Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

A discussion of fillers would not be complete without allotting adequate time to that unique and different mineral material, diatomaceous silica. Historical references to its use challenge the aesthetic aspect of man's curiosity; whereas modern scientific tools have provided a dimension of measurement that has motivated many of its more recent applications. A brief review of what is already known is in order. The diatom is a unicellular, plant-like, microscopic organism, related to the algae1. Its natural function appears to be that of a food for other marine or lacustrine organisms, The diatom's "claim to fame" from an industrial viewpoint is dependent on the fact that it secretes an opaline siliceous skeleton that must survive, chemically and structurally, the geologic processes that caused its formation in the first place; its subsequent demise, burial, consolidation, and alteration; and finally, its ultimate exploitation by man--in somewhat that order. In addition to its siliceous nature, the skeletal structure of the diatom possesses a variety of shapes, patterns, and sizes that have not only been the intrigue of microscopists for decades, but have served to set diatomite apart as an industrial mineral; and have reserved for it applications that cannot be satisfied by any other material. Its chemical inertness, its intricate structure, and its intrinsic hardness-coupled with fineness and friability-- are generally known and have accounted for its application in a wide range of filtration and filler uses. The increasing use of the electron microscope has considerably expanded our knowledge of heretofore unseen secondary and tertiary structure of submicron dimension in the diatom valve. The development of electro-sensing methods of particle size measurement which seem to supersede sedimentation and microscopic techniques has portrayed size distribution in a more realistic light. Interest in the surface chemistry of these silica structures has further strengthened our understanding for certain uses. Thus, new tools and ideas have made possible the introduction of diatomite into such sophisticated technologies as that of chromatographic supports, its improved utility in paint applications for the control of gloss and sheen, and its usefulness in the entirely new concept of "Dry Acid Concentrates." Each of these applications has been discussed elsewhere before people particularly interested in analytical chemistry, in paint, and in liquid handling, respectively, where the diatom has been a means to an end. The emphasis here, before the mining fraternity, however is on diatomite as a raw material with reference to the uses mentioned above. It is therefore toward an awareness and understanding of diatomite properties pertinent to these newer filler applications that this paper is directed.

Open-type, or main beam, tunnel boring machines (TBMs) can achieve impressive rates of production. This type of TBM typically outperforms other types of TBMs currently in use around the world. In good ground conditions, main beam machines have achieved production rates of more than 1 km/month (0.6 miles/month). However, installing effective ground support in poorer rock conditions on main beam machines can be a major cause of concern. The design of main beam machines is such that installation of ground support equipment, particularly at the front of the machine in what is commonly known as the L1 support zone, has traditionally been limited to rock drills and basic working platforms. Personnel working in the L1 zone have, until recently, been protected by roof supports and extended fingers, often with restricted access to working areas.

A complex soil profile and high water table, together with a below-grade active subway tunnel, an above-ground elevated railway line, and the heavily travelled Northern Boulevard all passing above the tunnel alignment, combined to make mining of New York?s Northern Boulevard Crossing what is widely considered to be the most challenging portion of the multi-billion-dollar East Side Access project. The Northern Boulevard Crossing is a vital link in this project which, when completed, will connect the Long Island Railroad to Grand Central Terminal in Manhattan. How the sequential excavation method (SEM) tunneling was successfully accomplished, despite the obstacles, was a textbook exercise in practical innovation coupled with close cooperation between the owner, the construction manager, Hatch Mott MacDonald, the designer, Parsons Brinckerhoff, the contractor, Schiavone-Kiewit, and spe-cialty geotechnical contractor, Moretrench.

Disposal of solid and liquid wastes from mining and milling operations has become a focus of attention in recent years. Consequently, the geotechnical engineer is more frequently associated with design of disposal facilities. Jack Wulff, a representative of one consulting firm with experience in this field, talks with ME about the geotechnical engineer's contribution. Mr. Wulff, mine and mill waste disposal today is receiving more attention-and many more dollars-than in the past. Regulatory agencies have promulgated demanding requirements concerning mill waste (especially uranium mill waste) and specialist consultants are frequently being called in to design disposal systems. What can the experienced geotechnical engineer add to the development of waste disposal facilities? It's true that geotechnical engineers have recently become more visible in the minerals sector, but we've been around mining for a long time-as foundation engineers for mills and mine support buildings, and for geotechnical aspects of open pits, pipelines, railroads, roads, and so forth. We've also designed water supply dams, which could be considered recursors of tailings dams and other forms of disposal systems. Let me show by example what geotechnical engineering can contribute to waste disposal facility design. I recently was asked to look at several small wastewater impoundments which had failed. The design of the earth embankments appeared, at first glance, to be rather conservative. They were only about 3.4 m high, the slopes were 3 to 1 (horizontal to vertical), with about 24 m of bottom contact with the earth foundation. But there had been three episodes of dike failure in a few months, all upon initial filling. The problem was the nature of some material in the foundation that was also used as structural fill-a dispersive clay that readily deflocculated and eroded on contact with water, especially with seeping or flowing water. The design did not include the defensive measures required to use such materials confidently in water retention structures. Apparently, the designer did not recognize the potential problem posed by the unusual soil characteristics. That's the sort of engineering information fundamental to geotechnical engineering, but not necessarily a part of the civil, structural, or mining engineer's professional training or experience. This difference in expertise is not simply a matter of factual knowledge-it also affects the design process. The structural engineer can design a structure to fit the client's functional needs and can specify the materials (grade of steel, PVC, concrete, etc.) to meet the demands of the design. The geotechnical engineer's design process is somewhat different, since we work with natural materials, not man-made materials produced according to specification. We first have to establish what kinds of materials are available (usually directly from the chosen site, since importation of materials becomes expensive). Then we determine the engineering properties and characteristics and, finally, design the structure to fulfill the client's requirements within parameters dictated by available materials. In brief, the geotechnical engineer works with naturally-evolved, not man-made, materials and uses design techniques developed to accommodate that fact. Because no two sites are exactly alike in configuration, structural (geologic) characteristics, or available materials, there will be unique design and judgmental questions to be resolved on virtually every project. You said that water retention dams could be considered the precursors of tailings dams-you probably had a specific technical relationship in mind. There's a very strong carryover of methodology from earth dam design, where the technology developed and where it has become very refined. I found the article most interesting and timely. I consider it imperative that mine case studies like this one are undertaken and published so that the practicing mining engineer and geologist can obtain a better idea of what is the preferred estimation and grade control technique to use, given the site-specific geology, grade distribution, etc. Mining companies, with operating gold mines, need to be encouraged to either initiate such a study in-house or support a research project by financing a graduate student at a local university. The latter method is preferable since it not only provides research support but also an excellent training opportunity for young mining engineers and geologists. Ideally, what is needed is exploration drill-hole, blasthole, and mine production data that can then be used to assess the efficiency of different estimation and grade control methods by comparing the results obtained using the drill/blasthole data with the actual production data. It is hoped that more mining companies will realize that the rewards from this kind of research greatly outweigh the sometimes perceived problem with releasing data to a university. I noticed that the authors used the blasthole data to represent the actual values, the validity of which relies heavily on the two key assumptions with regards to blasthole assays and in-pit ore-waste selection procedures. Could the authors comment in more detail on these assumptions and what criteria they used to check the validity of the assumptions? Since the article was published, have the authors had the opportunity to compare their estimates with the actual production data? It would be interesting to compare other estimation methods with OK and IK. For example, how well did the polygonal method used at the mine compare with the results given in the paper? I would like to mention that B.L. Kwa completed a similar study (Kwa and Mousset-Jones, 1986) using exploration drill-hole, blasthole, and production data from the Alligator Ridge mine in Nevada. The study compared a variety of estimation methods, including IK, and the final results are shown in Fig. 1. The IK method exhibits the smallest difference between estimated and production results and is closely followed by the indicator moving window (Ind MW) method. Reply by Y.C. Kim We do concur with Professor Mousset-Jones with respect to the merit of such case studies performed at a local university. In answer to his request for comment on the validity of the two key assumptions made in our paper, we can only say that these are reasonable assumptions that are very difficult to validate. The assumption of blasthole assays being accurate can be partially validated by a careful analysis of the nugget component of blasthole variograms. The assumption that the in-pit ore-waste selection procedure in use was an effective one can only be validated through comparison between the mine estimates of ounces of gold versus the actual recovered ounces of gold at the mill. Neither one of these two assumptions has been validated to date due to several reasons. For example, the mine went through a change in management since the study. Similarly, no compelling reasons have been presented to perform detailed variogram analysis of the blasthole data. We do, however, believe that the obtained results (i.e., indicator kriging performs better than OK in a highly skewed deposit) will remain the same in relative terms, regardless of how valid these two assumptions may be. The most recent com¬parative study by D. S. Lindsey of the Montana College of Mineral Science and Technology also shows a similar ranking between ordinary kriging versus probability kriging, which is an extended version of indicator kriging.

Introduction In 1969, the US devalued the dollar. This action, over the ensuing four years, resulted in the deregulation of the price of gold. It also resulted in the rebirth of the US gold mining industry. The extreme inflation of the 1970s led to a dramatic increase in the price of precious metals. Consequently an increased interest in the exploration for and mining of gold has occurred. The enthusiasm began among the few existing producers who had survived on marginal profits during a prolonged period of fixed prices and constantly rising costs. Then came the newcomers. They saw the opportunity to enter an industry that historically had been characterized by small operations with low capital costs. It was not until the late 1970s that large mining companies recognized that investment in gold projects could generate cash flows that previously were associated only with world class deposits. Today, gold mining has tremendous appeal for the entire mining community, despite the dramatic price drop from 1980-81 levels. There are even major firms with no mining experience attempting to gain a foothold. But how easy is it to enter the US gold mining industry? What are the basic factors that influence the economics, and what are they likely to be for the remainder of the decade? How has the industry changed, and what is the composition of the active companies? An examination of the industry will help to answer these questions. US Gold Mining History The current gold rush is actually the fifth in the 208-year history of the US. The first recorded commercial mining of gold occurred in the Southeast in 1799. It produced from small, yet relatively rich placer deposits. The famous California gold rush of 1848 placed the US as a major producer with 62.2 t (2 million oz) of output by 1850. More than 93.3 t (3 million oz) was mined by 1853, equivalent to 63% of world production. But the richest placers were soon depleted. By the end of the Civil War, annual production had declined to well below 62.2 t (2 million oz). The government then relaxed prices and the industry again began to recover. The 1870-80s saw the great mines of the California Mother Lode and Grass Valley brought into production. This was followed by development of Nevada's Comstock, which later became one of the world's largest producers. Discovery of gold at Cripple Creek in 1892, coupled with technological advances in the use of large dredges in the California placers, resulted in significant production increases. And, with the expansion of the Homestake mine at Lead, SD, in 1898, US output again surpassed the 93.3-t (3 million-oz) level. By 1905, produc¬tion climbed to 124.4 t (4 million oz) with the addition of the Alaskan deposits, new output at Goldfields, NV, and the introduction of the cyanidation process. During World War I, the government forced the shutdown of gold mines. By 1920, total production had dropped to little more than 62.2 t (2 million oz). Many mines remained closed until the Great Depression. This, combined with the devaluation of the US dollar in 1934, resulted in a gold price increase to about $1.13/g ($35 per oz). Then began the fourth gold rush. By 1937, output had again surpassed 124.4 t (4 million oz). This was due to an increase in primary production and new byproduct production from base metal operations. The control systems engineer is often faced with the problem of designing tests or experiments where the cost of obtaining information is expensive. This is true in aerospace as well as in process industries. In these situations the details of test design should be carefully evaluated before any test is executed so that the maximum amount of information is obtained for the effort expended. In recent years there has been a steady trend towards increased automation, In the process industries this has resulted in extensive review of process variables and, instrumentation techniques, development of more accurate system mathematical models, and extensive investments in complex control system as well as completely automated plants. When planning and developing these systems, where each measure of a process variable is usually expensive, it behooves the design engineer to give considerable thought to the details of test design before instrumentation is procured and installed so that the maximum amount of information can be obtained for the expense incurred.

The pyrometallurgical industry plays a vital role in producing metals for our modern life. With growing demand of metals and declining grade of mineable ore, innovations become critical for many mining companies. It is necessary to conduct a review of recent improvements to the pyrometallurgical industry on analytical measurement techniques, process and equipment improvements, and computer-based modeling/simulations. Many of those innovations tend to be applied to wider metals, and not be restricted by a specific metal. Therefore, we tried to cover the wide applications of many improvements to wider range of metals to reflect this trend. The challenge of a lack of robust measurement techniques under high-temperature conditions within a furnace hinders optimization of a pyrometallurgical process. Some future opportunities in the pyrometallurgical industry on analytical measurement techniques, computer modeling, and processes and equipment have also been outlined and discussed.

The first introduction of fuzzy theory in the nineteenth century created room for continuous research and application in various fields. Fuzzy set theory has been globally applied in geotechnical engineering, and research in this field continues to date. The fuzzy inference system is considered to be one of the most popular techniques adopted to resolve some of the geomechanical challenges faced in both surface and underground excavations. This paper unpacks fuzzy theory-based approaches in mine geomechanics with the aim of expanding the innovative application of the same approach specifically in tunnel geomechanics. This aim was achieved by conducting a review of recent successful and unsuccessful applications of fuzzy inference systems in underground excavations/tunnelling geomechanics. Indeed, this review has outlined some cardinal points associated with the ability of the technique to solve complex geomechanics problems. However, the success of the technique was accompanied by a few limitations associated with the methodology. Finally, a future outlook associated with the technique has been established.

Major scientific discoveries cannot be programmed. New or frontier technology, however, can and should be planned. It is the vital force underlying the business strategy of a vigorous and profitable mining and metals company. Generally, a radical departure from existing technology is required to 1) Enhance the competitive value of ore reserves, i.e., to turn low grade or marginal reserves into competitive reserves. 2) To increase yield and decrease costs. 3) To decrease investment. 4) To increase mine life. 5) To improve product quality to meet market demands. The very definition of frontier technology implies that it is so different that the old plant, if already in use, will not be directly suitable to use the new technology. Therefore, a priori, we know that the commercialization of frontier technology would very likely involve new investment. The recommended technique of economic and technical analyses, I propose to call "Success Analysis.'' Simply stated, it forces us, by step- by-step procedure, to ask "what is needed to succeed in this particular project." Therefore, the first step is to identify what is meant by "success." The definition of "success" requires an agreement with top management or sponsors of the project as to what constitutes "success" in their minds as contrast to the technical people perception of "success." In general, top management would define success as meeting: 1) The technical objectives, i.e., technology being developed would produce the desired product at the acceptable quality. 2) Financial objectives, i.e., when commercialized, the project's capital cost, operating costs and return on investment are better than the projects they would normally fund. The return has to be better because new technology is inherently riskier. 3) Marketing objectives in that the project will produce product of acceptable quality in timely manner, i.e., scale-up to commercial plant would not cause undue delays in start-up and that large fix-up expenditure would not be required to protect the projected return on the investment. Many projects are started in the laboratory to develop "Frontier Technology" in mineral processing. Most of these efforts are never commercialized. There are many causes of these failures. The major causes are listed below: 1) Inadequate communication with the sponsors of the project so that project objectives are defined and agreed to by all. 2) Ill-defined technical problems. 3) Incomplete development of technology. This has two major components: a. Incorrect scale-up and engineering. b. Inadequate start-up and maintenance planning. 4) Incomplete and over-optimistic process economics. 5) Lack of understanding of the future behavior of the marketplace. 6) Failure to complete R&D effort by the time it is needed. 7) Incorrect projections of human resources required; that is, technical, managerial and operating. 8) Lack of realistic forecast of the expenditures required for commercialization of the R&D effort. These pitfalls can be avoided by comprehensive planning and the use of success analysis technique. The major elements in "Success Analysis" are: 1) Identification of the "need" for new technology. 2) Unbiased and complete scale-up engineering. 3) Acceptable economic feasibility with enough margin of error in the sale prices and operating costs to justify the inherent risks in frontier technology. 4) Timing for availability of investment capital and markets. Many failures in the development of frontier technology are preordained because the initial selection of the research project has been without adequate attention to the needs of the industry. Many very innovative hydrometallurgical processes for recovery of copper from concentrates were developed in the past twenty years. They have not adapted except to a very minor extent because they did not satisfy the success analysis criteria. Adequate planning for the development, operation, and reclamation of a surface mine requires attention to present and future uses of land, both on the mine site and in the mine vicinity. This paper outlines land use implications of surface mining, and suggests procedures that could link mine planning, mine operation, and reclamation to more general land use concerns. Mining is viewed as a temporary use of land that may irretrievably commit land resources away from some future activities, but may allow other uses, some of which may have been impossible without mining. The paper accepts the premise that surface mining should attempt to minimize environ-mental damage and to maximize future alternative uses of land by appropriate reclamation. I. Back injuries arising from such activities as lifting and handling objects have been and continue to be constant and undiminishing hazards of the occupational environment. Back pain in one form or another is experienced by almost a1 1 individuals. During the past 30 years, a great deal of accident prevention education has been directed toward the reduction of back injuries. However, it has not produced any great change in the incidence of the injury. The human spine remains the most vulnerable area of the muscul o-skeleta1 system. Back injuries account for about 400,000, or 18 percent, of all disabling work related injuries each year according to the National Safety Council estimates. These serious back injuries are estimated to have cost more than 1.9 billion dollars in 1976 in medical and lost time pay. 1

"The Mine Improvement and New Emergency Response Act (MINER Act) of 2006 (PL 109-236, 2006) established the Office of Mine Safety and Health, functioning as the Office of Mine Safety and Health Research (OMSHR) within the National Institute for Occupational Safety and Health (NIOSH) and provided authority for awarding contracts related to the development and implementation of new mine technology and equipment. The OMSHR external contracts program was formed in accordance with the MINER Act and has since issued more than 150 contracts focused on worker health and safety. Initial funding for this program was provided by the United States Congress through two emergency supplemental appropriations and was focused on emergency oxygen supply, communications and tracking, and refuge chambers (also referred to as refuge alternatives). Since the MINER Act, Congress has continued to fund the program through the annual NIOSH mining appropriation. The program was supplemented starting in 2009 with an academic capacity building program directed toward developing university tenure-track faculty and facilities and supporting graduate students in the area of mine ventilation. Ground control was added as an additional focus area in 2011. A short synopsis of each contract is published on the NIOSH Mining website (www.cdc.gov/niosh/ mining/researchprogram/contracts/index.html) and final reports are available in digital form on request. A summary of the progress OMSHR has made over the 10-year period since the Act was published previously (Snyder et al., 2016).External contracts are broadly classified by the following areas:• Communications and tracking.• Emergency response.o Self-escape and breathing air supply.o Refuge alternatives.o Escape and rescue.• Equipment safety.• Fires and explosions.• Ground control.• Mine environmental systems (mine ventilation and atmospheric monitoring).• Worker/workplace health and safety.Contracts are issued under a request for proposal (RFP), through a broad agency announcement (BAA) or under an interagency agreement (IAA). An RFP solicitation includes a specific problem description and a well-defined scope of work to guide the offeror in preparing a bid to conduct the research. Under the BAA several problem areas are described and the offeror develops the scope of work and deliverables for the proposed research. An IAA is very similar to an RFP except it is a contract between two federal government agencies."

My topic today is future trends in compensation for the mining industry. I seldom get the chance to speak about a subject of such immediate interest to an audience, so I would like to make the most of this opportunity by telling you that everybody in this room will become rich beyond his wildest dreams in the next few years. I would like to open my talk that way, but in compensation, like everything else, there is a great gap between dreams and reality. Back to reality. When we talk about compensation, we?d better define our terms because the compensation is really an umbrella for five basic types of payment made to management personnel. Slide 1 identifies these elements of compensation.

Five years ago, Phillips Coal Company decided replacing mainframe/workstation programs with a new package of PC software was clearly the best solution for computer reserve modelling. The system chosen was actually three programs distinguished by their power, cost, flexibility and integration, and ease of learning and use. Phase I involves the use of the "StratiFact" program for borehole data management and seam correlation. This phase is probably the most neglected, yet critical aspect of reserve evaluation. Phase II employs the "PC/Cores" package for modelling and volumetrics. Phase m is "PC-Cut" which provides mine planning tools including an automated cut generator.

Traditionally the mineral and mining engineers have characterized particles by sieving and-settling. Within the last decade, with the advent of the scanning microscope, entirely new experimental and theoretical approachesl-10/have been taken to minutely define the particle surface. This approach depends upon a hypothesis which takes advantage of the recent developments in automatic micro-scopes which can acquire large amounts of information in a short time and the new computational techniques which rapidly reduce the information to meaningful, practical data. Historically sieving and settling are the oldest sizing techniques known. Sieving by square screens, shown on Figure l-A is a common, current practice for sizing particles both in the laboratory and in production. A woven cloth is used which usually has square openings. The size particle that will pass through the opening dependent on a complex relationship of its morphology. As can be seen in Figure l-A, neither the width nor the height of the particle is determined by sieving. From square screens one and only one parameter characterizing the particle size is obtained, that parameter is the radius of the sphere that will just pass through the given sieve opening. A variation of square mesh screen is the slot screens, shown in Figure 1-B. Here another measure more closely associated with the height of the particle is obtained. Once again, one and only one parameter is associated with the particles' shape.

Historically, geologic data has not been adequately considered in resource estimation. More recently, with the use of computers and sophisticated estimation algorithms and geostatistical justifications, geology has been downplayed further. Many reasons are given for not using geology including cost, time and irrelevance. None are valid. Boundaries defining mineral domains should be used in computer-generated resource estimates. Selected case histories show how the failure to honor geology has caused discrepancies in resource estimates of actual in-place resources. After-the-fact differences have included decreases in tonnage of up to 50% and increases up to 300%; partial to complete recategorization of material; and 50% decreases in grade. The costs of gathering and using geology are trivial compared to the potential consequences.

Booster fans increase air pressure to overcome resistance, the objective being to force adequate amounts of air through distant workings. They are used in areas that are difficult or uneconomic to ventilate with main fans alone. Booster fans are currently permitted in underground coal mines in some countries; the United Kingdom, South Africa, Australia and Canada have been defined as major users. Booster fans are not allowed in the US bituminous and lignite coal mining operations at the present time due to safety concerns. This paper presents the history of booster fan use in coal mines of the United States during the last 90 years. Changes in regulations, as well as advantages and disadvantages of booster fan use, are discussed. Research and petitions for booster fan use are highlighted in order to bring the debate into focus.

REMARKS OF DR. THOMAS V. FALKIE, DIRECTOR, BUREAU OF MINES, U.S. DEPARTMENT OF THE INTERIOR, BEFORE THE FALL MEETING OF THE SOCIETY OF MINING ENGINEERS, AIME, ACAPULCO, MEXICO, SEPTEMBER 23, 1974 This is a time of uncertainty on the world mineral scene. For some of the world's so-called "developed" countries, reserves and production of key minerals are running low. And, we have learned that the energy so essential in recovering and processing minerals can suddenly be in short supply. The prospects for a "no growth" situation in energy or mineral consumption are practically nil. Moreover, the world's economy is in flux, casting doubt on whether traditional mineral customers will continue to be able to buy, or for that matter, whether traditional suppliers of minerals will continue to sell. It is clear that the mineral situation is contributing to world inflationary pressures, which incidentally are most damaging to developing countries. So it is appropriate that SME members devote this session to the world mineral situation, and consider some of the opportunities and responsibilities that we will inevitably encounter as the future becomes the present. The opportunities, to me at least, are especially intriguing, I have the good fortune to be part of an organization that has an enormous capability and responsibility for dealing with mineral problems. The Interior Department's Bureaus, including the Bureau of Mines, have been around for a long time. They are. a prolific and reliable source of both mineral information and mineral technology.
