A series of studies are described that identify critical health and safety skills needed by underground coal miners for coping with mine emergencies. First aid and self-rescue and escape were selected as two broad performance domains that include many subordinate skills needed for preventing, recognizing, and coping with mine emergencies. The performance domains and their objectives were used to direct the development of simulation exercises that permit miners to experience mine emergencies in the safety of a training room. The exercises are based upon research with similar simulations for the training of flight crews and medical personnel in information gathering, judgment, and decision making skills. Exercise content is drawn from actual mine accident and disaster case studies. The exercises incorporate recent principles of cognitive psychology and instructional design. Initial studies in annual refresher training classes suggest the exercises are stimulating for miners and their instructors, easy to use and read, sufficiently complex and authentic to challenge experienced miners, and useful devices for both teaching and testing critical problem solving skills required for preventing and minimizing emergencies.

While the original purpose of the Bureau of Mines work on fluorine micas was to synthesize large single crystals or film suitable to replace natural muscovite or phlogopite, the objective was broadened to include development of the many possible fluorine micas as a class of new, unique materials. Several methods were developed for the synthesis of the fluorine micas at atmospheric pressure: Solid state reaction, melting in crucibles, internal resistance electric melting, and arc resistance electric melting. The latter two methods are now med commercially. A variety of fluoromicas were synthesized by isomorphic substitution including disilicic, trisilicic, and tetrasilicic. Fluorophlogopite, KMg3AlSi3O10F2, is the fluoromica most widely used. While most of the fluoromicas are completely stable to water, many water-swelling fluoromicas and fluoromontmorrilonoids were synthesized. Single crystals or film of the fluorine micas were grown to 2 inches or more by a number of methods, but none has as yet proved economical. In the report, detailed data and descriptions are given on compositions, syntheses, products, and uses, and on the properties including physical, dielectric, chemical, X-ray, optical, and structural.

Results of laboratory studies on the flotation of marmatite (zinc sulphide) with new reagents, which were developed to avoid the use of copper sulphate, are presented.Episulphides were chosen as the basis for the reagents because of their reported special chemical affinity for zinc and cadmium. Attempts were made to solubilize and introduce functional groups to episulphides with organic and inorganic acids, ammonia, phenol, aniline, diethylamine, and pyridine. As a class of reagents, the episulphide-amine combination was generally found effective in zinc sulphide flotation to a greater or lesser degree. Best results were obtained with ethylene sulphide-aniline combination. Although anilino-ethyl mercaptan is the reported reaction product of aniline and ethylene sulphide, it was concluded that the effective zinc sulphide flotation with ethylene sulphide-aniline combination is not entirely due to the formation of the mercaptan.The economics of the possible usage of ethylene sulphide-aniline reagent are discussed.

"Mineral processes are complex operations that have been modelled, controlled and simulated by diverse technologies. A review is given of some models of mineral processes in the literature. A new variation of a dynamic process model of flotation is presented. Process models, describing real and relevant physical variables, give a good understanding and good qualitative simulations, but can sometimes require prohibitively difficult and extensive parameter estimations. Process control is usually best done on operational plants by the fitting of system models, which can home in on the plant?s observability, controllability and optimisation. System models can be of a form amenable for use in appropriate techniques for controller design and implementation. The control of milling circuits is discussed in some detail. While simulators use models to predict dynamic or steady-state information, part of their strength lies in their ease of use and their ability to test different types of specifications. Some results are given for a flotation circuit simulator that can be made to solve virtually any permutation of specifications.INTRODUCTIONThe development and application of appropriate technology can lead to better operation of mineral processing plants in industry, so that substantial benefits can be obtained from improved throughputs, efficiencies and product qualities. This paper addresses some of the principles and methods used in the quest for the development and exploitation of such technology. Although the design of processes is often the motivation for modelling and simulation, this paper also targets the use of modelling and simulation for the control and optimum operation of mineral processes. The two processes given the most attention here are flotation and milling.Technologies used in mineral processing range from simple to very complex. Simple technical principles, such as mass and component balances, can yield some simple and effective strategies for modelling and control. However, minerals in particulate form have such complex physical and chemical properties that there appears no end to the complexity that emerges, as more and more factors are taken into consideration in the pursuit of better results and understanding. The situation is aggravated by the complexity of mineral processing plants, which have spatially variable conditions within process units, and complicated interactions between these units."

Land rehabilitation transcends the mere establishment of vegetation across disturbed landscapes; it should be conceptualised as a specialised ecological reconstruction process. This process necessitates a long-term perspective on the success of land repurposing. It requires a continuum of inputs specified and managed by a dedicated and qualified custodian to ensure the sustained economic viability and ecological sustainability of the land. Based on the authors’ combined experience and field observations over more than five decades, they advocate for an urgent shift away from traditional land rehabilitation practices toward a paradigm centred on land capability reconstruction and active stewardship. The prevailing rehabilitation culture has overly simplified the process to a mere revegetation exercise, which is fundamentally incompatible with the comprehensive requirements for sustainable land capability. While the regulatory framework provides valuable guidance for obtaining a mine’s social license to operate, a paradigm reset is essential. Mining companies must prioritise rehabilitation quality, set achievable sustainability goals, and establish clear pathways and milestones to meet relinquishment criteria. Currently, the status quo view—a model reliant on outsourced contracting and agricultural approaches—is that mine rehabilitation is a one-time intervention rather than a complex, systemic, and long-term process. This perspective presents significant challenges, as the success of ecological interventions can only be accurately assessed over extended timeframes, often spanning decades. Shortsighted rehabilitation specifications, coupled with practitioners operating within agricultural decision frameworks, frequently overlook the necessity for integrated rehabilitation expertise and the scientific foundation essential for designing and implementing effective ecological outcomes. A lack of long-term monitoring information impedes the required adaptive management of rehabilitated systems and, therefore, delays the transition towards final land use. This short approach costs the mining industry millions annually, often without recognising the inadequacies in long-term liability mitigation and the substandard ecological success achieved. The disconnect between procurement processes, the qualifications of service providers, and the unclear pathways to relinquishment can impede successful rehabilitation outcomes, frequently undermining progress toward sustainability milestones. Unless there is a fundamental reset of the land rehabilitation culture, pursuing sustainability will remain an elusive and perhaps impossible objective. Consequently, mining companies may hesitate to fully commit to sustainable land rehabilitation practices, as the prospects for relinquishment and final closure remain uncertain at best.

Noise-induced hearing loss (NIHL) is the most common occupational illness in the United States, with 30 million workers exposed to excessive noise levels [NIOSH 1996] every day. Of particular concern is the mining industry; which has the highest prevalence of hazardous noise exposure of any major industry sector [Tak et al. 2009] and is second only to the railroad industry in prevalence of work-ers reporting hearing difficulty [Tak and Calvert 2008]. This document is for operators, safety personnel, and mechanics in the mining industry who are not specialists in noise control engineering or acoustics. Evaluations of successful and unsuccessful attempts at controlling noise on several large, underground metal mining machines are detailed to illustrate the basic principles of noise control. Once personnel understand the guidelines and principles of noise control, they will be able to ¦evaluate the extent of a noise problem; ¦determine the best approach to the problem; and ¦apply the most appropriate solution. Because of the insidious nature of NIHL, it can go unnoticed until a considerable loss of hearing has occurred. In some cases, diagnosis is delayed because an exposed individual claims to have become accustomed to the noise. In reality, that person may have already suffered irreversible hearing loss. Humans can hear sounds in the frequency range from about 20 to 20,000 Hertz (Hz). Within this range, NIHL usually begins in the frequency region around 4,000?6,000 Hz, the upper levels of the speech region. The first noticeable symp-toms include difficulty understanding higher pitched voices, such as the voices of females and children, and difficulty understanding certain consonant sounds, which are primarily high frequency in nature. The extent of NIHL varies depending on the level and duration of noise exposure and on an individual?s susceptibility; despite having similar noise exposure, individuals can experience differing degrees of hearing loss, or none at all. NIHL is almost always preventable. To reduce or eliminate the possibility of NIHL, an individual?s noise environment must be analyzed and appropriate action taken to reduce noise exposure.

The Enaex Mining Technical Solutions (EMTS) team, in partnership with the Anglo American Iron Ore Brazil drilling and blasting team, has developed a study aiming to optimize the blast design applied at the Minas-Rio project. The study consists of modeling the dynamic behavior of one lithological domain to determine the best configuration of drilling pattern, charge configuration, timing and sequencing of the blast designs applied at the mine in this particular rock. The first stage of the study was the recognition of the rock blast process in question, assessing key parameters that characterize this whole operation: geomechanical characteristics of the rock mass, implementation quality for drilling pattern and explosives charging (collar position, hole depth, explosive charge length and stemming size) and recognition of current blast designs. The central element of this study was the creation of a vibration attenuation and seed wave model which has allowed the characterization of this particular rock mass present in a region determined by the Minas-Rio team. The model is based on a near field vibration test, constituting a powerful tool in the design of blast parameters. The blast design developed from the damage model simulations basically modified the timing and sequencing. The aim of this study was to improve the fragmentation resulted by the blasting, considering the particle size requirements demanded by the primary crusher and to increase the productivity of the loading and hauling operations. The results were analyzed considering the effective productivity of the two different fleet types of shovels, where the authors have analyzed more than 46000 values of productivity from September 2017 to August 2018, taken from the Dispatch system. A significant increase in effective productivity of these load equipment was observed, 7% to the PC4000 fleet and 2% to the PC5500 fleet, values supported by statistical parameters and hypothesis tests.

Study on Iron Ore Sintering with High Proportion of Specularite Concentrates Specularite iron ores have poor hydrophilicity and ballability as well as bad high-temperature reactivity, which restricts the large-scale application in the sinter and pellet production. In order to make the best use of the specularite concentrates, a novel process of Composite Agglomeration Process (CAP) is applied in this research. By the CAP, the percentage of specularite concentrates in the sinter mixture is increased to no less than 40 per cent as well as the coke breeze dosage is reduced by 0.3 per cent~0.6 per cent. Moreover, all the sintering indexes are obviously improved compared with the traditional sintering process. Researches on the metallurgical performances shows the finished composite agglomerates are high-quality burdens for blast furnaces. This investigation can provide an effective route to utilize the inferior specularite concentrates in the sintering.

"The Agrium Kapuskasing Phosphate Operation mines and processes up to 6,000 t/d of phosphate ore. Run-of-mine crushing to minus 6 inches is done in a toothed roll-crusher, while grinding is done in a SAG/ball mill circuit with no coarse recycle classification of the SAG mill discharge. The primary grinding mill is an 18 ft. diameter x 9 ft. EGL, 1500 HP variable speed SAG mill and the secondary mill is a 13 ft. diameter x 21 ft. long, 2000 HP fixed speed, ball mill. When hard ore was treated, the SAG mill restricted the tonnage that could be ground. When soft ore was treated, the ball mill and spiral classifier overloaded and restricted throughput.Recommendations from a plant benchmark and an ore hardness testing program, on how to better utilize installed energy in the Agrium SAG mill when treating soft ore, and how to obtain best results when treating hard ore, were checked in a pilot plant investigation in the 3 ft. diameter Starkey-PRO Pilot SAG Mill. Correlation between the ore hardness testing and the results from the 3’ Pilot SAG mill was very good. In addition, the work indicated the results that could be expected for the different ore types when using either smaller SAG discharge grates and/or a vibrating screen with 10 mesh square opening cloth.INTRODUCTIONAgrium Kapuskasing Phosphate Operation consists of a 6800 t/d open pit mine and a concentrator, near the Town of Kapuskasing in Northern Ontario. The two major ore types are sandy ore and cemented ore. Sandy ore is fine, soft and easy to grind in the SAG mill while cemented ore is harder, more blocky and difficult to grind in the SAG mill. The ratio of sandy to cemented ore in the deposit is thought to be in the order of 80:20 but can range as high as 5050, so it was important for the grinding circuit to have the ability to treat sandy ore alone."

The mining industry has already become highly technical in all its branches and this has brought about specialization in mining engineering. Increasing demand for mineral products, exhaustion of high-grade or easily worked deposits, and narrowing of the fields for exploration and development, are forcing mines to greater depth and to working material of lower grade and more complex composition. The Robinson Deep at its Turf shaft has reached a depth of 7,640 feet and expects to go to 8,500 feet; Miami Copper, under normal market conditions, profitably mines ore running only 16 pounds of copper to the ton; Alaska Juneau is making a profit on ore running about $1.15 per ton in gold; and many mines are handling ores of an extremely complex nature. In each case, the success of these operations has depended upon the application of sound scientific principles and good business methods to the problems involved. It may be safely assumed that mining at great depth and the exploitation of low-grade, or complex, ores will become more and more important as time goes on, and this means that greater and greater demands will be made upon the technical skill and business ability of the mining engineer. To meet this condition, the more or less haphazard methods of training the young engineer that have prevailed in the past will have to be replaced by a careful and well-ordered system of engineer-training. Upon the industry and the older mining engineer falls the duty of providing means to assist the deserving student to prepare himself properly to meet the increased demands that are falling upon his profession. Consideration of the training of the mining engineer involves a decision as to what constitutes his field of usefulness. Technical control of methods and processes obviously falls upon the engineer, but there are still many who doubt his usefulness in the executive field, particularly in positions involving close contact with the men and work. Many engineers have failed as shift-boss or foreman, but this has been generally due to fault in the management in using men who were temperamentally unfit to lead others or who lacked the necessary preliminary training.

Abstract. The Surface Mining Control and Reclamation Act of 1977 (PL-95-87), Section 522, establishes requirements for state regulatory authorities to designate lands unsuitable for all or certain types of surface coal mining. Any party having an interest in an area that may be affected can petition for it to be designated unsuitable for mining. Section 522 could present major obstacles to the mining industry by typing up an area in an endless round of petitions. The program under development for Kentucky is examined and analyzed as an example for determining the hidden impacts of Section 522.

The common couplings for joining sections of helical piles with an outside diameter of 6.625 inches and 8.625 inches are square on square, flange to flange, external pipe sleeve, internal pipe sleeve, and welded joint. Each coupling type has advantages and disadvantages in efficiency, capacity, installation, and economics of the installed pile. All these coupling types have been used in the helical pile market in North America for various applications and designs. Many of these applications require on-site testing, but test records are not typically shared with the helical pile industry. Due to the lack of information available for square on square and flange to flange couplings, testing was done to compare the efficiency of installation, load to deflection response, and torque to capacity ratios of external pipe sleeve, square on square, and flange to flange couplings. Field installations and tension tests were performed in non-cohesive (sand type) soil near Caseyville, IL and in cohesive (clay type) soil in Centralia, MO. The data collected during these tests included rotations per foot during installation, installation torque, applied pressure, and deflection. The results of this testing are analyzed and discussed within this paper. The results indicate that the square to square coupling is the most efficient of the designs tested, but this conclusion cannot be fully justified without further testing due to the variability in the data gathered and the small number of piles tested. INTRODUCTION A helical pile is typically assumed to be a medium-displacement pile when its outside diameter is between 4.5 inches and 8 inches and a high-displacement pile when its outside diameter is greater than 8 inches. A helical pile designer has multiple options when selecting a coupling type for medium- to high-displacement helical piles. Each type of coupling has advantages and disadvantages and must be analyzed on the bases of efficiency, capacity, installation, and economics of a project. Typical couplings that are chosen by designers include square on square, flange to flange, external pipe sleeve, internal pipe sleeve, and welded joint. A helical pile must transfer axial, lateral, bending moment, and torsional loads, and the coupling must be designed to effectively and efficiently transfer each type of load. During the installation of a helical pile, the rate of penetration and the disturbance of soil layers play a critical role in the efficiency and torque to capacity ratio (Kt) of the pile. It is common for a coupling to protrude beyond the outside diameter of the pipe section being used. This is necessary for the function of certain coupling types and can facilitate the connection of couplings. However, it causes increased soil disturbance and reduce the rate of penetration due to soil bearing against the protruding portion of the coupling. A helical pile designer typically will work to minimize the coupling protrusion and create a pile with optimal installation efficiency but may choose a design with lower installation efficiency if it offers sufficient economic advantage.

The knowledge in the field has been reviewed by literature studies, and using experience from personal research in sublevel caving. There is a lot of material regarding gravity flow of fine grained materials in bins, but the theory of flow for these materials are normally not applicable for coarse blasted rock. This study was therefore concentrated towards coarse rock fragments created after blasting in orebodies. The goal for this study was to make a report of known knowledge, and identify what we have to learn more about. The Bergmark-Roos theory, which was a paradigm shift in the science of gravity flow, is today the best mathematical theory for calculation of the drawbody for a homogenous material or when we wish to simulate the mean drawbody for a large number of draws. The Bergmark-Roos theory must however be improved, so it can take into account the flow pipe effect. When this is done it will be possible to construct a 3D computer model for calculation of ore extraction versus amount of loaded ore and waste. It is also necessary to find the 3D shape of the flow cone to be able to calculate the 3D ore rest that normally develops on footwalls. Calculations should be done for the draw on several sublevels including determination of the ore losses and temporary ore rests. First when this computer model is developed optimisation of ore recovery and waste rock dilution can start for a given orebody. The author points out the importance for international co-operation and standardisation of methods for determination of material properties. More field tests and model blasting tests should also be done. When enough knowledge is gathered, a handbook of the optimisation of sublevel and block caving should be written.

THIS REVIEW contains abstracts of the technical literature and patents dealing with the history, development, and commercial application of the Bergius and related processes for the hydrogenation, under greater than atmospheric pressures, of liquid and solid carbonaceous materials such as coal, lignite, their distillation and extraction products, pitches and tars, and petroleum and its distillation residues. An attempt has been made to include abstracts of all the published information on this process and of as much of the captured foreign documents and reports compiled therefrom as could consistently be reviewed during the time allotted for the work. Contact with the current literature and patents and review of the foreign documents will be continued and the information will appear in Synthetic Liquid Fuels Abstracts, which is being issued bimonthly as a Bureau of Mines publication. The material in this review is divided in two sections-literature (part I) and patents (part II)-and each of these is arranged alphabetically by author or patentee with further chronological arrangement under each author or group of authors. Secondary authors and patentees as well as assignees are cross-indexed to the main entry. In serial articles or reports the complete series appears in the alphabetical arrangement according to the author of the first member of the series. Succeeding members as well as secondary authors are entered as cross references; Titles of the journals are used as the authors in anonymous references. By following such a procedure, an author index is not required. However, a detailed subject index and a numerical patent list by countries are given as part III.

Blasting for armour stone presents many unique challenges. Often, blasters get themselves into trouble when they assume that they can simply modify a production blast design to produce the required fragmentation size. They bid on the project using typical production blasting costs, and then find that they cannot produce the required fragmentation size and quantity with an acceptable waste factor. This paper outlines industry best practice guidelines for the production of riprap or armour stone. The paper examines the importance of initial geological mapping to determine the average in-situ block size, and the advantages to be gained from working with the geology to maximize production of the required block sizes. The paper also outlines the importance of proper explosives selection as well as blast designs that are unique to the production of riprap. The paper reviews challenges and experiences realized from multiple projects in differing rock types.

By the use of flotation tests it was confirmed that there are two functions of oxygen in sulphide mineral flotation. Sized samples of galena and chalcopyrite were sulphidized, washed free of excess sulphide ion, and then contacted with air-saturated water for certain lengths of time. The mineral was then transferred to a sealed laboratory flotation cell containing water with a known concentration of dissolved oxygen. Xanthate and frother were added and flotation continued for 1 min.Both the extent of mineral oxidation and the concentration of dissolved oxygen markedly affected the flotation of galena and chalcopyrite. The response to a change in either variable is dependent on the level of the other, but generally best flotation occurs with the maximum degree of air saturation of the pulp. Galena flotation is hindered by oxidation of the mineral beyond a small threshold amount but a similar effect for chalcopyrite was not observed...

To respond to ever-increasing challenges, mining companies have been experimenting with new business models and technologies in order to speed mine development, production, processing and transport. The various experiments involve mobile access to data, intelligent sensors, advanced analytics, virtual collaboration, virtual reality, automated equipment, remote operations centres and strategic partnerships. A synthesis of all these components creates a ænetwork effectÆ that can dramatically improve the overall effectiveness the entire operation. The concept of the Network Centric Mine borrows from a variety of industries, including defence, aerospace, aviation, petroleum, utilities and manufacturing. Central to the concept are improvements to the way we connect people, process, information and technology, resulting in a more highly collaborative business model, with roles and processes designed to take advantage of new technology. The components of the mining operation act as a connected network aimed at a common goal, regardless of the boundaries of organisation and location.The design of this new organisational ecosystem needs to take into account the recent advances that enable the æplug and playÆ integrated collaborative environment, including cloud computing, mobile devices and social networking, as well as advances in augmented reality, robotics, geo-location, digital labelling, simulation and analytics. However, the most consistent lesson from other industries is the need to properly enable the behaviour and process changes necessary for success.Benefits from adopting a network centric approach include the more effective use of scarce expertise, improved data and information analysis, reduced process variance, and more informed and faster decision-making. The future of mining will see more technology savvy workers who will drive the adoption of new processes. "There is a very small fraction of wireless experts in the world with experience in underground installations and an even smaller fraction that have experience specific to building mine shafts. This is why in 2009, Redpath created its own system to meet the demands specific to shaft construction. Today, after a dozen shaft installations and several design iterations, Redpath has the expertise and experience required to implement a proper wireless system in any shaft configuration.This document will explain multiple proven wireless communication designs optimized for shaft construction and suitable for permanent use. This includes line of sight installations up to 1200m, short range mesh networks, and the latest patent pending Integrated Leaky Feeder Mesh Network. Additionally, this document will explore the lessons that Redpath learned in its effort to interconnect the people and tools needed to build a mine shaft.INTRODUCTIONAfter a decade of research, several design iterations and a dozen application examples, Redpath has resolved two wireless network architectures to best serve mine shaft construction projects.The long range line-of-sight (LOS) wireless network is the most cost effective and easiest to install, but requires an unobstructed line of sight between the working area and the shaft collar. As such, long range LOS is best suited for ventilation shafts or during the first phase of shaft sinking, before the shaft is equipped.Alternatively, a mesh network uses radios installed at pre-determined intervals in the shaft and on each conveyance. A wireless mesh uses antennas to bridge intervals between shaft radios, whereas Redpath’s integrated leaky feeder mesh network (patent pending) bridges these intervals with radiating cable, commonly referred to as a “leaky feeder” cable. Both of these mesh configurations can provide continuous reception for hoist conveyances travelling in shafts without line-of-sight, making them ideal for shaft equipping and even permanent installations in both mine shafts and lateral drifts.BACKGROUNDHoist operators receive operating commands using what are commonly referred to as “bell signals.” These instructions are delivered in a series of coded pulses on a spring returned pull cord or push button that is typically hardwired back to the hoist operator’s control room from one of many locations in the mine shaft. During the construction phase of a traditional mine shaft, a worker riding inside the sinking bucket would have to reach out to a pull cord installed on the shaft wall to send a stop command to the hoist operator. Today, Redpath’s modern shaft sinking projects use fully enclosed personnel carriers to protect the workers riding inside, specifically preventing workers from reaching out into the shaft while the hoist is in motion. According to the literature, material transportation represents 50 per cent of the operating costs for an open pit mine. In this context, this paper proposes a multistage approach for dynamic truck dispatching in real open pit mine environments, implemented in the commercial package SmartMine®, marketed by Devex SA. The first stage of this approach defines the optimal number of trucks that maximises the tonnage production by means of a robust linear programming model, which considers the operational constraints of a real mining process. The second stage uses a dynamic dispatching heuristic joining computational simulation and multicriterion optimisation techniques for decision making for truck dispatching. Another relevant contribution of this work is the publication of realistic instances collected from a dispatching system managing an iron mine, containing information about the hauling and loading fleet, the equipment situation when the working shift starts, cycle times, the time line of special occurrences (such as equipment stoppages and loading equipment reallocation in order to satisfy the mine planning) and the tonnage produced. These instances were generated with the purpose of submitting the method to the test of a real situation that considers the uncertainties existing in the process studied. The proposed approach was validated by means of a discrete event simulation model applied to the previously designated instances. Using this multistage approach as a mechanism for dispatch decision making, it is shown that the haulage fleet productivity increased when compared with the real values reported from the instances. These results are important because they show that the proposed approach generates efficient dispatch decisions for trucks operating in a real environment, reducing operating costs, increasing production and making the mine more competitive.

"Sub-standard practices and their adverse impact on safety performance remain a challenge in the South African gold mining industry. The purpose of this study was to investigate the root causes of sub-standard practices and the effect on safety performance in South African gold mines. The focus of the study was on the underground production operations at a gold mine in the Free State. The study consisted of three parts: a personal investigation into the causes of sub-standard practices at the mine, a comparison with the results of a cultural study performed at the West Wits mines, and a behavioural survey.The studies described in this article yielded similar results in terms of the key drivers behind sub-standard practices. It was found that substandard practices had a number of causes, and the origin lies within the habits, attitude, and behaviour of employees. Current methods of addressing sub-standard practices and improving safety performance were found to discourage employees from performing work safely and according to procedure. It was concluded that the lack of critical behavioural habits for the given work environment and job title could possibly be the primary reason for the occurrence of sub-standard practices. IntroductionSince the implementation of the Mine Health and Safety Act, 1996 (Act 29 of 1996) and the Mine Health and Safety Regulations, the South African mining industry has been striving to improve health and safety performance and has placed great emphasis on adherence to mine standards.The Act requires employers in the mining industry to report accidents, incidents, and dangerous situations to the Regional Principal Inspector of Mines in their area. The data provided by employers is captured and analysed by the South African Mines Reportable Accident Statistical System (SAMRASS) and made available for public viewing (Department of Mineral Resources, 2011).A review of the national mine-related accident statistics shows clearly that the South African gold mining industry has improved its safety performance markedly over the past decade, but is still facing major challenges (Figure 1).
