Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

The rapid acceleration and sweeping consequences of what we have all come to characterize as "the ecology movement" has marked a period of serious trial and challenge for the mining industry. It has blossomed into a major factor in our daily business and added a significant new dimension to the problems we must meet and overcome as businessmen. It has become an important new consideration at a time when we must continue to cope with our traditional problems such as rising costs, competition at home and abroad, and the always vital necessity to continually develop new and better technology to produce more minerals more efficiently and more economically. The body of law embracing environmental quality control grows each day, becomes more complex and offers more contradictions. And there is little relief in sight. The requirement to comply with sound environmental regulations -- or challenge unrealistic legislation -- has placed a major burden on the private sector. The economic significance of enacted and proposed environmental regulation is pressing management in the mining industry to some hard decisions -- basic day-to-day business decisions and decisions which acknowledge the undoubted long-term effects of the new laws.

The Sherritt Fort Site nickel refinery and fertilizer plant has been operating continuously since 1954. The ammonia leach process uses ammonia and sulphuric acid, producing nickel and cobalt and an ammonium sulphate byproduct. One of the features of an ammonium sulphate based process is the aggressive nature of ammonium sulphate to concrete and structural steel. After six decades of operation, portions of the structural steel and concrete at the Sherritt facility have become degraded. Infrastructure replacement programs started in earnest more than a decade ago with pipe racks and electrical infrastructure, and have now progressed into building steel. Replacing and repairing this infrastructure within the tight constraints of an operating plant poses several difficult challenges. This paper will outline the approach taken to accomplish this difficult work, and discuss key learnings from the ongoing program.

In considering the importance of lease management and administration, I have decided to approach the topic, Lease Default and Eviction: Action or Reaction, as a three phase presentation dealing with mineral lease administration and management, past, present and future. Though the title may border on the dramatic, it illustrates the real consequences of alternative approaches to property management viewed from my experience in the evaluation, development and implementation of a number of such systems and services in my current position as president of Talco Group Corporation. First, though, let's take a moment and set the stage by giving a few actual examples of why a pragmatic approach is needed in this particular area.

Most mining companies today perform site assessments and operational or compliance audits regularly. The purpose of these audits is to review company facilities and operations to determine whether they comply with applicable federal, state and local environmental laws and regulations. Why Audit? Companies adopt assessment and audit programs for legal and business reasons. The business reasons include: (1) ensuring efficient and cost-effective compliance through early problem identification and resolution; (2) planning and implementing business transactions such as real estate conveyances, loans, mergers and acquisitions, and waste disposal service contracts; and (3) employee training and relations. Legal reasons for audits include: (1) avoiding enforcement actions and penalties; (2) meeting environmental permit certification requirements; (3) avoiding corporate officer liability; and (4) meeting Security and Exchange Commission disclosure requirements. The Environmental Protection Agency ("EPA") encourages environmental auditing by regulated entities. The EPA Environmental Auditing Policy Statement was published in the Federal Register on July 9, 1986 (51 Fed Reg 25,004). Although the existence of an auditing program does not create a defense to or otherwise limit a company's regulatory responsibilities, EPA considers compliance programs in setting inspection and enforcement priorities. If a company makes an honest effort to identify and correct its own problems, EPA's enforcement approach is influenced, especially if violations are promptly reported. Perhaps more significant to many executives, the U.S. Department of Justice issued guidelines last year on when it will pursue criminal prosecution for environmental violations. See Department of Justice, "Factors and Decisions on Criminal Prosecutions for Environmental Violations in the Context of Significant Voluntary Compliance or Disclosure Efforts by the Violator," (July 1, 1991). It is now commonplace to read about major companies pleading guilty to criminal violations and paying millions of dollars in penalties. For example ALCOA recently agreed to pay criminal fines and civil penalties totaling $7.5 million for unauthorized excavation of PCB-contaminated soil and improper dumping of spent caustic solution. No executive wants to explain to the Board of Directors or shareholders why the company was criminally indicted. The Department of Justice considers factors such as voluntary disclosure, preventive measures and compliance programs, such as a regular compliance auditing program, the effectiveness of the disciplinary system for employees who violate the company's environmental policies, and subsequent compliance efforts by companies to remedy ongoing noncompliance. The Department of Justice looks closely at whether audit recommendations were implemented in a timely way and whether environmental compliance was a standard by which employees were judged. The Lawyer's Role. The role of the lawyer in an audit team deserves special attention. When enforcement action or litigation has begun or is contemplated or a government agency has asked for information, attorneys always should be directly involved in environmental audits. In most situations, however, environmental audits are management tools, and the attorney's role should be to advise the team. The attorney can help make sure the team understands the laws and regulations the facility must

Base and precious metals prices have reached levels not seen before. Coal prices have also shot up. And there are encouraging signs in the uranium market. China’s nearly out-of-control economic growth is said to be the leading driver for much of this resurgence. But demand for energy, metals and building materials in the United States and elsewhere are also major factors. The result is that expansions and new mines for nearly every commodity are in the works.

February 2, 1999 marks a milestone in earth-moving technology at the San Miguel Lignite Mine. It was on that date that The North American Coal Corporation (NAC) took delivery of three Leica Dozer 2000 GPS grade control systems for use on its bulldozers. The primary reason for the purchase of these grade control systems was to prove that GPS technology could safely and economically be utilized on heavy equipment to assist operators in defining grades. If these properties were successfully demonstrated, the ultimate vision of the project was to expand the system to a point were GPS would become the primary means of providing grade control information to the mine’s heavy equipment operators. Twenty nine months and more than $500,000 later this vision was realized when the final bulldozer operating at the San Miguel Lignite Mine was equipped with Dozer 2000. Presently North American Coal has a total of nine Dozer 2000 GPS grade control systems at San Miguel providing cut and fill information to operators 24 hours a day. These grade control systems have allowed the San Miguel operation to increase safety, reduce costs, improve operator attitude, and better ensure environmental compliance. This article discusses both the nature of these benefits and some of the issues that had to be dealt with along the way.

J.B. Gwiazda's article deals with an interesting problem that has not been studied thoroughly up to now. Gwiazda has proposed a technical solution that eliminates horizontal load imposed by the supports on the roof. That load is due to a discrepancy between the trajectories of the canopy and the overlying strata. It is true that the improved lemniscate guidance system proposed by Gwiazda could eliminate the horizontal load but, on the other hand, it would represent a higher cost of the supports, and it would complicate the already complicated mechanism of powered supports. That is why we must be convinced that the proposed improvement is actually needed. From that point of view, the following questions should be elucidated. As shown in Fig. 1, the point O travels along the curve a, which produces horizontal load on the roof in the direction of the coal face. As stated by Gwiazda, "such a high load is capable of destroying the roof above the support, causing rock debris to be scattered around the face." The results of our measurements carried out in coal mines of the Czechoslovak part of the Upper Silesian Coal Basin show that the convergence in the modern type of powered supports does not exceed several millimeters (not more than 10) per hour. Similar conclusions were drawn from field measurements performed in the Polish part of the Upper Silesian Coal Basin (see S. Romanowicz and H. Szopka in Proceedings, Scientific and Technical Symposium Simmex '85, Katowice, Poland, pp. 73 -83). The small vertical closure of the supports also induces a small horizontal displacement of the canopy. In addition to this, the transmission of forces between the roof and the canopy is influenced by the layer of debris on it, i.e., the canopy slides without imposing a greater horizontal load on the roof. This was deduced from field measurements of Voest-Alpine F 4/4600 powered supports equipped with rams instead of the common front lemniscate links (denoted as 1 in Fig. 5). In these supports, the pressurizing of the space of piston rod (la in Fig. 5) causes the rams to close, which induces horizontal load on the roof in a direction toward the coal face. After the setting of a support unit, the pressure in the piston rod space was found to drop, frequently to zero, because the small horizontal sliding between the canopy and the debris or inside the debris caused the horizontal reaction from the roof to fall to zero, although the debris were pressurized by vertical setting load. As a consequence, there was no horizontal load imposed by the canopy on the roof. A rise of the horizontal load was recorded only after the passage of the shearer and during lowering of the adjacent units. Thorough information on the results of the afore- mentioned field measurement is given in the paper, "Analysis of the performance of shield powered supports installed in a thick seam," published in Mechanizacja i automatyzacja gornictwa, 1982, No. 12, pp. 38-46. An English translation of the paper is available from the author. We have deduced from our observations that horizontal load imposed by the canopy on the roof in the direction toward the coal face is useful for roof control because it limits the displacement of the immediate roof toward the gob particularly during the rise working. We came to the conclusion that horizontal load on the roof toward the coal face should be induced on purpose. Could Mr. Gwiazda prove with the results of field measurements his contrary opinion? Has the small horizontal displacement of the canopy toward the face actually had an adverse effect on the roof? As shown in Fig. 2, during the setting of a support unit, the canopy imposes a horizontal load on the roof in the direction of the gob. In this case, I share Gwiazda's opinion that the horizontal load could have an adverse effect on the roof. Nevertheless, two important circumstances were neglected - the horizontal compliance of the supports and the interaction of adjacent units. The question of the compliance was analyzed precisely by I. Krumnacker in Gluckauf -Forschungshefle, 1984, NO. 5, pp. 219-223. "Located in the northwestern portion of the Abitibi greenstone belt, Les Mines Selbaie is a copper-zinc (silver and gold) producer. Felsic tuffs host three are zones. Composed of sulphide veins and breccias, they are not exhalative massive sulphide deposits, except, probably, for the massive pyrite (fragments and lenses) localized close to the top of the volcanic sequence.The total of all (mineable and mined) ore in the three zones presently outlined on the property, is of the order of 29.9 x 106 tonnes with average grade of 0.63 g/t Au, 37.02 g/t Ag, 1.21% Cu, 1.91% Zn.The ore reserve calculations and the daily production procedures are computerized to various degrees. The data handling process is highly computerized.Various geophysical methods are, and have in the past, been successfully used on the property. The ""mise-à-la-masse"" surveys were used to perform the geological interpretation of an underground deposit (A-2 zone). Presently the PULSE-EM surveys are performed in all surface and underground exploration drill holes."

"Selbaie is located northwest of Joutel, in what is known as the fourth east-west volcanic sediment group in northwestern Quebec. The main metals produced are copper and zinc, with important contents of both silver and gold. Production comes from an open pit mine on the A-I zone and from two underground mines on the B zone and the A-2 zone. Each mine gave rise to important capital investment programs for construction and mine development, all of which were managed and staffed with company-hired, non-union personnel. Production from all sources now exceeds 7000 tonnes per day, with a workforce of540 regular employees. Of particular interest technically is the high recovery of B zone reserves underneath 40 m of overburden cover, the low-cost capital development of the A-2 zone, the computerized control of open pit mining, and the successful copper-zinc separation of the A-1 zone ore. Due to the low grades of the mineral deposits, efficiency in all aspects of the production process, is essential to the success of the operation.LocationThe Selbaie Mine is located 82 km northwest of the locality of Joutel in northwestern Quebec (Fig. 1). Access to the mine is by an all-weather road, 32 km of which is paved. A second road access to the site is via La Sarre, 134km to the southwest. Employee access, materials transport and haulage of copper and zinc concentrates take place over both of these roads."

"Les Terrains Auriferes (LIA) Property, formerly the Malartic Goldfield (Goldfield) mine, ceased its remaining milling operations in the fall of 1994. During the early years of mining, the mill processed a non-sulfide ore with substantial carbonate mineralization.In 1977 after an extended shutdown period, ore from a nearby property, Bousquet, was processed at the mill. That ore contained larger quantities of sulfides, and unlike the old tailings, had acid generating potential.Upon closure of the L TA mill, the closure options considered for the L TA site were to cover with:1. a thin cover of 0.5 metres of borrow material (sand and gravel) to achieve basic erosion protection/stabilization; collect acidic seepage and treat with lime.2. a 2 metres of non-acid producing tailings; final surface to be vegetated.3. an engineered oxygen barrier consisting of a multi-layer cover; final surface to be vegetated.The paper presents the technological and economical methodologies that were used to select the closure alternative, describe the associated construction challenges that were faced with the implementation of the multi-layer cover concept and presents preliminary results on the cover's performance.In addition to being the first full scale demonstration of this type of multi-layer engineered oxygen barrier cover technology, this project had the advantage of using non acid producing mine tailings as part of the reclamation solution."

The Beverley uranium mine û now in production in the remote north of South Australia at1000 tonnes per annum û was established to meet a projected, but little-recognised shortfallin primary supply of uranium for the nuclear industry in the early part of the 21 st century. Inthe context of AustraliaÆs lack of bi-partisan support for uranium mining and the nuclearindustry, a four to five year window of opportunity existed from 1996 to establish the mine.During this period, Beverley achieved a number of firsts including: the first ISL mine inAustralia; the largest ISL mine in the world and the first to produce more than 500 tonnes ofuranium in its first year; the first to be established under a new Native Title regime in SouthAustralia; and the first to be established under a revised South Australian Mining Act.Beverley was brought into production against the background of: hostile opposition fromanti-nuclear activists; a suspicious and at-times hostile media; a regulatory and politicalcommunity that was unfamiliar with ISL mining; commercially unsophisticated Aboriginalgroups; an inexperienced engineering industry; and the vagaries of development in a remote,semi-arid location with neither population nor infrastructure. The lessons that can be learnedfrom Beverley showed that û like much in life û flexibility and timing are crucial.

"Thousands of years ago large glaciers carved much of the landscape in Puget Sound, depositing abrasive and granular soils including cobbles and boulders along with layers of clay and sand. Tunneling in this geology – common throughout much of the northern and Midwestern U.S. – is challenging and can be unpredictable. The use of pressurized face tunneling, most commonly with EPBs and slurry type machines, has made excavation in this geology more efficient, but difficulties still remain. Through analysis of recent projects in Seattle, Washington, USA, the authors draw conclusions as to the best TBM designs and operational parameters for this variable, abrasive and often difficult ground. Performance data is analyzed with an eye towards advancements that could further improve TBM tunneling in glacially deposited soils. INTRODUCTION Over the past several ice ages, extensive glaciers reaching across much of North America transported large amounts of soil and rock as they moved across the land. The substantial weight of the ice sheets mixed together a wide range of soil types along with a variety of rocks and boulders of all sizes. Many rock formations were reduced to pebbles or particles containing highly abrasive characteristics. Any boulders able to survive were generally comprised of harder materials which were scattered throughout the soil. This highly variable mixture of abrasive soils and high strength boulders can be found in many areas across the Puget Sound and other regions of the country. Wherever they are encountered, they often create a difficult and unpredictable mining environment for both equipment design and operation. Modern TBMs are optimized early in the design process for specific types of geology, and unexpected conditions can require extensive modifications. In addition, several operational parameters need to be adjusted to balance ground characteristics, and advance rates can be significantly affected in mixed and rapidly changing soil conditions. Both EPB and slurry machines can be used successfully in these conditions, provided their respective operating criteria are met. Ideally, an EPB is operated with a relatively thick muck consistency, which accommodates moderate changes in soil permeability and maintains a reliable pressure differential through the screw conveyor. However, this also causes higher wear on rotating components and requires more energy at the cutterhead. Alternatively, slurry machines typically operate best with a more fluid muck consistency that can be easily pumped through the extensive piping system. This tends to reduce the wear on the cutterhead and the power requirements of the cutterhead drive system, but also makes the machine more sensitive to changing soil conditions. Generally, the slurry piping system is also more capable of controlling high face pressures, although the screw conveyor of an EPB can also be designed to accommodate most high-pressure conditions."

Augered-Cast-in-Place piles have been used successfully for many deep foundation applications. The four case studies discussed herein describe ACIP pile projects where problems developed during ACIP pile installation requiring changes to the method or sequence of installation. The case studies illustrate the importance of understanding existing subsurface conditions and project constraints when considering the use of this pile type. Ground loosening and over-excavation of soil during the augering process, a significant concern for fine to medium sands and non-plastic silts below the water table, can significantly impact adjacent structures. This risk needs to be evaluated during the pile selection process and generally can be mitigated, but not eliminated, by requiring the use of piling equipment with high torque and slow auger rotation speed to advance the auger with minimum amount of rotation. In some applications where high lateral stresses in the soil are present such as at the base of retaining structures, the use of ACIP piles may not be appropriate if structure movement is unacceptable. On the other hand, ACIP piles have been successfully used to support deep foundations in preconsolidated varved deposits where the new structure was designed to accommodate recompression settlement.

Accidents involving powered haulage and mobile equipment such as haul trucks often account for the greatest number of fatalities in the mining industry each year. Despite previous analyses that have identified root causes and other contributing factors, there is still a need to better understand the events leading up to these types of accidents, what lessons may be learned, and what strategies can be employed to prevent fatal accidents from occurring. This study examines Naturalistic Decision Making (NDM) using the critical decision method (CDM). The CDM is a retrospective interview approach used to explore timelimited, high-stakes, decision-making. In this study the CDM is used to obtain more information about what happens prior to, during, and after a potentially fatal situation such as a near-miss event, loss of control, or minor accident involving equipment damage. Researchers captured firsthand accounts from 21 haul truck operators involved in near-miss events from mine sites of various sizes and commodities throughout the United States. These accounts provide rich and detailed narratives from the perspective of haul truck operators themselves and reveal insights into what decisions haul truck operators make, what sensory cues they perceive, and what strategies they employ during challenging and non-routine situations. Decision-related themes that emerged from the data are presented and discussed. These results, along with potential solutions offered by study participants, can help to inform future research, raise awareness about hidden hazards, and build more creative interventions and realistic training scenarios for use by the industry to address haul truck safety issues.

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. Six recent case studies were selected based on the eight main causes of mine disasters (Table 1; Quinlan, 2014) and where credible information sources, such as Royal Commission Reports, Coronial Investigations and textbooks, were available.
