Safety management, both personal and that associated with catastrophic hazard, helps to define the success of mining companies. Personal safety management, often categorised by slips, trips and falls, has been widely addressed, with numerous approaches successfully adopted. Catastrophic hazard can lead to a single accident (eg ground collapse, underground fires etc) causing multiple fatalities. Catastrophic hazard management in mining is an emerging discipline, and as such there are various pitfalls to demonstrate its effectiveness and success; it is much more than simply meeting regulatory requirements. Various standards, guidelines, and leading practices are available for reporting key performance indicators. While measuring and reporting safety related key performance indicators (KPIs) such as Total Recordable Injury Frequency Rate (TRIFR), Lost Time Injury Frequency Rate (LTIFR), are important, they often lack detail, and give little insight into the effectiveness of catastrophic hazard management strategies and controls. Catastrophic hazards are low likelihood, high consequence events, and as such demand a different management approach. Companies cannot use the suite of personal safety metrics to help demonstrate management of catastrophic hazards. In-field verifications have revealed that senior management commitment to safety is often clear, well organised, and resourced. However, there are often some inconsistencies between what management promotes and plan, to what the operational teams understand and implement. Communication and clear understanding of expectations were the common themes identified to help empower supervisors to reinforce requirements of catastrophic hazard management. A lack of specific, measurable, and time-sensitive goals and objectives for the short- or medium-term, making it challenging to track progress was another key observation. Development of an integrated, multi-disciplinary auditing process executed by an experienced team of technical subject matter experts, provided an in-depth understanding of the requirements of catastrophic hazard management. Areas such as underground fire prevention, explosive storage, electrical safety, mobile vehicle interactions, ground control, inrush, and shafts and winders management have substantial overlap and intertwined processes, and similar strategies for emergency responses. Auditing of catastrophic hazards and safety management systems compliance is only the first step in safety assurance. Completing operational performance audits in technical areas and benchmarking the site verification of safety management documents and verifications forms a critical part to clearly demonstrate industry leading safety practices. No longer is it the external inspection or regular hazard management plan review requirements that dominate, but rather the culture of continual hazard identification, management, controls verification, underpinned by improvement and follow-up verifications of the corrective actions.

The selection of a process for treating a gold ore is governed primarily by the characteristics of the ore under consideration. The form in which the gold occurs, its mode of association with other minerals, and the size and surface condition of the gold particles are important factors which influence materially the choice of procedure for recovering gold. A knowledge of these characteristics may be obtained by microscopic and chemical methods combined with experimental testing. Cyanidation and amalgamation, either alone or in conjunction with flotation and gravity methods, are peculiarly adapted to the treatment of gold ores, especially those of relatively low grade. Gold alloys readily with mercury provided both present clean surfaces. The presence of oils and grease, antimony and arsenic minerals, or soluble sulphides have a harmful effect. Very small, flaky particles of gold frequently are lost in amalgamation because of noncontact with the mercury. Fine gold in pyrite, or gold having rusty or contaminated surfaces, is often lost for the same reason. Grinding or some form of abrasion sometimes removes surface films of iron oxide or substances of a similar nature, and alkalies or other chemicals are used to remove grease. Gold is readily soluble in cyanide solution, and this method of treatment is particularly effective in the recovery of gold from the tailings resulting from other types of concen¬tration procedure. Cyanide solutions must contact the gold to insure recovery, and the porosity of the ore treated and the fineness of grinding required are important governing factors. The use of lime in cyanidation is necessary to protect the cyanide, to precipitate undesirable substances, and to settle slimes. Certain substances in ores are harmful in cyanidation; chief among them are those that combine with cyanide, forming cyanides, and carbon which precipitates gold from solution. Arsenic and antimony are also harmful if present in sufficient quantity. These and other difficulties encountered in cyaniding certain ores often are overcome by roasting prior to leaching. Zinc dust is used almost universally for precipitating precious metals from cyanide solutions. The refining and melting of the product may be accomplished by oxidizing the zinc before melting it in graphite pots with fluxes, although sulphuric acid is now used largely to remove the zinc before melting. Straight cyanidaticn practice is being replaced by flotation as a major process, followed by cyanide leaching of the tailings, but amalgamation is still needed as a primary process for the recovery of coarse gold before either flotation or cyanidation.

"Broadmeadow Mine is an underground longwall mine located in Central Queensland, Australia. The mine introduced a top coal caving (TCC) longwall face in 2013 but since then has experienced severe convergence events at the start of each panel after 60–70m of retreat, resulting in equipment damage and the longwall almost becoming iron bound. A significant geotechnical monitoring effort during LW10 and LW11 to characterise the caving mechanics and operational changes during LW11 were successful in mitigating the risk posed by these initial convergence events. This paper describes the geotechnical monitoring programme undertaken during LW11, the operational methods used to mitigate these initial convergence events, and the contribution to these events of the TCC technique.A thick sandstone unit close to the extraction roof was likely the cause of the convergence events at the start of each longwall panel. However, stress change monitoring cells, surface extensometers, an inclinometer, and the blockage of holes designed for pre-conditioning using hydraulic fracturing indicated that the convergence events were not caused by the sudden collapse of sandstone bridging across the goaf. Rather, they were caused by the propping action of strong units within the overburden strata. Equipment design modifications were made to allow the longwall to remain operational during the convergence events and an operational strategy was implemented to mitigate convergence in LW11. The initial convergence event was successfully negotiated, and Broadmeadow went on to be the highest producing longwall for CY16 in Australia.INTRODUCTIONBroadmeadow Mine is an underground longwall mine located in Central Queensland, Australia, owned and operated by BHP Mitsubishi Alliance (BMA). The mine is a punch longwall mine using open cut pit access provided by Goonyella/Riverside Mine. Broadmeadow commenced longwall operations in 2005 using a conventional high reach longwall, but later changed to the top coal caving (TCC) method of extraction. Broadmeadow extracts the easterly dipping 5.5–7.5m-thick Goonyella Middle Seam in ranging in depth from 90m to 330m. The coal is washed and blended at the adjacent Goonyella/Riverside open cut mine to produce a premium coking coal product for export."

An attempt is made here to collect together various reports and publications that are based on research supported all or in part by Contract No. H0155008 for the past two years. The subject matter deals primarily with the excitation, transmission, and reception of electromagnetic waves in mine environments. This contract from the U.S. Bureau of Mines with the Office of Telecommunications supports the efforts of James R. Wait (Principal Investigator and Consultant to OT), David A. Hill (Co-Investigator), and various associates within the Boulder scientific community. The contributions of Samir F. "INTRODUCTION This report embodies the results of a detailed statistical microscopic examination of the ore and the mill products of the Silver King Coalition Mines Co., Park City, Utah.The study was conducted under the joint auspices of the United States Bureau of mines is cooperation with the University of Utah at Salt Lake City, Utah. The work was done in the cooperative Microscopic Service Laboratory, which was equipped by contributions from the following companies: Utah Copper Co., International Smelting & Refining Co., American Smelting & Refining Co., United States Smelting, Refining & Mining Co., Tintic Standard Mining Co., Silver King Coalition Mines Co., and Park Utah Consolidated Mines Co. The salaries and the other costs involved in operation of the laboratory were met by contributions from certain supporting companies. The Bureau of Mines developed hypereutectic aluminum-silicon alloys which 11.7.d better mechanical properties than the unmodified hypereutectic alloy. A commercial aluminum-20 weight-percent silicon LIr alloy was selected as a ary base for improvement. The primary silicon. grains were reduced from ASTM 4-5 to ASTM 8.5 by the use of phosphorus and phosphorus compounds. The remaining eutectic matrix was refined by sodium metal additions of 0.2 weight-percent. Solid solution strengthening of the alpha aluminum was obtained by adding 2 percent copper and 0.5 percent magnesium. Also, 1 percent nickel was added to harden the alloy. Optimum thermal treatment consisted of a solution treatment for 10 hour:, at 480° C with hot water quench, followed by aging for 6 hours at 175° C. The final alloy exhibited a Rockwell B hardness of 75 ±2 in the heat-treated condition. The ultimate tensile strength was 39,5O0 psi with no recordable elongation. The coefficient of linear thermal expansion was 20.4 x 10-6 per °C in the temperature range 20° to 300° C. No wear resistance data were collected.

Precipitation in the northeastern United States, Florida, California and in many industrialized areas of the world is now more acidic (lower pH) than can be accounted for by natural causes. At the same time, the acidity of many lakes and streams in the northeastern United States has been measured and found to be similar to that of actual rain. These facts have led to conclusions that the two conditions are related. Coal burning utilities, 700 to 1000 miles upwind, have been accused of being the cause, leading to proposals for strict emission controls on the alleged offenders. The costs of these controls would place an estimated $10 to $15 billion capital burden on utility customers in about 15 states plus $2 to $6 billion annual operating costs. There appears to be no direct well-established scientific evidence that the accused are guilty but that the indictment is based largely on circumstantia1 evidence. This study examines the evidence, describes the situation and proposes what must be done to reduce the acid precipitation in an orderly cost beneficial manner. It is interesting to observe that studies by the different adversaries on the acid precipitation issue come to different conclusions largely because they do not read the same literature, but selectively develop evidence to support their own points of view, This study will try to review the issues impartially and reach a position of scholarly balance.

"An electrochemical sensing device for hydrogen in molten metal has now been available owing to the development of the proton conducting solid electrolyte made of ceramics of metal oxide. The structure of the sensor and the theoretical limitations of its applications are discussed based on the electrochemical properties of the electrolyte used. The practical performance of the sensor is reviewed from the results of test measurements at the plants of aluminum and copper melting industries. The effectiveness of the sensor to the process control of continuous casting of copper is also discussed.IntroductionSince the in-line monitoring of the amount of oxygen dissolved in molten metal was realized by using the oxygen concentration cell based on zirconia solid electrolyte a similar method has been expected to be developed for sensing of hydrogen, which is a very important element of gas species in liquid metals, especially, in aluminum and copper. In 1981; Iwahara and coworkers found the dominant proton conduction at a high temperature in some perovskite-type oxide (1). Since then, the effort has been made to apply this proton conducting material as the electrolyte of the hydrogen concentration cell for high temperature use. The hydrogen sensor for liquid aluminum of this kind was first produced in 1991 (2) and released commercially from TYK Co., Ltd. Recently, we have developed the mother one usable at higher temperature such as copper and silver melting processes (3). In this note we discuss their structure, design and applicable limitations based on the electrochemical properties of this new type electrolyte. The performance of the sensor and benefit to use it to control the melting process is also discussed"

Anticipating the availability of space-altitude imagery from the Earth Resource Technology Satellites (ERTS), the Bureau of Mines investigated the possibilities of using suck imagery in its activities in solid waste disposal, reclamation of mine workings, and mineral resource inventory. The study indicates that monitoring open pit and strip-mining operations can be done using conventional techniques if the sharpness of the imagery is sufficiently high. Possibilities also exist for semi-automated processes for change detection. Additional experimentation is needed for multispectral discrimination and identification of mine-related phenomena.

In central Washington, six miles south of the small town of Vantage, the flow of the Columbia River is harnessed by the Wanapum Dam which is part of the Priest Rapids Hydroelectric Project owned and operated by Grant County Public Utility District (GCPUD). Extending more than 8,300-feet from bank to bank, the dam includes left and right earth embankments, left and right concrete gravity sections and a 10-unit powerhouse. Adjacent to the right embankment is a concrete gravity spillway featuring radial gates measuring 50-feet-wide and nearly 70-feet-high. In the early 1960’s when Wanapum Dam was built, these seven-story spillway gates were the highest in the world. In late February 2014, 50-years into its service life, during a routine maintenance inspection, observant workers noticed a slight misalignment in the roadway deck curbs and handrails located on top of the spillway. Further survey and inspection efforts uncovered a 65-foot-long by two- inch-wide crack that spanned the full length of Monolith 4. On discovering the extent of the damage, GCPUD initiated immediate drawdown of the reservoir to relieve pressure on the spillway monoliths. Wanapum Reservoir and the areas directly downstream of the dam were immediately closed, boat ramps and inlets were left completely dry, campsites were closed and boating activity was banned on the lake until further investigations and remedial procedures were in place and completed. An extensive root cause analysis ultimately led to the conclusion that errors in the initial design calculations and late changes to the final spillway design led to a condition that generated tensile stresses in the concrete at the upstream face, which over a period of 50-years resulted in the formation of a crack along a horizontal lift joint located about 85-feet below normal pool elevation. Further analysis showed that, although visible distress was found at Monolith 4, there was nothing preventing this issue from happening at other locations along the dam spillway. Although no visible cracking was found in the other monoliths, repair procedures for Monolith 4 would have to be replicated at the remaining monoliths.

Gold and precious stones were mined in Egypt in prehistoric times. This mining, carried on at intervals, extended over thousands of years, until about 1300 A. D. Then ensued a dormant period of several hundred yoars. In the mean- time Europe made great rogress in mining practice, metallurgy, and geology and sent out various expeditions during the nineteenth century to investigate ancient mines. But the exploratory work produced no material results, and not until the beginning of the present century did actual mining recommence. The reopening of old gold mines in Rhodesia aroused interest in London and Paris, and these cities provided the capital necessary for the investigation of the ancient minee in West Africa and Egypt. Speculation resulted in the division of most of the uncultivated area of Egypt and the Sudan into huge concessions, and a number of expeditions were made; but after an unprecedented activity in West Africa, roaction set in, and the flow of capital ceased. The Ambuklao hydroelectric dam was originally constructed in the 1950s in the remote and mountainous region of the northern Philippines. Following its construction the dam has suffered heavily due to siltation and a partial collapse of the intake structure due to a 7.8 magnitude earthquake that struck the region in 1990. Between the years 1990 and 1999 the power station progressively lost its capacity to generate power. In 1999 the owner took the power station out of commission. The power station remained idle over the next 12 years. In 2008 the power station was sold to a new owner who commissioned the rehabilitation works.The rehabilitation project restored the operating status of the power station in June 2011 and increased its capacity from 75 MW to 105 MW. The civil contract of the rehabilitation project required the construction of a new intake structure behind an in situ rock coffer dam on the edge of the existing reservoir, new 6.2?m diameter × 124?m deep blind sunk shaft behind the intake structure, which connects to a new 6.8?m horseshoe profile headrace tunnel that ultimately connected onto the existing vertical penstocks which feed three No 35 MW Horizontal Turbines rated at 155?m head.The unique challenge in rehabilitating this existing hydropower station involved finding solutions that would conclusively deal with the challenging geological and hydrogeological conditions associated with excavating new tunnels and a drop shaft beneath a live dam.McConnell Dowell was awarded the civil works package for the rehabilitation works in August 2008. The Ambuklao rehabilitation program took 33 months to complete.Over 600?m of tunnels were excavated in highly weathered and often challenging geotechnical conditions via drill and blast with the extensive use of high-pressure fissure grouting to control groundwater ingress that was surcharged by the dam.This paper focuses on the use of high-pressure fissure grouting that took place to enable the excavation of the new waterway system and intake structure.CITATION:Bowskill, A and Harrison, C, 2014. It is well known that the refinement of grain size in metals leads to a significant improvement in important mechanical properties. As part of a European collaborative project, processing schedules have been investigated aimed at producing a homogeneous ultra-fine ferrite and spheroidised carbide aggregate microstructure structure in high carbon (CMn) steels (0.6-1.2wt%C), via conventional 'warm' rolling and innovative Equal Channel Angular Extrusion (ECAE). Suitable deformation schedules were determined from dilatometry and thermo-mechanical Gleeble simulations. Evidence of an ultra-fine ferrite and carbide aggregate microstructure following 'warm' rolling is given. A significant improvement in tensile strength, particularly proof stress (0.2%) was also noted in comparison to material deformed at higher temperatures. Concurrent Equal Channel Angular Extrusion (ECAE) experiments investigated microstructural evolution with incremental strain. Extensive analysis was carried out using various techniques, including high resolution Electron Back Scattered Diffraction (EBSD). Evidence of ferrite grain refinement was noted in a eutectoid composition steel. A sub-micron ferrite structure was observed following high strains (E-3.33) and the mechanical properties exhibit a marked increase in tensile strength.

A laboratory investigation was carried out to study the influence of asphalt, bentonite and corn starch binders on the pelletization kinetics of coal fines and on the quality of the resulting pellets. Pellet quality was evaluated in terns of the compression strength, impact resistance (drop test), abrasion resistance and resistance to severe weather conditions. The amount of binder addition did not significantly influence the pellet growth kinetics. However, the pellet quality varied markedly depending upon the type and the amount of binder used. Experimental results are explained on the basis of the mechanism of interaction between the coal particles and the binders. Of the three binders tested, corn starch proved to be the best in all respects but for the resistance to water penetration. Finally, a combination of corn starch (imparting strength) and asphalt emulsion (imparting resistance to water penetration) was found to yield the best quality coal pellets.

In 1939, at the request of The Koppers Coal Co., the Koppers Company Research Department established a Stoker Coal Research Laboratory for the purpose of investigating the performance characteristics of stoker coals in typical domestic stoker installations. Fig. I is a photograph of part of the laboratory. The material in this paper deals with the performance of one coal in a single installation. Presented are details of this installation, mechanical characteristics of the stoker, and the results of a series of burning tests in which adjustments were systematically varied. Summary OF Results Mechanical Features I. A large percentage of the total power consumed by a domestic stoker is used to operate the gear case and motor (Fig. 3)- 2' The power consumption per ton goes down with an increase in rate of coal feed (Fig. 4). 3. The automatic air control in this stoker showed advantages Over manual control (Fig. 5). 4. The air velocities through the retort tuyeres were uniform around the retort, but varied between the rows of tuyere rings (Fig. 6). 5. The crushing of coal with this stoker increased the minus 1/8-in. from 11.6 per cent to 19.2 per cent. There was a notice- able segregation of coal as it came from the retort and distribution was not uniform (Fig. 7). 6. Measurement of air entering over the fire showed that with some not uncommon adjustments this air may be a high percentage of the total air supply (Fig. 8 and Table I). Operating Performance ,. The, removal of clinker was shown to give a period of poor performance characterized by a high fuel-bed resistance and a high percentage of carbon monoxide in the flue gases (Fig. 10). 2. The fuel-bed resistance was found to decrease as the fuel bed built up in a normal response to heat demand after a prolonged hold-fire (Fig. II). 3. with intermittent operation the burning rate was almost always less than the feeding rate, The wide variations were found to be greatly dependent upon the length of the On period and the length of the intervals between On periods (Fig. 12). 4. The maintenance of a low furnace draft led to less fly ash, better clinker formation, and higher efficiencies (Fig. 13). 5. The maintenance of a minimum of excess air also resulted in less fly ash, better clinker formation, and higher efficiencies (Fig. 14). "IntroductionTHE PURPOSE of this paper is to give a •brief history of iron and steel in British Columbia, a quick look at the present operations of Western Canada Steel, Limited, a review of iron ore resources and iron and steel markets, and a projection of what the future might hold for the industry in British Columbia.From the Past to the PresentWhile the paper is entitled Past, Present, and Future, in covering the past, which we shall call the time prior to 1947, there is very little to be said. Small shipments of iron ore were made from the Texada deposits by the Puget Sound Iron Company from 1886 to 1907. Total ore shipped did not exceed 70,000 tons. Part of this is believed to have been used in building a United States battleship in 1907 in the Bremerton Naval Yards in Seattle, Washington.In 1914 a small steel mill was started on Lulu island, rolling bars from •box piles and used shafting. This mill operated spasmodically for a few years but finally burned down in 1921. The rolling mill stands were sold to China and are no doubt in operation in China today.In 1916 the City of Port Moody assisted in the financing of a mill which operated on re-roll rails, box piles, and steel ingots made in an experimental electric furnace in Port Moody. This furnace also produced small quantities of pig iron scrap. The aim of this work is to clarify the role of microstructure heterogeneity in the initiation and growth of cracks in a lost foam cast A319 for low cycle fatigue (LCF) loading condition. Because 2D analysis fail to establish the scenario of damage events, this study is based on 3D in situ analysis using synchrotron X-ray tomography during LCF test performed at 200°C. If X-ray tomography is now an established technique to study damage development in 3D, its application for relatively high temperature cyclic tests has never been done so far. Initial 3D observations of damage show that the cracks that lead to fracture initiate near large pores and propagate into the hard particles 3D network of the eutectic regions.

This publication contains routine analyses and related source data for 374 natural gas samples from 24 States and 5 foreign countries. These samples were collected during calendar year 1966 as a part of a continuous survey of the free world for occurrences of helium in natural gas. This survey has been conducted in the United States by the Bureau of Mines since 1917. In late 1965, the survey was expanded to include foreign gasfields. The analyses of foreign samples contained in this publication represent the first samples received as a result of this expanded program. The analyses published herein were made by mass spectrometer and a special helium analysis apparatus, which are described in Bureau of Mines Bulletins 4863 and 576.4 Eight previous publications have been made to present the results of 7,244 gas analyses made? by the Bureau of Mines Helium Activity prior to 1966.
