Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

This paper focuses on a case study at a base metal mine located in a tropical semi-arid area of Western Australia (WA). It is demonstrated that understanding the geochemical nature of the mine tailings forms an integral part of innovative management strategies developed throughout the life-of-mine (LOM) when planning for mine closure. The tailings are currently stored at an above-ground valley fill tailings storage facility (TSF) and one option for long-term management under consideration is leaving the tailings in situ at mine closure and covering using the concept of an enhanced æstore and releaseÆ cover system.Extensive geochemical and mineralogical studies have found that the geochemical behaviour of the tailings in the TSF is significantly more benign than initially predicted. The sulfide content of the tailings is primarily pyrrhotite, which can be acid-forming, acid-neutral or acid-consuming depending on a number of factors including redox conditions. Tailings pore water at depth is pH neutral with excess alkalinity, low concentrations of dissolved metals and elevated sulfate concentration.Recent geochemical tests on tailings samples indicate that a reduction in the acid potential and a corresponding increase in the neutralising potential of the tailings, through partial removal of pyrrhotite at the ore processing stage, has the potential to further reduce the long-term risk of potential impact to the environment from any TSF seepage. In this paper we follow various technological discoveries and developments in the purification of minerals and metals in the mining industry by ion exchange. For approximately 100 years, ion exchange has been a known and variously applied purification and separation technology used by scientists and producers. It would seem obvious that the producer’s objectives have always been to achieve the highest quality products and the lowest operation costs, but also with the least amount of technological complexity. As a strategy to reach these goals, much effort has been expended in the quest to improve continuous ion exchange (CIX) systems by reducing utility needs and resin degradation or loss while at the same time improving productivity and reliability. Recent developments from a number of companies have pushed equipment boundaries, resulting in a variety of unique commercial equipment designs each with a varying degree of success. The challenge set forth for all new designs has been a truly continuous process with no sacrifice in the afore mentioned benefits. One very recent equipment design based on system symmetry appears to meet many of these objectives with greater flexibility and less complexity resulting in better economics for the producer.

The blast -hole method for drilling and breaking ground is not new to the mining industry . For several years it has been used rather extensively in Canada , particularly in the gold mines , where it was first developed and where the major advances in the technique as applied to present - day mining problems have been made . At present , 40 mines in Canada and the United States are using the method in one form or another , and from all indications its use will increase in the future . In two California mines , blast -hole diamond drilling is now used for a part of or for all stoping operations , and in one of these it is used to some extent in development work . At the Mountain Copper Co.'s Hornet mine in Shasta County, about 50 percent of the pyritic sulfide ore produced is mined by the blast - hole method , and the U. S. Vanadium Corp. mines 80 percent of the ore produced at its Pine Creek tungsten mine in Inyo County by this method . At Pine Creek , long -hole drilling and blasting is used for mining pillars also , and in pre - stoping development . The blast -hole diamond - drilling method is not applicable to all types of ore bodies . A third property in California , the El Dorado Limestone Company , El Dorado County, tried the method for several months in 1943 and 1944 , but , owing to certain factors that greatly increased mining cost , the company soon returned to conventional mining methods .

High Pressure Water Jet Assisted Tunneling The high pressure water jet assisted tunneling project was conducted in a granite quarry near Skykomish, Washington, approximately 90 miles east of Seattle. A 7-ft diam tunnel boring machine was modified and equipped with high pressure water jets. The concept that tunneling rates can be improved by using high pressure water jets was validated, with an average increased advance rate of 50% to 60%. For some tests, the advance rate was more than doubled. A comprehensive economic analysis has shown that very significant savings are possible. At a 50% increase in advance rate, cost savings of 14% and 24% were projected for 10-ft and 20-ft diam tunnels respectively. These benefits were seen to be relatively insensitive to water jet equipment costs.

ABSTRACT: Determining tunnel stability is a key issue during preliminary site investigation. In contrast, problems of excavatability have been largely ignored. While the choice of an economic tunnelling method is admittedly a clear priority in the planning stage, special investigations focus-sing on rock fragmentation (e.g. drilling or cutting performance, rock mass blastability or tool wear) are rarely carried out. This paper explores possibilities to quantify key parameters for rock mass excavatability in drilling, blasting and cutting by TBMs and roadheaders. RÉSUMÉE: La détermination de la stabilité du tunnel est une question clé lors des études prélimi-naires. En revanche, les problèmes d'excavabilité sont souvent en grande partie ignorés. Tandis que le choix d'une méthode de percement économique est évidemment une priorité claire dans l'étape de planification, des investigations spéciales se concentrant sur la fragmentation de la roche (par exemple le comportement de la roche au forage, à l’excavation ou au dynamitage et l’usure des outil) sont rarement effectuées. Silver-lead-zinc replacement deposits occur in limestones of the anticlinal Caja range at Providencia, near Avalos, Zacatecas, Mexico. The description deals with intrusives, outcrops, preferred horizons, changes from bedding to fracture, control in depth, and variations in abundance and ratios of minerals throughout a vertical range of 500 m. THE purpose of this paper is to record a few field observations and accumulated office data concerning outcrops, relation of ore occurrences to intrusive and host rock, and mineral zoning. Reasons for some of the phenomena are not clear to the author so the facts are given with the hope that other investigators may be able to furnish the explanations. The Avalos unit of the Compania Minera de Penoles, S. A., operates the Alicante, Bonanza, Providencia, Albarradon, San Eligio, Nazareno, Leona, Salaverna, and Santiago mines situated between Avalos railroad station and Concepcion del Oro on the Coahuila and Zacatecas railroad in the State of Zacatecas, Mexico. Some of these mines are within Penoles property while others are on mining claims leased from the Compania Nazareno y Catasillas, S. A., Mazapil Copper Co., and the American Smelting and Refining Co. The main office and camp are at Providencia, about 170 km southwest of Monterrey, N. L., so the silver-lead-zinc mines of the district are usually referred to as the Providencia mines. The Aranzazu, Catarroyo, and Cabrestante copper-gold mines are situated a few kilometers to the southeast on the same mountain range near Con- cepcion del Oro, the name by which the camp is known. It is operated by the Mazapil Copper Co. The main theme in the following pages is a description of the silver-lead-zinc ore occurrences of the district. Rock Formations Sedimentary Rocks: Many competent geologists have contributed generously to an ample fund of information concerning the sedimentary columns of northern Mexico. Among them are Carlos Burck-hardt, E. Bose, L. B. Kellum, and R. W. Imlay. Carlos Burckhardt, who covered a number of extensive areas in many parts of the Mexican Republic, devoted years to mapping the geology of the Caja, Zuloaga, and Santa Rosa mountain ranges around Mazapil, Concepcion del Oro, and San Pedro de Ocampo, Zacatecas. Platinum group metals are extracted almost exclusively in South Africa from three very different zones in the Bushveld Igneous Complex, called the Plat, Merensky, and the UG-2 reefs. The Plat reef is currently being exploited only by Anglo American Platinum, whereas the Merensky reef is exploited by them, Impala Platinum, Northam, and Lonplats. The UG-2 reef, on the other hand, is also processed by these producers, but Kroondal and Barplats have operations that exclusively treat ore from this reef. At present the extraction of pgms from ores in South Africa follows the traditional route of upgrading to a concentrate in a minerals processing circuit, further upgrading to a matte in a smelter, and final upgrading and separation of the individual metals in a refinery. Thus all operations that extract pgms require a minerals processing circuit, the design and performance of which is a topic of much interest, as it is a poorly understood and not a very efficient process in relative terms, compared to the other processes. Recoveries across the minerals processing circuit of between 75 and 90% are the norm.

The electrokinetic properties of MgO have been utilized to simultaneously filter suspended solids and remove trace amounts of dissolved heavy metals from synthetic mine water. Greater than 95 pct removal of metals from influent at neutral pH containing 1 to 6 mg/L of dissolved Cd, Cu, Mn, Ni, and Zn can be routinely achieved by filtering through a granular bed of MgO at flow rates typical of rapid sand filtration (0.3 cm/s). Filtrate concentrations of most metals can be maintained below 0.1 mg/L for 160 bed volumes or more loading. The more mobile metals (those with larger solubility products) are more difficult to remove, and are strongly bonded to the MgO granules so that chemical stripping with chelating agent is required to clean the bed. Less mobile ions are easily removed by the filter and are effectively stripped by conventional filter backwashing. The fraction of metals recovered by chemically stripping the loaded MgO filters correlates to the zero points of charge of the metals. This process may be applicable to removal and recovery of trace amounts of heavy metals in mine drainage that is partly neutralized by liming. Also, since many of these metals are critical and strategic, the technique can be adapted not only to recycle process water from mineral and milling waste streams, but also to reclaim their metal values.

ASIDE from the John ,Fritz Medal, in which the Institute participates through its representation on the John Fritz Medal Board, the Institute itself has four awards it may make annually, as follows : the Robert W. Hunt Medal and Prize, the J. E. Johnson, Jr., Award, the James Douglas Medal, and the William Lawrence Saunders Medal. ROBERT W. HUNT MEDAL AND PRIZE The partners of Robert W. Hunt established a fund which was presented to the Institute at a fitting ceremony on May 27, 1920, to establish an annual medal and a sum of money to be awarded under the following rules: 1. The award shall consist of two prizes: first of a gold medal, second of a sum of money; a certificate to accompany each prize. The money prize shall not be awarded to a member over 40 years of age, but under unusual circum- stances, both prizes may be allotted to one person provided that he is not over 10 years of age. In general it will be understood that the Committee shall award the money prize to the younger men, rather than the medal. 2. The awards shall be made not oftener than once a year to that person or persons contributing to the Institute the best original paper or papers on iron and steel. The scope of the term "iron and steel" shall be determined by the sub-committee considering the awards. In general, papers dealing with the practical side of the subject have preference over those dealing with the theoretical side in recognition of the fact that Captain Hunt's main' contributions to the industry have been in the improvement of production and quality of material. 3. A sub-committee of three to five, including the Chairman of the Iron and Steel Committee, shall be appointed by the Iron and Steel Committee annually to adjudge the award subject to approval. 4. The awarding committee shall submit its report to the Iron and Steel Committee at its October meeting, and the award shall be certified to the Secretary of the Institute in time to permit the presentation to be made at the Annual Meeting of the Institute. 5. The recipient of the award shall be designated "The Hunt Medallist." This prize is administered by the Iron and Steel Committee. The principle of personal safety is as old as the human race. The application of this principle to industry is of much more recent origin, and may be said to be almost exclusively the product of the twentieth century. The enormous strides in technique and production mode prior to the turn of the century had been achieved without any real regard for the crippled human by-products of that expansive era. It was accepted that. accidents were inevitable and formed part of the price that must be paid for production. It was not until 1885 that the first Workmen's Compensation Act was passed in Germany making the employer financially liable for injuries suffered on his plant.With awakening social conscience came the first organised efforts to reduce accidents in industry. Early 19th century literature dealing with the development of powerdriven machinery and the growth of industry in Europe contains many references to the unsafe and unhealthy working conditions that accompanied the change-over from the home workshop to the industrial plant. By the year 1900, there were in Germany - and a little later in England governmental regulations dealing with the guarding of hazardous machinery. American industry, now the acknowledged leader in industrial safety, admits freely that when an organised safety movement commenced in that country, there was already a considerable amount of information available to them from European experience. Iron ore mining in the Adirondack region of northern New York dates back to the Revolutionary War. It is recorded that Benedict Arnold in his campaigns in the Lake Champlain area during the American Revolution utilized the local ores to produce armament and cannon balls. Through the efforts of early pioneers, the iron mining industry in northern New York gained impetus so that in the 1880's mines in the area accounted for nearly one fourth of the iron ore produced in the United States. Both magnetite and red hematite ores were mined during this period. With the discovery of the Lake Superior ores, the iron ore industry in New York started to decline and remained relatively unimportant until 1938. Modern day methods of mining and beneficiating sufficiently high tonnages of low-grade ores to justify the large expenditures for plant and equipment prompted a resurgence in northern New York mining. During the past several years, the production of iron ore from the Adirondack magnetites has greatly increased, and production which amounted to .6%, of the United States total during the 1918-1928 period (Mining and Metallurgy, November 1943) has been increased to 2.85% of the total in 1948. "This paper examines test results and grinding mill design for the recent Fortune Minerals’ NICO Project, located in the Northwest Territories. SAG milling and rod milling options are compared. Rod milling is clearly shown to be the best option because of the ore’s extreme resistance to breakage in a SAG mill environment.Surprisingly, the Bond Ball Mill Wi was 13 kWh/t for this ore. The Bond Rod Mill Wi was 20 kWh/t, whereas the SAG pinion energy to grind the ore to T80 1.7 mm was 30 kWh/t. The existence of ore with this ratio of SAG hardness to Bond BM Wi has not been previously observed. If not considered, this critical relationship may adversely impact the accuracy of mill design. This paper substantiates the significance of these findings to achieve successful grinding design on ore that is macro-competent during primary size reduction, but relatively normal during secondary grinding.INTRODUCTIONThe NICO Cobalt-Gold-Bismuth-Copper deposit is located 160 km northwest of the City of Yellowknife, Northwest Territories. It is the largest IOCG-type deposit (aka Olympic Dam-type) currently recognized in Canada with Mineral Reserves of 21.8 million tonnes contained within a larger 57 million tonne, lower-grade Mineral Resource. The project, owned 100% by Fortune Minerals Limited, has been assessed in a positive definitive feasibility study, subject to underground test mining and pilot plant testing, and is now in the permitting stage to develop a combined open pit and underground mine and process plant. An updated process flow sheet incorporates conventional crushing, grinding and bulk flotation, followed by filtration for shipment of a concentrate to a purpose-built refinery proposed to be built near Saskatoon, Saskatchewan. At the refinery, separate bismuth and cobalt concentrates will be produced following regrinding. The hydrometallurgical process methods include: pressure acid leach, precipitation, ion exchange, and electro-winning for recovery of cobalt as 99.8% cathode; solvent extraction and electro-winning of by-product copper cathode; ferric chloride leach and electro-winning for recovery of bismuth as 99.5% cathode; and cyanidation of the leach residues for recovery of gold as doré."

THE Star shaft is near the north boundary of the group of mining claims belonging to the Ground Hog Unit of the American Smelting and Refining Co., near Vanadium, N. Mex. The shaft bins and surface plant are served by a siding from the Atchison, Topeka and Santa Fe railroad, and this factor of railroad transportation influenced the choice of site. This paper describes the shaft sinking only, and does not cover the surface plant or other parts of the mine or its equipment. Experience with this shaft seems of interest because of the mechanical mucking, good rate of progress, and the costs obtained at the present level of labor and material costs. The shaft has four compartments, as shown in fig. 1. Equipment: Riddell Mucker: The mucking was done by a clamshell bucket and a Riddell mucker, which, as in other shafts sunk with this machine, consisted of a carriage mounted on axles with double flanged wheels that traveled back and forth on a swinging track, parallel to the long axis of the shaft (fig. 2 to 6). Three air motors were mounted on the carriage; one hoisted the clam, one operated the bucket opening and closing device, and the third moved the carriage along the track. The carriage was designed with guide shoes and could be hoisted to the surface in either skip compartment without trouble (fig. 4). During blasting operations the clam was tied on the rails and the carriage was hoisted up a few sets to protect it from flying rock. No attempt was made to hoist the carriage and clam together lest the clam catch in the timbers. The track on which the carriage operated was built of extra heavy 6 in. pipe with 1x2 in. strap welded on top for rails. A 1/2x4 in. angle was welded on the bottom of the 6 in. pipe with a 1 in. round rod welded in the angle for a filler and additional strength. The end or cross pieces of the track which kept the track to gauge, were also made of reinforced, heavy 6 in. pipe and were bolted to the long members on which the carriage traveled (fig. 2 and 3). This section of track was used, instead of heavy railroad rails, for two reasons: (1) this section would bend before it snapped and was much safer for the men in the bottom; and (2) it was a better blasting shield, and protected the timbers better, in fact, only five wall plates were broken by blasting in 1926 ft of shaft, although 150 to 225 lb of 40 pct powder was loaded per round, depending on the ground conditions. The swinging track was supported by four 5/s in. hoisting cables anchored to the end plates. There were also four safety cables fastened two to three sets above the carriage, which would catch the carriage if one of the supporting cables should fail for any reason (fig. 7). Clamshell Buckets: Two clams were used, each of 3/s yard capacity, both manufactured by Blaw Knox, Nos. 662 and 663. The No. 663 clam was 4 in. wider and 5 in. shorter than the No. 662, but seemed to dig and load more satisfactorily. It was easier to hoist and lower when it came up for repair. "The years following World War II were nation building years in Australia. Soldiers returned from the European and Asian theatres of war sparking a major infrastructure construction boom. Dams and highways were built to keep up with the growth of towns and cities, which were bursting at the seams, not just from the returning soldiers settling down with their families but also from the many waves of refugees from war torn areas around the globe.The demand for electricity generation and distribution required for the growing population sparked the expansion of the national power grid with new high-voltage transmission towers appearing across the landscape. However, with an original design life of 50 years, those transmission lines constructed in the 1950s are nearing, or are at the end of, their anticipated service life.In recent years, the Australian power distribution network has changed from a period of high construction activity in greenfield high-voltage lines to a period that is now focused on reducing cost and maintaining existing assets. Replacing aboveground structural members and electrical components is relatively simple even under live conditions. PCA has been working on a potential solution with Powerlink Queensland for the past 5 years to develop a quick and cost-effective foundation replacement system that can extend the life of its existing asset base anywhere from an additional 5 years to 50 years.With design support from Quanta Subsurface, USA, PCA recently completed the foundation replacement of 26 lattice tower foundations in the Townsville region of North Queensland. Using a steel bracket (patent pending) to connect hollow bar micropiles to the lattice towers, the installation techniques that were developed enabled the micropiles to be installed within a tolerance of less than about 3/8 in (10 mm) through the existing steel grillage foundation while maintaining live line conditions. The project was an Australian first and a trial by Powerlink for wider use within its network, and it was delivered on time and within budget at one tenth of the cost of traditional methods."

"The years following World War II were nation building years in Australia. Soldiers returned from the European and Asian theatres of war sparking a major infrastructure construction boom. Dams and highways were built to keep up with the growth of towns and cities, which were bursting at the seams, not just from the returning soldiers settling down with their families but also from the many waves of refugees from war torn areas around the globe.The demand for electricity generation and distribution required for the growing population sparked the expansion of the national power grid with new high-voltage transmission towers appearing across the landscape. "INTRODUCTIONGomez and Austin (1,2) have recently described calibration procedures for quantitative chemical analysis of particles by energy dispersive spectroscopy using particles mounted and polished in a resin matrix and scanned by an SEM under computer control. Their software is specifically designed for the analysis of Si, Al, Ca, Na, K, Fe and S in coal particles, and in this case the resin is loaded with iodine (3) to enable the exposed cross-section of an embedded coal particle to be differentiated from the matrix. However, the basic ideas of their procedures could be extended to any system of particles where it is desired to measure inorganic species in particles containing light elements such 0, C, H and N which are not directly detected. In these circumstances it is necessary to perform a quantitative determination of the measured components, so that the equivalent volume of light elements in the excited sample volume can be estimated by difference using ZAF and other corrections applied to the raw count intensities.Part of their procedure involves a new technique (2) for estimating background spectrum intensities; the intensity for a particular energy window is then subtracted from the total measured intensity for that window to enable the concentration of the corresponding element to be calculated. In addition, corrections for overlap of peak spectrums are applied. For example, Figure 1 shows some typical spectrums for known materials for the system they used, demonstrating a wide Gaussian spread about the theoretical peak energy. Since background spectrum and peak overlap corrections require a knowledge of the composition of the material, as does the application of the ZAF corrections, it is necessary to have an iterative solution of the equation sets, starting with estimates of the composition based on initial count rate corrected solely for dead time and detector efficiency."
