In Australia, Safe Work legislation requires mines to have a documented ‘Safety Case’ explaining how a mine will safely manage all major hazards. A similar requirement is legislated or inferred by regulation in all jurisdictions. For small scale, instabilities leading to day-to-day seismic hazards, a ‘Stability Safety Case’ built around observational methods and measurement triggered responses can be effective because adverse situations are easily identified and the costs of lower control measures are not onerous. Also, if the influence of a volume of yield on the mine load system is small, the response of the system will be more predictable. However, for episodes of large scale mine instability, the hazard derives from the widespread, multi component influence of the event on the mine load system. As the system response depends on interactions between many parts, the resolution of predictions will be lower and situations can arise without local precedent. Further, as episodes of large scale instability change the stiffness and nature of the connectedness between parts of the mine load system, the behavior of the system before the event will be a poor empirical basis for forecasts of event related hazards. Most importantly, Triggered Action Response Plans (TARPs) and ‘traffic light’ warning systems cannot make design choices or identify new hazard phenomena. They are ‘lower’ control measures targeting explicit human-made instability hypotheses. As such, the Safety Cases related to management of hazard from potential large scale seismic events and mine instability cannot rely solely on TARPs. Mines that successfully manage meaningful dynamic instability always acknowledge the ultimate reliance on expert human intuition, knowledge and imagination to appreciate the range of possible hazard scenarios, to make stability hypotheses and propose solutions. Complex analysis, TARPs and measurements are critical steps in the human decision cycle, but only support the making and testing of ‘Stability Hypotheses’ made by expert engineers. Relying on expert skill and judgement as essential components of a Safety Case, however essential, introduces human vulnerabilities. Another challenge is that the expert decision making process is hard to document. Beck et al 2019 first described an organizational and procedural framework for maintenance of an ongoing large-scale ‘Stability Safety Case' where a degree of expert judgement and imagination is needed to manage a seismically active mine. The structured data appreciation process outlined sufficient measurements, analysis and appreciation to confirm the stability state of the mine and was applicable to all underground mines. After several years of implementation in various forms in stoping, caving and room and pillar mines, this update addresses lessons learned and describes some considerations for numerical forecasting to establish a base case, and some challenges for empirical hazard identification.

Maintaining and improving the economics of open pit mining in the future requires understanding of the characteristics of open pit mining that are different to other mining or business activities, designing mines in the first place to be less susceptible to changes in these important characteristics, and adopting strategies which yield reliable returns despite unforeseen change. Planning and production personnel require guidelines and performance standards that are based on sound economics not solely on production criteria. The guidelines adopted for long-term implementation must assume a declining price for mineral products along with declining real costs, with research efforts directed at development of both evolutionary and revolutionary ideas aimed at implementation. Decision-making must focus on risk and probability criteria rather than just simple return-on-investment criteria, and the approach for management of personnel must support the higher level of communication essential for fast decision-making in the new environment of continual change.

Mill crushing drive systems are essential to cement producers. With the increased emphasis on operating efficiency, it is vital that these systems be dependable and reliable. No one needs the costs or headaches associated with unscheduled downtime or inefficient machinery operation. A reliable drive system starts with the proper selection of the ring, or girth, gear and continues through proper maintenance. Ideally, selecting the right girth gear brings together the gear manufacturer, the system designer and the end user. The gear manufacturer must know the details of the application, the demands that will be placed on the gear and the nature of the equipment to be driven. Similarly, the user and the system designer must be familiar with the variables that affect gear drive performance and service. For example, an application that places a torque load on the drive in excess of its rated capacity will result in gear tooth distress and, in severe cases, breakage.

Successful evaluation and exploitation of resources and reserves relies on the collection of high quality sample data. The quality of the sample data must be maintained through the application of a robust quality assurance and quality control (QAQC) program, including documentation of sampling procedures, training of sampling personnel and the regular monitoring of control samples. However, the application of QAQC programs in the minerals industry is often poor and the purpose misunderstood. It is all too common to have good quality control data collected, but no analysis work carried out until after drilling is completed. In a lengthy drill program, this can lead to problems not being discovered and corrected as they happen. Moreover, rectifying problems afterwards will be costly in terms of time and money, if not impossible. The consequences of poor quality sample data are many and include such effects as over- or underestimation of block grades, misclassification of ore and waste, and lower confidence in the resource/reserve models. The purpose of this paper is to highlight, using case studies, the importance of good QAQC practices to monitor the quality of sampling and laboratory sample preparation and analysis.

The successful operation of diesel equipment underground does not entirely depend on selection of robust, reliable, easily maintained machines. Results are also heavily influenced by the following factors: Design and condition of the operating area. Skill of operators. Correct maintenance facilities. Experience, skill and initiative of maintenance personnel. If the mine layout does not allow access to a central workshop area, the effect of any shortcomings will be magnified. The best results will only be obtained by close co-operation between mining and maintenance departments right from the initial planning stages.

Engineering availability of the 120/200 tonne Wabco haulpak fleet at Mt. Whaleback was in the mid fifties and unstable. The project to achieve a reliable 66% during 1980 highlighted "plannable" maintenance, short cycle scheduling, condition monitoring, backlog control, and, above all, the importance of involving all affected people. It is not sufficient for consultants to design systems and make them work. The people who use the system must understand it, support it and "own" it. In this way, they will develop it and improve results after the outside help has been withdrawn INTRODUCTION In early 1979, PA proposed a programme to improve engineering availability of the Mt. Newman Mining fleet of 70 Wabco haulpak trucks of which 53 were 120 tonne and 17 were 200 tonne units. (Subsequently five 200 tonne trucks from Hamersley Iron were added to the fleet). The ore truck workshop was also responsible for 75 tonne trucks used as water trucks and general purpose vehicles. The need for action was clear. In recent years, miners have tried many approaches to maintenance such as: - Highly sophisticated computerized programs. - large planning groups designed to preplan and schedule work. - Contract maintenance. - Break down maintenance. Each of these approaches has faced the added constraints: - High replacement cost of equipment necessitating longer useful equipment life - management?s increased scrutiny of all indirect costs such as maintenance. Faced with these factors, what system of maintenance control is right? After visiting many properties recently, it appears there is no right system. Maintenance programs need to be tailored to the people and production constraints that occur at an individual property.

A new sense of urgency permeates the mining industry concerning maintenance. Mining's continuous process environment demands around-the-clock maintenance support. And profitability conscious mining operations require that production targets be met on time - and economically. These goals require nothing less than an effective maintenance program. In turn, the effectiveness of the maintenance program depends on quality information: easily procured, complete, timely, accurate, and properly used. Information picture is changing Some of the fully integrated, mainframe maintenance information pro¬grams of better days remain. Their upkeep and cost of operation, though, is becoming more difficult to justify. Some operations, faced with production cutbacks a few years ago, downscaled and found they liked it. Now, during an upswing, these operations question the need for the more elaborate, administrative-heavy programs they used to operate. Package programs are not taking their place. Too often, the packages are vague and generic. Some vendors claim that if their offerings work in a hospital complex, they must work in an underground mine. That is not so. Maintenance information in the mining industry is different. The situation has changed There are now smaller, self-contained maintenance groups whose information needs are different. These include an electrical crew in a smelter, an underground mobile equipment shop, or a remote service shop between several distant operating pits. These working environments have changed the character of the information needed as well as the methods for obtaining and using it. Operations-maintenance link In coming back from a downturn, operations people realize that their ability to meet production targets hinges on reliable equipment and avoidance of downtime. They have become familiar with the need to use maintenance services effectively. They also want quality maintenance information. When coupled with the sense of urgency about maintenance, a business-like working environment emerges that has significant new differences in both work control and information. Work control impact Work control has been streamlined to improve communications. Supporting information has been streamlined as well. • Verbal orders are the rule rather than the exception to quickly bring a multi-craft crew to the flotation section or a pit crew to a disabled haulage truck. • Written work requests and work order forms are seldom used, or even needed. Rather, electronic work control logs permit operations to request work, maintenance to acknowledge it, get ready, and do it - quickly, without delay. • Routine servicing is triggered by operating hour accumulation (mobile equipment) or tons of throughput (fixed equipment). Operations is now taking the initiative in delivering mobile equip¬ment to the "bad order line," or switching to a bypass conveyor. Modern production equipment has been designed with greater productive capacity and higher reliability to meet the rigorous operating demands of the mining industry. These improvements have also made the equipment more complex and challenging to maintain. Whether these equipment enhancements contribute to more effective mining operations depends on the overall quality of maintenance. Maintenance is not a stand-alone activity. Although its quality depends primarily on maintenance, cooperation and support from other departments, coupled with the skillful guidance of mining managers, greatly influence maintenance success. Therefore, maintenance risk management must look beyond maintenance itself. Successful maintenance risk management begins with an assessment of the maintenance-related interactions of all mining departments within the mine’s stated operating strategy.

Background of Erie Mining Company Erie Mining Company, managed by Pickands Mather & Co., is one of the largest producers of iron ore pellets in the United States. The plant is located at Hoyt Lakes, Minnesota, sixty-five miles north of Duluth on the eastern end of the Mesabi Range. Taconite Harbor, situated on the north shore of Lake Superior, is the location of its Power Plant and shipping facilities. (Figure 1) Even though the plant commenced operation in September, 1957, its true be-ginning can be traced back to 1931 when Pickands Mather, in conjunction with Bethlehem Steel Corporation and Youngstown Sheet and Tube Company, began investi-gating the possibilities of processing low grade ores. In 1940, Erie Mining , Company was formed. During the early and middle 1940's, laboratory and pilot plant research was proven to a point where a preliminary plant was deemed feasible and built in 1948. This plant had an annual capacity of 203,200 t (200,000 long tons) per year and, until mid-1952, was the only taconite plant in existence using commercial-size equipment. During this period, ownership of Erie Mining Company was expanded to include Interlake, Inc. and The Steel Company of Canada, Limited. In 1953, authorization was obtained from the owners to build a 7.62 M t (7.5 M long tons) per year plant at a cost in excess of $300,000,000.

Barrick Goldstrike Mines Inc. (Goldstrike) operates a six circuit (train) pressure oxidation plant for the pretreatment of gold bearing sulfide ores. The property is located 27 miles north of Carlin, Nevada. Since the commissioning and start-up of the first circuit in 1990 until the present total plant availabilities have exceeded 90%. "IntroductionMaintenance is a business, which to be successful, requires good management, systems and reporting. The prime objective of the maintenance management group, be they large or small, must be to anticipate and prevent interruptions in operations by keeping equipment in good mechanical condition. This objective should be carried out at minimum cost consistent with maximum productiveness.The objective of this chapter is to give an insight into the various aspects of maintenance management that must be considered to effectively and efficiently operate a mill complex. The degree to which the maintenance sub-systems are developed in an operating mill will be determined by the size of the mill and the needs of the operation.The criterion of the management system used is that the system serve the operation and not vice versa. The systems presented should be considered as tools and, as with all tools, they must be sharpened and replaced as the need arises or as more effective tools become available.To understand what is involved in maintenance management, it is worthwhile to consider a systems approach to analyzing management problems. This approach was developed by J.A.B. Rae while reviewing maintenance management systems for Cominco Ltd.A very simple system involving managing results and objectives is shown diagrammatically in Figure 1.The systems approach to organizational development involves four basic elements: an objective, a result, an activity and feedback. In the analysis, the activity element is broken down into individual actions that people could be held responsible for. If we introduce the action of operating into the diagram of Figure l, we arrive diagrammatically in Figure 2."

The Internet is creating new distribution channels and offers new technology platforms with significant advantages over more traditional methods. Mines will have to re-evaluate both the strategies and the tactics employed in these areas. Mines will use e-commerce to integrate their maintenance, repair, and operating supply chains. The integration will extend beyond traditional trading partners to a worldwide network of communities with similar interests and goals. There are many concepts of preventative maintenance. The basic objective of all maintenance organization, however, is the efficient (accomplishment and control of all) inspection, repair, overhaul, material and preventative maintenance functions necessary to maintain facilities and equipment in a condition that will meet operating requirements at a level consistent with low costs and high availability. To formulate a preventative maintenance program requires sound judgment and the ability to apply in a practical manner a system that meets Plant operation requirements, based upon organization, experience, conditions, design and historical data. A maintenance program must be divided into to categories consisting of: 1. Scheduled tasks to be accomplished at planned intervals. The objective of these tasks is to maintain reliability and prevent as much as possible abnormal failures. 2. Non-scheduled tasks that result from periodic reports and which can be scheduled to fit in conveniently with other routine work. The non-scheduled work items may include long-range preparation for lenghty shutdowns requiring Engineering and Shop services.

"A large-scale expansion to Pacific Coast Bulk Terminals Ltd. has now been completed and placed in operation. The original plant, built approximately eight years ago, consists of the direct rail-car to ship transfer of bulk commodities. The extension includes additional rail-car unloading, a transfer house system for directing commodities to storage or ship, major storage and reclaim facilities, a second shiploader and a second deep-sea berth. In addition to the facilities for overseas shipments, a wood-chip loader and a barge berth have been installed for domestic shipments of wood chips and, occasionally, for other local commodities. The design considerations imposed by the nature of the products to be handled are covered in this paper."
