Haul truck fatal accidents and injuries continue to be a significant concern for the mining industry. However, the availability of high-quality training materials continues to be limited. Near-miss incident accounts, if packaged well, could help fill this gap, because for every fatality, there are hundreds of reportable accidents and thousands of undocumented near misses. Researchers from the National Institute for Occupational Safety and Health (NIOSH) collected detailed accounts of 21 near-miss incidents in virtual interviews with mineworkers at surface mining operations across the country. From these interviews, researchers created four simulation videos. The videos bring these events to life through first-person retelling and various visual perspectives of actual events. Each video exemplifies a critical safety message and a common haul truck hazard. This paper describes the process of taking narratives and turning them into impactful visual stories. NIOSH plans to co-release these simulation videos with the Mine Safety and Health Administration (MSHA) to the mining industry to raise awareness and ultimately help reduce haul truck-related accidents and fatalities in mining.

Introduction During the past 10 years, energy costs in mining have risen from about 4% to as much as 25% of total production cost. When combined with declining ore grades, these costs exert inordinate stress on mine productivity. Industrial engineering techniques have traditionally been aimed at improving manufacturing facilities productivity, but they have been applied to mining as pressure for productivity improvement has increased. These techniques can perform four useful functions for mining operations by improving methods, equipment selection procedures, energy use through scheduling, and managing energy costs rather than being managed by them. This article discusses some of the standard techniques for productivity improvement, and how they can be adapted to perform these four functions. Pre-Production The best time to seek high productivity is when a mine is being planned. During pre-production, when most options are still open, an optimum mine design is achievable. At this time, major equipment selections are made, the layout, material handling systems, and processes are designed, and critical energy source decisions are made. By including explicit energy cost projections, decisions can be made that will save significant amounts of money per ton of ore over the life of the mine. Attention can also be given to often overlooked details such as lighting. Some of the same tools used in new mine planning are applicable to productivity improvements at an existing plant. These tools are the charting and modeling methods that have been developed by industrial engineers over the past 75 years. Energy Consumption Before any analysis of energy consumption can be performed, preliminary work is necessary to find the kind of energy used, how much it costs the plant, and when electrical peaks occur. Past bills should be examined to determine the monthly consumption rate and the cost of each energy type. These data should be adjusted to remove effects caused by production fluctuations or weather. Knowing when electrical peak demands occur is necessary because these determine either the electrical capacity needed or the amount that must be bought. At $500-$800 per kW for construction or $5-$15 per kW per month for purchase, they represent a significant cost. By the time these data are collected, planners should be aware of the importance of energy in the total cost of mine operation, and should have the most crucial time data for electrical peak demand. Flow Process Chart Reducing costs can be accomplished by a thorough look at mine systems and how altering one system causes changes in others. By developing and exercising models of production processes outward to the transportation of the final product, otherwise obscured weaknesses may become apparent. There are tools available for analyzing the mine and its associated systems; one of the most useful is the flow process chart. The flow process chart gives a system analysis of a production procedure by classifying all actions into one of five types: operations, inspections, transportation, delays without paperwork, and delays with paperwork. Operation: In an operation, the nature of something is changed. Examples are blasting, washing coal, and processing matter. Denoted by 0. Inspection: Examining an object for identification or to measure some attribute. Examples are determining whether a drill hole is deep enough, deciding how far to cut under an unsupported roof when operating a continuous miner, or checking the lubricant level on a truck. Denoted bye. Transportation: Moving an object from one place to another. Examples are transporting overburden, moving coal on a conveyor belt, and pumping water. Denoted by =>. Delay without paperwork: A delay that is an inherent part of the system. Examples include idle time of a continuous miner while waiting for shuttle cars or for roof bolting, crushed ore in a storage area waiting for grinding and flotation, and people from one craft waiting for another craft to finish. Denoted by D. Delay with paperwork: A delay that can be terminated only by some written authorization. Examples are coal that cannot be removed until authorized by a bill of sale and transportation orders, personnel records, maintenance, or spare parts. Denoted by V. In practice, the flow process chart is completed after data collection and a detailed examination of the process. Data collection is needed to answer questions on distance, quantity, and time for each of the operations. As the analysis is being performed, each step of the process is subjected to these questions: • Why is the step necessary? • What is being moved? • When does the step take place, and why?

The United States Cement, along with other industries, is being devastated by an unprecedented penetration of domestic markets by imported products. The overriding questions are: 1. Have foreign manufacturers improved their processes to an extent they can outperform U.S. producers in their home markets? 2. Is it gross inefficiency on the part of the U.S. producers that is allowing foreign producers to be capable of replacing a local U.S. producer in his normal home market? 3. Is there a combination of the first two questions that economically justifies this type of market overrun? The answer is a simple NO!. In a fair trade market atmosphere, U.S. producers are definitely competitive.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

A team of researchers at the University of Kentucky, working under a USBM contract, developed over 65 simulation exercises for teaching and testing problem-solving and decision-making skills to miners and mining engineers. These skills involve both improving judgment and decision making in mine emergencies and enhancing the learning of real-world engineering design principles. The exercises are ideally suited for either latent image format or for computer-aided instruction. Exercise features are summarized, the structure and field testing of typical exercises described, and sample frames of individual exercises presented. Empirical data are summarized that describe the effectiveness of using these short, time-efficient performance tasks that simulate real problems and that provide immediate corrective feedback for teaching and testing critical skills. Any improvement in the accuracy of commercial comminution calculations under the Third Theory must be accomplished either by increased precision in energy input measurements, or by a better evaluation of the total new crack length produced, as indicated by the feed and product size distributions. Since at the present time feed and product sizes are approximated solely by the microns 80% passes, the most promising opportunity for increased accuracy lies in consideration of their plotted size distribution characteristics. In the short period of one mans? career we have seen specifications for bulk material handling systems and equipment grow from two or three pages to as much as two or three volumes for a single contract. The old 'short form' described what the purchaser expected the equipment to do. The long form does the same but also includes nut, bolt, and lockwasher depth of detail. What are the events that led to this proliferation of verbiage, and does it benefit the buyer in the final analysis? Cyanidation has established itself as the main process for extracting gold from ores in this century. However, many ores exhibit refractoriness to the conventional cyanidation process. The main cause of the refractoriness is generally the presence of carbon, tellurium, and sulfides, particularly those of antimony, arsenic, and iron. The problem is usually solved by either modifying the cyanidation conditions or providing an oxidation pretreatment. This paper presents results from studies that help in understanding the causes of refractoriness of various gold ores. This is followed by a discussion of many processes that have been developed to solve the problem such as intensive cyanidation, pressure cyanidation, carbon-in-leach cyanidation, and pretreatment processes such as roasting, chemical oxidation, autoclave oxidation, and bacterial oxidation.

The advent of Nuclear Energy has caused a phenomenal growth in the uranium mining industry. To keep abreast of this surge the mining industry has located many ore bodies, determined the best types of mining adaptable to this particular type of ore and developed a highly specialized type of operation. But, has the mining industry gone far enough in the planning? Has ?built-in? safety been included in the efficient methods of operation? Preplanning safety in the mining industry, as in all industries, takes engineering and ingenuity combined with the experience and cooperation of all concerned. A system of mining can be planned on a map and it will look good. However, when the actual mining takes place, conditions may be such that deviations from the original plans may be required because of excessive water faults or other conditions encountered. With safety we may also be required to change, to conform to the characteristics of a particular operation. People in the mining industry are well aware that loose back has killed miners each year. This condition occurred in practically all types of mines. The condition is not new to us. It is also known that explosives, haulage, machinery and conditions common to all types of mining have taken a heavy toll of human life in mining. In addition to these hazards in the uranium mines, the hazard of radon is also present, Radon is a radioactive gas liberated from ores containing radium. This gas is released into the active workings by the blasting or breaking up of the ore. Sometimes it is caused by radium contaminated water seepage from abandoned workings or from the ore body itself. Until the action of this gas on the human was known, many miners were affected. Un- less adequate precuations are taken to protect the people in these mines,, they may be exposed to this hazard, Health researchers made a study of this substance to determine how it would affect the health of the men, They also studied men who had worked in previous years, in mines known to have contained radioactive materials. All of this research contributed to the present knowledge of radon and its daughter products. It is not flammable nor explosive, but, if allowed to enter the body through the respiratory organs it can cause serious damage. Mines have been operated in the past and are still being operated, which were known to contain explosive and noxious gases, indicating that this problem can be controlled. Several states have instituted laws that make it mandatory to provide adequate ventilation and use of monitoring devices to limit radon and its daughter products to a maximum of 300 micro-microcuries per liter of air. This is considered a safe limit for sustained periods of human exposure. While the findings of research do not prove conclusively that the inhalation of excessive radon or its daugher products cause lung cancer, indications point to this as a possibility, particularly radium A and radium C1 of the daughter products. The threshold limits set by law in most uranium producing states are believed to be safe. In a few years, when tests and data now in the process of collection by interested state officials and laboratories is complete, these limits may be found to be ready for revision. Studies such as these take time. At present, the limits that are used are based upon estimates. The permissible limits of exposure to radon and daughter products include a large safety factor. It is considered that a good supply of fresh air delivered to the working places, adequate dust control, the sealing of unused or abandoned workings, the removal of mine water which is generally contaminated and a safe waiting period after blasting, will hold to a minimum the dangers from this source. There are, of course, problems accompanying each of these procedures. For example, the ventilating of untimbered raises and bringing fresh, uncontaminated air to the working places may be a problem. However, a good mining man can solve these problems as they apply in each particular operation. From the information available at the present time, the daughter products containing polonium and emitting alpha radiation, may be a health hazard. Fortunately, the half life of most of the daughter products is short, however, radium D has a half life of 22 years and presents a problem of contamination in the storage of old tailings. So we can see that failure to comply with the accepted good mining practices may have harmful effects on the health of miners. It is gratifying to note the interest in this potential hazard by the officials in the uranium ore producing states, who are making extensive studies and keeping records of the men who work in this type of mining. This all provide information necessary for preplanning safety. By contrast, little has been accomplished to control the injuries and deaths from some other factors connected with most underground mining operations. Uranium mill tailings disposal facilities present unique problems with respect to siting, design, and long-term stability because their contents remain potentially hazardous for 100,000 years. Dawn Mining Company, operator of a uranium mill in Washington, undertook geological and geophysical studies relative to planned expansion of existing disposal facilities. Seismic refraction and reflection, resistivity, and ground magnetics data were integrated with limited drill data to determine the configuration of Quaternary sediments and the underlying basaltic and granitic bedrock. Analysis of multiple reflections and magnet modelling on a micro-computer with CRT graphics added detail to the interpretation.

The recent discovery of large amounts of helium-3, a valuable thermonuclear fuel, on the surface of the Moon has prompted engineers and scientists to examine the commercial attractiveness, thetechnological feasibility, and the environmental features of this important resource. The main feature of this fuel cycle is the low (˜ 1%) fraction of energy released in neutrons. Such low neutron fluxes result in important reductions in the amount of radioactivity, afterheat, and radiation damage in a fusion power plant. On the other hand, the high fraction of energy in charged particles and synchrotron radiation can be directly converted to electricity, resulting in very high efficiencies of electricity production (˜60.70%). It has been shown that there is 10 times more energy in He3 on the Moon than there ever was in fossil fuels (i.e., coal, oil, and gas) on the Earth. The techniques and equipment needed to extract the helium-3 have been examined and internationally acceptable mechanisms for the industrial development of this resource have been outlined. This paper expands on the financial, environmental, and technical aspects of the recovery of helium-3 for use in the 21st century.

The Safety Pays in Mining v2.0 web application, developed by the National Institute for Occupational Safety and Health (NIOSH) Mining Program, helps mines determine the potential costs associated with mining injuries. This web app categorizes injury cost by part of body injured, by the cause of the injury, or by the nature of the injury. When the user selects one of over 150 common types of mining injuries, the app provides information on the distribution of costs of workers’ compensation claims for that type of injury. Based on other user inputs, Safety Pays in Mining v2.0 will estimate the total costs of the selected injuries and the estimated impact of total injury costs on mining company profits and will provide some examples of services and personal protective equipment on which companies could spend the savings that result from the prevention of injuries. This paper reviews the Safety Pays in Mining version 2.0 web application by discussing the development and updates to the app, how it is used to show the true costs of mining injuries, and how mines can benefit from using this app.

Most contemporary mines have considerable amounts of data. Structured and unstructured large data sets are colloquially named big data. Much of the data in mining operations come from a variety of systems, each with different databases and reporting environments. Standard technology deployments create a Bsilo-ification^ of data leading to poor system benefit. Through modern server monitoring and systematic approach, data utilization and value can quantifiably be measured. The Data Utilization and Value Index (DUVI) can quantify business intelligence best practices and user interaction. This study seeks to provide a data management tool to measure data utilization across the process of converting data into action.

To facilitate the installation of the longwall equipment for each extraction panel, West Elk mine develops set-up room roadways that are between 26.5 and 30.5 feet wide. Historically at the mine, the set-up room roadways have been driven in a single 20-foot-wide pass, then incrementally widened to the required width during installation of the longwall shields. The first pass roof is supported with a generic roof support plan consisting of partially encapsulated roof bolts and point-anchored cables. This ground support plan was utilized regardless of the geotechnical conditions encountered in the roadways or consideration of the required stand-up time of the roadway before the longwall shields were installed. This "blanket" approach tends to result in over-conservative or inappropriate roof support designs, particularly in changing geotechnical environments. Guided by the desire to optimize the ground support plans and increase mining efficiency for future set-up rooms, the West Elk mine and Agapito Associates, Inc. have developed and implemented a strata management program. Strata management, as a formal and structured process, allows ground support and mining methods to be tailored to the anticipated and encountered geotechnical environment in a con- trolled manner (Thomas 2007). To arrive at adequate site-specific ground support plans for the set-up rooms at the mine, several struc- tured processes were utilized. Firstly, the geotechnical environment was characterized by examining the roof lithology and geomechanical properties from nearby rock core obtained from exploration boreholes. To increase both mining efficiency and continuity, and to achieve longer stand-up times before the longwall shields are installed, a multi-pass approach was implemented into the design. During mining, the geotechnical conditions of the roadway are mapped, and the roof is strategically surveyed with a borescope. This process was used to ensure that the encountered ground behavior was in alignment with the design standards used for the roof support design. To support this process with measured data, roof extensometers were strategically installed throughout the roadway. This data was used to monitor the magnitude of roof displacement at various horizons and record displacement rates up until the longwall shields were installed. Lastly, a Trigger Action Response Plan (TARP) was used during mining to cater for any unforeseen changes in ground conditions that would warrant additional con- trols such as increased monitoring or the installation of additional ground support. This paper presents two case histories at the mine where the above strata management approach was implemented to arrive at a site-specific ground support plan. The main changes to the mine's ground control plan included a multi-pass mining approach, the utilization of fully encapsulated roof bolts, and the transition to a stiffer roof support system. The geotechnical mapping, the roof borescope results, and the roof extensometer data will be presented to demonstrate the effectiveness of the ground support system up until the longwall shields were installed in both presented cases.

In Australia, Safe Work legislation requires mines to have a documented ‘Safety Case’ explaining how a mine will safely manage all major hazards. A similar requirement is legislated or inferred by regulation in all jurisdictions. For small scale, instabilities leading to day-to-day seismic hazards, a ‘Stability Safety Case’ built around observational methods and measurement triggered responses can be effective because adverse situations are easily identified and the costs of lower control measures are not onerous. Also, if the influence of a volume of yield on the mine load system is small, the response of the system will be more predictable. However, for episodes of large scale mine instability, the hazard derives from the widespread, multi component influence of the event on the mine load system. As the system response depends on interactions between many parts, the resolution of predictions will be lower and situations can arise without local precedent. Further, as episodes of large scale instability change the stiffness and nature of the connectedness between parts of the mine load system, the behavior of the system before the event will be a poor empirical basis for forecasts of event related hazards. Most importantly, Triggered Action Response Plans (TARPs) and ‘traffic light’ warning systems cannot make design choices or identify new hazard phenomena. They are ‘lower’ control measures targeting explicit human-made instability hypotheses. As such, the Safety Cases related to management of hazard from potential large scale seismic events and mine instability cannot rely solely on TARPs. Mines that successfully manage meaningful dynamic instability always acknowledge the ultimate reliance on expert human intuition, knowledge and imagination to appreciate the range of possible hazard scenarios, to make stability hypotheses and propose solutions. Complex analysis, TARPs and measurements are critical steps in the human decision cycle, but only support the making and testing of ‘Stability Hypotheses’ made by expert engineers. Relying on expert skill and judgement as essential components of a Safety Case, however essential, introduces human vulnerabilities. Another challenge is that the expert decision making process is hard to document. Beck et al 2019 first described an organizational and procedural framework for maintenance of an ongoing large-scale ‘Stability Safety Case' where a degree of expert judgement and imagination is needed to manage a seismically active mine. The structured data appreciation process outlined sufficient measurements, analysis and appreciation to confirm the stability state of the mine and was applicable to all underground mines. After several years of implementation in various forms in stoping, caving and room and pillar mines, this update addresses lessons learned and describes some considerations for numerical forecasting to establish a base case, and some challenges for empirical hazard identification.

The end of an era is here. The pony-express era of industrial relations in the coal industry is ending. Those professionals who know it must end to insure the industry's future are happy, relieved, but somewhat saddened. They are sad because it did have its elements of suspense, thrills, frustrations, and at times, plain fun. The days of the Company town are ended; the wildcats are declining; violence (while a big part of today's industrial relations) is beginning-to decline. The spirit of sound industrial relations is beginning to take hold. This has to happen for the industry to compete in today's economy and it is causing major changes for those who have grown accustomed to the pony-express type industrial relations.

For many years, the combined efforts of machinery manufacturers and mine operators have endeavored to achieve greater productivity from the men involved and the machinery and capital invested, by developing methods to drill blast holes more effectively. Obviously, this means much more than simply developing a drill that will drill a single hole faster. Considerable attention over these years has also been given to methods of carrying and positioning the drilling machine on versatile jumbos, whereby the man's time can he better utilized by the faster drilling machines. Likewise, the art of blasting to utilize the holes that were drilled to achieve the maximum amount of fragmented ore and the methods of moving the broken ore have also progressed. But each phase of the production cycle just described must be made to balance with the other phases of the work. Obviously, major breakthroughs in only one phase don't always generate more "rock in the box" per manshift. Yet, through the persistent effort of the machinery developers and the researchers, the progressive attitudes of mine managers, backed by corporate capital investments, and battle after battle at the operating level, progress is made. Then, if the innovated system is good enough to "stick", in a few years it becomes the system that the newer method must replace, after the same repeated struggle. This is the situation that many mine operators find themselves in today, Looking at their production cycle, which is assumed to be efficient and balanced between drilling, blasting and materials moving capabilities, they tend to guard any visable disruption of the balance of that day's production, and many would prefer to maintain the status quo indefinitely.
