"Broken and jointed ground is extremely hazardous and requires unique stabilization techniques to protect worker and critical entries. Bolting techniques, even the most advanced with pre-load to create roof compression, can leave large gaps of unconfined and separated roof. This roof can also be susceptible to water inflows which only exacerbate the support problem. These types of conditions existed on long term development main entry accessing several future longwall panels. The geotechnical assessment identified well-defined jointing systems that also permitted water to flow between the roof separations, which “lubricated” the roof material and minimized any frictional advantages. Chemical injection of a Polyurethane material, Minova Carbopur WS, was placed beyond the bolted horizon to move the water to the rib lines and reinforce the fractured and broken roof. The injection material permitted the effective use of bolting patterns and helped ensure long-term stability of these critical entries. The paper describes the evaluation techniques, design patterns, basis for chemical selection and application, and results and recommendations.BACKGROUNDThe injection of mine and underground civil structures using polyurethane (PUR) has been ongoing for over 50 years. The applications in mining have included gate road stabilizations, longwall set-up and tear-down entries, and simple consolidation of coal mine roofs. PURs have also been used for roof stabilization during longwall shield recoveries. Polyurethane injection was initially developed by the Bergbau-Forshung GmbH, a German coal mine research institute, in the early 1960s (Jankowski, 1972). The product became a standard stabilization method in Germany after its commercial introduction in 1971 (Knoblauch, 1994), and was successfully manufactured at the Minova Essen, Germany location for over 50 years. With the recent closing of the German underground mines, that manufacturing facility has been closed and moved to Katowice, Poland, where production continues today Injection products are also being successfully manufactured by Minova in Australia and the United States.Polyurethane injection in underground coal mines is used in difficult ground conditions in fractured immediate and main roofs. It is also common as a rock stabilization remedy to prevent longwall face caving during shield removal. As presented in the case study, polyurethane was applied to minimize the inflow of water from beneath a stream channel while providing reinforcement and consolidation of the fractured roof that had lost confinement. Polyurethane is a two-component system (Part A and Part B) that is injected into a borehole and can chemically bond to the rock, not relying on friction or mechanical interlock to provide support. The material is injected under pressure, coupled with a low viscosity and small particle size; it flows into roof fractures and separations, which are the paths of least resistance. New developments in low viscosity PUR materials allows penetration of cracks as tight as 0.002 inch (0.05 mm) wide. This also allows for the closures of extremely small cracks to minimize the inflow of water, even if the water is under pressure. Combinations of designed expansion properties and low viscosities make the materials ideal for minimizing and stopping flows of water from underground structures."

In February of 1989 the Mission Complex of ASARCO Incorporated completed an expansion of its concentrator, raising the base mining rate from 25,900 TPO (28,500 STPD) to 37,200 TPO (41,000 STPD). The expansion was accomplished by extending the twelve existing Mission ball mills in length, by installing the grinding circuit from Asarco's idled Sacaton Concentrator along with various auxiliary equipment, and by modifications to the flotation circuit and water systems. This paper will give a brief history of the consolidation of the ore body which made the expansion possible and explain the various parts of the expansion.

This paper describes the technical status of the atmospheric pressure fluidized bed combustion (AFBC) process for utility steam generation. Major areas where AFBC development work is needed are discussed. Various approaches to solving these key issues are identified and the ongoing research and development programs addressing the key development areas are described. Other significant programs pertaining to the development of AFBC for utility steam generation are also discussed and needs for future work identified.

The lean, but extensive iron ore deposits of the Dunderland Valley north of Mo i Rana, Norway, is the raw material for the Rana Mines which is a division of the state owned steel mill A/S Norsk Jernverk. Activities for the exploitation of the orebodies were commenced in 1902 by the English Dunderland Iron Ore Company. During its operating period the company constructed a total of two large scale concentrating plants. The activities were strongly marked by technical problems and labor disputes. Thus actual production was limited to the years 1904-1908, 1928-1931 and 1937-1939. During these periods a total of 1.9 million tons of crude ore was mined and 0.6 million tons of concentrates produced for export. The present plant commenced operations in late fall of 1964. Up to and including 1976 about 26.6 million tons of crude have been put through the concentrating plant and about 10.7 million tons of concentrate produced.

Through a regimen of process control and documentation, routine audits and a commitment to quality management, this kaolin producer is a competitive pigment supplier in a worldwide market. Background In 1987, the International Organization for Standards developed the ISO 9000 quality management system. This series of standards is used as a means to ensure customers that a quality management system is in place. Certification of a company does not mean that an individual product is of high quality. Rather, it demonstrates a supplier's ability to meet its customers' product and service needs. It also shows that a company has documented records to assure product consistency. The ISO 9000 system was begun in England during the early 1980s. During the mid-1970s, the British manufacturing industry was experiencing low quality. In an attempt to boost quality standards, the British government established the BS 5750 standards system. In 1987, BS 5750 was incorporated into ISO 9000. The ISO 9000 series consists of three levels of certification. ISO 9001 is the model for quality assurance in "design/development, production, installation and servicing." This standard applies to companies that rely on research and development to bring products that frequently change to market. ISO 9002 involves "production and installation" only. This standard is relevant to industries that produce a specific product according to specifications - either their own or their customer's. ISO 9003 is the model in "final inspection and test." This standard is the least detailed of the 9000 series. It is also the most applicable to companies that purchase, inspect or sort and resell. The quality system requirements for certification are outlined in the 18 elements of ISO 9002. These include: 4.1 - Management responsibility. 4.2 - Quality systems. 4.3 - Contract review. 4.4 - Design control. 4.5 - Document and data control. 4.6 - Purchasing. 4.7 - Control of customer-supplied product. 4.8 - Product identification and traceability. 4.9 - Process control. 4.10 - Inspection and testing. 4.11- Control of inspection, measuring and test equipment. 4.12 - Inspection and test status. 4.13 - Control of nonconforming product. 4.14 - Corrective and preventive action. 4.15 - Handling, storage, packaging, preservation and delivery. 4.16 - Control of quality records. 4.17 - Internal quality audits. 4.18 -Training. Continuous improvement, though implied in maintaining these requirements, could potentially be included in the quality system requirements in the next revision. Roping in hydrocyclones is a well-known phenomenon in mining and mineral industries which put serious restrictions on their continuous and maintenance free operation. Roping is mostly observed in association with high underflow solid concentrations which can be attributed to higher fraction of solids reporting to the underflow or the feed solid concentration itself being higher. This paper proposes a newly designed spigot with a truncated conical geometry which can allow for a rope-free discharge at higher feed solid concentrations. The experimental observations revealed that in the operation of a 50.8-mm-diameter hydrocyclone, roping ensued at 10% to 12.5% (wt%) feed solid concentration depending on feed inlet pressure for the cylindrical spigot, whereas with the modified spigot, rope free discharge was attained even at 25% (wt%) feed solid concentration.

The Orchard Valley Mine, a large underground coal mine located in western Colorado, experienced a major mine fire during an idled Sunday on June 1, 1986. This mine had extensive problems with spontaneous combustion near the coal outcrop. Spontaneous combustion has been determined to be the probable cause of that fire. Preparing for a mine emergency was an on-going task. Once the emergency occurred, much of the preparation work was implemented and aided in controlling the situation. Later, the successes and shortfalls that arose from that emergency were analyzed for future course of action. This paper summarizes the activity that led up to the major fire, the handling of that emergency, the lessons learned, and the changes implemented.

Minerals contribute to U.S. gross domestic product (GDP) at several levels ? from mining and processing to the manufacturing of Anished products. In 2009, the estimated real GDP of the United States decreased by 2.4 percent compared with, and the estimated nominal GDP was about $14.2 trillion (Table 1). Trends in various sectors of the domestic economy are often reflected in mineral production and consumption rates. In 2009, the continued decline of the housing market was reflected in reductions in the production and consumption of cement, clays, construction sand and gravel, and crushed stone (those mineral commodities that are used almost exclusively in construction). Production of mineral commodities associated with the manufacture of goods for the housing market, such as ceramic tile, heating and air conditioning equipment, paint, plumbing and sanitaryware, roofing, wiring and wallboard also decreased. The 2003 Rapid Excavation and Tunneling Conference (RETC) was held at the New Orleans Marriott, New Orleans, LA, June 15-18. The New Orleans conference was the 16th since the RETC was formed in 1971. This year’s conference was sponsored by the Society for Mining, Metallurgy, and Exploration, Inc.(SME), with co-sponsorship of the Construction Institute of ASCE. Attendance at the meeting was 1,178, a conference record. This was up significantly from the771 attendees at the 2001 meeting in San Diego, CA. The RETC was formed to “advance the engineering and scientific knowledge of rapid underground excavation and tunneling through interdisciplinary meetings, publications, technical-educational exhibitions and encouragement of research.” The RETC conferences are now held every other year. They provide an opportunity for individuals to meet with designers, contractors and manufacturers associated with the tunneling industry to share experiences in all phases of tunnel design and construction.

"The Coeur d’Alene mining district in northern Idaho was first discovered more than 130 years ago and has been the most prolific silver producer in the United States. The Lucky Friday Mine of Hecla Mining Co. (Fig. 1) is the oldest (76 years) and deepest mine in the district, producing approximately 725 t/day (800 stpd) from narrow, vertical lead-zinc-silver veins. Cut-and-fill stoping is currently employed at depths to about 2.3 km (7,500 ft) below ground surface, making it one of the three deepest mines in the western hemisphere. The #4 Shaft, a winze collared at approximately 1.8 km (5,900 ft) below surface and extending to a depth of nearly 2.9 km (9,600 ft), was recently completed to exploit the high-grade 30 Vein, whose resource has been identified to at least 3 km (9,700 ft) in depth.As with other deep mines worldwide, a number of challenging engineering issues occur, including: high in situ rock stress, squeezing ground, seismicity, high in situ rock temperature that requires refrigeration, diesel equipment that generates diesel particulate matter (DPM) requiring high ventilation airflow rates and long travel times for worker transport to mining areas.This article describes the methods that Hecla Mining Co. has employed during the past 50 years to combat these issues, with emphasis on the current efforts to implement continuous, mechanical mining with automated or teleremote operation and to introduce electric and battery-powered equipment to minimize refrigeration and ventilation requirements for the removal of DPMs.Geology and in situ stressThe host rock mass is an extremely thick sedimentary sequence (the Belt Series) that has been tilted vertically by mountain building. The rock mass in the current section of the mine consists of weak, heavily foliated argillites and stronger, bedded silty-argillites termed siltite. The vertical veins (80° to 85° dip) are parallel to the foliation with an approximate width of around 3 m (10 ft). There are two regional, vertical faults that parallel the vein; one forming the hangingwall of the orebody and one approximately 90 m (300 ft) in the footwall. Additionally, there are several thin, rough northdipping faults that occur in the footwall of the vein which terminate on the major vertical faults.Both the argillite and vein package rock have moderate strength, with uniaxial compressive strength averaging about 100 MPa (14,700 psi with a range from about 70 to 125 MPa), and are moderately abrasive, with CERCHAR abrasivity index of 2 to 3. The major in situ stress is horizontal and oriented in a northwest direction with magnitude about 1.5 times the vertical stress. Consequently, the Lucky Friday stress magnitude at depth is analogous to significantly deeper South African gold operations."

"NIOSH’s mine fire simulation program, MFIRE, is widely accepted as a standard for assessing and predicting the impact of a fire on the mine ventilation system and the spread of fire contaminants in coal and metal/nonmetal mines, which has been used by U.S. and international companies to simulate fires for planning and response purposes. MFIRE is a dynamic, transient-state, mine ventilation network simulation program that performs normal planning calculations. It can also be used to analyze ventilation networks under thermal and mechanical influence such as changes in ventilation parameters, external influences such as changes in temperature, and internal influences such as a fire. The program output can be used to analyze the effects of these influences on the ventilation system. Since its original development by Michigan Technological University for the Bureau of Mines in the 1970s, several updates have been released over the years. In 2012, NIOSH completed a major redesign and restructuring of the program with the release of MFIRE 3.0. MFIRE’s outdated FORTRAN programming language was replaced with an object-oriented C++ language and packaged into a dynamic link library (DLL). However, the MFIRE 3.0 release made no attempt to change or improve the fire modeling algorithms inherited from its previous version, MFIRE 2.20. This paper reports on improvements that have been made to the fire modeling capabilities of MFIRE 3.0 since its release. These improvements include the addition of fire source models of the t-squared fire and heat release rate curve data file, the addition of a moving fire source for conveyor belt fire simulations, improvement of the fire location algorithm, and the identification and prediction of smoke rollback phenomena. All the improvements discussed in this paper will be termed as MFIRE 3.1 and released by NIOSH in the near future INTRODUCTION The MFIRE program, originally developed in the 1970s by the U.S. Bureau of Mines (now the Office of Mine Safety and Health Research (OMSHR) at the National Institute for Occupational Safety and Health (NIOSH)) and Michigan Technological University, has been a mainstay in modeling of the mutual influences between a fire and the mine ventilation system during an underground mine fire. Several update versions have been released over the years, including MFIRE 1.27, 1.29, 1.30, 2.0/2.01, 2.10, and 2.20. The MFIRE program logically comprises four parts: (1) a conventional network calculation, where it performs the basic network balancing without considering heat or mass transfer; (2) a temperature calculation to establish the reference temperature distribution before a non-steady state (transient-state) simulation; (3) a transient-state simulation that follows changes in ventilation step-by-step to produce a continuous description of the temperature distribution, smoke, and contaminant spread through the ventilation system during a fire event; and (4) a quasi-equilibrium simulation to predict the state of the ventilation system after a relatively long period of time (defaulted as 5 hours in MFIRE) as the fire reaches a quasi-steady state. In summary, MFIRE is a computer simulation program that performs normal ventilation network planning calculations and dynamic transient-state simulation of ventilation networks under a variety of conditions including the influence of natural ventilation, fans, fires, or any combination of these (Chang et al., 1990)."

The trip from Denver southwest to Gunnison crosses seven main geologic and physiographic areas as shown on the accompanying map. These are: I Mesozoic and Tertiary sedimentary rocks of the Denver Metro area from downtown Denver to the foothills near Morrison. II Precambrian metamorphic and igneous rocks of the Front Range from near Morrison to the west side of Kenosha Pass. III Sedimentary and volcanic rocks of South Park (Paleozoic, Mesozoic and Tertiary) from the west side of Kenosha Pass to Trout Creek Pass. IV Precambrian granitic rocks (1,350-1,480 M.Y.) between Trout Creek Pass and the Arkansas River. V Tertiary and Quaternary sedimentary rocks and sediments of the Arkansas River valley. VI Precambrian metamorphic and igneous rocks and Paleozoic sedimentary rocks of the Sawatch uplift from Maysville to near Doyleville on the west side of Monarch Pass. VII Mostly sedimentary rocks of Cretaceous and Tertiary age from the Doyleville area west to beyond Gunnison. Each area will be elaborated in the following pages but no detailed road log is given since these are available elsewhere. An attempt has been made to point out some deposits and features of interest to economic geologists but it is by no means complete.

Meramec Mining Company, a joint venture by Bethlehem Steel Corporation and St. Joe Minerals Corporation, is engaged in the mining, concentrating, and pelletizing of iron ore from the Pea Ridge Mine located near Sullivan, Missouri, about 70 miles southwest of St. Louis. The orebody was delineated in the mid-1950's by St. Joe during an exploration program to discover new lead orebodies. An airborne magnetometer survey which was run by the U. S. Geological Survey, the Missouri Geological Survey, and several min¬ing companies was published in 1950 and showed an anomaly in the Pea Ridge area. Test holes drilled in 1953 and 1954 revealed the presence of a large orebody of probable commercial significance; additional drilling in 1956 and 1957 provided confirmation of this. Meramec Mining Company was incorporated in 1957 and shaft sinking began in the latter part of that year. The Pea Ridge orebody lies under about 1300 feet of sediments. It is tabular, about 2600 feet long and up to 600 feet thick, and lies in an approximate east-west delineation. The orebody dips about 800S and is of unknown depth. The ore is primarily high grade magnetite with smaller zones of specular hematite, The Pre-cambrian wall rock is rhyolite porphyry. ABSTRACT Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage organizations to work together, executing such risk assessments comes with challenges. This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

The current economic downturn has been at the center of media attention for several years. Today, the mining and mineral sectors are being impacted by highly fluctuating supply-and-demand conditions, extreme price volatility and tremendous market uncertainty on a world-wide basis. There have been numerous plant closings, project cancellations and employee layoffs. In this gloomy economic environment, it is easy to forget that the world?s gross domestic product (GDP) has more than doubled in real terms during the last 30 years. While current market turbulence may continue for some time, the long-term trend is unmistakably upward. Living standards are rising all over the world, thereby creating demand for food, clothing, transportation and housing. This translates into tremendous demand for the mining and mineral sectors. In the future, new capacity and plant expansions will be necessary to meet this need. These investments, which typically have a life span of 30-plus years, will have to be based on long-term economic considerations rather than current market conditions.

The University of Kentucky conducted inerting tests and angle of ejection tests on three types of rock dust. A newly developed hydrophobic dust was compared against typical dry and wet dust applications. This new product is designed to alleviate respirable dust concerns for dry dusting and caking issues with wet dusting. Samples for both test types were prepared in a laboratory and subjected to temperature and humidity cycles that are typical of an underground coal mine. Inerting tests were conducted in a 38-L chamber with a 5-kJ Sobbe igniter. Angle of ejection testing was conducted at the University of Kentucky Explosives Research Team’s laboratory in Georgetown, KY. The results of these tests indicate that the newly developed dust has improved results for flame suppression and ejection compared with typical wet dust applications. The significance of these results is discussed with regard to existing respirable dust regulations and the issues associated with caking.

"Sound Transit’s East Link Project, a $3.7 billion (2015 $), 14-mile light rail extension, will connect downtown Seattle with the rapidly developing east side cities of Bellevue and Redmond. Among the most technically challenging aspects of the Project is the 2,435 foot long Downtown Bellevue Tunnel (DBT), which will be constructed through the central core of Bellevue underneath 110th Avenue NE using a combination of Sequential Excavation Methods (SEM) and cut and cover tunneling. The 1,980 foot long central SEM component of the DBT will be mined at shallow depth through challenging and complex geology, comprised of glacially overconsolidated soils, containing both regional and perched groundwater conditions and in close proximity to existing high rise buildings. The paper shares lessons learned from the planning decisions and risk based approach to the investigation, design and procurement processes which shaped the DBT configuration and enabled a successful transition to the project’s construction stage. INTRODUCTION The East Link Project is a voter-approved $3.7 billion (2015 $), 14-mile extension of the existing Sound Transit (ST) light rail transit (LRT) system from downtown Seattle, across Lake Washington via I-90, to the cities of Bellevue and Redmond on the east side of Lake Washington. With targeted completion in 2023, East Link will provide riders with an efficient and reliable connection between the largest population and employment centers on the east side and downtown Seattle. The project alignment is indicated in Figure 1 below.ST completed the Preliminary Engineering (PE) phase of the East Link project in late 2011. Subsequently the final design contract for the easternmost seven miles of the project was awarded to the H-J-H Team, a joint venture comprised of HNTB, Jacobs Engineering and Hatch Mott MacDonald in February 2012. H-J-H’s final design scope includes approximately 7 miles of double-track LRT between the east end of the I-90 East Channel Bridge and the Overlake Transit Center Station. While the project scope is extensive and diverse, the focus of this paper is the DBT. The DBT is located immediately north of the East Main Station, and travels approximately one half mile through downtown Bellevue under 110th Avenue NE between Main Street and NE 6th Street; terminating at the Downtown Bellevue Transit Center (BTC) Station. The location and extent of the DBT is indicated in Figure 2."

Engineering properties of rock are essential in most phases of mining from site reconnaissance through mineral processing. Review of the literature indicates, however, that property data is scarce for most coal measure rocks in the United States. The Bureau of Mines, U.S. Department of the Interior, has undertaken a wide-ranging testing program to provide more complete property data for coal strata in the U.S. coal basins. The property data are expected to have application in geophysical exploration, the development of pre- mining and hazard detection probes and instrumentation, mine design, engineering and rock mass classification, subsidence engineering, and ground control. This paper describes the comprehensive program encompassing in situ geological, geophysical, and geotechnical properties and laboratory determinations of acoustic, electromagnetic, physical, mechanical, and index properties and discusses results for 500 ft of coal measures at a minesite in the Northern Appalachian, Eastern Coal Province.

In this study, the Reddy-Blander model was applied to predict the sulfide capacity (CS) values of FeOx-CaO-SiO2, FeOx-MnOx-SiO2 and FeO-MgO-SiO2 ternary slags, which were experimentally studied in the literature within the temperatures between 1,673 and 1,923 K. As expected, sulfide capacity values of the slags increased with increasing temperature and decreased with increasing acidic content. The model was found to be in agree-ment for all slag systems at the temperature range of 1,773-1,923 K. Mr. Chairman; honored guests; ladies and gentlemen: The subject assigned me today by your committee, according to the program, is "the mineral industry and the public land law I have preferred to title the talk, "you can't mine here", for that is the essence of the message we, as an industry, have been getting from so many agencies of the federal government for so long. And, unfortunately, we can expect to hear the same message even louder and clearer -- with increasing frequency in future, unless. Every segment of the industry devotes the necessary time, energy and creative thought to combat successfully the forces that are writing and broadcasting that message. I said that agencies of the federal government are sending the message --- and this, of course, is true, the forest service, for example, and some bureaus of the department of the interior as well as others have appeared in recent years to be abiding by a policy designed to block mineral entry to as much of the public lands as possible and to wipe out existing mining claims.
