Most mine ventilation engineers are involved in ventilation planning and design in some capacity. Ventilation modelling software used by a competent experienced ventilation engineer is extremely useful in developing good ventilation designs by allowing assessment of a wide range of potential options. Unfortunately, ventilation modelling undertaken by persons who do not have sufficient experience or knowledge usually results in a façade that covers up a fundamentally unsatisfactory design. Ventilation models are produced for a wide variety of purposes including: fault-finding of a problem area in a mine and options analysis for resolution of such problems; a complete review or optimisation exercise of an entire mine’s ventilation system; or a much longer life-of-mine type of study. Models can be intended as the basis for studies with diverse purposes such as primary shaft or airway sizing, primary fan specification, examining re-entry times after blasting, resolving leakage or recirculation, or investigating the impact of fires or underground climate or cooling requirements. The concepts of ‘materiality’ and ‘fitness for purpose’ are essential to developing or using a ventilation model and serious mistakes have been made in wrongly using a ventilation model for purposes for which it was never intended, often because it is simply the most recent model on the mine site. Mistakes at this level often translate into faulty ventilation strategies and inefficient or ineffective use of scarce capital for ventilation projects. Validating a ventilation model is a time-consuming and expensive process and not every model must (or even should) be fully validated to meet the objectives at that time. This paper discusses the application of quality assurance in ventilation planning with particular respect to the ‘basis of design’ (BOD) as well as the standards for validating a ventilation model. It also provides a recommended way of dealing with non-conformances in measured versus modelled values of critical parameters in the model.CITATION:Brake, D J, 2015. The engineering company's participation in quality assurance in the mining industry is twofold. Not only must it have such a program to ensure the necessary high standards for its own design and construction of a minerals processing plant, but it is also responsible as the "engineer" for both the selection and the quality of purchased equipment which goes into that plant. It is in this latter role that the engineer finds the most difficulty, or controversy, since while it is relatively easy to control his own performance, this is not ncessarily so when dealing with companies, all with their own ideas or standards. The engineer has perhaps rather reluctantly become more involved with the quality assurance of purchased equipment. This is because he has been used to writing performance type specifications and expecting the equipment manufacturer to provide the degree of quality, and control of that quality, for the product to meet the specified operating conditions. However, this has sometimes placed the manufacturer in a difficult position since to remain competitive in today's market conditions, he cannot always insist on product improvements or options which may in the long run be vital to reliable operation of the equipment.

"Abstract-In recent years, several unsavory events in international mineral exploration, as well as several widely publicized project difficulties and failures have occurred. The report Setting New Standards has "raised the bar" and proclaimed a "commitment to continuous improvement... and change." We need to evolve from our "generally accepted" practices to more effective quality assurance policies, guidelines and standards. Several steps will be required to support the work of the "qualified persons" now wholly entrusted with technical and professional matters:• adopt a mineral development model and objectives appropriate to orient the selection of workmethods and verification procedures suitable to increase the profitability of these activities;• review the definitions and implications of the terms practice, guideline and standard;• review three quality assurance systems with emphasis on commitment of company management and complementary emphasis on systematic quality policies, plans and manuals (ISO 9000), continuous improvement through "profound knowledge" (Deming, 1986), and self-appraisal of continuous improvement and client satisfaction using the Qualimeter Guide (Québec Society for Quality);• determine, from the objectives of the mineral development model proposed and the problems often encountered, the requirements of resource/reserve estimation and the production feasibility study. To regain investors’ confidence and improve the returns from mineral development activities, an explicit commitment to quality by the board of directors and the management of every company is required.A number of mining companies have gained ISO 9002 certification for their operations but exploration companies should start by developing and implementing quality assurance guidelines and standards based on the ISO 9001 pattern, without achieving actual ISO certification in the short term. Both professional associations and industry organizations must be involved in the process of developing the required quality and continuous improvement guidelines and standards."

"Abstract - Resource modelling is a complex process involving different specialists with relevant experience using a multi-disciplinary approach and the best available technology and reviews by independent auditors. The reliability of the final resource estimate is highly dependent on the quality control exercised at each stage of the process. At each step in the resource modelling process it is necessary to define the specific objectives, the methodology proposed to achieve those objectives and to establish a set of checks and validation tools to assess the effectiveness of the proposed methodology. Designation of responsibility and authority for meeting these objectives must also be clearly identified. External audits must also be incorporated to review and validate the implementation of new procedures.Resource modelling is the basis for any economic appraisal of a mining project and includes a number of steps from data acquisition and validation to resource reporting, classification, and risk analysis."

This paper describes the general principles of the expander body with post-grouting (EBI) system, and how it is similar in nature to a large-scale pressuremeter device. The results from the installation of an EBI can be used to provide information about the stress-strain characteristics of the soil adjacent to the device and to compute estimates of nominal axial resistance. As the EBI expands during grouting, pressure is applied to the in-situ soil surrounding the device thereby compacting/densifying the soil. The nominal axial resistance for small diameter piles/ground anchors can be increased due to the greater soil stiffness and shear strength around the device, larger surface and end area, and improved soil-to-EBI interaction in side resistance. The shape of the response curves depends on the geotechnical conditions of the soil (e.g., in-situ stress state, strength, and stiffness) and the installation method of the piling/ground anchor. This technique has been used successfully with different pile types and ground anchors in a wide range of conditions from very loose sands/silts to very dense sands and medium-dense gravels as well as in soft to stiff clays. This paper also describes a mini case history in which 80 piles were installed to support an office building, and the results of the static axial compression load tests performed on a production FDP+EBI pile is also included.

The purpose of this paper is to inform members of the coal producing and consuming community about techniques used by The Carter Mining Company to control product quality at a large surface coal mining operation. During the early years of mining operations, coal quality received adequate attention but the Company's main emphasis was to build up its production rate and minimize mining costs. While these goals are still of prime importance, coal quality has received substantial attention in recent years. A Company guideline that no coal shipment shall exceed any contract or company-imposed coal quality specification has been successfully implemented. To implement this guideline, geology and coal quality modeling procedures were improved, in-pit drilling and blending programs were refined, and detailed shipment procedures were developed. Background Surface coal mining of subbituminous C rank coal in the Cillette. (Campbell County) Wyoming area has become increasingly competitive during recent years. Prior to 1974, one small surface mine produced coal for local consumption. During the last ten years, 13 new surface coal mines have been constructed. Coal production from Campbell County, Wyoming totalled 88 million short tons (79.8 million metric tons) in 1983, the majority being shipped to electric utilities in the central and southern United States via unit trains having a capacity of 10,000 short tons (9,072 metric tons).

"Following damage caused by the 2011 Christchurch earthquakes, the NZ$127.5million Christchurch Town Hall Restoration project is due for completion in 2018. The building is undergoing extensive repairs and a strengthening regime to meet New Zealand’s New Building Standard. One major aspect of the project is the stabilisation of the soil directly beneath the building using interconnected jetgrouting columns installed a cellular configuration, the purpose of this being to decrease the likelihood and severity of foundation movements caused by earthquake-induced soil liquefaction and lateral spreading. The soils encountered on the site ranged from cobbles and gravels through to sandy silts with a significant quantity of timber and rootlets. This wide variation of soil types presented a number of challenges with regard to effective and consistent treatment by jet grouting. The paper presents an overview of the quality assurance practices and also the testing regime adopted on this project.INTRODUCTIONMost of the deformation experienced by the Christchurch Town Hall structure over the course of the Canterbury earthquakes was caused by movement of the underlying ground. Two primary mechanisms of ground deformation were observed:- Liquefaction of the soils underlying the building foundation- Lateral spread of surface soils towards the free face at the river edge which runs 20 metres from the building. This mechanism also resulted in additional vertical settlement of the building foundations.The overall foundation design philosophy adopted by the project team for the Christchurch Town Hall Conservation Project has been to improve the ground directly beneath the structure to mitigate against these two mechanisms of ground movement. Additionally, a robust raft slab is proposed to distribute the foundation loading more evenly and increase the ability of the structure to resist lateral stretch of the underlying ground.The ground improvement system proposed by the Client’s geotechnical consultants, Tonkin &Taylor, comprised a cellular arrangement of interlocking jet grout columns, this arrangement would effectively encapsulate the soil within the cells preventing lateral spread.In total 1,097 interlocking jet grout columns, extending over the entire floor plan of the structure, were required to satisfy the design requirements."

Shotcrete usage in declines and developments has become pervasive as operators strive to achieve production improvements. Issues of mix control, fibreload, thickness control, temperature and curing conditions all play a part in the quality control. Often to overcome some of the quality control issues designers over specify the strength requirements in order to meet their operational constraints. Adherence to the standard civil testing requirements of 28 days moist cured at 21ºC without properly calibrating these results to the changing environment underground leads to contractual disputes. There are a number of standard tests specified including cylinder tests, cored tests, beam tests and round determinate panel (RDP) panel tests. The logistics of storing these samples and then transporting them to concrete testing laboratories without due regard to temperature changes with time can influence the results. All the samples can be readily handled by personnel except the RDP panel. The standard RDP panel is 800 mm diameter by 75 mm thick and requires an ITH machine at the mine and a fork lift at the laboratory to move it around. This paper will examine the possibility of a panel which provides scalable results to the standard RDP panel but can be easily handled by personnel and/or tested on site with suitable equipment. This allows for faster turn around and better quality control for the sample prior to testing. This enhances the identification of quality control issues at the mine and improves safety.

The Tom Price orebody, in the Hamersley Ranges of North West of Western Australia, is mined by Hamersley Iron Pty. Limited. The orebody is a folded sequence of hematite and shale bands capped with goethite, with a con- sequential variable iron content. The overall grade is below that required to yield spec- ification product, necessitating selective mining, 'Zone Plans' based on geological structure have been developed to yield forward information on material types and grades. Short and medium term mine plans - the initial stage of quality control - are based upon these 'zone plans'. Blasts are laid out to break to the grade boundaries, thus minimising dilution, and the samples taken from the blasthole cuttings provide blast grades. Blending of shovel out- put from different blasts forms the operational basis of grade control. Shales, being more friable than hematite, produce fines that are lower in iron grade than lump. Quality control in the mine centres around maintaining the target fines grade, compensation being applied in the mine if the fines grade lies outside the target range.

Mt. Whaleback, the property of the Mt. Newman Joint Venturers, contains more than one thousand million long tons of high grade hema- tite averaging 64% Fe, ranging from 70%, the grade of absolutely pure hematite, to 54%. The ore occurs in the Dales Gorge and Joffre Members of the Brockman Iron Formation. The interbedded shale bands common in other orebodies occurring in these horizons are seldom more than a few inches thick and are commonly partially enriched to about-35% Fe. In the generally hard, massive hematite orebody, the gangue minerals tend to break preferentially to the fines, or -6 mm product, and for this reason it is of a somewhat lower grade than the lumpy product. The control of the chemical qualities is concentrated on the fines product for it has been found that maintaining the iron and alumina levels in this product ensures that all chemical specifications are met in both the lump and fines products. Physical qualities are monitored to ensure that the physical quali- ties of decrepitation and degradation of the lumpy product are maintained at levels accept able to the buyers.

For many years practicing geotechnical and structural engineers in Texas and Louisiana were reluctant to include augered cast-in-place (ACIP) piles as a viable deep foundation alternate. Three primary concerns were the lack of a final driving resistance to confirm adequate capacity, the inability to inspect ACIP piles prior to installation, and the fact that pile integrity is highly dependent on the skill of the ACIP pile contractor's field personnel. The Deep Foundations Institute (DFI) Committee on Augered Cast-In-Place Piles has responded to these quality control concerns by establishing standards of practice for ACIP pile installation and ACIP pile inspection. Regional areas of the country have different subsurface soil and rock conditions and many times this will influence local installation and inspection requirements. The best way structural and geotechnical engineers can address quality control concerns is to be familiar with the DFI standards of practice, to know the special requirements for a particular project site, and to incorporate them into a well-written ACIP pile specification. The purposes of this paper are to: 1) review the main fundamentals of a successful quality control program, 2) describe how these fundamentals can be applied to the installation of ACIP piles, and 3) add personal experiences from ACIP pile projects in Texas and Louisiana. Nondestructive test methods are frequently used as a quality control tool to evaluate questionable piles. An additional brief discussion of two nondestructive test methods is presented to complete the paper.

The installation of augered cast-in-place (ACIP) piles requires careful quality control to assure a successful pile installation. Unlike steel, pre-stressed concrete or other types of driven piles, which are shop fabricated, delivered to the site and typically driven to a minimum tip resistance, augered cast-in-place piles are installed to a pre-determined tip elevation or to a refusal criteria. The structural section of the ACIP pile is constructed during pile installation and cannot be inspected prior to installation. Therefore, a carefully administered quality control program is essential to a pile foundation that will meet or exceed design requirements. This requires an experienced team consisting of the Geotechnical Engineer, Contractor and Inspector. The focus of this paper is to discuss the responsibilities and duties of the ACIP piling Inspector. Qualifications of the Inspector The qualifications of the inspector are related to the complexity and difficulty of the proposed project. Ideally, the Inspector should be an individual with a degree in civil engineering with special emphasis in geotechnical engineering. A properly trained technician with geotechnical engineering experience working under the direct supervision of a geotechnical engineer may be able to provide competent inspection on some projects. It is important that the Inspector have a good understanding of subsurface and ground water conditions and how they will affect pile installation. The Inspector must understand ACIP pile installation procedures and be able to communicate with the Contractor, Structural and Geotechnical Engineers; and maintain a detailed pile record for each installed pile.

ABSTRACT: Metals market improvements in 2005 drove the desire to increase nickel throughput and the production of nickel oxide sinter at the Vale Ltd. Copper Cliff smelter. Significant difficulties were encountered due to the unreliable quality of Bessemer matte being produced. Concentrate feed changes and the loss of technical and operational expertise during the previous market downturn contributed to the challenge of making the required improvements. Significant progress has been made in the converter aisle by focusing on employee engagement and standardizing best operating practices for a new generation of skimmers. This paper documents these initiatives and seeks to balance the art and science of converting.

In Novemeber of last year I visited a mica mine in Hunan Province, China. For nine months , we had been trying to sell Chinese mica to a customer in Belgium. Talking to the customer was like negotitating with a stone wall. The buyer refused to communicate any -thing other than his steadfast refusal to consider Chinese mica. He would not advise us what he wanted, what specifications or grades. He would give no indication if our price at market, or reasonable, or totally out to lunch. He would not even accept samples. It took no genius to guess that the mica buyer had at some time in the past bought a shipment of Chinese mica and had received a disaster. I invited the mica mine in Hunan in order to see for myself what could be done - without massive investment in equipment and instruments - to upgrade the quality of the mica for export. We had seen this type of customer reaction before. One disgruntled customer who had pur¬chased pigment grade barites direct from China ranted and raved about unreliable and inconsistent quality. It did no good to explain to him that China is a big country, with hundreds of mines scattered throughout many areas, each producing a different product. Our further explanation that Chinese export organizations also varied in degree and efficiency of customer service fell on deaf ears. Samples had no effect on his perception. Eventually we showed him photoraphs of two mountains of pigment grade barites. One lot was our sales of pigment barites to a major European minerals processor. Our Chinese barites stood proudly white and beautiful next to a hill of other material which now appeared at best mediocre and dull by comparison, and which the buyer had decided to reject after he received our Chinese barites. Finally the customer was sufficiently impressed to look seriously at our guaranteed specification and the outturn analysis performed by our other customer.

SINCE the deposits were found in 1927 in the Paradise Range of western Nevada, more than 1 mil- lion tons of magnesite and half a million tons of brucite have been mined and processed into a variety of granular refractory materials used as furnace linings by the steel industry. An additional million tons of magnesite were used during World War II to produce magnesium metal, and some magnesium oxide from the area is going into magnesia cements. Located near Gabbs, Nev., on the west flank of the Paradise Range, the principal bodies occupy an area a mile long by half a mile wide, and lesser occurrences have been found in a belt more than four miles long and in places almost two miles wide. Magnesite reserves containing less than 5 pct lime are estimated at more than 25 million tons. Reserves of better grade material are considerably less.

Quality assurance and quality control (or QAQC) plays a key role in safeguarding the level of confidence of the data generated during mineral exploration and mine development, and is also part of the reporting code requirements for resource estimation and reporting, as well as mining investment. Implementation of good policies and working protocols in quality management will result in the generation of reliable information, which is critical for decision-making on the future of the mining project. The use of certified reference materials, i.e., standard, blank, and duplicate samples, proves effective and efficient for ensuring the level of data confidence in terms of precision, accuracy, and sample contamination. This is well demonstrated by the practice of QC in the exploration programme for the chrysotile tailings storage facilities of Havelock Mine, using 1254 samples from 53 drill-holes. Different statistical tools were applied to analyse the assay results from these samples. The outcome substantiates that the QC practice established during the Havelock exploration project is aligned to best practice guidelines in the industry. The resource delineated indicates that the tailings material at Havelock could potentially become a significant source of supply to the global magnesium market.

The reliable quality assessment of grouting operations is one of the key issues in establishing impermeable grout curtains around shafts, drifts and tunnels. Reliable opera¬tional information on the decrease in the permeability of rock during the grouting process facilitates the assessment of the effectiveness of the completed curtain and assures production quality. Quality control and assurance also fa¬cilitates the early detection and elimination of possible de¬fects in the grout curtain. Thorough control of the grouting process provides the capability to evaluate decreases in the permeability of water-producing fracture zones prior to the beginning or the continuation of excavation of the shaft, tunnel or drift. In order to achieve this objective, post¬grouting hydrodynamic investigations are conducted prior to the initiation or re-initiation of work on the underground workings to measure the decrease in the permeability coef¬ficient of aquifers during grouting operations. 8.1 QUALITY ASSURANCE AND CONTROL OF THE ISOLATION OF ROCK BY GROUTING THROUGH HOLES DRILLED FROM THE SURFACE Quality assurance and control are based on measuring the decrease in the permeability coefficient of water-pro¬ducing horizons as a result of injecting grout into the drill¬holes. For this purpose, the value of the acceptable residual permeability coefficient is calculated beforehand based on the acceptable value of ground water inflow into the shaft, drift or tunnel as specified by the "construction norm and integrated practice (SNIP)" (Anon., 1976). Having ob¬tained the values of aquifer thickness, permeability, and head distribution for each aquifer from flowmetric data (dis¬cussed in Chapter 3), the average value of the permeability coefficient of all aquifers intersected by the shaft is calcu¬lated as [ ] where Ki is the permeability coefficient of the "ith" aqui¬fer; Pi is the pressure of the "ith" aquifer; and M; is the thickness of the "ith" aquifer. The total expected inflow of ground water into the shaft is [ ] where RK is the radius of the contour of influence; RCFB is the radius of the shaft; and p is the coefficient of dynamic viscosity of the ground water. The necessary reduction of the coefficient of permeabil¬ity of the aquifer [ ] where Q is the expected inflow of ground water into the shaft from eq. 8.2; Q.-fin is the acceptable inflow of ground water; and a is the safety factor. By knowing the average permeability value of the rock and the permeability reduction coefficient, it is possible to determine the value of the acceptable residual permeability coefficient for all the aquifers [ ] The values of the permeability coefficient, pressure, and thickness for each aquifer are determined from the data gathered during the hydrodynamic investigations conducted in grout holes prior to beginning the grouting operations as discussed in Chapter 3. After injecting the grout into the first of the holes, the injection into each of the following grout holes is preceded by repeated hydrodynamic investi¬gations. Using the data obtained for each water-producing zone, a graph is constructed where the numbers representing the grout holes in their order of grouting are placed on the abscissa, and the values of the aquifer permeability coeffi¬cient are placed on the ordinate (Fig. 103). This curve char¬acterizes the change in permeability of the given hydro¬stratigraphic unit as grouting operations are completed, one hole at a time. The results of grouting operations are considered to be positive when each sequential observation records a steady decrease in the permeability of the aquifer. The permeabil¬ity measured prior to the injection of the grout into the last hole must be lower than or equal to the acceptable perme¬ability as determined by the application of eqs. 8.1 through 8.4. Table 8.1 presents illustrative data on the hydrogeo¬logic properties of aquifers at a mine shaft prior to the initiation of grouting operations. Data also are presented on the reduction in the permeability coefficient during the in¬jection of the grout. The shaft in question intersects eleven aquifers. Based on data concerning aquifer pressure, thick¬ness, permeability, and the expected inflow rate of water for each of the strata, the average permeability coefficient is calculated according to eq. 8.1 as K = 0.2 x 10-12 m2. "1. AbstractMagnesium alloys have been successfully recycled for decades and have won acceptance for use by die casters and most of the automotive industry. As the supply chain for magnesium and magnesium alloys continues to change, due to the consolidation of producers, it is essential that quality control methods be closely examined with each new supplier added. An examination of the various process steps that are needed to assure quality in the final product will be discussed thus giving a basis for the auditing of new magnesium suppliers.2. IntroductionMagnesium recycling in the past has been somewhat limited due to its use more as an alloying agent for aluminum based alloys and as a desulfurization agent in iron and steel than in more readily recycled structural products. Recycling efforts continue to improve as more production is finding its way into automotive structural components, hand tools, and a variety of computer and cellular telephone applications.With the closure of several large North American primary magnesium production facilities over the past decade we have only recently witnessed a tightness in supply that is driving magnesium prices significantly higher, nearly doubling the price in the last year alone. The rapid escalation in pricing will provide additional economic incentives to fully utilize recycling for the recovery of an even greater percentage of both old and new sources of magnesium scrap. Political pressure continues to intensify towards the secondary recovery of any and all recoverable resources of which magnesium recycling readily applies.Economics will also likely drive the development of new sources of magnesium production, both from primary and secondary production. As new sources of magnesium are offered to the marketplace the potential for product quality issues may increase. When considering any new raw material supplier a decision must be made as to the impact of defective material on the intermediate or finished product. Quality defects in magnesium ingot can result in a variety of appearance, corrosion, and even adverse structural effects in the finished product. The potential severity of the impact of defective raw materials on the finished product will determine and often justify a close look at any new supplier. An on-site audit of a new supplier of a critical raw material is often justified to ensure that an adequate quality assurance system is in place. We will now look at a number of key production points that should be examined during an on-site audit that have can have an impact on the overall quality of the finished product."

"Control of analytical data quality is usually referred to in the mining industry as Quality Assurance and Quality Control (QAQC), and involves the monitoring of sample quality and quantification of analytical accuracy and precision. QAQC procedures normally involve using sample duplicates and specially prepared standards whose grade is known. Numerous case studies indicate that reliable control of sample precision is achieved by using approximately 5% to 10% of field duplicates and 3% to 5% of pulp duplicates. These duplicate samples should be prepared and analyzed in the primary laboratory.Bias in the analytical results can be identified by inclusion of 3% to 5% of the standard in each sample batch. Several different standards are used, with values spanning the practical range of grades in the actual samples. A blank (a sample in which the concentration of metal of interest is below detection limit) should also be included. Standard samples alone cannot identify biases introduced during sample preparation, and therefore approximately 5% of the duplicate samples (coarse rejects and pulp) should be processed and assayed at another, external, reputable laboratory.This paper discusses techniques used for estimation of errors in precision and accuracy, and overviews diagnostic tools. It is shown that one of the most commonly used methods, the Thompson-Howarth technique, produces consistently lower results than other methods. These results reflect the nature of this method, which relies on the assumption of a normally distributed error, and thus produces biased results when errors have a skewed distribution.
