"Geologic structure and properties are the primary influences on ground control at San Juan Mine. Geotechnical properties of the coal seam and near seam strata, the presence of above seam sandstone paleochannels and water, are thought to be the leading contributors to ground control issues. It is the confluence of specific geologic conditions rather than isolated features that most often produce adverse conditions. Altering primary support installation and mining practices are done when unfavorable conditions appear during development. Installation of standing support is most often employed when adverse conditions develop at later dates. REGIONAL GEOLOGIC SETTING San Juan Mine is an underground coal mine located on the western flank of the San Juan Basin in northwest New Mexico. The San Juan Basin is contained within the south eastern region of the Colorado Plateau geographic province. The basin contains multiple coal bearing formations and is also a major producer of coalbed methane gas. San Juan Mine is stratigraphically located within the Fruitland Formation. From a prominent monocline marking the western boundary of the basin, the formation dips gently to the east towards the center of the basin. The primary driver for the deposition of coal bearing formations within the basin was the Cretaceous Inland Seaway. Multiple cycles of transgression and regression driven by sea level rise and tectonic subsidence deposited rock types from marine shale’s to terrestrial sandstones. The Sevier Mountains to the west of the seaway likely provided the sediment supply for the Fruitland Formation. During a major cycle of seaward regression, the Pictured Cliffs Sandstone Formation prograded over the marine Lewis Shale. The coal bearing Fruitland Formation followed, being deposited over the Pictured Cliffs. The Fruitland Formation contains numerous coal seams and is approximately 400 ft. thick at San Juan Mine. Fruitland Formation coal seams are numbered in order of superposition. San Juan mines the Fruitland number 8 seam. Dating has placed the age of the seam at approximately 73 to 75 million years [1]. LOCAL GEOLOGIC SETTING The Fruitland 8 coal seam dips approximately 1-3 degrees towards the E-NE. At San Juan Mine, the seam displays a mild plunging, synclinal structure, the axis roughly bisecting the mine from west to east. The number 8 seam consists of five coal benches and four volcanic ash band partings labeled sequentially based on superposition. The lowest portion of the seam is the 8-1 bench, the 8-2 through 8-5 benches following sequentially to the top of the minable seam. Ash band partings are labelled P1 through P4 from bottom to top respectively. The presence of distinctive partings and benches allows for seam definition and aids in identifying mining horizons."

"A longwall gob is mainly filled with broken rocks from the collapsed roof and becomes a porous medium where explosive methane-air mixtures can accumulate. In bleeder ventilation systems, this mixture can migrate out of the gob into the longwall face and other active areas in the mine during barometric or ventilation-induced pressure changes. When external pressures change, the gob does not perceive this disturbance instantly but with a delay due to air flow resistance in the porous gob material. The delay of gob response to outside pressure changes increases the pressure differential across the gob and may cause an outflow of explosive air mixtures from the gob into the adjacent bleeder entries. With computational fluid dynamics (CFD) modeling, researchers at the Colorado School of Mines have evaluated the impact of gob pressure fluctuations on the outgassing of the explosive gas zones (EGZs), the magnitudes and rates of pressure changes, the volumes as well as the potentials location of outgassing if it occurs. INTRODUCTION The air pressure field in underground mines is controlled by many factors such as operating fan settings, ventilation controls, surface atmospheric conditions, and other local events that can intermittently disturb mine pressure, including blasting, roof falls, or inundations. Mine fan settings are rarely changed. Mine atmospheric pressure fluctuations due to adjustments in ventilation controls occur more often during the mining process. Atmospheric conditions and barometric pressures can generate fluctuations in mine pressure on a large scale. Atmospheric pressures change regularly every day but can fluctuate abruptly and become increasingly hazardous in adverse weather conditions. Investigations conducted in coal mines in the United States, Australia, Poland, and South Africa observed higher methane emissions from the gob during periods of falling atmospheric pressures, which led to mine explosions (McIntosh, 1957; Boyer, 1964; Kissell et al., 1973; Fauconnier, 1992; Belle, 2014; Wasilewski, 2014; Lolon et al., 2015). Falling external pressures are understood to cause potential outflow of explosive mixtures from the gob, while rising pressures may push fresh air into the gob. This phenomenon is widely known as gob breathing. The actual processes that trigger gob breathing are not fully understood, nor are the parameters that contribute to the process. Other major events such as roof falls or fan failures can also change the pressure conditions in the mine. In contrast to atmospheric pressure changes, these events can change mine pressures much more rapidly or immediately. TIME LAG AND GOB BREATHING CONCEPT In a bleeder ventilation system, the pressure in the gob is designed to be lower than that of the headgate entries and the longwall face. Air from the gob and gate entries flows towards the tailgate-bleeder returns. When the external pressures are disturbed, this designed pressure arrangement may change, potentially resulting in increased outflow from or inflow into the gob."

"Early on Jan. 9, 2014, approximately 37,800 L (10,000 gal) of crude 4-methylcyclohexanemethanol (MCHM) leaked from a storage tank sitting on the bank of the Elk River near Charleston, WV (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal and is considered nonhazardous in terms of transportation, storage regulations and also toxicity (CDC, 2014a). Freedom Industries Inc., a producer and vendor of specialty chemical products, had acquired the storage facility from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein and McWhirter, 2014).The spill site was located a little more than 1.6 km (1 mile) upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a "Do Not Use" order was issued for all customers (WVAW, 2014a).With little information readily available regarding the potential human health effects of MCHM, the U.S. Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the "Do Not Use" order remained in effect for a total of eight days, until Jan. 17, when the WVAW distribution water was confirmed to have MCHM concentrations below the approved level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at sub-ppb concentrations, a fact that undoubtedly added to growing public concern (Rosen et al., 2014; Gallagher et al., 2014). In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until Feb. 25 when MCHM levels were measured below 2 ppb (WVAW, 2014b), and eventually replaced the affected filter media (WVAW, 2014c)."

"How Wall Street Darling Molycorp Undermined the U.S. Economy, Degraded our National Security and Distorted the Global Economic Balance and How These Falsehoods Continue to be Promoted Even Today INTRODUCTION & SUMMARY In 2007 the Molycorp Mountain Pass rare earth mine was resurrected from its desert-grave in California by its then-owner Chevron Mining. Responding to elevated rare earth prices Chevron began processing on-site rare earth concentrates left over from past mining operations that were terminated in 1998i. Chinese demand had sparked what was being described as a commodities super-cycleii. Commodity prices were on the move. Wall Street smelled opportunity. In 2008 Goldman Sachs, Pegasus Partners, Resource Capital Funds, Traxys North America LLC and Carint Group LLC acquired the mining assets from Chevron. In an environment of ever-declining ethical standards and regulatory consequences the private capital group and their investment bankers would bring Molycorp to market. Molycorp, with its high powered investment banking team of Morgan Stanley and J.P. Morganiii, easily dominated the contest. Molycorp promoted itself, largely based on its past operating history and proven reserves, as the best company to challenge China’s monopoly control over the production of rare earths: a group of elements critical to most advanced technologies and U.S. defense systemsiv. Most of Wall Street’s hottest investment sectors were dependent on these materials: alternative energy, green technologies, computing, personal communications, advanced electronics and defense systems. By late 2010 there were dozens of prospective rare earth start-up ‘mining companies’ seeking capital. Eventually the number of prospective rare earth mining companies seeking capital exceeded 400, despite the reality that supply requirements of this market were disproportionally small when compared to other natural resource sectorsv. In the world of finance ‘smart money’ picks the winners. Consequently just twovi projects attracted nearly all of the capital (Molycorp and Lynas). Wall street picked Molycorpvii. This paper will primarily focus on Molycorp. By early 2010 the words “Molycorp” and “Rare Earths” electrified many people in the mining industry, most non-Chinese manufacturing technology companies, U.S. & allied defense contractors, technocrats in the U.S. national security community and eager investors across most of the world’s stock markets. It was the hottest story of the yearviii. The financial, technology and defense press coverage verged on euphoric: heaping fuel on fuel until the highly combustible Molycorp-pyre cast a long shadow over all of its rivalsix. When the IPO match was lit the spectacular flaming pyre sucked all of the oxygen out of the rare earth spacex. Molycorp was ranked the most lucrative IPO of 2010, resulting in record breaking profits to its private investors and massive returns for its early stock holding investors."

BOB FALCONER: Our next topic leader is Scott Lawson. Scott is manager of human resources for Independence Mining Company, Cripple Creek and Victor Gold Mining Company, Victor, Colorado. Scott has 25 years of human resources management experience in the mining industry, surface and underground, precious and base metals. His topic this morning is "It's the Employee That Counts." SCOTT LAWSON: When reviewing the topic of productivity in a union versus nonunion workplace, we need to look at who, and not what, can make the difference in ensuring the success of a production-oriented operation. It's not explicitly whether the property is unionized or not, but whether the employee team (including the "management group") is working together to accomplish common, shared, and meaningful goals. The bottom line is what action or actions can be taken to develop trust and to strengthen working relationships that challenge and compel employees to willingly share their particular areas of expertise. I want to share a real-life experience that happened to me during the summer of 1961. Perhaps some of you will be able to relate to the following story. I was an 18-year-old fresh out of high school, ready for my first true work adventure. I found summer employment for myself and two friends with the New York Central Railroad in Cleveland, Ohio. Our jobs were as laborers, laying ties and rails, and unloading ballast. We made $2.62 an hour. We were fat cats! My friends and I took on the job with a great deal of vigor, to the dismay of our fellow workers. These workers were unionized by the Railway Workers, which I believe were affiliated with the Teamsters. Talk about being naive, we wrote the book in 1961. When we took the jobs, we did not realize the employees were represented by a union, even though we didn't think they worked too hard. Within our first two weeks of work, we established new daily records for laying virtually twice as many ties as had been standard, and we nearly tripled production by the end of our hall of fame railroad careers. We even took naively to the next level by lining up the ties for our afternoon work shift during our lunch break. In 1961, we thought we were the Mantle, Maris, and Colavito of tie laying. For the younger set in the audience, we would have felt like the McGwire, Griffey Jr., and "The Big Hurt" Thomas of tie laying. A section of the union contract allowed for a 60-day probationary period during which the employee was evaluated and also exempt from joining the union. "A three dimensional nonlinear finite element model was constructed to simulate and analyze the behavior of large diameter segmentally lined shafts when subjected to seismic waves. To reduce computation time, a less complex shell model was also constructed. Transient excitation was applied to the supported nodes of both models, at the base of the shaft, with a time-function representing the 1994 Northridge earthquake. The models are developed at the end of an original phased construction-stage analysis of the shaft. Displacements, forces and moments are monitored and compared during the analyses steps. During the initial 15 seconds of the earthquake, the top and bottom of the shaft show similar displacements. The maximum horizontal displacement within the shaft is approximately 3.5 inches in the solid model and the shell model results conform to this by a factor of 50%. Keywords: Segmentally Lined Shaft, Soil Structure Interaction, Seismic Analysis, Finite Element Modeling, Ground Modification INTRODUCTION TO SEGMENTALLY LINED SHAFTS Large diameter segmentally lined shafts (SLS) are underground systems, constructed sequentially using precast concrete segments to form a ring which provides both temporary earth support during excavation and permanent structural support. The segments are connected via alignment pins, bolts, packer material and other structural components that ensure the continuity of the SLS. In this analysis, the construction process uses a top-down sequence wherein each soil layer is excavated, damp-proofing is installed and concrete segments are assembled to form each ring. This process is repeated until design depths are achieved completing the shaft. It is known that cylindrical structures sustain lateral (earth) pressure mainly through the mechanism of hoop-stress. As Timoshenko’s ring theory goes, a cylinder shell structure is prone to buckling under external pressure [1]. In typical designs of small diameter shafts and tunnel linings with small diameter to wall thickness ratios, nonlinear buckling analysis (i.e., nonlinear geometric analysis) is not considered, since the buckling capacities for those structures are much higher than typical pressures encountered at their design depths. However, in shafts with large diameter to wall thickness ratios, buckling could become a critical issue. In any given mode, as buckling occurs in a ring, some sections of the structure move inward while alternating sections protrude. Although the presence of the surrounding soil could mitigate the buckling by resisting the movement (Soil structure interaction) seismic loads can cause ground movements and lateral pressures, exceeding the capacity of the structure. It is therefore essential to design the load bearing capacities of the shaft to minimize the possibility of any catastrophic failure due to earthquakes. This study explores the structural response of the SLS when subjected to seismic loads."

"On October 11, 2000, an estimated 306 million gallons of water and coal waste slurry drained from an impoundment in Martin County, eastern Kentucky into an adjacent underground mine. Approximately 230 million gallons of the water and slurry discharged from two underground mine portals and affected over 75 miles of streams in Kentucky and West Virginia. In response to this and several other similar events, the U.S. Office of Surface Mining Reclamation and Enforcement (OSMRE) commissioned an oversight study that evaluates how well state and federal regulatory programs established under the Surface Mining Control and Enforcement Act are ensuring that impoundment operators are minimizing the potential for impoundment-basin breakthroughs. The study focuses on slurry impoundments constructed in the hollows of Appalachia where numerous coal seams and steep topography combine to result in a large number of mined seams intersecting and underlying impounding facilities. Factors under consideration include: (1) full accounting of all mineable coal seams intersecting and underlying the impoundment; (2) identification and accurate location of underground mines close enough to the impoundment to potentially affect its stability; (3) assessment of the stability of coal barriers between the impoundment basin and adjacent mines, and stability of roof rock and pillars in mines subjacent to the structure; (4) determination of the flowability of the impounded slurry when expanding or undermining the facility are being considered; and (5) measures undertaken to reduce breakthrough potential when necessary. The oversight study began in 2012 after program-evaluation criteria were defined in an OSMRE technical position paper. The study is scheduled to be completed in 2017. BACKGROUND The Surface Mining Control and Reclamation Act (SMCRA) authorizes the disposal of coal mine waste by constructing a coarse coal mine waste embankment across a valley and pumping the fine coal mine waste (slurry) into the resulting basin (Figure 1). A concern shared by many engineers, geologists, and mine inspectors familiar with coal mine waste slurry impoundments is related to the common occurrence of underground mine workings adjacent to or beneath the impoundment: the potential for slurry “breakthroughs” into mine works and subsequent breakouts into the surface waterways (Figure 2). On October 11, 2000 a combination of coal refuse slurry and water from the Big Branch impoundment in Martin County, Kentucky broke through into an underground mine and subsequently discharged into the receiving streams. The location of the point of breakthrough, pathways of slurry/water flow within the mine workings, and two points where the liquid exited the mine are shown in Figure 3. An estimated 306 million gallons of water and coal refuse slurry drained from the impoundment into the adjacent underground mine. Approximately 230 million gallons subsequently discharged from the underground mine at two portals."

"Testing of the roof bolter canopy air curtain (CAC) designed by the U.S. National Institute for Occupational Safety and Health (NIOSH) has gone through many iterations, demonstrating successful dust control performance under controlled laboratory conditions. J.H. Fletcher & Co., an original equipment manufacturer of mining equipment, further developed the concept by incorporating it into the design of its roof bolting machines. In the present work, laboratory testing was conducted, showing dust control efficiencies ranging from 17.2 to 24.5 percent. Subsequent computational fluid dynamics (CFD) analysis revealed limitations in the design, and a potential improvement was analyzed and recommended. As a result, a new CAC design is being developed, incorporating the results of the testing and CFD analysis.IntroductionExposure to respirable coal mine dust can cause coal workers’ pneumo-coniosis (CWP), also known as black lung. If silica is present in the dust, the miner may develop silicosis in addition to CWP. CWP and silicosis are occupational respiratory diseases that are ultimately fatal and have no cure. The only method of prevention is to eliminate exposure to respirable coal mine dust and crystalline silica, or quartz. The occupational exposure limit on a Mining Research Establishment (MRE) basis for respirable coal mine dust had been 2 mg/m3 during each shift that a miner is exposed in the active workings of the mine or in mine facilities. Under recently enacted legislation, this standard was changed in August 2016 to 1.5 mg/m3 for a full working shift (The Office of the Federal Register, 30 CFR 70.100, 2015). When respirable quartz is present, the mine must maintain its average concentration at or below 0.1 mg/m3. If the mine exceeds the 0.1 mg/m3 respirable quartz dust concentration, then the applicable respirable dust standard is reduced, calculated as 10 divided by the percentage of quartz present (The Office of the Federal Register, 30 CFR 70.101, 2015).The sources of exposure to respirable coal mine dust for roof bolting occupations include infrequent maintenance and cleaning of the vacuum dust collection system and operating the bolter downwind — in the return air — of the continuous miner (Good-man and Organiscak, 2003). Recent studies have demonstrated that vacu-um dust collection systems are effective in capturing bolter-generated dust if maintained and operated correctly and if proper face ventilation techniques are used. However, most expo-sure to respirable coal mine dust for a roof bolter occurs when the roof bolter machine is operated downwind of the continuous miner (Potts, Reed and Colinet, 2011; Colinet, Reed and Potts, 2013). Because of this higher exposure downwind of the continuous miner, the roof bolter is often limited to working downwind of the miner only once during a production shift."

"In the present study, the effect of organic/inorganic (hybrid) polyacrylamide polymer on fine coal flotation was investigated. Raw coal samples contain about 37% of ash-forming mineral at a feed size of -75 + 38 um were subjected to flotation tests in the presence of in-house synthesized hybrid Al(OH)3-PAM at different polymer dosages and pH values. Results show a significant improvement in both combustible recovery and ash rejection at 1 ppm polymer dosage. Further improvement of combustible recovery was obtained when flotation was performed at pH 5. Interfacial properties of coal particles before and after flotation were examined through electrokinetic (zeta potential) measurements of coal/water slurries. Zeta potential vs. pH curve were established and compared. In the case of raw coal/water slurry, zeta potential-pH curve resembles the ash-forming minerals (silica and clays) more than coal. After Al(OH)3-PAM-assisted flotation, the zeta potential of froth/water slurry shifted to more positive values point which strongly indicates significant removal of ash-forming minerals from coal by Al(OH)3-PAM. INTRODUCTION Coal has been a major commodity in meeting the world energy demand during the past several decades (Milici et al., 2013). According to the international energy agency, global coal demand will continue to increase for the coming decades. With increasing world population, the global electricity demand would also increase as more people would get access to basic electricity especially in developing countries (Maggio & Cacciola, 2012). Due to this increase in energy consumption, operations in coal mining and beneficiation processes have been highly mechanized to meet the demands. This mechanization has increased the quantity of raw fine coal particles produced which contain high amount of ash-forming mineral matter (slimes). Aplan et al., have reported that the major constituents of ash-forming minerals in U.S.A coal are clay minerals such as kaolinite, montmorillonite and illite (Aplan, 1997). Conventional froth flotation for fine coal cleaning suffers mainly from two major problems: 1) Low recovery of ultrafine coal particles due to slime coating by ash-forming minerals, and 2) The lack of selectivity, which results in the flotation of middlings and entrainment of mineral fines in the froth (Polat et al., 2003). Slime coating on both valuable and air bubbles have been proposed to explain the observed reduction in flotation recovery especially when fine grinding is required. In this case mineral matter may be degraded to extremely fine or colloidal sizes thereby creating difficulty in the water clarification process such as flotation. Slimes inhibit bubble-particle attachment, and increases reagent consumption due to the increase in solid/liquid interfacial area, resulting in low coal recovery."

"Gob ventilation boreholes (GVBs) are widely used in underground coal mines for longwall gob degasification purposes. GVBs are often drilled within 10 to 30 m above the top of the coal bed into the fractured zone, completed with 20 cm (8 in.) diameter casing and 60 m of slotted pipe at the bottom. The purpose of this completion strategy is to create a pressure sink to capture the emissions before they can enter into the underground workplace. For GVBs to work effectively, they must be drilled close enough to the working areas to capture methane from the fractured zone while the setting depth must be kept above the caved zone to minimize the amount of ventilation air that is drawn into the GVBs. Well completion parameters are important for creating safe working conditions. In this paper, computational fluid dynamics (CFD) studies will be presented to analyze the effect of different practices for GVBs completion parameters for methane extraction, formation of explosive gas zones in the gob and methane concentrations at the longwall face and tailgate. Authors have identified the optimum completion parameters for GVBs, which can maximize the benefit and minimize fire and explosion risk. INTRODUCTION Coal mines may be considered gassy based on the presence of CH4. Gas from the mined coalbed or from rider seams above or below the mined coalbed can migrate into the gob and then potentially move towards the working areas. In longwall coal mines with high methane emissions, sufficient ventilation and additional methane control measures are usually required to reduce the methane concentration in working areas to below statutory levels. Several techniques or combinations of techniques are available to control the methane hazard. These may include mine face ventilation to dilute methane, coal bed degasification, nitrogen injection into the gob to render a methane atmosphere inert and the use of gob ventilation boreholes (GVBs). Longwall face ventilation and dilution alone can only handle methane emissions up to a certain amount. When methane emissions exceed the face ventilation capacity, additional control measures are required. Drilling horizontal or vertical methane drainage holes into the coal seams as well as gob ventilation boreholes from the surface into the gob area to extract methane are common practices. GVBs are usually drilled from the surface above the longwall panel ahead of mining. When the face moves outby the GVBs and the gob begins to form, methane production starts from the boreholes. It is important to control the performance of GVBs to maintain a safe working environment in the mine. The performance of GVBs depends on multiple factors, including borehole location, length and diameter of slotted casing, setting depth of casing above the coal seam being mined, wellhead vacuum pressure, and permeability of caved-fractured zones."

"INTRODUCTION AND BACKGROUND The world needs visionary people to break new ground and bring development forward. The human race has an inherent power and lust to explore and push borders, but also to connect and bring people together. Transport of people and goods has through history been a key factor to develop societies and increase collaboration between people and regions. Today, in the era of globalization, efficient transport systems are still vital for further development. Often, ideas of bold projects, like long bridges or tunnels, have been raised long before those projects are realized and actually built. This shows that visions must gradually be transformed into feasible business cases and sustainable political solutions. In some cases also technological development is necessary to make visions come true. Isambard Kingdom Brunel, indeed a visionary engineer, completed the oldest underwater tunnel in the world when the Thames Tunnel in London was opened in 1843. This tunnel is still in operation and now part of London’s rail transport system. Another milestone for the tunnelling industry was passed when the 54 km long Seikan tunnel in Japan was opened in 1988 after more than 15 years of construction. This tunnel is presently the world’s longest railway tunnel. Together with the 50.5 km long Channel tunnel between the UK and France, which has the longest undersea portion 37.9 km, they constitute examples of pioneering engineering projects that bring the industry forward. In 2016, the Gotthard Base Tunnel will be opened and then becomes the longest railway tunnel in the world with its 57.1 km. The trend is obvious that tunnels throughout the world are getting longer and longer. The Brenner Basis Tunnel presently under construction will yet again break the record being 64 km long. Presently, a number of even longer tunnels are planned and under design, some of these also have very long subsea portions, e.g. Bohai Straight Tunnel (123 km), Irish Sea Tunnel (around 100 km) and the Helsinki – Tallin fixed link (approximately 90 km). Northern Europe sees an increasing demand on efficient transport corridors to meet the growth of freight transportation between the Scandinavian Peninsula and central-Eastern Europe. Sustainable solutions will be required, and a shift from road/maritime transports to rail is imperative. Consequently, this paper describes yet another visionary project, a fixed railway link between Sweden and Germany, the Shortcut, to enhance traffic between these regions and to make it greener. It is true that this project still requires some work from politicians, economists and engineers to be realized, however, building a 100 km long tunnel under the Baltic Sea may become reality sooner than we think."

"INTRODUCTION The Lisbon Valley Mining Co LLC (LVMC) operates the Lisbon Valley Mine located in SE Utah. The mine includes 3 open pits, 150-acre heap leach pad, and solvent extraction electro winning plant (SX/EW). Approximately 25MM tons of ore has been mined since 2005. The ore is stacked on the leach pad to an average height of 55 feet and occurs as a blend of crushed material (minus 2”) and run-of-mine (minus 24”). The host rocks include Dakota and Burro Canyon Sandstones. Copper occurs finely disseminated in this material at an average grade of 0.46%. Minerology is 63% sulfide, 27% oxide, and 10% insoluble. Total inventory mined through 2016 is approximately 230 MM lbs. METALLURGICAL CONSTRAINTS Copper production is constrained by areas of low permeability ore and geochemical issues, including acid consumption. The intrinsic permeability the ore ranges from 10-2 to 1 Darcy. This permeability does not permit sufficient aeration by natural convection, therefore oxygen transport is largely limited to the mass of oxygen that can remain dissolved in leachates. As a result, greater than 50MM lbs of copper remains in inventory and occurs secondary sulfides, including chalcocite, covellite, and bornite. BIOLEACHING FUNDAMENTALS Due to the higher activation energies of secondary sulfides, notably covellite and bornite, a strong oxidant is needed to remove an electron from the metal-sulfide lattice and release the copper cation into the sulfate solution. This process occurs most efficiently using the strong oxidant ferric iron (Fe3+). Ferrous iron can be oxidized to ferric iron by oxygen via chemical oxidation or biologically; which was measured to occur 500,000 times faster when using microbes versus oxygen.1 The microbes that oxidize ferrous iron (Fe2+) to ferric iron are naturally abundant and will develop biomass when appropriate conditions and nutrients present themselves. Most nutrients are provided by the ore/irrigation (P,N,Mg, Ca & Fe2+) while the others (O2 and CO2) are provided by air. In most cases, oxygen transfer is the rate limiting step during copper sulfide leaching. 1 Lacey & Lawson 1970 FORCED AERATION SYSTEM DESIGN This paper describes LVMC’s efforts to bioleach the remaining inventory by forced aeration. Two basic aeration systems were installed and tested including vertical wells and horizontal piping. Vertical System The vertical system was installed using the mine’s blast hole drilling rig. Wells were constructed using 4” high-density polyethylene (HDPE) connected to 4” polyvinyl chloride (PVC) screens similar to conventional bioventing wells. Ore samples were collected at each well location to characterize the spatial distribution of the remaining inventory along with fines (minus 200 sieve), pH (acid consumption), and iron. A block model of each analyte was developed and mapped in 3D."

INTRODUCTION This paper presents an assessment of conventional flotation technology. In addition it presents some ideas which can be used to change the perception of what may be done with conventional flotation equipment to maximize its efficiency. Perspective of Conventional Flotation Technology 1. Flotation in many coal preparation plants is defined as something between a necessary evil and a non-viable concept because: -Too much art is required on a continuous basis to achieve uniformly acceptable results. -It is expensive, the capital cost in dollars per ton of circuit feed for flotation and dewatering is high. -It is a difficult process to use when SO2 reduction governs system design and equipment selection. -It creates a product (fine coal froth) that is difficult to dewater in the presence of BTU/lb as-received pressures. -It is a difficult process to control from a management standpoint, there are no dependable set points to rely on. 2. Many froth cell circuits are 0.5m x 0 (nominal 28 M x 0) feed which is sourced from screen/cyclone underflows. The original intent was to obtain maximum yield at acceptable product quality and moisture using disc filters. (See Figure #1) 3. An alternate to conventional flotation and its perceived to be limits of efficiency, capital costs, etc. is to move towards such developing flotation technologies as column flotation, reverse flotation, and microbubbles. A lot of work is being done to move these concepts thru pilot stages into full scale, commercial plant operation. The majority of the coal producers and coal preparation plant designers view these concepts as being in the development/pilot plant stages. OBJECTIVES Based on the preceeding comments of the perspectives of the coal producers and plant designers, the options are limited. They can make do with what they have available to them in the way of industry standard equipment and practices or they can move forward with developing technology. I feel there is a third option: what can we really do today with what is available to us, without reinventing the wheel? APPROACH 1. When processing coal against ash and SO2 restrictions, the fine clean coal stream can be the best stream on the clean coal conveyor due to the release by size and the ability to create a high BTU/low sulfur (dry basis) product. 2. Targets: -Consistent raw coal size consist in the plant feed . -Do not put pyrites in the froth cells. -Adjust criteria defining good flotation away from yield towards maximum BTU recovery at minimum ash. -Limit the froth cell feed size consist to 0.15 mm x 0 (nominal 100M x 0). -Establish and maintain a correct, consistent percent solids in the froth cell feed. -Select flotation reagents with care. -Introduce reagents in the proper sequence.

Introduction A study was completed that investigated the enrollment in mining engineering programs in 24 universities and colleges. All programs considered in the study award four-year bachelor of science degrees in mining engineering. They also provide all the engineers for the mining industry and a large portion of the geotechnical engineers working on environmental problems, storage of hazardous waste, civic, and Federal government underground projects. The numbers used as the base for the study are those reported by each program to SME-AIME and published yearly (SME, 1986). Data for the study came from the period 1975-1985. Also, since some programs include students in related disciplines and special categories, such as cooperative students, a few inaccuracies were unavoidable. A 23-page document has been prepared that reports on the overall picture of current enrollment, the trends, and discusses the results, perceived effects, and what can, or should, be done. There has been an obvious, and dramatic decrease in total undergraduate enrollment since the late 1970s. In 1978, 3117 students were enrolled. In the 1985 fall semester, enrollment was 1022, less than one-third. There were 485 seniors, 265 juniors, 147 sophomores, and 125 freshmen. The downward trend in total enrollment is likely to continue for at least another two years. Class size The catastrophic drop in enrollment is shared by all programs across the country. It does not discriminate between types of programs, coal or hard rock mining, or location. In general, 1985's enrollment is about 30% of whatever the maximum enrollment was in any program, though this varies from the teens to 50%. The latter percentage occurs only for some programs with special circumstances, particularly new programs not well established in the late 1970s. The beginning of the decline in enrollment has occurred at slightly different times as affected by the demise of each program's local industry. The numbers in each class produced from a specific freshmen year (Fig. 1) are worthy of comment. The dramatic drop in enrollment for all classes after 1982 is clearly demonstrated. The timing coincides with the general downturn in the mining industry, when many employed engineers were laid off and new graduates could not find jobs. Also, significantly fewer students are being attracted to mining programs as freshmen. The number of seniors shown seems inappropriate. Where did so many seniors come from and why are so few graduates produced? There is an increasing trend for students to take four-and-a-half or five years to graduate. Therefore, they are counted twice, once when they are first classed as seniors and again the following year. Some students purposely delay graduation to obtain more summer work experience and take additional courses. They believe that this gives them a better

INTRODUCTION The decontamination of the former Shiprock uranium millsite and the interim stabilization of its tailings piles began in March 1974, and ended in June 1978. Personnel monitoring for whole body gamma ray exposure was conducted for selected individuals working at the mill site throughout most of this period. The millsite and exposure conditions are briefly described to develop a basis for interpretation of the exposure data. Millsite The Shiprock uranium millsite is located in the community of Shiprock, New Mexico on a bench overlooking the San Juan River (Figure 1). Both Shiprock and the millsite are located on the Navajo Indian Reservation. About 1.5 million tons of tailings were produced on the millsite during its operation from 1954 to 1963. The tailings were pulped with waste process fluids and water, and discharged to form two tailings piles. The piles (ponds, dams) were formed with earth starter dikes and the dikes were then raised using the separated sand fraction of the tailings. The slime fraction of the tailings were discharged into the diked impoundment. The mill was shut down and partially dismantled before the lease expired in 1973, and the remainder of the mill and tailings reverted back to the Navajo Tribe which was the lessor. Portions of the millsite were then occupied by the Navajo Engineering and Construction Authority (NECA), a tribal enterprise. In addition to its regular activities, NECA established an on-site training school for heavy equipment operators. The trainees principally used the south tailings pile as a practice area and spread the tailings into a 10 hectare row of raffinate ponds located along the southwest side of the millsite. Overall, the training activities inceased the effective area of the tailings piles from 20 to 35 hectares. Another 56 hectares were contaminated with wind and water eroded tailings. Exposure Conditions Persons decontaminating a millsite may be subject to radiation exposures from several sources. Only whole body gamma exposures are discussed in this report. These exposures are arbitrarily divided into three levels: high, intermediate, and low. The levels are related to areas in which the decontamination work was being done and have no connotation concerning imminent hazards. High exposures would result when persons were working with raw tailings, and intermediate exposures would occur when persons were working in the ore storage and mill yards. Low exposures would result from working in areas where surface contamination exists from wind and water erosion. [ ] The millsite and its contaminated environs were divided into major areas to facilitate the decontamination work (Figure 2). Surface mining, from ore extraction to processing, requires many types of equipment to obtain the final product. Mine equipment often includes shovels, drills and haul trucks. Ore processing equipment, including plants, mills and concentrators, are also part of surface mining operations. Surface mining operations emphasize production. Production is where the bills are paid and fortunes are made. However, there must be support to keep the equipment operational. To most production people, equipment maintenance is a necessary evil. Maintenance is not glamorous work. It is often neglected when praise is being passed around. It is easy to recognize higher production, greater capacities, faster cycle times or more loads hauled. But who cares if an engine is replaced in 12 hours instead of 48? The answer is, everyone should - from the hourly miner to the mine manager. Surface mining equipment needs constant maintenance work. It must be lubricated, components replaced, electrical problems repaired and other problems addressed. Repair work for any surface equipment takes time away from production. It costs money. Maintenance costs cannot be avoided. However, they can be minimized. Minewide communication should include operations cooperating with maintenance. Traditionally, operations resist surrendering equipment to maintenance if it can continue to produce. The rationale is, every load that can be hauled today is money in the bank. If the unit is taken down, the money is lost. This thinking is prevalent in many surface mining areas, including the mill. "Not on my shift," the operator thinks. "Let the next crew worry about it. The problem with this thinking is in probability and statistics. The longer equipment goes without maintenance work, the higher the probability of equipment failure. When the failure does occur, it is certain to cause the equipment to remain down longer than if the maintenance department had been allowed to take it down. Many mining operations have closed early in their lives because of poor maintenance practices. This has become the industry standard because substandard levels of performance are acceptable. Management believes it gets five to six hours of work from maintenance people during an eight-hour shift. Actually, the industry average is less than two hours of work per shift. Real productivity levels of less than one hour are not unusual. There are few exceptions. However, a daily work schedule (DWS) will increase productivity at many properties by 50% to 100%. A 200% improvement is not unusual. The DWS will work. Maintenance people are among the most intelligent and motivated people on the property. They deserve the chance to be productive. [ ] In preparing a DWS, management needs to understand two productivity principles - the productivity spread and the real maintenance workday.

Gypsum, the dihydrate form of calcium sulfate, has a history of usefulness to man dating back several thousand years, and a worldwide industry has been built on the mining and processing of this versatile industrial mineral. However, anhydrite, the other prin¬cipal calcium sulfate mineral, has very limited use, so much so that separate statistics for it are not always kept, but are usually included with those for gypsum. Although often found in close association, the two minerals have important chemical differences in that anhydrite is the anhy¬drous form of calcium sulfate (CaSO4) whereas gypsum is the dihydrate (CaSo4.2H20). These two molecules of combined water result in significant differences in hardness, density, and solubility of the two minerals. Of most importance, however, is the difference in thermal properties wherein gypsum, upon the application of a modest amount of heat, can be converted to the hemihydrate of calcium sulfate (CaSO4 '21H20) or plaster of paris, an intermediate product upon which is based over 90% of the value of all calcium sulfate end products. Anhydrite, on the other hand, is inert at these temperatures and its uses have been limited to only those few cases where calcium sulfate in the anhydrous form may have an advan¬tage. The earliest known use of gypsum plaster dates back some 5000 years to the time of the Pharaohs of Egypt who had discovered that gypsum rock, when exposed to fire, broke down into a powder which, when mixed with water, would form a putty that could be plastered on a rough mud brick or stone wall to make a smooth finish. It was used down through the centuries in a limited way for ornamental purposes, but did not achieve wide acceptance because of the quick (25 to 30 min) setting or hardening time which made it difficult to use. The first real understanding of gypsum chemistry was developed in France about 1755, and the gradual growth of its present utilization dates from that time. In the United States, the first utilization of gypsum was to grind it to a fine material for use as a soil conditioner at about the time of the Revolutionary War, and the term land plaster for ground gypsum was coined and has remained in use. Another cementitious material-lime or calcium oxide-had become the usual plastering material, but by about 1870 a method for retarding the set of gypsum plaster was discovered; today it has almost completely replaced lime as a plastering material. In recent years, gypsum products sold in the United States have reached a total valuation of approximately $2 Gpy as illustrated in [Table 1].

GENERAL DESCRIPTION The Climax mine is located 161 km (100 miles) southwest of Denver on Fremont Pass astride the Con¬tinental Divide. Climax employs 2800 people to pro¬duce 42 670 t/d (47,000 stpd) of molybdenum ore. The Storke level, the main underground level, produces 19 970 t/d (22,000 stpd); the 600 level, the lowest ac¬tive underground area, produces 7260 t/d (8000 stpd); and the open pit presently produces 15 430 t/d (17,000 stpd), Fig. 1. A panel caving method is possible because of the favorable geology and geometry of the Climax ore body. Geology The Climax deposit is composed of three separate ore bodies formed in a massive Tertiary stockwork which penetrates Precambrian granites and schists. The ore bodies are elliptical in plan with overall dimensions of 914 x 1219 m (3000 x 4000 ft). In section the ore bodies are arcuate with an overall height of 457 m (1500 ft), although the depth is not firmly ascertained. Ore grade decreases rapidly into a highly siliceous cen¬tral core and more gradually toward the outer bounda¬ries of the deposit. The mineralization is relatively continuous in the ore zone and contains few dilution-causing waste inclusions. Molybdenite is contained in pre-ore fractures which were largely recemented during the three mineralization pulses. Postore fractures, with a spacing of a few inches to several meters (feet), render the deposit amenable to caving when it is undercut. The peripheral rock caves similarly to the ore, but the central core is competent and is difficult to cave. Caving Feasibility Successful caving action requires an undercut area of approximately 122 by 122 m (400 by 400 ft). When the undercut area exceeds this minimum distance, the arching effect is lost, and the ore column collapses. Ore extensions from the main cave area less than 122 m (400 ft) wide usually cannot be caved. Waste or low¬grade areas within the deposit can be left if they are large enough to carry weight around openings passing through them. Experience indicates that these areas should be over 30 m (100 ft) in length and width. Smaller unprofitable units of waste, such as postore dikes, are mined to avoid weight problems and result in ore dilution. Successful caving and ore recovery are dependent upon timely ore removal from the undercut zone because columns of immobile ore inhibit caving and concentrate weight onto the underlying workings. Climax experience indicates that a finger (drawpoint) spacing* averaging 10.4 m (34 ft) yields an excellent caving action and a 92.5% ore recovery at the estimated grade. An additional 15% of the ore is recovered-at a diluted grade (Fig. 2). Mine Layout The slusher drift is the primary production facility (Fig. 3). The ore is undercut and caved into the fingers,

Dr. Roussey Roussev The use of chemical additives on grinding is of significant importance considering the high energy consumption of grinding processes. My research indicates that the dispersant approach explains the effects, in this particular case of wet grinding. Further, the dispersant approach combined with torque measurements give the possibility of clarifying the mechanisms involved when using grinding aids. I find the conclusions derived from the data obtained to be correct and useful for grinding. In general, I agree with the explanations. However, I would like to discuss several additional points that I consider to be important, and in the same time to mention some of my own results from investigations similar to those presented in the study under discussion. The "bed volume concept" and the change of slurry viscosity explain adequately the data plotted on fig. 3, in conjunction with Figs. 1 and 2. However, the significant increase of the average torque in the case of 83% solid with XFS-4272 introduces the necessity of establishing the optimum of percent solid when dispersant is used. For example, the optimum percent of solid for different materials varies. One of the reasons is the material's mass density. On the other hand, it is necessary to determine the concentration of the chemical used. Because of this, it would be interesting to know how the cited concentration was selected. The supportive discussion and verification by Dr. Roussev, with results similar to that of the paper, is appreciated. I will try to answer the additional questions he has raised. To begin with, the mill was fitted with six lifters of approximately one-quarter inch height. There has been extensive lab and plant level work to understand downstream influences of using dispersants like XFS-4272*. In sulfide ore processing, which is the area in which most industrial scale testing has been performed with this approach, neutral to positive influences have been observed. For example, positive effects (e.g., increased recovery) have been noticed in flotation operations involving ores that have a tendency to produce excessive slimes upon grinding. (1) Also, the use of a dispersant has been shown to demonstrate a positive influence on classification by improving the sharpness of separation and preventing a decrease in d50 value that usually accompanies increasing flow rates to classifiers.(2) The biggest potential downstream problem is in thickening, which at the dosages involved, has not been a problem in plant scale usage. The commercial price of dispersants like XFS-4272 is quite variable depending on source, concentration, etc, but a typical cost of XFS-4272 to achieve a 10% increase in throughput in industrial mills has been between 20 and 35- US per ton of feed ore (1981).

Within the metal/nonmetal mining sector, fall-related incidents account for a large proportion of fatal and non-fatal injuries. However, the events and contributing factors leading up to these incidents have not been fully investigated. To help provide a clearer picture of these factors, an analysis of imminent danger orders issued by the Mine Safety and Health Administration (MSHA) between 2010 and 2017 at both surface and underground metal/nonmetal mine sites revealed that most orders are associated with fall risks. Of these cases, 84% involved the workers not using fall protection, fall protection not being provided, or the improper use of fall protection. Fall risks for workers most frequently occurred when standing on mobile equipment, performing maintenance and repairs on plant equipment, or working near highwalls. In most cases, a single, basic, corrective action (e.g., using fall protection) would have allowed workers to perform the task safely. Overall, these findings suggest that a systematic approach is needed to identify, eliminate, and prevent imminent danger situations. Furthermore, to protect mineworkers from falls from height, frequently performed tasks requiring fall protection should be redesigned to eliminate the reliance on personal fall protection.
