Over the last 30 years, a transition away from end-anchored bolts to fully grouted rock bolt systems has occurred in many underground mines. It seems geomechanically intuitive that the success of such bolts, which interfere with rock movements around underground excavations through a variety of complex interactions, is precisely why they remain challenging to instrument and monitor. Whereas measuring the load profile for an end-anchored mechanical bolt is trivial, for a fully grouted system, the load may vary dramatically due to discrete rock movement at cracks and joints. Despite the challenges, measuring the strain (i.e., load) along fully grouted rock bolts remains very appealing, and a lot of valuable research has been conducted using foil resistance strain gauges, particularly in US coal mines (Signer and Jones, 1990). Despite good intentions, more widespread adoption of this instrumentation remains unlikely for the following reasons: (i) The technology is expensive, partly because Intrinsically Safe (IS) requirements limit the number of suppliers and solutions. (ii) The technology is perceived as challenging to successfully implement except by those who are skilled in the art. (iii) The reliability of the technology is questionable in harsh environments. (iv) The technology relies on discrete pairs of strain gauges that may not capture the complete strain distribution along the bolt. No existing alternative technology can offer a solution to all of these limitations. However, in this paper, we introduce results from a new Distributed Optical Sensing technology that promises significant potential. Since the technology is capable of distributed sensing, our results suggest that this new technology can, for the first time, reveal the detailed strain profile along fully grouted rock bolts.

Panel-scale modeling of stress changes induced by longwall coal mining is a challenging problem, particularly in deep mines of the western United States. Two common approaches to this problem are the empirical and boundary element modeling methods. One boundary element method, LaModel, was recently calibrated to field measurements at a mine in Colorado (Larson and Whyatt, 2012). However, the calibrated overburden properties were extreme, resulting in very low estimates of gob loading. Next, the empirical method was calibrated to field measurements and applied to the problem, but this approach clearly extrapolated this method well beyond the characteristics of underlying cases. This paper examines a third option, the elastic continuum boundary element program Mulsim. The program was expanded to handle LaModel-size meshes in a version dubbed Mulsim/ Large. Properties for the elastic, homogeneous overburden model in Mulsim were estimated as a thickness-weighted average of typical published elastic properties for each major stratum. These properties, along with a commonly used pillar strength equation and coal properties, produced the measured load transfer distance and reasonable gob loading without further calibration. Given the number of input parameters required for all of these methods, it is not surprising that LaModel and the empirical approach could be adjusted further to incorporate similar gob loads. However, these extraordinary measures failed to bring gate road pillar loading in line with Mulsim results. Differences in pillar load estimates continued to be significant (as large as 65% in this study). These results demonstrate that, for this particular case, the choice of analysis method impacts results in ways that cannot be removed by calibration of input parameters. Finally, the natural fit between the Mulsim model and mine conditions suggests that approximating overburden at the particular site used for this research as an elastic continuum is superior to the alternatives examined.

"Comparative roof beam performance, as measured by delamination within the bolted horizon, was used to evaluate two types of roof bolt drill hole geometries in a development setting. Short encapsulation pull tests had demonstrated a higher grip factor in drill holes featuring helical grooves in the side walls in comparison to drill holes featuring continuous smooth to slightly rough walls. The study incorporated the two geometries during development mining in which fully grouted mechanically anchored bolts were installed as primary roof support.bolts were installed as primary roof support.The study site, in an entry adjacent to a longwall panel, was instrumented with extensometers and data collected during longwall retreat. Statistical analysis was conducted to validate null and alternative hypotheses used to compare drill hole geometries. Analysis yielded no statistically significant difference between drill hole geometries. Inspection of the data does reveal less delamination within the smooth walled site in comparison to the site containing grooved hole geometry.INTRODUCTIONFrom their introduction as a means of ground control over sixty years ago, roof bolts today are the primary mechanism of roof support utilized throughout mining. Whether through suspension, keying, or beam building, roof bolts are employed to reinforce the inherent strength of the strata comprising the immediate roof with the goal of creating a competent bolted horizon of sufficient load bearing capacity to be self-supporting for a given mine's design life, stress regime, and geomechanical properties.For grouted bolts in sedimentary strata, axial loading due to vertical rock movement and bending due to horizontal rock movement are the principal means of loading (Signer, 2000). Regardless of the type of bolt and extent of grouting employed, the success of any roof bolting design is determined, to a large extent, by the effectiveness of load transfer between the rock, grout, and bolt.At San Juan Mine, an underground longwall mine located in the northwestern corner of New Mexico, a number of different roof bolt types, grades, and lengths have been used over the years in an effort to optimize primary roof support. One common denominator shared throughout has been the use of full length grout. Load transfer for full length grout is achieved primarily through mechanical bonding between the grout and rock. It is this interaction that is the focus of the study described in this paper. The study assessed and compared two types of bolt hole geometries employed during gate road development at San Juan. Underground instrumentation and evaluation of the bolted roof provided the means of assessment and comparison."

Colombia is 11th largest coal producing nation in the world, and the world?s fifth largest coal exporter. But while most of Colombia?s coal is mined at large modern surface mines located near the Caribbean coast, there are also underground mines that produce high-grade metallurgical coal. These mines employ traditional hand-loading techniques in a variety of geologic environments, and they have some of the highest accident rates of any Colombian industry. The National Institute for Occupational Safety and Health (NIOSH) has been collaborating with POSITIVA, the primary provider of worker?s compensation insurance to the Colombian underground coal industry, to reduce the economic and social cost of rock falls in these mines. The specific goals of the collaboration are to 1) document practices and conditions in Colombian underground coal mines, 2) identify relevant international best practices, and 3) provide recommendations for developing a national campaign to reduce fatalities and injuries associated with roof falls. NIOSH conducted a survey of seven underground coal mines in four separate coalfields. Five of the mines were characterized by steep dips and weak roof and floor rock. The coal was loaded by hand at all seven mines, though two employed partially mechanized longwall techniques. Wood posts were by far the most common ground support. Roof bolts were not observed in use at any of the mines, and they are unlikely to replace timbers any time soon. While long-term entries generally appeared to be adequately supported, the survey found that less attention was paid to the temporary support that is installed right after the coal is removed. This temporary support is critical for protecting the workers from rock falls, however, because with hand-loading mining methods most of the activity takes place between the permanent supports and face. The study concluded that improving the temporary support in the working faces would be the most effective way to reduce the number of rock fall injuries. The key principle should be that every worker is protected by planned roof support at all times. Ground Support Management Plans, consisting of specific rules about when, where, and what roof support is to be installed, should be the focus of the campaign. Each Plan should be tailored to a specific mine based on the geologic conditions and mining methods in use. The Plan provides miners with clear, simple rules that can be easily understood, while management can easily check to see if it is being followed. Ground Support Management Plans will also provide the campaign with a concrete focus for the necessary cultural change.

A multi-stage triaxial procedure has been developed for the enhanced servo controls and feedback systems at the National Institute for Occupational Safety and Health (NIOSH) rock mechanics laboratory. Due to the difficulty of preparing multiple rock specimens for single-stage triaxial tests at different confinements, multi-stage testing provides a huge advantage if only a few samples of coal measure rocks can be prepared from the drill core. A multi-stage test can provide mechanical properties, post-failure properties, and a complete failure envelope for a single specimen. For the multi-stage test procedure, a parameter used to define the slope of the stress-radial strain curve was developed and monitored in real time in order to successfully predict peak strengths at different confining stages. For this paper, multi-stage tests have been conducted on sandstones of varying strengths, including Berea sandstone which has well-documented properties, and fine-grained and coarse sandstone from a western US coal mine. For the tests conducted on the fine-grained sandstone, it was found that the ramp rate was an important factor in the success of obtaining peak strengths during the multi-stage tests. For each rock type, a table including Young?s modulus, Poisson?s ratio, unconfined compressive strength, friction angle, cohesion, residual friction angle, and residual cohesion are reported. The values from these tables have been used as input parameters for projects at NIOSH requiring numerical model input parameters.

"In order to ensure the stability of roadways, Anglo American Metallurgical Coal (AAMC) has developed a roof support design methodology that integrates analytical, numerical and empirical modelling. This methodology currently is based on a deterministic approach (a single factor of safety against failure is calculated). However, an improved methodology, based on stochastic modelling technique, has also been developed and currently being evaluated. The main advantage of this methodology is that the design is based on the probability distributions of input parameters; thus the outcome is based on a distribution of factors of safety rather than a single factor of safety. An evaluation of factors of safety may also be used to determine the likelihood of failure, which in turn may be utilized to quantitatively determine and evaluate the associated risks in decision making process. This methodology has been evaluated at Grasstree and Moranbah North Coal Mines in the designs of roof support in various critical areas and has been proven to be successful and a better way of determining the roof support requirements.This paper demonstrates the application of this methodology in a design of an install road roof support at Moranbah North Mine, where the 1,750 tonne capacity ''world's highest rated longwall"" has recently been installed.INTRODUCTIONAnglo American Metallurgical Coal Australia (AAMC) operates three longwall (LW) mines located in Central Queensland. There is an increasing emphasis on the reliability at these operations, as the longwalls are getting deeper and facing more geologically challenging conditions. In addition, the Anglo American Vision is to achieve ""Zero Harm"" through the effective management of safety at all businesses and operations. In order to accomplish this vision, AAMC has developed a proactive ground control management system for a safe and efficient production of underground reserves.Proactive ground control management involves understanding the impacts of the geotechnical environment on likely ground behavior, and it consists of approximately 15 major elements. One of the most essential fundamentals of proactive ground control management is to utilize a roof support design methodology that considers different failure mechanisms and also takes into account all important elements. The design methodology becomes even more important when the area in question is a critical area, which is defined as a high risk roadway where any failure may cause increased levels of safety and financial risks to underground workforce and operations."

China is the largest coal producing country in the world. Coal is widely distributed in China and thus the mining conditions are complex and diverse. This paper summarizes the important achievements in the last 20 years in various mining methods including the extra thick seam with large mining height top coal caving, the thick seam with large mining height and top coal caving mining, the steep seam with top coal caving and fully-mechanized mining, the thin seam automated mining, and the backfill mining. At the same time, the rapid advance in the development of mining machinery and manufacturing has also contributed to the advancement of coal mining technology and the great improvement of coal mine safety. The future direction of China?s coal mining is to further improve mine safety as to improve the coal recovery rate, promote the pillar less mining technology, develop high-efficiency backfilling technology, develop steep seam high-efficiency mining technology for seams less than 10m thick, and high recovery mining techniques for extra thick seams, improve the reliability of mining equipment, and strengthen the protection and restoration of surface environments and water resources.

Rockburst conditions were first encountered at Inco's Sudbury area operations in the mid 1930's when mining advanced to below the 610 m (2000 ft.) horizon. Now, mining is being carried out at depths down to 2130 m (7000 ft.) at Inco'a Creighton Mine. While rockburst potentials have increased with depth. improvements in mine planning, methods, support systems and developments in destressing have alleviated the condition to where it is less of a problem now than it was in the early years. Hang of the improvements were developed, of necessity. through observation and reasoning because these were the only tools available at the time. Now, we are making use of stress - measurements, numerical modelling and microseismic source location technology to - help further reduce rockburat hazards.

"Understanding coal mine rib behavior is important for inferring pillar loading conditions as well as ensuring the safety of miners who are regularly exposed to ribs. Due to the variability in the geometry of underground openings and ground behavior, point measurements often fail to capture the true movement of mine workings. Photogrammetry is a potentially fast, cheap, and precise supplemental measurement tool in comparison to extensometers, tape measures, or laser range meters, but its application in underground coal has been limited. The practical use of photogrammetry was tested at the Safety Research Coal Mine, National Institute for Occupational Safety and Health (NIOSH). A commercially available, Digital Single-Lens Reflex (DSLR) camera was used to perform the photogrammetric surveys for the experiment. Several experiments were performed using different lighting conditions, distances to subject, camera settings, and photograph overlaps, with results summarized as follows: The lighting method was found to be insignificant if the scene was appropriately illuminated. It was found that the distance to the subject has a minimal impact on result accuracy, and that camera settings have a significant impact on the photogrammetric quality of images. An increasing photograph resolution was preferable when measuring plane orientations; otherwise a high point cloud density would likely be excessive. Focal Ratio (F-stop) changes affect the depth of field and image quality in situations where multiple angles are necessary to survey cleat orientations. Photograph overlap is very important to proper three-dimensional reconstruction, and at least 60% overlap between photograph pairs is ideal to avoid unnecessary post-processing. The suggestions and guidelines proposed are designed to increase the quality of photogrammetry inputs and outputs as well as minimize processing time, and serve as a starting point for an underground coal photogrammetry study.INTRODUCTIONThe stability of underground mine ribs has been a safety concern for underground coal mines for decades. Unfortunately, the average rib fatality rate has been about 1.3 per year between 1996 and 2013 (Jones et al., 2014). In an effort to better understand coal rib behavior, photogrammetry is being investigated in this study as a measurement tool for surveying ribs in an underground coal mine environment."

"Terrestrial laser scanning (TLS) is a useful technology for rock mass characterization. A laser scanner produces a massive point cloud of a scanned area, such as an exposed rock surface in an underground tunnel, with millimeter precision. The density of the point cloud depends on several parameters from both the TLS operational conditions and the specifications of the project, such as the resolution and the quality of the laser scan, the section of the tunnel, the distance between scanning stations, and the purpose of the scans. One purpose of the scan can be to characterize the rock mass and statistically analyze the discontinuities that compose it for further discontinuous modeling. In these instances, additional data processing and a detailed analysis should be performed on the point cloud to extract the parameters to define a discrete fracture network (DFN) for each discontinuity set. I-Site Studio is a point cloud processing software that allows users to edit and process laser scans. This software contains a set of geotechnical analysis tools that assist engineers during the structural mapping process, allowing for greater and more representative data regarding the structural information of the rock mass, which may be used for generating DFNs. This paper presents the procedures used during a laser scan for characterizing discontinuities in an underground limestone mine and the results of the scan as applied to the generation of DFNs for further discontinuous modeling.INTRODUCTIONAccording to the Mine Health and Safety Administration (MSHA), between 2006 and 2016, the underground stone mining industry had the highest fatality rate in 4 out of 10 years, compared to any other kind of mining (MSHA, 2016). Additionally, during that same time, 40% of the fatalities were due to ground control issues, such as roof and rib collapses and pillar bursts. The National Institute for Occupational Safety and Health (NIOSH) developed guidelines for designing underground stone mines (Esterhuizen, Dolinar, Ellenberger, and Prosser, 2011). However, these guidelines do not apply to all underground stone mines.Monsalve, Baggett, Bishop, and Ripepi (2018) present a case study of an underground limestone mine experiencing a structurally controlled mode of failure. The authors analyze different methods to study that type of instability. They conclude that the integration of terrestrial laser scanning (TLS) with discrete element modelling (DEM) can be used to prevent rock falls in underground excavations to enhance worker safety. However, an adequate rock mass characterization and structural mapping must be performed in order to generate reliable models that allow the engineer to have a better understanding of the rock mass behavior."

"In the process of Geologic Hazard Mapping (GHM) for underground coal mining, geologists evaluate the effects of various geologic and geotechnical conditions to assist with optimization of mine productivity and with selection of more favorable areas for future development. The methodology involves correlation of multiple geologic factors (such as overburden depth, proximity to sandstone channels, and areas with weak immediate roof strata) with conditions encountered during mining (roof falls, floor heave, pillar spalling, etc.). Such studies yield maps that identify areas where mining is likely to be difficult and allow mining engineers to foresee favorable and unfavorable mining areas when planning new or expanding mines. To expand the benefits of GHM, work has been done to correlate geologic factors with room-and-pillar mining advance rates. Average mining advance rates are mapped and then correlated to different areas, as characterized by GHM. This process exposes the geological characteristics that have a material impact on mining rates of advance, improving the rates of advance into future mining areas with similar geological conditions. The final result of the work allows mine planners to not only design the mine qualitatively accounting for areas of unfavorable conditions, but also allows the mine planner to more accurately apply rates of advance for mine timing and production sequencing. The use of GHM to predict mining advance rates in unmined areas enhances the benefits of traditional hazard mapping and provides quantitative data that can be incorporated into mine sequencing models.INTRODUCTIONThe role of a geologist in the coal mining industry has evolved significantly over the last 30 to 40 years. Geologic Hazard Mapping (GHM) mapping for coal mines began in the late 1970s and early 1980s (Chase, Newman, and Rusnak, 2006). Since its beginnings, the GHM process has evolved significantly and continues to take on new levels of sophistication. Numerous case studies of GHM application to predict future mining conditions exist in the literature. For example, Artrip, Nelson, and Newman (1993), documents the geotechnical characterization of roof and floor for mining in the Imboden Seam in Virginia where previous mining efforts had been abandoned due to adverse mining conditions. In addition, Keim and Miller (1999) summarize evaluation of geological influences on a longwall mine in West Virginia. In both examples, significant geotechnical and geological information was available, including the opportunity to compare predicted mining conditions against actual conditions as mining progressed."

"High performance production under challenging geological conditions is a product of several parameters. Main factors are the technical equipment and the knowledge in processing. The design and the support system of maingates must also be taken into consideration. The extraction of seams in German underground hard coal mines of RAG in a range of heights between approximately 1.1 and 4.0 m ( 4 and 13 ft) is done by plow and shearer technology. A complete system for quality management of production is required. The process examination of processes begins with procurement of the equipment. Education and training of the engineers and specialists, as well as for the miners are requirements for success as well.This report gives an overview of RAG measures for achieving high performance. Starting with examination of the processes accompanying procurement of equipment, the following important elements are described. Standards in education and training, technical design and elements of automated processing using stateof- the-art in control systems are critical to this system.The application of automation plays a decisive role in modem coal mines. The latest development in German coal mines is the fully-automated shearer system for longwalls. This helps to enhance safety, particularly in difficult ground conditions, by removing the shearer and shield operators from the immediate mining activity.INTRODUCTIONLongwall mining in carboniferous strata in Germany faces various challenges. Different longwall systems are used to mine up to 24 seams, each with a thickness between 1.1 m (3.6 ft) and 4.0 m (13 ft). High stresses due to a depth of up to 1,400 m (4,593 ft) requiring pillars between the panels, require special systems for roadway development, roadway support and reinforcemen.t as well as for longwall advance operations. Two main types of equipment for extraction are shearer and plow systems. While shearers are utilized for seam thicknesses above approximately 1.8 m (6 ft), plow systems can be used for smaller seam thicknesses between 1.8 m (6 ft) and approximately 1.0 m (3 ft)."

"A coal outburst is a severe safety hazard in room-and-pillar mining under deep cover. It is more likely to occur during pillar retreating. Multi-seam mining dramatically increases the risk of coal outburst within the influence zones created by remnant pillars and gob-solid boundaries. Though coal outburst is generally associated with heavy loading of coal pillars, its occurrence is difficult to predict. Risk management provides a proactive tool to minimize coal outburst in room-and-pillar mining under deep cover. Risk assessment is the first step in identifying and quantifying outburst risk factors. The primary risk factors for coal outburst are overburden depth, roof and floor strength, geological anomalies, mining type, multi-seam mining, and panel width. A risk assessment chart can be used to proactively screen out mining sections with high risk of coal outburst for further analysis. Gob-solid boundaries and remnant pillars are critical factors in evaluation of the coal outburst risk of multi-seam mining. Risk identification, risk assessment, geologic influence mapping, geotechnical evaluation, risk analysis, risk mitigation, and monitoring are essential elements of coal outburst risk management process. Training is an integral part of risk management for risk identification and communication between all the stakeholders including management, technical and safety personnel, and miners.INTRODUCTIONA coal outburst is a sudden violent burst of coal from a pillar with broken coal or blocks of coal forcibly ejected into open entries. A coal outburst is a severe safety hazard as the mining crew is highly exposed at the site when the event occurs. Though deep cover and strong roof and floor are underlying geologic conditions of a potential burst incident, its real occurrence is also the result of additional mining factors. In room-and-pillar mining, a coal outburst could occur during both development and pillar retreating, but the latter greatly increases the risk of outburst. The other risk factors of coal outburst also include mining layout, multi-seam mining, presence of adjacent gob, cutting sequence, and local abnormal geologic conditions. Over the years, the cases of coal outbursts have been studied by many researchers and mining practitioners(Peng, 2008; Newman, 2008; Iannacchione and Tadolini, 2008; Gauna and Phillipson, 2008; Hoelle, 2008; and Newman, 2002). It is commonly believed that the coal outburst is the result of a sudden release of elastic strain energy stored in coal pillars and is highly associated with cutting into heavily loaded coal pillars, but its occurrence is a rare event and is difficult to predict. A number of engineering controls have been recommended to mitigate outburst potential. For room-and-pillar mining, sufficient pillar sizes have been the primary control for coal outburst prevention. The pillar design tools such as ARMPS and AMSS developed by NIOSH have played an important role in the design of stable pillars to prevent pillar collapse and squeezing as well as coal outbursts. In fact, with the implementation of pillar design using proper stability factors, pillar collapse and squeezing have been almost eliminated, and the number of coal outbursts has been greatly reduced in the US over the past decade. However, after a few outbursts occurred during pillar retreating in the US over the past a few years (MSHA, 1996; MSHA, 2014; MSHA, 2013), it has been realized that sufficient pillar size is still not enough to prevent coal outbursts. The investigations of the incidents showed that other factors such as multi-seam mining, panel layout, cutting sequence, and local geologic factors also seemed critical in causing the events. Therefore, it has become imperative that additional proactive measures beyond proper pillar design be implemented to prevent coal outbursts."

"In 2010, the fracking discussion in Germany caused a number of changes in German law, which came into force in 2016. Especially the production of gas had to be regulated. With the legislation amendment, the “Subsidence-Area Mining Regulation” (in German: Einwirkungsbereichs-Bergverordnung) has been revised, too. The changes expand the compensation of mining damages, especially to the extraction with drilling from the surface and underground storage. Although the “Subsidence-Area Mining Regulation” has been revised, the area of main influence (subsidence of 10 cm) remains to determine a relevant boundary for mining damages. The determination and prediction of this boundary above caverns are presented in this paper. In addition, further elements of ground movements and their relevance to mine damages are analyzed. The usage of the area of main influence to fix a relevant boundary for mining damages does not correspond to the relevant elements of ground movements. A limit for differences in subsidence (tilt) or horizontal changes in length should be preferred to describe the relevance of mining damages on buildings. Furthermore, this paper outlines the the meaning of using the area of main influence to fix a relevant boundary for mining damages. INTRODUCTION The “Federal Mining Act” (in German: Bundesberggesetz, short: BBergG) forms the central legal standard to cover subsidence in Germany. BHP Billiton Illawarra Coal operates Dendrobium Mine in an area 10-20km west-northwest of Wollongong in the Southern Coalfield of New South Wales, Australia. The mine recently completed longwall mining in Area 3A adjacent to an overhanging rock feature known as Sandy Creek Waterfall, which had been identified as a significant, and potentially sensitive natural surface feature of the environment. This waterfall lies within the Sydney drinking water catchment area, in a surface terrain of variable topography where non-conventional subsidence behaviour such as valley closure, and far-field horizontal movements are known to occur, over and above conventional subsidence responses to mining. As a part of the conditions of approval to mine, Illawarra Coal undertook to protect the waterfall and the section of Sandy Creek immediately upstream of the waterfall from the effects of longwall mining using an innovative management process and an array of very high resolution monitoring systems. This paper outlines the type of non-conventional subsidence effects that can occur; the range of different monitoring regimes adopted in response to such effects in order to develop an understanding of the valley closure phenomenon in proximity to the waterfall and provide suitable early warning data. The paper also describes the management processes that were adopted, incorporating the use of these monitoring regimes in order to successfully protect the waterfall from the valley closure effects of mining four adjacent longwall panels in close proximity to the waterfall while continuing to maximise recovery of the coal resource in the area. The management structure adopted involved a technical committee, a steering committee, and an external independent reviewer. The technical committee was responsible for design of the monitoring systems, interpretation of the results, and making timely recommendations to the company steering committee who were then able to make informed decisions on when to cease mining each adjacent panel. The steering committee comprised Illawarra Coal management and technical personnel. Although the steering committee took advice from the technical committee, all decisions relating to mining were made by the steering committee. An external reviewer was engaged by the steering committee at the end of each longwall panel to review the results, interpretation and management decisions. This management structure was effective in successfully protecting the very sensitive structure of Sandy Creek Waterfall from potential impacts of nearby mining in the midst of active ongoing natural erosion processes.

Rib-related accidents in underground coal mines continue to cause injuries and take lives at a rate of 1.3 fatalities per year for the last 17 years. Prevention of these accidents cannot simply be a matter of installing more rib support, though the eventual final solution will undoubtedly include that component. Ultimately, rib safety will be brought about through an understanding of the causes and mechanisms that cause rib fatalities, and by fine-tuning mining methods and procedures that are currently in place. This paper investigates the factors likely to be most influential in the occurrence of rib fatalities. A careful analysis has been conducted of the rib fatality reports documented by the Mine Safety and Health Administration between 1996 and 2013. The primary outcome is the determination of the relative impact of each type of rib fatality factor and a better understanding of the impact each factor has. Among the rib fatality factors considered, the influence of rib profileis considered for the first time. Mining height, rib stratigraphy, mining cycle, and the standing position of a miner next to a pillar are also considered. Questions are also raised about the current interpretation of rib fatalities due to mining depth, with the indication being that mines operating at the greatest depths may actually have the most respectable rib fatality record among all mining depths. Additionally, a new method of considering rib exposure is developed. Based on the results, recommendations are made for future work. By gaining a better understanding of how particular factors contribute to rib fatalities as opposed to rib failure or stability, it is hoped that future efforts can be more precisely directed towards the desired outcome of fatality reduction.

"The long-term viability of seismically active and/or deep mining operations hinges upon the industry's ability to maintain profitability in a climate of increasingly difficult conditions, escalating operating costs and fluctuating metal prices. Within this context, it is prudent to explore mining techniques that could provide means for safer and more profitable extraction of seismically active and/or deep seated deposits that are vital to the long term survival of the industry.The first step was to examine Penetrating Cone Fracture (PCF) and other relevant concepts with the objective of semi-continuous mining in deep underground hard rock mines. This was followed by an assessment of the current conventional drilling and blasting practice. Thirteen blasts were monitored in five different seismically active workings of an operating deep mine. The main concerns during the current drill and blast practice were identified.On the basis of the literature review, three short round blasting techniques were short listed for further review. These techniques were evaluated based upon the available literature and discussions with the persons associated with their applications. Justification, requirements and major challenges for the application of short round blasting have been discussed in this paper. Suggestions for improving the current drilling and blasting practice have also been outlined.INTRODUCTIONRock fragmentation is the key to mineral excavation and production. Increasing working depths, environmental concerns and trends towards continuous mining necessitate the development of new and improved rock breakage methods.The excavation of hard rocks is still dominated by the sequential routine of face preparation, drilling, charging explosives, blasting, ventilating for fume and dust removal, mucking and roof support. Worse still, drilling and mucking require specialized equipment that fills the end of the heading so that no other operation can be performed and the equipment must be removed from the face to protect it from fly rock."

Constructing tunnels underground is generally described as a high-risk activity because conditions in the surrounding area tend to be unpredictable. This would and can create unique dangers in every situation. It is important to gain a deep understanding of the risks associated with tunneling in order to reduce the likelihood of their occurrence and severity of their consequences should they occur. For this to be achieved, different risks should first be evaluated and ranked according to their relative importance and criticality. The Amir Kabir tunnel is considered one of the most dangerous tunnels in Tehran, Iran, in terms of the risks involved in its excavation and their potential consequences. In particular, part T4 of the tunnel passes through a zone of different strata and also a compound of various soils. This paper studies part T4 of the Amir Kabir tunnel from a risk analysis and risk management point of view. In doing so, factors increasing total cost, causing delay in the project completion time, decreasing the operation rate and downgrading project quality were identified and ranked according to their importance using the aggregate primary index risk method. For this purpose, a comprehensive questionnaire was designed and completed by tunneling experts. By responding to the questionnaires, the experts determined the relative likelihood of occurrence of the studied factors and their potential consequences. As a result, the experts suggested several alternatives to reduce the risks involved in this particular study. Moreover, the experts completed new questionnaires whilst taking into account different alternatives. In the next step, the risks before and after applying the alternatives were compared. As a result of performing the said analyses, it was concluded first that the aggregate primary index risk is an effective tool in identifying the risks involved in such projects. Second, it was concluded that, by taking into account different alternatives when analyzing and ranking the risks, severity of the said risks can potentially be reduced. Also, by considering such matters when analyzing tunneling risks, the projects? efficiency can greatly increase as a result of reducing risks that threaten financial and human resources. The approach introduced in this paper, together with the methodology described, can be adopted by the mining design engineer in all similar situations. They will be of particular advantage in such methods as room and pillar mining, where the number of similar tunnels required is high.

"Engineers at the National Institute for Occupational Safety and Health (NIOSH) and Itasca Consulting Group, Inc., conducted a study to determine the procedure for calibrating a caving model implemented in FLAC3D. The model is calibrated to measured surface subsidence. This sedimentary caving model was developed by Itasca for NIOSH for the purpose of more closely simulating mechanical processes during caving. When a model is considered calibrated to surface subsidence, the load between the extracted panel's roof and floor is a reasonable estimate of gob loading. Such information can also provide an estimate of the amount of overpanel weight that is transferred to the abutment. This load distribution is a parameter needed, for example, to estimate loading of pillars in longwall gate roads. For the current case, a calibrated model resulted in a calculated abutment angle of 30.6°considerably higher than the assumed value used in empirical methods, but reasonable considering the presence of massive stratigraphic members in the overburden. Because of additional information available after this study that the extracted seam height was actually greater in the first panel, a corrected range of abutment angle was estimated to be from 27.7° to 30.6°, still significantly higher than the empirical assumption. The expectation is that better estimates of abutment loading profiles will result in improved evaluations of mine layout design and, therefore, reduce the number of fatalities and injuries resulting from falls of ground.INTRODUCTIONThe safety of miners in underground coal mines depends on careful consideration of the mechanisms involved in failure and transfer of load. The successful design of pillars and panels in longwall and room-and-pillar coal mining requires thorough attention to any information and measurements available that help understand these mechanisms. Although empirical methods, such as Analysis of Pillar Stability (ALPS) Mark (1987); (Mark, 1992) or Analysis of Retreat Mining Pillar Stability (ARMPS) (Mark and Chase, 1997), can provide a sense of success or failure, complex geology and failure in the roof or floor material or complex material behavior of the coal can make evaluation by an appropriate numerical modeling tool necessary. The numerical tool must simulate important mechanisms and appropriate detail that underlie the mechanism. For example, Starfield and Cundall (1988) advised using ""the simplest model that will allow the important mechanisms to occur, and could serve as a laboratory for the experiments you have in mind."" To that end, data gathering about the geology, geometry, and material behavior is very valuable. Such data facilitate selection of an appropriate model and model inputs. Moreover, this data facilitate improved assessment of risk to miners and how to minimize that risk."

"The angle of draw (in Germany, referred to as Limit Angle) plays a significant role in the field of mining subsidence engineering. This applies both to the assessment of the affected area by underground mining activities as well as applied methods for the prediction of ground movements.Recent observations using remote sensing show that ground movements outside the affected area defined by the angle of draw have been detected in German hard coal mining. Since the affected area by underground mining activities also plays an essential role regarding claims of landowners with damages to their houses and infrastructures, possible solutions for limiting the size of the affected area will be presented in this paper.Ground movements which exceed the affected area of an underground coal mine, as defined by the angle of draw will be shown in an exemplary manner. Present knowledge indicates an expectation that ground movements outside the affected area will not cause damage of major significance.This paper outlines the consequences for all affected parties regarding ground movements due to underground coal mining that exceed the extent of the area defined by the currently assumed angle of draw. This will be visualized in a case study.INTRODUCTIONGermany has a long tradition of coal mining, especially in the mining area of the Ruhr district. Following a long period of mining activity, the area is now highly populated and densely developed. In such an environment, it is of utmost importance to calculate and predict ground movements initiated by mining activities; therefore, the research here focused on the prediction of ground movement for the last decades. Special attention was given to the development of accurate ground movement models, which can be utilized in all mining areas. These models are based on different influencing factors such as depth of mining, worked thickness and the angle of draw.Depending on these models, the mining companies calculate the area of land subsidence; the predicted results are used for financial calculations and planning. For structures on top of zones with a high risk of deformation, structural measures can be applied, construction warnings can be given and, in cases of damage, compensation can be planned. The technical and economic planning was performed for all areas of mining subsidence. The most important factor for the outer limits of mining subsidence is the angle of draw (Ger. Grenzwinkel), which plots the border of the area of mining subsidence."
