are designed to support the overburden load and protect the surrounding entries and crosscuts. Pillar design is the most popular and, frequently, the only ground control technique employed by coal operators for mine design. There are many pillar design formulae available (Bieniawski, 1983; Peng, 1986). Nearly all consider both size and width/height effects. Specifically, pillar strength decreases with increases in pillar size and increases with increase in the ratio of width to height. However, the manner of those changes varies with investigators. Consequently, the pillar size determined varies considerably from investigator to investigator. And the variation grows much more pronounced as the seam depth increases (Fig. 1). The question, then, is which one should be used? One may say each formula is developed for a particular coalfield. A particular formula may have been used to design a pillar, which has not failed. Does that mean it was over designed? Not necessarily, because no one can confirm its stated "safety factor." Recent case studies (Peng, 1986; Tang and Peng, 1988) show that all pillar formulae are too conservative for chain pillar design in room-and-pillar mines due to the following two problems: • Uncertainty about in situ pillar strength. All formulae employ uniaxial compressive strength of selected samples tested in the laboratory and scaled to that for underground pillars. The scaling factors are not well proven. In situ testings by Bieniawski (1969) suggested that 1.5 m (5 ft) pillars are representative of in situ pillars that, after being tested in various US coal mines, are not universally applicable. • Uncertainty about the actual loading on the pillar. The most frequently used tributary loading theory defines the loading as total overburden weight tributary to each pillar (from roofline to surface). This is too conservative when it applies to coal measure rocks. The overburden consists of layered strata of different stiffness. Many stiffer strata will self-support over small roof spans. Depending on the location of these strata above the coal seam, it will reduce pillar loading in various proportions. It follows that total structural analysis using, for instance, finite element method is more appropriate. It can take into account the interaction of roof and floor strata with coal. Loudoun Water is currently implementing a quarry to reservoir conversion that will improve water supply reliability in periods of drought and flexibility in water supply operations when turbidity in its Potomac River supplies are high. The Milestone Reservoir Project consists of an inter-tie to the existing Potomac River Pipeline; new pipeline and tunnel; and a pumped intake and outlet works. Planned construction involves significant risks due to underground conditions which includes hard, igneous rock and high groundwater at the creek crossing. In addition, the site is constrained by Goose Creek, an adjacent residence, active mining in a quarry next to the new reservoir, public use of adjacent park space, and the Washington and Old Dominion Trail. Working with these constraints will require close coordination with active mining operations and public and private property owners. Safety of personnel involved with construction also represents a concern with quarry high walls over 30 meters in height near the planned construction. In consideration of key projects risks, Loudoun Water determined that an innovative CMAR delivery approach provided the best opportunity to achieve its goals in quality, innovation, front-end cost and schedule certainty, and operational flexibility and reliability. What matters most to Loudoun Water is the value in having a builder participate in the design process where key decisions can be made based on consideration of risks, cost, schedule, quality, and operability and maintainability of the system. While a progressive design-build delivery model may have produced similar outcomes, Loudoun Water had already invested in substantial development of the design for the work, and ultimately, CMAR provided Loudoun Water the opportunity to retain the partnership with the design team while gaining valuable input from a contractor. Among the challenges with CMAR in the Commonwealth of Virginia is a substantial limitation on self-performance corresponding to 10 percent of the construction value of the work. A significant industry outreach program was initiated by Loudoun Water to encourage participation with incentives in contract terms that allowed for the CMAR Contractor to make a reasonable profit. This paper reviews the CMAR procurement process and discusses contract incentives that were adopted to promote the work within the constraints of contract law in Virginia.

As the popularity of precast concrete segmental linings for tunnels is growing in North America, some problems related to grouting of, and near, these linings are starting to appear. The installation of a segmental lining behind a TBM requires that the annular space around the lining be quickly filled with grout to prevent collapse of the ground. In practice, the filling of the annulus is frequently delayed and takes place a distance behind the machine typically via grout ports centrally located in the segments. If partial collapse of the annulus has occurred, it may become necessary to use higher grouting pressures in an attempt to distribute the grout. There have been several occasions where these increased pressures have caused damage to the tunnel linings. Similarly, compaction grouting adjacent to a tunnel lining can cause damage. This paper quantifies the effects of such grouting on a typical precast lining.

A big issue in crushing and grinding operations in raw materials handling is high energy consumption and low breakage effectiveness. According to the Legendre [7], only 10% of energy input is directly consumed on the feed material breakage, while the remaining portion is dissipated in the form of heat and noise [8]. In the industry of limestone powder production, the grinding operations constitute integral part of technological circuits, and the energy consumption in operations of industrial comminution consume as high as 2% of world energy [14]. High-pressure grinding is the technology with proved energy benefits both in ore processing circuits and in limestone grinding. The promising results concerned mostly the achieved energy savings and good results of material breakage were achieved and demonstrated in numerous investigations [1,3,12]. However, it appears that a proper selection of operational parameters of the press devise, determination the feed material characteristics and adjustment of process course san be of a key-significance in improving an effectiveness the HPGR operation. Operational parameters of devices and process course should be verified towards the characteristics of the feed material, which mainly applies to the particle size and shape of the feed material and its moisture, and also mineralization in case of ores [2,4,9]. There are various investigations concerning an impact of the operational pressing force on comminution results which mostly refer to the determination of energy consumption and crushing ratio [5,6,10,13]. Investigations on the moisture content are less common, however their results suggest that, in general, the HPGR feed with higher moisture shows the tendency to higher comminution intensity, while the energy consumption increases together with increasing of feed material moisture. Investigations on olivine sand feed [11] showed that crushing ratio decreases for higher moisture, especially when the material is crushed at higher pressing force. HPGR devices, however, shouldn’t be regarded as a unique and perfect solution for comminution of each type of material. But they indeed may bring technological and economic benefits, comparing to conventional crushing and grinding machines, resulting in more favourable breakage of material particles and lower consumption of energy per unit mass of processed feed material.

The combustion of pulverized coal is a century-old practice. Many people have investigated coal dust and the residual fly ash particles that re-main after combustion. But the combustion process and fly ash formation mechanism are not under-stood. Working at our Maysville, Kentucky lime plant, the Dravo Lime Company Research team has collected samples of partially burned coal dust from the flame of a pulverized coal burner. Specimens were placed in a scanning electron microscope (SEM) equipped with an energy dispersive spectrometer (EDS). Microscopic examination of the particles, together with a large collection of micrographs of lime dust fused to fly ash particles, now make it possible to theorize as to how combustion advances through a piece of coal dust. Coal is a physical mixture of carbon-based compounds interspersed with bits of ash-forming materials, primarily alumina-silicate compounds. The alumina-silicate materials are commonly in angular nodules measuring 100 A to 700 A across their major dimension. It would appear that if the coal particle is in a well mixed gas stream, combustion starts at the surface and advances radially toward the particle center. With poor gas mixing, it is common that combustion moves from one end of a particle and advances on a broad front almost normal to some axis. In either case the combustion process and ash-forming mechanism are the same. As combustion occurs the carbon-based compounds oxidize in a highly exothermic reaction. Heat from the re-action melts the alumina-silicate nodules causing them to flow together and coalesce into an open sponge-like structure. As melting continues surface tension of the liquid and free surface energy causes the sponge-like structure to simplify into fewer, larger pores. Most likely the lighter molecular weight hydro-carbons burn before the heavier species. This means that as the melting ash structure is metamorphosing to a single large pore, there continues to be out-gasing throughout the structure due to combustion. Surface tension will form a spherical piece of fly ash if the ash material is liquid for a sufficient period of time. Out-gasing from combustion of the heavier hydro-carbons will provide internal gas pressure to keep the particle spherical until it freezes.
