"Broadmeadow Mine is an underground longwall mine located in Central Queensland, Australia. The mine introduced a top coal caving (TCC) longwall face in 2013 but since then has experienced severe convergence events at the start of each panel after 60–70m of retreat, resulting in equipment damage and the longwall almost becoming iron bound. A significant geotechnical monitoring effort during LW10 and LW11 to characterise the caving mechanics and operational changes during LW11 were successful in mitigating the risk posed by these initial convergence events. This paper describes the geotechnical monitoring programme undertaken during LW11, the operational methods used to mitigate these initial convergence events, and the contribution to these events of the TCC technique.A thick sandstone unit close to the extraction roof was likely the cause of the convergence events at the start of each longwall panel. However, stress change monitoring cells, surface extensometers, an inclinometer, and the blockage of holes designed for pre-conditioning using hydraulic fracturing indicated that the convergence events were not caused by the sudden collapse of sandstone bridging across the goaf. Rather, they were caused by the propping action of strong units within the overburden strata. Equipment design modifications were made to allow the longwall to remain operational during the convergence events and an operational strategy was implemented to mitigate convergence in LW11. The initial convergence event was successfully negotiated, and Broadmeadow went on to be the highest producing longwall for CY16 in Australia.INTRODUCTIONBroadmeadow Mine is an underground longwall mine located in Central Queensland, Australia, owned and operated by BHP Mitsubishi Alliance (BMA). The mine is a punch longwall mine using open cut pit access provided by Goonyella/Riverside Mine. Broadmeadow commenced longwall operations in 2005 using a conventional high reach longwall, but later changed to the top coal caving (TCC) method of extraction. Broadmeadow extracts the easterly dipping 5.5–7.5m-thick Goonyella Middle Seam in ranging in depth from 90m to 330m. The coal is washed and blended at the adjacent Goonyella/Riverside open cut mine to produce a premium coking coal product for export."

In this study, the "Mine Project Risk Management" methodology, MPRM (Botin et al. 2011), has been applied to characterize the sources of uncertainty and to quantify risk in the mining engineering stages of Block Caving mining method. As the result, rock mass caveability and ore fragmentation were identified as the two most relevant parameters of this process and a methodological model has been developed to quantify the risk associated to each of these two parameters. Furthermore, the applicability of the proposed model has been validated using as a case study the Chuquicamata Underground Mine Project in Chile.

Technical & Procurement Services Limited (TECHPRO) in Ashford, Kent, UK, have been appointed as Project Managers for the major rehabilitation of the Copper Smelting complex of Nkana Division, Zambia Consolidated Copper Mines Limited, (ZCCM) Zambia. In outline, the overall scope covers three main areas: a) Transfer and implementation of new technology into Zambia for the production of primary copper. b) Replacement of gas collection and gas treatment circuits to guarantee high quality feed to the acid plants. c) Completion of a number of minor projects in the copper smelter and associated utility plants to complement changes in the primary smelting technique. This paper describes the Project Management methodology and style used by TECHPRO to tackle typical problems that arise in retrofit projects of this type. Reference is made specific aspects of the smelter projects to illustrate solutions to the problems. The subjects. addressed highlight the complexities not only of having a -Client 5,000 miles from the Project Office, but also the difficulty of co-ordinating and controlling the performance of min sub-contractors who are also geographically distant.

Due to limited available level ground, Nkomati Nickel Mine cut a weathered rock slope at the base of a mountain spur in order to create a platform for construction of the primary crusher plant and run-of-mine stockpiles. As space is limited around the mining area, ore processing at Nkomati is based on a high reliability of flow of material through the crusher plant, with minimal usage of other and larger designed stockpiles. Evidently, any crusher plant shutdown will render the mine as a whole unproductive and put excessive strain on the medium- to long-term large ore stockpiles, the deposition rates for which are restricted by founding material consolidation requirements. At the onset of the 2012 rainy season, movement was identified on the slope monitoring system and cracks developed on the slope. After a minor failure on the crusher slope an assessment of the slope stability was conducted and a slope management plan recommended, which included deployment of real-time monitoring. An evaluation of the conditions leading to instability was conducted and the likely causes for the failure identified. A full evaluation of the slope monitoring, rainfall, and mining conditions was undertaken and movement triggers were determined. This paper describes the events leading to the development of the failure and the evaluation of the monitoring data to determine a management plan for the failure that allowed for minimal shutdowns of the primary crusher.

The difficult and often harsh working environment of the mining industry can stress both personnel and equipment to the extreme. Often, the unintended consequences of this environment lead to confrontations between departments, as each goes about its tasks of mining, maintaining or supporting these activities. On occasion, the confrontations may result in criticism of other departments for perceived or real sins or omissions. The offended department may not be able to change the circumstances, but can only complain while frustration persists. The most effective solutions are the actions of the mine manager. By establishing mutually supporting roles of each department and specifying conditions for the interactive conduct of maintenance, the mine manager can better assure that the inherent problems of the mining environment are neutralized.

In today's highly competitive worldwide market, it is becoming extremely important for managers to make sure that all resources are used where needed the most. That way, all resources are generating added value, process control equipments are maintained in an optimal state for best performance, and final product is produced at least cost. Performance monitoring is the only way to ensure that everyone is working on top priorities toward the same goal. How can we add value to these warehouses of data? The data is there and the information is hidden; it is just waiting to be extracted. A process performance monitoring system is able to determine if a loop, a system, a unit, or a plant meets the expected performances. The system prioritizes the areas that need attention. It also provides historical reports of the plant status by areas, by loops, and by identified problems. This information is provided to different categories of users in an appropriate format for each one. The system also includes diagnostics and tools to detect and analyze problems.

"The Copper Mountain Mine is located in Princeton British Columbia, Canada and produces 80 – 90 million pounds of copper annually, as well as gold and silver by-products. The ore processing plant has a conventional SAG/ball mill circuit with inline pebble crushing, followed by flotation. At most plants, much time is spent evaluating and optimizing the comminution and flotation circuits. The cyclones are a key contributor to the effective operation of the plant: They define the circulating load in the comminution circuit and provide the classification that ensures liberated material is passed through to flotation. If the cyclones are not performing as they should, the results are often a combination of lost recovery, grade, capacity and reduced throughput in flotation. For such an impactful unit operation, cyclones have historically been under-instrumented. Consider as well that the cyclones are typically located at the highest elevation in a plant, and it becomes clear why they often underperform. Copper Mountain Mine recognized the critical role that cyclones filled in their plant and worked to optimize their operation through the implementation of a Portage Cyclone Detection (PCD) system to identify plugging and roping incidents, Portage Pumpbox Control (PPC) to manage the flow to the cyclones, and AwaRE to control the grinding circuit. As a result, cyclones overflow stability has increased, switching out a roped or plugged cyclone now happens within seconds rather than many minutes, and both the grinding and flotation circuits perform better. For such an impactful unit operation, cyclones have historically been under-instrumented in an otherwise highly instrumented process plant. Traditionally, mill operators have relied on visual observations of the apex discharge to determine the operating state of the cyclones. Visual cues corresponding to the different cyclone operating conditions have been discussed by Neesse et al. (2004) as shown in Fig. 1."

"Most engineers are bright, hard-working, reliable, and prefer to avoid conflict. An engineering curriculum tends to self-select these characteristics. By most standards, you would expect workers exhibiting these traits to require minimal supervision. But is this true? Is this how most current engineering managers lead? Looking at some current theories on leadership combined with personal anecdotes, this presentation will look at some common misconceptions about leading engineers. INTRODUCTION Engineers are different. There, I said it. From cartoons to movies to television shows and more, there is a reason stereotypes of engineers resonate. Yes, you can find a lot individual variation among specific engineers that you may know, but they do not detract from some of the overarching commonalities that engineers are known for. CLASH OF CULTURES What distinguishes a professional? Raelin (1985), in his book Clash of Cultures, describes professionals as having superior intellectual training, maintaining their own standards of excellence, and being supported by associations that maintain the quality of the profession. Engineers obviously fit all three of these ideas: an engineering degree is still regarded as one of the most difficult to attain; we maintain our own standards both through peer-review and in academia through ABET accreditation; and through professional associations like SME to promote the discipline and foster technical growth. Six characteristics representing professional status are described by Raelin (1985, p. 9): Expertise—prolonged specialized training in a body of abstract knowledge. Autonomy—the freedom to choose the means to solving a problem. Commitment—Primary interest is in pursuing the practice of one’s own chosen specialty. Identification—identifying with the profession and with fellow professionals, both through formal associations and through peers external to the organization. Ethics—providing service without concern for oneself or without becoming emotionally involved with the client. Standards—committed to help in policing the conduct of fellow professionals. Engineers are smart—in the vast majority of cases they are hired specifically for their expertise. At a typical mine the ventilation engineer knows more about mine ventilation than anyone else on site; the same is true for the rock mechanics engineer, the explosives engineer, the planning engineer who uses sophisticated computer software—each is the expert for their particular area at the mine, that is why they have that responsibility. This can present a dilemma for the manager for each (sometimes all) of these engineers. The manager needs to keep everyone on task and focused on the objectives of the organization. Because they have reached their position by following the norms and expectations of the organization, managers tend to have a certain amount of loyalty to that organization. Engineers, on the other hand, tend to have mixed loyalties, having as much loyalty and affinity for their profession as they do for the particular company they happen to be working for at any given time. This dynamic can often lead to power struggles between the managers, based on their positional authority in the organization, and the engineers, based on their specialized knowledge (Camm, 2013)."

Springvale Mine is an underground coal mine in the western coalfield of New South Wales which utilises retreat longwall mining technique. The mine encountered significant gateroad floor heave issues during retreat of LW415. Floor heave has subsequently impeded longwall advance as well as longwall bolt up and recovery to some extent during the extraction of the next five longwalls. Floor heave at Springvale Mine develops throughout the lifecycle of the mine roadways, and is most prevalent in longwall gateroads. Floor heave has been measured as early as 2 weeks after roadway development and continues to develop through the post-development phase, with a rapid increase in close proximity to the longwall face. This paper presents a case study of the evolution of understanding of floor heave behaviour at Springvale Mine based on geotechnical characterisation, an intensive convergence monitoring program (roof and floor movement) and spatial correlation of monitored locations with longwall face position. The analysis considers previously known factors contributing to floor heave including mining depth, geological and physical properties of the rock mass and mine design geometry. Investigation results indicate a strong correlation between floor heave magnitude and roof dilation magnitude, geophysically inferred faulting in underlying strata, areas of localised stress concentration and mine roadway dimensions. This paper also discusses the predictive model which was developed as a result of the study, which is now the basis of the floor heave removal program used to manage roadway convergence at the mine. Furnace run-outs can cause extended furnace downtime for repair, while leaks or more severe failures in the cooling system can lead to brick hydration or in severe cases, steam explosions. These incidents can result in lost production, damage to equipment, injuries and casualties. To prevent such incidents, reliable and accurate techniques to monitor the critical components of the furnace such as refractory lining, furnace structure, taphole integrity and main water circuits are required. This paper describes several advanced methods that have been developed to assess the condition of furnace integrity, refractory lining, tapholes and cooling water circuits.

The Beta Hunt orebody is situated on the south western limb of the Kambalda dome. It is one of the nickel occurrences in the area which fall into the category of æKambalda Style OrebodiesÆ. These orebodies are renowned as being continuous uniformly mineralised ribbons of nickel sulfide mineralisation. Interestingly, Beta Hunt does not conform to this notion of continuity and uniformity and is in fact highly irregular in its ore distribution due largely to the influence of post depositional structure. In an attempt to understand the ore distribution at delineation stage rather than development stage a detailed footwall topography model is created using all available drilling data. To augment this drilling data, the routine use of downhole EM and borehole radar has been introduced to provide data beyond the confines of the drill core to enhance the geological interpretation of the footwall topography and the distribution of the mineralisation. The implementation of a detailed footwall model and the use of downhole geophysics has proved to be very effective in resolving the orebody complexity issues. Ongoing development of the methods used will continue to refine the interpretation of the geology to enable more efficient extraction of the orebody.

Corrosion affects the performance of rock support and reinforcement at various underground mines, including the George Fisher Mine, where recent studies showed that this risk to mining operations is generally not well understood. Corrosion reduces the capacity and life expectancy of ground support and generally is undetected unless the traditional destructive pull out test is employed to detect defect bolts. Where it is still acknowledged that the pull out test has still an important role to play in determining critical bond lengths for static and quasi static ground support designs, it does not provide an underground operation with any reassurance regarding its bolt's integrity, which has been compromised by in situ aggressive conditions. Non-destructive rock reinforcement calibration testing on single and twin strand cable bolts, rebar bolts and friction bolts at the George Fisher Mine enabled the testing team to detect bolts with compromised integrity in one area of the mine. The non-destructive rock reinforcement integrity testing conducted uses a complex 'Stress Wave Analysis' package based on the processing of clear seismic signals imparted into the rock reinforcement element that is being tested. The seismic signals are processed by 'Fourier Transform' into various criteria which can be used to produce models of the element such as mechanical admittance, frequency spectra and velocity which are all being used in the final modelling of the rock reinforcement element under analysis. This paper highlights the enormous potential to effectively manage the geotechnical risk that corrosion presents for an underground mining operation.

As national and international health and safety management system (HSMS) standards are voluntarily accepted or regulated into practice, organizations are making an effort to modify and integrate strategic elements of a connected management system into their daily risk management practices. In high-risk industries such as mining, that effort takes on added importance. The mining industry has long recognized the importance of a more integrated approach to recognizing and responding to sitespecific risks, encouraging the adoption of a risk-based management framework. Recently, the U.S. National Mining Association led the development of an industry-specific HSMS built on the strategic frameworks of ANSI: Z10, OHSAS 18001, The American Chemistry Council’s Responsible Care, and ILO-OSH 2001. All of these standards provide strategic guidance and focus on how to incorporate a plan-do-check-act cycle into the identification, management and evaluation of worksite risks. This paper details an exploratory study into whether practices associated with executing a risk-based management framework are visible through the actions of an organization’s site-level management of health and safety risks. The results of this study show ways that site-level leaders manage day-to-day risk at their operations that can be characterized according to practices associated with a risk-based management framework. Having tangible operational examples of day-to-day risk management can serve as a starting point for evaluating field-level risk assessment efforts and their alignment to overall company efforts at effective risk mitigation through a HSMS or other processes.

Didipio gold-copper mine operation is situated in the island of Luzon, approximately 270 km north of Manila in the Philippines, in an area of high seasonal rainfall. The hydrogeological water flow model implied that there would be high connectivity between regional structures and underground operations. This model predicted that underground mine groundwater inflows were expected to quickly rise as the decline progressed to a peak of 510 L/s. Designing a capital pump station to deal with 510 L/s has its own challenges, however developing the decline down to the required level, always dealing with 300 L/s of water and constructing the pump station is extremely challenging and required a robust water management plan and mine design. When dealing with expected high volumes of water, underground mine design and scheduling plays a key role in protecting electrical infrastructure, equipment, secondary pumps, and the decline from flooding. A good drainage system and controlled dewatering plan is essential for safety and progress. This paper details the dewatering techniques and mine designs required to accomplish decline development with high water inflows as used in the Didipio project. It will describe the learnings and outcomes that are achievable with a flexible design, as new structures are mapped, and water intersections are increased. It also highlights the potential for adaptability in mine planning to provide evolving improvements or to evade problems that are expected from the constant reconciliation of data recorded during development.

Although much talked about, social licence to operate (SLTO) is a relatively new concept that has been deemed as one of the four biggest business risks to a mining project over the life-of-mine. There is no accepted single definition of SLTO and this author understands SLTO to be stakeholder perception of the legitimacy of a project, a company or an industry. At a specific site, and across different sites within the same organisation, SLTO ranges through different levels according to stakeholders’ degrees of acceptance in time and space. Management of SLTO is vital for the mining sector but is difficult as it is informal and intangible. Although there are no key performance indicators, there are certain indicators that enable relative measurement of the ‘level’ of the SLTO. Because of the importance of the SLTO, information management during the entire life-of-mine, from exploration to rehabilitation, becomes crucial for the continued operation of a project. Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

The West Australian Government hold an annual ‘Mines Safety Roadshow’ around the State’s regions and the capital city, Perth. Each year presenters provide an update on legislation and safetyrelated themes. A recurring theme is the mining industry fatality rate. Professor Michael Quinlan’s ‘Ten Pathways to Death and Disaster’ (Quinlan, 2014) has been referenced regularly to assist in structuring workshops and conversations concerning fatality prevention in the mining industry. The ‘Ten Pathways’ are the outcome of an analysis of fatal injuries in the mining industry, which identified a series of common themes to the fatalities: Pathway 1: Design, engineering, technical and maintenance flaws. Pathway 2: Prior warnings or causes for alarm ignored. Pathway 3: Failures in risk assessment. Pathway 4: Failures in management systems and hazard management plans. Pathway 5: Failures in auditing. Pathway 6: Economic pressures compromising safety. Pathway 7: Failures in regulatory oversight and inspection. Pathway 8: Worker and others expressing concern prior to the incident. Pathway 9: Poor management – worker communication and trust. Pathway 10: Emergency and rescue resources and procedures. To determine a fatality risk profile we asked participants at the Mines Safety Roadshows to indicate their perception of their employer’s performance in each of the Ten Pathways. We administered the questionnaire at the Mines Safety Roadshows in 2017, 2018, 2019 and 2023. In this paper, we report on the results of the questionnaires, and suggest how organisations can improve their fatality risk score.

This work has addressed the problem of creating a predictive model of the maximum particle velocities in the rock mass (PPV). The model designed specifically can model the subsequent vibrations of blasting based on geometric parameters, explosive loading, and the blasting process itself. Models based on artificial intelligence (AI) algorithms can find useful non-linear patterns to solve classification, regression, clustering, among other problems. They are flexible mathematical transformations adaptable to various objectives and types of data. With their different levels of abstraction, they can find a function that relates the input variables to the sought-after response. These models have been successfully used in similar contexts (Prashanth, (2018)) and have achieved state-of-the-art results in the field (Lawal, (2021)). The main challenge is to adapt them to the context of Chilean mines, in addition to including the geometric information of the blasting and the heuristic to obtain the set of optimal parameters, it is proposed to follow the work of (Simonyan, (2013)) and obtain "saliency maps" adapting it to tabular variables. We implemented an Artificial Neural Network (ANN) with a large number of neurons in the hidden layers. Models based on neural networks can find useful non-linear patterns to solve classification, regression, clustering, among other problems. They are flexible mathematical transformations adaptable to various objectives and types of data. With their different levels of abstraction, they can find a function that relates the input variables to the sought-after response. The literature shows that, for the problem of rock type identification, basic neural networks perform well (Simonyan, (2013)), but lack uncertainty estimates. This is a data-driven model, and in this sense, the model training was carried out using data from multiple operations in the large copper mining industry in Chile. In these databases, data consistency and the balance of the various independent variables that have the greatest impact on the model were ensured. With the above, the optimal hyperparameters of the model were determined, obtaining a predictive R-squared of 0.94.
