This project will expand sanitary sewer within the City of Columbus. It consists of 2.5 mile long, 72-inch finished diameter pipe grouted inside 10.8-foot diameter tunnel excavation in shale. This paper describes the multi-faceted geotechnical exploration to minimize the underground risks. The tunnel will cross below a buried valley with minimum cover of highly weathered rock. The risk mitigation to cross the valley includes contingencies and cost allowances. Provisions to prevent impact on adjacent groundwater resources will be described. Latest construction updates will be provided including contractor alternate shaft support and pre-excavation probing to mitigate tunneling risk below the valley.

Since 1993, Newmont has been actively growing through the development of three major foreign mining operations with a fourth under construction. The first project, Yanacocha in Peru began operating in October 1993 at a time when political and business risk in that country was considered high. In 1995, Newmont began operating the Muruntau Joint Venture in Uzbekistan, the first of its kind in the CIS. In early 1996, Newmont started operating the Minahasa mine in Indonesia. Now, in early 1997, Newmont is undertaking the construction of a $1.9 billion project in Indonesia and is completing a feasibility study with our partner for the development of a gold operation in Mexico. All of these projects have been considered "risky" by most standards, and Newmont has been very successful in all of them through risk mitigation programs including utilizing strong and successful in-country partners, concentrating on developing a strong national workforce with minimal expatriate employees, utilizing World Bank, EBRD and IFC financing and building and maintaining strong ties with host governments. This paper describes in more detail, some of the risk mitigation programs that Newmont has used to successfully grow the company through development of major foreign mining operations.

Tunnels and underground projects are inherently more risky than vertical projects, which typically utilize more conventional design and construction approaches and are undertaken in the context of relatively ascertained and defined conditions. On tunnel and under-ground projects, there are a host of special risk factors, including unknowns and uncertainties as to the physical and behavioral characteristics of the ground, the inextricable interdependence and necessary interaction of design decisions and construction means and methods with those ground conditions as well as the manner in which risks are allocated among project participants for un-anticipated subsurface conditions. Public-private partnerships (PPP) projects, in their own respects (and independent of any major subsurface component), pose significant risk for all private sector participants. The main driver of owners to procure projects using PPP is financial, as often all, or substantially all, of the design and construction risk are transferred to the private sector consortium, including the design-build (D-B) team. For a concerning number of those public owners, that risk transfer regime includes rather onerous and aggressive contractual terms that allocate to the private sector participants substantially all risks associated with the encountering of unanticipated subsurface conditions. These aggressive risk allocation provisions, while directly impacting project participants upstream of the consulting engineer, have an indirect and corresponding risk intensifying effect upon consulting engineer professional liability exposure. There are many factors that result in increased professional liability exposure for consulting engineers involved in PPP projects. The total number of tailings accidents has reduced from over 50/decade in the 1960s-1980s to less than 30/decade in the 2000s to 2010s. However, the rate of significant tailings dam failures has not been reduced. The significant tailings dam failures the mining industry experienced in the recent history include: Merriespruit, South Africa, 1994; Los Frails, Spain, 1998; Kolontár, Hungry, 2010; Mount Polley, Canada, 2014; and Samarco, Brazil, 2015. The conventional Factor of Safety approach itself may not be enough to further reduce the rate of significant tailings failures due to its limitations such as uncertainty. For example, the same factor of safety could have a vast range of annual probability of failure for different tailings dams, and insufficient consideration of failure consequence. This paper presents a risk based approach for design with the objective being to minimise tailings dam failures, especially those significant tailings failures with high consequence and low probability. This approach includes risk based design and risk management using bowtie tools. In the risk based design, quantitative analysis will be used to estimate the annual probability of failure and the tolerable failure criteria is linked to the consequence category. Bowtie analyses will be used to understand hazards and risks and identify and select independent controls that are effective in preventing and mitigating catastrophic failures. The safety of persons from accidental explosions and fires during manufacture, storage, transport etc. of explosives, pyrotechnics, propellants, firework and ammunition is most commonly assessed according to prescriptive rule-based approach: Safety is given when all requirements which are listed in numerous regulations and directives (such as e.g. safety distances, limited quantities, protective installations etc.) are complied with. Such a deterministic prescriptive rule-based approach is quite easy to apply but has got disadvantages, however: The actual state of safety in a specific case is not explicitly and quantitatively expressed, and it is not distinguished between the endangered individuals and the whole group. As a result, the effects of the safety measures cannot be measured. This is why uneconomical and in the worst case useless safety precautions cannot be avoided. In addition, waivers are difficult to negotiate, meaningful and sound arguments for pro and con being lacking.

This study considers realistic data for a planned open pit iron ore mine, but is applicable to any open pit situation. By interpolating drill hole data, grades are generated for a rectangular block model. Each blockÆs grade designation vector has components for each analyte (chemical element or compound) influencing ore value. Interpolation by means of geostatistical kriging leads to each block being assigned its expected value, and thus underestimates the overall grade variability. Alternatively, interpolation by means of conditional simulation is a method that implements random sampling from an infinite population of solutions. Each conditional simulation has appropriate overall grade variability, but estimating any blockÆs mean and variance requires sampling from multiple conditional simulations. For a block model, an ore/waste selection criterion maximises the expected tonnage at a target grade. This criterion is a linear composite of the grade components, with positive coefficients for the beneficial analyte (iron) and negative coefficients for the deleterious analytes (such as silica, alumina and phosphorus).Although conditional simulation gives the same expected grade as kriging for each block, the expected maximum tonnage of ore selectable at a target grade may differ from that obtainable from the kriged solution. We apply the linear composite selection criterion to each of 25 conditional simulations, as well as to the kriged block model. The resulting distribution of product tonnage is confirmed to have an expected tonnage that is over 20 per cent greater than that of the kriged model. The method also enables a selection probability to be computed for each block, and thus a probabilistic pit boundary distribution to be identified and used in mine planning. Proposed extensions will consider risk-based scheduling of the multiple selection solutions through minimisation of a derived æstress factorÆ and treating the mining process as an iterative system with actual or artificial ædepletionsÆ modelled in line with the mine plan, using the updated state (with new information) to re-evaluate the mine plan for subsequent periods.CITATION:Everett, J E and Grobler, F, 2013. Fiji is a developing nation that relies on economic opportunities and recently experienced increased interest in mining and mineral exploration that could potentially benefit the country. In order to understand the lack of, or failure of, past rehabilitation activity, this study applied risk-based approaches to identify the significance of activities or actions applicable to rehabilitation of post-mined land in Fiji. This involved the use of qualitative, semi-quantitative and quantitative risk methodologies, which allow assessment and evaluation of mechanisms of mining administration and practices, as well as rehabilitation methods and approaches that have been applied successfully to post-mined land in other countries with similar environmental and climatic conditions and social and regulatory environments to Fiji. The semi-quantitative risk-based technique followed an earlier approach applied for mine closure by Laurence (2006) and provided a systematic way to assess and evaluate rehabilitation issues that were identified on-site by assessors, who were able to estimate through expert judgement the likelihood and consequences based on conditions, situations observed and circumstances on-site. It demonstrated that experienced professionals could reproducibly score likelihood and consequence of selected items relevant to mine site rehabilitation, despite the wide-ranging issues identified during post mined land rehabilitation. The quantitative risk-based approach was adopted as a method to determine environmental bonds for mining projects using the assessment of consequences and likelihood results from the semi-quantitative approach, then incorporating unit cost of materials or activities for local conditions required to fully rehabilitate each issue at net present value. Certain degrees of safety, economic, and financial risk are implicit in any mining operation. At Thabazimbi Mine, slope stability was one of the major sources of risk, largely due to data uncertainties. Consequently, a risk-based approach in slope design was followed. On 6 June 2015, a slope failure occurred in Thabazimbi Mine’s Kumba pit, involving approximately 65 million tonnes of rock failing into the pit and the valley below. Although the failed mass moved rapidly and resulted in a run-out distance of approximately 900 m, controls had been put in place to successfully evacuate all personnel. Risk assessment during the onset of failure supported the decision to not compromise the safety of personnel in any way, leading to unavoidable equipment loss. This contribution presents the failure as a case study in the context of a risk-based approach to slope design as well as the importance of implementing geotechnical controls to effectively manage slope instability risk. Furthermore, a comprehensive back-analysis was performed, on a fully constrained 3D model, utilising 3DEC software, to gain insights into the failure mechanism. In conclusion, lessons from the Thabazimbi slope failure are valuable as they demonstrate the importance of following a risk-based approach in slope design to effectively manage safety and financial risk in open pit mines.

This paper presents an application of the newly developed risk-cost-benefit (RCB) framework to support risk management decisions for the selection of wall monitoring technologies at an open cut mine site. The RCB framework quantitatively assessed the risk of unstable slope-induced wall failure and its impact on both loss of life and coal recovery. The RCB framework was utilised in the selection of an optimum group of risk-controls to effectively manage the risk of wall failure to ensure safety and optimal production occurred. The RCB framework incorporates mathematical modules that integrated RCB calculations and risk-control effectiveness assessments to determine the risk reduction and financial impact of the examined safety initiatives.This paper provides an overview of the five-staged RCB framework, incorporating the RCB decision support tools, RCBGEN (Risk-Cost-Benefit Generator) and RCEMETHOD (Risk-Control Effectiveness Method), and demonstrates its application to slope stability risk management at the case study site. The RCB charts are designed to display a balance between risk, cost and benefit for easy interpretation by decision makers to support the objective selection of an optimum suite of risk-controls. The RCB analysis revealed that a combination of ævisual inspections, automatic extensometers and radarÆ for fast- and slow-wall failures with structure were sufficient to reduce the probability of loss of life (PLoL) to the æModerateÆ zone as defined by the Australian Geomechanics Society (AGS, 2007) for the case study site. The new framework presented in this paper provides a method to effectively determine the suitability of risk-controls and supports industry efforts to reduce risk to as low as reasonably practicable (ALARP).CITATION:Kizil, G V, Bye, A and Joy, J, 2012. The design of tailings containment structures relies on well-understood engineering methods being applied consistently. Unfortunately, not all facilities are designed or managed to the same level of competency. To standardize procedures, the Mining Association of Canada (MAC, 1998) prepared a “Guide to Tailings Management.” It recommended review of designs and an assessment of the risks associated with tailings impoundments be implemented during all phases of amine life. This paper describes how the quantitative risk assessment (QRA) process can be used to define risks associated with the design, construction and operation of a tailings management facility. And it discusses how, once the risk tolerability curves (RTC) are defined for a project, they can assist the risk management (RM) process.

Australia has an enviable global reputation as a leader in health and safety outcomes in the hazardous industry of mining, in part due to an increased application of risk management regarding operational planning and mining practice. In 2010, the Australian Coal Association Research Program (ACARP) initiated a major project, RISKGATE, to build, capture and deliver a national body of knowledge to assist companies with the management of risk in coal mining. The online system contains information for tyres, collisions, fires, isolation, strata underground, ground control open cut, explosions, explosives underground, explosives open cut, manual tasks, slips/trips/falls, outbursts, coal bumps and bursts, human–machine interface, tailings dams, occupational hygiene and inrush. In 2014, RISKGATE is developing content for fitness for work (fatigue, drugs, alcohol, physical well-being, psychosocial well-being).This paper analyses underground mining fatalities in Australia and the United States since 2002 and introduces RISKGATE with a focus on the knowledge base for management of hazards in underground operations. Through capturing coal industry expert operational knowledge, RISKGATE provides a cumulative corporate memory at a time of high personnel turnover within the mining industry. A study was undertaken to investigate the relationship between the quantity and value of economic demonstrated resources discovered for 12 mineral commodities over a ten year period and the expenditure required to yield such mineable resources. The results showed that for every dollar spent on exploration, there is an apparent in-ground average value of around $54 dollars contained in the economic resources found. While this appears to be a satisfactory return, it should be remembered that more than 99.9% of exploration programs are judged to be failures because a viable ore body or mineral deposit was not proved up. What is behind this seeming paradox?   The fundamental truth behind mineral exploration is the fact that the more you explore, the more you find, and that one success will often lead to further discoveries. It appears that one rich and viable deposit creates such surplus that its inherent high value ('the pot of gold') makes up for all the failures. In short, mineral exploration is a high risk business with uncertain returns. However, with skill, enthusiasm, experience and venture capital, it can yield larger than normal returns for the brave and the fortunate.   Analysis of exploration expenditure and economic resources discovered for the period 1987 to 1996 revealed that the apparent in-ground value of economic ore discovered per dollar of exploration expenditure varied from a high of $429 for iron ore and $261 for bauxite to a low of $22 for gold and for lead, and $5 per dollar of diamond exploration expenditure. Preliminary conclusions drawn on the reasons for the variations observed include the inherent ease or difficulty of discovery for different mineral commodities which may in some cases, relate to the natural prospectivity of Australia as a whole. Layered deposits like iron ore and bauxite appear to yield higher in-ground value returns on the exploration dollar. To some explorationists, this may be considered as seemingly obvious, but it may have more to do with very large-exploration tenement grants pursued by various Australian State Governments in the early 1950s and 1960s than the apparent ease of delineating a stratabound deposit.   Despite the apparent relatively low in-ground value produced by the gold exploration dollar of $22, the 5660 tonnes of gold discovered in this ten year period was worth $9.1 billion and representing over a quarter of the new mineral wealth created in the ten years to 1996. This contribution, along with that from the other minerals discovered over the period, led to a significant boost to Australia's export income and standard of living.   It is concluded that, given the nexus between the amount of mineral exploration expenditure and the quantity of new resources likely to be discovered, this knowledge may help policy makers seek to influence the rate of resource discovery in countries like Australia for the benefit of the nation.

The commercially available finite element program Plaxis has a key benefit over other finite element analysis programs in that it has a relatively simple user interface to input and output data. This benefit though may also be it?s weakness in that anyone, regardless of their grounding in geotechnical engineering, may use the program and produce results for design. It is essential, as for all engineering design, that the models are checked by competent engineers. This paper describes the key recurring problems found by the authors when checking Plaxis models, such as the generation of the initial in-situ ground stresses and the modelling of groundwater. Interestingly, the authors have not encountered similar problems when checking finite element models produced using other programs and conclude that this may be due to the higher calibre of modeller needed to use these other programs. The geotechnical industry is in a situation at the moment where senior managers who may not have been trained in finite element modelling are relying on less experienced staff who have had some training in modelling, either at university or post graduation. The quality of this training is variable and all organisations running easy to use programs need to be aware of the potential risks and dangers. Management procedures and engineering checks that are essential to validate the results of these models are described, assuming that those in charge are not experienced modellers themselves. The model checking process by competent persons is absolutely essential and it is hoped that this paper will help those organisations that do not have such procedures as part of their own internal Quality Assurance or design validation system. The procedures described are valid for all finite element programs.

"The Riyadh Metro, commissioned by Arriyadh Development Authority “ADA” - the executive arm of The High Commission for the Development of Arriyadh -, forms part of a new public transport-system of Saudi Arabia’s fast growing capital city. The Design & Build project consists of 176 km of Metro Line with 57 km of tunnels. Beyond the Design and Construction, an additional aim of the ADA is to introduce up-to-date methodologies and improve safety in construction projects. A systematic dilapidation survey of buildings and infrastructures was a prerequisite for designing the required instrumentation and monitoring tools. Site investigations required of existing methodologies and the use of geophysical methods for achieving reliable results. Large diameter TBM tunneling is challenging in hot weather conditions and karstic ground. The present paper shows how modern technologies for underground construction are introduced into the local industry and gives an outlook on future projects in the Kingdom of Saudi Arabia. INTRODUCTION Riyadh, the capital city of the Kingdom of Saudi Arabia, is a fast growing city with more than 5.7 million inhabitants. The growth of the city is linked to the recent history of the Kingdom, where some initial milestones from the 1950’s can be highlighted: Revenue generation from the exportation of oil from the Kingdom; downstream investment of oil revenue in the reconstruction and development of the Kingdom; a general plan for the settlement of the Bedouins; the provision of a public electricity network for the city; the opening of the Riyadh - Dammam railway corridor providing a link from the capital to the Eastern Region; the commissioning of Riyadh airport (converted to Riyadh airbase, after the opening of King Khalid International Airport in 1983); and the relocation of ministries and other central government agencies in 1955. In 1974, The High Commission for the Development of Arriyadh was established as the organizational, planning, executive and coordinating body responsible for the development of Riyadh. Arriyadh Development Authority “ADA”, established in 1983 is the High Commission’s executive, technical and administrative arm. The ADA is responsible for implementing the High Commission's organizational, planning, executive and coordinating decisions and missions. The ADA’s president is a member of the High Commission (ADA 2015). One of the remits of the ADA is the preparation and execution of a transport plan for the city of Riyadh."

The eightieth regular meeting and convention was called to order by the President, John T. Atkins, at 9:15 a.m. Monday, June 25, 1984 in the Colorado Ballroom of the Marriott's Mark Resort in Vail, Colorado. President Atkins welcomed the members and their guests to the convention and noted that the annual business meeting would be held Tuesday afternoon at 2:00 p.m. in the same room. A convention update was made by the Conven¬tion Liaison Officer, Loren V. Linville. This was followed with a program report by the Program Chairman, Doug Ingram. The chair then called on Clifford Barrett, Upper Colorado Regional Director of the Bureau of Rec¬lamation of the United States Department of the Interior. He presented the Bureau's Citizen Award to RMCMI former president Ira E. McKeever for his outstanding contributions to the joint private/ Federal effort and the open planning process for AQUATRAIN. According to Barrett, "from 1981 to early 1984, McKeever led private industry work in the AQUATRAIN project, a proposal for a 1,400 mile long pipeline system. AQUATRAIN was conceived as a possible alternative to solve the government's problem of rising salinity levels in the Colorado river, as well as the private sector's need for a more economical coal transport method. The Citizen Award cites McKeever's efforts in developing a model for future cooperative ventures between government and industry that can benefit the nation." Following this presentation, the formal program of the 80th meeting and convention began with the president, John Atkins, introducing the convention kickoff speaker, Andrew Franklin, President of Nerco Mining Co., Inc., followed by the convention keynote speaker, Dr. Margaret N. Maxey of the University of Texas. These speeches are printed in this Proceedings together with those of all other convention speakers. I'm Andrew Franklin, President of Nerco Min¬ing Company, and I'm delighted to be here to kick off the annual convention of the Rocky Mountain Coal Mining Institute. This has always been a very special convention in my eyes and I'm sure this year's will be no exception. I am also here to share my thoughts on the western coal industry and how we're all affected - for good and bad - by public policy decisions. Although the title of my talk is "Western Coal: An Industry Under Siege," I believe a more accurate title might be "Western Coal: The Pit and The Pendulum," because of the wide swings in public policy and coal development in the past 15 years which have cut a zigzag path for growth over this period. As one of the nation's largest coal producers, Nerco is well aware of the problems facing western coal production. Nerco Mining Company is the western coal mining subsidiary of Nerco Inc. We have been "in coal" for more than 25 years through our parent company, Pacific Power & Light. To state my conclusion at the outset, I feel it's extremely important that consistency and common sense should guide our national policy for the development of valuable energy resources. In particu¬lar, we must reduce the wide swings in government policy and activities which have affected energy development. In short, we must decide what kind of game we're playing so that the "players" and the "referees" can keep their eyes on the "ball." Business today faces two main types of uncertainty market uncertainty and public policy uncertainty. As businessmen, we expect and are used to facing the familiar challenges and uncertainties of the market in which we operate. Over the past 10 years, the market has appeared to be bullish on coal. Pictured on these pages are the seven RMCMI Scholarship Award Winners for the 1992-93 year followed by a brief profile. All winners were present as guests of the Institute at the June convention in Beaver Creek, Colorado to receive recognition at the Institute Awards Luncheon Monday, June 28. 1993. Each state vice president served as the host for the collegiate winner. In addition to receiving a plaque from the Scholarship Chairman, Alan Czarnowsky, each winner receives $750 tuition credit for the coming academic year. As this is a continuing award, the winners from the previous year will receive another $750 for tuition credit in their senior year if they have maintained good academic standing and are recommended by their university. A resident from each of the eight RMCMI member states is selected from among Board approved institutions. Criteria for these awards to be made in the spring annually is (1) That the award be made to a sophomore student and used as tuition during the junior year. (2) That the recipient be both a U.S. citizen and a resident of the member state. The transport network in Europe is probably one of the most important systems for the European economy and society. Trans-national transport routes play a vital role in the traffic of goods and the supply of the population. In this respect, critical road infrastructures, like bridges, tunnels or control centres, can have a bottleneck function. Any disturbance of these structures could lead to negative consequences for the population and the economy as a whole.Project SecMan – Security Manual – is a simple four-step procedure for the identification of critical road infrastructures; assessment of these infrastructures regarding various man-made threats and the determination of effective protection measures was developed. These methodologies are summarised and combined into a comprehensive best-practice manual that allows for a trans-national structured and holistic security-risk-management approach for owners and operators of road infrastructures in Europe.Currently no common processes, methodologies or standards exist in Europe regarding the consideration of security relevant events for critical road transport infrastructures, eg tunnels, control centres and bridges. Attacks on the former can have severe short- and long-term effects with high media attention. A trans-national risk management approach would increase the possibilities for prevention as well as mitigation. A trans-national emergency response strategy would increase preparedness, as well as facilitating and accelerating the reaction in the case of an event.The main objective of the project is to develop a practical process for the identification of critical road infrastructures based on generally accepted security aspects. "IT is indeed a pleasure to have the opportunity of addressing this group today. Not only does it give me the opportunity of meeting with representatives of what is considered by many to be the most dynamic segment of industry in Canada today, but it also allows me to talk about a subject of para-mount interest to myself, the development of the North, and to explain, if even only briefly, some of our beliefs and hopes in this regard.It is inconceivable that all lands north of the 60th parallel, representing 35 per cent of Canada's total land area, which we believe contain a great natural resource potential, will remain as an undeveloped portion of Canada. The re-sources are there, although the people and private industry are as yet not there in sufficient numbers.It has been estimated that in the west and north of Canada there are 1.6 million cubic miles of sedimentary rocks. Of this figure, 342,-000 are thought to lie in Alberta, 115,000 in British Columbia and about 1 million cubic miles are thought to lie in Canada's northern regions, the bulk in the Arctic Islands. On the basis of apparent volumes of sedimentary rock, we estimate that as much as 13 billion barrels of petroleum may lie beneath the permafrost of the North-west Territories, another 3 billion barrels in the Yukon Territory and 33 billion barrels in the Arctic Islands. This is, indeed, a rich and elusive prize, and one which must be sought with a view to fulfilling demands on the production of petroleum."

WITHOUT transportation, mining would be quite impossible. Any person familiar with the operation of a mine knows full well that from the minute ore is blasted, it is kept moving until it reaches the treatment plant. In the mill, by means of conveyors and elevators, the ore is brought from receiving bins to one machine after another until finally its valuable mineral is separated from the gangue. The finished product must then be shipped -either by boat, rail, or motor trucks to industrial centres, where it undergoes further treatment and transformation into finished commercial goods. Everybody appreciates that the winning of an or-e includes, amongst various operations, the moving of large quantities of material of generally low value. Transportation, therefore, must be considered One of the most important factors in the economics of mining. My assignment, this afternoon, is not to treat of that phase of transportation as it directly affects mining operations, but rather to consider some of the aspects of overland transportation and its influence in bringing about the development of mining camps in Canada. I shall take the liberty of rapidly surveying conditions facing prospectors, mine developers and operators from the time a region is still in a pre-pioneering stage until such time as a mine starts shipping its concentrates.
