The paper describes the design process, trial testing and contract pile installation of 772 pre­cast concrete piles at a site in Surrey, South East England. Recommendations on how to improve the design, construction trials and main contract piling phases of the project are made. The building layout is shown in Figure 1 and is comprised of a central circulation area with five office wings radiating out from its southern side (only three of these wings were constructed in Phase 1 of the project). Approximately 60% of the circulation area and one office wing were built with a single level basement. The basement was constructed "bottom-up" in open cut with the void between the basement walls and the cut slope being back-filled with compacted site won soil above a perimeter drainage system at basement slab level. The building has up to 3 stories above basement level and is a reinforced concrete structure. The client for the project is Pfizer Ltd. Pfizer Ltd was represented on site by Laing Project Management (Laing). The consulting engineer was Ove Arup and Partners (Arup) while the piling contractor was Roger Bullivant Ltd (RBL).

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

"The Cenex orebody occurs at the intersection of northwest and northeast-trending fault zones that formed during post-Hudsonian uplift. The ore is localized in breccia zones, along faults, and in veins and adjacent wallrocks. The host rocks are Tazin quartzo-fe/dspathic mylonites and ultramylonites and chloritesericite- quartz-graphite-feldspar mylonite schist. The uraniumbearing minerals are finely disseminated pitchblende and uraniferous titanate.The earliest mineralization may have been associated with /ate-stage retrograde metamorphism in the mylonite schist. The main stages of mineralization were associated with brecciation, hematization, carbonatization, and chloritization of the mylonites and ultramylonites. Uranium mineralization was clearly much later than any igneous or metamorphic activity associated with the Hudsonian orogeny.Late post-orogenic uplift, erosion, and deposition of the Martin continental clastics in fault basins were coeval with uranium mineralization. The deposit is consistent with a meteoric-hydrologic system constituting a source for orebearing fluids, hence the close spatial relationship of the uranium deposits in the Beaverlodge area with the basal unconformity of the Martin Group."

This paper describes a collaborative project undertaken by Newcrest Mining Limited (Newcrest) and pitt&sherry to identify, develop and then implement global best practices to manage the exposure to inrush (dry and mud) hazards in underground mining operations. An extensive literature review was undertaken to identify past experience globally and the current state of knowledge and practice in managing inrush hazard. A key requirement of the literature review was the development of a list of guiding questions which would form a framework for approaching operators to discuss the topic, capture recent learnings and understand current operating practices to allow the development of a best practice solution. An initial cohort of 14 mines across four continents chose to contribute and detailed interviews were carried out by the project team. The outcome from the literature review, interviews with participating mines and various internal discussions was a comprehensive summary document detailing Basic versus Best Practice management of the inrush hazard. This document is being used as a gap analysis tool by Newcrest to assess and guide the management of inrush hazard at their existing and future operations. The summary document has also been provided to the participating mines for their consideration. The project is iterative and will continue to evolve as new experiences, knowledge and practices come to light.

"The valuation of mineral properties in the U.S. is only partially regulated. The regulations which have jurisdiction or impact appear to have such mainly as a consequence of unintended fallout, since the regulations were designed for other purposes. Court case history can also be important. The combined results are a mixture of bad and good, with important lessons to be learned.Since 1981, the Securities and Exchange Commission has prohibited U.S. listed companies from reporting quantitative estimates of mineralization and the value of mineralization, other than proven and probable reserves. This results in the minerals appraiser (valuator) working with a shortage of data in his everyday work, both on the subject property and in sales analysis.Between 1989 and 1995, all 50 states and essentially all Federal agencies adopted the Uniform Standards of Professional Appraisal Practice (USPAP) for real property appraisals (valuations). The major national appraisal societies also require their members to abide by USPAP. A significant portion of minerals appraisals must now follow these standards. This paper will discuss the content of USPAP. It provides a very good framework for the valuation of a minerals property or a mine, both as real property and as a business. However, the credentialing standards for real property appraisers are now prohibitive for minerals appraisers."

"Even with the cyclic nature of mining, when it comes down to it, the bottom line is that whatever direction the commodity market is going there is always a desire to improve the economics of an operation in order to deliver a greater return to investors. Adapting to market pressures by introduction of an operational process change can often have a downstream impact. What may be less obvious is that reconciliation can play an important part in measuring the effectiveness of many operational changes. Through the adoption of best practice reconciliation and related comparisons, it is possible to measure the impact of changes across the mine value chain and identify variability that may have been introduced. Using analytical business improvement tools, examining and monitoring planned changes can be crucial in understanding whether the introduced changes have been effective and whether they have provided the expected result. "Construction of large facilities underground in urban areas has many advantages. In order to show the advantages, a case study of the Norsborg metro depot in Stockholm Sweden has been made. The Norsborg metro depot will be the largest and most modern underground metro depot in northern Europe. The depot has been built in hard rock in an urban area with small impact on the environment and local residents. The paper describes how it is possible to build underground facilities in a cost efficient way and take care of the challenges regarding working environment that comes with locating a permanent working place underground. INTRODUCTION The Stockholm area is one of the five fastest growing regions in Europe and home to more than a fifth of Sweden's population on just two percent of Sweden's surface. The population is growing rapidly, with about 35,000 people per year and is expected to reach 2.6 million in 2030. In order to meet the development and enable continued growth, housing projects and expansion of infrastructure need to keep the same pace; this is an easy conclusion to make but much more difficult to realize in practice. The main challenge is to accomplish development in a sustainable and efficient manner, minimizing the use of resources and land. By constructing facilities underground more urban space will be made available for development and/or for greening of city centers. Considering sustainability factors such as land value in decision making for placing of structures or facilities, underground locations could become a first choice alternative. During 2015 and 2016, several major underground infrastructure projects in the city of Stockholm will start, including a 14 km sewage tunnel and an 18 km by-pass highway tunnel. Furthermore the Stockholm metro system is currently undergoing the largest expansion since the 1970’s, a 19 kilometers extension of the metro system including new trains and new stations is planned. During 2013 - 2015 Skanska Sweden took part in the construction of a new metro depot placed underground in the southern part of Stockholm; the Norsborg metro depot. This is a top modern urban underground facility that in 2017 will serve as depot for 17 new metro trains. The project was selected for “Outstanding Project of The Year” at the ITA Tunneling Awards 2015."

On June 20, 1972, Ocean Energy was awarded Contract No. H0220055, entitled Portable Task Luminaire System Development. Initially, this was a 12 month program consisting of three phases: Phase I -- Design of the Luminaire System and Battery Chargers, Phase II -- Fabrication of Prototype Units, Phase III -- Fabrication of Production Prototype Units and Battery Chargers. In addition to developing the Portable Task Luminaire System, however, this contract was amended a number of times in the course of the program to include the development of a Low Coal Portable Task Luminaire, a Rib Mounted Luminaire System, and a Machine Mounted Luminaire System, all based upon the same high frequency fluorescent lighting and safety technologies involved in the Portable Task Luminaire. This report covers the activities of this program as amended, and includes the development and fabrication of all of the systems. This report constitutes the final fulfillment of the contract.

The combined effects of the addition of Silicon (Si) as alloying element and low temperature annealing on the tensile properties of 70/30 brass with Nickel (Ni) and Iron (Fe) contaminants is investigated in this paper. Melts of cartridge brass were made to which 0, 0.5, 1, 2, 3 and 4 wt % of Si was added. These were sand cast into rods of 600 mm by 10 mm diameter and were thereafter machined into standard tensile test samples. Some of these samples were annealed in a muffle furnace at temperatures of 250, 300, 350, 400 and 450 oC respectively and the others were used as control. Subsequently, all the heat treated and non heat treated samples were subjected to tensile tests on a Hounsfield Extensometer and the load – extension plots retrieved were analyzed. Microstructural characterizations of the samples were carried out using Accu-Scope Optical Microscope. The results showed that cartridge brasses subjected to alloying and annealing treatments had improved tensile and yield strengths. However, the tensile properties were increased and maintained within acceptable limits at stress relieve annealing temperatures.

A considerable debate has arisen regarding the best route for the beneficiation of iron ore. Supporters of jigging technology claim that although the technology is old, due to recent advances in jig control and a better understanding of the separation processes within jigs, it offers significant advantages over dense medium separation, especially in terms of reduced capital and operating costs and simplicity of design. Supporters of dense medium separation claim that this technology is proven to provide the best possible separation and that the gain in efficiency over the life of the plant, compared to that of a jig, will more than offset any differences in capital or operating costs. The truth is that both viewpoints are valid and that the choice is ultimately determined by specific characteristics of the ore being treated. Factors that need to be taken into account in deciding upon the correct option include the size/density distribution of the ore to be treated, the different size fractions which must be treated (lumpy or fines), differences in yield at grade, plant layout and capital and operating costs. Each of these factors will be discussed in detail and a case study for both lumpy and fines will be presented.

The report is as follows: The Joint Committee on Licensing of Engineers, which included a Committee of the Boston Society of Civil Engineers and representatives of seven other societies,* submitted to the members of the respective societies, on Jan. 8, 1921, a report in which it was stated that on Jan. 4, 1921, a petition was filed in the Legislature of Massachusetts by the Boston Chapter of the American Association of Engineers accompanied by a bill to provide for the registration of professional engineers and land surveyors. The Geita Gold Mine is located 80 km southwest of the town of Mwanza in the north-west of Tanzania and is 100 per cent owned by AngloGold Ashanti Limited. Open pit mining commenced at Geita in 2000. In 2011, the operation produced 494 000 oz at a cash cost of US$536/oz. This is a significant improvement from 2008, when 264 000 oz were produced at a cash cost of US$742/oz.A period of relatively poor operating performance at Geita was recorded over the period from 2006 to 2009. A period known at Geita as æthe turnaroundÆ commenced in 2009, and has been remarkable to say the least. Over the last couple of years the mine has embarked on a journey, which saw various interventions and initiatives. Among the key initiatives were Project ONE (which focuses on business process framework, systems for people and safety transformation), asset integrity, fatigue management and contract mining of satellite pits.Since the commencement of the turnaround at Geita, gold production has matched market guidance. The mine has returned to positive cash flow on the basis of strong performance on cash management, improved production and increased gold price.This paper explains some of the details of the mine operationsÆ improvements, how they were implemented, their relative contributions to improved mining performance and the path ahead to best practice. This report summarizes the cost of U.S, mining and milling industry compliance with the Environmental Protection Agency (EPA) interim water quality guidelines for waste discharges. Applicable excerpts from Public Law 92-500 are stated and interpreted in conjunction with the guidelines being used for implementation of the law. Over 200 U.S. Army Corps of Engineers applications for discharge permits were used to develop the quality and quantity data base needed to estimate the cost to comply with EPA guidelines, Cost of three basic treatment methods were considered: Aeration, addition of lime, and sedimentation. From the process design, cost-capacity graphs were developed to consider maximum flow rate, maxi lime addition, and average flow rates. The graphical method used in this evaluation should provide a reasonable guide to the effect of waste treatment and, therefore, Public Law 92-500 and corresponding EPA guidelines on the mineral processing industry. The cost of complying with the EPA guidelines will have the effect of prompting the mining industry to minimize the production of waste water and of undertaking some additional treatment, Two case studies are presented to illustrate efforts that might be fruitful for minimizing waste water production and for treatment of combinations of acid mine drainage, mill wastes, and metallurgical process wastes when treatment is deemed necessary.

"Hearing loss due to excessive noise exposure is the most prevalent industrial disease. It is ranked on the World Health Organization’s (WHO) priority list for major work related injuries. Uncontrolled noise not only has a physical effect, but is also a psychological hazard affecting the performance and well being of man. Noise induced hearing loss (NIHL) is irreversible, yet it is preventable. The first defence against noise (noise control engineering) is a high priority in the mining industry. Second defences should be a last resort when the engineering control of noise is inadequate. Hearing conservation programs and the use of conventional hearing protection devices seem to have failed in the prevention of noise induced hearing loss. If compensation payouts that quadrupled in the past ten years are used as a barometer for the success of existing hearing conservation programs, then new strategies and technologies need to be investigated. The best practice Hearing Protection Device (HPD) may be seen as the minimum standard for second defence noise control.Conventional hearing protection is widely applied, but also misused in the industry as well as in the mining sector. Empirical research results, ownership, general attitudes of workers as well as qualitative aspects will be explored. The paper provides reasons, evidence, case studies and strategies regarding the best practice hearing protection philosophy (the Noise Clipper Model) as implemented in the Implats Mining Group. Worldwide tendencies towards custom-made HPD’s, findings that indicate improvements in hearing conservation programs and finally the successful application of Custom-made Hearing Protection Devices, will be outlined."

"Risk management in tunneling is more important today than it was a few years ago, considering increasing demands for safety and environmental and socio-economic sustainability, which are now required by citizens, owners, lenders, and insurers. Moreover, our current risk management philosophy should not only address risk avoidance and mitigation, but should also be used as a means of value creation, benefiting the overall project. Given the strategic implications of risk management, the authors intend to present, in this paper, a comprehensive overview of references and experiences from current practice—e.g., what is missing or weak, as well as possible or necessary developments that can have a key role in implementing effective risk management, particularly for a project’s design and construction phases. Therefore, as God has set some things in broad daylight; as he has given us some certain knowledge, though limited to a few things so, in the greatest part of our concernments, he has afforded us only the twilight, as I may so say, of probability (John Locke) PREFACE Many large-scale engineering and construction projects suffer millions of dollars in cost overruns, delays in design and construction, and operability troubles once finally completed (Edward W. Merrow, 2011). The undesired effects of these problems can become disastrous for large-scale megaprojects (those that typically cost US$1 billion or more), particularly considering that these are increasingly used as the preferred delivery model for infrastructure (Bent Flyvbjerg, 2014), often in public-private partnerships (PPPs). McKinsey G.I. (2015) has estimated that bridges and tunnels average 35% in cost overruns, and roads 20%. Some of the values published by Flyvbjerg (ibidem, 2014) for projects with relevant underground elements are the highest, with cost overruns from 60% to 300%. The analysis of 32 other case histories (from 1994 to 2010) given by Hartmut, R. (2011, IMIA Annual Conference, Amsterdam) indicates an average increase of costs around $55 million, with a 20-month average delay."
