Comminution tests are an essential element in the design of ore beneficiation plants. Many methods have been developed to assess the breakage characteristics of rocks to generate parameters for modelling. However, most of the existing methods are laborious, expensive and require large samples. Consequently, test work has been traditionally conducted with a few representative reference samples. On the other hand, geometallurgical modelling programs require an extensive amount of test work to allow understanding the variability of ore properties within the deposit and establish spatial geometallurgical domains that show the differential responses to mineral processing. Therefore, comminution tests for geometallurgy need to be more efficient in terms of time, cost and sample size. This paper introduces a new testing device (Geopyörä) which is capable of measuring forces and energy applied to rock particles during the breakage process. The new testing method uses smaller samples and can be up to 50 times faster than other commonly used tests in the industry. This paper presents the concept prototype and the preliminary test work results, which have been benchmarked against some of the industry standard tests, such as the JK Drop Weight Test (JKDWT), the SMC Test® and the Bond ball mill test. On 10 August 1976, The Geosat Committee was organized in Denver to institute The Geosat Program. This program is an industry backed effort to evaluate and help select optimum space remote sensing systems for geological purposes. It is believed that the recommendations of The Geosat Committee can be supported by industry as additional exploration tools which can materially benefit industry and the Nation with respect to our energy and other natural resources needs. Appended is a general summary of The Geosat Committee Organizational Meeting and general review of The Geosat Program. The Geosat Workshop Report. "South African mining operations depend on the skilled professionals produced through the tertiary education sector. In geoscience courses at the University of the Witwatersrand, which lead into the mining sector, there has been a significant increase in student intake over the past 15 years. This increase has been characterized by a radical shift in demography: in classes where white male students once formed the majority, black female students are now in the majority. This demographic shift appears closely linked to policies addressing issues of social justice and transformation in post-apartheid South Africa. We document this change and consider the role played by employment equity legislation (as in the Mining Charter) and corporate social investment strategies in the mining industry, notably through bursaries offered to black female students. We also look at the changing needs of the industry, where the critical need for managerial skills, not just technical knowledge, is highlighted through data from surveys of past students and industry managers. Our findings indicate the need for a curriculum adjustment to meet these changing requirements, as well as more rigorous selection criteria for bursars than simply gender, race, and appropriate matric results. IntroductionThe South African economy has historically been linked to mining and the exploitation of mineral resources. South African mining operations are dependent on the professional labour that is produced through the tertiary education sector, with critical thinking, teamwork, and the art of negotiation having been identified as the crucial skills required for this industry (Sideropoulos, 2014). Over the past three decades the South African system of education has gone through a period of dysfunction that it is struggling to rectify (Maphai, 2014). This has impacted on the levels of academic preparedness of students applying for places in higher education. Concurrently, government and university policies have widened access to higher education in response to the need to address issues of social injustice and transformation related to South Africa’s history of apartheid. As a result, the number of students admitted to universities has increased dramatically. In the School of Geosciences at the University of the Witwatersrand (Wits), the increase in student numbers has been accompanied by a significant change in demographics: classrooms which in the past were dominated by white males have given way to classrooms in which black females are often in the majority. This demographic shift is related to legislation such as the Mineral and Petroleum Resources Development Act and the Broadbased Socio-Economic Empowerment Charter, developed to reshape the socio-economic and equity frameworks of the country (Botha and Cronje, 2015). Requirements aimed at transformation in terms of management, skills development, and employment equity, together with corporate social investment (CSI) requirements, have resulted in bursaries being made available to attract students from designated groups, which in turn has increased the number of black students, and notably black female students, taking courses in geosciences. However, due to the economic downturn, instability in the mining industry has recently resulted in the downsizing of some mines, with international investors pulling out of South Africa. This instability resulted in devastating strikes across all sectors of the mining industry (Stewart, 2013) and in Marikana, an important platinum mining area, the strike culminated in a deadly shoot-out between miners and police. The resulting tensions spilled over into the gold mining sector, resulting in further strike action, shaft closures, and retrenchments. These challenges have implications not only for the mining industry in South Africa, but for the tertiary education providers whose graduates would have been absorbed into the mine-related workforce."

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

The Cripple Creek Mining district is located in Teller County, Colorado, 30 km west of Colorado Springs and 9 km southwest of Pikes Peak (Fig. 1). Since its discovery in 1891, the district has produced almost 21 million tr oz of gold (Gott et al., 1967), most of which has been from high-grade vein deposits. Globe Hill is located in the northern part of the district (Fig. 2). Production from its summit has yielded over 100,000 tr oz of gold. Early production was concentrated along high-grade veins and fault structures, but emphasis was soon placed on the extensive, low-grade (0.0X oz/t), breccia-hosted mineralization. From 1977 to 1981 Newport Minerals, Inc., a subsidiary of Gold Resources, Inc., mined 680,000 t of low-grade gold ore from the Globe Hill pit (Fig. 3) and production is expected to resume in 1986. These strongly-altered and -oxidized breccias are amenable to bulk-mining methods, and thus similar deposits are currently being sought elsewhere in the district. Numerous publications and reports discuss the breccia-hosted mineralization at Globe Hill. They include Cross and Penrose (1895); Argall (1905, 1908); Lindgren and Ransome (1906); Keener (1962); Peters (1982); Thompson, Trippel, and Dwelley (1985); and Trippel (1985). This preprint is a summary of the recently-completed research documented by Trippel (1985).

"It is important to make a clear distinction on the scope of work of the Global Mining Standards and Guidelines (GMSG) guideline for sampling grinding circuits. Many mineral processing practitioners are familiar with the sampling and surveying of comminution circuits, in particular grinding circuits, for performance diagnosis and the identification of optimization potential. These surveys often provide information for population balance modelling of individual comminution and classification unit processes, and consequently require a large amount of information to calibrate and validate the unit models. As a result, these surveys are labour intensive, somewhat expensive, depending on internal capabilities of the mine site or company and have a long time span from initial planning to final reporting. They also require a high level of expertise to process the survey data, simulate the circuit and make sensible recommendations on follow up actions.As a result of these factors, full grinding circuit surveys are not a common occurrence at a large number of processing facilities, and several years may pass before follow up surveys are conducted. In the ensuing gap between surveys, the efficiency of the grinding circuit must often be taken on faith, and this is considered an unacceptable lapse in an environment of strained natural resources and low commodity prices.To support the rapid and productive energy efficiency analysis being promoted by the ICE working group, a much simplified survey and analysis process is required. The process reflected in the GMSG guideline is indeed more conducive to frequent analysis, at a skill level that is well within the capabilities of the average plant metallurgist. As a result, it becomes feasible to conduct mini-surveys of plant performance, which can be used to quickly and precisely diagnose the high level impact of process changes. These methods can also be manipulated by an experienced practitioner to provide sufficient information for modelling of the circuit to estimate the impact of process changes, allowing an initial rate and rank exercise to focus plant optimization efforts."

"Location. The iron formation of the Gogebic district (including the Penokee) comprises a narrow belt south of the western part of Lake Superior extending from Michigan for a considerable distance into Wisconsin. From the mouth of the Montreal River (about twenty-five miles west of Ashland) the iron formation is only about twelve miles distant, but it is about twenty miles south of Ashland. Beginning on the east at a point about eight miles east of Wakefield, Mich., it extends through Sunday Lake, Bessemer and Ironwood in Michigan, and thence south of west through Hurley, Wis., and only a short distance south of Mellen, Wis. About eight miles westward from Mellen, the formation disappears, but six miles further west it is known to reappear and continue thence for about eight miles. The total length of the range is about sixty miles, two-thirds of this distance being in the State of Wisconsin; but nearly all of the ore has been obtained from the eastern third of the district, which is in the State of Michigan.History. This iron range was first noted in 1848 by Dr. Randall, assistant geologist to Dr. David Dale Owen, while following the fourth principal meridian northward. It was also reported by Charles Whittlesey in an account of a survey made in 1849, and published by Owen in 1852. Analyses of the iron ore are given with the following account. ""The bed of magnetic iron ore south of Lac des Anglais is of extraordinary thickness,25 to 60 feet. * * In the wild and deep ravines where the Bad River breaks through the range, there is a cliff of slaty ore * * * probably three hundred feet thick. * * * I estimate more than one-half of this face to be ore, and in places the beds are from ten to twelve feet in thickness, with very little inter-mixture of quartz * * * . The position of the best exposures of ore which I saw is such as to require from eighteen to twenty eight miles of transportation to reach the lake."""

Gold was first found in Nova Scotia about 1830-40, but its significance at that time was not appreciated. Probably the first discovery that resulted in any work was made by Lieut. C. !'Estrange, at Mooseland, on the Tangier river, in 1858.1 Since that time gold has been mined with varying success until now, and from 1868 onwards work has never altogether ceased. Gold mining in Nova Scotia is at the moment under a cloud, and the idea generally accepted by most people is that it has failed in the past, and that there is no prospect for it in the future. The actual reasons for the present situation have never been carefully analysed, and it seems a good thing to see what really is the state of the case and whether or not this general idea is worthy of credence. It is the purpose of the present paper to analyse the situation in the light of present knowledge, and to set forth some data which may assist in the future development of the Province.

"Abstract - The Louvicourt Au-Ag-Cu-Zn-Pb sulfide deposit is hosted by a Middle Ordovician calc-alkaline sequence of aphyric rhyolite flows and associated fragmental rocks at the top of the Flat Landing Brook Formation. The Zr/TiO2 (average = 0.110) and Y/TiO2 (average = 0.021) ratios in the footwall felsic volcanic rocks are consistent with known values for the Reids Brook member. The deposit consists of two concordant lenses of semi-massive to massive sulfides, approximately 150 m long and up to 9 m thick, which represent a fold repetition of a single exhalative horizon. The deposit contains a geologic resource of 136 000 tonnes grading 1.2% Pb, 1.0% Zn, 0.4% Cu, 90.9 g/t Ag, and 0.96 g/t Au; the deposit locally contains elevated Au (2.9 ppm) and Ag (320 ppm) contents. A laterally continuous iron formation that contains barite caps the deposit, and is gradational vertically and laterally into laminated siltstone. The exhalite confirms the stratiform nature of this deposit, but a large percentage of sulfides occur as late replacements of altered felsic fragmental rocks in the footwall. Abundant examples of colloform pyrite and perlitic and spherulitic textures in volcanic clasts are well preserved. The base- and precious-metal content of the Louvicourt deposit is atypical of the Bathurst Mining Camp (BMC). Reported assay data (n = 27) indicate an average Pb/Zn ratio of 2.6, the highest in the BMC (average = 0.39). Lead exhibits a strong Spearman Rank correlation (r' = 0.92) with base metals throughout the sequence, indicating a preponderance of galena in both the stockwork and stratiform zones. The Ag and Au contents in this deposit are among the highest in the BMC and exhibit a Au-Ag- Cu-Pb association. Barite-rich exhalative pyrite exhibits a moderate Au correlation with Ag (r' = 0.77; n = 10) and Cu (r' = 0.76) and occurs in portions of the exhalative sulfides that are enriched in Pb. Chemical exhalative rocks are conformably overlain by trachy-andesitic volcaniclastic (possibly pyroclastic) rocks that are assigned to the Little River Formation. These are enriched in Nb (average = 38.4 ppm) and P2O5 (average = 0.17 wt.%), and have much lower Zr/TiO2 (average = 0.056) and Y/TiO2 (average = 0.0062) relative to rocks higher in the hanging wall. The intermediate volcanic rocks are conformably overlain by massive to pillowed basalt flows and related hyaloclastite that have Nb/Y = 0.60 and high TiO2 contents (average = 2.8 wt.%). This chemistry is consistent with rocks of the Brunswick Mines member (Little River Formation), except that these rocks are considerably enriched in P2O5 (average = 0.82 wt.%). This syngenetic exhalative deposit is unique for the BMC with a high Pb/Zn and elevated Au and Ag contents, as well as an epithermal signature (Sb-Ag-Au-Tl-Hg), analogous to high sulfidation volcanogenic massive sulfide deposits. Its intimate relationship with an emergent felsic dome (Flat Landing Brook Formation), chlorite-silicate-oxide-sulfide exhalite (locally hematitic), and the abundance of barite are classical features of many of the Kuroko deposits."

The Savage River open cut magnetite mine is located in rugged terrain in densely rain forested northwest Tasmania. The mine and concentrating plant are both in the Savage River valley. Pickand Mather Incorporated (PMI) operated Savage River as an open cut magnetite mine between 1966 and 1996. Acid rock drainage (ARD) was first noted in the last ten years of the operation, leading to elevated concentrations of heavy metals downstream of the mine. When operations ceased in 1996, PMI returned the mine to the Tasmanian Government along with a remediation fund of $11.4 million. At that time, the assessed cost of remediation was in excess of $120 million. As a result of the ongoing ARD contamination and to facilitate both a new operation and effective remediation, Goldamere Pty Ltd negotiated an agreement with the Tasmanian Government in 1996, which resulted in the Goldamere Act and the Goldamere Agreement. The agreement has allowed Australian Bulk Minerals (ABM) to remediate the Savage River mine in conjunction with the Tasmanian Government while re-invigorating a previously closed mine by providing ABM with an indemnity against past pollution. ABM can configure its mine and waste planning activities to maximise remediation benefits without fear of future litigation for past mistakes. The effect to date has been the development of a long-term strategy to remediate the Savage River and short-term remediation works such as water management, which provide benefits to the ecosystem. After five years, the Savage River is no longer significantly impacted by ARD for 30 kilometres downstream and trout are now reported within the reaches of the mining lease. The arrangements between ABM and the Tasmanian Government are an example of a genuine WIN WIN situation where co-operative environmental management of the site results in significant improvements and where the æregulatorÆ and the æminerÆ are working together to achieve mutually beneficial goals. The end result is a significant benefit to Tasmania.

Whereas mine reconciliation is a well-established month-end process for most modern mining operations, the resource sector has yet to endorse uniform reporting factors against which model accuracy and metal recovery can be formally assessed. Logically, any comprehensive reconciliation framework should facilitate comparison between period reserve, mine and mill and production figures. Historically, Parker’s F Series ratios (Parker, 2012) are often used in some form by mine operators, although they are rarely shared externally, as formal definitions have yet to be accepted. This paper presents a structural extension to the reconciliation process, wherein resource ratios (R Series factors) are used to quantify the long-term impact of the information effect, together with dilution and ore loss. A case study from the El Limón Guajes (ELG) Mine Complex in Mexico is used to demonstrate how an R Factor Series can be implemented to facilitate operation optimisation, over and above conventional F Series reconciliation treatments. This approach is applicable to any mining operation.

Silicon dioxide, silica, is the most common oxide in the earth' s crust. The health effects of silicon dioxide dust have been known since Biblical times. Agricola's book, published in the mid 1500's on occupational medicine, described a lung disease that afflicted miners. He observed a disease in miners that caused shortness of breath, coughing and wasting. Add fever, chest pains and fatigue and we have the current symptom complex of silicosis. As we approached the 21stcentury, the Labor Department began an aggressive campaign to combat silicosis-a preventable disease. Unfortunately, many publications do not clearly differentiate the polymorphs of silica known to mineralogists. Crystalline silica, i.e., alpha quartz, tridymite and cristobalite need to be considered differently from the amorphous forms of silica. Freshly fractured surfaces of the silica and host specific responses will be examined. Recent research focuses on the biologic interactions of free radicals generated by mineral particles. This results in cell injury and DNA damage, in turn leading to fibrogenesis and possibly carcinogenesis. In addition to particle-cell interactions, emphasis is given to lung defense mechanisms.

Introduction The first experiment in underground gasification of coal in the United States was conducted at Gorgas, Walker county, Alabama, under a co-operative arrangement between the U.S. Bureau of Mines and the Alabama Power Company, at the suggestion of the Power Company. The test was conducted on property of the Alabama Power Company, that owns some 19,000 acres of coal-bearing lands on which there is developed a highly mechanized mine producing some 2,500 cons of good quality steam coal per day. At the same locality ?there is a 200,000 kilowatt steam plant one of the largest in the southern portion of the United States. The Alabama Power Company owes much of its beginning in 1912 to British vision and courage. The original plans for building hydroelectric plants in Alabama, and for the construction of necessary transmission lines, were submitted to Sperling & Company, bankers of London, who agreed to underwrite certain bond issues, the proceeds of which would be used in launching the new Company on its programme. The set-up by which these funds from England were received was through an intermediary Canadian company, which the English investors desired. The Canadian company - Alabama Traction, Light and Power Company, Ltd. became owner of the securities of the Alabama Power Company and marketed its own securities through the banking house of Sperling & Company. Without assistance at this time the Alabama Power Company could not have gone ahead with its programme of expansion and the bringing together of the properties necessary for laying the foundation for a strong Company, fortified with ample power resources essential for future requirements and for dependable service to the public.

For the past 30 years there has been a gradual evolution of mechanical excavating systems in underground hard rock mining applications. This paper traces many of the tests of various tools and methods to break hard rock, up to the present. The tools include impact hammers, boring machines, disc on boom mining machines, disc on revolving wheels and drums, picks and drag bits on revolving hubs, rock splitters, slot cutting systems, and with customized explosives/powder mini-blasting. While the progress has been slow, successes are being achieved in hard rock and more opportunities exist today than ever before.

The Grande Côte Mineral Sands Deposit (GCMSP) is located on a coastal mobile dune system in Senegal on the west coast of Africa. Senegal is a stable, democratic republic, having gained its independence from France in 1960. It has a population of about 12.5 million people. The dunes begin about 25 km north-east of Dakar, the capital city of Senegal, and extend northwards for more than 140 km. The mineralised dune system averages about 2 km wide with some areas extending to up to 4.5 km wide.Mineral Deposits limited (MDL), a Melbourne-based company with over 60 years operational experience in mineral sand mining in Australia, holds, through its Senegalese subsidiary MDL Senegal SARL (MDLS), a 25-year Mining Concession covering the deposits. The main heavy mineral (HM) areas identified to date are, from south to north, Mboro, Fass Boye, Diogo and Lompoul. Within the Mining Concession there is potential to identify additional resources beyond the limits of present drilling. The dunes are mobile or semi fixed, pale yellow in colour and overlie older white marine sands. The dunes range between 5 m and 30 m in height and the mineralised zones, which are essentially flat-lying, average around 15 m in thickness. The mineralisation consists, principally of ilmenite with accessory zircon, rutile and leucoxene. Zircon and ilmenite are the main commodities of interest.Independent consultants AMC estimated mineral resources for the four main deposits in 2006 of 1330 Mt of sand averaging 2.0 per cent HM at a cut-off grade of 1.5 per cent HM. At a cut-off grade of one per cent the resources were 3200 Mt averaging 1.5 per cent HM. All of the estimate was classified as Inferred Resources under the 2004 Australasian Code for Reporting of Exploration Results, Mineral Resources and Ore Reserves (JORC, 2004).Subsequent completion of infill drilling, sampling and assaying of the initial mining area, centred at Diogo, was completed in 2008, enabling a final dredge path to be designed and ore reserves estimated, based on a JORC compliant Measured and Indicated Mineral Resource. Infill drilling is continuing in the Lompoul and the results of this campaign will be included in the JORC compliant data base by AMC in April 2010 to enable the 15 per cent year dredge path and mine plan to be finalised.Development studies have shown that the project can be optimised as a dredging operation, with mineralised sand being treated in a conventional floating spiral preconcentrator, wet concentrator and dry mineral separation plant. Engineering work now substantially completed by Ausenco Limited (Ausenco) has developed the combination of some existing plant from MDL’s past operations, and new plant for the complete process from dredging to final product separation and grading. Testwork on a series of bulk samples has determined that the project can yield a high quality zircon product and an ilmenite product along with minor rutile and leucoxene.Work to date indicates a likely output of around 80 000 t of zircon in various quality streams (Primary, Secondary, Standard and Foundry Grades), around 550 000 t of ilmenite plus around 15000 t of rutile and around 12 000 t of leucoxene/secondary ilmenite. Planned zircon output would represent around 7per cent of the total world production, which would make the GCZ&IP a significant producer on a world scale.The studies have indicated capital costs to be in the range of US$400 M total operating cost (after completion of ramp-up) of around US$50 M - $55 M/a. Revenue is expected to be in the range $127 M - $157 M/a.

SEVERAL years ago I became interested in computing the historic lead production of the United States, and the mines, or mining districts whence derived. This led me subsequently to an examination of the recent records- of some of the American zinc-mining districts and of the great lead and zinc-mining districts in foreign countries. My present article thus will be to a large extent an addendum to my book, "World Survey of the Zinc Industry," which was published by the Mining and Metallurgical Society of America in 1931, and to which I shall refer hereinafter as Ingalls, op. cit. Throughout this article all expressions of tons mean the ton of 2000 lb. Where original data have been given in metric tons I have converted in many instances simply by multiplying by 1.1, and long tons by 1.12. I have not been fussy about digits and have rounded off many figures. These may explain differences between data herein and other data.

Oil can enter a flotation circuit from a number of sources (for example, from mining equipment, oil leaks from the grinding equipment and coatings on the grinding media, but to name a few). Folklore generally states that any oil that contaminates the ore entering the flotation circuit is considered to have a negative impact on flotation. That is, a loss in concentrate grade and recovery.This represents a dilemma for some grinding media suppliers, who use a variety of quenching medium (air, polymer, oil or water) to achieve the desired physical properties of the ball. In the case of high chrome grinding media it is necessary, in some instances, to quench in oil to give the ball the right degree of toughness, which invariably leads to these balls having a thin film of oil on their surface. This paper provides both laboratory and plant data describing the effect quenching oil has on the flotation of copper sulphide ore, with surprising results!

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Tunnel boring machine (TBM) tunneling is an ever-increasing prospect for underground construction, and, with each new tunnel bored, there are unknown elements. When boring through the earth, even extensive geotechnical baseline reports can miss fault lines, water inflows, squeezing ground, rock bursting and other types of extreme conditions. This article draws on the consider-able field service experience within Robbins to analyze successful methods of dealing with the most challenging conditions encountered. Many tunnel projects are located in areas with relatively poor access along the tunnel alignment and bored under extremely high overburden. These two factors often result in limited geological information. It would be reason-able to state that the deeper the tunnel, the greater the lev-el of uncertainties. When faced with these uncertainties everyone involved with the project including the owner, the contractor and the machine supplier must be prepared to tackle geological surprises. This article describes the problematic geological conditions and associated difficulties faced on three separate projects and the measures that were taken to overcome these difficulties.
