"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

Safety work in mining at the Andes Copper Mining Company, Potrerillos, Chile, is divided into three parts: (1) accident prevention, (2) fire prevention and protection, and (3) silicosis prevention and control. In the company's operations a steady campaign has been waged to reduce accidents and silicosis, and to prevent fires. In the following pages the safety record for the past twelve years will be examined and the methods explained by which the accident rate was reduced. Table 1 contains yearly data pertaining to the scale of the mining operations and the accident record. Block caving is used for all ore production and the tonnage mined has aver- aged 30,000 short tons per day for the past seven years. Table 2 classifies 44 fatal accidents which have occurred since 1935. The leading causes of accidental death are transportation of ore and use of explosives, followed in order by per- sons falling, falls of ground, and by asphyxiation. Probably the newest thing in mining safety, or safety for mines, is the apparent dissatisfaction on the part of the mineral industries, as represented by both management and labor, and the general public of the Nation, with the safety record of the mining industry. This is reflected by published comments and papers in the technical press;l-9 in labor periodicals,10,11 in popular journals and periodicals,l2, 13 and in the daily press; also by discussions of the Safety Committees sponsored by the National Safety Council,9, 14-15 the American Mining Congress,l.l6 the American Institute of Mining and Metallurgical Engineers,2,4 the Mine Inspectors' Institute of Amerthe Coal Mining Institute of America,5 19 the American Standards Association, and others.20.21 Mining safety is a joint problem of management and the individual workman. Safety is not attained and will not be attained solely by laws, decrees, and commands because, if it could be done in this manner, acceptable mining safety would have been achieved long ago by the individual states in our Nation and by the nations abroad. The best safety performance is found at plants where management provides a safe environment for the workers; where supervision is efficient and super- visors, others concerned with promoting safety, and the individual worker know through training, experience, and personal contact the safe and unsafe practices of their field; and where the trained individual worker has become safety-minded by experience, quiet influence, unconscious suggestion, and personal guidance. Despite what may be said to the contrary, progress has been and is being made in mining safety. This is shown in Fig 1 to 4. It is beyond the scope of this paper to give possible reasons for the valleys and peaks in the fatal-injury rates. Some recent factors that appear to be contributing to the improvement in these rates are briefly discussed. Aside from mining equipment, "what's new in mining safety" for some mines has been in effect or was tried by others long ago. A safety pro- gram is a living thing; as such, it must be vitalized and continuously fed with enthusiasm, constructive action, and improvements. Because safety is concerned with human conduct, it is necessary to practice safety at all times; if not, it is soon forgotten, and accidents occur. Mining-safety Records The large employment and production concerned with anthracite contribute much to the mining-safety records of the mineral industry. Approximately 81,000 persons are. employed in this industry having mines concentrated in an area of 480 square miles in northeastern Pennsylvania. Because of the achievement of the anthracite industry in safety during 1948, the other branches of the mineral industry can profit by a study of what has been done to achieve that record. Where one mineral is produced by other large groups having methods of mining that are similar in many respects to the anthracite-mining methods, an achievement in such a large industry is important. Salient factors concerned with safetye in the anthracite industry are: no radical change has occurred in the mining-safety regulations pertaining to anthracite mines. Some countries are infinitely poorer than others in mineral resources-despite the perfect equality of their political leaders ambitions. Compare the mineral poverty of Kenya and Tanganyika with the riches of Katanga Province and Northern Rhodesia. Compare Paraguay with Chile, There never has been any equality in the distribution of nature's wealth. Minerals cannot play any role at all in economic development where they don't exist, and there are bound to be differential rates of economic development where there are basic differences in the wealth of mineral resources available. Africa is my principal area of interest and I know that there is an unfortunate but widespread misconception about Africa's "vast mineral resources, "its "untapped mineral wealth." Why is it untapped? Why is it undeveloped?

During the time period from 1990-2007, 1601 reportable fires occurred in the U.S. mining industry (an average of 89 fires per year). The leading causes of U.S. mine fires were flame cutting and welding operations, frictional heating and ignitions, electrical shorts, mobile equipment malfunctions, and spontaneous combustion. The fact that mine fires continue to occur with an alarming regularity reinforces the importance of recognizing and eliminating the potential hazards and the overall need for improved fire control and suppression technology to ensure the best possible outcome during a mine fire. The National Institute for Occupational Safety and Health (NIOSH) is conducting an evolving comprehensive program of research that is addressing mine fire prevention, detection, management and suppression. This paper presents a summary of recent research accomplishments and provides an overview of the next phase of the NIOSH mine fire research program.

A DISCUSSION of the developments of the past decade in the field of refractories, and the effect of these developments on the performance and life of open-hearth furnaces, is perhaps best introduced by reference to Fig. 1. This shows the most important types of refractories doing into various parts of the furnace, together with their cost, and remarks on their use. It has been estimated that half of the refractory makers market comes from the iron and steel industry, and something like one-third of all refractory brick used finds its way into open-hearth furnaces. To melt steel, a furnace capable of withstanding temperatures close to 3000 deg. F. is required. The furnace is assembled of various refractory materials selected by experience and economic considerations. Furnace structures today, from a refractory standpoint, are considerably different from a quarter century ago. Furnace operators, interested in the performance of refractories, care little how they are made or where they come from so long as they give reasonable service. But, the entire history of the refractory is important and technical knowledge of the ceramic materials used in making the refractory is necessary to the manufacturers, so that the proper article gets into the hands of the ultimate consumer.

"Agrium’s Canadian Phosphate Business (CPB) is comprised of Kapuskasing Phosphate Operations (KPO) in Northern Ontario, Canada’s only operating phosphate mine, and Redwater Phosphate Operations (RPO) in Alberta, which manufactures the fertilizer from KPO’s concentrate.Production from KPO’s South Pit commenced in 2009 and introduced contaminant problems: high aluminium from crandallite and kaolinite clays. These issues caused a drastic decrease in P2O5 recovery and throughput in both plants. KPO attempted to reduce Al clays by means of aggressive desliming. Meanwhile RPO experienced high amounts of dissolved metals in their feed that are detrimental to fertilizer production.This paper covers the mineralogy of the problem, its impact on contaminant separation at KPO, on RPO’s process and on Agrium’s Canadian Phosphate Business as a whole.Potential solutions to the problem are discussed and analysed, including a process change to deslime high aluminium streams more aggressively and high grade ore flotation, which is atypical at KPO. The latter is a demonstration of the value of communication between the concentrator and the client, as KPO accepted a recovery loss and increased production costs in order to increase overall CPB profits. General lessons learned are discussed, followed by a section on ore blending.INTRODUCTIONAlthough the existence of the Kapuskasing phosphate deposit has been known since the 1970s, it’s only since 1999 that Agrium has been operating the Kapuskasing Phosphate Operations (KPO). The mine consists of three major pits (Center, North and South) from which a mining contractor hauls the apatite ore to the Agrium concentrator. The beneficiation plant processes the ore, separating contaminants such as Fe, Mg and Al which are detrimental to its client, Agrium’s Redwater Phosphate Operations (RPO). The use of electrical or electronic safety systems to protect personnel, and the engineering tasks associated with the design, manufacture and maintenance of these systems are referred to using the term ‘functional safety’. In the past ten years, the use of such systems has transitioned from its traditional base, in oil and gas major hazard facilities, to being a part of most large mining and materials handling projects. In particular, functional safety is rapidly becoming a significant component of Australian iron ore projects, typically in the form of emergency stops, remote isolation, drive through interlock, robot cell entry or mobile machine anti-collision systems. While these types of systems have been in use in iron ore for a long time, functional safety standards (eg AS61508, AS62061, AS4024.1503) require more rigourous demonstration of the level of risk reduction provided by the systems.The early engineering project development phases require a number of key decisions to be made around functional safety implementation. The decisions made at this point in a project will go on to dictate the required types of risk assessment and analysis, equipment selection, sizing of substations, and training and maintenance requirements for owners. For project teams that are unfamiliar with functional safety, or inexperienced in its application, an incorrect decision or failure to understand the requirements of the chosen standard can lead to a failure to meet contract requirements, as well as major cost and schedule overruns.This paper examines the implications of functional safety on early engineering studies, focusing on the following questions: How does a designer or company determine what standard is best suited to their project? What key aspects of functional safety design need to be considered during project development? Future metal prices play a direct role in both the economic evaluation and short to medium planning of a mine scheduling sequence. It is difficult to predict with accuracy the future price of volatile metal commodities, however, it is possible to fix some future metal price, outside of long term contracts by the selling of futures contracts through a commodities exchange. The futures contract represents a fixed amount of a speci- fied quality of a commodity for future delivery at a predetermined price. Several different types of hedginc are possible, one in particular which is best suited for mining companies. There are relative advantages and disadvantages when trading in futures contracts and particular reference is made to gold contracts on the Sydney Futures Exchange.

Where the equipment, technique and materials are available for the construction of large diameter rock socketted piles, this is frequently the most economical and environmentally acceptable method of supporting very large concentrated foundation loads in geoglogically appropriate areas. This paper outlines the current approaches in Australia to design and installation of large diameter rock socketted piles. The equipment and procedures described generally reflect those developed or used by Vibro-Pile (Aust.), a company which, while installing the usual range of driven and bored piles, specializes in the design and construction of large diameter rock socketted piles and possesses special expertise in drilling in high strength rock. A high capacity crane attached drill for large diameter drilling in rock is briefly described. Discussed in detail are the construction techniques employed and structural and geomechanical design methods adopted.

To aid in winning the recent world war, the Bureau of Mines conducted a National Fuel Efficiency Program, directed to commercial plants throughout the Nation, to save greatly needed fuel, a basic resource vital to industrial production. In planning this program it was realized that to accomplish a satisfactory reduction in fuel consumption in any plant, it would be necessary to reach, enthuse, and usually educate to some extent the actual operators of the various types of plant equipment. A major part of the programs consisted, therefore, of visits to plants and work with the operators, including a review with them of their operating conditions and of educational and guiding literature pertinent to the operating problems involved. Through a Nation-wide organization, local volunteer engineers served in making visits to local plants. Each cooperating plant appointed an employee as the "waste chaser," who headed the work of saving fuel in that plant. The educational literature was designed to interest the operator, as well as serve as a guide in eliminating waste of fuel. Volumes have been written on many subjects relating to the saving of fuel. Writings of the treatise type contain considerable good information but do not have wide appeal and probably will be read only by a few studious operators. The problem is not one of getting clown on paper everything that the experts know and would like to have the operator know, but rather to write something that will actually interest the operator and stimulate him to action. Past experience led to the adoption, therefore, of the "quiz-sheet" type of writing, using a quiz sheet of not more than two pages for any principal item. The results obtained during the National Fuel Efficiency Program in many types of plants further demonstrated that the short quiz sheet is satisfactorily appropriate and does accomplish results. Strainbursts are common in overstressed brittle rock around deep mine openings. Such failures are characterized by a biaxial state of stress in the plane of the opening boundary. A zone of compressive stresses imposes initial failures near or at the surface of the opening that results in spalling of thin slabs of rock roughly parallel to the free face. While the failure of rock under biaxial loading conditions is increasingly common in deep underground mines, only a few representative laboratory studies have been reported in the literature, without emphasis on rockbursting aspects, and none in the last decade. This paper presents the results of a testing program on a granite rock type. Biaxial testing, conducted on 100 mm cubic samples, was carried out to examine the influence of lateral confinement and loading rate on the rock’s ultimate strength. The paper reports the test results in terms of normalized biaxial failure envelope s1/sc-s2/sc. A discussion of the failure mechanisms observed is presented. Although mining is an old industry its management is quite a new science, and its complications are now being recognised as demanding expert knowledge in many branches of technical and professional education. Not the least amongst these isaccountancy, for probably its most important problem is "How to control the expenditure." Unlike most businesses a mining company stands almost entirely in the light of a purchaser in the world's markets, and not as a seller, except say for product so the established and perfected systems of modern industries cannot be adopted for mining purposes. For this reason it has been left for late years to consider the matter seriously, and now a number of well-managed concerns have devised costs systems to suit their own requirements.Very little, however, has been published and even that is disjoined and difficult to make use of. Some of the costs systems in vogue, although elaborate and imposing, are not of very great value for keeping check on wasteful expenditure, or for displaying the commercial effect brought about 'by any particular change in practice, or by an addition or alteration to a portion of existing plant.

Laboratory testwork was carried out on an overall orebody composite, of 2.9 per cent Cu and 4.2 per cent Zn, prepared from drill core taken from the Currawong fine-grained, copper-zinc massive sulphide deposit near Benambra in Victoria. While 90 per cent of the copper was readily recovered in a concentrate of 20 per cent Cu, after grinding to 95 per cent passing 38 microns, zinc responded poorly to conventional flowsheet/reagent regimes. Experience elsewhere with the use of reverse cleaning, to separate pyrite from sphalerite, led to a series of tests in which better (but still modest)metallurgical results were obtained. The best result was a zinc recovery of 55 per cent in a concentrate of 50.5 per cent Zn. The major loss of zinc occurred in the reverse cleaning stage where the unit recovery of zinc was similar to pilot plant results achieved at Mount Isa Mines on similar feedstock. Diagnostic mineralogy suggested that an overall zinc recovery of 70 per cent was probably the practical upper limit, at a 50 per cent Zn grade.

Occidental Oil Shale, Inc. constructed two commercial-sized modified in situ (MIS) retorts, Retorts 7 and 8, at its Logan Wash Oil Shale Mine in February and April 1981, respectively. One of the primary objectives of this two-retort program was to examine large-scale blasting reproducibility for commercial-sized retorts. The blast design used 545 blastholes and 550,000 pounds of explosive in ANFO equivalent per retort. Each retort was blasted using millisecond delay blasting caps tied into a single round. The blasts resulted in rubble beds very close to one another and to predicted results obtained using newly developed limited void blasting technology. The heights of the remaining void rooms over the beds after blasting were within 0.3 feet of one another. This excellent reproducibility is brought into true perspective when compared to the scale of the rubble beds which were 230 feet high with volumes of over six million cubic feet each.

This report presents the results of an evaluation of several indirect methods of measuring the water content of oil-reservoir rocks. The water content of oil-base cores is considered to be most nearly representative of the water content of the undisturbed petroleum reservoir and therefore, the direct method of measuring the volumes of water in cores cut in oil-base drilling fluid was adopted as a standard for comparison. The indirect methods evaluated included (1) displacement through a semipermeable membrane by air, (2) centrifuge, (3) evaporation, (4) mercury injection, (5) chloride dilution and (6) electric logs. Approximately 270 samples of cores from petroleum-reservoir rocks were used in the study. The cores, most of which were cut with oil-base mud in the hole, were taken from oilfields in California, Texas Gulf coast, Colorado, Oklahoma, and Alaska.

Deep level hardrock mining faces increasing challenges from elevated in situ stresses and associated rockbursts. This study aims to develop a systematic framework for evaluating destress blasting effectiveness, with a focus on deep Swedish hardrock mines. The methodology integrates a critical review and adaptation of global best practices: advanced numerical modelling (static and dynamic simulations), field-based rock fracture monitoring using ground penetrating radar and borehole imaging, and application of rockburst prediction criteria including the strain energy storage coefficient (F), brittle shear ratio, and burst potential index. Geostatistical simulations, machine learning models, and industrial internet of things-based, real-time monitoring are proposed to enhance predictive accuracy, model calibration, and operational adaptability. Key findings show that effective destress blasting evaluation requires multi-modal integration of numerical outputs, field observations, microseismic trends, and uncertainty quantification, accounting for site-specific geological variability and dynamic stress redistribution. The study advances the field by proposing a predictive, feedback-driven evaluation framework tailored for deep Swedish mining conditions, capable of improving proactive rockburst risk management. It offers both practical tools for mining practitioners to optimise stress management, reduce seismic hazards, and enhance excavation safety, and academic foundations for future refinement of destress blasting models, geotechnical monitoring strategies, and adaptive design protocols. This research contributes to safer, more efficient deep mining operations by bridging gaps between theoretical modelling, empirical monitoring, and real-time, data-driven blast design optimisation strategies.

"The ISO 9000 Quality Management System Standards have become internationally accepted as the way to manage quality assurance. The author has developed a Maintenance Management System Standard using ISO 9001as a model and is working with a major basemetal mining company to implement this. This paper describes the structure of the standard and the steps required for its implementation.Using a management standard ensures that a system is maintained, reviewed by internal audit, and when instances of failing to follow the procedures (nonconformances) occur, that corrective action is taken. It also ensures that regular reviews are made and that the sys-tem is continuously improved.The Standard is designed so that if an organization decides to register to an ISO 9000standard, the Maintenance Management System will be in a format that is compatible and can be incorporated with little effort into the Quality System.IntroductionIn reviewing maintenance operations at many mining companies, and often seeing those same organizations some years later, two recurring issues have been noticed..The first issue is how dependent effective maintenance is upon individual supervisors and crews. In one situation, a mine that had an excellent maintenance system in the 1980s. Now, with new supervisors and crews, it is starting to develop a system that will bring it back to where it once was. No written procedures remain from the earlier excellent system.The second issue is that most operations feel that maintenance is the sole responsibility of the maintenance supervisors and crews. Nothing could be further from the truth. Maintenance, like safety, is everyone’s responsibility. If the operator does not report problems and perform daily checks, and if the production supervisor runs equipment until it fails, then nothing the maintenance crew does will prevent emergency breakdowns and lost production. More importantly, equipment will be operated in unsafe conditions, putting employees at risk and, with today’s regulations, leaving supervisors open to prosecution.The Maintenance Management System outlined in this paper is designed to establish the present best practices as standard procedures, to provide a continual review so that these practices are improved in the future, and to identify instances where the system fails so that corrective action is taken. Estimation of the dynamic stress field in terms of its amplitude and decay around a blast hole is a key input parameter in all numerical modeling and blast prediction exercises. However, there is very limited information available in literature on these critical parameters, which would be considered essential in any blast modeling exercise. A laboratory-scale experimental study has been carried out in two different rock types to study at very close range the stress wave decay from a blast hole at distances ranging from 3 to 10 borehole radii. The single–hole blasting experiments employed a centrally located detonating cord (5.3 g/m) in 6 mm(1/4 inch) and 10 mm (3/8 inch) boreholes. The coupling medium employed was air and water, in addition to using a thin-walled copper tube in intimate contact with borehole wall to prevent of penetration of explosion gases into the emerging cracks. The paper describes the nature (amplitude and decay) of the transmitted shock pressure and their evolution in the two rock types for these various experimental conditions, which are present to varying degrees in all blasting operations.
