"Cutoff walls are the most common seepage improvement measure for levee flood control projects throughout California’s Central Valley. Cutoff walls are widely used because they can be implemented within an existing right of way in either heavily urbanized areas with limited access, or in high-value crop agricultural areas. The most commonly used seepage cutoff wall types are soil-bentonite (SB), soilcement- bentonite (SCB), and cement-bentonite (CB). Contract values for cutoff wall rehabilitation projects in the Central Valley range from several millions of dollars to hundreds of millions of dollars, and play a significant role in the reduction of flood risk in urban and rural areas. However, recentlycompleted construction of several levee improvement projects has shown limitations and inconsistencies among the commonly used technical specifications for construction of seepage cutoff walls. These limitations were evaluated by practicing engineers involved in different stages of cutoff wall design and construction. This paper describes the specification areas that typically generate design and construction issues. Recommended improvements to specifications are suggested to address these issues. The paper’s topics include: the applicability of the different types of cutoff wall materials and construction methods, quality control testing requirements, backfill gradation requirements, mixing procedures, and settlement criteria for backfill.INTRODUCTIONFlood control projects on the Sacramento and San Joaquin rivers in the Central Valley region of Northern California consist of a large system of reservoirs, bypasses, and levees. The thousands of miles of levees within the system were largely constructed and raised by local entities as need arose; this construction occurred prior to the development of modern design and construction techniques. The majority of these levees were initially constructed to protect land for agricultural development; however, a significant portion of these areas have become or will soon be urbanized. This urbanization has historically occurred in locations right up to the levee footprint. This expedient form of historical levee construction, when combined with adverse riverine geomorphology, urban development, and productive agricultural regions, has led to a levee systems with severe seepage performance issues in a real-estate-constrained setting."

"Due to the way drilled deep foundations are being constructed, their structural integrity and geotechnical capacity are highly variable. Drop weight systems are increasingly being used to dynamically test the capacity and integrity of drilled deep foundations. Conventional pile driving hammers are often either inadequate or not economical to test these foundations and drop weight systems have therefore been developed to allow for dynamic testing.The advantages of the tests include high mobility, short testing time, low cost relative to static load tests (allowing multiple tests at a single site), and integrity/construction quality evaluation concurrent with capacity determination. The dynamic testing method for drilled deep foundations encounter several difficulties including: the need for a pre-testing analysis for matching pile, soil and testing system, adequate mass and drop height to reflect the mobilized capacity without structural damage, test interpretations for irregularly shaped and/or non-uniform piles, need for reliable dynamic measurements depending on the impact and the distance between the impact and the measuring point, and limited knowledge about the accuracy of the predictions.The principles of the tests are presented using simplified models and demonstrated via a case history. Drop weight systems are reviewed. Dynamic measurement principles are explained and the construction and testing quality effects are outlined and demonstrated via numerical modeling and case histories. A comparison of dynamic and static tests is presented via a database, case histories and finally a short review of relevant codes is provided.1 IntroductionTraditional static axial load test is performed by a slow application of a force produced against independent reaction, imitating structural loading. It is the most reliable method to determine the pile’s performance as commonly required under typical service conditions. The major limitations of this testing are: (i) high cost associated with set-up, test duration, interpretation and construction delays, and (ii) inability to obtain information about the pile-soil interaction along the pile without additional testing means (e.g. tell tales, strain gauges, etc.). These limitations are acute when high capacity foundations are involved."

"SynopsisAlthough women all over the world have been involved in mining activities for centuries, mining has always been considered a very masculine industry due to its heavily male-dominated workforce as well as the physicality of mining work. The mining industry has not been an obvious career choice and preferred place of employment for women; women were mainly employed in administrative and advisory positions. Until 1994, women were legislatively prohibited from being employed in underground operations in South Africa, but the Mines Health and Safety Act, No. 29 of 1996, removed these restrictions. In addition, new mining legislation (the Mineral and Petroleum Resources Development Act, No. 28 of 2002) and the accompanying Mining Charter make specific provisions for the inclusion of women in core mining activities and require 10% of core positions to be filled by women.This article voices perceptions of the physical ability of women employed in core mining positions. Findings are drawn from empirical work undertaken at platinum, phosphate, and copper mines. Quantitative and qualitative research paradigms are used. It is evident that women find it extremely difficult to perform mine work that requires physical strength and stamina. Practical recommendations, informed by the literature review and empirical findings, are made with the objective of contributing to the sustainable deployment of women in the mining industry.IntroductionFor many years, mining has been considered the foundation of the South African economy. Although the mining industry in South Africa is currently under considerable pressure and experiences various challenges, including escalating operational costs, electricity tariff increases, safety-related issues and associated production stoppages, poor productivity, labour unrest, and reduced demand both globally and domestically, it remains a key contributor to the national economy and development of the country (IDC, 2013, p. 9).Until 1994, women were legislatively prohibited from meaningful participation in the mining industry and were denied access to skills and jobs, self-employment, and entrepreneurship. The newly elected democratic government in 1994 initiated substantial socio-political and economic transformation in South Africa. Nearly every sector in the country was transformed and re-shaped, primarily through sector-specific legislation, but also through negotiations between government, industry, and labour unions to create and refine the so-called sector charters, which included the Mining Charter. The vision of the new regime, the African National Congress (ANC), was to ‘transfer power to the people and transform society into a non-racial, non-sexist, united, democratic one, and change the manner in which wealth is shared, in order to benefit all the people’ (ANC, 2007)."

"IntroductionDynamic failure events in an underground coal mine, or “bumps,” are defined as “the sudden, violent bursts of coal from a pillar or pillars or a block of coal, resulting in a section, the whole pillars, or the solid block of coal being thrown into an open entry” (Peng, 2008). Reports of disastrous and often fatal dynamic failure events date back over one hundred years in the United States. Mining practices and technologies have significantly evolved over the course of the last century, yet these events continue to occur. The events at Crandall Canyon, Utah (Gates et al., 2007) and Brody No. 1 Mine in West Virginia (Barker and McNeely, 2014) are two recent failure events that resulted in a total of eleven fatalities. These events testify to the fact that dynamic failure remains an imperative safety concern. Furthermore, their continued occurrences indicate that engineering controls have proven inadequate at wholly mitigating the problem.Multiple conditions have been associated with the occurrence of dynamic failure phenomena, including• thick, competent strata that can create a bridging effect, resulting in high abutment stresses (Rice, 1935; Holland and Thomas, 1954; Iannacchione and Zelanko, 1995; Agapito and Goodrich, 2000; Peng, 2008; Whyatt, 2008; Whyatt and Varley, 2010)• overburden thicknesses greater than 500–700 ft. (Rice, 1935; Peng, 2008)• a strong coal that is resistant to crushing (Rice, 1935; Peng,2008) or that is “uncleated or poorly cleated, strong…sustains high stress and tends to fail suddenly” (Agapito and Goodrich, 2000)• the presence of sandstone channels or rolls that can serve to concentrate stresses (Iannachione and Zelanko, 1995; Agapito and Goodrich, 2000)• fracturing of strong units above or below the coal seam (Whyatt and Varley, 2010)• slip along pre-existing discontinuities (Peperakis, 1958; Whyatt and Varley, 2010)• multiple seam mining interactions (Campoli, Kertis, and Goode, 1987; Iannachione and Zelanko, 1995; Newman, 2002; Peng, 2008)• mining sequences that can cause anomalously high stress concentrations (Campoli, Kertis, and Goode, 1987; Iannachione and Zelanko, 1995)"

"This paper, an update to the 2003 paper titled "Knowledge, Technology and Profit" (Twigge-Molecey, 2003) analyzes 45 "problem projects" from the mining and metals industry over the last eleven years representing $133 billion in CAPEX. The projects have been designated as "problem projects" as they did not meet their owner’s initial expectations for project performance as defined in the paper. This paper explores some of the challenges that were present, through interviews with the knowledgeable individuals that were familiar with the projects and investigations using publically available information. The aim of the exercise was to identify the key challenges faced by the projects, and the contributing factors that led to the project failures so that the lessons can be applied to future projects.The project challenges have been categorized into four themes, technical, organizational, environmental & social and missing "homework" prior to board approval. Of noteworthy interest is that environmental permitting aspects have risen to become a significant and notable contributor to problem projects in 11 years from 2003 to 2014. It is further observed that many projects have been challenged by increasing size and complexity and with execution in more remote locations since the previous paper.It is a reality that every project will face challenges. Some projects are simply more challenging than others, and success or failure of the project is often determined by whether the project team is fully prepared to respond to the challenges. This in turn is largely dependent on whether the project "homework" was adequately completed and considered for all aspects of the project."

"Orway Mineral Consultants Ltd (OMC) as a consultancy has specialized in comminution circuit design for over 30 years. The OMC Power Model is OMC’s in-house software for specific grinding energy power-based circuit calculations. The model uses ore hardness parameters from testwork, circuit configuration, equipment geometry and operating conditions as inputs. The model combines proprietary and published power models to predict the total specific energy and the specific energy for each of the components of the comminution circuit. This makes the software a valuable tool for the design and validation of JKSimMet modelling or actual plant performance. Proprietary calculations have been developed and reconciled with over 100 plant benchmark projects encompassing a variety of comminution circuit configurations.This paper describes the OMC approach for determining power requirements for the design of grinding circuits. The accuracy of the OMC Power Model software is confirmed through plant survey benchmark performance and comparisons are made with other power modelling techniques. Practical examples are presented, showing applications for single stage SAG (SS-SAG), SAB and SABC circuit configurations.IntroductionConventional low throughput grinding options, consisting of multi-stage crushing followed by rod and ball mills, have almost become obsolete in the move to higher milling rates required to process lower metal grades. Grinding circuits with fully autogenous (AG) or semi-autogenous (SAG) mills, often combined with ball mills are now the standard to meet the high capacity requirement for processing ores with lower metal grades. Simulation software and power-based equation methodologies have been established for designing AG/SAG based grinding circuits and predicting their energy requirements.Presently, power-based equations remain the principal method for designers to size mills and estimate the power requirement for comminution circuits. The technique is simple and it relies on the design to specify a feed and product size plus a hardness index to produce a specific energy value (kWh/t). The desired throughput of the mill is multiplied by the specific energy to give the mill power requirement, which is used in turn to size a mill for dimensions, internal geometry and loading conditions to match the power draw."

"Lakeshore Gold’s Bell Creek Mill is located in Timmins, Ontario. The mill has been in operation since 2009 and has undergone several small expansions since then. In 2012 - 2013 the first major mill expansion was commissioned raising the mill capacity from 2,000tpd to 3,150tpd. This was done through the installation of a new ROM ore truck dumping facility, jaw crusher, ore storage dome, SAG mill, leach tank, CIL tank, and electrowinning cell. This paper will cover the equipment installed to raise the tonnage as well as the commissioning of the new equipment including problems encountered and how these problems were solved.IntroductionLake Shore Gold acquired the Bell Creek Mill, along with the Bell Creek Mine, from the Porcupine Gold Mines Joint Venture, a joint venture between Goldcorp Canada Ltd. and Kinross Gold Corporation, in December 2007.The Bell Creek mill, which processed ore from the Bell Creek Mine has consistently achieved recoveries of over 95%, is a conventional gold mill circuit, involving crushing and two stage ball mill grinding, gravity and leaching, followed by Carbon-In-Pulp (CIP) process for gold recovery. The facility had been placed on care and maintenance in 2002. At the beginning of 2009, the mill had been refurbished with an initial capacity of 800 tonnes per day and was subsequently refurbished to a capacity of 1,500 tonnes per day near the end of the third quarter of that year. The Mill was then expanded in 2010 to a capacity of 2,000 tonnes per day.In late 2011, the Company commenced a 50% expansion of its milling facility, from a processing capacity of 2,000 tonnes per day to 3,150 tonnes per day. Phase 1 of the expansion, which mainly involved enhancements to the solution circuit (“back end”) and increased the mill’s capacity to 2,500 tonnes per day, was completed at the end of 2012. Commissioning of Phase 2 of the expansion, to a capacity of 3,150 tonnes per day, commenced at the end of July 2013."

"The Meuse/Haute-Marne Underground Research Laboratory (URL) is located on the eastern boundary of the Paris Basin. The URL started to be built in 2000 (shaft sinking operations) in the framework of ANDRA’s (French national radioactive waste management agency) research program aimed at determining the feasibility of a reversible deep geological disposal of high-level and intermediate-level long-lived radioactive waste (HLW, IL-LLW). Its underground drifts are used to study the Callovo- Oxfordian claystone layer between 420 m and 550 m in depth.The ongoing research program (started in 2006) following the “Technology Readiness Level” (TRL) Scale is more dedicated to technologies improvement and demonstration issue of the different disposal components, even if characterization and monitoring studies are still ongoing.In rock mechanics, at the main level of the URL (located at 490 m), construction and evolution of disposal cells of different size are studied for HLW and IL-LLW packages. A step by step approach, started in 2006, is carried out based on comparison of HM behavior of parallel drifts excavated/supported by different construction methods. These various configurations give insight of the influence of construction method on the EDZ extent and evolution and on the progressive loading of the support, which are key issues to design the Cigéo disposal drifts and excavations.The concept of disposal cells studied for HLW consists in horizontal steel lined micro-tunnels 80 m long and 70 cm in diameter. Several campaigns of tests have been carried out since 2009 in order to demonstrate the feasibility of excavating such micro-tunnels and study their behavior under different conditions (with and without back filling the annular gap between the rock and the casing, watertight or not casing). A full scale demonstrator representative of the benchmark concept has also been excavated, to study its behaviour under thermal loading with heat produced by electrical heaters."

"According to the characteristics of rock mass and the directional cutting technology of water jet, a new idea of light shock controlled blasting with water jet slotting was put forward. Based on the fracture mechanics theory and the stress wave theory, the mechanism of damping with water jet slotting was analyzed. On the basis of the above, the evolution law of the pressure wave and displacement field during the blasting were simulated by FEM software ANSYS/LS-DYNA. The result shows that: the oriented slot plays a good role in orientation during the blasting, and makes the energy generated by gas released by the explosives gather at the tip of the oriented slot, which makes it easier for the main crack to expand along the oriented slot and indirectly reduce the stress effect on the protected object. Meanwhile, the energy of the seismic wave is reduced while transmitting through damping slot. Finally, compared with the normal blasting with damping ditch, the maximum acceleration is reduced by 15.62%, the maximum velocity is reduced by 52.99%, and the maximum displacement is reduced by 68.32%. INTRODUCTIONIn recent years, with increasing demand for energy and space, an increasing number of underground projects have been under construction. Drilling-blasting method, as a traditional main method for rock excavation, is still an indispensable means in the engineering construction. As is known, blasting vibration will cause the deformation and unstable of the existing building structures, so how to reduce the influence on the existing building structures caused by the blasting vibration has become the focus. Traditional shock absorption measures mainly include three kinds: the first one is pre-splitting blasting, the second measure is to change the parameter of explosive filling, and the last one is to excavate damping ditch between the protected object and blastholes. However, engineering practices show that: the protected rock may be damaged in the process of pre-splitting crack construction by blasting, the second measure is difficult in operating, and the third measure is troubled by large work quantity (Song Guangming, 1999; Lou Jianwu, 2004; Lou Jianwu, 2005; Li Xibing et al., 2008)."

"What are the basic physical parameters and main geometric parameters that are capable of representing the microscopic characteristics of rock? How can these parameters be used to accurately describe the macroscopic mechanical behavior of rock? With such questions in view, and based on the microcosmic structure analysis of rock slices, this paper presents a rock skeleton-pore structure model, holding that rock's mechanical behavior depends on the properties of rock skeleton materials and the form, size and distribution pattern of pores. According to rock stress and deformation behaviors, and in line with the skeleton-pore structure model, an essential mechanics theory for rock is advanced. The assumption that the skeleton materials are incompressible in volume and an introduction of logarithmic strain, generate the relation between porosity and apparent strain. Then, the notion that positive damage and negative damage are represented in terms of porosity is put forward; threshold value of porosity is derived by the damage threshold value of apparent volume strain; a general damage variable of rock is obtained; eventually, with the skeleton-pore model as the groundwork, a global damage constitutive equation is developed, which, with definite physical significance and material parameters to be easily determined, is convenient to apply. It has been proved that the results calculated with this new equation are well in agreement with experimental results.INTRODUCTIONRock is a form of material that contains pores, cracks and skeletons. Skeletons are composed of particles and cements. Pores or cracks within rock actually undermine the assumption of material continuum in solid mechanics. Hence, grave errors — or even false results — are inevitable if some solid mechanics theory is quoted in the discussion of rock mechanics. Many researchers in the field of rock mechanics have attempted various remedies, but the outcome is far from satisfactory — a mechanics theory for rock, in its true sense, has not been established."

"Mineral deposits require substantial amounts of energy to transport and be processed into useable form, and as such present a real processing challenge in that the deposits are never homogeneous and ore body compositional characteristics are all over the map! Extracting the desired mineral(s) from the deposit at sufficient profit margin, despite severe fluctuations in market price, requires process automation architecture capable of supporting an integrated enterprise solution. Such an architecture will encompasses the activities of individuals, process units, process areas, plant level activities, mine management and business planning and accounting systems. Key to the automation process is the aspect of value generation. This paper brings together the various requirements for the automation of a mining enterprise and presents the different aspects of how to obtain value across the entire mining operation.INTRODUCTIONMinerals’ processing sites are historically long lived, 20 year life of mine is not uncommon and 75 years is not unheard of. Considering that hardware technology advancements are introduced every six months, or less, it is prudent to select process automation systems that adhere to “open” industrial standards versus proprietary based systems. In the minerals processing industry, the key to survival is driving the cost of production per unit volume to the lowest possible level and provide enough profit to sustain operations. Control system architecture and Business information technology are rapidly becoming important tools in the quest for productivity. Getting key dynamic business performance information at the right time to the right person or process unit is crucial to successfully achieving corporate goals. The process automation architecture selected must therefore be able to integrate to the various networking enterprises and business systems.The mining industry is generally known as an “extractive” industry that is very physically-oriented with respect to operations and the activities in which workers engage. Large numbers of people performing manual tasks in difficult surroundings, produce very variable outputs. But, times are changing – and so are industry best practices. While the nature of work still consists of activities such as ore extraction, ore movement, ore concentration, refinement and logistics, automation innovations are starting to change the nature of many of the production steps. The industry has transitioned from requiring more physical activities by workers, to more information management, automated control and collaboration within the operation. Humans are good at situational intervention and non-linear reasoning and do best when empowered with an overall production perspective. Machines and automation are good at repetitive functions, steady state operation, and managing transitions. Thus, a combination of advanced technology solutions, reliable and useful information and full human contribution are necessary to mining operations for value generation."

"Pyhäsalmi Mine in Finland has a long history of continuous process development. The concentrator processes Cu-Zn-sulphide ore with high recoveries. An on-line flotation simulator has been developed to maintain and further improve the flotation performance. Consisting of a flotation kinetic dynamic model, and interconnected with process control software, the simulator can be used for training new control room operators in various scenarios. For daily operation, the model parameters are continuously adapted to take into account the changing process conditions. On-line simulator tracking the process can provide the process operator new information in several ways: a) values of unmeasured process variables are obtained by displaying the corresponding simulator values, b) predictions of future process states and outputs are calculated and displayed, c) information of changing process parameters can be used as an indicator of e.g. change in the processed ore type. This paper describes the adaptive simulation system and its usage at the Pyhäsalmi concentrator.IntroductionPyhäsalmi massive sulphide orebody is located in central Finland. Operation of the mine started in 1962 and it is currently producing copper (Cu), zinc (Zn) and sulphur (S) concentrates. The concentrator control and optimization system has a long history of successful continuous development during the past decades (see e.g. Huuskonen & Moilanen, 2010). An on-stream analyzer has been a part of the automatic process control since 1968 (Lähteenmäki, Miettunen & Saloheimo, 1999). First control algorithms based on a flotation mathematical model were developed in 1971 (Koivistoinen & Miettunen, 1985). In 1990’s the development of a froth machine vision systems started (Kaartinen, 2009; Kaartinen, Roine & Hätönen, 2009). More recently Pyhäsalmi Mine Oy has been involved with several national development projects dealing with particle size distribution measurements (Kaartinen & Tolonen, 2008), spectral analysis of flotation slurries (Haavisto, Kaartinen & Hyötyniemi, 2008), and dynamic simulation of flotation processes. The latter topic is further elaborated in this paper."

"SynopsisThis study proposes a methodology for evaluating the risk of coal bumps in multiple-seam longwall mining. Both the stress field and the total energy release (TER) during retreat were evaluated in the specified case involving multiple-seam mining using the LaModel program. The results of numerical simulations indicated that both the peak vertical stresses on the panel edges and the peak TERs in the outby longwalls increased significantly as the horizontal offsets were reduced from 60 m to zero. With the comprehensive consideration of the stress field and TERs, a conservative offset of 60 m was ultimately adopted when developing the gateroads of the lower panel in the field. The field measurements indicated that coal bumps were avoided completely by employing the proposed design, and the maximum roof-to-floor and rib-to-rib convergences of the tailgate during retreat were only 360 mm and 576 mm, respectively.IntroductionCoal bumps – sudden, violent bursts of coal – pose a serious threat to the safe excavation of coal. Their occurrence depends on the properties of the surrounding rocks, stress field, and dynamic disturbance. The complexity of the mechanism of coal bumps is further enhanced in multiple-seam mining due to seam interaction. Multiple-seam mining is most frequently found in the coalfields in eastern China, such as Xuzhou, Xinwen, Zaozhuang, and Huainan. In the past, due to issues of availability, economics, and ignorance of the potential risk in multipleseam mining, coal seams in such conditions were mined without proper planning to account for seam interaction. Fortunately, studies published over the past three decades have raised awareness of the problems associated with multiple-seam mining, and indicated that the ground control issues induced by multiple-seam mining can be avoided and/or minimized with proper planning.A number of technical studies pertaining to the design of multiple-seam mines have been published. These studies can be divided into two groups based on their methodologies of design: empirical (Chekan and Listak, 1993; Haycocks and Zhou, 1990, Matetic et al.; 1987a, 1987b) and numerical approaches (Li et al., 2000; Maleki, 2007; Mark, 2007; Peng, 2007; Zipf, 2005). The majority of the extant studies on the interaction mechanisms in multiple-seam mining have focused on the load transfer during operation, resulting in the proposal of the pressure bulb theory (Forrest et al., 1987; Haycocks and Karmis, 1983; Peng, 2008; Peng and Chandra, 1980) and pressure arch theory (Holland, 1973; Oram and Ponder, 1997) to guide multiple-seam mine design. However, neither, of these theories can be used to evaluate the potential for coal bumps."

"Ask an average non-industry person on the street “What is coal? What is coal mining? What is a coal miner?” The most common answers are coal is something Santa puts in your stocking for being naughty, coal mining is destroying the earth causing global warming, and a coal miner is a person that uses a pick and shovel to dig coal out of the earth. As farfetched as these answers seem, unfortunately this is the opinion of many Americans. Realizing the lack of educational opportunity regarding coal and coal mining, a first-of-its-kind grassroots coal education program known as CEDAR (Coal Education Development and Resource) was created. What started from a general conversation at the December 1992 North Carolina Coal Institute (NCCI) Winter Meeting at the Mid Pines Club in Southern Pines, North Carolina led to the creation, and later expansion of, CEDAR. NCCI President, Gene McBurney, challenged John Justice, NCCI Board Member from east Kentucky, to come up with an idea of how to improve the image of the coal industry, pledging $5,000 from the NCCI as seed money to assist in the development of any such program. John then met with the leaders of Coal Operators and Associates, Inc., (COA) in Pikeville, KY where they agreed to match the NCCI commitment, as well as to assist in the program’s development. After further meetings with representatives from industry, business, and the local school system, CEDAR, Inc. (of east KY) was born. The first year of implementation, the program offered coal education programs to public and private schools in one county in east Kentucky. Although it started in the coalfields of eastern Kentucky, it certainly did not end there. After an amazingly successful first year of the program, it didn’t take long for industry leaders in the western Kentucky coal-fields to learn about, and take note of this new, unique, and exciting program that provided opportunities for teachers and students to learn, “the rest of the story” about the coal industry. Through the joint efforts of Costain Coal, and the Western Kentucky Coal Association, CEDAR of West Kentucky, Inc. was formed in 1994, and has operated since that time, except for the two year period from 2004-2006. The program restarted in 2006 under the name of CEDAR West, Inc. and continues to be offered in six western Kentucky counties."

"On January 9, 2014, a large chemical spill occurred on the bank of the Elk River near Charleston, WV. Within hours, the potable water supply for more than 300,000 people was contaminated, and the incident captured national headlines for weeks. Although the primary chemical, crude 4-methylcyclohexanemethanol, or MCHM, is deemed nonhazardous, the impacts of the spill are significant – with all stakeholders, including the public, members of industry, and relevant government agencies experiencing losses. This case highlights the need for a more proactive approach to environmental management, shifting from crisis control and remediation to crisis prevention and risk management. INTRODUCTION On the morning of January 9, 2014, approximately 10,000 gallons of crude 4-methylcyclohexanemethanol (MCHM) leaked from a 48,000-gallon storage tank sitting on the bank of the Elk River near Charleston, West Virginia (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal, and is deemed nonhazardous in terms of transportation and storage regulations and also toxicity (CDC, 2014a). Freedom Industries, Inc., a producer and vendor of specialty chemical products, had acquired the storage facility where the spill occurred from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein & McWhirter, 2014). The spill site was located just over a mile upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a “Do Not Use” order was issued for all customers (WVAW, 2014a). With little information readily available regarding the potential human health effects of MCHM, the US Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the “Do Not Use” order remained in effect for a total of eight days, until January 17, when the WVAW distribution water was confirmed to have MCHM concentrations below this level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at concentrations as low as 0.55 ppb by individuals experienced in odor identification (Rosen et al., 2014); and even individuals with no such training can detect pure MCHM at just 7 ppb (WVTAP, 2014). The fact that this chemical can be sensed at such low levels – indeed far below those considered safe – undoubtedly added to growing public concern. In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until February 25 when MCHM levels were measured below 2 ppb (WVAWb, 2014), and eventually replaced the affected filter media (WVAWc, 2014)."

The wide range of potential feed materials, process steps, and products in metallurgical facilities means that standard equipment might not be available to adequately meet the project technical and economic needs. Custom designed equipment should be considered only after rigorous research precludes purchasing standard equipment. However, the decision to use custom equipment introduces a new element of risk into the project. To manage the risk, custom equipment design should be developed through the same front end loading (FEL) phases as project development. If more than incremental development of custom equipment is required, appropriate test work or pilot trials are required to test key uncertainties. Important activities during custom equipment design include maintaining clearly defined performance objectives, conducting appropriate reviews, carefully crafting commercial and intellectual property agreements with design and fabrication partners, and assembling a multidisciplinary team with sufficient development expertise.

"This paper will share my ‘shift in career direction’ journey from corporate mine engineer, with 31 years of mining various commodities with a wide range of responsibilities in a variety of countries, to the world of academia. There are times in one’s career that a change is required, albeit perhaps not as drastic of a change as moving to teaching. This paper will share how the transition arose, the decision to make the change, the challenges of the first year in academia, and the journey within academia going into the second year. INTRODUCTION As a graduate of the Montana College of Mineral Science and Technology (Montana Tech) with a B.S. Mining Engineering degree in May of 1982, my first job with Utah International in the Farmington area took me back to my home state of New Mexico. Some of you will remember that the early 1980’s was not a period of prolific opportunities for mining engineers; given low commodity prices, especially for copper (for example, Anaconda Company had been taken over by Arco and was in the midst of closing down its Butte operations). Coal was good and the learning opportunities abounded with Utah (which became BHP and then BHP Billiton). Gold was taking off in the mid to late 1980’s thanks to use of cyanide and heap leaching technology improvements, coupled with deregulation of the gold price, so I ventured off to work in the gold industry with a (then) junior Canadian company called Barrick at a new acquisition of theirs known as Goldstrike. That same gold surge was causing a shortage of iron ore mining engineers in Australia so after a few years in Nevada, I uprooted the family and left for a little town in the Pilbara of West Australia, known as Newman, to work in the iron ore industry with BHP Iron Ore. An initial short overseas posting turned into six plus years before transferring to BHP’s Indonesian steam coal mines in Kalimantan on the island of Borneo. BHP was going through a rough period following some ill-fated projects (Magma copper purchase, Beanup and HBI) and was reviewing assets and staffing so internal opportunities became scarce. Employees were encouraged to ‘leave the company and find work elsewhere.’ So, off I go to Argentina to try copper mining in the Andes with Minera Alumbrera. This short stint in South America was followed by a move to another mining town, Morenci, Arizona, and a posting with Phelps Dodge (now Freeport McMoran). After 12+ years without living close to snow we moved to the Denver area in a new position with Newmont Mining Corporation. Along the way I became a Professional Engineer (Nevada) and completed a Masters in Project Engineering and Management. After 11 years with Newmont, I moved to Butte, Montana where I accepted a position as Assistant Professor of Mining Engineering."

"Large subhorizontal joints have been found in the foundation excavations in basalt and granite of Brazilian Dams. Some spread for hundreds of square meters showed slickensides—polished surfaces plus striations—and laminations interpreted as related to shear displacement, what substantially reduced their shear strength and imposed special reinforcement measures against the potential sliding of overlying concrete structures. The geological characteristics of these discontinuities have raised speculations related to their origin, be it related to mechanisms of bulging, cambering or to some tectonic or primary geological event.Several geologists and engineers from the northern hemisphere including Terzaghi already showed to be concerned with their role in granite foundations while in the southern regions Brazilian authors have found similar occurrences but also affecting individual lava flows of continental flood basalts in the foundations of major dams of the Parana Basin, which they designated under the particular name of “fault-joints”. This double-standard designation is self-explanatory of the difficulties involving their origin and related geomechanical properties: faults or just shear joints?Recently, similar features were encountered in the foundations of large dam constructions in anorogenic granites inserted in the realm of the Brazilian Amazon, where some of these discontinuities exhibit features common to tectonic thrusting combined with deep tropical weathering.The evidence of preshearing along the surfaces of such fractures, also associated to products of deep weathering, reduces their geomechanical strength substantially, suggesting values of ultimate phi spanning 12 to 28°. The detection of these descontinuities during initial investigation phases continues to be a major complex task in geological risk evaluations surveys. Trenching associated to some geophysical exploration techniques, although considered to be very basic exploration techniques are still the suggested methods for their forecast and previous determination."

"The International Organization of Standards defines risk as “the effect of uncertainties on achieving objectives (15).” In mining, many uncertainties can influence the outcome of personal or professional objectives and are often difficult to predict. Risk management, the minimization and control of adverse effects from exposure to identified risks, is a well-established concept that can summarized as “being smart about taking chances” (12). Risk management now consists of management processes unique to numerous organizations across many industries, as varying sectors encounter a variety of needs, including the analysis and mitigation of risks related to physical security, product liability, information security, various forms of insurance, regulatory compliance, and workplace safety (12). The reoccurrence of multiple fatality events in the US mining industry, especially the underground coal sector, suggests an opportunity for improved methods of major safety hazard identification, assessment, and mitigation. A risk management approach would include ready identification and mitigation of these risks, inclusive stakeholder engagement, and rapid integration. While many solutions to reducing the risk of mine disasters have been proposed including stricter regulation and improved technology, a comprehensive risk management approach has yet to be fully integrated in the US mining industry. In December of 2006, R. Larry Grayson of the National Mining Association’s Mine Safety Technology and Training Commission called for “a new paradigm for ensuring safety in underground coal mines, one that focuses on systematic and comprehensive risk management as the foundation from which all life-safety efforts emanate (7).” A risk management approach has already been implemented with success in Australia’s minerals industry as part of a transition during the mid-1990s from a prescriptive based health and safety attitude to a more proactive, duty-of care [risk-based] philosophy (13, 16, 17).FUNDAMENTAL ELEMENTS OF RISK MANAGEMENT TECHNIQUES The fundamental components of a comprehensive risk assessment/management program are well-established within the realm of mining health and safety. Figure 1 depicts the elements of a typical mining hazard management framework. The first step in any safety-based risk management system is to identify and assess hazards by their location, nature, and magnitude within a mine. Next, a decision is made about choosing to eliminate, mitigate, or tolerate each hazard. Ideally, a hazard will be eliminated during the design stages of mining, though dynamic mining conditions often require continuous improvement and reevaluation of existing hazard controls. If elimination is not possible, hazard mitigation actions can consist of methods, rules, equipment, competencies, or other mechanisms. If a hazard is simply tolerated, as many inherent mining hazards must be, then specialized administrative controls must be utilized to minimize losses. During this process, the performance of each action is monitored and modified as needed (13)."

"Usually, steady state mass balances are used for the design of mineral processing plants. However, this tool does not take into account the impact of process inputs variability nor does it evaluate process transient behaviours between stationary states, such as mill start-up and shutdown. The use of advanced simulations enables the integration of dynamic behaviours in process design decision-making by highlighting situations that are undetectable by using only steady state mass balances. Applying these simulation tools to brown field projects allows the simulator performances to be calibrated and validated on actual plant data before it is used as a prediction tool for the modified plant.Three (3) simulators were implemented at the design stage of a major upgrade project for an ore processing plant in order to assess process behaviours under different production scenarios. These simulators were built to evaluate the impacts on major elements such as mine production, ore stockpile, concentrator production as well as water demand. A validation of individual equipment models was performed whenever sufficient measured data was available. The simulators were then used to study the impact of ore variability (grade, hardness, throughput), to assess day-to-day production special events, and to evaluate the true stockpiles live capacity.The use of a stochastic simulator for equipment sizing allowed a more precise sizing of many pieces of equipment and avoided important expenses in the fresh water pumping system. The dynamic simulator also confirmed that the water in place system would suffice, even in the most demanding production scenario. Finally, the stockpile simulator allowed an important production bottleneck to be quantified and then made possible the assessment of various practical solutions for the stockpiling-reclaiming issues.Post start-up, process data underlines the fact that using advanced simulation tools was useful to mitigate process-related risks associated to the plant expansion, showing generally accurate prediction and adequate equipment sizing, leading to smooth process commissioning. The presented methods are part of what is now routine in Soutex’s participation in both greenfield and brownfield projects."
