Serious Injury and Fatality (SIF) prevention remains a priority focus within high-risk industries sector. A major mining operation sought to strengthen their approach to the management of high-risk activities through an in-depth review of all historical incidents that had serious injury or fatality potential (SIFp) events (sometimes known as hi-potential events). The analysis found that the organisation invested a disproportionate level of resources, time and effort on low consequence incidents with a high proportion of SIFp incidents going under the radar. It was found that 76 per cent of SIFp incidents were rated in moderate or lower risk categories and conversely, almost half of those incidents with a ‘serious risk’ rating had little or no potential for a serious outcome. As a result of this project, the operation engaged in a renewed focus on critical risk management and enhanced its operating cadence in critical risk for the workforce. This included bow tie analysis, an update to critical risk standards and increased workforce engagement in the use of critical control verifications in the field. Data analytics with lead and lag indicators continue to provide senior leadership team and risk owners with an understanding of exposure for SIFp and control effectiveness. The audience will take away learnings from a real-world case study that provides data-driven insights of the human, local and organisational factors contributing to incidents with SIF potential. In addition, participants will understand the steps taken to categorise and analyse this incident data and the initiatives currently being implemented by the organisation as a result.

A project was funded by the Australian coal industry to investigate the mechanisms that might lead to sample bias when using cross-belt cutters, in order to help coal industry personnel to make better decisions about the purchase, maintenance and operation. It concentrated on DEM modelling of skew cutters. These are set at an angle to the belt with the intention of minimizing disturbance to the non-sampled material. Two bias mechanisms are likely to cause bias for cross-belt cutters. Waves of material are bulldozed off the belt by the upstream side of the body of square cutters and material is thrown by the leading edges of cutter blades for all types of cross-belt cutters. These mechanisms cause some parts of the load of material on a belt to be over-represented. The effects of these mechanisms cannot be made to be negligible, so cross-belt samplers cannot be trusted to produce unbiased samples, especially for segregated streams of material. However, it is possible to give a bound on the maximum likely bias. The grades of two portions of the stream can be estimated by stopping the belt and shovelling off 1/3 of the cross-section of the load on the belt into a container, concentrating on the final side of the belt and the top of the load. The remaining material should be put into another container and the difference in grade determined. The maximum likely bias is typically about 10% of this difference. For a cross-belt cutter, having an extraction ratio near to 100% is not a reliable indication that the cutter has little or no bias. Some bias mechanisms affecting cross-belt sample cutters make sample mass too high and some make it too low, so an extraction ratio near 100% can occur if two bias mechanisms are both active. Valuers of mineral properties should be aware that accountants are a major user of their opinions of value. Reliance by accountants can range from obtaining comfort that book values in balance sheets are not overstated to establishing values by which to assess the fairness of a takeover offer. Accountants using mineral property valuations clearly can't be expected to have the same level of expertise as the valuer and ""second guess"" the valuation. But they do have an obligation, under the ASC guidelines and their own professional standards, to'check the logic and reasonableness of the valuation before being able to rely on it. This means reviewing the valuation report for general sensibility, discussing the methodologies and assumptions with the valuer and being satisfied as to his competence and independence. This paper outlines how accountants use mineral property valuations, the level of scrutiny applied to such valuations and the problems commonly encountered in practice by accountants seeking to rely on them. The paper also explains how values of-mineral properties are established in company balance sheets and why these often differ, quite validly, from fair market values.

The Outokumpu Flash Smelting process today is the most feasible method for copper matte smelting. The economic feasibility of the process has always been one of the main decision making criteria in smelter projects. Thus, today about 50 % of the world primary copper production utilizes Outokumpu Flash Smelting Technology. However, in the future, environmental criteria will have stronger and stronger impact on decision making. Flash Smelting technology offers the best pollution control features of any process. today. It is foreseen that, in the near future, the global trend will be that all the smelters must decrease their emissions at an accelerating rate, compelling the smelters to make increasingly larger investments on environmental protection. In most of smelters ?today, Peirce-Smith converting and transfer of the molten material in ladles are the number one pollution source. The key today for copper converting and minimizing emissions is the Kennecott-Outokumpu Flash Converting process, which has proven to be the most effective converting method in terms of pollution control and, production efficiency including capital and operational costs. Outokumpu Flash Smelting technology is applicable also for small to medium scale smelters but the most interesting area is the upper capacity limit for smelting in one line, which has not yet been reached. We predict that by the year 2000 to 2005, there will be at least one smelter which produces 500,000 tonnes of Cu per year based on one Flash Smelting and Flash. Converting unit. The current state-of-the-art of the Outokumpu Flash Smelting - Flash Converting process is presented in this paper as well as some new options for copper production to be utilized in the near future but which are available already today.

"Bit wear has been proven to have a significant effect on respirable dust generation in coal cutting system. The mechanism of bit wear can be adhesion, abrasion. oxidation, or diffusion. depending on cutting conditions. This paper presents a portion of an extensive ongoing series of studies to better understand the principles of bit wear and dust generation. Used bits collected from underground coal mines were analyzed to identify the basic principle of bit wear in coal and rock cutting. Distribution of weight loss and conical height loss of worn bits were studied and bit wear was divided into four stages according to weight loss. Utilizing these worn bits, Laboratory experiments have been carried out to study the effects of bit wear on coal cutting system and respirable dust generation. An Automated Rotary Coal Cutting Simulator was use to perform the experiments. Acoustic emission monitoring is applied to study the effect of bit wear on fracture formation and dust generation. INTRODUCTIONContinuous miners were introduced in the 1950’s and became the major equipment in underground coal production. Now the larger production in the coal industry has increased the demand for continuous miners and their use. It is well known that serious dust problem is also introduced with the use of continuous miners, especially with recent increase in productivity. On the other hand, legal restrictions are made ·more strict by the Mine Health & Safety Administration for the respirable dust generated by the continuous miners to prevent coal workers from incidence of pneumoconiosis. The incidence of pneumoconiosis depends on dust concentration, exposure time, and physical properties of dust. Of them, dust concentration is the best parameter for the assessment of the risk of pneumoconiosis. Although a lot of research work has been done to reduce dust concentration substantially, some areas are still required further investigation to meet the strict regulations. One of the areas is about bit wear and dust generation. In this area, the principles of bit wear and effect of bit wear on dust generation are major concerns."

Nickel and cobalt in lateritic ores can be extracted by either a hydrometallurgical or a pyrometallurgical route. The high-iron limonitic fraction of a deposit is best processed' hydrometallurgically while the magnesium-rich transitional and saprolitic fractions are more suited for pyrometallurgical processing. During hydrometallurgical processing, it may be attractive, for economic reasons, to blend the limonitic component with transitional and saprolitic ores, which contain higher nickel levels. However, blending increases the concentration of magnesium in the feed, leading to increased acid demands. The decision on how much ore to process then becomes an economic one. Ore samples with different magnesium contents were prepared for the leach test program by blending limonitic and transitional ores. The ore samples were slurried in seawater and tested in the Falconbridge Hydrometallurgical Pilot Plant. As expected, acid consumption increased with increasing magnesium content of the ore. For target extractions of 95% nickel and 92% cobalt, the required residual free acid concentration increased from 30 g/L at 1.7% magnesium to 50 g/L at 2.7% magnesium. When the magnesium content was raised to 3.8% and 4.6% in subsequent pilot plant testing, the final free acid requirements did not increase beyond what was observed at 2.7% magnesium. Thus, the cut-off ore composition to maximize nickel units and minimize acid requirements is between 1.7- 2.7% magnesium. Keywords: high pressure acid leaching, laterites, magnesium

Refractory gold ore contains sulfide minerals like pyrite, chalcopyrite and sphalerite that are difficult to be directly leached by cyanidation or by lixiviants such as thiourea and halides. In such cases, it is necessary to perform oxidative pretreatment of the ore to oxidize the sulfide matrix, and gold particles can be exposed to the solution during the leaching process [1]. In recent years, hypochlorite and chloride solutions have been introduced as alternative materials for hydrometallurgical methods of gold extraction instead of cyanide solution [2]. A previous study by Hasab et al. [3] reported that the gold dissolution rate in hypochlorite solution was higher than in the cyanidation process, due to the presence of hypochlorous acid (HOCl) as a strong oxidizing agent. In a hypochlorite leaching system, the change in pH can influence the presence of oxidant species in the solution: the hypochlorite ion (OCl−) is stable in pH greater than 7.5, HOCl can be found in pH between 4.0 and 7.5, and aqueous chlorine (Cl2 (aq)) is the predominant oxidant when pH is below 4.0. As HOCl is the most effective oxidizing agent for gold extraction, pH should be maintained in the range of HOCl stability [4].

Wheel loaders became important bulk materials handling tools in the early fifties. As they grew in stature, productivity, reliability, and acceptance, the demand for larger, more rugged machines evolved. In the early sixties, the introduction of a 6-yard model by a major equipment manufacturer led quickly to widespread experimenting and rapid worldwide acceptance in loading blasted rock in stone quarries and smaller metal mines. As the wheel loaders grew in popularity for shot rock loading, larger models, 12- to 15-yard size were developed and saw widespread use by 1970. In the past 20 years, at most quarries and small to medium size mines, most cable operated power shovels have been replaced by large wheel loaders and more recently, hydraulic excavators as the primary loading tool. Wheel loaders larger than the 15-yard class are not used widely. Until about 1970, very few overburden haulage trucks were used in domestic coal mines. About that time, many states started enforcing stringent mine relcamation regulations and in 1977, even more stringent federal legislation was passed. These controls brought about drastic changes in coal surface mining methods in the mountainous areas of Appalachia, in smaller midwestern mines and in the large mines of the western states. Basically, simple material casting methods were replaced at most eastern mines by "haul-back" and "mountain top mining" methods using controlled placement of overburden as it is removed from the coal. These methods dictated much more extensive use of loaders/ shovels and trucks to the point that most coal mining in those areas is now by loader/shovel trucks methods. And those areas have become the home of the country's largest population of large wheel loaders and trucks in the 35- to 85-ton size class with a trend to even larger trucks and shovels. To maintain equal production or provide for growth, the new methods have caused a sharp increase in machine intensity to the extent that at some mines, up to three times as many machines were required to mine a million tons of coal in the late seventies as compared to the machine needs in the sixties. Mining costs and coal prices also took a big jump in the mid-seventies, paralleling the drastic price increases in oil and other forms of energy.

The replacement or reconditioning of oil storage tank bottoms due to external corrosion is an expensive maintenance item to the oil industry. Cathodic protection as a means of mitigating this problem has proved very successful in the past few years. One pipe-line company recently used magnesium anodes to cathodically protect two 55,000 bbl and two 20,000 bbl tanks. The tank bottoms were protected as individual units with considerable factual data being taken on one of the 20,000 bbl tanks. Twelve 60-lb anodes were installed about 2½ ft from the tank on approximately 17-½ft centers around the tank. Previous installations and tests have shown that sufficient protection is provided when the tank-to-soil potential using a copper-copper sulphate half-cell as a reference electrode is — 0.90 volts at the edge of tank. Potential measurements taken a few months after completing the installation indicated that the potentials were higher than were needed for protection. Resistors were then inserted in the individual anode circuits which increased the life of the anode installation and still provided adequate protection for the structure. All connecting pipe lines were then insulated from the tank. This limited the anode current strictly to the tank resulting in a greater degree of protection with a still longer anticipated anode installation life. The total installation cost for this tank was less than $300 which amounts to $20 per year The corrosion of metallic structures in contact with soil or water has been a very serious and costly problem for centuries. The annual loss to our nation's pipelines alone is estimated at $200,000,000. Mitigation of this problem by various methods has been carried on quite successfully for a number of years. An electrical method called cathodic protection has been used with considerable success to control corrosive influences on pipelines, tank bottoms, cable sheathing and other buried metal structures. Corrosion of oil storage tank bottoms has been a serious problem among the oil industries for years. In addition to damage costs and loss of oil, the replacement of one tank alone represents several thousand dollars. A reconditioning program at periodic intervals was instituted by several organizations and found to be a very expensive process. A few years ago two of the pipeline companies installed cathodic protection units at their tank farms.3'4 The average current required for protection of the tank bottoms varied from 0.7 to 1.0 milliampere per square foot. The results indicated that adequate protection was obtained at a cost of approximately 10 pct of the previous reconditioning program. The occurrence of rock bursts in some deep mines throughout the world constitutes a severe operational hazard. In addition to endangering the safety of the miners, the rock-burst problem is a major factor in mining economically in deep mines because of the loss of production and expense owing to cleanup and repairs following a burst. Developing means of controlling or eliminating rock bursts has been the subject of intensive research for over 50 years. While much has been learned about the effects of rock bursts, little is known about their basic causes; hence, control measures have been at best only partially effective--attested to by the number and severity of bursts that continue to occur throughout the world each year. In the Coeur d'Alene mining district of north Idaho, the rock-burst problem continues as the silver and lead veins persist at depth. These veins are presently being mined at greater than 7,000 feet below the surface and will be mined to some 10,000 feet in the future. The majority of rock bursts in this district occur in pillars. While pillar bursting has been essentially eliminated in some deep-mining districts by longwall techniques, modification or application of this technique to the thin, irregular, and near vertically dipping veins of the Coeur d'Alene district has not been found to be practical. The creation of pillars is a consequence of the horizontal cut and fill mining method used. Figure 1 shows a vertical longitudinal projection of the silver vein at the Galena mine, illustrating the mining method. Stoping progresses between raises and is carried upward between levels. A horizontal pillar is created as a stope approaches a mined-out level. Recent broad-band micro seismic monitoring of rock-burst-prone stopes by the Bureau of Mines has shown that potential burst zones could be delineated weeks in advance of a burst and that a definite sequence of events took place prior to the occurrence of a pillar burst (1). Hence, time was available for control measures to be effected when a stope pillar reached a critical burst geometry.

MR. NEILSON was born Aug. 12, 1842, at Philadelphia, Pa., where he died Dec. 30, 1906. His business career began with his graduation, in the class of 1862, from the Polytechnic College of the State of Pennsylvania. For a short time after graduation he was instructor in mathematics at the college, and then became associated with Messrs. Booth & Garrett, chemists, of his native city: In 1867 he was sent to the Adirondack mountains to care for mining interests of the late Jay Cooke, and spent about three years there, operating the forges at Elizabethtown, Essex county, N. Y. He also prepared an interesting monograph on Catalan forges and their operations. In 1868 he published a chart showing the material progress of the United States, and the cost of certain staple products during a series of years, which is believed to be the first of its kind issued in this country. In 1870 he made a trip to the Pacific Coast, prior to entering the service of the Pennsylvania Steel Company. In 1871, when the Logan Iron & Steel Co., of Burnham, Pa., failed, Mr. Neilson was appointed receiver, re-organized the company, became' its General Manager, and spent more than four years in this work. In the meantime, he became interested in the Freedom forge of the same place, and, with Mr. Wm. Burnham and Mr. Milton Brosius, laid the foundations for what is now the Standard Steel Works. From Jan. 1, 1877, to April 1, 1890, he was General Manager of these works, and closely associated with the Baldwin Locomotive Works. In 1878 he was in charge of the first consignment of Baldwin locomotives to Europe, which, with a picked crew of men from Baldwin's, he took to Eydtkennen, Russia, and placed in service. THE time of the Section delegates was economized this year by providing the section reports in mimeo- graphed form, together with the reports of the officers and committees of the Institute, for their consideration on arrival. The meeting was called for Thursday forenoon and despite a certain amount of oratory managed to get through before all the luncheon was gone. In all, nine section delegates and one from the Iron and Steel Division attended and took part. One delegate, who had come many miles, got lost in the complication of the program and failed to attend. Six appointed dele- gates failed to attend and two sections neglected to make appointments.

"THE MICHIGAN COPPER DISTRICT.Location. The copper mines of Michigan are all in the north¬ern part of that state on the Keweenaw Peninsula, which extends from the (Michigan) mainland for about seventy miles northeast¬erly toward the middle of Lake Superior. The mines occur in a belt or zone occupying the central portion of the peninsula from end to end.. The most productive mines for many years past have been those located not far from the city of Houghton in' the central part of the zone, in a section extending about ten miles southwest of Houghton and fifteen miles northeast. The most important mines in order of.location, from the northeast to south¬west, are the following: Mohawk, Ahmeek, Allouez, North Kearsarge, Wolverine, South Kearsarge, Centennial, Calumet. and Hecla, Osceola, Franklin, Quincy, Hancock, Isle Royale, Superior, Baltic, Trimountain, and Champion. The first three named are in Keweenaw county and all of the others are in Houghton county. Still farther to the southwest are the Lake, Mass, Adventure, Michigan, Victoria, and White Pine mines. These are in Ontonagon county.History. The exploitation of these copper mines extends back into prehistoric times. There is little evidence that the American Indians carried on mining operations in this region in the period shortly before the advent of white men, but there is abundant evidence that the ancestors of the Indians, •or some earlier inhabitants of the region, carried on primitive mining operations. For example, Mr. S. O. Knapp, Agent of the Minne¬sota Mining Company, in the winter of 1847-48, made the first discovery of prehistoric mining. This event is described as fol ,lows by Foster and Whitney : Location: As shown on the accompanying map, the Mar-quette district extends from the region of Marquette on Lake Superior westward for about forty miles. The main basin of the district is only about three to six miles wide, but outlying areas which are commonly regarded as belonging to the same mining unit, make the north and south dimension of the district about twenty miles. The most productive portion of the area is in the vicinity of Negaunee and Ishpeming, .about ten miles from Mar¬quette, but important mines are found also all along the range as far west as Michigamme and also to the south in the fold known as the Republic Trough. Comparatively recent developments indicate that important ore bodies have been developed in the outlying Swanzy district about eighteen Miles nearly due south of Marquette.The Marquette Iron District.History: The first discovery of iron ore in the Lake Supe¬rior region was probably that made in 1844 near the site of Ne-gaunee by the government surveying party in charge of William A. Burt, sent out under the direction of Douglass Houghton. This surveying party was using a solar compass, invented by Mr. Burt, and noticed variations in the direction of the magnetic needle at various times and places, but. on the 19th of September, while running the east line of Township 47 North, Range 27 West, the needle varied from its normal position until it stood nearly at right angles thereto."

AT Lime Spur, Mont., the East Butte Copper Mining Co. has been quarrying limestone for twenty years. The quarry is beside the Northern Pacific R. R. in the Jefferson River canyon, 4 ½ miles east of Cardwell, at Lime Spur, Montana. HISTORY This quarry was opened early in 1900 by Dan Morrison of Whitehall, the discoverer of the Morrison cave, to supply a demand for smelter flux. Limestone was sold to the Pittsmont smelter of The East Butte Copper Mining Co. and to the Butte Reduction Works. As no particular effort was made at the quarry to select the rock that delivered to the smelters was unduly high in magnesia and impurities. The East Butte com¬pany, therefore. Bought the quarry. in 1909 and confined operations to the high-grade beds in order to supply rock of the desired purity for its own smelter and the Butte Reduction Works until the latter shut down. It also developed a market for some of its rock with the sugar factories. The latter market increased until in later years all rock, larger than 4 in. in size over a bar grizzly, was being shipped to the sugar factories, and the rock smaller than 4 in. was being used as a flux in the blast furnaces at the Pittsmont smelter. In 1924, the East Butte concluded a contract with the Anaconda company for the treatment of its ores, and the Pittsmont -smelter -was shut down: which made the minus 4-in. rock a useless by-product. Since there was not enough revenue in the sugar rock alone and also because of limited waste room it became necessary, if Lime Spur was to continue to operate, to increase- the proportion of plus 4-in. rock and to develop a market for the minus 4-in.

Air blast associated with detonation of explosives in mining, construction or military applications is often responsible for complaints by people who live or work in the vicinity of the activity. Most often, the complaints are based on the perception that vibration caused by the airblast damaged one or more residential buildings that may be located within several hundred feet or several miles from the blast site. The potential for airblast to cause damage is related to the amplitude and frequency of the airblast overpressure wave at the receptor. Amplitude and frequency of the overpressure wave are, in turn, dependent upon the type and amount of explosive detonated; the degree of confinement; the distance the wave travels; the topography between the blast site and the receptor; and, atmospheric conditions. It is well known that, as the result of atmospheric focusing, the amplitude of airblast overpressure that exists relatively far from the blast site may be substantially higher than the amplitude associated with the attenuated wave in an isotropic atmosphere. Two methods are available to predict baseline overpressures 1) the Sandia Laboratories, Atomic Energy Commission method as extended by the Naval Ordinance Laboratory (NOL method) and the Ballistic Research Laboratory method (BRL method). These methods are developed by different techniques, are based on different idealized atmospheres and predict widely different results. The amplification factor applied to account for the effect of focusing must be consistent with the method used to predict the baseline overpressure. Theoretically, the amplitude of the overpressure at distances several miles from the blast site could be more than 25 to 100 times the amplitude of the attenuated wave predicted by the BRL method. However, based on comparisons with measured overpressure data is probably less than approximately 3 times the amplitude of the attenuated wave as predicted by the NOL method. This paper is based on research conducted as part of investigations of airblast damage complaints associated with an accidental explosion near Eveleth, Minnesota and with the disposal of military explosives by detonation at the Sierra Army Depot near Herlong, California. The techniques available for estimating the airblast overpressure that might be generated at various distances from the blast site and criteria for evaluating the potential for damage are examined.

Media mills operate by having energy transferred to the media which in turn grinds the feed material in a packed bed. Regardless of the mill type, the media energy density per unit surface area is constant. Since for optimum grinding conditions, both peak pressures and particle number of the bed depth are constant, the frequency of energy input is inversely proportional to feed size. As the particle size decreases the voles energy density increases linearly necessitating cooling in stirred mills. Grinding finer particles therefore, requires moving from rotary to vibratory mills, and the finest, the stirred mills.

The following paper is based chiefly on work done for the North Carolina Geological Survey, and is presented here by permission of Professor J. A. Holmes, State Geologist. It represents, in a very general way, the field-results obtained during the summer months of 1893, and 1894, and part of 1895; and the portions pertaining to North Carolina are, to a certain extent, abstracts of a report shortly to be published as Bulletin No. 11 of the State Survey. Less emphasis, however, is here laid on the petrographic descriptions of the peridotites, and more detailed treatment is given to subjects pertaining to methods of mining, cleaning, and utilization of the commercial product, corundum. Some of the most extensive fields for investigation which this subject presents have been scarcely more than suggested.

Multiple-point simulation is a family of methods used to generate conditional facies simulations that present the same spatial dependence as a user-defined training image. The main advantage of using a training image is the easy integration of geological concepts in numerical subsurface models. The strength of multiple-point methods is that it allows making the best use of geological expertise when data are too sparse for inferring a model of spatial continuity. However, despite being appealing to geologists, the multiple-point methods are often not used in practice due to the difficulty of building complex 3D training images that contain realistic features.A new framework for multiple-point simulation is presented that involves small and simple training images. The main idea is to use random transformations to expand the range of structures available in the simple training image. The training image is not regarded any more as a global conceptual geological model, but rather a basic structural element of the subsurface. Complex geological structures are obtained, whose spatial structure can be parameterised by adjusting the statistics of the random transformations, based on field data or geological context. In most cases, such parameterisation is possible by adjusting two numbers only. One advantage is that the training images are so simple that they can be easily built even in 3D. The method allows building 3D models that reproduce shapes corresponding to a desired prior geological concept and are in phase with different types of field observations, such as orientation, facies or geophysical measurements. The flexibility of the approach allows for naturally integrating different types of data. This is demonstrated on an example involving seismic data assimilation. Examples where complex 3D folded structures are built from a simple training image and a few orientation data are shown.
