The design process must anticipate all construction and operational conditions and make an allowance for all eventualities. Prevention of failure during operational life, and in some cases post abandonment, requires that a risk assessment is undertaken during the design stage which defines not only the factors of safety to be used but the criteria for the structure during its operating life. These operating criteria should clearly identify the at-risk structures and the inspection and monitoring routines required to ensure compliance with the design objectives. On a mine site, it is the role of mine management to ensure that the operating rules be applied throughout the mine life and that a system of inspection and monitoring procedures be instigated. Where the risks are significant, high-level auditing by an independent engineer will also be required. Within the inspection and auditing framework, the designer needs to incorporate instrumentation to enable the operators and management to confirm that the facility meets the requirements of safety, stability and environmental compliance whilst continuing to meet its operational function. This paper reviews the use of inspection and monitoring to prevent failure and provides, in summary, examples of where monitoring has been undertaken and prevented failure. A tabulated review of legislation, regulations and technical guidance generated as a result of previous failures or untoward events is provided.

Will Rogers unwittingly commented on the essence of evaluations when he said: "We don't know what they don't know!" In so doing, he explained why managers have an obligation to verify that their personnel really know what is expected of them. Rogers further stated that "What we know may not be so!" Here, he suggests that managers must establish the correctness of facts on which they make decisions concerning the future of their organizations. Yet, there are some managers who may not have considered this basic wisdom. These are perhaps the ones who shun a solid, fact-finding evaluation. Instead, they trust to guesswork as a basis for attempting improvement. Without realizing it, they may be making incorrect decisions harmful to the very organization they are trying to improve. Unfortunately, an attitude of avoiding evaluations often permeates whole organizations when their managers fail to appreciate their importance. Recently, the following was found prominently displayed in the office of a maintenance manager. Regrettably, the facts of a subsequent evaluation established how imprudent his attitude was:

Water Only cyclones were introduced into commercial operation for cleaning of metallurgical and steam coal around 1950. Since its original introduction as a primary cleaning device, the employment of Water Only cyclones as recleaning and scalping devices has also grown in popularity. Examined are examples and uses of the water only cyclone as primary, re- cleaning, and scalping devices for fine coal cleaning. Also evaluated are conditions that should exist to ensure favorable cleaning in Water Only cyclones.

INTRODUCTION Volcanic tuffs on and adjacent to the Nevada Test Site (NTS) are being considered by the Department of Energy (DOE) for the possible geo- logic disposal of high-level radioactive wastes. The Nevada Nuclear Waste Storage Investigations (NNWSI) project was established in 1977 to evaluate such disposal. Sandia National Laboratories (SNL), one of the participants in the NNWSI project, has as one of its responsibilities the development of a rock mechanics program for the Nevada site. SNL conducted three small-diameter heater experiments in welded and nonwelded tuffs in G-Tunnel on the NTS, to investigate the thermal (heat transfer) and hydrothermal (thermal- induced water migration) behavior as part of the rock mechanics program. These two aspects were investigated to increase knowledge about the behavior of tuff so that predictive capabilities are available for use by repository designers, waste-package designers, and performance analysts. Also, knowledge of the behavior of the tuff is invaluable for planning and designing future in situ experiments at Yucca Mountain, the candidate site for the repository. The evaluations of these three experiments is the subject of this paper. Designers, analysts, and experimenters are seeking a better understanding of the behavior of tuffs under the influence of thermal loads expected from radioactive-waste storage, because of two physical features peculiar to tuffs. First, tuffs are inherently porous because of the manner in which they are deposited (Smith, 1960). Tuffs utilized in G-Tunnel testing have porosities ranging from 15 to 46%, which cover the ranges of expected porosities in the target horizon at Yucca Mountain. It has been established that the G-Tunnel tuffs have similar bulk, thermal, and mechanical material properties to those at Yucca Mountain (Zimmerman et al., 1984), thus, G-Tunnel testing can be used to evaluate the effects of porosity on the thermal and mechanical behavior. The second feature is that tuffs above the water table contain pore water that influences the thermal properties of the rock; displacement of the water must be accounted for in repository designs and analyses. For example, G-Tunnel tuffs, located some 600 m above the static water level, have saturations in excess of 60%. Similar percentages are expected at the target horizon in Yucca Mountain (Zimmerman et al., 1984). The presence of and the removal of the pore water affects the thermal behavior of the tuffs, and these three experiments were designed first to determine the phenomenological behavior and then assess influences on predictive capabilities. The focus in this paper is to present the results and evaluations of the experiments as they apply to improving predictive capabilities. The thermal aspects are emphasized by comparing the measured temperatures (in and around heater emplacement holes) with calculations of predicted temperatures using numerical models, so that the effects of significant parameters can be evaluated and integrated into future predictive models. Observation of the hydrothermal aspects is needed to assess the effects of loss of pore water and subsequent migrations on the temperature fields during the heating, and also to define the expected environment in the vicinity of the outer metallic surface around the waste. The metallic boundary can be either the outer shell of the waste package system, which is the main engineered barrier for waste containment, or the borehole liner. A final decision has not been made as yet about the necessity for lining boreholes; thus, the emphasis is on predicting the characteristics of the environment around the outer metallic surface. With the development of underground mining, the deformation of roadway floor has increasingly become a major constraint on the safe and efficient production of coal mines. To address the floor deformation that was caused by massive stress concentration in roadway, an innovative method based on deep-hole directional tensile blasting technology is put forward. By taking No.122108 working face in Caojiatan Coal Mine as engineering background and analyzing the mechanism of deformation, a numerical simulation and field test are carried out to verify the effect on deformation control of this technology. It is found that the deep-hole directional tensile blasting technology can actively control the fracture position of the roof in the gob area and cut off the stress transmission path, so as to relieve the stress in the roadway. And since different blasting parameters have different impacts on the stress relief effect, a numerical simulation is implemented and it is found that the stress relief effect is the best when blasting depth is set to be 32 m and the blasting angle be 60°. By applying the optimal blasting parameters to the field engineering test, it can be seen that the deep-hole directional tensile blasting technology could effectively reduce maximum stress value and stress field range of the both sides of the roadway, and make the stress field far away from the roadway, thus effectively controlling the deformation of the roadway floor. The deep-hole directional tensile blasting technology can effectively solve the problem of roadway floor deformation and can be safely applied to engineering sites.

Dredging can be traced to early times where it may have been derived by the Phoenicians prior to the 4th century B.C. The first tangible design drawings of a bucket ladder (BL) dredge can be found in the collections of Leonardo da Vinci (1452-1519), which he produced when he was attempting to create better methods of channel and port dredging. The perfection of the system did not commence until after the invention of the steam engine in the early 1800's; providing the impetus to progress from man-powered mud mills, horse drawn along polders or dikes in the Netherlands, England, France and Germany, to steam-driven. In the 1860's following the various gold rushes in the U.S., Australia, New Zealand, Siberia and South America, with placers in particular, efforts were begun to apply mechanization to achieve deeper digging. There is a recording of a dredge being installed to mine gold in Montana in the 1860's but did not include a processing system. Then in the 1880's in New Zealand, some practical engineers developed a system that is said to be the first bucket ladder mining (BL/M) dredge for mining gold placers. In studies of the New Zealand gold dredging experience, it is estimated that some 500 dredges were built to mine gold between the 1880's and 1942. Soil erosion is strongly related to soil hydraulic properties. Understanding how surface coal mining impacts those properties is important in developing effective management practices to control erosion during reclamation. Soils from undisturbed areas, roughly graded mine spoil, replaced topsoil before seeding, and revegetated areas at the Rosebud Mine in Eastern Montana were evaluated to determine impacts on soil hydraulic properties. Field and laboratory studies were conducted to determine soil hydraulic properties, including particle-size distribution, organic matter content, and saturated hydraulic conductivity. WEPP simulations and risk analyses were performed on the soil hydraulic property data to determine the impacts of changes on soil erosion. The hydraulic properties of the soils in each representative area have significant differences due to the mining and reclamation practices at the mine. WEPP simulations show that the potential for soil erosion increases due to mining activities disturbing the soil. WEPP simulations also indicate that the potential for erosion may return to pre-mining levels over time with effective revegetation practices.

As construction projects continue to get larger, greater demands are placed on systems to provide higher production rates at reasonable costs. This paper shows the application of dozer-trap loading versus other methods of moving bulk materials. Most conventional methods employ multiple types of heavy equipment to accomplish a single task. Trap loaders fed by dozers and discharging onto overland conveyors offer an alternative/economical means of high volume bulk material movement. This paper will discuss the process of trap loading to overland conveyors.

"The Ninetyeast Ridge (NER) extends roughly N-S as a topographic feature for 5000 km, from 31° south to 10° north (Fig. 1) where it becomes buried beneath the Bengal Fan at its northern end. These two geographic characteristics make the NER an ideal natural laboratory for determining the influence of latitude and terrestrial input to ferromanganese (Fe-Mn) crust formation and composition. In addition to study of geographic controls on crust composition, the NER is the largest feature in the Indian Ocean that may host crusts of economic interest.Crusts were collected from 9 of 33 dredge sites from 1.4° north to 26° south latitudes during the 2007 R.V. Roger Revelle Knox06 cruise. Nineteen samples were analyzed for 69 elements each. Nine bulk crusts and 10 layers from four crusts were analyzed. Three of the crusts sub-sampled for layers did not have the bulk crust analyzed and the weighted means for those layers were calculated to obtain bulk compositions. One sample was left out of the statistical analyses because it is phosphatized and chemically quite different from the other samples (Table 1).In order to determine changes in composition with latitude, Pearson product-moment correlation coefficients were calculated for the chemical dataset and latitude of dredge sites, as well as water depth. At the 95-99% confidence level, the following elements increase in crusts moving north along the NER: Mg, Ti, S, Cd, Ni, Tl, Mn, Ba, Co, Cu, In, Li, and Zn; and the following elements decrease northward: Fe, Fe/Mn, P, As, Sc, the rare earth elements plus yttrium (REY), Bi, Ge, and Pb. These correlations indicate that the iron oxyhydroxide component and its associated elements are more prevalent to the south, and the manganese-oxide phase and associated elements increase toward the north end of the NER. This change occurs between dredges D20 and D14 (Fig. 1), with crusts from D20 and south having Fe/Mn !1.0 and those north of D20 ""1.2, with the exception of the phosphatized crust. Crust composition also changes with water depth: Th, Ge, Pb, and the middle rare earth elements (REEs) increase with increasing water depth. Thorium commonly increases in deeper water crusts, which explains the higher Th concentrations in Atlantic and Indian ocean crusts in general compared with Pacific crusts (Fig. 2 and Hein et al., 2012), including the samples analyzed here with a mean water depth of 3584 m. However, water depth does not explain the latitudinal changes because the mean water depths of samples along the northern and southern segments of the NER are roughly comparable. Surprisingly, Si and Al, representing the aluminosilicate/detrital phase, do not correlate with latitude; that is as the ridge approaches and is buried by the Bengal Fan the Si and Al contents do not increase. However, overall, Si and Al have higher concentrations in NER crusts than in central Pacific crusts. Ba can be used as a proxy for primary productivity and statistically does increase to the north along the ridge in the direction of the equatorial zone of high biological productivity. Correlations of Mg, Li, and Zn with latitude may also reflect increased primary productivity at lower latitudes."

The Ruby Hill Mine agglomerates crushed low-grade ore (nominally -1 .5'') with high-grade filtered tailings (filter cake, nominally 80% -65#). Cement is added as a binding agent. The agglomerated ore is then conveyed and stacked on the heap. In 1998 approximately $480 000 was spent on cement for agglomeration. Cement for mill pH control and agglomeration was the largest single reagent cost at Ruby Hill. In an effort to reduce agglomeration costs a test-work program was initiated to investigate alternative binders and optimum binder additions. This paper summarizes the test­work results to-date.

Diesel particulate matter (DPM) emitted from diesel engines is currently considered to be a health problem in underground mines. As a result, attempts have been made to control DPM both by reducing emission rates and by managing the DPM that gets into the mine atmosphere. This paper presents a method for modeling the generation, transfer and distribution of DPM in the mining sections through the use of a convection-based computer model so that controls for emitted DPM can be evaluated. A variety of ventilation strategies and diesel deployment schemes for room-and-pillar development sections were simulated to compare their effects on average DPM concentrations at several fixed and mobile personnel locations in the section. Input data for the model was gathered from actual diesel development sections and derived from experimental study results provided by other researchers. Studied using the model were ventilation schemes using vent tubing and brattice, exhaust location strategies on the vehicles, and the direction of ventilation at the feeder-breaker location. A summary of results from this study will be presented.

The design of most underground support systems has largely been based upon empirical rules. This design method has proven to be satisfactory in most cases as witnessed by the number of successfully completed works. However, the adequacy of this approach to the developing technology of ground support systems associated with deep excavation has not been substantiated. In cases where the material has definite time-dependent behavior parameters, rules of past experience become extremely vague and conservative. The Spokane Mining Research Center is currently engaged in extensive research to develop rational design methods for underground structures which will account for the many interacting factors such as in-situ properties of the rock mass, excavation methods, proposed types of support system, construction sequence, and environmental variables. Explanations of commodity price levels, or movements, commonly focus on the supply and demand fundamentals of the particular commodity, or else are assigned to speculative activity. Comparisons of metal price histories with monetary and other economic data support the argument that the supply of and demand for money are no less important in price determination than are the supply and demand of the commodity itself. That these relationships should exist is evident in the price identity and are consistent with monetary theory. The patterns developed have limited usefulness as predictors, but their recognition is essential for the construction of price scenarios for planning purposes.

"The support pressure of a slurry shield corresponds to the in situ earth and groundwater pressure to ensure the stability of the tunnel face. The support fluid (bentonite suspension) penetrates from the excavation chamber into the pore space and transfers the support pressure to the granular structure of the soil within this zone. At the same time, the cutting tools rotate periodically with the cutting wheel and remove the penetrated soil up to a certain cutting depth at the face. Shortly after the tool passing, the penetration process starts again. Both, the support pressure and the soil excavation, act in the same direction at the tunnel face, but they are subject to different cyclic intervals. This relationship between the time needed for the transient penetration process and the interval of the cyclic passage of the cutting tools at a local point at the face is examined and the impact onto the global face stability is discussed. INTRODUCTION Slurry shields are usually deployed for excavations in soft ground. The main application range is in non-cohesive soils (DAUB Recommendations 2010). In non-cohesive granular soils, the tunnel face stability is one of the key design issues. The slurry shields make use of pressurized bentonite suspension (slurry) to support the unstable tunnel face. To support the tunnel face effectively, the slurry pressure has not only to counter the groundwater pressure, but also has to be transferred into an effective stress to support the soil grains. In general, the pressure transfer can be achieved in two ways: (1) with a thin flexible membrane or (2) a penetration zone. The membrane (1), called filter cake, creates a thin impermeable layer from bentonite particles directly on the tunnel face, and enables a transformation of excess slurry pressure into effective stress. In case of penetration zone formation (2), the excess slurry pressure is transferred onto the soil skeleton along the entire penetration depth by shear stresses between slurry and soil grains. These theories, presently applied to assess the pressure transfer on the grains within slurry shield tunnelling, are originating from diaphragm wall technology, e.g. Müller-Kirchenbauer (1977), Kilchert and Karstedt (1984). They assume that the support pressure transfer mechanism is built-up instantaneously on the whole tunnel face and is not damaged or influenced by the interaction with excavation tools. These assumptions are valid for conditions during the excavation of diaphragm wall trenches whose bottom is excavated and the walls of the trench are supported. At the tunnel face of a slurry shield, the support suspension acts on the same area as the soil excavation. Hence, the conditions on the tunnel face are transient (time dependent) due to the time period needed for the penetration process of the slurry and the periodical damage of the pressure transfer mechanism by excavation tools (Fig. 1a)."

Three sedimentation ponds had their performance evaluated before and after installation of modifications. Each pond had a floating baffle installed, two had siphon outlets, and one had a floating outlet device. Baffles inhibited short circuiting, and the outlet devices retarded flow following a rainfall event and drained the pond, creating a large storage volume to maximize detention time. Total suspended solids and settleable solids tests were measured on samples of influent and effluent to evaluate pond performance. Particle size analyses were conducted on samples containing measurable amounts of solids. Performance improved in each pond, with baffles providing the major improvement.

Proven and probable coal reserves in Iran are estimated to be in excess of 5 Billion tons while the annual coal production is a mere 2 Million tons. Underdevelopment of the industry has proved higher production levels to be impossible, despite growing demand for energy supplies. The existences of numerous legislation are thought to have a detrimental effect on coal mining activities to the extent that some business entities have become uneconomical in many areas. This paper provides an analysis of the prevailing mining laws, policy making, legal frameworks and obstacles. It also outlines some other obstacles enshrined in the law and also highlights areas where foreign direct investment should be encouraged. The costs of lost opportunities are then estimated. The paper concludes by demonstrating that deregulations, amendments and in some cases complete abolishment of the incumbent legislations governing the coal mining industry will greatly enhance development of the industry. The analysis provided in this paper together with the results obtained, can prove useful to macro-economic policy makers both at local and national levels.

This paper describes the serious injuries that have occurred to bulldozer operators working at domestic coal, metal and nonmetal mines in the United States. A serious injury is defined as one that results in either a fatality or in lost work time. Examined were 873 injury records compiled by the Mine Safety and Health Administration (MSHA). The time period covered was 1988 through 1997. These injuries resulted in 18 fatalities and 31,866 lost work days. All of these injuries occurred to dozer operators while they were performing common production tasks. An injury classification system was developed to code the narrative information that describes the circumstances surrounding the injuries. Jolts and jars accounted, for 604 or 70% of the serious injuries to bulldozer operators, and jolts and jars accounted for 75% of the workdays lost. Vertical jars comprised the largest set (354 incidents) of jolting and jarring injuries. In more than half of these incidents, the operator was backing up the dozer. Working near an edge carries significant risk for fatal injuries. Of 116 incidents where the dozer fell over an edge, rolled over or fell into a hidden void, 14 (12%) resulted in fatal injuries. In cases where the dozer operator either jumped or was thrown out of the cab in a fallover or rollover, seven out of 17 of the operators were killed. This study finds that further reductions in injury risk would require more widespread use of seat belts; consideration (and maintenance) of the seat and seat suspension systems that might reduce injury risk from jolts and jars; research interventions to assess the effect of alternative engineering designs to dampen or isolate the effects of shock and vibration; and continued attention to skilled performance, i.e., the integration of hazard awareness, recognition and response with dozer operational skills.

A drill system configuration capable of working in the arctic offshore environment was developed to evaluate the placer gold resources contained in the glacio-fluvial sediments offshore Nome, Alaska. This system is capable of working on the shore fast ice during the winter months and is deployed on a drill ship during the summer. Transfer of the drill between its summer and winter configurations is accomplished with a minimum of modification to the design. The system is designed around the Becker drill. This percussive, reverse circulation drill is capable of penetrating the indurated glacial sediments present in the offshore Nome environment. This drill has operated effectively to depths in excess of 50 m (164 ft) below sea level. Since early 1987, in excess of 2000 holes have been drilled using the system. The drill system often operates in extreme weather conditions. It has averaged approximately 10 holes a day of drill production. The ore reserves defined during these drilling programs have resulted in the recovery of more than 70,000 oz (2.18 t) of gold to date, by the mining vessel Bima.

The Ute-Ulay project area is located approximately five miles west of Lake City, in Hinsdale County, Colorado. The Ute-Ulay Mine site is approximately one mile east of the project area. The elevation of the site is 9,200 feet and is adjacent to Henson Creek (Figure 1) [ ] The Ute-Ulay Mining complex is an inactive gold, silver, and zinc mining/milling operation, that operated from the 1880?s until the 1970?s, and sporadically into the late 1990?s. The mining complex comprises several levels of underground mines, a flotation mill, and an abandoned town site. Due to the narrow, steep sided canyon, milled tailings were pumped upstream one mile in a sluice box to a wider section of the canyon, and deposited into five tailings impoundments and one berm located between the impoundments. An estimated 9,000 cubic yards (yd3) of fine grained tailings were located in the impoundments adjacent to the creek. The Ute-Ulay project area consisted of the mill tailings impoundments located upstream from the historic mine.
