During the past several years — much to the delight of producing firms and countries — copper prices have surged from under $1 to more than $3/lb. Anyone familiar with the industry also knows that since the early 1970s the long-run trend in real copper prices has been significantly downward. This sharp contrast between short-run and long-run trends raises the issue of the forces behind these developments. What has been driving short-run prices up and long-run prices down? Simple economics suggests that it is all a matter of supply and demand. When demand is rising faster than supply, prices will rise and vice versa.

The ability to identify hazards during a workplace examination is a critical skill for mineworkers to have in order to maintain a safe workplace. While research suggests that being able to successfully recognize hazards requires a complex set of competencies, it is unclear which competencies are most critical to adequately perform a workplace examination. This paper presents a hazard recognition competencies framework as an explanation of the competencies that affect mineworker hazard recognition. To better understand how the industry is selecting and preparing workplace examiners, researchers fromthe National Institute for Occupational Safety and Health (NIOSH) interviewed nine health and safety (H&S) professionals at surface stone, sand, and gravel mine sites throughout the USA. A primary purpose of these interviews was to verify the competencies included in the hazard recognition competencies framework and to understand why these competencies are critical. Finally, the results of this study are presented within the context of current NIOSH research findings related to hazard recognition.

Jerry's group uses predictive tools to monitor and predict the equipment failure throughout the mine, from pumps to shovels to motors, you name it. The difficult and often harsh working environment of the mining industry can stress both personnel and equipment to the extreme. Often, the unintended consequences of this environment lead to confrontations between departments, as each goes about its tasks of mining, maintaining or supporting these activities. On occasion, the confrontations may result in criticism of other departments for perceived or real sins or omissions. The offended department may not be able to change the circumstances, but can only complain while frustration persists. The most effective solutions are the actions of the mine manager. By establishing mutually supporting roles of each department and specifying conditions for the interactive conduct of maintenance, the mine manager can better assure that the inherent problems of the mining environment are neutralized.

The concept of risk management is applied to multiple aspects of the mining lifecycle including safety, mine design, business plan—anything that affects getting the ore to the mill for production. However, risk management is not a topic that can readily be taught in universities. Many situations require knowledge gained from experience to know how this impacts current and future operations. Recent graduates, who must develop and apply decisions based on geotechnical monitoring, likely have not developed “operational experience” and may not be aware of options and typical applications. Thorough resources that explain different monitoring systems, and when and how to implement them, are available for geotechnical professionals. Accessibility to this information can often cause information overload, leaving one unsure of where to start, without knowing where to start. This paper is intended to serve as a high-level overview of development practices, levels of risk, and risk tolerance for entry-level geotechnical and mining professionals who do not come from a geotechnical background. Summarized detail on typical monitoring practices, types of instrumentation, and how to communicate what the data indicates to key stakeholders are provided.

Someone once said, "The safest mine is one in which no one goes into." We all recognize this as being perhaps wishful thinking. We also recognize in it a certain amount of truth and an ultimate goal if we are optimistic. All of us are aware of the many things that have been done, are being done and will be done to reduce manpower in mining. For those of us; who are optimistic about this ultimate goal it would appear from the records-we are more than well on our way to the so called safest mine. Let us see what has been done in the past 20 years. T e period from 1935 to 1956. For example in the coal mining industry: In 1935 there were employed 462,903 men who produced an average of 805 tons per man per year or 4.50 tons per man per day. In 1955 there were employed 225,093 men who produced an average of 2,064 tons per man per year or 9.84 tons per man per day. There are no comparable figures compiled for the metal mining industry as a whole that can be broken down in this manner. However, from examination of figures available for different groups in metal mining there is a trend similar to that in coal mining. No-dbout the above figures reflect the intense development and perfection of machines and mining systems to tear out, transport and process our mineral wealth. The figures also show the mining industry has been able to double its production and at the same time reduce its manpower by half. When you compare the number of men employed today with the number which would have been employed 20 years ago to gain todays production at that time it become apparent that greater safety has been accomplished. For example the years 1935 to 1956 for all mining show. In 1935 there were approximately 649,226 men employed in mining and reports show 1,399 fatalities and 74,913 non-fatal injuries. In 1955 when there were 313,883 men employed in mining 496 fatalities and 25,365 non-fatal injuries were reported. To have produced the same tonnage of material in 1935 that was produced in 1955 it would have required the employment of approximately 1,298,452 men. 2,798 could have been fatalities and 149,826 could have had non-fatal injuries. You can now measure the full impact of what has been accomplished in the past 2o years by the aggressiveness of mine management, the wizardry of our engineering personnel and the thoughtfulness and devotion of our safety people and their programs. Therefore, there is a trend towards the goal of our opening statement. We whoare toptimistic believe this trend will accelerate at a-faster pace. Why? Because anyone engaged in the mining industry today can well afford to invest a minimum of $30,000.00 in capital equipment todisplace the labor of one man. This fact alone is a terrific incentive for mine management to apply and manufacturers to develop equipment of a type to get the men out of the mine. Now you can determine this problem not only becomes one of safety but also one of economics, When productivity is stepped up and manpower reduced management is learning the hard way what it means to them; in the profit and los s column when a few minutes of delay or downtime of costly machines and processes occur. They have learned that fast and efficient communication systems are the best means of combating breakdown, delays, bottlenecks, supply problems, the saving of lives and property and increasing production. One large western metal mine was able to increase Its tonnage by 4000 tons a day by using modernup to date electronic type communication system. The National Coal Board of Great Britain recently allocated 45,000,000.00 to study and develop faster and more efficient communications when a survey indicated what a serious bottleneck poor communication in their present systems of mining turned' out Lobe.

Mine exploitation is a broad subject whose focus is the technical skills or efforts needed for the extraction of mineral from a known ore body. Mine exploitation can be considered the very core of the mining engineering discipline. It begins at the conclusion of the exploration and development stages of mining and ends when the ore body is exhausted and the mine (or mines)closes and is reclaimed. Section 8 on mine exploitation is comprised of seven chapters: (1) classification of mining methods, (2) mine surveying and mapping, 3) systems engineering, (4) computer methods, (5) labor relations and training, (6) management and administration, and (7) mine closure, sealing, and abandonment. Each chapter focuses its efforts towards providing detailed and useful information for the engineer on basic mine exploitation principles. The information can be applied to a broad range of mining problems without regard to mineral type or mining method.

At the operations level, productivity is a principle key to profitability. If you are competing with other mines that are paying roughly the same wage rate and the same prices for capital, materials, and energy, the only way your profit can be greater is by producing more output per unit of input. Mines with higher productivity have higher profit margins. Higher profit margins lead to greater operating cash flow, and cash flow is what distinguishes a dog from a cash cow. The turnaround in U.S. metals during the 1980s was in large part due to incredible improvements in productivity. According to statistics compiled by Kaufmann, 1991, labor productivity, measured as tonnes of primary production/manhour, increased 64% in steel, 94% in copper, 75% in lead, 143% in zinc, 132% in molybdenum, and 75% in gold. These improvements were hard won. However, with most modernization programs now complete, measuring and monitoring productivity becomes even more important as producers strive to maintain the gains they have achieved.

INTRODUCTION The small mining company faces the dilemma of how to finance the development of its properties because it doesn't have the financial resources to pay for the development costs from its own funds and doesn't have the financial strength which will enable it to borrow the money needed for development. However, with a little imagination and the right set of circumstances, a financing can be arranged for the small mining company. This paper is an attempt to describe the fundamental steps on how such financing can be obtained --in other words--what is required to provide ad- equate capital at a low cost for developing and bringing a mining property into production. Innovative financing follows from this. The basic keys for building and maintaining a successful mining company are "the four M's": Management, Market, Mine, and Money. The writer believes their order of importance is as presented and each is a building block required in order to obtain the type of financing that best suits the borrower and lender. MANAGEMENT Management must be experienced, highly organized, realistic, able to take advantage of opportunities and fulfill promises. Mine financing requires a variety of skills and concentration on the part of management. Management should have a thorough understanding derived from experience in geology, construction, mining and metallurgy and environment, marketing, and financial matters. Management must realistically visualize what will be produced at what cost and truly evaluate the risks associated so the proper security can be correctly designed. Since risk cannot be eliminated, the objective of management is to identify the risk which will be present in a given venture and assess the level of that risk which will be acceptable to the firm. Generally the risk which is present is subject to little or no modification. After management identifies and measures the acceptable risk the firm will proceed in a way that it will be least exposed. Management must determine its long-term objectives and strategies within the context of a constantly changing world. This question must be addressed before examining the sorts of risk which affect the development and operation of a specific property. The compilation and interpretation of "hard data" by competent people is good to a point but the experience and instinct of seasoned individuals are the important factors in management choosing a long-term direction for the corporation. The attitude towards risk greatly affects the goals of the corporation. Major risks in a mining project can be placed in four categories: 1. Market - Price, demand, substitution. 2. Costs - Capital, operating, financial. 3. Regulation. 4. Taxation. The feasibility study for a project is the fundamental tool used in the management of risk. Management should employ an experienced team with an established and well-understood set of ground rules for the preparation and assembly of a feasibility study. The necessary degree of realism is built into all levels of the study and if the project is technically feasible, the company's hurdle rate is then used to discount the base-case project cash flow in order to determine its financial viability. Understanding the geological significance of the mineral deposit is critical. There should be strong interplay among the pure regional geologist, the detailed mine geologist, the mining engineer, and the metallurgical engineer. The collaboration among these different disciplines will yield a feasibility study that is more reliable. The essence of this work must be appreciated and understood by the executives planning the financing. It is vital that they thoroughly understand the risks and make the proper risk assessments to cope with our rapidly changing market environment. Projecting future revenue values is paramount and is subject to much estimation. One way to mitigate fluctuations and the risk of falling prices is by selling production for future delivery. MARKET Since mining is a worldwide industry it is mandatory that bankers be aware of what is occurring in the marketplace, both foreign and domestic. The methodology of quarrying changed significantly in the Indiana limestone industry in the 60 years between Sept. 11, 1941 and Sept. 11, 2001. The former was the date of the Pentagon groundbreaking ceremony, while the latter was when the building was damaged by American Airlines Flight 77. So too, though to a lesser degree, did the machinery and methods used to fabricate the stone. The rededication ceremony on Sept. 11, 2002 marked the end of the Phoenix Project, as the rebuilding effort was named. The story of this remarkable project illustrates what the American construction(and Indiana limestone) industry can accomplish when working toward a common goal.

Laboratory experiment as a basis of commercial plant design has been the subject of much analysis and thought and on which a large number of articles and papers have been published. We would be forced to choose between an unduly brief exposition if we wanted to cover it completely, or no exposition at all. We must therefore concentrate upon the essentials without ignoring at the same time too many of the qualifying factors which, by the choice of what we want to examine, may become side issues. In doing so, we do not imply that these side issues are therefore any the less important. In every case, what is said here applies with almost equal force to the pilot plant which may be necessary as it does to the commercial plant. In discussing a recent paper by Derek C. Shelton on the Santa. Barbara Mill in Chihuahua at a meeting of the Inst. of Mining & Metallurgy (London), D. J. Ottley said on the matter of relating laboratory results and small scale tests to full scale plant results that he would like' to draw the attention of members to the great difficulty of scaling up mineral dressing plants and processes. He felt that this was an important yet neglected subject which merited considerable fundamental scientific attention from mineral dressers and plant design engineers in particular. How much of this fundamental scientific attention has, as a matter of fact, been accorded to the subject, mainly by our sister profession, Chemical Engineering. The close similarity to chemical engineering problems that we meet, particularly in hydrometallurgical operations, suggests that Chemical Engineering publications offer a rich source of information and discussions of methods useful to us. We have only to think of pressure leaching, ion exchange, liquid-liquid extraction, precipitation and so on, all problems in chemical engineering unit processes or of the new Freeport Sulphur Nickel plant in Cuba or the Sherritt Gordon plant and many others.

"Mining is the main activity that supports civilization. Actual mining presents contradictory concept to the health of the planet. Its business model is linear, fragmented and characterized by inefficiencies in the utilization of water, energy and mineral resources which generate waste and pollution. Before mining begins, it is critical to determine the vulnerability of the ecosystem and after mining, it is critical to determine the depletion of nature. Attending to these vital matters will require quantifying and measuring them. The purpose of this study is to provide and establish the metric to assess the performance and manage a sustainable future for our existence and that of our planet. This index is defined to measure and control Nature Friendly Sustainability (NFS) based on understanding nature. This index will determine the condition of nature constructed on the measurement of physical, chemical and biological equilibrium. Proposing a new mindset for mining design to satisfy nature’s needs would be to offer the same opportunities to future generations. Hence, sustainability is synonymous with equilibrium under a limited ecosystem. Nature is interconnected. Finally, this index of NFS is readable, standardized and relevant to keep nature’s needs in equilibrium. Establishing and NFS index is the next metric for humanity. INTRODUCTION There are a multitude of definitions of what sustainability is. Most of them are defined in terms of the corporate entity from which they emanate. The classical definition of management states that, “if we do not measure it, we cannot manage it.” Management of ecosystems is the definition of sustainability. Indicators of quantity and quality of sustainability are vital in order to create a metric for sustainability. These indicators will measure the depletion and degradation of nature as they occur through human intervention. The first step is to create a metric of sustainability to simplify its definition. The definition of sustainability is based on whether or not you consider nature to be finite or infinite, which is described in Nature Friendly Sustainability (NFS) [1]. The definition of NFS applies to any anthropogenic activity with an understanding of the equivalent activity in nature. That is to say, human activity with nature requires balance and equilibrium in order to be sustainable. The importance of this equilibrium is quantified in a set of metrics in order to establish measurement and acknowledge indicators for our dependence on nature. Sustainability is synonymous with equilibrium in any ecosystem that is limited and fragile. In the graph is the explanation of sustainability definition. See Graph 1"

Introduction More and more women engineers are joining the traditionally male mining industry. Are they successful? Do they encounter special problems? How do men in mining respond? How can the mining industry effectively use its female resources? What contributes to the success of a woman in mining? What can male engineers do to ease the entry of female professionals into the workforce? An STM survey conducted last spring provided some answers to these questions. All BS graduates in mining, mineral engineering, and extractive metallurgy from 1960-1979 were surveyed. Nearly 60% of the female graduates, from 17 universities, completed questionnaires. The women were asked why they chose mining, how they relate to predominantly male co-workers, what job skills they have gained, and what areas of work they most enjoy. They were also asked how problems of a two-career family are solved. Data were gathered on salaries, titles, specialties, and products handled. Respondents, assured of confidentiality, are identified only by title. Many male mining executives were also questioned for their advice to women in mining. Profile of the Survey Participant The "typical" female mining engineer works with coal, is employed by an operating company, feels she works very hard to prove her abilities, does not want to be treated as an anomaly, and someday wants to be an independent consultant. Of the respondents, 44% listed coal as the most familiar product, 12% said copper, and the rest listed products from industrial chemicals to oil. Titles varied widely but women managers are rarely found. However, the majority of the survey population graduated after 1976 and thus lacks substantial experience. Of 102 female graduates, only 25 graduated before 1977. Since then, there has been a steady increase. The American Association of Engineering Societies reported that in 1980 more than 100 degrees were awarded to women in minerals engineering. Education Highly Valued Twenty-two percent of the women surveyed had graduate degrees, 38% planned further education, and 12% listed a PhD as their goal. The majority of graduate degrees are in technical fields. There were only two MBAs. Five women intended to pursue law degrees, and one woman, with a mining engineering degree and an MBA, is a part-time law student. Along with advanced education, the women agree that production experience is needed for career advancement. However, only 28% of the women have worked in production. Many women, especially younger grads, commented that these are the toughest jobs to get. This is a universal complaint, from males as well as females, though women seem to encounter more opposition. Several women mentioned they were small or slight of build and managers tended to shy away from hiring them for production jobs. Two-Career Marriages The most difficult problem for two-career couples is relocation. Whose career takes priority? Fifty percent of the women said transfers or offers would be examined on a case-by-case basis, weighing pros and cons for both parties. The husband's career took priority with 47% of the women; their own careers, with 3%. These statistics conflict with comments from two employee relations managers. Both said most female engineers they've encountered are married to men who follow their wives' careers. Seventy per cent of the married respondents said their husbands were also engineers. Four said their husbands were engineers at the same company. Some male and female engineers met on the job; others joined the company as a "package deal." Both partners being engineers can ease relocation problems, since an area or company offering one spouse a job can often provide a spot for the other. A company may benefit by getting two new hires for the relocation costs of one. Most companies contacted have policies that allow married couples to work at the same division, but discouraged them from working in the same department. With 40% of today's marriages ending in divorce, one risk is that one or both parties may quit. One personnel executive said there were several couples, each spouse in a different discipline, who were work¬ing out very well at his division. However, he recognized the potential for problems if, for instance, one gets promoted and the other doesn't, or salary increases are unequal. The personnel executive said he has actively recruited married couples, and would again, were each person competent.

"Miners worldwide use a tremendous amount of energy in producing finished minerals for the end user market. In accomplishing this goal, they incur energy costs that can range from 20% to 30% of the total cost of production, so there is fertile ground for cost savings. To unlock this huge value in cost reduction, energy analysis is the key and is broken into several segments: 1. Fuel analysis for power generation 2. Energy supply & demand 3. Opportunity cost for lack of power 4. Backup power 5. Power contracts 6. Energy efficiency 7. Renewable energy 8. Continuous energy management FUEL ANALYSIS FOR POWER GENERATION What is the first thing we evaluate when looking at power supply alternatives for the mining industry’s large power requirements? Is it 1) site, 2) engineering, procurement, and construction companies, 3) independent power producers, 4) turbine vendors, 5) grid connections, or 6) fuel supply, logistics, and infrastructure? In evaluations of power supply alternatives for mining companies with special emphasis in developing countries, there is one over-riding conclusion: reliable fuel supply drives the energy generation economic engine. Evaluating several different fuels including liquefied natural gas (LNG), natural gas, diesel, heavy fuel oil, and liquefied petroleum gases (LPG) among others, the best project outcomes are those where fuel supply, pricing, logistics, and infrastructure are given an extensive amount of time during the analysis stage. In an assessment of the twenty-year costs of generating power from multiple fuels, each fuel’s costs were placed into three separate net present value (NPV) buckets of: 1) capital, 2) operating and maintenance, and 3) fuel. Within a range of 65% to 85% for all fuels, fuel cost was the largest component of the NPV of the life cycle power generation costs. So if 65% to 85% of your life cycle power generation cost is one item, shouldn’t miners spend a corresponding amount of time in the analysis stage fully understanding this key determinant for a successful power project? In developing countries where power generation infrastructure may not exist or does not exist at the same level as is found in North America and Europe, fuel analysis becomes even more important. One of the key questions for fuel importation in developing countries is the draft at existing ports to allow larger vessels to dock at the jetty. Can larger ocean-going tankers dock at the port for off-loading, or is the port’s draft too shallow, thus forcing a Single Point Mooring (SPM) system or Conventional Buoy Mooring (CBM) system to be constructed at substantial capital expense? Once fuel can be imported, sufficient onshore storage capacity should exist, or must be built to off-load the entire fuel cargo from the larger ships. Additionally, the throughput capacity of the pipeline from SPM/CBM must allow a rapid unloading of the fuel from ship to shore to eliminate steep demurrage costs for the project."

This paper describes the design and construction of a 22.86 m (75 ft) long jacked tunnel, which was successfully installed, despite difficult mixed face conditions at Metro-North’s Westport Station, in Connecticut in the fall of 2003. The tunnel utilized what is believed to be, the first application of a steel grillage system within the United States. This grillage was installed before the jacking started and allowed the tunnel to be jacked under four live railway tracks, which included the Acela high speed line, without affecting train operations. The clearance between the top of the tunnel and the rails was only 0.9 m (3 ft).

"This study sought to identify changes in continuous mining machine (CMM) operators’ risk perception and risk behaviors as a result of adding a proximity detection system (PDS) into their environment. To accomplish this research task, 9 CMM operators from 5 locations were interviewed. Interviews were thematically analyzed for constructs including patterns in risk perception and behavior before and after PDS integration and the process of adapting to the PDS. Operators discussed standing in the red zone less than they did before using the PDS; however, they also discussed other risky decisions they made while learning how to operate their CMM with the PDS technology such as “cheating” the technology and working faster to meet production goals. The process of relearning job tasks with technology and unintended behaviors as a result of that relearning process need to be taken into account when introducing new technology into the mine environment. The study results show an in-depth view of mine workers’ perspectives and how their job tasks and environment could be or are affected when learning how to use new technology that are relevant to companies and mine operators when introducing new technology into the mine environment. INTRODUCTION Established “red zones” can help prevent striking and pinning incidents by providing guidance to continuous mining machine (CMM) operators about where they should stand and not stand while operating a CMM. Although red zones influence operator placement positions, these guidelines are not foolproof safety mechanisms. Depending on the job task and circumstances of the mine environment, a CMM operator may unintentionally or intentionally stand in a red zone. For example, CMM operators may work in close proximity to their CMM for better visual attention cues and the ability to work at a higher rate of production [1-3]. To further enhance protection and prevent collisions between vehicles and mine workers, a proximity detection system (PDS) was developed for the industry. The PDS determines an individual’s position relative to a specific piece of machinery, issues visual and/or audible warnings to the mine worker to change positions, and, if necessary, disables machine motion [4]. The introduction of new technology within other sectors has been identified as a factor that influences work decisions. Specifically, previous research demonstrates that when technology debuts there can be an initial intrinsic risk perceived among users about how the technology will interact with their current environment and behaviors; however, these ambiguities, including the possible risks and benefits, are seldom studied [5]. Some research suggests that the constant presence of technology can disrupt situation awareness and decision-making capabilities by increasing worker responsibilities and worker cognitive demands [6-8]. Further, if workers’ perceived risks both with and without technology are misjudged, unsafe decisions that result in human error and injury may be more likely [9-11]. Although these statements are provided within a broad contextual framework, they support the need to study how technology is perceived among end users in their own environment. Because research about technological impact on worker behaviors has suggested both positive and negative consequences of technology, this issue is worth addressing in the mining industry. Specifically, research has not determined whether CMM operators view their placement choices in the red zone as safe or risky and what effect PDS technology has, if any, on risk perception and subsequent risk behavior."

The current economic climate of low commodity prices and increasing costs has put a lot of pressure on mining companies to ensure their operations will survive through the current economic downturn. While history tells us that the cycle will turn around and prices will go back up again, how many mining companies will be left to take advantage of the next upturn? On average in Australia, the maintenance of mining equipment accounts for up to 50% of operating costs; which equates to nearly $15bn per year. In addition to trying to boost productivity, mining companies are also putting a lot of pressure on reducing costs; all with the intention of maintaining/increasing profit margins and thereby keeping shareholders ?happy?. The whole premise of our economy is based on ?growth? and ?profits?. Movements in the market place force mine owners to ?react? to short term fluctuations. And if that movement is downward; operations people at the sites are forced to start reducing costs. Is this cost reduction done in a logical, systematic way where all risks (short-, medium- and long term) are considered? Invariably, the answer is no! Short term ?fixes? are normally implemented with little, or no, regard for medium- and long-term impact. And the reason for this is that management is not fully focused on the long term; focus is mainly on survival. Decision making is driven by shareholder sentiment and history tells us that this is largely short-term. So given these paramteres to work within, what role can ?Asset Management? play in assisting management to meet shareholder and market requirements whilst still maintaining long-term viability of their operations (to the long-term benefit of shareholders dare I say). As maintenance costs account for nearly half the operating costs of mining companies; the ?smart? management of assets is not only crucial; it is essential for survival in today?s economic climate. What is asset management? Asset Management, according to the UK-based Institute of Asset Management (IAM) is ?the systematic and coordinated activities and practices through which an organization optimally and sustainably manages its assets and asset systems, their associated performance, risks and expenditures over their life cycles for the purpose of achieving its organizational strategic plan.? How important is asset management in today?s mining companies? IAM goes on to say: ?This definition of asset management represents a significantly greater scope than just the maintenance or care of physical assets, and is closer to the central purpose of an organization. Good asset management considers and optimizes the conflicting priorities of asset utilization and asset care, of short-term performance opportunities and long-term sustainability, and between capital investments and subsequent operating costs, risks and performance. ?Life cycle? asset management is also more than simply the consideration of capital costs and operating costs over pre-determined asset ?life? assumptions. Truly optimized, whole life asset management includes risk exposures and performance attributes, and considers the asset?s economic life as the result of an optimization process (depending upon the design, utilization, maintenance, obsolescence and other factors).?

The US mining industry today is fighting for its existence. We are plagued by the shortage of materials, of equipment, of fuels, of capital, of manpower. If we solve those shortages, we are then frustrated in our attempts to open a new mine or, perish the thought, a new smelter. If we manage to make a profit, demagogues will accuse us of stealing from consumers; and, if we do not make a profit and have to shut down, the same demagogues will accuse us of creating unemployment. Meanwhile, in every aspect of our business - from research, exploration and development, to production, marketing and selling - we are hindered in every way by the bipartisan mismanagement and crippling controls of a government bureaucracy gone amok. We are fighting a war on every front. Some of you realize that ours is a war of ideas. In order to win, our industry must make its ideas heard. But to be heard, we must have something to say - which means: we must know our case logically and consistently, all the way down to essential principles. A principle is a primary truth applicable to a great number of instances. A principle is like a road map: it shows you all the possible alternatives for getting from where you are to where you want to go. Although practically everybody recognizes the usefulness of a map and the need to consult it to find the best way' to get from one city to another, most people are not as careful in charting the course of their life, or the course of their industry, or the course of their nation. And so they act blindly, at random, by the whim of the moment, and get nowhere fast.

"In the recent past, the mining industry has witnessed a substantial increase in the numbers of fatalities occurring at metal and nonmetal mine sites, but it is unclear why this is occurring. One possible explanation is that workers struggle with identifying worksite hazards and accurately assessing the associated risk. The purpose of this research was to explore this possibility within the mining industry and to more fully understand stone, sand and gravel (SSG) mine workers’ thoughts, understandings and perceptions of worksite hazards and risks. Eight mine workers were interviewed and asked to identify common hazards they come across when doing their jobs and to then discuss their perceptions of the risks associated with those identified hazards. The results of this exploratory study indicate the importance of workers’ jobrelated experience as it applies to hazard identification and risk perception, particularly their knowledge of or familiarity with a task, whether or not they had personal control over that task, and the frequency with which they perform that task. IntroductionAccording to the U.S. Mine Safety and Health Administration (MSHA, 2015a), 37 mine workers were fatally injured in accidents at metal and nonmetal mine sites between October 2013 and January 2015, following a record low of 16 in 2012 and in 2011. In 2014 alone, 29 mine workers were fatally injured at a metal or nonmetal mine site, and 52 percent of these fatalities occurred at a surface stone, sand or gravel (SSG) mine. In January 2015, Joseph A. Main, Assistant Secretary of Labor for Mine Safety and Health wrote to stakeholders in the metal and nonmetal mining community and identified several areas where improvement was necessary to promote the health and safety of mine workers. One area identified was an increased focus on “daily and effective workplace exams to find and fix hazards” (MSHA, 2015b).Hazard detection is challenging in the mining industry, despite the existence of regulation. Mining is a major undertaking involving the use of complex heavy machinery, equipment and processes as well as numerous and diverse worker activities that take place in a dynamic, challenging environment (Scharf et al., 2001). In other industries that are similarly dynamic and unpredictable, such as the construction industry, a large proportion of hazards go undetected (Carter and Smith, 2006). It is critical that miners are able to both recognize worksite hazards and accurately assess the risk associated with the hazards, because their health and safety depend on their deciding whether and how to remove hazards and mitigate risks."

The intent of this paper is to provide overview context to allow broader understanding of conventional tailings dam failure mechanisms. The research leverages global knowledge, experience, and data collection and interpretation for the safe and controlled management of the geotechnical stability of a tailings storage facility. The motivation for this review is to facilitate transparent access to tailings dam background and understanding. This paper addresses the core understanding of geotechnical failure mechanisms, and how these eventuate to instability and failure of tailings dam structures. This research focuses on foundation failure, internal erosion and piping, overtopping, seepage, seismicity, and slope instability and provides insight into what factors contribute to failure, how failure progresses due to such failure, anticipating and monitoring for the aforementioned failure modes, and designing to mitigate their risk.
