"In metal and nonmetallic mineral mines during the 15-year period 1911 to 1925, 30.17 per cent of all fatalities and 16.57 per cent of all injuries were caused by falls of rock or ore from the back or roof or from the wail; 1.24 per cent of the fatalities and 11.05 per cent of the injuries were caused by rock or ore while loading at the working face; 6.66 per cent of the .fatalities and 2.49 per cent of the injuries resulted from men falling down 3 chute, winze, raise, or stope; and run of ore from chute or pocket caused 1.48 per cent of the fatalities and 1.93 per cent of the injuries.3 Hence, accidents due to falls of men and material caused. 39.55 per cent of the fatalities and 34.05 per cent of the injuries in these mines. Accident prevention can best be assisted by each company's preserving complete records of accidents classified according to cause and rate per thousand or 10,000 shifts. A systematic method of recording accidents and the employment of graphs or charts will be helpful to the company safety department in analyzing the cruses of accidents. The purpose of this publication is to analyze data on accidents caused by falls of men and material in metal -mines of the United States, and to suggest methods of preventing or reducing such accidents."

DOCTOR Albert Sauveur, distinguished scientist and Honorary Member of this Institute, predicted in the first Howe Memorial Lecture that the privilege of delivering this annual address would be considered a highly significant honor. The intervening years have well borne out Sauveur's prophecy. I wish to assure the American Institute of Mining and Metallurgical Engineers that I also deeply appreciate this honor, recognizing at the same time the present and even more the future responsibilities with which it is accompanied. Several of the previous lecturers had the good fortune to attend the classes of Professor Howe and to work in his laboratories. I was similarly favored, although in my instance and to my great regret the period was all too short. However, Howe's book, published in 1903, "Iron, Steel and Other Alloys," afforded me abundant opportunity to continue these studies. How well I remember the inspiration I derived from this famous book by Howe, the metallurgical master, pioneer, and educator! Remembering that Professor Howe contributed numerous papers to The Metallographist, a magazine supported from a technical point of view by the world's pioneers in metallography, I was curious a few days ago to learn the nature of Howe's early contributions to this magazine. The first paper by Howe in the first year of The Metallographist (1898) dealt with the allotropic theory; the second also treated of allotropy, and I quote the first sentence of that paper: The allotropic theory of the hardening of steel may, like so many other important theories, have to be accepted or rejected not on direct proof or disproof, but on the accumulation of circumstantial evidence for or against it. Howe's ability to stimulate others by his deep understanding and clarity of expression was truly remarkable; but just as striking was his intuitive judgment concerning the implications of metallurgical data in the arts and sciences. It therefore seems fitting that this lecture comprise a broad treatment of the subject chosen, rather than a closely documented thesis on any one phase of the problem; that in it attention be focused

Introduction Although autogenous grinding applications date back to the early years of mineral processing when tumbling-type mills were first used, there has been a renewed interest in this type of grinding over the past few decades. A review of the historical aspects of autogenous grinding is provided which follows the development of the various types of mills and processes. Wet and dry grinding operations using either autogenous or semiautogenous mills, including the Aerofall mill, are described. Em¬phasis has been placed on the test work required to evaluate the feasibility of using autogenous or semiautogenous grinding for a spe¬cific application. The factors which must be considered to establish the appropriate design criteria for these types of grinding circuits are discussed in detail. Successful application of these grinding circuits results from a thorough test program, a careful analysis of the test results, and a practical engineering approach to the design of the process flowsheet and milling equipment. Certain pebble milling applications, where the pebbles consist of the ore or rock itself, can be classified as autogenous pebble mill grinding, or better known as ore-pebble mill grinding. In the generic sense, pebble milling includes those grinding operations using a tum¬bling mill in which the grinding media consists of various types of pebbles, either ore or other pebbles, instead of metal balls. Conse¬quently, this subject is covered separately in Chapter 5, Pebble Mills. Operating data from many of the installations located throughout the world are included in this chapter. Our appreciation is extended to those companies who contributed this important information. History of Autogenous Grinding F. C. BOND Autogenous grinding, the grinding of ore by itself rather than by special metallic or nonmetallic grinding bodies distinct from the ore, has an old and honorable place in the art of mineral processing. It antedates the use of the rod mill and is only a few years younger than the development of the continuously fed rotating tumbling mill in which it is accomplished. Its history71, 72 is inextricably bound up with that of gold mining. Every important early development of autogenous grinding took place on ores of gold, except in one case of silver ore. In common with many other features of modern mineral processing, autogenous grinding would have been greatly retarded if the winning of gold had been so small a part of mining in the past as it is today. Gold unlocked the secrets of rock-on-rock grinding. In years past gold ores were crushed in stamp batteries and then flowed over copper plates covered with mercury which amalgamated the gold. The McArthur Forrest cyanide process for the dissolution of gold in a dilute alkaline cyanide solution appeared in 1890. With finer grinding it recovered more gold than could be won by amalgama¬tion alone. The first response was to stamp in cyanide solution and finer mesh screens guarding the stamp discharge. However, the fine screens greatly restricted the tonnage stamped, and they wore out rapidly. Rotating tumbling mills70, 73 were the obvious choice to grind the coarse product from stamps. Many of these were Gates tube mills; more than 1,000 of these were sold between 1907 and 1913. They were made in 6, 51h, and 5 ft diam, with lengths of 20 and 22 ft. The 5 x 22-ft size was the most popular. The term tube mill did not refer to the mill shape, but included all tumbling mills using grinding pebbles. The flint or chert rocks found along the coasts of Denmark and Normandy are probably the most durable and resistant to abrasive and tumbling wear of any on earth. They were much used in the early tube mills. The early linings were all silex or Danish flint blocks, cemented into place with portland cement. Local hard stones or cast iron blocks were also used later. The cast iron liners carried lifters and were not bolted to the shell, but were held in a tight circle against it by steel wedges hammered home between them. The first ribbed metal liners designed to hold grinding pebbles pounded into them by the mill action were developed at El Oro, Mexico, about 1905. These and the Forbes, Komata, and Tonopah types, as well as the Osborne type developed in South Africa within about five years, were very successful in reducing liner wear. They sometimes lasted two years or more, and equalled or exceeded the life of silex lining at a lower cost. Some of the types were patented. The pioneer name recorded in the history of autogenous grinding is that of Kenneth L. Graham. In 1907 at the Geldenhuis Deep Ltd. mine near Johannesburg, he ran a comparative test on two tube mills. One mill used pieces of the common gold ore, or banket, and the other used the customary imported Danish pebbles as grinding media. He published the first account of autogenous grinding.'' In a test lasting 81 days the ore pebbles showed a definite saving over the Danish pebbles. Graham's historic paper also describes the invention of B. Chew of the Crown Deep Mines of the first trammel to be attached to a grinding mill discharge. The first known account of autogenous grind¬ing printed outside of South Africa was the description of the Graham test in Mining and Scientific Press's The use of grate- vs. overflow¬discharge was also described at this time." The news of this successful test appears to have had immediate effect, for by the end of 1908 the number of tube mills on the Rand had increased from 72 to 120.76 Presumably, most of these mills were now using autogenous grinding. The ore was principally a hard conglomerate of quartz peb¬bles cemented by silica, and was eminently suitable for use as a grind¬ing media. It is unfortunate that in South Africa the new autogenous process continued to be called by its old name of pebble milling, since the name does not distinguish between grinding with ore and grinding with extraneous pebbles. For increased clarity the former is called rock pebble milling. The first historical stage in the development was what is now designated as secondary autogenous grinding, or rock pebble milling. It consists of grinding a feed all passing 'A in. or finer with ore pebbles about 2 to 4 in. in diameter. The next stage to develop was that now called intermediate autogenous grinding, where larger amounts of larger pebbles about 5 in. in size were used to grind ore which had been crushed to about rod mill feed size or somewhat finer. It was developed on the Rand and in other places where coarse stamp screens were used to increase mill tonnage. In many cases the early secondary and intermediate autogenous mills have now been replaced by conventional ball mills and rod mill-ball mill circuits, with the object of further increasing grinding capacity. The third stage to develop is designated as wet primary autogenous grinding, in which run-of-mine ore or a primary crusher product is all fed to the autogenous mill together. It developed many years later indepen¬dently in the United States and in South Africa. The fourth and most recent stage is that of dry primary autogenous grinding, which developed in the United States and Canada. All four stages originated in gold mining. The first use of commercial autogenous grinding outside of the Rand apparently took place five years later in 1912 at the Santa Gertrudis gold mine at Pachuca, 55 miles northeast of Mexico City. This paper presents the main features of coal mining in Japan, as determined by the author while attending the International Coal Conference in October of 1963. The general economy of the country is considered, and the requirements for coking coal are outlined. Statistics and comments on production and consumption are then given, with accompanying cost figures, and the characteristics of Japanese coal mining are compared with those of Canada. Mining methods and underground transportation are covered, with emphasis on undersea mining. Finally, a description is given of a visit to the Miike colliery, the largest in Japan. I T was my privilege to attend the International Coal Conference held in Tokyo in October, 1963, to meet many prominent members of the Japan Coal Association and to visit one of the major coal fields. Indoor physical security, as a perpetual and multi-layered phenomenon, is a timeintensive and labor-consuming task. Various technologies have been leveraged to develop automatic access control, intrusion detection, or video monitoring systems. Video surveillance has been significantly enhanced by the advent of Pan-Tilt-Zoom (PTZ) cameras and advanced video processing, which together enable effective monitoring and recording. The development of ubiquitous object identification and tracking technologies provide the opportunity to accomplish automatic access control and tracking. Intrusion detection has also become possible through deploying networks of motion sensors for alerting about abnormal behaviors. However, each of the above-mentioned technologies has its own limitations. This paper presents a fully automated indoor security solution that leverages an Ultra-wideband (UWB) Real-Time Locating System (RTLS), PTZ surveillance cameras and a Building Information Model (BIM) as three sources of environmental data. Providing authorized persons with UWB tags, unauthorized intruders are distinguished as the mismatch observed between the detected tag owners and the persons detected in the video, and intrusion alert is generated. PTZ cameras allow for wide-area monitoring and motion-based recording. Furthermore, the BIM is used for space modeling and mapping the locations of intruders in the building. Fusing UWB tracking, video and spatial data can automate the entire security procedure from access control to intrusion alerting and behavior monitoring. Other benefits of the proposed method include higher accuracy and robustness, more complex query processing, and interoperability with other BIM-based solutions. A prototype system is implemented that demonstrates the feasibility of the proposed method.

The concept for a Mine Escape Vehicle (MEV) is motivated by the desire to enable personnel to evacuate a mine rapidly as the first and most desirable response to any underground emergency. Under a NIOSH contract, Raytheon UTD developed a conceptual design for a vehicle that coal miners could use to facilitate their rapid exit from an underground coal mine in the case of emergency, including fire or explosion. The vehicle requirements include the capacity to provide life support functions for a number of miners, operate in an oxygen deficient, low or no visibility atmosphere, and travel at speeds faster than miners can walk out of the mine. To develop the MEV concept, three primary tasks were identified and executed. The first task focused on developing a realistic concept of operations (CONOPS) and associated high level vehicle requirements. The second task identified and evaluated currently available equipment and technologies that could be applied to address the CONOPS. The final task developed a design concept that integrated the identified components for the five (5) MEV sub-systems to provide the foundation for future detailed engineering design and prototype construction. The resulting MEV design concept is centered upon retrofitting existing mine personnel carrier chasses with available, mine-worthy equipment capable of addressing the MEV requirements. This approach offers a practical and economic avenue to prototype development and delivery with minimum lead-time and risk. Furthermore, by integrating familiar and available equipment into the design and CONOPS, the resulting Mine Escape Vehicles should also be more readily accepted by the mining industry.

"The geological features of a rock mass have a direct influence on its blasting characteristics. It has been observed that perimeter control techniques provide excellent results in massive crystalline formations but may give poor results in weak and or highly jointed formations.A field study was conducted with the objective of understanding the role played by the geological features of a rock mass in producing and controlling blast damage. The field work involved the assessment of blast damage at several operating mines and the blasting of drift rounds in a geologically dominant rock mass.The effect of rock strength, rock quality designation (RQD), joint parameters and hydrogeological conditions on blast damage is described below. The influence of number and thickness of weak layers on blast damage is also discussed below on the basis of quantitative data. In addition, critical particle velocity ranges for damage to different rock types have also been outlined in this paper."

DURING the past 25 years, mining engineers have seen the development of a multitude of specialized engineering curricula in the mineral industry field. Bachelor degrees are now !ranted in the fields of mineral dressing petroleum engineering, geological engineering, fuel technology, and many others. These fields were all, previously considered to be part of a mining enginieer's work and elementary phases of them were included in mining engineering curricula. Their emergence has left some mining engineers with the fear that nothing will he left for a purely mining engineer to teach, or that mining engineering graduates will be unable to complete in any of these fields with the more specialized students and thus will have neatly restricted employment opportunies. A secondary reaction has been that if students who began to say that the pining engineering curriculum taught everything but mining, since so many of :he subjects that had always been udedAuded in the mining engineering curriculum now were fields in their own right.

To advance technology that can conserve critical materials, the Bureau of Mines investigated a cast-on hard-surfacing that wear resistance and confines alloy additions to wear-prone surfaces. Wear-resistant material in form is on a polystyrene , the is embedded in a mold of unbonded s and the wear-resistant material is transferred to a cas surface at the time of For low-stress abrasion, derived from high-carbon ferro-chrome had higher wear resistance than did commercially available weld-hard. The microstructure of the was typi-white iron carbides. Free carbon added to the ferrochrome powder produced more M7C3 carbides than did material contained in surfacings made from ferrochrome alone, and improved the wear resistance. Several white iron powders with greatly reduced chromium contents also white iron contain-M7C3 carbides. One derived from white iron powder had wear resistance to that of weld-deposited hard facings. Among the parameters investigated, surfacing thickness was found to have a or influence on diffusion of elements out of the , microstructure, and wear resistance. The results of field tests on bucket-wheel excavator teeth and plowshares with cast-on hard surfacing are given.

This work is a first-order study to assess the suitability of an organic acid lixiviant to extract rare earth elements (REE) from coal preparation fines refuse sourced from a Pennsylvania mine with a total REE of ~ 300 ppm. The extraction of REE using an organic acid, in this case 0.1Mcitric acid and 0.5Mtrisodium citrate solution, is compared against 0.5M(NH4)2SO4, 1MHCl, 1.2MH2SO4, and 0.5Methylenediaminetetraacetic acid (EDTA). Ashing the coal waste material prior to leaching tests with the citrate solution nominally improved REE extraction. Buffered citrate solution recovered 7% of the total REE in the as-received Isabella Fines as compared to 11% in ashed samples, whereas (NH4)2SO4 extracted 5–6%, respectively. EDTA recovered up to 33%of the total REE, most likely due to the higher coordination chelate bond. Mineral acids, however, outperformed the organic acids on ashed material (16–52%REE recovery), suggesting that organic acids may not be a suitable competitive option for REE extraction from these types of feedstocks.

Projections of coal reserves in Central Appalachia are essential for long-term planning in the region. Annual production is declining in some states, such as Virginia, and mineable reserves for the Central Appalachian Coalfield are challenged by difficult geological, mining, and market conditions. In order to project coal mining longevity in this region, interviews with mining companies were conducted to assist in the determination and assessment of technical and economic factors that impact coal seam mineability. This information was applied to seam and coal reserve data developed by state and federal governments. The results were carefully analyzed to determine the effects of various factors on the overall mineable coal reserve base.

This report on the state of the art of horizontal boring technology for underground power transmission installations was prepared by the Bureau of Mines at the request of the Department of the Interior. This paper describes the different machines and methods used in augering, impacting, pushing, drilling, and machine tunneling horizontal holes through soil and rock. A review of the borehole survey and guidance tools and techniques applicable to these methods is also given. Nonboring methods used for emplacement of power distribution and transmission lines are briefly discussed. The paper ends with a brief technical forecast, an analytical summary of the state of the art, and recommendations based on power industry needs. Horizontal boring techniques for power distribution lines installation are adequate to meet the requirements of the power industry, particularly where such burial involves relatively short distances in soil or soft rock. For burying power transmission lines, particularly in the harder rocks and over relatively long distances, horizontal boring methods are not as well advanced. Research and development as well as economic incentives to equipment manufacturers could substantially improve the state of the art. Obstacle detection and guidance systems for directing boreholes to specified targets need major improvements to meet the requirements of the power industry for underground burial of distribution and transmission lines

The Chino Mines Company, a Kennecott-Mitsubishi partnership, located in Hurley, New Mexico, recently completed a $405 million modernization of its mine, concentrator, and smelter facilities. The mine/ concentrator modernization commenced in 1980 and the smelter modernization began during 1982. Modernization of the mine and concentrator facilities involved increasing the mining rate from 98 kt/d (108,000 stpd) to 152.2 kt/d (167,800 stpd). Ore mining was designed to increase production from 20.2 kt/d (22,300 stpd) to 34.0 kt/d (37,500 stpd). A new concentrator was constructed near the mine using semi-autogenous grinding. The new smelter uses the INCO flash smelting process, increasing capacity by 40.8 kt/a (45,000 stpy) to 99.8 kt/a (110,000 stpy) of salable copper.

The performance of RETIMET metal foam as a flame arrester on explosion-proof enclosures was investigated by the U.S. Bureau of Mines both in laboratory tests and at the U.S. Mine Safety and Health Administration's (MSHA's) Approval and Certification Center, Triadelphia, WV. The objective of this research was to develop a permissible pressure vent for use on lightweight, vented, explosion-proof enclosures. In laboratory tests, four grades of RETIMET, a stainless steel foam material, were evaluated: (1) 45 NC-7, (2) 45 NC-13, (3) 80 NC-7, and (4) 80 NC-13. Explosive gas mixtures were prepared by a dynamic flow system. Ignition was by a low-voltage arc. Each grade of RETIMET metal foam successfully arrested the flame front in all methane-air tests. To evaluate RETIMET metal foam on large, commercial size enclosures,a multi compartmented enclosure was designed and tested in MSHA's explosion gallery in Triadelphia, WV. Extensive explosion testing revealed that a minimum vent-area-to-enclosure-volume ratio of 11.33 in2/ft3 is required to keep internal pressure rises below 3 psig. The RETIMET metal foam functioned satisfactorily in all tests as evidenced by the absence of external ignitions.

Environmental and cost pressures sparked renewed interest in gravity separation. Industrial sands in Mexico are generally processed by screening, desliming, and flotation. Gravity separation with spirals was evaluated as an alternative to flotation in order to lower operating costs and decrease water pollution. Outstanding operating results are obtained in glass and foundry sand operations, plus a more competitive position for Mexican producers due to lower operating costs. Coal fines, generally recovered by flotation, have the same cost and environmental advantages when processed with spirals.

In Electric Arc Furnace (EAF) steelmaking, dust emissions are collected in a baghouse to prevent their escape to the Environment. This fine dust, predominantly iron oxide, also contains oxides of various other metals introduced into the EAF from the scrap charge. For example, mixed or galvanized scrap brings with it zinc, lead, and small quantities of cadmium which preferentially accumulate in the collected off-gas dust. Stainless steel scrap brings chromium into the charge, some of which also makes its way into the baghouse dust as hexavalent chromium-containing oxides. Because many of the contained heavy metal oxides are water-leachable, the U. S. Environmental Protection Agency has listed EAF dust as a hazardous waste which, as of August 8, 1988, cannot be 1andfi11ed without first being treated. Moreover, beginning August 8, 1991, all dusts with zinc concentrations of 15% or greater will be required to undergo high temperature thermal treatment to remove the heavy metals prior to any further disposal. This deadline represents several EPA-granted time extensions to the original requirements; extensions which were granted to allow for further development of process technologies.

This report, sponsored by the Bureau of Mines, presents the results of an examination of the safety of the Blind Shaft Borer (BSB) shaft sinking system and an alternative modified BSB design concept in comparison to conventional shaft sinking technology. Detailed study of the two systems was also performed to identify and recommend design changes for optimum safety conditions. A comparison of the BSB systems with conventional shaft sinking was made an the basis of relative risk levels. Risk incorporates both the frequency and the potential consequence of accidents. A safety evaluation of the component operations of the two BSB systems was performed using a Failure Modes and Effects Analysis (FMEA) methodology. The results of this analysis show generally that both BSB systems are safer than conventional shaft sinking. The potential risk of conventional shaft sinking operations could be reduced by over 30%, on the average, with the use of the original BSB system, and by an average of about 10% with the modified BSB. Design change recommendations to further improve the safety of these systems include a better fire suppression system, improved man haulage systems and more compact hydraulics. An operational safety manual was also developed for the modified BSB system.

Formulation and commercial utilization of solid-waste incinerator slag by-product is the focus of this paper. Particular attention is given to iron aluminosilicate melts having compositions similar to that of nature's basalt The earth?s crust can be viewed as an environmental laboratory for predicting stability of various silicates; small-scale laboratory experiments are necessary to confirm findings there from. An ideal slag consumes flux additives at acceptable cost, is compatible with furnace refractory, flows readily from the furnace, and has physical properties suitable for its commercial utilization -all within the critical constraint that the slag is environmentally stable. Examples of successful solid waste slagging practice, including tonnage, utilization and pricing, are provided from metallurgical and waste treatment operations of North America. "Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

This paper is based on observations made during a recent visit by the writer to three of the dust research laboratories .in West Germany, each .of which is mainly concerned with studymg the composition and character of the respirable dust produced in mines or at industrial sites. The three institutes are discussed as regards their orgamzat10n and duties, their methods of dust sampling and analysis, and recent trends in research and development. An extensive bibliography completes the paper. Introduction A RECENT trip to Europe to attend international conferences m Austria and m Belgium gave the writer the opportunity to visit a number of research laboratories and mines in Western Europe. The visits to the dust research laboratories in West Germany are the basis of this report. In Germany, cooperative associations formed prior to and since the turn of the 20th Century to promote the safety and welfare of their workers have recognized the effects of airborne dust on the general health of the labour force.
