"Mines around the world are inherently and operationally diverse. Current methodologies for the design of mine ventilation and cooling infrastructure involve controlling major safety and health hazards. Globally, it can be estimated that mine ventilation systems from various commoditiescirculate over 150 000 m3/s of exhaust air through shafts. Additionally, over 2 000 000 workersare transported every day through shafts in labour-intensive mining countries like China, India,Southern Africa, and the Americas. A key objective of this paper is to share experiences and provide a case for the revision of ætime-honouredÆ ventilation design parameters based on a benchmarking exercise on design exhaust shaft velocity from various global operations (gold, diamond, platinum, coal and base metals). An active fi eld observation on exhaust shaft velocity has indicated that, regardless of the commodity or continent where the mine operates, almost all life-of-mine (LOM) design parameters provided are in the same range of 20 m/s to 22 m/s, which is not convincing considering signifi cant variations exist in shaft depths, geography, commodity, operating and capital costs. From a due diligence perspective, there are no documented views that are available on the most widely used design exhaust air velocity. Based on the global exhaust shaft velocity data, it is noted that a significant portion of shaft velocities are outside the current design velocity of 20 m/s (some operating up to 30 m/s) and other signifi cant portions of the exhaust shafts operate in the critical velocity range. In the case of Australian coal mines, limited data suggests that none of the mines have ever reached the LOM design exhaust shaft velocity. It is self-evident that when the shafts are not operating at their design quantities either the mine system resistance is low or the ventilation and main fan is operating ineffi ciently. The most common reasons given for not reviewing the shaft system were, ætime-honouredÆ design parameter are being used and cannot be changed, mine fan pressure limits, possible fan blade damage, possible spontaneous combustion risks, leakage or simply did not consider such a possibility. In conclusion, the study has reiterated the historic lessons that a safe and expensive airway should not be utilised uneconomically by circulating too litt le air through it."

The case history, which is the subject of this paper, involves the expansion of a lime plant in Texas. The major new production unit is to be a 545.45 tonne per day coal fired rotary kiln. Auxiliary facilities include additional quarry haulage equipment, modifications to the crushing plant, a hydrator, storage facilities, and a loading system. The existing plant consists of a high calcium limestone quarry, two coal fired rotary and 10 gas fired shaft kilns, nth crushing, screening, stockpiling, hydrating, storing and loading facilities. The proposed modification would increase quarry volume from 545,000 tonnes per year to 805,000 tonnes per year. The plan also includes the addition of dolomitic lime to the product line which requires the purchase and handling of 186,000 tonnes of screened, washed dolomite yearly. A decision to undertake such an expansion was previously a simple and straight forward process. Such a decision now faces a major and complicating obstacle which can delay the project, raise the expected cost, or prevent it entirely.

This paper presents a case study of the deformation-based design, construction, and performance of stone column ground improvement (GI) beneath a mechanically stabilized earth (MSE) wall and bridge abutment with heights up to 50 feet. As part of the widening of Interstate 5 (I-5) to allow High Occupancy Vehicle (HOV) lanes in Tacoma, Washington, a new approach and span over the Puyallup River will be constructed. During the soils investigation and design phase of the project, low plasticity silts (ML) inter- bedded with silty sand layers and organic silt where identified as being potentially liquefiable. Stone column ground improvement was designed using a deformation-based approach to address static settlement and seismic stability of the proposed MSE embankments. The deformation-based design of the stone columns resulted in significant savings over a limit equilibrium-based approach. This paper presents the results of sonic coring taken from the initial stone columns installed and compares these results to the real-time data acquisition reports from the stone column installations. Vibration monitoring results during stone column installation are included. Settlement monitoring at the face of the MSE wall and buried vibrating wire settlement monitoring elements are presented and compared to the original settlement predictions.

The work discussed in this case history was part of the Washington State Department of Transportation (WSDOT) I-5/Portland Avenue to Port of Tacoma Road, Stage 1 Northbound HOV contract, developed as an early work element to a follow-on bridge construction project. The area requiring stone column ground improvement is shown in Figure 1. The improved ground supports permanent geosynthetic walls and embankments up to 50 feet high. This wall can be seen in Figure 1. The selected ground improvement primarily provides global stability of the soils beneath the walls during anticipated seismic loading and also reduces the time and magnitude of consolidation settlement under the weight of the planned embankment.Approximately 74,000 cubic yards of liquefiable alluvial soils were improved by stone columns. Stone column construction occurred in late 2010 and early 2011. This was followed by the embankment and wall construction. Settlement monitoring began with the embankment construction and continued into 2012."

Anchored sheep pile is normally used in deep excavations, many of which only require stability in accordance with Terzaghi theory. In this paper, based on the experience, new lateral earth pressure method is adopted taking account into interaction among soil, anchors and sheet piles so that a group for the project under very difficult conditions and very severe requirments, especially, for the displacements are determined according to that mentioned above. During and after construction, displacements monitoring were carried out for about four months, which was proved that there was a little difference between calculation and practice.

"The bridge across River Mandovi in Goa is a cable stay bridge structure proposed parallel, along existing bridges for regulation and reduction of vehicular traffic; the entire bridge structure is supported on pile foundation socketed in rock. This paper describes drilling related issue encountered during execution of pile where bed rock varies considerably from factual geotechnical investigation, which in turn affected the productivity of bored piles socketed in rock then chiseling is advocated for rock socketing with limited number of blows based on penetration resistance of rock encountered or equivalent energy principle to increase the productivity and also to avoid excess chiseling which otherwise would have fractured the rock mass below and around the pile tip resulting in increased settlement of pile subjected to design load. Contractor decision on socket length have been taken without compromising either quality or productivity; the paper presents various procedures and the equipment involved for installation of pile in rock along with the conclusion related to termination of pile in rock and also the lessons learned in process.INTRODUCTIONPile foundations are generally used to transfer heavy loads in deeper bearing stratum if the soil immediately beneath the base of the foundation does not have sufficient capacity. The bridge across River Mandovi, Goa is in its construction phase as a part of the development plan of Goa State Infrastructure Development Corporation Ltd (GSIDC). The project was awarded to M/s. L&T Construction, Heavy Civil Infrastructure; subsequently the geotechnical investigation was awarded to M/s L&T Geostructures.The client has specified the foundation of the bridge as rock-socketed. Rock socketing is generally done to achieve higher bearing pressure through concrete-rock shear interface which many building codes permit. The design and construction of rock socketed pile foundation entails a detailed analysis and judgement on the superstructure loads, predominant geological conditions as well as ground water levels. The termination of pile is normally based on thumb rule of half to three times the pile diameter into the rock. Quite often the criteria is based on chisel penetration rate (Gandhi,1990) In fact, this is the key factor to consider appropriate and economical depth of embedment of pile into rock stratum which necessitates enormous understanding on the type and quality of rock strata encountered which may significantly influence the selection of suitable rotary rig for pile installation. Such criteria are site specific and also applicable for the set of equipment used for the particular project."

"During the excavation of the north bound tunnel of the Devil’s Slide Tunnel project, deformation (convergence) was measured after passing through Fault Zone B. The tunnel convergence was not symmetrical, but was larger on the left side of the profile. It was determined that a shear zone nearly parallel with the tunnel drive was contributing to the uneven deformation. This paper will discuss how it was determined that the shear zone existed and how multi-point borehole Extensometers (MPBX) were used to verify that a significant amount of the deformation was occurring due to movement along the shear zone as the excavation was advanced. In addition, performance of the shotcrete liner in the area of the convergence will be examined. Finally, the paper will consider the means and methods that were utilized to ultimately control and stabilize the deformation. INTRODUCTION The Devil’s Slide Tunnel Project consisted of excavating two 1,250 meter tunnels each with a profile of 80 m2 utilizing the New Austrian Tunneling Method (NATM) or otherwise known as the Sequential Excavation Method (SEM). The tunnels are part of the California Coastal Highway south of Pacifica, CA, a suburb of San Francisco. The tunnels were opened to traffic in March of 2013. Geological Setting The tunnels run north-south through the San Pedro Mountain ridge, which is part of the Santa Cruz Mountains. The tunnel lies within the San Andreas Fault system and is 7.2 km west of the surface trace of the San Andreas Fault and 2.8 km east of the surface trace of the San Gregorio Fault. The 10 km strip between these two faults is referred to as the La Honda structural and terrain block (Decker et al. 2010; Amini et al. 2005; Earth Mechanics Inc. 2003). Locally, the tunnel passes through three blocks representing different geological conditions: South Block, Central Block, and North Block (Figure 1) (Decker et al. 2010; HNTB Corporation et al. 2005). The South Block consists of Mesozoic aged granodiorite and quartz diorite. The South Block also contains many local shear zones and a low angle thrust fault (Fault A). The South Block ends at Fault B, which is the southern border of the Central Block."

This case study describes the geotechnical aspects of the basement construction for a multi-storey, residential and hotel complex located in the newly developed Elizabeth Quay precinct in Perth, Western Australia. The development included a three-level basement for car parking covering the full site area which was constructed using the top-down construction sequence. Soil conditions generally comprised 2m of fill overlying medium to dense clayey sands that in turn were generally overlying a very stiff clay stratum at approximately 13m depth. Bedrock level was generally encountered at approximately 22m to 35m below ground level. The ground water table was at a depth of approximately 1m below the existing ground surface level. A site boundary basement wall was constructed using a diaphragm wall retention system. The majority of the vertical loads from the building were transferred to the Kings Park Formation (KPF) bedrock through the use of foundation piles constructed by the CFA method. The CFA piles were installed from existing ground level, resulting in approximately 9m sacrificial concrete length from pile platform level to basement 3 (B3) level. The permanent column loads are such that a single CFA pile was not structurally adequate. To allow a full CFA foundation system to be adopted, an innovative design and construction approach was developed for these heavily loaded columns. This paper discusses the site technical details and the design and construction aspects required to meet the challenges offered by this project.

NIOSH researchers collected basic data from two blast rounds at the SSX-Steer Mine as a component of a larger study on controlled blasting, the goal of which is to reduce injuries from falling rocks in underground mines. Drilling and blasting procedures at the mine were observed. Rock mass property data were collected, a program of seismic monitoring and analysis was conducted, and 3-D laser survey scans of the workings before and after each blast were performed. The geologic data showed that the rock mass quality was characterized as poor to very poor. It was essentially uniform within the panels examined. The recorded seismic data indicated that fill material acted as an effective damper to seismic energy. The pre- and post-blast laser scans showed both over-break and under-break conditions, highlighting the problems associated with blasting a weak rock mass. By comparing the achieved results to the intended design, potential improvements in the drilling and blasting practices can be identified. Comparison of these data with similar data from other mines will help define how various combinations of rock and fill behave during blasting. Through studies such as these, data collection procedures and analysis techniques can be refined. An ultimate goal is to develop blast procedures tied to rock mass characteristics that can minimize damage to rock in the perimeter of the opening, limiting the amount of loose rock and improving the safety of miners.

Although useful to encourage organizations to work together, executing such risk assessments comes with challenges. This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

This case study provides an example of how social marketing was applied to meet the needs of business customers. Federal regulations require that all companies develop a written program to track hazardous chemicals at their sites. The HazCom Writer, created by the National Institute for Occupational Safety and Health (NIOSH), is a software tool designed to help companies prepare their required Hazard Communications (HazCom) written plan and list all hazardous chemicals at the company's worksite. The social marketing goal was to provide a product for small- to medium-sized companies to use for federal HazCom rule compliance. The HazCom Wm'ter is a direct NIOSH response to customer and stakeholder needs (including the Mine Safety and Health Administration, the mining community, and Occupational Safety and Health Administration- regulated companies).

This paper outlines the evolution of managing high consequence risk scenarios in BHP Coal, with particular focus on managing the stability of an endwall that is adjacent to a public infrastructure corridor running through a Central Queensland coalmine. BHP Coal developed a Geotechnical Consequence Register after industry failures identified shortcomings in traditional risk assessment practices. Traditional risk assessments consider the consequence and likelihood of a geotechnical event occurring under normal operational conditions. As such, the risk rating for a high consequence, low likelihood failure may fall within the acceptable range, even where a lack of meaningful geological data exists due to the area being off lease or outside of mining reserves. The Geotechnical Consequence Register lists all geotechnical failure scenarios of high consequence, identifies if current controls are adequate, and highlights if specific Geotechnical Management Plans are required. This case study reviews a failure scenario that was identified through the geotechnical consequence register. The scenario had previously been considered low likelihood and subsequently driving a low risk; however, the likelihood ranking was impacted by a lack of geological data outside of the mining lease and predominately informed by an acceptable historical slope behaviour. Additional exploration identified previously unmapped faulting which increased the risk rating. The requirement to develop a specific management plan resulted in an increase in the required factor of safety, design changes, the implementation of additional monitoring and a corresponding monitoring Trigger Action Response Plans (TARP). In the case study area, the initiation of a geotechnical instability was identified early through the increased monitoring program. The instability was successfully managed and any impact to the infrastructure corridor off lease was prevented. BHP Coal has used the Geotechnical Consequence Register for several years to identify and assess high consequence geotechnical failure scenarios. In some areas this has justified additional exploration, more stringent design acceptance criteria and an increased level of geotechnical monitoring. The process has been successful in ensuring that geotechnical risk is controlled in areas that often receive less attention than active production areas.

Mine to Mill optimisation is not a recently developed concept. It is one which is finally gaining greater acceptance in the Australian Mining Industry, possibly as a consequence of the downturn in markets and certainly made possible by the development of tools and techniques to measure and predict performance of mining equipment and milling plants. Fragment size after blasting has a major bearing on the performance of all aspects of mining and a major influence on the economics of the mine and mill. The process to optimise fragmentation consists of identifying the key factors, measuring their sensitivity through a modelling and trialing program and conducting economic evaluations on the changes on both the mine and mill. Management of the Ernest Henry copper-gold mine in NorthernQueensland, Australia, has espoused the Mine-to-Mill concept and isactively pursuing excellence in blasting to drive down the total cost of mining. Ernest Henry has extremely hard, wet and heavily jointed geological conditions, and had high powder factors and a large percentage of oversize material reporting to the crusher or ROMstockpile. A blasting improvement program was undertaken betweenErnest Henry Mining, Thiess Contractors and Orica Explosives tooptimise the ore size distribution, particularly the oversize component, as a step to reducing production costs. Measures of success included bringing the rock breaker back from double shifts to part-time operation and increasing direct dumping into the crusher from 70 per cent to 92 per cent. Tools and technologies to predict and measure the performance ofindividual blasts are needed before it is possible to optimisefragmentation or the downstream processes. These include suchspecialised Orica Explosives tools as SABREX, POWERSIEVE, andPOWERLINE and commercially available drill monitors. As importantare the more common still and video camera, borehole loggers,measuring tapes, surveying equipment such as face profilers, and ahigh-speed camera if possible. There are many tools which can be applied however it is rarely necessary or possible to use all of them due to time constraints and a point of diminishing returns. The critical point is knowing what the true parameters of the blast were compared to design - only then can the predictive tools be effectively utilised. Stage one of the program has seen gradual improvement as theunderstanding grew and practices have been implemented resulting inreduced bridging in the crusher, geological mapping with respective blast designs and the oversize component under control. "In this paper, the advantage of using numerical models with the strength reduction method (SRM) to evaluate entry stability in complex multiple-seam conditions is demonstrated. A coal mine under variable topography from the central Appalachian region is used as a case study. At this mine, unexpected roof conditions were encountered during development below previously mined panels. Stress mapping and observation of ground conditions were used to quantify the success of entry support systems in three room-andpillar panels. Numerical model analyses were initially conducted to estimate the stresses induced by the multiple-seam mining at the locations of the affected entries. The SRM was used to quantify the stability factor of the supported roof of the entries at selected locations. The SRM-calculated stability factors were compared with observations made during the site visits, and the results demonstrate that the SRM adequately identifies the unexpected roof conditions in this complex case. It is concluded that the SRM can be used to effectively evaluate the likely success of roof supports and the stability condition of entries in coal mines.INTRODUCTIONSince the introduction of roof bolts in the coal mines during the late 1940s and 1950s, roof bolts promised to dramatically reduce roof fall accidents (Mark, 2002). However, ground falls still remain a significant factor in underground coal mine injuries and fatalities. In 2013, ground falls accounted for 4 of the 14 fatalities and 166 of the 1577 reported lost-time injuries in underground coal mines (MSHA, 2015).The design of appropriate support systems requires the understanding of: 1) the variable nature of the rock mass, 2) the performance and characteristics of the roof support, 3) the interaction between the rock mass and the installed support system, and 4) the in-situ and mine-induced stress distribution around the excavation. Over the past 25 years, multiple design approaches have been used in coal mine ground control. The approaches include empirical mechanistic methods, empirical statistical analysis, rules of thumb, and numerical methods (Hebblewhite 2006). In the U.S., Analysis of Roof Bolt Systems (ARBS) can be given as an example of an empirical method. ARBS uses relatively simple equations to calculate the intensity of support provided by a roof bolt system and compares it with a suggested ARBS value (Mark et al., 2001). The suggested ARBS design equation is derived from an analysis of 100 case histories. The ARBS design equation is dependent on two parameters: depth of cover and Coal Mine Roof Rating (CMRR). More recently, a probabilistic design approach was developed by Canbulat and van der Merwe (2009) in South Africa. In this method, the variability of the rock mass, the mining geometry, and support characteristics are included in the analytical models. The major advantages of these two methods are: 1) they can be applied rapidly and easily, 2) complex rock mass/ roof support interaction mechanisms are represented with simple equations, and 3) they are supported by large databases. However, both methods generally ignore mining-induced stress distribution, details of the roof support system, details of the geological setting, and the interaction between the support system and the rock mass."

The San Miguel Mine is owned by San Miguel Electrical Cooperative, Inc. and operated by Atascosa Mining Company, a wholly-owned subsidiary of Morrison-Knudsen Company, Inc. This multiseam, single-zone lignite surface mine is located near Jourdanton, Texas, 110 kilometers (68 miles) south of San Antonio. Lignite extracted from four closely-spaced seams is used to fuel the 400-MW San Miguel Power Plant. Recovery depths range from 7 to 37 meters (25 to 120 feet). The mine began stripping operations in the spring of 1980 and now produces approximately 2.5 million tonnes (2.8 million tons) of lignite annually. Atascosa Mining Company is responsible for production, maintenance, engineering, and management services. Production responsibilities include topsoil removal, overburden removal, lignite mining, spoil pile regrading and topsoil replacement, as well as ash disposal and construction and maintenance of water retention and diversion facilities.

INTRODUCTION The small mining company faces the dilemma of how to finance the development of its properties. Many of these small companies don't have the financial resources to pay for development costs from their own funds and frequently they do not even have the financial strength to permit them to borrow the money needed for development. A project financing can sometimes be arranged in such a way as to solve this dilemma. A typical project financing will not provide the answer to the "little guy's" problem because most project financings require that the sponsors of the project provide completion undertakings and sometimes other undertakings. These undertakings are often a prerequisite to obtaining financing, and if the sponsor is not financially strong enough to provide the comfort that the lenders require, then the sponsor will have difficulty obtaining financing no matter what undertakings are offered. However, with a little imagination and the right set of circumstances, a project financing can sometimes provide the solution to the small mining company's problem. The Strathcona Sound Project is unusually suited as a case study because the sponsor, Mineral Resources International Limited,(MRI) was able to arrange a project financing for the development of the project even though the lenders probably would not have been willing to loan a similar amount of money direct to MRI based on the full faith and credit of MRI's balance sheet. The way that MRI was able to arrange this financing makes for an interesting story. THE CASE In early 1974, MRI owned 100% of a small to medium sized zinc - lead - silver massive sulfide deposit (Property) located 475 miles north of the Arctic Circle less than two miles from deep water on Strathcona Sound which is a shipping zone in the Canadian Arctic. The Property was situated at a desolate location except for an Eskimo settlement (Arctic Bay) which was about 20 miles west of the Property. In other words, the project required that complete infrastructure would need to be installed in this far northern location. Independent reserve and feasibility studies (Study) had been completed on the Property which confirmed a total of 6,825,000 tons of reserves classified and summarized as shown below on Table I: [ ] These reserves were based on 229 diamond drill holes totalling 58,909 feet and were established on a cut-off grade of 7% zinc and a minimum mining height of seven feet. The overall mining recovery rate was estimated at not less than 85% and not more than 90%. The feasibility study indicated that it would cost an estimated $45,044,000 to develop an underground mine, a concentrator, a town-site, a power plant, storage facility, loading dock, a tailings disposal area and an airstrip on the Property with an annual capacity of 525,000 tons of ore per year. The construction period was expected to take two years and projected capital costs were broken down as set forth below on Table II:

The complex interaction between multiple seam pillar extraction panels and variable topography can generate unexpected ground control problems. This case study describes stress-related damage in multiple seam workings that resulted in difficult operating conditions and compromised the safety of the support crews. The damage appeared to be caused by horizontal stress but the unexpected location of the damage baffled the authors. A 2D Finite element analysis was conducted of the mine layout and surface topography in an attempt to explain the observed failure. It was discovered that the topography resulted in the rotation of the major stress at the location of the current workings. The stress rotation caused asymmetrical interactions between the workings on the different coal beds, which can explain the unexpected response observed in the mine. Consequently the layout of the next panel was changed by locating the vulnerable entry 30 ft away from its originally planned position. The new location was selected to locate the entry in favorable stress conditions. The change in layout resulted in a significant improvement in conditions when the next panel was mined. At the time of writing this paper, a full panel has been developed using the modified layout without undue ground control problems. The case study demonstrates that numerical models can help engineers to interpret and solve complex multi-seam interaction problems.
