Risk Management has been applied relatively slowly in the mining industry. Except for the risk concept application in financial considerations, the application of Risk Management in safety management has been far from satisfactory. When applied it was treated in a global occupational safety and health sense. Regardless of how much the mining industry has progressed over the years it is still, in many respects, `state-of-the-art' business. The safety aspects of mining are well known and yet little understood by the public. Regardless of what has been achieved so far and how much effort has been put into it, mining is regarded as an unsafe and somehow rude business. Safety records of the mining industry vary from State-to-State. Latest mining related disasters in PNG and Queensland and some other fatal accidents support this public perception. They purport a need for change in managing safety in the mining industry. The latest mining fatalities have raised the issue: What must be done to improve safety standards? Duty of care, risk management and system engineering concepts can provide some answers. Risk Management principles which have been advocated for some time provide many answers and explanations and can make safety management more constructive and workable. What has been missing so far is that this concept has neither been applied nor has been modified for its practical application and purpose on a larger scale. A risk management approach to the management of safety and health in the mining industry is addressed, herein. This approach does not attempt to provide the solution. It does, however, provide a basic understanding of what Risk and Risk Management are all about. Rational management of risk is addressed in the life cycle and system contexts. A system of continuous improvement is applied. There is always a chain of events, outcome, exposure, consequence(s), and consequence(s) value(s). It means that development of an event takes place in time and space. Dynamic circumstances have been created in or within the system. It is a decision-making process designed to provide a methodology which is directed not toward a mathematical model but toward easy use and application in dynamic working environment, by decision-makers who can be anybody from either a risk taker or a risk creator. The emphasis is placed not only on arriving at the best practical, possible and fair decision but also on structured and systematic acquisition of knowledge about the system, its hazards and associated risks, hence increasing the decision maker's ability to manage the decision-making process related to the management of risk.

"The phased application of statistical process control (SPC) infers that processes are classifiable as pre-SPC (or phase zero), phase I and phase II processes, with different objectives being pursued in the different phases. Phase zero processes, defined as processes affected by assignable causes of variation, are characterised as follows: out-of-control with unknown autocorrelation structure having an unknown statistical distribution not performing consistently on target with minimum variability potentially incapable of conforming to the product specifications. The objectives of phase I SPC is to obtain estimates of the mean (µ), variance (s2), probability density function (f(X)) and the autocorrelation structure, as well as transforming the process into a phase II process (defined as a process conforming to specifications and performing on target with minimum variability). As in any other type of research or pragmatic improvement intervention, sampling is required is to achieve these objectives. The challenge of deciding a phase I sampling plan is multifarious since it has to cope with the aforementioned effects of assignable causes and provide for rigour estimates of the unknown parameters. Furthermore, deciding sampling plans in phase I invariably presents a classical catch-22 situation: appropriate sampling is required to determine the autocorrelation structure of a variable, but knowledge of the autocorrelation structure is required to formulate an appropriate sampling plan. This paper suggests, for random variables, guidelines to formulate sampling plans, and presents a methodology to estimate the inherent process parameters of process variables and an approach to identify assignable causes in phase I. CITATION:Breytenbach, W P, 2014. Indications are that underground mining is gaining favour with large and small mining companies due to environmental and sustainability concerns. Dramatic changes to mine fill technologies will be needed in order to keep pace with these emerging trends. The industry will be compelled to gravitate toward zero discharge mining operations. Innovative handling of mine tailings will become a focal point in this effort. The challenges are to use a greater portion of the residues and to manufacture a fill with better properties. This is a tall order that can only be addressed through a systematic research and implementation process involving mining companies, universities, research organizations, manufacturers and governments. The mining industry has to be at the forefront of this technology push in mine fills for the future mines. It will also be called upon to lead the way in other research areas. The industry will have to partner with other technology providers: universities, government laboratories and consultants.

The Raub Gold Mines are situated approximately in the centre of the Malayan Peninsula in the State of Penang. Part of the area, now known as "Raubhole," was opencut some 800 years ago by the Mon Khmers, a Cambodian people, whose vestiges mark numerous outcrops in South Eastern Asia. With their decline and extinction the work was abondoned, leaving a large pond in a dense thicket of Beluka (secondary growth).From the year 1885 onwards the history of Raubhole and the water therein records a chain sale starting from an enterprising local "Rajah," who had "assumed" authority, to an equally wakeful Singapore Arab; thence through the agency of a "down-and-out" beachcomber to a hopeful Australian company in the "goldbrick" days. Subsequent catastrophe on a depleted stringer was followed by reconstruction; the deserved reward of a long mining life, and an output of nearly 800,000 ounces of gold. Owing to the loss of notes and data stored in Singapore, this paper is not as complete as it should be, especially in respect to its metallurgical phases. Apologies are therefore tendered in advance for any deficiencies which may be noted.The necessary regional geological data have been taken largely from the admirable investigation of the Raub area by J. A. Richardson, A.R.C.S., B.Sc., F.G.S., of the Malayan Geological Survey*, and, throughout the paper, months represent lunar periods of 28 days and money is expressed in sterling.MINING SECTIONSAs shown hereunder the reef system has been divided by nature into seven isolated sections, which have been of necessity worked as seven separate small mines. La primera, estriba en mantener el ritmo de producción e incrementar la ley de cabeza. Mientras que la segunda, refiere el cierre total de la minería convencional para enfocarse a la franja mesomática tipo skarn de la sección Catuva, utilizando métodos altamente mecanizados que incrementan la producción paulatinamente. En este sentido, se optó por la segunda alternativa puesto que el potencial minero en Raura proyecta a mayores niveles de producción y las reservas actuales se corresponden con la amortización de la deuda. Asi también se describe las características geológicas y locales del distrito minera de Raura, producción histórica, potencial minero, reservas económicas y se analiza los métodos de minado de tajo abierto y mina subterránea como los resultados de dicha valuación económica.

The Ravensthorpe Nickel Project (RNP) flow sheet provides for the upgrading of Ni & Co in a size based beneficiation process prior to leaching. The upgrading effect on Ni and Co makes the beneticiation process a powerful addition to the flow sheet. Multiple linear regression (MLR) analysis based on 351 individual batch tests, 52 large scale composites and 21 pilot plant tests has been used to successfully model beneticiation product grade, recovery, upgrade and mass recovery. Statistically, 83% and 96% of the variability in nickel recovery and nickel product grade respectively was accounted for. Algorithms have been developed for Ni, Co, Mg, Al and Fe, which represents the revenue and operating cost factors for the process. The algorithms have subsequently been applied to the resource model, allowing prediction of elemental grade and recovery for the development of the mine schedules based upon these parameters and not head grades alone. The decade of the 1980's saw "advanced materials" become a key factor among the industrialized nations, with the United States, Japan, and the European Economic Community being the major competitors. All three have identified communications technology, advanced materials, and biotechnology as the keys to future industrial leadership and the closing decade of the 20th Century is expected to be an era of rapid development and restructuring. The progress of EC'92, the effort to develop uniform standards and remove trade barriers in Europe; the determination of Japan to commercialize advanced structural ceramics, alloys, composites, and superconductors; and the emergence of less developed countries as major supplies of commodity materials pose enormous challenges to the position of the United States as an industrial and financial leader. A consensus is developing that the industries that are first to commercialize the large array of new structural materials and electronic, optical, and magnetic materials and devices will be those who determine the structure of national and world economics. Many technical, economic, legal, political, social, and cultural factors are important determinants in the race to commercialize new materials. Millions of words have been written about these factors and their interdependence; yet the sheer complexity of the problems, the rate at which changes are taking place, and the economic risks involved are so great that government policy makers and industrial decision makers find adequate understanding to be very difficult. Of the many factors involved in advanced materials, the Bureau of Mines has the statutory responsibility to provide information on where materials can be found and how they are mined, extracted, purified, combined, and recycled. It is obvious that minerals availability is essential to implementation of any new technology just as there can be no commodity metals industry if the ores are not available. Because "minerals availability" for commercialization of advanced materials and devices is more complex than for commodity materials and a new mix of materials and processes is emerging, the Information and Analysis Directorate of the Bureau of Mines is redirecting resources to provide reliable information for policy and decision making. The Office of Advanced Materials Coordination (OAMC) was established as a staff operation in July 1987 to assist the efforts of the divisions and branches. The key issues and problems were defined and the role of the Bureau was outlined (Sorrell, 1987, 1988). In February 1990, the functions of OAMC were transferred to the new Branch of Advanced Materials, a line organization parallel with the Branches of Ferrous, Nonferrous, and Industrial Minerals, and was given the responsibility for developing new methods to acquire, analyze, and disseminate information about new materials. The purpose of this paper is to provide definitions, a summary of major issues and problems, an overview of minerals and processing availability problems, and a perspective of the ways in which the mining and materials industries can help to ensure domestic availability of raw and processed materials.

PREPARATION CONTROL IN UNDERGROUND MINING Selective Mining According to district Depending upon the particular geographic location or seam characteristics modem raw coal preparation practices can be used to obtain improved product quality and uniformity. Nevertheless, certain coal markets may require only marginal quality while others may require very high quality and these objectives may be sought by commencing the preparation of coal underground at the "working face." Underground preparation involves choosing those portions of the bed which can be cleaned to satisfy predetermined market requirements involving (1 ) maintenance of uniformity in chemical quality, (2) elimination of removable impurities, (3) control of sizes and size ratios and (4) control of moisture. The methods employed to attain these objectives vary widely with the (1) seam under development, (2) mining equipment available and (3) changing trends in the consuming markets. When a mining property is opened and development progresses from one section to another the (1) petrography of the coal in the bed does not re- main uniform, (2) character of roof and floor are subject to change and (3) character and prevalence of mineral impurities are subject to considerable variation. The quality of coal shipped to market from any mine depends upon intelligent foresight, planning, execution and supervision. Mining becomes a highly technical art. The quality of coal as determined by chemical and petrographic analysis is without question one of the most important considerations in the development and life of any coal-mining property. Variations in such proper- ties as volatile matter, ash, sulfur, Btu content and petrography are of considerable interest and to a limited extent so are such properties as phosphorous content, free swelling index, plasticity, hardness, pressure formation during coking and ash composition. The maximum and minimum specifications of these characteristics as established by competitive market conditions place definite limits on the marketability of a coal. The prospecting and evaluation of any area containing one or more salable coal beds by means of channel samples taken from outcrop openings and/or from diamond-drill cores can yield valuable data on coal characteristics. By correlating the data developed from distinct petrographic sections of channel and/or drill core samples, the quality of the bed according to stratification will be evident. Lithium raw materials usable with present technology consist of pegmatite ore and surficial evaporite brine. Those advantageously located now supply world markets. Materials of similar grades and magnitude, but less favorably located, will come into production as present sources are depleted and markets expand. Some of these potential supplies have been explored, and are reserves in the sense of known availability and quality. Reserves herein are of two classes, those proved by close drilling and assaying, and those of only probable status because their scale and grade are unsystematically indicated. Possible resources include similar materials whose bulk can only be inferred on indirect evidence. All figures apply to total materials in the ground. No allowance is made for losses in mining, evaporation, beneficiation, or chemical processing.

"In a technologically advanced era, where process control is accurately maintained within the smallest error margins, and accuracy and precision are continuously improved, sampling is still left to the lowest paid employee in the company without training him or her properly to understand the relevance and importance of the task. In this paper the objectives of sampling in a process control environment is discussed and an attempt is made to link it to the theory of sampling. Some of the practical problems associated with implementing theoretical sampling principles in an industrial environment will be elucidated and some examples given on how compromises are made. Different methods of sampling will be discussed with real life examples from different manufacturing industries."

This paper demonstrates experimental studies on bioprocessing of different waste streams. In Part I of this paper three tailings from mining industry were bioleached with mixed acidophilic culture in order to solubilize valuable metals, like copper, nickel, zinc and manganese to leaching liquor. Bioleaching resulted in > 70 %, > 85 %, 100 % and > 70 % leaching yields for Cu, Ni, Zn and Mn, respectively. However, contaminants like arsenic and chromium were not leached efficiently and they will cause challenges in residual solids and sometimes also in metal recovery from leaching liquor. Bioleaching resulted in 99 % leaching yield for copper. Part II demonstrates mine water treatment and also gold recovery from process liquids with biosorption method. The novelty of biosorption was finding that certain fungal species were able to be cultivated to self-immobilized systems, which are easier to operate than traditional suspended biomass methods. Biosorption resulted in more than 90 % removal for all contaminants, but was also seen to be gold selective.

At 9.45 pm on Thursday, 12 July 1962 glass began to flow in an endless stream from a massive melting tank in a new Whangarei factory and a vital new industry was born û right on schedule. It wasn't good glass. It was ridged. It cracked. It rippled. It obstinately refused to emerge upstairs in the cutting room in one solid continuous sheet. But all through the night, drawing speeds were adjusted, temperatures changed. And on Friday morning, at 8:30, it finally came through as sheet û not perfect as yet, but sheet glass all the same. The rest is only a matter of time. Unfortunately, the buoyant enthusiasm displayed by the founders of the sheet glass industry in New Zealand, and the technical expertise of their European consultants, failed to bring the glass quality up to commercially acceptable standards and within six months, the company was in receivership. Taking over the role of producing New Zealand's sheet glass requirements, New Zealand Window Glass Company was formed as a partnership between two companies with long experience in sheet glass manufacture û Pilkington Brothers (NZ) Limited and Australian Consolidated Industries Limited. This paper sets out to identify the raw material requirements and controls necessary for the successful manufacture of sheet glass, and the availability and supply of suitable materials for use at Whangarei.

THE composition and quality of finished steel depend upon selection and proportioning of the raw materials of the charge as well as on control of furnace practice. This chapter deals only with those raw materials of the basic open-hearth process which are important in determining the composition of the steel or slag. They are the metallic charge, oxygen-bearing materials, fluxes, and alloying and deoxidizing additions. Refractories, which may also affect the slag composition, are discussed in the previous chapter. Wide variations are possible in the type and composition of charged materials, but in all cases certain fundamental relationships among the elements of the charge must be maintained. Since the process consists essentially of refinement by oxidation of undesired elements, a balance must be established between the sources of oxygen and the elements in the charge that are subject to oxidation (see Chapters II and VII). For example, if it is desired to use a high ratio of blast-furnace hot metal to scrap, it is necessary to add an oxygen-bearing material such as ore or mill scale to eliminate the excess carbon and silicon that are present in the hot metal. An example of the opposite extreme would be the use of a metallic charge composed entirely of steel scrap. With this type of charge, it is common practice to substitute burnt lime for limestone as a flux and to charge some carbonaceous material such as coke to avoid an excess of oxygen in relation to the elements of the charge that are subject to oxidation. Elements such as copper and nickel, which are not subject to oxidation or other reactions resulting in their elimination, may be introduced into the charge only to the extent to which they are permissible in the finished steel. An example of adjustment of the charge for elements of this type would be found in the use of steel scrap high in copper. In this case, metallic components of the charge other than the scrap should below in copper, or a sufficiently high ratio of pig iron to scrap should be charged to dilute the copper from the scrap, so that the average copper content of the melt will be below the required maximum.

He traveled about 1,900 km (1,200miles) in a lifetime and resided in a home with an ice-box for food storage and oil or gas for lighting. He communicated by mail, telegraph and crude telephones with limited availability and range. By 2000, the average citizen’s life span was 77 years. He traveled an average of 19,000 km/a (12,000 miles/year) by automobile alone. He resided in a home with many electrical appliances, including refrigerators and electric lights. And he communicated almost instantaneously with any other part of the globe by several widely available means, including portable phones and e-mail.

The Building Products Company is Phoenix based and owned by Mission Clay products of California. Building Products is the only manufacturer of vitrified clay pipe in Arizona. The marketing area includes Arizona, Nevada, Utah, New Mexico, and California. Building Products, promoted by Arizona Public Service, was formed in 1970. Raw-materials prospecting was undertaken for 2 years, after which a $5-million plant was built. Initially, a satisfactory vitrified product could not be made with the raw materials then in hand. Subsequent prospecting and testing led to an acceptable raw-materials mix. Testing to upgrade the final product is a continuing process. The basic raw-materials supply must be adequate, secure, and capable of sustaining close tolerances in the final product. These needs are met by mining four geologic materials at three different localities: (1) refractory aluminous shales (two horizons-one pit) Colorado Plateau Province (Mogollon Rim); (2) less refractory aluminous materials from late Cenozoic lacustrine materials near Dewey in the Transition Zone (TZ); and (3) Precam¬brian "slate" from near New River along the southern edge of the TZ. Other additives include grog (ground-up, broken pipe) and barium carbonate that ties up what gypsum there is. Calcium magnesium carbonates are deleterious components that are minimized by careful selection of the mined products. The raw materials are blended and mixed, ground to 12 mesh, mixed in a pug mill, depleted of air, extruded into pipe ranging from 6 inches to 42 inches in diameter, transported to a hot-air drying room, fork-lifted to an appropriate kiln, and fired at a 1900-2000° F range. The "Rim" kaolinitic shales, being the most refractory ingredient, stabilize the pipe during the firing process. They are very plastic, and therefore facilitate extrusion. The Dewey clay fuses at a low temperature and forms an impervious glasslike binder. It also is plastic. The "slate" forms platy particles that tend to orient themselves during laminar flow. This provides strength for both the green and dried product. It doesn't absorb water, which helps the drying process. Grog remains stable during firing, and therefore helps to control shrinkage. The development of appropriate raw materials and proper mixtures has been done empirically.

The glass container industry is the largest producer of glass in the United States. As of this writing, there are approximately 89 active domestic production locations. In 1984, the industry shipped 11.8 million tons of glass containers. Approximately 70% of this production was clear or flint glass, 20% amber, 7% green, and the remainder miscellaneous colors. Although the production of nearly 12 million tons of glass is impressive, anyone following the industry over the last five years is aware that the industry has changed from a pattern of growth in the 1970's to one of maturity in the 1980's. Some observers, less generous, perhaps, have described the 1980's as a period of decline. In 1981, there were 119 production facilities which produced 13.2 million tons of glass the previous year. This equates to a 10% reduction in glass container production from 1980 to 1984, with a corresponding impact on the segment of the industrial minerals producers serving the container glass industry, as shown in Figures 1 and 2.

IRON ORE is widely distributed throughout the world. Ores sufficiently high in iron content to be practical for the operations of iron and steel making occur in so many places that it is only by the accident of political subdivisions that any given nation does not have adequate and suitable iron ore. Data prepared by various authorities show that in the Lake Superior region alone there are reserves of 1,200,000,000 tons of iron ore of an iron content and at the level of production cost to which the industry has been accus tomed. At an annual consumption of 80,000,000 tons these reserves will last fifteen years. Taken alone these figures would be alarming, but over a considerable period of years the total reserve tonnage has been more or less steadily increased by improvement and extension of mining and beneficiation methods. The rate of depletion of reserves has been only about one third of the rate of consumption. On this basis the cheap high-grade ores of Lake Superior would last 45 years.

IN COMMON with every other good citizen I long for conditions that will make perpetual peace a reality throughout the world. I have studied the statements of our public men relating to what we must do to bring this about after this war is over. In most of these I find one common error. They always build their plans on the unstated, subconsciously accepted premise that there can be such a thing as an ideal nation whose citizens are possessed of infinite wisdom and actuated only by motives that are above reproach. This heavenly vision fails to correspond with the facts. We wish it could be true but we are forced sadly to admit that the human beings that will live here after this war is over are going to be much like the fallible race that has occupied our planet for the past centuries.
