The 1889 Johnstown Flood, which killed 2,209 people, was a preventable disaster stemming from systemic decisionmaking failures. This paper applies modern Risk-Informed Decision Making (RIDM) frameworks to analyze the historical context and explore alternative outcomes through hypothetical multiverse scenarios. By reconstructing the risk landscape of the South Fork Dam, we demonstrate how structured hazard identification, risk assessment, and stakeholder communication could have mitigated or prevented the catastrophe. Three counterfactual scenarios—proactive maintenance, community engagement, and regulatory oversight—are evaluated to quantify potential lives saved and economic benefits. The study acknowledges historical barriers, including technological limitations, lack of governance, and socioeconomic disparities, which constrained risk-aware decisions in the 19th century. The analysis draws critical parallels to contemporary engineering challenges, particularly in tailings storage facility (TSF) management, where similar risk blind spots persist. The paper concludes that RIDM is not merely a technical tool but a governance imperative, essential for ethical and sustainable engineering practice. By learning from historical near-misses and failures, the modern engineering community can strengthen resilience and accountability. This work contributes to the growing body of retrospective risk analysis, offering a methodological framework for applying RIDM to historical disasters to inform future safety standards.

Mining companies registered with the SEC were required to comply with Regulation S-K 1300 for their annual report covering the first fiscal year ending on or after January 1, 2021. Consequently, most mining registrants filed their first S-K 1300 compliant disclosures in 2022. The US Securities & Exchange Commission (SEC), as part of its disclosure review program, has been issuing comment letters in response to these filings. This paper presents an overview of the trends in these comment letters. The authors reviewed comment letters retrieved from the SEC’s EDGAR database based on Standard Industrial Classification (SIC) Codes that cover mining, the content of the comment letter (whether it references S-K 1300 or not), the names of SEC staff listed in the comment letters, and other criteria. The analysis shows the SEC staff's reviews have been comprehensive covering the breadth of S-K 1300 and eliciting compliance with the rule. The staff through their comments are eliciting all the disclosures required by S-K 1300 and are not allowing registrants to substitute disclosures based on other standards for S-K 1300 disclosures. It is important, however, not to over generalize as the comments are based on the particular context.

As more phosphate deposits are discovered and new mines are developed, the necessity of critically evaluating various rocks for phosphoric acid production recurs ever more frequently. This need is accentuated by the increasing number of, fertilizer plants being constructed on deep water at various locations to serve world markets and to take advantage of locally-available sulfur or sulfur-bearing ores. A major proportion of all phosphate rock mined is consumed by wet-process plants and this acid in turn is consumed, usually captively, in the manufacture of fertilizer. It is apparent, therefore, that the first question asked by the developer of a new deposit is - How will it work in a wet-process plant? and conversely, when developing a deep water, project, one of the early studies required is - Which of the available rocks shall we use? What is the correct price to pay? How do we compare one rock with another? The economic importance of even minor aspects of these questions

Geostatistical literature has been dealing with recoverable reserve estimation techniques for the last 30 years. There have been well established support correction techniques (Le. affine correction, indirect log-normal correction, etc.) to deal with volume -variance relationship globally in determining what will be the reserves above a cutoff grade during mining. Kriging has become the accepted technique in the mining industry for ore reserve estimation. Many large companies are using this method for gold and copper deposit modeling. In general, the estimates coming from kriging are smooth. The variances of the block grades estimated by kriging are often lower than the material that will be mined selectively. As a result, kriging tends to under estimate reserves at high cutoff grades and overestimates them in low cutoff grades. It is necessary to perform additional steps after kriging if the goal is to determine recoverable reserves at some cutoff and not the in situ resources. In the mining industry, it is becoming more and more common for the low grade heap leach projects to consider whether the higher grade portion of a deposit may be processed by a mill at higher profits. Similarly, a concentrator -refinery operations are implementing heap leach -SX-EW option for the lower grade portion of their deposit. These options are making it very critical to estimate the low and high end of the grade distributions more accurately. Ore reserve estimation is normally done by using widely spaced exploration data which has very small size support (Le. core samples). Selective mining is carried out on ore control that is based on closely spaced blasthole data normally averaged into larger polygons based on some cutoff grade. Estimated grades from exploration data need to reflect the variability of the grades of the selectively mined units if they are used without further adjustments to predict recoverable reserves.

Long range transport of pollutants, particularly of sulphur dioxide and the associated acidic precipitation, has received much attention in the press and in international discussion during the last few years. Studies revealing increasing acidity of lakes and the dying out of fish populations over broad regions of the eastern half of North America have created alarm. In this paper, I will not attempt a description of the acid rain phenomenon. For this r recommend the balanced treatment of the subject by Gene Likens et al (1979) in Scientific Americana For my purposes I accept that the problem is serious and direct my attention to exploring a solution. Ultimately, the key lies in the answer to the question; "What do we do with the enormous tonnages of pollution abatement byproducts, chiefly sulphur compounds, that would arise from pollution control?"' Little thought has been given to this most limiting factor to environmental clean-up. This is symptomatic of a basic flaw in the [philosphy] that under-ins environmental policy. Although this paper is to propose a solution to the abatement byproduct problem, the basic flaw - the firmly entrenched, universally adopted, 'polluter pays [philosphy] as it is currently [understood] - needs some comment.

"This paper analyzes mine emergency management and rescue practices in German underground coal mines and compares them to those used in the United States. The German mining industry has developed these practices over more than 100 years, and they have successfully managed numerous fire, water inundation and ground control emergencies using these techniques. The authors interviewed German mine emergency professionals about rescue team and emergency operations. These operations were then analyzed to determine which of these German principles would be of interest to United States mine rescue teams. Implementing these programs into United States mine rescue and emergency operations was analyzed. INTRODUCTION Coal mining has become much safer industry than it has been in the past. Advances in safety and health regulations and company polices have improved working conditions for miners all over the world. Even with these advances there is still a need for continued attention to safety and health. It is also important for mine safety professionals to observe practices and procedures in other countries and cultures and learn what and how to apply this knowledge in their own systems. Coal mine rescue has been an integral part in improving coal mine safety. Mine rescue procedures have evolved with time, experience and technology development. Today, coal mine rescue teams are still needed and must be continually trained and prepared for any mine emergency. Although it is hoped that improved mine safety policies and practices will eliminate coal mine disasters in the future, accidents remind us that disasters can still happen today. In May 2014, Turkey experienced a severe coal mine disaster where 301 people were killed [G.Tuysuz, 2014]. Our industry must learn from these incidents, focusing on disaster prevention and making improvements in mine rescue operations. Mine rescue procedures differ from country to country due different regulatory environments and different historical evolution of emergency response. Sharing practices and procedures from one country to another enables learning from different experiences and cultural backgrounds. Sharing is also a way of preserving information, identifying and learning best practices."

Geomechanics signifies the science and engineering of soil and rock materials and of rock masses. It is a field of professional practice and research that draws heavily on the elements of soil and rock mechanics. Geomechanics contributes to a number of disciplines: mining engineering, civil engineering, geological engineering, petroleum and natural gas engineering, and engineering geology (including the elements of geohydrology and geophysics). These disciplines are involved in the design and construction of such projects as mines, tunnels, foundations for structures, excavated rock slopes, dams, shafts, boreholes, oil reservoir wells, underground storage facilities for oil and gas, and hazardous waste repositories. Geomechanics allows us to improve both our environment and our quality of life. In the context of this Handbook, the scope of geomechanics is discussed mainly from the point of view of applications to mining engineering. However, the coverage of the subject mate¬rial is so comprehensive that it is relevant to other disciplines as well. Section 10 covers the field of geomechanics in a structured fashion, beginning with the fundamental principles and leading into practical applications in mining engineering. Throughout the treatment of the subject, knowledge of geologic conditions is assumed since this is discussed in other sections of the Handbook. An important feature of this section is a series of practical examples illustrating the various aspects of geomechanics. Throughout the 1980's the tem Mine-Wide Monitoring System has had various meanings for manufacturers of these systems and underground mine operators alike. System suppliers had visions of grandeur in the early 80's as they envisioned each mine, that has more than a single section, installing a computer based monitoring system to provide surveillance, control and documentation of the entire mining operation, including everything from a single heat detector to the weekly payroll. In a few cases this may have become reality but the truth is these systems have quietly become a steady but unspectacular workbase for the modern mining industry of the 1990's. As it exists today the total market for underground monitoring systems may not exceed $10 million annually. Suppliers have lowered their expectations of the dollar value of this particular market. Some were entering a new market because of experience with computer based data acquisition systems. Others had been active in the mining industry for years with more traditional equipment (communication systems, methane monitors, safety equipment, etc.) and decided to become involved with Mine-Wide Monitoring Systems.

Unrecognized roof beam failures resulted in 69% of the falls of ground injuries occurring in underground U.S. stone mines from 1990 to 1996. Field investigations at 45 underground stone mines suggest that excessive beam sag or deflection is the principal unstable roof behavior characteristics associated with miner injuries. Laminated roof structures, common in most underground stone mines, allow the immediate roof to separate into distinct beams. The timely determination of roof beam deflection could provide stone miners with advanced knowledge of potentially hazardous roof. Existing methods to assess roof conditions consist of observational and monitoring techniques. Observational techniques include open drillholes, sounding, wedging, and borescopes. Monitoring techniques include scratch tools, extensometers, telltales, Miners Helpers, and Guardian Angels. 1 Miners have difficulty utilizing these techniques because roof beams are located an average of 8 m (26.5 ft) above the miner's head in environments that are dark, dusty and noisy and these deflections are measured in millimeters of movement. To help mitigate the potential for falls of ground injuries to underground stone miners, the National Institute for Occupational Safety and Health (NIOSH) has developed the Roof Monitoring Safety System (RMSS) to aid in monitoring dangerous levels of roof beam deflection. If enough site-specific information is collected and placed on highly visible maps, individual mining operations could establish general guidelines for responding to specific roof beam deflection occurrences. This would help in implementing a pro-active, comprehensive roof control plan for reducing falls of ground injuries.

Many recent projects have been developed and mined based upon imaginary reserves. Imaginary or "geo-fantasy" reserves have cost many companies millions of dollars. The significant and varied technological advances in the mining arena have done little to curb reserve calculation problems. These advances may have served to increase the prevalence of reserve problems by putting more tools in the hands of personnel not properly trained or experienced in reserve estimation. This paper will discuss potential problems related to calculating reserves, which include: SAMPLING ERRORS -obtaining a representative sample. -Decay analysis of wet RC drilling -Bias introduced by sample preparation. -Assay bias; Coarse gold. -Specific gravity samples. "Recent government initiatives and commercial activities have targeted asteroids for in-situ characterization, manipulation, and possible resource extraction. Though emergent technologies may be effective for such undertakings, uncertainties remain concerning the effects a space environment may have on the mechanical behavior of rocks. Various non-destructive rock characterization techniques can empirically predict in-situ rock strength on Earth; however, these techniques must be adapted to account for unique space conditions. In order to mimic one element of a space environment, various rock samples were evaluated after prolonged exposure to low temperatures ranging from -75°C to 0°C. Tests included uniaxial compressive strength, Schmidt hammer, and ultrasonic pulse velocity, while rock types included shale, limestone, and chalk. This data was used to generate a temperature correction for the non-destructive methods and evaluate the accuracy of these methods in predicting strength values. Overall, these results provide insight on the mechanical behavior of rocks at low temperatures and may be used to design robust systems needed for asteroid rock characterization INTRODUCTION NASA and various commercial entities have recently targeted near-Earth asteroids for exploration and scientific investigation to better understand their in-situ properties, mechanical characteristics, and formative processes. In particular, NASA’s Asteroid Initiative encompasses several studies and mission plans that will enable future investigations while vetting technologies that can be applied to deep space exploration. One component of the Asteroid Initiative is the Asteroid Redirect Mission (ARM). This multi-stage space mission proposes to rendezvous a robotic spacecraft with a large near-Earth asteroid. Upon approach and after landing, the spacecraft will prospect the surface and identify a multi-ton boulder (up to 5 m in its largest dimension) suitable for capture. The robotic spacecraft will engage the boulder, remove it from the parent asteroid, and return the sample to a controlled lunar orbit. Once the asteroid mass is in stable orbit around the moon, astronauts aboard the Orion spacecraft will engage with the retrieved asteroid for manned exploration that will include sample collection and geologic prospecting. Launch plans for the ARM robotic craft are expected as early as 2020, and manned explorations of the asteroid mass in lunar orbit are expected to follow sometime in the mid-2020s (NASA, 2015). Overall this mission will demonstrate NASA’s ability to intersect, manipulate, and redirect an extraterrestrial body, while supplying scientific information that is relevant for planetary defense and in-situ resource utilization (ISRU) applications."

Mechanical mining holds great promise for the mining industry. Realizing this promise depends on giving mine operators the capability to evaluate the higher production, the lower costs, and safety enhancement that comes with mechanical mining. The opportunities for low cost and high production must outset the high capital costs associated with mechanical excavation. Giving the mining industry the tools to effectively evaluate and successfully implement mechanical methods defines the challenges and directions of the mechanical mining industry.

In 1986 the U. S. Bureau of Mines (USBM) embarked on a major research effort to develop technology that can substantially reduce worker exposure to face hazards simply by relocating the equipment operators to an area of relative safety. This reduced exposure mining system (REMS) research effort enhances the USBM ongoing health and safety research efforts to reduce and control dust and noise and to reduce accidents through training and better mining practices. This paper reports progress of the research that is developing and will demonstrate the technologies required for the computer-assisted tele-remote operation of continuous mining machines, haulage systems, and roof bolting machines.

The capability for on-line measurement of the quality characteristics of conveyed coal now enables mine operators to take advantage of the inherent heterogeneity of those streams and split them into wash and no-wash stocks. Relative to processing the entire stream, this reduces the amount of coal that must be washed at the mine and thereby reduces processing costs, recovery losses, and refuse generation levels. In this paper, new approaches for optimal control of the segregation process, using time series models and empirical distributions, are presented and demonstrated using field data.

To address the problem of grade estimation in a general three-dimensional shape and the uncertainty associated with the estimate, the technique of jackknifing/block kriging short-cut is proposed. A new estimator called jackknife kriging is introduced first. Through the jackknife variance, a confidence limit for the reserve grade estimate can be calculated. Unlike the kriging variance, which is more a measure of data configuration and is independent of data values, the jackknife variance reflects the true variability of the data. This approach provides a simple way to set a confidence level for global estimation of resources.

"One of the most significant technical and practical challenges facing the tunnel construction team on the Legacy Way road tunnel project in Brisbane, Australia was how to erect the pre-stressed precast concrete slabs to form the smoke duct in the tunnel boring machine (TBM) tunnels. An essential part of the tunnel ventilation system, the smoke duct, extends the full length of the 4.6km long twin, two lane tunnels. The vast majority of the tunnels (approx. 4.3km) were constructed using two 12.4m diameter double shield TBMs. The refurbished Herrenknecht machines broke several world records on the way to completing the tunnel excavation works in just over nine months. With the excavation complete the focus turned to solving the complex civil tunnel fit out challenges. A key component of the civil fit out of the tunnels was constructing the smoke duct in the TBM tunnels which was designed as a precast solution. The slabs which each measure 8.62m long by 2.0m wide by 200mm thick and weigh close to 9 tonne need to be erected above cast in situ corbels whilst other tunnel finishing works continue. The space proofing clearance offered by the design to the support corbels below and the precast segmental lining above was in the order of only 100mm. This paper details the events of how the Transcity JV tunnel fit out team, comprising some of Europe and Australia’s leading tunnel construction specialists, drew upon the global strengths of the existing JV member companies and the global reach of leading TBM manufacturer Herrenknecht to develop an innovative system to solve this challenging problem. This globetrotting story, which starts in Australia, visits Europe and mainland China before returning to Brisbane where it all began."

The feasibility of maintaining equipment location, personnel location and vital statistics in surface and underground mining operations has increased with advances in radio frequency identification (RFID) technology. The paper addresses the use of this technology, relatively new in the mining industry, to track surface equipment in mine pits, loading points, and processing facilities. Specific applications are discussed including a simplified and a complex truck tracking system and an automatic pit ticket system. The paper concludes with a discussion of the future possibilities of RFID technology in mining, such as monitoring biofeedback of personnel (heart and respiration rate, body temperature, exertion level, etc.) repetitious movements to study work habits, and logging air quality via personnel sensors.

Under the Wilderness Act of 1964 and the accompanying House Conference Report, the Geological Survey and Bureau of Mines are charged with the responsibility of making recurring mineral studies of wilderness and wilderness candidate areas in national forests within and proposed for the National Wilderness Preservation System. The Wilderness Act, a major shift from the multiple-use concept of national forests, identified for the first time the value of mineral resource information in decisions for single-use designation of public lands. This concept was further realized when the Secretary of the Interior in May 1974 recommended that mineral studies be made prior to wilderness recommendations for five large Interior-managed game ranges and the Glacier Bay National Monument aggregating 7.8 million acres. Presently, our studies have been completed on 126 wilderness and proposed wilderness areas totaling 15.4 million acres in the national forests. In 1973, 1 year in advance of the act's 10-year review deadline, we completed the 34 primitive areas, originally the only potential additions considered by the 1964 Act.
