Safety conditions in industry have greatly improved over time. However, around the world, in Canada, and in each province or territory, people are still the victims of serious industrial accidents. The increasing safety of people at work then continues to be an imperative in industry today.<p> So how do we achieve a workplace where no one gets hurt? Well, in simple terms, folks need to be protected from unplanned exposures to equipment or energy sources that can do harm. If you think about it, all of our efforts in loss management and safety should then be directed toward avoiding these uncontrolled events. And it works! Our safety performance over the years has enjoyed tremendous improvement due to this support for loss management and safety. We now explore best practices toward achieving that next step in safety improvement.<p> As shown following, a large combination of loss management activities act together to support safety performance. Many of these activities are interrelated and some people have described them as the ?inputs? into an approach or program for achieving safety. Using this description, it follows that the ?outputs? of the program are some of the advertised measures of safety we see around us today, such as injury frequency rates. <p> It is asserted that a leadership focus to regularly measure safety and loss management activites (so-called ?inputs?), as a basis for taking supportive actions to ensure their effectiveness, is the basis for safety results (so-called ?outputs?) that consistently achieve high standards of excellence.<p> The concept of managing input activities, as enabled by effective measurements, can significantly act to transform our focus and priorities in the workplace. Supporting input activities like strategies, plans, or procedures with our best efforts will get us to our intended goals. Tracking outputs alone will not.<p> Safety and production have traditionally been cast as competing objectives. When both objectives are mutually supported and considered, however, people will invent ways of doing work that support both safety and production, and any other business parameter. When done right, innovation through meaningful involvement of affected people in the workplace will support safety excellence in a manner that in turn improves business efficiency overall.<p> For a workplace to be safe, it then requires everyone?s help and participation to make it happen, a collaborative effort by management, leaders, and frontline folks with the power and responsibility to take appropriate action at every level in the organization. In this way, everyone becomes a leader in safety.<p> The achievement of commitment to loss management input activities by folks in the workplace is the basis for attaining safety excellence. A workplace founded on commitment supports a safety culture with a vision for safe work performance at outstanding or world-class levels of achievement.<p> One may ask if injury incidents will be eliminated with this commitment and focus on workplace safety? Well, despite our best efforts in this area, there is never a guarantee that things can?t go wrong. We are simply not perfect. There remains in our workplace complexities and unanticipated risks that can have disastrous consequences if not managed at every moment. And when it goes wrong, we need to know we?ve truly tried our best, and now we have more to learn so we can get even better. <p> That?s the challenge represented by the safety imperative, a challenge to take safety performance that next step, a fulfillment of our commitment to the moral obligation we have to each other?s safety. It?s an effort that will continually raise the bar of world-class safety performance achievement for future generations. It?s a legacy we can be proud to be part of.

COAL-mine opera- tors face a difficul- ty in notifying men when there is no work. If the miners come to the shaft when there is no work, they must be paid, by agreement, two hours&apos; wages. This involves a considerable loss since it is not uncommon to em- ploy two or three hun- dred men or more at a single shaft. Many mines have solved the problem by using an electric siren. This carries the "no work" message to all of the miners within a ra- dius of from one and one- half to two miles, so they do not trudge uselessly to work and the operator saves the two-hours wage payment. At the Valley Smokeless Coal Co., Johns- town, Pa., such a siren serves as a start and quit- work signal for three mines which previously re¬ quired three individual signal systems. Similar units are used by the Old Ben Coal Corp. mines, Dolly&apos;s Smokeless Coal Co., Rhonda Coal Co., Bird Coal Co., and hundreds of others. Many other applications of the electric siren have been found in mines. They are used on underground locomotives, to awaken miners in the morning, to tell them it is time to come to work, to sound the start and quit-work signal, and to warn of fire or accident. The sirens are most generally used to give a start and quit-work signal, fire, and general emergency alarms as well as no-work signals. They require 3 and 5 hp. to operate. The smaller unit has a sound radius of from three-quarters to one mile, while the larger siren can be heard over a distance of from one and one-half to two miles. The 5-hp. unit has been heard for as great a distance as 14 miles, and 10 miles is not at all uncommon. One of the outstanding features of the 3 and 5-hp. units, which is uncommon in the smaller sirens is that they may be adapted to sound code signals. When used to sound a fire or general emergency alarm the siren may tell at the same time where the fire or acci-dent is. When a remote control device is used with a siren it is possible to operate the siren from any number of distant points. Any number of "start" buttons may be placed at various places. By using one "start and stop" button where the remote control is mounted, and one or more "start" buttons at various places about the plant, it is necessary to run only two wires to the various "start" buttons. If "start" and "stop" buttons are used altogether, then three wires must be run.

Although the interierometric method for the determination of length changes was devised by Fizeau more than three quarters of a century ago, it has not achieved the widespread use in the metallurgical field that its precision and simplicity would seem to warrant. The interferometer and the basic principle are both very simple. When optically plane surfaces held at a very slight angle to each other are illuminated by monochromatic light, rectilinear, equally spaced interference fringes arc formed. Each fringe corresponds to a defiuitc thikness of the wedge formed by the optical flat.;. The difference in wedge thickness between two consecutive fringes is cqual to one half the wave length of the illuminating light. Instruments for vicwing the intertero~ncter have passed through a number of modifications. Outstanding among the improvements were those Abbe. in 1884. His improved instrument permitted micrometric mcasurements to the order of one-hundredth of the width of an interference fringe (i.e., lengths were measured to the order of one two-hulldredth of the mave length of light ). It also permitted the consecutive use of light of several wave lengths. which made possible the determination of the order number of any fringe and therefore the absolute thickness of the wedge at any point. The Abbe dilatometer and methods of measuretment, including corrections to he applied for the change of index of refraction with change in tempcrature and barometric pressure in an air wedge, have been thoroughly described by Pulfrich.&apos; Pulfrich2 made further improvements in the interference measuring apparatus. His instrument embodied all the features of the Abbe dilatometer but was made more versatile by its complete mechanical separation from the interferorneter. The introduction of an Amici prism in the piece permitted the simultaneous observ tion of the whole set of fringe systems developed in the interferometer from the visible light of a geisler tulle. .Also, the introduction of a direct-vision, reflecting prism made it possible to set the fringes parallel to the two vertical fiducial line. of the instrument without the necessity of rotating the interierometcr. Interferometric methods employed ill this country have been developed and dcscribed by Priest, Pelers and Cragoe," Austin,5 Merritt,= Saunders,? and Nix and MaNair. Since none of these authors used the type of instrument that was employed in the present investigation, they Will not be discussed here. INTRODUCTION * The United States is fortunate in having enormous reserves of metallurgical-grade coals. Although these coals are better in quality and more accessible than most coals found throughout the rest of the world, the best grades of these coals are being depleted. Consequently, more efficient exploration tools and techniques, such as advanced methods of core drilling and logging, correlation by palynology,** stratigraphy, paleontology and petrography are being used to help assess the extent and quality of the remaining coal reserves. Exploration and assessment of new and existing coal properties become of utmost importance in determining (1) the economic justification for mining the coal, (2) the optimum design of the preparation plant and (3) the most effective use of the coal. Fortunately, the cost of mining coal has remained stable because of better mine planning, more efficient operations and increased mechanization of mining operations. However, increased mechanization raises the amount of impurities in the coal mined, and this requires more and better coal beneficiation to meet the more stringent quality requirements set by the market. Besides, mechanical miners also produce relatively fine coal and, to ensure maximum liberation of impurities, much of the coal presently being used must be crushed to a finer size prior to preparation. The bulk of all coals presently used in this country is mechanically cleaned, much of it at a lower specific gravity than in the past. This necessitates the use of new and improved processes and equipment for coal washing and recovery, such as heavy-medium cyclones and vessels, and froth flotation cells. However, because wet-cleaning processes are pre- dominant in the United States and fine coal retains large amounts of moisture, the increased production of cleaned fine coal accentuates the need for new or improved methods and equipment for dewatering and drying this coal. The larger amounts of coal being beneficiated and the use of new and improved beneficiation processes will aid in controlling and ultimately eliminating sources of pollution. Modem coal markets demand coals that meet rigid specifications. Most coals are purchased under contracts that specify moisture, ash, and sulfur contents, or heating value, depending upon the end use. There has also been a growing trend toward specifying other properties, including particle size and coking capability. Manufacture of lightweight concrete aggregate from common shale is one of the most rapidly growing industries in the field of nonmetallic or industrial minerals. Although expanded shale" has been in use since it was developed by Stephen J. Hayde in 1917, it was not until after World War II that it became a substantial competitor with other aggregates. There are some 70 plants now producing this aggregate in the United States, approximately half of them in states west of the Mississippi River. Expanding plants throughout the U. S. are grouped around major urban centers such as Los Angeles, New York, Chicago, and Denver. Concrete made with expanded shale aggregate combines the desirable features of low weight with relatively high strength, making it especially useful in concrete block, in structural concrete for multi-story buildings and in a variety of specialized concrete products such as prestressed panels and beams, and storage tanks. As the cost of construction continues to rise, lightweight concrete will probably be used in increasing quantities because of its low cost relative to the more traditional construction materials such as wood, brick and steel

The National Institute for Occupational Safety and Health (NIOSH), Pittsburgh Research Center (PRC), has conducted research to improve electrical fault detection on coal mine direct current (dc) trolley systems. Present circuit breaker protection systems are current-magnitude-based and cannot discern between normal traffic and high resistance electrical faults. Tests were conducted at cooperating mines to assist in developing a neural-network-based detection algorithm for distinguishing between electrical faults and normal operation. The Fast Fourier Transform of the trolley rectifier current signature served as baseline data for the neural network. During field tests, the algorithm performed with greater than 90% accuracy.

THE 142D MEETING of the Institute is rapidly shaping up with splendid prospects of a strong technical program and the lighter side of this annual occasion amply provided for. The date is February 20 to 23. The technical sessions will be held in the Engineering Societies building; the smoker-dinner, the informal dance, and the annual reception and dinner, at the Commodore Hotel, Monday, Tuesday and Wednesday respectively. The news to date on the technical side is as follows: Mining Methods-Subsidence The Mining Methods Committee has prepared an interesting program which will be divided into two sessions to be held early in the week. At the first there will be papers on mining methods and development, including a description of the "Sinking and Equipment of No. 2 Shaft at Minas de Matahambre," by Dudley D. Homer and R. H. Cromwell. They describe in considerable detail the equipment used and the practice followed in sinking this important shaft, giving detailed costs and performance figures. Hoist¬ing ?arrangements and mining practice at the "Red Ore Mines of the Woodward Iron Co. at Bessemer, Ala.," are described by Tenny C. DeSollar, superintendent of mines, in a second paper. At this session of the Mining Methods Committee, H. M. Roche and R. E. Crockett, of the Committee on Eastern Magnetite Mining and Milling Methods, will present their paper, "Iron Ore Mining at the Scrub Oak Mine." This mine is in the highlands of New Jersey about 40 miles west of New York City. Prof. Robert K. Warner of Yale University will present a paper on "The Selection of a Mining System," which should have an interesting discussion. Pierre R. Hines, of Portland, Ore., will present, with stereopticon slides, his paper on "The Owyhee Tunnels, U. S. Reclamation Project, Owyhee, Oregon," describing one of the recent irrigation undertakings.

"Throughout the years Cyanamid has been a supplier of cyanide and a supplier of information to the mining industry. In general, the knowledge of mineralogy is greatly overlooked. Mineral deposits in the Western hemisphere have been exceedingly rich in the past but now they are becoming poorer. In other words, we will have to use our talents to the utmost to extract the precious metals and base metals from these ores. Modifications of some of the mineralogical equipment displayed here belong in the mill.Cyanidation of copper has been one of the main projects in our labs in recent years. De-rimming or de-filming of copper in order to upgrade copper concentrate was the main aspect investigated. A study was made as to how wide spread was the filming of secondary copper on pyrite, and to what extent it affected the copper industry. It was very wide.Often there is no conception as to what the minerals are. One open pit copper producer with a chalcopyrite ore was producing a copper concentrate with a 5% copper value whereas chalcopyrite contains 35% copper. They used primary and secondary grinding and a regrinding section. The ore contained about 8 parts chalcopyrite to one part of other copper minerals. In tests, a 3 1/ 2% copper rougher concentrate, cleaned without regrinding but conditioned with cyanide for a low contact time of 30 seconds produced a 30% copper concentrate with 90% recovery by removal of the film. It was of utmost importance that some quantitative mineralogical analysis be made before flow sheet changes were made. Every man should be trained to do his own microscopic examination in order to get quick answers. The fundamental thing in all mineral dressing is to first of all know the minerals and the mineral associations."

To improve the understanding of the support interaction mechanics between grouted bolts and coal mine roofs and to help lay the foundations for improved design and evaluation techniques, the 0.8. Bureau of Mines has conducted laboratory, field, and analytical studies of the load transfer mechanics of grouted roof bolts. Mechanisms by which load is transferred from a bolt through the grout to the rock mass are analyzed. Strain gauges were installed on over 70 bolts and tested to observe the rate of load transfer when a force was applied at the bolt head. Laboratory and field tests were conducted to determine bolt-grout- rock interaction behavior in both the elastic and postyield phases. Long-term laboratory tests were run to determine the time-dependent properties of both polyester resin- and-gypsum-grouted bolts. Numerical studies using finite- element techniques were compared to the experimental results. Elastic tests showed that 90 pct of the applied load was transferred within 24 in of bolt length. Long-term tests indicated that creep is more significant in gypsum-grouted bolts than in resin- grouted bolts. Plastic studies showed that yielding of steel will translate down the length of the bolt from 6 to 20 in.

THE function of a pyrometer protecting tube is to maintain an atmosphere about the thermocouple most favorable to its continued accuracy and long life, and at the same time permit the weld of the couple to attain the full temperature of the area or object being measured. In addition to protecting the thermocouple from the chemical or alloying effects of the products of combustion or bath being measured, the tube must also protect the couple from mechanical injury. The conditions met in practice vary so much that no one material is suitable for all applications. The usefulness as well as the expense of maintenance of a pyrometer system are influenced greatly by a choice of the most suitable protecting tube. The tube must protect the couple without itself becoming too great an expense item. The protecting tube should have the following qualities: (1) A melting point well above the highest temperature to be encountered. (2) Sufficient strength through the entire range of temperatures to hold up its own weight and resist such shock and jar as are unavoidable. (3) It must be impervious to the atmosphere or bath to which it will be subject. (4) It must not give off any vapor injurious to the couple. (5) It must not form any oxides fusible below the highest temperature to be measured. The proper location of the tube in the furnace is almost as important as the choice of material. The tube must be placed as near to the work to be heated .as possible. It must be kept out of the direct path of flame, both because that would give too high temperature readings and because partly burned fuel will destroy the tube faster than the quiet products of combustion.

"Process control technology holds very significant potential benefits for many mineral and coal processing plants. The impact can be expected in terms of performance improvements (e.g. increased throughput, better quality control, higher availability), through reduced operating costs (e.g. lower reagent consumptions, manpower reductions) and in the form of intangibles (e.g. reduced vulnerability to manpower turnover, increased productivity in the user group). However, the installation of a process control system will represent considerable capital expenditure and will generate an operating cost. No management team would approve the funding for such a project without some reasonable measure(s) of the profitability of a proposed system. This paper describes a methodology for addressing the justification problem. Of particular interest is the approach to quantifying performance improvements, frequently found to be the predominant benefit of process control.INTROPUCTIONProcess control has become widely recognized as an essential component of any processing operation. It has direct impact on productivity, process efficiency and product marketability. The reported pay backs associated with successful installations underscore this observation. As a consequence, many operators who have little in the way of process automation are looking to initiate control projects, while those who currently enjoy some level of automation are looking to upgrade existing control systems (= process + instrumentation + hardware + software + operators + support). While the emphasis here would appear to be on retrofit process control projects, new plants, or those currently in the design stage, will usually find themselves in one of these categories shortly after startup 1."

James Barnes is Managing Director of James Barnes &amp; Associated Ltd which is a company that has invested ten years of development to provide developers with a transparent, user friendly consultation framework that identifies who needs to be consulted, how they are to be consulted and procedures required to manage the risks associated with getting all the wrong! Despite the assertion above, this paper is not about to provide delegates with the answer to whom they ought to consult or identify the expectations that Maori might have in regard to the consultation process. It would be a very brave person indeed who would venture an opinion on such a sensitive subject. There are no short cuts to finding out what expectations Maori might have on any given situation. Nothing can be taken for granted. What this paper will attempt to do however is provide practical insights into what might be on the minds of Maori interests as they deal with the issues associated with mineral resources, particularly those under Treaty of Waitangi Claim. The Treaty of Waitangi has a significant bearing on the current debate between Maori and the Crown regarding ownership of mineral resources in New Zealand. The Waitangi Tribunal is currently hearing a case presented by a number of Iwi over the ownership of oil and gas resources in Taranaki and Hawkes Bay. Others will follow. The Crown and Maori have never been able to settle differences over ownership rights without resorting to legal action and it looks like this scenario is about to be repeated in regards to minerals. The Crown has a duty to actively protect the Maori interest. This is a fundamental expectation in the relationship between Maori and the Crown. In the case of minerals, Maori have yet to see the Crown do its duty. The impact on developers has become evident through Maori objections to consent applications as the issues of mineral right remain unresolved.

Many improvements in the art of underground excavation have appeared during the past two decades. These improvements have been both in methods and in machinery and have resulted in more economical construction of today&apos;s tunnels. In the realm of machines, the developments of continuous boring heads for either soft ground or rock, random arm excavators and the sliding tunnel floor (Magic Carpet) have each made contributions toward more rapid tunneling. CONTINUITY OF ADVANCE Tunnel boring machines are often referred to as "continuous excavators". Continuous advance (without cyclical interruptions) has not been achieved, however, in any of the machines now in use. In most rotary-head tunnelers the thrust needed to make the cutters bite into the face is obtained from hydraulic cylinders reacting against anchoring devices held stationary by pads which grip against the sides of the tunnel. With this system the forward motion of the cutting head is continuous only until the propelling cylinders have reached their limit of extension. At this point the grippers must be released, moved forward and reset before a new cycle of advance can be begun. Some machines are built to obtain their propelling thrust from hydraulic cylinders pushing backward against newly-installed rings of tunnel support. This is the customary method for propulsion of shields. With it, each cyclical advance or "shove" must await the erection of a new ring of supports. Faster tunnel driving

The principal objectives of this study are to: (1) survey electrical practices in metal and non-metal mines, (2) survey electrical practices in metal and non-metal mills, (3) clarify the federal regulations governing electrical practices in metal and non-metal mines/mills, (4) analyze electrical accidents, and (5) make recommendations aimed at improvement of the safety of electrical practices in metal and non-metal mines. Information was obtained from literature searches, accident reports, visits to 27 mines and 12 mills, and discussions with knowledgeable industry and government people. The various electrical power systems and practices at the mines and mills visited are described. Similarities and differences based on the type of mining and the minerals being mined or milled are presented. The federal regulations governing metal/non-metal mine/mill electrical practices (Code of Federal Regulations, Title 30, Sections 55.12, 56.12, and 57.12) are presented and discussed. Explanations of each regulation are provided in an appendix. The attitudes of mine/mill operators towards the federal regulations are described. An analysis of the electrical accidents in metal/non-metal mines and mills during the period 1972-1978 is presented. Recommendations, including specific research and development activities, aimed at improvement of electrical safety in metal/non-metal mines are submitted.

This general report considers what promoters of deep basement projects want from their teams and whether they get it often enough. Most large projects are based on complex financial models where the funding is related to a future income stream. Project delays and cost over-runs can critically affect the financial model. Too often, delays and cost over-runs originate in the substructure and foundations. We must take positive steps to improve our industry&apos;s reputation for reliability.

A predicted increase in aluminium production and the possibility of a diminishing supply of petroleum coke for anode electrodes in alumina reduction cells makes it desirable to find alternative electrode materials. Browncoal char, with a lower ash content and with an elemental purity similar to petroleum coke, was produced by leaching in hydrochloric acid, washing with water, and charring at 1,150&#176;C. The cost of processing is estimated tobe competitive with petroleum coke.Without further processing the bulk density and real density were undesirably low, but by briquetting the coal before charring and crushing, the bulk density was improved to 92 per cent of the petroleum coke value andthe particle density was improved to 92 per cent of the density of petroleum coke by water displacement or 75 per cent by mercury displacement.Pre-baked electrodes were prepared from the char alone, mixtures of petroleum coke and char, and from petroleum coke alone, all bonded with coal tar pitch.The electrodes were tested for the usual physical properties and were evaluated for performance in a laboratory size alumina reduction cell.Electrodes based on brown coal char alone were found to be inferior in physical properties and performance, but mixtures of the char and petroleum coke, despite poorer density, electrical resistivity, and carbon consumptionon a theoretical basis, in performance tests gave up to 17 per cent better carbon consumption per weight of aluminium produced and 21 per cent better electrical current efficiency than the electrodes based on petroleum,coke alone.It is suggested that larger scale tests are warranted.

The use of phosphate depressants in the cationic flotation of silica from francolite in the Florida phosphate industry is rarely practiced. The occasional use of gelatinized starch derivatives and pH adjustments to assure a slightly alkaline flotation feed slurry pH have been used in some processing plants to improve selectivity. This paper presents a preliminary laboratory comparison of several potential phosphate depressants for use with the flotation of siliceous phosphates. It was found that starch is ideal for floating fine (-35 mesh) silica from phosphate, while sodium tripolyphosphate may be the best for coarser (+35 mesh) feeds.

INTRODUCTION The work described in the following paper was under-taken with a view to determining, if possible, the relative importance of some of the phenomena on which flotation seems to depend, in the hope that, if this information could be obtained, it would be of value in dealing with the problem as a whole. At the outset, brief reference will be made to three papers which had a considerable bearing on the course followed in the present investigation. In 1919, Langmuir, in a paper on &apos;The Mechanism of the Surface Phenomena of Flotation&apos;, showed that mono-molecular films of very great stability could be produced on solid surfaces with oleic acid, but that pure paraffin oil, though spreading readily on a dean dry surface, was easily displaced by water.

"The cleanliness and system evidenced in the accompanying photographs of the West Steward change-house will appeal to all mine operators. This design is the most efficient in use at Butte. It is the standard which will probably be used in Butte in all future construction and is the result of a long period of investigation and experimenting. It is important that such a structure be as nearly fire-proof as possible, because of the frequent fires originating within the lockers from matches, etc., in the men's clothes. The concrete floors permit the thorough washing of the building whenever necessary.The lockers are roomy and well ventilated. They are constructed of sheet steel and are warmed from below by steam pipes."
