"Extensive literature surveys, research in the project environment and personal experiences both within and consulting to project teams have revealed some consistent ‘people factors’ that contribute most to ultimate project success. Outside of purely technical and business case issues, there are a small number of leadership characteristics – probably only ten – that if correctly applied can positively leverage a team’s impact on project success.Of the top seven project success characteristics that were consistently highly rated in a recent study, five of them were around leadership. The authors have focused on this topic, noting that good leaders need to display the following traits:passionate commitment to deliveryconsistent modelling and reinforcement of correct values and behaviourfocus on managing relationships well and energising teams and individualscarefully clarifying project vision from front-end loading stages onwardseffective decision making.Not every project manager can and does practise these behaviours all of the time, but being aware of them and receiving training or facilitation in improving their application of these skills can equip project managers with the essential tools to markedly improve their own performance and that of their teams.This paper draws upon experiences in the field and provides lessons – both good and bad – to illustrate. As well as examining leadership traits, the way in which a leader can establish, organise, motivate his or her team and ultimately lead them to success is examined. The potential benefits of implementing the Last Planner System (LPS) on mining infrastructure projects were explored through two case studies in the South African mining industry. The first part of the study involved measurement of the impact of LPS through earned value techniques on secondary project data. The second part sought to establish the lean construction success factors leading to improvements in construction performance. This was done by means of a research questionnaire distributed to the project owner’s teams, contracted parties, and the LPS facilitators. The results revealed a positive correlation between LPS application and planned percentage complete. However, the performance achieved during the LPS pilot was not sustained. The success factors and benefits considered most evident in the case study pilot differed among the three stakeholder groups. The findings are expected to guide construction stakeholders to better define performance measures and focus on factors required to make LPS implementation more effective in the South African mining sector.

Two adjacent open pits in Mali hosted by deeply weathered saprolitic materials were mined consecutively over the course of approximately 12 months. During excavation, both pits experienced significant displacement and tension cracking as the result of deeper than expected depth of weathering coupled with relatively steep slope angles and significant groundwater. In response, geotechnical and operations teams overcame numerous monitoring challenges with prisms and radar to develop a simple and effective monitoring system coupled to a Trigger Action Response Plan (TARP) which enabled continued safe operations while reducing ore loss. Observations of previous slope performance of these two pits allowed for greater confidence in empirical failure runout calculations that was used to specify the standoff widths for the exclusion zones in the pit. Back analysis of the slope as-built performance after the first indications of slope distress was also conducted. The back analysis was used to confirm rock mass parameters for modelling while being utilised as a reality check for the stability analysis. The stability analysis indicated that leaving a large step-out or buttress would not prevent the displacement, resulting in some ore being sacrificed to ensure the safety of personnel. Key learnings from this experience highlight the importance of: • up-to-date geological interpretation of the depth of weathering • ensuring slope design guidance is reviewed and updated as latest information (ie geology and monitoring) is received • having multiple complementary slope monitoring methods to ensure redundancy • ensuring visual observations and as-built performance are utilised as a reality check to provide greater confidence in stability analysis and design guidance.

Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

In June, 1893, while I was with the Old Dominion Copper Co. at Globe, Ariz., I received an offer to take charge, as manager, of the Baltimore Electric? Refining Co. which had recently built a plant at Baltimore, Md., having a capacity of about 1200 tons of electrolytic copper per month, using the Hayden series system. This was a very large output, for in those days the plants were small and the art was in its infancy. At the beginning of 1893 there were 11 electrolytic refineries in the United States, most of them using the multiple system. The Hayden series system was used at Baltimore and Bridgeport and the Smith series system was in operation at the Electrolytic Copper Co. at Ansonia, COM., and at the Pennsylvania Salt Co. at Philadelphia. In the latter system the plates were placed in a horizontal position, but the operating difficulties were so numerous it was soon discarded. The total production of these 11 plants in 1892 was very small; I have not been able to discover reliable figures of production. Safety and health at work is about people. It is about people who plan for work, organise work and perform work. It is about those who come in contact with the workplace and those who are responsible for the workplace. There are wider ranging safety and health obligations that are imposed on the industry stakeholders that span all phases of the production process. In all cases, those responsible for discharging the obligations must ask the questions: Is what we propose safe?, can it be done safer? and are we sure? What we learn from recent legal cases before the courts is that it is essential that all stakeholders work together in a collaborative way when discharging these obligations. Safety and health at work cannot become an issue that is viewed only through the eyes of a single stakeholder group. The reason for this is quite simple. The interrelatedness of the responsibilities that are imposed on each of the stakeholder groups, means that the performance based legislation simply wont work, where an active model of communication and consultation is not in place.

Area wide pillar failures, or “squeezes,” were once fairly common in underground coal mines. While rare today, they do occur. This paper describes seven case histories that were investigated by MSHA Technical Support. They occurred across the range of US coalfields, in both longwall and room-and-pillar mines, and involved development mining, pillar recovery, longwall panel extraction, and multiple seam interactions. Significant lessons from each are discussed.

"Thousands of years ago large glaciers carved much of the landscape in Puget Sound, depositing abrasive and granular soils including cobbles and boulders along with layers of clay and sand. Tunneling in this geology – common throughout much of the northern and Midwestern U.S. – is challenging and can be unpredictable. The use of pressurized face tunneling, most commonly with EPBs and slurry type machines, has made excavation in this geology more efficient, but difficulties still remain. Through analysis of recent projects in Seattle, Washington, USA, the authors draw conclusions as to the best TBM designs and operational parameters for this variable, abrasive and often difficult ground. Performance data is analyzed with an eye towards advancements that could further improve TBM tunneling in glacially deposited soils. INTRODUCTION Over the past several ice ages, extensive glaciers reaching across much of North America transported large amounts of soil and rock as they moved across the land. The substantial weight of the ice sheets mixed together a wide range of soil types along with a variety of rocks and boulders of all sizes. Many rock formations were reduced to pebbles or particles containing highly abrasive characteristics. Any boulders able to survive were generally comprised of harder materials which were scattered throughout the soil. This highly variable mixture of abrasive soils and high strength boulders can be found in many areas across the Puget Sound and other regions of the country. Wherever they are encountered, they often create a difficult and unpredictable mining environment for both equipment design and operation. Modern TBMs are optimized early in the design process for specific types of geology, and unexpected conditions can require extensive modifications. In addition, several operational parameters need to be adjusted to balance ground characteristics, and advance rates can be significantly affected in mixed and rapidly changing soil conditions. Both EPB and slurry machines can be used successfully in these conditions, provided their respective operating criteria are met. Ideally, an EPB is operated with a relatively thick muck consistency, which accommodates moderate changes in soil permeability and maintains a reliable pressure differential through the screw conveyor. However, this also causes higher wear on rotating components and requires more energy at the cutterhead. Alternatively, slurry machines typically operate best with a more fluid muck consistency that can be easily pumped through the extensive piping system. This tends to reduce the wear on the cutterhead and the power requirements of the cutterhead drive system, but also makes the machine more sensitive to changing soil conditions. Generally, the slurry piping system is also more capable of controlling high face pressures, although the screw conveyor of an EPB can also be designed to accommodate most high-pressure conditions."

Accidents involving powered haulage and mobile equipment like haul trucks are a major safety concern in the U.S. mining industry. They account for more than 50 percent of surface mine fatalities each year [1]. A recent National Institute for Occupational Safety and Health (NIOSH) analysis of 91 fatal incidents identified loss of vehicle control and situational awareness as key contributors [2]. However, the first-hand perspectives of operators are often missing, emphasizing the need for deeper insights into operators’ decisionmaking, lessons learned, sensory cues and mental strategies used during near-miss events. The findings in this analysis will help inform the development of training and safety solutions to prevent or reduce haul truck accidents.

High Speed 2 (HS2) mandated that Building Information Modelling (BIM) processes be implemented during design and construction. Gall Zeidler Consultants (GZ) was responsible for developing the BIM Models for the portals, tunnels, and cross passages of the of the 16 km-long Chiltern twin-bore Tunnel. In addition, GZ developed the GBR for the Chiltern Tunnels and incorporated the relevant information into the Project’s BIM framework. This paper presents the implemented BIM framework, describes the modelling approach, and discusses the advantages and challenges of implementing BIM, particularly how BIM improved interface coordination and how BIM was used in feasibility assessments of different construction options.

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. I chose the title "Let's Clear the Air" for this afternoon's presentation because it has two meanings, the literal and the figurative. Most of you know what MSHA is. Most of you know what MSHA does. And, for the most part, most of you know why. What I hope to accomplish today is to "clear the air" around our health standards, to clarify for you how MSHA views its congressional mandate under Title I Section 101(a)(6), (7), and (9) of the Mine Safety and Health Act. Asbestos, silica, radiation, noise, welding fumes, carbon monoxide, lead, hydrogen sulfide, nitrogen dioxide - the list of physical and chemical hazards encountered in the metal and nonmetal mining industry is as diversified as the industry itself, possibly even more so. In addition, miners are often exposed to several of these health hazards simultaneously.

I can think of no better candidate than Daniel that is deserving of this award ? an award to recognize technical excellence in advancing ground control technologies [and] approaches. Daniel?s education and career spans over three decades. Having graduated from National Cheng Kung University in Taiwan in 1972 with a B.S. degree in Mining and Petroleum Engineering, he spent the next four years in the Taiwanese military and mining industry, before moving to the United States, where he enrolled in West Virginia University?s Mining Engineering Department. He received his M.S degree in Mining Engineering from WVU in 1978; then in 1982 became the first person to ever receive a Ph.D. in Mining from WVU. His thesis was titled, ?Development of Ultrasonic Techniques for the Measurement of In Situ Stresses?.

"This paper discusses the unique design and construction circumstances of the cantilevered dual sheet pile walls and the Accordion Sheet Pile Wall for the repair and upgrade of the East Garden Grove-Wintersburg Channel in Huntington Beach, California that was concluded in 2014. It also describes the 2008 Emergency Sheet Pile Project for the same channel. Both projects utilized the Press-in Piling Method for its low noise and almost no vibration advantages.BackgroundThe East Garden Grove-Wintersburg Channel is part of the Orange County Flood Control District (OCFCD) and is operated and maintained by OC (Orange County) Public Works (see Figure 1 for its location). The channel, referred to as OCFCD facility # C05 (C05), had suffered significant toe undercutting and slope erosion of its levee due to a series of storms. These began after the facility was constructed in the 1960’s and culminated in 2005. Consequently, the levee was deemed significantly deficient and was eventually declared as a local emergency in 2007. C05 was an incised channel and its levee was constructed primarily from native materials. Operation and Maintenance (O&M) personnel notified the design staff at OC Public Works to be aware of major consequences associated with the dilapidation problem in the most downstream reach of the channel in the vicinity of the Bolsa Chica Wetlands Restoration Project. In 2007, the Orange County Board of Supervisors declared a local emergency to fix the levees with sheet piles to be driven by the Press-in Piling Method, hydraulic pushing piles into ground by holding onto previously installed piles. The project that ensued represented the first in-tandem use on the West Coast of the United States of the Press-in pile driver with its GRB System (Press-in pile driver, a support crane and pile transporter travel on top of already-driven sheet piles). This is further discussed in this paper in the sections titled “Press-in Piling Method” and “GRB System”, respectively. The project repaired approximately 3,800 feet of erosion on the north levee of C05 with 45-foot long cantilevered sheet piles. The sheet piles design constraints are discussed in this paper in the section titled “C05 2008 Emergency Sheet Pile Project (Phase 1)”. Between 2012 and 2014, another mile’s worth of levee reach was improved with the Soil-Mix-Sandwich (SMS) Method. This concept consisted of deep soil cement mix (DSCM) columns sandwiched within the dual rows of sheet piles. Additional details on the SMS, its DSCM construction, and its hybrid geo-structure design concepts are well beyond the scope of this discussion, which is primarily focused on sheet piles. Instead, the interested reader is referred to two papers on the SMS and its DSCM metrics (Fayad, 2015; and Arora, et al., 2015). These were presented and published in the Proceedings of the Deep Foundations Institute (DFI) 2015 Conference on Deep Mixing. Hence, in the section of this paper titled “C05 SMS Dual Sheet Piles Project (Phase 2)” only the sheet pile design constraints of the SMS are covered. Finally, in the area where the project traversed the Alquist-Priolo zone of the Newport-Inglewood tectonic fault, an expandable and deflectable sheet pile wall was used to isolate the flood channel from the Bolsa Chica wetlands. Its details are addressed in the section titled “C05 Accordion Sheet Pile Wall (Phase 2)” of this paper."

"Solids concentration (% solids) profiles and the clear liquid/slurry interface level (mudline) were determined from conductivity measurements in thickeners at Falconbridge' s Strathcona mill (Ni concentrate and non-mag thickeners) and Kidd Creek Division (Cu, Zn concentrates and final tails in the concentrator and thickeners in the Zn plant).The conductivity measurement system consisted of a probe 1.5 m in length with 24 electrode cells, a 24-channel relay, a conductivity meter and a computer. A conductivity profile is collected which locates the mud-line by the sharp change in conductivity. The conductivity measurements are readily converted to solids concentration profiles without need for calibration.The results from the conductivity measurements were compared with those from pressure profiles and direct measurements on local samples obtained using a modified Noranda sampler.It is concluded that the conductivity technique is applicable to locate the mud-line and to estimate the solids concentration profiles in thickeners. This on-line information could improve the operational efficiency of industrial thickeners.INTRODUCTIONAn automated, on-line technique for detecting the clear liquid/slurry interface (or mudline) and the variation in solids concentration with depth would enhance thickener operation and control. In some processes such enhanced control offers significant benefits. For example, in hydrometallurgical operations solid/liquid separation is often the process limiting step. Water treatment plants are increasingly expected to operate remotely and reliable sensing is vital.Current attempts to detect the mud-line include reflection of some signal (e.g. sonar), floats (Dahlstrom, 1985) and measurement of pressure (Rosenblum, 1991), and in one case, natural radioactivity of the solids (Johnston and Simic, 1991). A recent commercial development uses reflection from a vertical array of optical transmitters/sensors, which also gives local solids concentration (BTG Inc., 1991)."

Simultaneous mass-balancing of a mineral processing flow sheet results in adjustments to each data point that reflects the accuracy and precision across the full set of data. In this way, all the good work that goes into designing the sampling and analysis protocol for each sample point is leveraged in order to produce a æbetterÆ set of balanced numbers for every sample point in the circuit. Few operating plants employ whole of circuit balancing techniques to produce reported operating results. This situation is changing with the application of mass balancing technology which utilises redundant data for production accounting and metal reconciliation from product to ore source across multiple phases and multiple time periods. Mine and plant data can now generate credible information to improve productivity. Mine and plant personnel can now use consistent data sets for review and planning. Decisions based on balanced data can be made quicker and with increased confidence.

Building Information Modeling (BIM) is an increasingly popular method for generating and managing facility information during the life cycle of a building, ranging from facility conceptualization, through design, construction and its operational life. Organizations involved in Facility Management (FM) have the opportunity to use BIM as a knowledge repository to document evolving facility information and to support decisions made by the facility managers during the operational life of a facility. This paper demonstrates the potential of using BIM to develop algorithms that automate decision making for FM applications. The potential of utilizing BIM as an analysis tool is demonstrated through the scenario of HVAC (Heating, Ventilation, and Air Conditioning) system failure in an operating facility. In case of a typical HVAC malfunction today, facility occupants record complaints in a ticketing database maintained by the FM organization. Upon receiving notification of HVAC system failure, facility inspectors visit the location to confirm the reported failure. Upon confirmation, facility managers review building plans and specifications to develop a detailed plan of action to repair any HVAC components suspected of damage. Based on the plan of action, inspectors visit the facility to inspect and, in case of damage, repair the appropriate HVAC system components. These FM practices ? as currently implemented across the industry ? are labor intensive, time consuming, and often rely on unreliable and outdated information. To address these shortcomings, the authors propose an alternative methodology of HVAC fault detection in operational buildings. The authors implement an algorithm that leverages complaint ticket data and automates BIM to determine potentially damaged HVAC system components. Based on the list of HVAC components suspected of damage, the algorithm develops a plan of action for the facility inspectors. Finally, the authors discuss the advantages of the proposed method as well as the challenges of implementing automated BIM-enabled decision making processes in the FM industry.
