And a new generation of safety culture experts are telling mining companies how to use it: It is a situation that is still too common in the mining industry: the latest personal protective gear is in place. The safety rules are known and followed. And then, in a moment of inattention or carelessness, a mistake is made and someone gets hurt or killed. Common wisdom claims that such accidents are impossible to eliminate entirely. Mining is dangerous, and some miners will always be hurt and killed. An increasing number of mine safety experts, however, reject the common wisdom. A year with zero mining deaths is possible, they say; with the use of one of the newest weapons to the mining industry in the safety war: the concept of safety culture. Safety culture refers to the set of attitudes, beliefs, perceptions and values that influence safety in the work environment. It is also sometimes called the way an organization behaves when no one is watching. Robert Peters, of the Office of Mine Safety and Health Research (OMSHR) at the National Institute for Occupational Safety and Health (NIOSH), describes safety culture this way: ?It?s what?s believed on a day-to-day basis. For example, if a miner sees a workplace hazard and he truly believes that his boss wants him to identify the hazard and bring it to the company?s attention, that would be an example of a positive safety culture.?

Motivated by safety considerations, logistical issues, transportation cost savings, product diversification, and marketing preferences, several uranium producers are revisiting their business models and supply chain configuration and, instead of selling dried yellowcake to downstream refineries, they are moving to build in-house calcining facilities. Drying and calcining plants are relatively low-cost facilities to build and operate. The execution period of a typical yellowcake drying and calcining project is also relatively short and alternative construction methodologies, such as modularized solutions, are available. The drying and calcining technologies used for yellowcake are relatively mature, environmentally contained, and well supported by qualified vendors. The drying and calcining circuits also seamlessly integrate with upstream and downstream operations. These either produce a dried yellowcake or calcined U3O8 which are notably safer and easier to transport and market. This paper presents an overview of the recommended technology and project development considerations for yellowcake drying and calcining facilities.

"Soil mixing is a proven solution for the construction of ground improvement adjacent to sensitive structures due to the use of computer controlled and monitored equipment. The precision to which geometry is controlled can often be the deciding factor for selection of soil mixing versus other means of ground improvement. Protection of existing structures and utilities requires careful planning, investigation, and may demand supplemental control measures. During planning, it is imperative that the process be fully developed by the project team to ensure proper control measures are budgeted; otherwise, the success and safety of a project can be jeopardized.This paper discusses case histories of soil mixing performed in close proximity to large, sensitive, and critical utilities, including foundation support for an existing earthen dam impounding a water reservoir in upstate NY, a buffer zone for a tunnel drop shaft at an operating wastewater treatment plant in Washington DC, and a containment system to protect a major water line along an interstate widening project in Baltimore, MD. The function of soil mixing as ground improvement varied; however, the selection and application of the technique versus other means of ground improvement was specific due to the controlled manner of construction with soil mixing. Through the three case studies, small diameter (3- ft [0.9 m]), large diameter (8-ft [2.4 m]), and mass mixing applications will be discussed. Furthermore, the case studies offer insight into how the geotechnical community in the Mid-Atlantic utilizes soil mixing technology for widely varying applications. Additional topics of discussion will be the design and construction of the ground improvement systems, procedures and execution of the work to protect existing structures, and the QA/QC methods, procedures, and results from the projects.INTRODUCTIONWet soil mixing, also known as the Deep Mixing Method, is a ground improvement technique that improves the characteristics of weak soils by mechanically mixing them with cementitious binder slurry. To construct columns, a powerful drill advances a mixing tool with radial mixing paddles located near the bottom of the drill string. The binder slurry is pumped through the drill string to the tool as it advances and additional soil mixing is achieved as the tool is withdrawn.Wet soil mixing is applicable in a wide range of soils. Soil mixing has been performed at depths more than 100 lf (30 m). Soils vary widely in their ability to be mixed, depending on the soil type, strength, water content, plasticity, stratigraphy, and texture. Almost any soil type, including organics, can be treated with certain soil mixing technologies, although they may require high binder contents and/or pretreatment to achieve the required results. In cases when the target soil is very soft, including some very weak clays and peats, 100% of the soil may require treatment. This complete coverage of soil mixing is referred to as mass mixing."

"With stricter limits globally being imposed on cyanide discharge levels in plant tailings, having a reliable and well designed cyanide detoxification circuit is critical for any operation. The INCO SO2/Air method is most commonly used for cyanide detoxification where sulfur dioxide and oxygen combine to oxidize cyanide to the much less toxic cyanate. This paper discusses the steps involved with the development, from laboratory through to design, of a robust cyanide detoxification circuit. The goal is to produce a cyanide detox circuit that reliably meets discharge target with the lowest operating and capital cost.Design begins in the laboratory where continuous tests simulate full scale operation of a detoxification circuit. Detailed design relies on several design parameters, based on theory and experience, to ensure the reactor can satisfy requirements of the cyanide destruction reaction. Selection of the correct reagents and their supply to the reactor is important to an operable process with minimal operator workload. The operability of the system relies on correct process control strategies to maintain optimal reagent addition and chemical conditions in the reactor. These design factors are critical to providing a robust plant that will consistently meet cyanide discharge levels, ensuring uninterrupted production and plant profitability."

Many injuries are caused each year by rock falls in coal mines. Most of these injuries are not caused by major roof collapses, but from falls of smaller rocks from the immediate top or roof skin. Various surface controls are used in mines to control this surface rock. One that has been found to be very effective is roof screening. Depending on the size of the screen, roof coverage approaching 100%can be achieved. Many mines are reluctant to use screen for primary skin control because of additional costs of time and materials, but others are having great success at both controlling costs and surface rock. Data is presented from two mines that show a dramatic reduction in roof skin injuries when screening is used. Much of this success is due to innovations in roof bolting machines. Four case studies of roof screen experience are presented along with associated costs of materials, impact on bolting advance rates, and potential ergonomic risks. The effects of roof screening on skin control and safety are also included. Finally, this paper will provide information about features of different roof bolting machines that affect production and safety.

This paper describes a collaborative project undertaken by Newcrest Mining Limited (Newcrest) and pitt&sherry to identify, develop and then implement global best practices to manage the exposure to inrush (dry and mud) hazards in underground mining operations. An extensive literature review was undertaken to identify past experience globally and the current state of knowledge and practice in managing inrush hazard. A key requirement of the literature review was the development of a list of guiding questions which would form a framework for approaching operators to discuss the topic, capture recent learnings and understand current operating practices to allow the development of a best practice solution. An initial cohort of 14 mines across four continents chose to contribute and detailed interviews were carried out by the project team. The outcome from the literature review, interviews with participating mines and various internal discussions was a comprehensive summary document detailing Basic versus Best Practice management of the inrush hazard. This document is being used as a gap analysis tool by Newcrest to assess and guide the management of inrush hazard at their existing and future operations. The summary document has also been provided to the participating mines for their consideration. The project is iterative and will continue to evolve as new experiences, knowledge and practices come to light.

"Even with the cyclic nature of mining, when it comes down to it, the bottom line is that whatever direction the commodity market is going there is always a desire to improve the economics of an operation in order to deliver a greater return to investors. Adapting to market pressures by introduction of an operational process change can often have a downstream impact. What may be less obvious is that reconciliation can play an important part in measuring the effectiveness of many operational changes. Through the adoption of best practice reconciliation and related comparisons, it is possible to measure the impact of changes across the mine value chain and identify variability that may have been introduced. Using analytical business improvement tools, examining and monitoring planned changes can be crucial in understanding whether the introduced changes have been effective and whether they have provided the expected result. A considerable debate has arisen regarding the best route for the beneficiation of iron ore. Supporters of jigging technology claim that although the technology is old, due to recent advances in jig control and a better understanding of the separation processes within jigs, it offers significant advantages over dense medium separation, especially in terms of reduced capital and operating costs and simplicity of design. Supporters of dense medium separation claim that this technology is proven to provide the best possible separation and that the gain in efficiency over the life of the plant, compared to that of a jig, will more than offset any differences in capital or operating costs. The truth is that both viewpoints are valid and that the choice is ultimately determined by specific characteristics of the ore being treated. Factors that need to be taken into account in deciding upon the correct option include the size/density distribution of the ore to be treated, the different size fractions which must be treated (lumpy or fines), differences in yield at grade, plant layout and capital and operating costs. Each of these factors will be discussed in detail and a case study for both lumpy and fines will be presented.

The Geita Gold Mine is located 80 km southwest of the town of Mwanza in the north-west of Tanzania and is 100 per cent owned by AngloGold Ashanti Limited. Open pit mining commenced at Geita in 2000. In 2011, the operation produced 494 000 oz at a cash cost of US$536/oz. This is a significant improvement from 2008, when 264 000 oz were produced at a cash cost of US$742/oz.A period of relatively poor operating performance at Geita was recorded over the period from 2006 to 2009. A period known at Geita as æthe turnaroundÆ commenced in 2009, and has been remarkable to say the least. Over the last couple of years the mine has embarked on a journey, which saw various interventions and initiatives. Among the key initiatives were Project ONE (which focuses on business process framework, systems for people and safety transformation), asset integrity, fatigue management and contract mining of satellite pits.Since the commencement of the turnaround at Geita, gold production has matched market guidance. The mine has returned to positive cash flow on the basis of strong performance on cash management, improved production and increased gold price.This paper explains some of the details of the mine operationsÆ improvements, how they were implemented, their relative contributions to improved mining performance and the path ahead to best practice. This paper examines manning best practice in the Australian mining industry, comparing the mining of gold with mining of other commodities. In particular, the paper considers a range of mining operations and their manning levels as measured by operational benchmarking. The benchmarking results presented cover 7 open pit and 14 underground studies of gold mines in Australia from a database of over 50 studies. The data were collected as part of AMC?s benchmarking and operational improvement studies conducted between 1999 and 2009. The results examine overall manning levels in mining, processing and administration functions. Manning levels for a range of mining methods, including open pit and underground methods, are segmented. Productivity measures such as ore tonnes per man day and metres developed per man day are presented and where direct relationships can be established, the relationships are described. The proportion of supervisory to operations personnel and labour costs as a percentage of total operating costs is highlighted. Minimum manning requirements for the range of mining methods are indicated, irrespective of the size of the operation. The benchmarking results provide guidance by indicating a range of manning numbers and trends which are likely to be appropriate for the scale and type for a range of mining operations. When combined with benchmarked productivity and cost data, manning results help identify potential areas for business improvement, driving gains in the efficiency of labour. Finally, the paper considers perspectives on best practice for manning and the limitations in application of this type of analysis in a global context.

"Hearing loss due to excessive noise exposure is the most prevalent industrial disease. It is ranked on the World Health Organization’s (WHO) priority list for major work related injuries. Uncontrolled noise not only has a physical effect, but is also a psychological hazard affecting the performance and well being of man. Noise induced hearing loss (NIHL) is irreversible, yet it is preventable. The first defence against noise (noise control engineering) is a high priority in the mining industry. Second defences should be a last resort when the engineering control of noise is inadequate. Hearing conservation programs and the use of conventional hearing protection devices seem to have failed in the prevention of noise induced hearing loss. If compensation payouts that quadrupled in the past ten years are used as a barometer for the success of existing hearing conservation programs, then new strategies and technologies need to be investigated. The best practice Hearing Protection Device (HPD) may be seen as the minimum standard for second defence noise control.Conventional hearing protection is widely applied, but also misused in the industry as well as in the mining sector. Empirical research results, ownership, general attitudes of workers as well as qualitative aspects will be explored. The paper provides reasons, evidence, case studies and strategies regarding the best practice hearing protection philosophy (the Noise Clipper Model) as implemented in the Implats Mining Group. Worldwide tendencies towards custom-made HPD’s, findings that indicate improvements in hearing conservation programs and finally the successful application of Custom-made Hearing Protection Devices, will be outlined."

Modern remote mining operations in Australia are more commonly trending towards fly-in/fly-out (FIFO) sites. This trend is driven by numerous factors including increased business centralisation, reduction in operating costs, and a desire to live in coastal cities that offer closer family networks and a greater diversity of facilities. Consequently, the resource development industry has had to adapt. When WMCÆs Mount Keith Nickel Operation commenced operations in 1994, it was developed as FIFO for its entire workforce. Since then, there have been several permutations of rosters within a 21 day compressed work cycle including the 14/7 cycle. This paper discusses a case study of the change in management process to the 9/5 work cycle focusing on minimising fatigue in the workplace. It includes the discussion of the business case, employee reaction and the practical application. The impact of the best practice change after 18 months is discussed.

The status of sampling practices in the gold mining industry in Africa was determined as an initial step in a process to standardize sampling practices in the mining industry. Twenty-one gold mines, twenty metallurgical plants, and thirteen laboratories were rated for the potential influence of the relevant sampling errors on each component of the particular sampling system. The findings of the study on the status of equipment, standards, procedures, and management principles were presented in a dissertation (Spangenberg, 20121) and are now used by AngloGold Ashanti?s mines in the quest for correct sampling practices. The information is also used to compile a guideline on leading practice procedures for sampling methods in gold mining. Even though safety professionals and mine operators strive to reduce these types of injuries, there has not been a substantial decrease in the rock fall injury rate over the past decade. Various surface controls are used in mines to control the roof skin. One control that has proven to be very effective is roof screening. Roof screen effectively controls roof skin and provides a high roof surface coverage. Although many mines are reluctant to use screen for primary skin control because of the additional costs of time and materials, others are having great success at controlling both the costs and surface rock. In this study, injury data are presented that show a dramatic reduction in roof skin injuries when screening is used. Much of this success is due to the protection from falling rock that the operators receive from roof screen. Five case studies in which roof screen was used are presented along with the associated costs of materials, impact on bolting advance rates, and potential ergonomic risks. The effects of roof screening on skin control and safety are also included. Finally, this paper provides information about best practices and features of roof bolting machines that affect production and safety.

Reconciliation has become very topical over the last ten years but how do we distinguish what is good practice, and even more importantly, show how it adds value? Reconciliation at Anglo American has received support from the very top of the company and this has enabled success in the design and implementation of a range of processes, reporting activities and actions. Focus has been on full Mine Value Chain Reconciliation (MVCR) from Resource to Product and on proactive cross-functional engagement. A standardised set of reconciliation definitions and relationships are used at all mines across the company to diagnose planning and performance issues. Results are formally presented in an efficient dashboard format, accompanied by a concise narrative to communicate what has been learned and what is being done to correct, improve or change practices. Importantly, effort has also been made to sustain MVCR by embedding good practice and quantifying the benefit of reconciliation explicitly via linking results to value.This paper sets out the lessons learned from the past two-years’ journey, highlighting areas where good practice has been defined and achieved. A ranking for quantifying reconciliation confidence over time is presented as a means of putting results into context and continuously improving the reconciliation process. GOLD AND SILVER, the monetary metals, have presented in the last year a striking contrast; gold has experienced unusual prosperity, while silver has been depressed more severely than ever before. Gold mining has been hailed as the only basic industry that is able to thrive on depression; the new gold produced is thought by many to be the most important material for the reconstruction of prosperity. Silver production, meanwhile, has been sinking to its level of 40 years ago, and its price to a level never before recorded. These factors have reduced the value of silver produced in 1932 to about the value of the average annual production more than a century ago, that of the decade 1801-1810. No other major commodity in world trade appears to have suffered a greater decline during the present depression.

Heavy ANFO products made by mixing ANFO (or AN prills) with a relatively small amount of emulsion (typically 15-30 percent) have become very popular in recent years. They are less expensive than straight, sensitized emulsion and maintain the good shooting characteristics of ANFO but have higher bulk strengths. Their use, however, is generally limited to larger diameter, dry boreholes. Smaller diameter, wet-hole quarries have not been able to widely utilize this low cost bulk product.

Making progress on sustainable development will require greater attention to the management of materials produced from mine exploration through use to reuse, recovery and recycling. Material stewardship is a concept that embodies the range of activities required for optimal and appropriate use of minerals and metals in society. The following paper provides an overview of some recent initiatives that promote the stewardship of minerals and metals in society. The initiatives described are being conducted by organisations ? the International Zinc Association, the Canadian Copper & Brass Development Association and the International Council on Mining & Metals. These associations illustrate stewardship activities at three different levels: - an international association representing companies that produce a range of minerals and metals; - an international commodity association; and - a regional association. For each initiative, information is presented on its development, scope and outlook. This paper draws heavily from work of the associations but the viewpoints are the responsibility of the authors.
