"INTRODUCTION A positive workplace environmental, health, and safety (EHS) culture affords organizations and corporations many benefits, including increased trust and employee morale, sustainable EHS performance improvements, and reliable productivity and profit margin growth. Unfortunately, many leaders and EHS professionals struggle daily to achieve and maintain positive workplace cultures and consistent business results. While lack of management support, corporate or employee buy-in, resources, and time are common factors that conflict with an EHS culture and safe work behaviors and processes, there are many other actions that threaten the positive attributes of an organization’s culture. Often these culture killers are not recognized, or worse yet, ignored because they are considered taboo. For example, voicing observations or questioning long-held management practices or process that were initially designed to promote EHS values, but no longer do so. Unfortunately, this could be viewed as challenging authority, not being a team player or even not valuing EHS. And for those that speak up, the potential for negative consequences can be real impediments to positive change. Having an open mind, constantly striving to identify culture killers, and putting forth the effort to eliminate them are the first steps to begin reaping the benefits of having a sound workplace culture. This paper helps readers to: • Recognize culture killers that negatively impact fostering and maintaining sustainable EHS cultures in the workplace. • Understand how to prevent, avoid, and respond to culture killers, before they wreak havoc in your organization. • Assess EHS culture strength, and implement plans to continuously improve it. The desire to draw boundaries around concepts, put things into boxes, and nail down requirements for something that is virtually impossible to solidify for any length of time is quite fascinating! These desires can pose challenges when trying to define what the perfect EHS culture should look like. The very act of trying to define our culture tends to create silos, and these silos, in turn, can become “culture killers”. The following is one of the most common culture killers. UNBALANCED ORGANIZATIONS TRIGGER CULTURE KILLERS Unbalanced organizations occur when an organization’s constantly shifting focus forces other important needs to take a back seat--until the focus changes again. This balancing act can be detrimental and lead to catastrophic results including low morale, lack of trust, employee dissatisfaction, high turnover, increased incidents, poor profit margins, loss of market share, and ultimately into organization-wide failure."

Beneficiation of low-grade fine-grained hematite ores becomes increasingly important due to rapid depletion of high quality iron ore resources. However, fine particles, especially ultra-fine particles, make efficient beneficiation of these ores more difficult. Therefore, methods to reduce the influence of fine particles must be considered to achieve better beneficiation performance. Analysis and systematical beneficiation tests were carried out on an ultra-fine-grained hematite ore slime from washing a fine grained hematite ore. Ore particles were found to be presented in loose soil. While some of the ore particles were agglomerated, they were very fine and contained up to 76.5 per cent -0.023 mm material. In this paper, using a single magnetic separation flow sheet with sodium silicate (SS) as a dispersing agent, a concentrate containing 65.05 per cent Fe was obtained at an iron recovery of 68.58 per cent from the hematite ore slime of 56.65 per cent Fe. The single magnetic separation flow sheet was simple and reliable, the agent SS was effective in dispersing ultra-fine particles. THE petroleum economics session," held on Wednesday morning, Feb. 20, 1929, presided over by Campbell Osborn, chairman, proved to be of un- usual interest and resulted in serious and constructive discussion of the papers presented. The first paper on the program, "Gasoline Economics and Refinery Operation," was by H. J. Struth, petroleum economist of the Gulf Publishing Co., Houston, Tex. While this paper was intended to cover the re- finery situation, it also presented the author's reasons for believing that the oil producer should give cognizance to gasoline demand as a most reliable barometer by which to parallel the production of crude petroleum. The discussion which followed brought out some interesting points in connection with the immediate outlook for the industry as a whole during the present year. Some advanced the opinion that, regardless of the out- come of proration efforts, the refiner is in a position to conduct his operations on a reasonably profitable basis, provided that he exercises control over them. This would mean running less crude and resorting to wider utilization of cracking to produce the gasoline demand. Thus, if the refiner uses diligent care in scheduling his still runs, the producer will be obliged to carry over a large stock of crude this year, unless he is able to keep production below the limits prescribed by the demand for gasoline.

Site-specific blast vibration prediction can be more accurate than using a universal prediction equation. Vibrations from 37 blasts were monitored using three seismographs. Data analysis showed that square root, cube root, and site-specific scaling predicted ground motion with equal reliability. Imprecise prediction is caused by geologic and blast variability, particularly, inaccurate delay times. Vibrations at some mines were consistently lower or higher than vibrations predicted from US Bureau of Mines (USBM) equations. Vibration restrictions based on USBM studies could then be too conservative or too lenient. Predominant vibration frequencies were generally less than 50 Hz, but several blasts had significant energy up to 125 Hz. In some cases, energy was concentrated at frequencies corresponding to delay intervals.

"There are few people more qualified to speak of the tumultuous times in the global mining industry during the past four years than Newmont Mining Corp. president and chief executive officer Gary Goldberg.Goldberg stepped into his current position in 2012, just before MINExpo 2012. Gold prices had hit record highs not long before that ($54.15/g or $1,856.40/oz on Sept. 8, 2011), expansions were happening around the world and the gold mining sector was still living in the glow of one of its longest upswings in history.That all changed between April 8 and April 15, 2013 when the price of gold dropped by nearly $200/oz ($46.27/g or $1,586.20/oz on April 8 to $40.45/g or $1,386.80/oz on April 15), putting an end to the decade-long mining boom and forcing mining companies like Newmont to re-evaluateeverything.Three years later, with the gold sector recovering, Newmont Mining is not only surviving but is thriving with a pair of mines coming online, Long Canyon in Nevada and Merian in Suriname. On Oct. 1, the Merian Mine went into production on time and more than $150 million under the initial development capital budget.Goldberg, who was one of the featured speakers at the opening session of MINExpo International 2016 on Sept. 26, also spoke with Mining Engineering about the previous four years and the future of the industry.“Mining goes through cycles all the time and gold is no different,” Goldberg told Mining Engineering when asked about surviving the downturn since 2012. “I thought it was important that we take a good methodical and steady approach across the business to determine what was the right thing to do and to set the business up to improve performance. Anyone can cut costs for the short term, but we set out to do it in a sustainable and reliable way. We created a program we call Full Potential to look for sustainable ways to reduce costs and improve productivity. That program has taken more than a billion dollars out of our costs since we implemented it 2013 and that shows up in our results. Our all in sustaining costs are down 28 percent and our productivity is up 39 percent.”During the downturn, Newmont restructured its business model, sold off some assets and created a leaner, more nimble operation. It has paid off with the addition of two new mines and the acquisition of another, the Cripple Creek & Victor Mine in Cripple Creek, CO.“The gold industry had not seen a downturn in 12 years. We had half a generation that had not experienced a downturn, so helping them through that in an organized way and keeping a view toward the long term throughout the downturn was important. We were one of the few companies that continued to invest in assets,” he said."

This paper will address methods to manage hydraulic hose and coupling inventory so that parts will be available to build assemblies for the vast majority of equipment with as little inventory as possible. Topics also to be covered will include some basic hose assembly tagging techniques that will help keep equipment running by tracking problem hoses and building ?band aid? kits, having problem hoses premade in stock, and inventory usage tracking (one of the biggest problems for any inventory manager is knowing which couplings have been used so they can re-order). Cradle to grave tracking of hydraulic and industrial hoses insures reliable service of vehicles and ethical disposal of old hoses and couplings. With accountability for disposal of hose products that may contain chlorine (neoprene tube material in some hydraulic hoses) and carbon black that will be scrutinized more closely in the future, a proactive approach should be adopted in order to minimize maintenance facility problems.

The possibility of increasing the efficiency of mineral processing plants by means of computer control has prompted metallurgists to examine their flowsheets more critically in order to develop practical control strategies. Control schemes in use to date generally rely on the automation of traditional and previously manually regulated control loops, for instance the addition of reagents to maintain a constant assay or a constant pH. This approach is predicated on the availability of well-established primary sensors such as pH electrodes or on-stream analyzers. Noranda Research Centre, in cooperation with Noranda Group concentrators, has developed a froth level monitor and a pulp density meter susceptible of application in this type of control. Although some of the basic principles on which they are based have been known for some time, their incorporation into the design of reliable, direct-reading instruments which can be interfaced with a computer, is be-lieved to be novel*.

The Mons Cupri Cu-Pb-Zn deposit is situated within a complex Archaean volcanic sequence dominated by rhyolitic fragmental rocks. This sequence is underlain by a 150 m group of rhyolitic ash flow tuffs with minor conglomerate and sandstone, and is overlain by approximately 180 m of dacite and andesite flows. The Mons Cupri sequence itself consists of several elongate or circular rhyolite domes, flanked by rhyolitic fragmental rocks ranging up to 400 m in thickness away from the domes. The lithology of clasts (typically 1 cm to 1.5 m in size) provides strong evidence that the flanking fragmental unit was derived by explosive eruptions associated with the rhyolite domes. An approximately 15 m thick shale horizon, bleached and silicified at its base, overlies both the rhyolite domes and the frag- mental unit. A south-dipping chloritic pipe-like body, with near-surface dimensions of 1,000 by 250 m and narrowing at depth to produce a cone-shaped cross section, lies within the rhyolitic frag- mental rocks about 250 m north of the largest rhyolite'dome in the Mons Cupri vicinity. Its east west elongation parallels that of the rhyolite dome. Ghost fragments indicate that the material in this fissure is a severely chloritized derivative of the rhyolitic frag- mental unit. The last decade has seen a resurgence of interest in lunar exploration and the emergence of countries like China and India as space fairing nations. In 2004, the US announced a new Vision for Space Exploration, whose objectives focused on human missions to the Moon and then Mars. The near future is likely to see the emergence of a worldwide drive to revisit the Moon as the first step in investigating the Solar System. To date, the Apollo missions provide our only experience of human operations on the Moon or anywhere else beyond Low Earth Orbit. Much was learned from these missions. However, their short duration means that many of the environmental effects that will be important for longer-duration missions could not be quantified. In addition, long-duration missions and infrastructure on the Moon require new technologies and capabilities, which must operate successfully and reliably in this lunar environment. Developing these technologies poses significant challenges for the exploration program. The paper discusses the risks associated with the construction of lunar surface and underground structures and the challenges to adopting the standards and rock mass classification systems developed on Earth and their applicability to the Moon. Moreover, an exchange of views to explore near-surface geologic and geotechnical profiles of the Moon is emphasized and implications of the lack of knowledge on rock mass characterisation of the Moonrock mass, the lack of theoretical/empirical experience on the use of rock mass systems outside Earth, and uncertainties.

Solvent loading: It is agreed that it is advantageous, as far as reagent cost is concerned, to operate at maximum solvent loading. However, owing to the fact that no reliable automatic solvent flow controller is yet available, control tends to be erratic with the result that there is risk of losing uranium. As can be seen from Mr Tunley's plot, the graph representing total ammonia cost is fairly flat between loadings of 3.5 and 4.0 gpl, while the graph representing uranium loss in the raffinate is steep. It is, therefore, safer to operate slightly below optimum loading in the absence of absolute control. Should the purity of the final product gain importance, operation at optimum loading or even slightly above, may be advantageous. Solvent losses and clarification of pregnant solution: The cost of replacing solvent, as indicated by Mr Carman, represents a high percentage of the overall Purlex costs. It is, therefore, agreed that, amongst other factors, efficient clarification of the pregnant solution and mixer-settler design have an important bearing on the cost. However, as at Blyvooruitzicht, the Buffelsfontein plant can tolerate a suspended solids content of 50 ppm without serious deterioration in efficiency. An increase in the suspended solids content from 15 to 50 ppm normally accounts for an increase of approximately 1 ppm in amine loss. Clarification of acidic pregnant solutions is much more difficult than that of alkaline solutions. Sand clarifiers are being used at Buffelsfontein with moderate success. Clarified solutions containing less than 20 ppm of solids in suspension can be produced, depending on the quantity and type of flocculant used in the filtration process, the thickness and the grading analysis of the sand beds and the quality of supervision. An on-stream duty of approximately 0.15 gallon per sq ft of surface area is obtained. Hydraulic handling of clarifier sand, while cleaning, may effect the efficiency of a clarifier, as size segregation of sand particles may lead to inefficient "pockets" in a sand bed. Control of the solution level in the sand clarifier also affects the quality of the effluent. Exposure of the sand bed invariably causes a cloudy effluent. Recycling of the final and initial solution discharged before and after skimming the clarifier sand improves the clarification efficiency significantly. Post-precipitation of silica and possibly sulphate salts, after clarification, may give a false impression of inefficient clarification. This phenomenon is more pronounced when treating hot supersaturated solutions. Ageing and cooling of the solution before clarification assist in stabilising the solution and reduce precipitation after clarification. The maintenance cost of sand clarifiers at Buffelsfontein is in the order of 0.35 cent per metric ton of solution treated. A redundant ion-exchange column has been tested successfully, either as a primary or a secondary clarifier, depending on the quality of the filtrate treated. The conventional pebble and sand beds in the column have been retained. Operation was enhanced by the addition of a two-foot deep resin bed as a filtering medium. The suspended solids content was reduced to less than 5 ppm. Mixer-settlers: Mixer-settler design is a controversial subject but, in view of the high costs of solvent compared to relatively low pumping costs, it is believed that mixing and pumping functions should be separated. Separate facilities for pumping and mixing not only give greater operational flexibility but also permit eventual changes in the flow circuit as technology develops. Scrub technique: Unlike the pilot plant, where the crud was carried away in the aqueous phase, crud is carried over with the organic phase in the full-scale plant. This carry-over of crud into the strip bank aggravates crud formation in the first strip stage. This phenomenon may result from a slight difference in settler design and/or a variation in the relative phase depths in the two plants. Bypassing of the scrub aqueous solution from the fourth to the second stage was introduced in order to avoid dilution and partial neutralisation of the acid added to the third stage, as it is known that a low pH value enhances the transfer of iron into the aqueous phase. The low aqueous flowrate in the third scrubbing stage was overcome by introducing an aqueous phase recycle. Gleaning: Mr MacDonald's contribution on "gleaning" was most informative. This process for recovering solvent from raffinate seems to show at least as much promise as coalescers and other methods employed to date. Similar tests conducted at Buffelsfontein pilot plant gave 60 per cent amine recovery from raffinate containing 10 ppm amine. However, the circuit was not operated at equilibrium and recovery may be expected to drop as the amine concentration of the "Gleaner" circuit increases.

The uranium mine environment is characterized by airborne radon-222 and its daughter products to which miners are exposed through breathing and by gamma and X rays from the uranium-235 and uranium 238 chains to which the miners are exposed externally. The nature of the radon-daughter concentrations and the methods used to define and measure the exposures are explained through an extensive review of the literature. An improved passive track-etch dosimeter for personal use is proposed which has a measurement range of about 30-400 WLH with a 95% confidence interval to ~25% for individual detector measurements in relatively stable mine air. Some measurements of these dosimeters adapted to environmental use are also presented. A program of development for enhancement of dosimeter sensitivity and greater accuracy of measurement is outlined. Extensive gamma-ray survey measurements were made in two Colorado and three New Mexico mines. Follow-up personal miner dosimetry with LiF TLDs was done at the mine with the highest exposure rates, The average miner exposure rate measured was 4.27 mR per day, or 9.3 of the maximum permissible level averaged over 13 weeks. Comparisons are made with previous mine surveys and their value in estimating miner exposure is discussed. Some depth-dose measurements made using Lucite as the absorber are evaluated.

This bulletin is a revision of Bulletin 63 and is published by the Bureau of Mines in order that purchasers of coal for Government, State, municipal, or private use may be informed regarding advances or refinements in sampling coal deliveries and preparing specifications for the purchase of coal. The purchase of coal by the Government under specifications depending on the heating value of the coal, its content of ash and of moisture, and other considerations, rather than upon the reputation or trade name of the coal, was based on the fuel investigations begun by the Technologic Branch of the United States Geological Survey in 1904. The plan was first adopted by the Treasury Department in 1906. Since then the plan, variously modified in form, but the same in principle, has been gradually adopted by other departments until, at present, of the coal used by the Government, the total value of which approximates $8,000,000 annually, more than half is purchased under specifications. The Government publications dealing with the adoption by the Government of the specification plan, the number of contracts awarded on that basis, and the quality of the coal delivered under such contracts in the several fiscal years covered by the reports are given in publioations a listed at the end of this bulletin. Under the authority of acts of Congress making appropriations for analyzing and testing fuels belonging to or for the use of the United States Government, a laboratory is maintained at the headquarters of the Bureau of Mines, Washington, D. C., where samples representing deliveries of coal purchased under specifications for Government use are analyzed and tested. At this laboratory more than 1,200 samples have been analyzed and tested in one month. With the complete equipment and the efficient force employed, the analytical work has been so perfected that the analyses and heating-value tests of the samples received are as accurate as may be reasonably expected in laboratory work of this nature. One of the serious drawbacks to the general adoption of the specification method for the purchase of coal is the difficulty of obtaining at reasonable cost samples of coal that can be considered fairly representative of the commercial product delivered in wagons, railroad cars, or ships. Therefore the method of taking and preparing samples for shipment to the laboratory has been given fully as much care as the making of the analyses and tests, and a general plan of collecting samples fairly representative of the delivered coal has been evolved through various modifications and improvements based on experience and increased knowledge of the physical and chemical characteristics of the various coals that are purchased by the Government. The method that is in general use by the Government is described in the following pages. In connection with studies of the coal deposits of the country, of the best methods of preventing waste in mining, and of increasing efficiency in the utilization of coal belonging to or for the use of the Government, geologists and engineers of the United States Geological Survey and of the Bureau of Mines have visited more than 2,500 coal mines scattered through all of the coal-producing States and Territories. From each mine 2 to 8 or more samples were taken, the number depending upon the size of the mine and its output. The analyses and descriptions of the samples collected up to the end of the fiscal year 1913 are published in Bureau of Mines bulletins 22 a and 85,b and those of samples collected in the fiscal years 1914 and 1915 are presented in a bulletin that will soon be published by the bureau. A study of the analyses of these samples and of the samples taken from cars shipped from a number of the same mines shows that the mine samples are as a rule of higher grade than the average commercial shipments, particularly with respect to containing a lower percentage of ash and having a correspondingly higher heating value. This difference is due to the fact that the Government inspector, proceeding on the basis of what can be done by a good miner, usually removes more of the partings of bone, slate, and other extraneous matter from his mine samples than the average miner does in ordinary practice, the aim of the miner being to get the maximum number of tons past the tipple inspection, as his earnings are based on his output. The average miner, therefore, does not always take as much care as he might in rejecting the impurities. Long-term thermal stability of ionic liquids is essential for reliable performance of ionic liquids in any engineering applications. TGA studies were carried out on a series of ionic liquids with different cations and anions such as 1-butyl-3-methylimidazolium chloride ([C4mim]Cl), 1-hexyl-3-methylimidazolium chloride ([C6mim]Cl), 1-hexyl-3¬methylimidazolium tetrafluoborate ([C6mim]BF4), 1-hexyl-3-methylimidazolium hexafluorophosphate ([C6mim]PF6), 1-octyl-3-methylimidazolium hexafluorophosphate ([Csmim]PF6), and 1-butyl-3-methylimidazolium bis(trifluromethanesulflonyl)imide ([C4mim][Tf2N]). The short-term thermal stability of ionic liquids was evaluated by measuring the onset temperature of decomposition of ionic liquids, and the long-term thermal stability of ionic liquids was also evaluated by weight loss of ionic liquids over a long period of time at various temperatures. The experimental results showed that the long-term thermal stability of ionic liquids decreased with increasing temperature and was significantly lower than the onset temperature of decomposition of ionic liquids.

Stringent underwater noise limits at the Fairview Ave. N. bridge replacement on Seattle's Lake Union required an innovative approach to supporting the 470-ft long drill trestle. Screw-in foundations would reliably mitigate noise, but the required 500-kip bearing resistance were an order of magnitude higher than typical helical piles. This paper describes the process of designing, installing, monitoring, and testing 22-inch dia. helical piles 80 feet long with 36- and 42-inch dia. helices. A static load test confirmed that the conventional k-factor design method can be extended to capacities of up to 800 kips. A 375 ft-kip drive mechanism had to be fabricated because no North American equipment could supply the necessary installation torque. Real time installation monitoring and later "re-torque" tests confirm that helical piles gain capacity with time, analogous to driven piles under restrike. Underwater and on-land measurements confirmed very low noise, suggesting the possibility of widespread adoption where similar limits are imposed. Overall, this case history illuminates the need for innovative deep foundation types in response to ever-increasing regulatory constraints.

One phase of the Bureau of Minest work in the field of health and safety is the promotion of safe, satisfactory, and reliable respir atory protective devices. To insure that such devices will be available, the Bureau has established schedules of what it considers the minimum re- quirements for oxygen breathing-apparatus,3 gas masks, hose masks,5/ and mechanical filter-respirators (a schedule of requirements for hose- type supplied-air respirators is being prepared). Approval of oxygen breathing-apparatus is given by the Safety Division; approval of other types of respiratory equipment is given by the Health Division, both divisions being in the Health and Safety Branch, Manufacturers of respiratory protective devices may submit their equipment to the Bureau for approval under the requirements of these schedules. In mineral processing, the management of tailings may become increasingly more challenging with a change of ore type and the quality of water used in the plant. This, in turn, may affect the overall costs of the operation and water usage and, in the long-term, pose challenges for mine closure and rehabilitation. Depending on the applied treatment technique, the short and long-term properties of tailings are largely determined by the fine particles. Most clay materials and ultrafine particles exist in tailings from oil sands, coalmines and base metals, bringing about challenges in dewatering performance. Moreover, given the growing concern for water scarcity, the minerals industry is increasingly turning towards the use of saline water, which poses yet another challenge for tailings management. The high salt content alters the behaviour of clay particles, making dewatering a more complex process than in freshwater systems (Boshrouyeh Ghandashtani et al, 2022; Costine et al, 2018). High-density techniques for tailings management have been used for several years in the mineral industry. In this regard, one of the novel and promising dewatering methods is in-pipe or inline flocculation, where the polymer is added at high dosages to thickener underflows, giving additional (likely denser) aggregation, releasing more water on deposition compared to low solids thickening. Previous research in the field of flocculation has primarily focused on the process of flocculation itself, specifically the settling rate and size of aggregates, rather than the long-term impacts of the produced tailings materials (Liu et al, 2020; Jeldres et al, 2017). This is particularly true for the novel method of this technology, where polymer is added to the thickener underflow. Furthermore, these studies have used small-scale set-ups using buckets/beakers, plungers and mechanical stirrers for the flocculation process. Unfortunately, these methods lack the necessary control over the properties of the treated tailings, which can lead to irreproducible mixing and dispersion of viscous polymer solutions (Fawell, Costine and Grabsch, 2015). Consequently, a system that can work with a wide range of operational conditions for high-solid tailings material is essential for reliable and reproducible results. The objectives of this study were to determine the effect of different type of polymers in the presence of monovalent and divalent salts on the inline flocculation of artificial tailings. The next step was to evaluate the impact of high-dosage polymer addition on geotechnical properties such as settling rate, consolidated density, rate of consolidation and the hydraulic conductivity of tailings. In addition, this research aimed to fill a knowledge gap and investigate the feasibility of using polymer-treated tailing in removing the copper ions and then quantify the performance of produced aggregates under time-dependent processes.
