ABSTRACT This study analyzed the publicly available health and safety policies of 26 mining companies to determine the quantity of health and safety (H&S) practices that mining companies encourage in relation to the Plan-Do-Check-Act cycle. A thematic content analysis of the policies identified elements and practices within the text. On average, companies communicated information on about 7 elements (range 1–14, standard deviation = 3.49) and discussed 15 practices (range 2–34, standard deviation = 9.13). The elements in which companies highlighted the most practices were risk management, emergency management, leadership development, and occupational health. A discussion of the policy trends shows areas that mine sites can improve upon within their Plan-Do-Check-Act cycle, in addition to encouraging the use of both leading and lagging indicators to manage health and safety performance. INTRODUCTION Mining companies and their employees are expected to identify and manage risks at their worksites to ensure the health and safety (H&S) of everyone at the site. The primary mechanism in place to control these risks is a company’s health and safety management system (HSMS) (Boyle, 2012). Broadly, an HSMS is a set of standard, interrelated, and interacting elements used to promote and achieve occupational H&S goals (ANSI/AIHA Z-10, 2012; BS OHSAS 18001, 2007). The practices conveyed within an HSMS are expected to minimize incidents, injuries, illnesses, and even save worker lives (Alsop and LeCouteur, 1999; Arocena and Núñez, 2010). In the United States, both the Occupational Safety and Health Administration (OSHA) and Mine Safety and Health Administration (MSHA) regulate and encourage aspects of an HSMS (US Federal Register, 2011, 2010). Many HSMS documents (i.e., Chemical Industries Association, 1995; Health and Safety Executive [HSE], 1995; International Labour Office [ILO], 2001) attribute the source of their basic management system to Deming’s Plan-Do-Check-Act (PDCA) model of continuous quality improvement and organize their practices within this cycle (Johnson, 2002). The PDCA cycle is a well-adopted approach in health and safety management and promotes continuous learning and adaptability (Robson et al., 2007)."

Luzenac America was formed in July, 1992 as a result of the acquisition of the talc assets of Cyprus Minerals by RTZ of the United Kingdom. Cyprus Industrial Minerals was then merged with the other RTZ talc interests under the Luzenac name. Luzenac is a town in the Pyrenees of Southern France -and the home of Talc de Luzenac, RTZ's first talc acquisition. The Luzenac Group is the largest talc producer in the world.

The objectives of this generic study were to use numerical modeling techniques to determine under what conditions seismic waves generated by an earthquake might cause instability to an underground opening, or cause fracturing and joint movement that would lead to an increase in the permeability of the rock mass. In general, the study showed that moderate earthquakes (up to 0.41 g) did not cause instability of the tunnel or major fracturing of the rock mass. An artificial rock-burst tremor with high accelerations, however, was found to be amplified around the tunnel, and fracturing occurred as a result of the seismic loading in salt and granite. In shale, even moderate seismic loading resulted in tunnel collapse.

BOB FALCONER: Our next topic leader is Scott Lawson. Scott is manager of human resources for Independence Mining Company, Cripple Creek and Victor Gold Mining Company, Victor, Colorado. Scott has 25 years of human resources management experience in the mining industry, surface and underground, precious and base metals. His topic this morning is "It's the Employee That Counts." SCOTT LAWSON: When reviewing the topic of productivity in a union versus nonunion workplace, we need to look at who, and not what, can make the difference in ensuring the success of a production-oriented operation. It's not explicitly whether the property is unionized or not, but whether the employee team (including the "management group") is working together to accomplish common, shared, and meaningful goals. The bottom line is what action or actions can be taken to develop trust and to strengthen working relationships that challenge and compel employees to willingly share their particular areas of expertise. I want to share a real-life experience that happened to me during the summer of 1961. Perhaps some of you will be able to relate to the following story. I was an 18-year-old fresh out of high school, ready for my first true work adventure. I found summer employment for myself and two friends with the New York Central Railroad in Cleveland, Ohio. Our jobs were as laborers, laying ties and rails, and unloading ballast. We made $2.62 an hour. We were fat cats! My friends and I took on the job with a great deal of vigor, to the dismay of our fellow workers. These workers were unionized by the Railway Workers, which I believe were affiliated with the Teamsters. Talk about being naive, we wrote the book in 1961. When we took the jobs, we did not realize the employees were represented by a union, even though we didn't think they worked too hard. Within our first two weeks of work, we established new daily records for laying virtually twice as many ties as had been standard, and we nearly tripled production by the end of our hall of fame railroad careers. We even took naively to the next level by lining up the ties for our afternoon work shift during our lunch break. In 1961, we thought we were the Mantle, Maris, and Colavito of tie laying. For the younger set in the audience, we would have felt like the McGwire, Griffey Jr., and "The Big Hurt" Thomas of tie laying. A section of the union contract allowed for a 60-day probationary period during which the employee was evaluated and also exempt from joining the union. Mineral economics interfaces the mineral sciences and engineering with finance and economics in the analysis of appropriate questions facing the minerals and energy industries. Its practitioners include those trained in the earth sciences; mining, petroleum, and geological engineering; metallurgy and mineral preparation; as well as finance, information systems, economics, and statistics-most usefully in one or more combinations. Allowing for some overlap, three general areas may be identified: evaluation, economic analysis, and resource appraisal. Evaluation begins with mine investment analysis (exemplified by Gentry and O'Neil, 1984) and process costing, for which the STRAMM Handbook (Anon., 1977) provides a well-known basis for mining and beneficiation. But it extends from property appraisal, often used for the determination of taxes and royalties, through the determination and comparison of transport costs (e.g., Richer and Soo, 1977), and to benefit/cost analyses of externalities. Economic analysis, the subject of this section, is displayed in rigor and extent both theoretically and as applied to commodity classes in Vogely (1985), but it is implicit though apparent throughout Lefond (1983). The text edited by Vogely places minerals in their aggregative context with economic growth and trade, proceeds to the theoretical analysis of minerals and their markets, analyzes the structure and performance of the major mineral sectors, and concludes with policy issues: fuel, nonfuel, and environmental. Resource appraisal employs quantitative methods for the estimation of mineral and energy resources. It moves through geologic, geophysical, and geochemical properties to the identification of estimation procedures for mineral endowment and for the economic measure of stocks and flows of specific minerals (Harris, 1984). The most important witwatersrand gold placers occur on extensive unconformity paleosurfaces that were produced during protracted periods of fluvial degradation. The erosional etches on what was generally unlithified substrate indicate that low sinuosity erosion patterns prevailed. The transport systems were dominantly braided fluvial with a minor debris-flow component. The placer sediments were non-cohesive sands and gravels which were evidently transported by fluctuating, yet perennial, discharges across extensive, low relief, bra1dplains, not far from littoral environments. Progress made in understanding heavy-mineral transport processes now emphasizes the importance that selective entrainment probably had in producing concentrations. Cognizance that these characteristics are not unique to the Witwatersrand should be taken into account when designing exploration models.

As a result of the Farmington explosion, the U. S. Bureau of Mines commissioned the National Academy of Engineering to a determine what steps could be taken in a short time frame to significantly reduce fatalities as a result of underground fire and explosion in coal mines. Three areas were named by the Academy as having the greatest potential; survival, communications, and rescue. The Bureau subsequently awarded a $3-5 million contract to the Special Systems Division of Westinghouse Electric Corporation for the development of prototype equipment and procedures. These were recently tested in a simulated emergency situation and will be delivered soon. Contractor recommendations for utilization and for further development effort will be studied and appropriate action taken.

Unlike most ore minerals, the raw materials for lightweight-aggregate, such as clays and shales are abundant everywhere. In view of this ample choice, what are the reasons for many plant failures attributed to the raw material? How can we arrive at a proper choice of the raw material, the proper processing method for the chosen material, predict its behaviour in a plant and the properties of the final product? Determine the uniformity of the raw material and possible harmful impurities? One basic choice is sintered versus kiln-fired aggregate. It will be based on market preferences and availability of the raw material. Almost in every case a good raw material for one process is poison for the other.

A new mining project can raise questions about what effect it will have on the community after it closes. Historically, little was done by communities to plan for and accommodate the changes in the economic infrastructure when a mine project is finished either after the ore runs out or if market conditions deteriorate and make it unprofitable. Projects that were underfunded were generally unable to withstand periodic market downturns. However, data in a recent study conducted by the Seidman Research Institute leads to the conclusion that, even when a mining project is terminated, there can be (and, in the specific case studied, is) a residual beneficial economic effect in the community, which lasts for a long period of time.

General Electric introduced electric drive systems on 85 ton Off-Highway mining trucks in the early sixties. Due to favorable economics, trucks have increased in size to handle larger payloads. Series Motorized Wheel systems were developed to meet this demand. When the technology was available, General Electric switched from use of Series motors to separately excited motors with the added benefits of static exciters and simplified control. Over 20 years of electric drive experience have been incorporated in the STATEX drive systems, which covers truck sizes from 85 to 220 tons. This paper traces the drive system developments from the early stages to the present and takes a look at what's ahead.

Giant gold deposits are considered products of crustal-scale systems. Reduced fluids stored in mid to lower crustal reservoirs were discharged into mid to upper crustal sites of deposition. Elements of the architecture include crustal-scale structures, reduced packages of rock and regional seals. Gas-rich aqueous fluids were common but non-aqueous fluids may have been significant in the deeper parts of the systems. Fluid fluxes in over pressured domains were maintained by porosity waves. Abnormal heat input from the mantle may not be required once the systems were perturbed. Fluid-mixing and hypogene enrichment were probably the most significant precipitation processes.

In today’s challenging mining environment, everyone is focused on minimizing cost per ton. With haulage costs approaching up to 45% of the overall mining costs, and loader performance driving production, optimizing payload on the truck is a big opportunity. This paper will focus on technology’s role in loaders achieving the truck target payload. There are constantly new innovations in technology, but not everyone uses them correctly. Technology is intended to help achieve the next level of performance, but still requires the right people and processes to effectively utilize the benefits. In today’s mining business, miners, engineers, trainers, supervisors, and managers have ever increasing roles and responsibilities as never seen before in past decades. Technology can make these roles more efficient, but fully relying on the technology can detract from optimization. Sometimes the old-fashioned method of loading to a consistent pass match will provide a lower cost per ton. Before blindly trusting your technology feedback, know these three areas discussed in this paper: (1) Prerequisites to utilize technology in payload management; (2) Types of payload management technology; (3) How to use the data to manage operator behavior. The objective of this paper is to provide low-cost recommendations to efficiently increase production. INTRODUCTION Whether miners claim to have been around and experienced it all, or they are the data-driven new generation on the mine site, miners are expected to execute the mine plan at the lowest cost possible. So why is it so difficult to execute this task? Shouldn’t supervisors just have to staff the equipment, tell operators to have a safe day and tally up the numbers at the end of the shift? Let the technology system do the rest; what could go wrong? Lots. Having examined numerous mining operations that use the latest loading technologies over the past several years, one common denominator is the opportunity to improve loader operator performance. Mining companies seek help from consultants, training departments, performance reporting systems and an arsenal of resources. Armed with all this information, but the improvements don’t happen, what could have gone wrong? It comes down to people, not managing the resources to drive the right behavior and as a result, the mine’s performance is not where it should be. Understanding how to make “Great Miners” out of these people is critical to this constant struggle to move more with less and get the costs down. Extraction CPT (Cost-Per-Ton) is most sensitive to optimizing loader production which can be generalized into 3 categories of influence: machine specifications, cycle timing, and continuous management using technology. After establishing a basic production machine configuration, utilizing technology to manage cycle times, operator efficiency and payload can be advantageous. Prerequisites to Utilize Payload Management Technology It is important to begin with the primary considerations that enable the use of technology to continuously manage loading productivity. All systems are interdependent in mining, but these are divided into the simplest terms to remember. First, fix your timing and geometry.

The abstract highlights the increasing use of mobile equipment in mining and its contribution to accidents. The research analyzes injury narratives from Ghanaian surface gold mines to understand the involvement of mobile mining equipment in injuries and explore the use of injury narrative data as a human factors tool. The study identified priority human factor issues, such as operating mobile mining equipment, part removal/installation, and the operator's cabin. The use of Sankey diagrams for multivariable analysis effectively identified specific operator-centered issues that warrant further investigation for improved safety interventions.

When you think of the state of Maine, you probably think of lobsters or lighthouses. The last thing you would think of is metallic mineral mining, and that is for good reason. According to the Maine Geological Survey?s website, ?no metals have been mined in Maine since 1977.? Moreover, Maine?s metallic mineral mining laws and rules, which were last rewritten in 1991, have effectively made mining in the Pine Tree State impossible. Statutory changes enacted in 2012, however, put Maine on a path to make metallic mineral mining part of the local economy in the near future. History of metallic mineral mining in Maine Currently, Maine is not a metallic mineral mining state, though that has not always been the case. The 1800s saw various mining operations in Maine, including what the Maine Geological Survey calls a ?mining boom? from 1879 to 1882, where mining of iron, silver, copper, lead and zinc expanded rapidly. Mining and mining activity in the more recent past, however, drove the debate before the state legislature when this issue was debated in 2012.

Magnetic metals are the granddaddy of products which may be recovered from municipal waste. Aside from hand-picking of newsprint and corrugated boxes, magnetic metals are usually the first items a resource recovery facility considers extracting from the solid waste stream. Magnetic metals from incinerator residues and from municipally mandated source segregation programs were first used in significant quantities by the Copper Industry. The ability of the Copper Industry to consume metals from the municipal solid waste stream raised everyone's hopes that there would be more markets for municipal scrap when more of it became available. Somehow something has gone wrong. It is widely acclaimed that scrap is an environmentally sound and economical source of iron. It is greatly in demand for Its contained energy. The prices that the steel industry has been willing to pay for the scrap during the past year are all the proof anyone should need that scrap Is desirable and sought after. But the very fact that I'm speaking today on the markets for magnetic metals recovered from municipal waste indicates that something Is wrong. Clearly the problem is not that we do not need the scrap. The problem is we have not produced the kind of scrap that consumers are willing to buy - either through ignorance of what the consumers wanted or through a conviction that ferrous scrap is so obviously a good thing somebody will surely want it no matter what. Dr. Harvey Alter of the National Center for Resource Recovery has said "You can't sell garbage". In a sense that is only partly true. It really should be "You can't sell garbage for very long". The Steel Companies, in an effort to prove that steel is an environmentally desirable packaging medium, have bought some scrap that can only be described as garbage. They are to be commended for doing so. The fault lies not with the Steel Company for taking and paying for "garbage" but with the producer of that poorly prepared scrap. Far every instance where the steel industry has purchased the often undesirable product from resource recovery facility there are several magnetic installations sitting Idle because someone forgot the most essential part of producing the magnetic product - selling it to someone.

"The Safety Pays in Mining web application, developed by the National Institute for Occupational Safety and Health (NIOSH) Mining Program, helps mines determine the potential costs associated with mining injuries. This web app groups injuries by type, either by the cause of the injury or by the nature of the injury. When the user selects one of over 30 common types of mining injuries, the app provides information on the distribution of costs of workers’ compensation claims for that type of injury. Based on other user inputs, Safety Pays in Mining will estimate the total costs of the selected injuries, including an estimate of additional indirect costs, the estimated impact of total injury costs on mining company profits, and provide some examples of other items (services, equipment) on which companies could spend the savings that result from the prevention of injuries. This paper reviews the Safety Pays in Mining web application by discussing the development of the app, how it is used to show the true costs of mining injuries, and how mines can benefit from using this app. INTRODUCTION Injuries on the job cause pain and suffering. They can also profoundly affect profits and daily operations. In addition to paying direct costs or increased premiums for workers’ compensation insurance, a company might need to pay overtime for other workers to fill an injured worker’s job role, cover training costs for a replacement worker, or divert administrative resources in the wake of an injury. Safety Pays in Mining is a web app that estimates the distribution of these injury costs and assesses the impact that occupational injuries have on a company’s profits. The costs of specific types of occupational injuries in mining are not readily available as mining and insurance companies don’t usually share this information. As a consequence, companies only have cost information based on previous experience with their own employees. Therefore, if a mine never experienced a finger amputation for one of its miners, it would not be aware of the possible costs associated with this type of injury. In addition, injury costs are unique in that the cost distribution is so wide — with half of the injuries in our dataset having mean costs higher than 90th percentile costs — just using the average cost of a specific injury type does not provide adequate information. Some injuries involve immensely high costs, and even though the risk of these high-cost injuries occurring is low, mines need to be aware of their potential impact on their company’s financial health."

Fatigue in mining operations is a serious issue and a significant contributor to incidents and accidents. While mine operators are using or introducing new technology to monitor operator fatigue, there is little work that focuses on the subjective experience of fatigue and the use of fatigue monitoring technology. To investigate this issue, eight focus groups with a total of 44 operators of heavy haulage equipment working for two mining companies in the Mountain West of the USA participated in 1-h focus groups. The topic of the focus groups was the experience of operator fatigue and issues surrounding fatigue at the workplace. The results of the content analyses suggest that operators are well aware of the signs of fatigue, and that they understand the severity of these signs as well. In addition, they are aware of the effect of fatigue on driving performance and are using a wide range of methods to manage fatigue. Finally, based on participants’ assessment, current fatigue monitoring systems lack face validity and reliability and are perceived as not being effective. The results of this study emphasize the importance of a socio-technical systems perspective in the context of miner health and safety management. Only such a comprehensive perspective can result in improvements that are sustainable.

One might ask why someone like myself, involved in the determination of taxes, should be speaking at a "cost engineering" session of a national engineering society. While it is true that taxes, especially income taxes, whether they be federal, state, or local, are not usually thought of as a "cost" of doing business, it is equally true that these tax costs do reduce the distributable profits just as much, as the cost of power, supplies, or labor. Therefore, the consideration given to taxes is often as important as that given to other costs. Hopefully yea will recognize that, within certain limits, taxes can be controlled to an economic advantage. That question may then lead to another question: What does the amount of taxes have to do with me? I'm an engineer or manager; not an accountant or a lawyer. If I may take a few minutes of your time, I might be able to present in a brief discussion some interesting facts that will provide the answers to those questions. My discussion will be directed mainly to Federal taxes. However, much of it will be applicable to state and local taxes, also.

A major question in the design and permitting of a tailings storage facility is the quality and quantity of seepage which will occur from the base of the tailings facility into the underlying foundation and groundwater regime. A corollary to this is what liner system, if any, is necessary to protect the groundwater regime from unacceptable contamination. Modern tailings storage facilities are generally designed as drained tailings deposits whereby all free water is removed from the facility by decant, drainage and evaporation. This removes the source of on­going seepage and allows for efficient and timely reclamation of the facilities. Quantifying the residual seepage from the base of the tailings deposit is complicated due to the following factors:
