"Risk quantification may be used as a sustainable development index in the mining industry, since it encompasses both economic and social factors. In addition, it may assist in the development of a framework, which is also indirectly related to sustainable development, to prevent accidents and improve health and safety standards. However, risk quantification in the mineral sector involves diverse activities and is considered a very difficult task. Although statistical data report accident severity rates, incident rates and accident frequency rates, none of these allow for accurate risk calculations. In this paper, two risk calculation formulas, which may be used to quantify risk, as well as to identify hazardous activities in a workplace, are proposed. Numerical examples are given based on statistical data available through the MSHA database. Also a combination of a decision/fault/event tree that allows risk quantification is presented."

One of the biggest challenges for the mining industry is, quite literally, how to the keep the equipment rolling. There is a limited amount of companies that manufacture the 4.2-m- (14-ft-) tall, 5,670-kg (12,500-lb-) tires needed for the haul trucks that are approaching the 362 t (400 st) pay-load class, such as Terex?s MT6300-AC and Caterpillar?s 797F. And, while these might be the most impressive tires, they are not the only ones that are in short supply. The shortage of tires has affected just about every kind of machine, and the demand for those tires has been extremely high for more than five years with no end to the shortage in site. American Tire Corp. (www.americantire.us) is one of the companies working to supply tires to the mining industry. It announced that the demand for its 1.6-m (63-in.) tires that have a zero failure history globally is great and that the company signed contracts worth more than $500 million at MinExpo. The Getchell Mine is located on the east flank of the Osgood Mountain Range in the Potosi Mining District, Humboldt County, NV. Mining in the area first began in 1883 for copper, lead and silver in skarn-related deposits. Tungsten was discovered in the skarns in 1916 and was mined sporadically until 1957. The first major discovery was made in 1933 when two Winnemucca prospectors, Edward Knight and Emmett Chase, found gold at the present site of the Getchell gold mine. In 1934, after a small amount of preliminary prospecting, the claims were purchased by Noble H. Getchell of Midas, NV. The Getchell Mine Inc. was organized and exploration and development work began on a broad scale. Major funding was provided by George Wingfield and investors he assembled. Laxalt described Wingfield’s immediate history prior to his financing of Getchell as follows.

The Brightwater conveyance system includes 26 km of tunnels, a 130 mgd pump station, 2 km of micro tunnels, and a 2.4 km outfall. The tunnels extend from Woodinville to Puget Sound through varying soils in 1 to 7 bars of hydrostatic pressure and depths to 185 m. The tunnel diameters range from 4 to 6 m and include numerous lining systems. This paper highlights major shaft and tunnel design, including dual cellshafts, multiple pipe configuration tunnels, polymer concrete segments, and the use of a one pass lining designed to convey high pressure effluent.

Domestic bauxite used in aluminum production comes from Arkansas where syenite was lateritized forming bauxite. Part of the residual bauxite and kaolinized syenite were eroded, deposited downdip, lateritized with bauxite formed in place, and covered by sediments. The unconsolidated overburden causes problems in mining. Bauxite for refractories and chemicals comes from Alabama and Georgia. Sediments were deposited in karst lakes and were lateritized forming bauxite and kaolin. Ferruginous bauxite in Washington, Oregon and Hawaii, derived from weathered basalt, can be used in aluminum production.

Phase One of Copenhagen Metro opened on October 19th, 2002 and is already a great success. 11 stations are open and Phase Two with 6 additional stations will open during 2003. Phases One and Two include 21 km dual track, whereof 10 km is in tunnel. 17 stations (six deep, three shallow, and eight above ground) will service passengers with trains running at 90 seconds intervals. This paper will discuss the various aspects of the Metro, where construction methods include TBM bored, NATM, Cut & Cover, Secant Piles and Diaphragm Walls.

In July 1997, Kvaerner Environmental signed a unique contract with Echo Bay Alaska called the Transfer Risk and Accelerate Closure (TRAC). The contract called for Kvaerner to assume the responsibilities and obligations associated with closeout of the Alaska-Juneau (A-J) Mine. Kvaerner is an international engineering and construction company with global interdisciplinary experience in the mining industry and environmental remediation. Echo Bay’s requirements called for Kvaerner to: •Provide a fixed price for closure with no change orders. •Negotiate, prepare and implement a final closeout plan as outlined by Echo Bay. •Complete a closure program that exceeded current regulatory requirements. •Improve, expand and maintain a proactive public and community relations program. •Hire Echo Bay employees where possible. •Provide adequate performance guarantees and judicial relief of environmental claims.

Ground freezing occupies a unique and important niche in underground construction, fulfilling a geotechnical need that is difficult, or sometimes impossible, to fulfill through other groundwater control and excavation support methods. Where complete groundwater cut-off and in-built ground support are required, and deep excavation is planned or the work occurs in difficult or disturbed ground, ground freezing offers the assurance critical to safe and timely construction. Compared with other groundwater cut-off or excavation support methods, a frozen wall is easily connected to underlying bedrock and will also adhere to adjoining subsurface installations or obstructions, if necessary, to provide a composite cutoff structure. Ground freezing is effective in all soil and ground conditions and has been used for emergency situations in disturbed, displaced or loose ground, where detailed soil characterization could not practically be performed.

The Kaldo furnace is a type of top-blown, rotary reactor that is widely used for the production of precious metals. During the production of doré, a gas jet of compressed air is injected through a water-cooled lance to impinge upon the molten bath surface in the presence of molten metal and a slag phase. A common problem during operation is the difficulty in controlling the distance that the injection lance can penetrate through the reactor mouth without being splashed by the molten phases. Splashing may produce accretions on the lance surface as well as plugging of the nozzle at the lance tip. An additional problem may occur when the gas jet impinges upon the furnace wall and causes rapid wearing of the refractory linings. Because the conditions prevailing in the Kaldo furnace offers poor visibility to the human eye, its operation mostly relies on the empirical knowledge of plant operators and process engineers. A systematic study in a scale model of the Kaldo furnace under controlled laboratory conditions may be valuable to estimate the maximum penetration distance of the injection lance, and thus help improve the operation of an industrial reactor. From a fundamental perspective, the observation of the basic features of the flow patterns in the scale model may help elucidate the relevant phenomena governing the hydrodynamics in this type of system.

The Bureau of Mines has investigated the fundamentals of the flocculation process. The objective of this study was to determine the effects of various physico-chemical parameters such as shear rate, polymer dosage, and polymer concentration on floc growth and to determine the applicability of a model in predicting polymer flocculation behavior. A rapid in-line particle size analyzer was used to follow the progress of both the floc growth and the breakage as a function of time. A genetic algorithm (GA) was used to determine the unknown parameters in the model and to fit a curve to the experimental data. Results show that the model, using GA as a tool to determine the unknown parameters, reproduces the experimental data with only reasonable accuracy and was relatively insensitive to the parameters. Results of the flocculation study showed that increases in polymer dosage had greater effect on the time required to reach the maximum floc size than on the floc size itself. It was also determined that, given all the aforementioned parameters, the degree of slurry mixing during the flocculation process had the greatest bearing on the floc growth.

In 1986 the city of Johannesburg celebrates its first century of existence. The fact that the city was founded to exploit the gold deposits which underlaid it, prompts this review of the gold process development in the mining industry of South Africa. Clearly a complete history of, the gold recovery process to date is not called for and it is proposed therefore to outline the most significant developments which have taken place since the middle 1930's, that is to say the last 50 years or so. Prior to this period the industry had seen the development of comminution processes employing cone crushing in closed circuit with vibrating screens followed by grinding in rotating mills and the gradual disappearance of stamp mills. The all sliming process was well established as a viable route and continuous dewatering in thickeners, continuous leaching and filtration were serious rivals for the earlier batch processes. Continuous processes for gravity concentration had also been developed and the recovery of gold from pregnant solutions employed cementation on zinc dust as opposed to shavings. Thus at the beginning of the half century under review the industry had adopted, or started to adopt, many of the processes still accepted as modern practice. However in some areas little change was evident this was especially so in the smelthouse where some thirty more years were to elapse before the hand stoked coal fired calcine and reverbatory furnaces were replaced by more environmentally acceptable electric furnaces. The most significant feature in recent history has been the mining of progressively lower grades of ore and the retreatment of tailings from earlier operations, much of the latter comprising tailings only a decade or even less in age. Notable among these operations are those on the East Rand near Springs in the Transvaal, in the Orange Free State at Welkom and more recently, massive reclamation of tailings close to the city centre itself. The carbon in pulp (CIP) process has been much publicised in recent years and all new plants erected during the 1980's have adopted this route for gold recovery. However the major proportion of tonnage milled in the industry is still subjected to the more traditional process of liquid solids separation by vacuum filtration. The implementation of the Surface Mining Control and Reclamation Act of 1977 (SMCRA) has evolved into one of the most complex environmental protection programs to come out of the late 1970's. The level of detail in the statute and the regulations have resulted in a rigid and complex set of requirements for the administration, permit-ting, inspection and enforcement of the program. In addition, the constant change of the federal program caused by regulatory changes, reorganization of the Office of Sur-face Mining Reclamation and Enforcement (OSMRE), ongoing litigation involving OSMRE, and the use of the budget to establish additional mandates on OSMRE and the State Regulatory Authorities have resulted in an unstable program. The instability of the program makes it extremely difficult for the States to implement their programs and to achieve the goals of SMCRA.

"The Bypass Tunnel has high head conditions ranging from a minimum of 600 feet (183 m) under the Hudson River to a maximum of 875 feet (267 m) on the west side. The ground cover ranges from 425 feet to 900 feet (130 to 274 m). A two-pass lining is designed for the Bypass Tunnel. The bolted, gasketed precast segments carry the temporary hydrostatic and permanent ground loads. After the tunnel is put into service, the design assumes that the final lining, which varies from reinforced concrete to reinforced concrete with steel interliner, will carry the permanent hydrostatic loads. The design head for the segments is assumed at 775 feet (236 m), and the internal operating pressure head is around 1,200 feet (366 m). This paper discusses the design hydrostatic head and different load conditions considered for the segments, the load sharing between the segments and final lining, and the means employed to control water and hydrostatic pressure. PROJECT BACKGROUND The Rondout-West Branch Tunnel (RWBT), a segment of the Delaware Aqueduct, was built from 1937 to 1944 and provides about 50% of New York City’s total water supply. Monitoring and tunnel operations have shown that the RWBT is leaking up to 35 million gallons per day (MGD) (132.5 million L/day). In particular, two locations have been identified as areas of concern: Roseton and Wawarsing, as shown in Figure 1. The Wawarsing reach of the aqueduct will be repaired with an extensive grouting program when the RWBT is unwatered. However, because of the extensive nature of the leakage in the Roseton area, a Bypass Tunnel will be constructed to divert the flow around this area."

INTRODUCTION Radcont is a program designed by the author of this paper for the industry to use as an instrument for radiation contamination evaluation and ventilation planning system. Radiation in mines are associated with the mining of gold and gold bearing minerals, as uranium and thorium is incorporated in the mining of these minerals. Radiation contamination in South African mines is not a new concept as it was investigated by the Chamber of Mines in the early 1960's and found not to be hazardous at the time. Since some of our mines export scrap metal to customers abroad, it came to light (1991) that some of the scrap metal was radioactive. The authority that oversees the nuclear aspects in South Africa is the Council for Nuclear Safety (CNS). They investigated these matters and found that the mines needed further information regarding radioactive material and the handling of these contaminated materials. As the various mines were licensed (with various conditions incorporated) thereafter, the mines had to do their own investigations as to what extent their properties (Surface and underground) were radioactively contaminated. Some mines were found to be highly contaminated over the years of operation and controlling conditions were installed and measures installed to reduce the contamination levels. One of the conditions when issuing a licence by the Council for Nuclear Safety (CNS), is that a screening survey be carried out to determine the radiation exposure levels and corrective action to be taken if necessary. These surveys must be done by a person trained in the required procedures for such a survey. The person must also measure the risk correctly and assess the results properly. In such a survey, the internal and external exposure levels must be determined to assess the total exposure of persons working in those conditions and take appropriate action if necessary. When doing such a survey, hundreds and more likely, thou- sands of data points are recorded. In order to assess the data recorded, various integrated and difficult calculations need to be made, and takes up enormous amounts of time. (This excludes the interpretation of the results ) The following explanation of the program shows the different parts of such a survey assessment calculations to be done. The paper details the program layout and the different sub- sections within the primary program. It must be stated that the program, as with any other program, is as accurate as the data inserted into the data base. The program and details thereof are given under the following headings: 1. TOTAL EFFECTIVE DOSAGE WITH REGARDS TO: • GME required gravimetric results obtained (mg/m3) • Thick layer or total contamination measured (Bq/m2) • Dry condition surveys with dust loads taken as a Standard l0mg/m3 • Wet conditions survey with dust loads taken as l mg/m3 • Airborne long lived alpha and beta activities as determined by analysis in Bg/m3 • LTD (Thermoluminescent Dosimeter). Results as obtained from the SABS (South African Buro of Standards) are recorded in this section for each month of the year for each individual worker. An average dose is then determined at the end of the year. • Bucket measurements as recorded. • Smear samples (Loose contamination). As determined by Electra or by analysis • Occupational factors for Metallurgical and Engineering occupations in and around the Metallurgical facilities of your mine. • All underground dosage determination and calculations. (Radon and Thoron) 2. INFORMATION REQUIRED WHEN PROGRAM IS INITIALISED: As the program is started, it opens up on the contents page. Here there are various options to choose from, but one is cautioned as a beginner in operating the program, not to perform any tasks before carefully reading these instructions. Firstly, one must go to the 'Information required" pushbutton. Press this button. The information required page is shown where the cursor can be moved to the block where one can enter the specific mines name. To enter a mines name, put the cursor in the block provided and just insert the mines name with the normal keyboard keys and press the enter button on the computer keyboard. To enter the other information required such as Alpha and Beta instrument efficiency, ALI (Annual limit of intake) and probe area, one can either press the 'Data required" button for a dialog box information or enter it manually by just putting the cursor in the block provided and entering as did above. In order to insert all the required information for the pro- gram to calculate the information required, one must proceed further by entering the area names surveyed in the spaces provided. There are 20 spaces to enter 20 different areas surveyed. One must further also provide the amount of days worked in each area (i.8. 250) in the block provided. The de- fault is 250 days. The grinding mill is probably the most common unit operation used in mineral processing. There is, however, much which can still be done in the way of optimization. This paper discusses the results of a well controlled pilot scale test in which the performance of a tumbling mill with a conventional rubber lining equipped with lifter bars is compared to that with a smooth rubber lining of the magnetic type. The results indicate that the smooth lining is about 14 percent more effective with respect to energy consumption. By optimizing the size of the grinding media, the mill capacity can be increased in many grinding operations. In this specific case the -20 mm ball charge required 26 percent less grinding energy than one with a -30 mm ball charge. The energy savings can. in most cases, be converted to increased capacity. The wear of the grinding media is difficult to assess over a short period test but the results suggest it is somewhat less when a smooth lining is used. The wear of the linings has not been included in this study but it is generally accepted that the use of the magnetic lining reduces the lining costs dramatically. Computer simulation using the Millsoft code, developed by Mishra e.a. confirms and explains the increased effectiveness of comminution when smooth lining and smaller grinding balls are used.
