Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are improving but more slowly and yet, serious accidents and fatality rates are not: in fact, these rates of more serious incidents have plateaued or deteriorating. In more dramatic cases, organizations that had ?exemplary? safety statistics, suddenly had a catastrophic or multi-fatality event occurring. Popular books such as In Search of Excellence of Peters and Waterman or Good to Great of Jim Collins created many delusional ?secrets? or recipes, to achieve great performance and yet, most of those ?great? or ?excellent? companies have now declined in their performance, some dramatically and some have fallen from grace, like Fanny Mae, a company Jim Collins identified as a ?great? one, that had to be bailed out in the latest financial crisis. These organizations share several ?delusions? about managing a business, and often, these delusions are created by academics, consultants and authors of popular books about (safety) management. More damaging however is that safety management is defined by a set of key practices or axioms: a focus on human error, compliance to rules and regulations, creating consistency of systems and actions, reacting to accidents and incidents and on prevention of those, measuring safety with performance metrics and rewarding employees on those measurements. All of these axioms contain the seeds of destruction, and they create conditions and dynamics that precede the disaster - and ironically, they are all 'good things' we do in safety management. For example, a mere focus on safety in the business, and the measurement of it, create a culture of repression of mistakes and incidents, leading to a situation where the organization knows incrementally less and less of risks and risktaking behaviour - until the day it all coincides destructively! This paper discusses seven such safety delusions and how they impact on the views and safety management approaches. They are: ??The delusion of (risk) control, that risks can actually be identified, analyzed and mitigated. ??The delusion of compliance, that mere compliance to rules will safeguard the business ??The delusion of consistency, that standard operating procedures will ensure safe operations ??The delusion of predictability, that accidents and trends tell us everything about the risks of the future ??The delusion of human error, that people are the weakest link and that it can actually be dealt with through discipline and enforcement. ??The delusion of statistical trends, that safety is measurable with statistics ??The delusion of invulnerability, that we have achieved high levels integrity and capability to deal wit risks. Organizations that suffered catastrophes all share some or all of these delusions and this paper traces how these played a role in many disasters, ranging from the Titanic, Challenger, Piper Alpha, Bhopal, BP Deepwater Horizon and many more.

The 8-millisecond rule has been in use for many years as a basic technique to help minimize vibration levels from surface blasting operations. The rule was based on work carried out in the 1960’s by the USBM by recording the vibration levels from a series of test blasts in a quarry over a range of distances. This work included the firing of a series of single hole blasts and a number of single row production blasts using a variety of inter-hole delay times. The delay times employed were limited by the technology available at that time to instantaneous, 9, 17 and 34 milliseconds. There have been several papers in recent years suggesting that the use of the 8-millisecond rule is no longer required, when using electronic detonators, due to their enhanced accuracy and greater initiation pattern design flexibility. This paper examines this proposition by revisiting the original work and employing the linear superposition technique to reprocess some of the original recordings. Similar techniques are then applied to a large database of single hole vibration recordings for a range of rock types, distances, and number of decks, with calculated reinforcements compared to the minimum separations in typical patterns. The results suggest that there is some validity to the 8-millisecond rule when applied to electronic detonators but not in the way it is traditionally applied. Simple guidance is suggested for those designing blasts with electronic detonators to minimize vibration levels even if the benefits of linear superposition trials are not available.

Electric drives hire done outstanding job in many-fields that are related to the mining industry. The equipment that is offered for electric-trucks was developed from the traction equipment which is used on the diesel electric locomotive. Rotating equipment available for use on electric trucks is identical in principle to that which is used on these locomotives. In as far as it is practical, similar controls are used. Another descendent of the diesel electric locomotive can be found in the oil well drilling industry, Here, locomotive traction motors and. electric controls have been modified to fit the particular requirements of the drilling industry, One 2400 H.P. diesel electric drilling rig drilled a 24,000 Ft. well in West Texas with an electrical maintenance cost in the order of 7¢ per hour This was a new rig and the costs are less than the average maintenance costs will be for the life of the equipment, Even soy since it took one (1) year to drill this well, this 7¢ per hour has real significance. Electric shovels use a similar type of electrical equipment; they too have shown real savings over the equipment they replaced. Since electric drives have done so well, in other fields, they warrant consideration for use in high horsepower, large capacity trucks.

"In the 1990s metal prices were trending in a long-term decline and the usual cost-cutting exercises were adopted. Cuts to research and development made innovation difficult and the outlook was grim. But necessity is a strong motivator, and these conditions spurred the Mine to Mill movement that optimised across organisational ‘silos’ and utilised new technology tools, innovative software and increased computing power. Many applications of Mine to Mill exploited the fact that comminution is usually the site processing bottleneck and blasting is more efficient at breaking rock than grinding. This approach sought to: understand and characterise rock breakage from mining to the milldevelop models and simulators for blast design, fragmentation, crusher and mill circuitsdevelop tools to measure in real time the particle size distribution of rocks on run-of-mine (ROM) muck piles, at the primary crusher truck dump pocket and on conveyorsensure effective communication across the silos between geologists, blast design engineers, mining engineers and metallurgists.These methods and the technology tools available in the 1990s were widely adopted at numerous sites around the world with significant benefits. Semi-autogenous grinding (SAG) mill throughput increases of 10–20 per cent were common. This was the advance the industry desperately needed. It was low capital cost; it was obvious. It was here to stay.Except, in too many cases, it didn’t. In the minerals price boom of the 2000s Mine to Mill was no longer necessary to ‘survive’, even though it was still good business. Some operations, including some of the early success stories, slipped back to old habits of optimising within organisation silos instead of across them. Fortunately, other operators still embraced Mine to Mill and developed new techniques. Mine to Mill was still successful, but not as widely adopted as we had expected it to be. Now the boom has ended and operations are again under cost pressure. Since these are the same circumstances that created Mine to Mill, it seems time to achieve the wider adoption. Since the 1990s many new or advanced technology tools are now available for Mine to Mill projects: blasthole sensors, new explosives formulations, new blasting techniques and modelling, ore tracking devices, improved image analysis to determine size, texture and colour of coarse ore, grade sensors, whole-stream simulation tools, and even more powerful computing hardware and data analysis software. Material transfer in underground mines often relies on ore and waste pass systems. Over the years the authors have investigated the design and performance of orepass systems in several Canadian and South African mines. It has been recognized that while most mining operations have either implicitly or explicitly clearly defined objectives, in the design and successfully operate orepass systems there seems to be an absence of a strategy on how to best attain these goals. Consequently, it is not surprising that the majority of operations reviewed by the authors experience a number of problems of varying degrees of severity and economic consequences. This is illustrated by reference to both South African and Canadian operations. The second part of this paper focuses on a review of tactical interventions to rectify orepass problems or mitigate their impact. The paper closes with a framework for a flexible strategy for the design and operation of orepass systems. The previous papers have been concerned only with the atmospheric oxidation of coals from the Barnsley Seam, for which it has been shown that an adequate explanation of the origin and nature of gob-fires can be based on the reaction between the coal itself and the oxygen of the air without the intervention of any other substance. It becomes of importance, therefore, to ascertain how far a similar explanation will hold good for spontaneous heating in other seams, and with this object in view samples including anthracite and lignite have been taken from a considerable number of pits in different parts of the UK (south Wales, Northumberland and Durham, north Staffordshire, Portrush and Ayrshire), some liable to fire, others not. There is abundant evidence to support the commonly-accepted view that spontaneous heating is due to the heat evolved during the absorption of oxygen from the air by coal or pyrites. The quantity of heat produced by the reaction has been measured and it has been shown that the observed rate of heating of a sample of coal agrees with the rate calculated from the heat of reaction and the rate of absorption of oxygen

THE property of Consolidated 1 Discovery Yellowknife Mines Limited, (Figure I) is situated in the Northwest Territories, approximately 50 air miles north of the town of Yellowknife. The mine is reached by a shaft which extends from surface to a depth of 4,000 feet, with working levels established at 1 50-foot intervals. Stopping operations are carried on throughout a vertical range of 2,000 feet, with both shrinkage and cut and fill methods being used, depending on the ground conditions. Production started in 1950 and milling is done in a 1 65-ton per day cyanide plant. During ten months of the year, the property is serviced entirely by air, utilizing charter aircraft from Yellowknife. A 4,000-foot runway near the mine was constructed from mill tailings. This strip is adequate to receive Bristol freighter planes carrying heavy loads. Other servicing is done by Otter aircraft, equipped with skis or wheels for winter and with pontoons for summer operations.

"There are two accomplishments of air and gas drilling: a new series of economic and mechanical challenges; and, acceptance as a standard drilling and completion tool.The new challenges are related to the concept of using a gaseous circulating medium in drilling for oil or gas. For purposes of discussion, a two part division has been made: the challenge of fluid invasion control and that of equipment and tools. Great strides have been made in the development of materials and techniques to combat formation fluid invasion. Chemicals and the ""mist drilling"" technique, now in use, have the versatility to cope with a wide range of downhole fluid entry conditions.Equipment and tools have been developed to meet the challenges of better, more economic drilling and completion possible with a gaseous circulating fluid. Economic and operational challenges remain.Acceptance of air and gas drilling as a normal operating tool was reached with difficulty. It is now established as a means of reducing costs in exploratory and develop-mental well drilling, exemplified in a recent study of 226 wells covering 419,407 feet of hole in the United States. Air reached the projected depth in 85 per cent of the operations, although formation water was encountered in 48.7 per cent of the holes. Slim hole bit performance and cost all compared for overall bit averages as part of analysis."

"FOLLOWING the Leduc discovery •by Imperial Oil in 1947, The California Standard Company began to acquire land in the area immediately west of Edmonton and in line with the Leduc reef trend (Figure 1).The California Standard Winter-burn Province No. 1 in Lsd. 10, Section 4, Township 53, Range 25, West 4th, which was spudded, on the basis of a seismic anomaly, in December, 1949, failed to find reef. This well was located on the eastern .edge of a large land block which California Standard and Imperial Oil had pooled as a joint venture, with California Standard a:s the operator (Figure 2).The culmination •of an extensive exploration and land aquisition programme was the drilling of the California Standard-Acheson Province No. 1 well .in Lsd. 13, Section 2, Township 53, Range 26, West 4th. On August 24th, 1950, a drill stem test was run in the D-3 zone which had been encountered at 4,969 feet, and a flow rate of 702 barrels per day •of 36° gravity oil was obtained. Just previously, Imperial Oil had completed a similar programme in the Stony Plain Reserve with the swabbing-in of Imperial Stony Plain No. 1. These two discoveries set off a drilling programme to determine the extent and value of the oil accumulation.Subsequent drilling of wells some successful .and others not by several other companies as well as California Standard bias, .to date, still left much to be determined as to the approximate delineation of the productive limits of the Ache-son-Stony Plain area. However, to August 31st, 1951, there were twenty-one D-3 producers north of the reserve and three in the Stony Plain Reserve."

The Acid Drainage Technology Initiative (ADTI) was initiated in 1995 by federal agencies, the National Mining Association (NMA) and the Interstate Mining Compact Commission (IMCC) to identify, evaluate and develop cost-effective and practical acid drainage technologies. In 1999, ADTI was expanded through the addition of the metal mining sector, which is focused on drainage quality issues related to metal mines. ADTI addresses drainage quality issues from abandoned, active, and future coal and metal mines. The guiding principle of ADTI is to build consensus among industry, federal and state regulatory agencies on acid drainage technology development and technology transfer issues. ADTI is focusing its efforts on mine drainage prediction, sampling/monitoring, modelling and avoidance/remediation. ADTI is not a regulatory or policy development program. ADTI includes the Coal Mining Sector (CMS) and the Metal Mining Sector (MMS) under the overall guidance of the ADTI Operations Committee. The Operations Committee consists of representatives from industry, state and federal government and academia. The Coal Mining Sector includes two committees; a prediction committee and an avoidance and remediation committee. The Metal Mining Sector includes steering committee and work groups on prediction, sampling and monitoring, modelling, mitigation, and pit lakes. The Coal Mining Sector has produced two publications on the prediction and prevention of acid drainage. The Metal Mining Sector has workbooks under preparation covering the work group topics. This past winter, the Metal Mining Sector, working with the US Bureau of Land Management, presented a satellite training session on acid drainage. ADTI has evolved in response to changing needs and technologies as evidenced by the addition of the Metal Mining Sector in 1999. The Operations Committee and the members of the Coal and Metal Mining Sectors will guide future direction in response to identified issues.

Sumitomo Toyo Copper Smelter and Refinery was commenced in 1971, and its production capacity of sulfuric acid was 550tpd at the beginning. In 2001 Toyo Copper Smelter and Refinery projected to expand its copper production capacity to 450,000tpy, and now the project is still in progress. To achieve this expansion project, 4,000tpd of sulfuric acid production capacity is necessary to the acid plant. So as a part of this project, the construction of the new converting and absorbing line and the improvement of the existing converting and absorbing line were carried out from 2001 until 2005. Then the production capacity of these 2 lines has already reached the target, 4,000tpd of sulfuric acid. Moreover, at the design of the new line, we adopted several latest technologies and materials in order to construct the world's leading acid plant. Basing on the operation performance of the new line, we have improved the existing line in 2005. Both of these 2 converting and absorbing lines are working quite well and contribute to big energy saving and excellently stable operation. Cu2007

(Cleveland Meeting, October, 1912.) THE practice of carbonizing steel for the purpose of case-hardening has assumed great commercial importance within the past, 10 years. Formerly, case-hardened steel was held in more or less contempt, since it. was considered a cheap substitute for tool-steel. This is no longer the case. The development of the motor-car industry, and coincidentally that of modern alloy-steels, has resulted in overcoming many difficulties formerly experienced in case-hardening. This result has been accomplished by increased knowledge regarding the analysis of the steel which would respond most readily to case-hardening, and by more careful methods of treatment of the carbonized steel. The ordinary method of carbonizing consists in packing the steel with the carbonizing-material in cast-iron boxes and placing them in a furnace at a temperature of from 1,500° to 1,900° F. for a time sufficient to give the required depth of cc case." The steel is then either quenched directly from the box, or is allowed to cool without unpacking, and finally given a single hardening-heat., or, for special work, two or three heats. During the year 1911, more than 100,000 tons of carbonizing-material was sold in the United States, at an average price of probably $55 per ton. More than 85 per cent. of this material consisted of granulated bone. Within the past four years, many manufactured compounds have been placed on the market, consisting largely of some form of carbon or carbonaceous material, with or without the addition of chemicals. Steel will absorb carbon placed in contact with it at the temperature of the atmosphere. The reaction is exceedingly slow, but. increases rapidly with increasing temperature. An investigation was carried out by Mintek to investigate the influence of additions of gold to ?Fecraloy?, alloys based on Fe-Cr-Al. The aim was to try and improve the oxidation resistance of the ?Fecraloy? alloys at elevated temperatures over extended periods of time. The ?Fecraloy? alloys are very oxidation resistant alloys already employed in industry as substrates for metallic catalysts in auto-catalyst systems for high performance cars. Since their life and durability is limited by the slow, but steady, oxidation of the substrates, any change in the growth or stability of the oxides could be an improvement.

My topic, "The Administration of Mineral Rights'.' is possibly an unusual one, as it deals with a subject upon which many of us have a more than passing interest, I thought ?it might prove consistent with the objects of this meeting, more especially as I propose to deal, not with those broad subjects of nation-wide importance which are often the subject'? of learned (and sometimes futile) academic discussion, but rather with problems in their practical light. To the scientist, the important question is What is it? to the technician, Can it be utilized? to the business-man, Will it pay? All three questions are important, and so quick has been the march of business on the heels of science? that the one question follows the other with astonishing rapidity and all three must be considered in the modern management of a mineral estate. A .good illustration of the force of this can be found in the recent advance of our knowledge of the element helium in which we have advanced from a solution of the first question to the last question in the short period of 55 years. It is to be borne in mind, however, that in the administration of mineral rights, the question "Will it pay?:' is of primary importance. What forms a mineral estate ? A former practice in western Canada in selling lands was to sell the hair with the hide. All transactions sold the lands in fee simple, which included all mines, minerals and rights and privileges whatsoever, excluding only the royal metals, gold and silver, which by imputation, though not by specific reference, in deeds. of title are reserved to the Crown. At that time the country was in the first ?stages of settlement and the value of our mineral resources was hardly appreciated and little understood.

The flotation performance of kaolinite in laboratory micro-flotation was investigated with 3- (dodecyloxy) propan-1-amine (ON12) as the collector. Compared with dode cylamine (DDA), which is commonly used as the bauxite reverse flotation collector, ON12 shows stronger collecting ability for kaolinite than using the same amounts of DDA, when experimental pH value reaches more than 6. With the addition of ON12, the isoelectric point of kaolinite shifts to 6.1 from 3.6, which suggests a strong adsorption between kaolinite and ON12. Infrared spectra analysis indicates that the type of ON12 adsorbed on the kaolinite surface is mainly of electrostatic adsorption. Molecular dynamics simulation further indicate that electrostatic adsorption of ON12 cation on kaolinite (001) surface based on electrostatic interaction between NH4+ and O atoms. ON12 will be suitable for bauxite reverse flotation with the efficiency depressors of the diaspora.

The mechanisms involved in the adsorption of modified thiol-type collectors on sulfide minerals have been investigated in the present work. These collectors belong to the broad classes of thionocarbamates, thioureas, thiophosphates and thiophosphinates. They differ significantly from the traditionally used thiol-type collectors, such as xanthates and dithiophosphates, in terms of structural features that involve either the introduction of an additional oxygen donor or the alteration of electron densities on donor atoms. Electrochemical studies suggest that the adsorption of these classes of collectors is probably the result of a coupled reaction of the EC-type, involving an initial electron transfer step (E), followed immediately by a chemical reaction (C). Thus, the overall adsorption process is controlled by both the E, and pK of the system. The role of structural features in changing the interaction of the donor atoms with sulfide minerals is also discussed.

The adsorption of oleic acid and amine, by Expanded Hydrophobized Vermiculite (EHV) was studied. In appropriate conditions EHV is a good adsorbent for these flotation re-agents and the best results are obtained at situations where the chemicals are at non-dissociated form. The adsorbent shows good chemical stability and may been used in many adsorption-desorption cycles.

IT seems proper to present in the Transactions of the Institute, from time to time, formal record of the advances made in the arts and sciences to which our organization is devoted-milestones in the highway of progress-from which our successors may measure what they in their turn have gained. The admirable address of the Hon. Abram S. Hewitt, then president elect of the Institute, on "A Century of Mining and Metallurgy in the United States," read at the opening of the meeting held in this hall in June, 1876, forms a point of departure, and the conclusion of the eighth decade of the nineteenth century and the end of the first five years of our country's second century seem to offer a fitting time for again making up our record. The period since 1875 has been marked by a degree of activity in mining and metallurgical industry never before equalled in our history, while the progress made in the science and art has been, if possible, still more noticeable. Whether we consider the mining of iron, ore, and coal, or of the precious metals,-the advances made in smelting the former and in reducing the latter, the extraordinary increase in the production of Bessemer steel, or the science of Thomas and. Gilchrist, which makes the hitherto rejected phosphorus the corner-stone of success,-the past five years are alike memorable for the progress they have shown and to some of the most marked features of this progress I will now oriefly call your attention. It may be of interest in this connection to note the following figures, showing the growth of the population of the United-States during the last three decades :
