This report summarizes the results of a research project entitled "Fatal Accident Data Base Development." This project is the last project under Contract No. S0110601. Volumes I and 1I, "Industrial Engineering Study of Hazards Associated with Underground Coat Mine Production," summarize the original industrial engineering project and the Fatal Accident Report Analysis project. These projects were completed and accepted by the U.S. Bureau of Mines on December 10, 1971. Both volumes of the report have been made available by the Bureau to the public through the U. S. Department of Commerce technical information service. Another project under the same con tract, entitled "Equipment Operations Safety Study, ' was completed in January 1972 and the report was accepted by the U. S. B. M. at that time. The analysis in this Natal Accident. Data Lose Development report is closely related to analysis in both of the earlier reports. The original project involved an industrial engineering and safety of production operations for a representative sample of underground bituminous coal mines. The contactor was required to make recommendations 'in work practices, mining methods, equipment, and safety practices which can effectively reduce the fatality rate from rib and roof falls.' (Longwall mining methods were excluded from the study.) Time study and work element analyses were performed in over 50 mines as part of the field data gathering effort. Another requirement of the study was to develop sets of multiflow. Process charts, which are diagrammatic illustrations of the time-related interaction of mining, cycle job elements.

IT is well known that ferritic steels containing more than 15 pct Cr when subjected to temperatures in the range of 700" to 1000°F exhibit increasing hardness and decreasing ductility. The phenomenon has been widely termed the "885 °F embrittlement," after the temperature of most marked effect.'.' In view of the excellent review articles available in the literature3-6 only a brief account of experimentally established facts need be given here. The extent of changes in physical characteristics during embrittlement depends on chromium concentration and time at temperature, higher alloy content and longer time both promoting more rapid and extensive changes. In a 27 pct Cr steel, changes in impact strength and in angle of fracture in bending can be detected after only a 1 hr exposure at 885°F; after 50 hr this steel becomes quite brittle. Hardness increases slowly with time during thousands of hours exposure and may attain a maximum hardness number twice as large as that of the unexposed steel. Microstructural changes accompanying embrittlement have been described as an initial widening of grain boundaries followed by eventual darkening of ferrite grains. Embrittled steels etch more readily, e.g., the weight loss of a 27 pct Cr steel in acid solution may occur at a rate one hundred fold greater following exposure at 885 OF. Marked changes which accompany embrittlement have been observed in electrical resistivity, specific gravity, and magnetic coercive force. Changes in physical properties may be readily removed by heating at temperatures above the embrittling range, such as a treatment at 1100°F for 1 hr. It has frequently been noted that the 885 °F embrittlement suggests precipitation on a submicro-scopic scale of a chromium-rich constituent, the nature of which has not been revealed by X-ray diffraction. Progressive broadening of the body-centered cubic diffraction lines during embrittlement has been observed," and recent observations by Lena and Hawkes' upon single crystals have shown early asterism in X-ray photographs, disappearing within an hour at 900 °F. Many workers have ascribed8-13 the phenomenon to a precipitation of a phase (FeCr), which is known to cause embrittling effects at temperatures much higher than 885°F. Two general observations, however. suggest that a precipitation cannot be responsible for the 885°F phenomenon: 1—prior cold work greatly enhances a formation, whereas it scarcely affects the 885 °F embrittlement, and 2—the presence of an alloying element such as nickel or manganese may have an effect on the 885 °F embrittlement which is opposed to its effect upon a formation. The slight enhancement of a formation and 885°F embrittlement observed in the presence of elements with strong carbide and nitride forming tendencies' is probably a consequence of lessened chromium depletion of the matrix. The bar graph in Fig. 1 shows a typical example, taken from two 27 pct Cr steels used in this work, of the hardness after exposure for 10,000 hr at 900°, 1050°, and 1200°F. Steel A (0.03 pct C, 3.13 pct Mn) showed marked hardening at 900" and 1200°F, whereas steel B (0.12 pct C, 0.63 pct Mn) exhibited only the 900°F hardening. The cr phase was found in steel A at the higher temperatures but not in steel B. Presumably a formation is enhanced by the low-carbon and high-manganese concentrations in A," Thus there are two distinctly different hardening phenomena present which cannot both be ascribed to v precipitation without invoking a transition phase possessing remarkable properties. Materials A number of chromium steels exposed for long periods (5000 to 34,000 hr) at 900°F, as well as unexposed samples of one of the steels, were available for this investigation. Table I gives the chemical compositions and aging treatments of these steels. In addition to these steels exposed in the elevated temperature test furnaces of the National Tube Division, a number of high-chromium steels were heated for short periods in small laboratory air furnaces and lead baths. Supplementing these commercial steels, a sample of high-purity (0.018 pct C, 0.002 pct N) 28 pct Cr iron, exposed 1000 hr at 887°F. was furnished by the Union Carbide and Carbon Corp. In addition, an alloy of iron and chromium of high purity containing 46 pct Cr was used. This

Two aromatic-type chelating agents, 1,10 phenanthroline (phen) and 8-quinolinol (quin), were tested as specific depressants for magnetite in the cationic silica flotation of magnetic taconite concentrates. Micro and batch flotation tests, along with zeta potential and adsorption measurements were used. In the presence of a cationic collector, dodecylammonium chloride (DAC), the Fe/phen surface chelate electro-statically repelled DAC, and magnetite was depressed. The negatively charged Fe(quin)2(OH)2-on the magnetite surface attracted the cationic collector and made magnetite floatable.

EXPERIMENTAL work conducted at the Petroleum Engineering Laboratory of the University of California by L. C. Uren, J. Domercq, Jr., and J. Mejia has shown that small diameter wells offer tremendous resistance to the flow of fluids in their immediate vicinity. This resistance is due in part to the constriction of the area through which the fluids must pass as they approach the wall of the well, but is also accentuated by the increased volume which results from gas expansion. The latter in turn is a function of the deduction in pressure, the fundamental cause of fluid movement. If the diameter of the bore is enlarged several diameters, the capacity of the well, or the volume of fluid passing per unit drop in pressure per unit of time, is greatly increased without necessitating such high fluid velocities. The fundamental features which should be taken into consideration in the design of casing joints were stressed by J. B. Graham and E. E. Smith. These involved such points as correct thread design, high efficiency, and small diameter, together with the insurance that leakage will absolutely be prevented. The practical features of such design were ably discussed by engineers from leading steel manufacturers.

Palabora Mining Copper has a unique ore body composition which unlike other copper ore bodies has three main components in significant proportions; these are copper bearing ore, magnetite and dolerite. The high content of dolerite, a hard barren material, creates a high proportion of pebbles which normally are recycled back to the mill. This action increases the mill bearing pressure and power progressively until corrective actions are taken. A logical action to solve the pebbles generation would be to incorporate a pebble crusher. This in fact was done however two fundamental issues were encountered; the high content of magnetite and existence of a cyclone classification section. Due to the high content of magnetite, metal detectors have not been able to work efficiently to separate tramp metal which causes damage to the pebble crushers. The existence of cyclones in the circuit had a detrimental effect on the final product when the pebbles crusher was online producing coarse sandy material. Due to the difficulties in treating pebbles via conventional procedures and the unique Palabora ore composition and characteristics, sorting technology was investigated at laboratory and pilot scale. It was found that a combination of optical and electromagnetic sorting could efficiently separate dolerite from the copper bearing and magnetite ores. This paper reports the findings of the laboratory and pilot scale work performed and discusses the additional benefits to the circuit.

Accurate geometric models of orebodies establish the framework for reliable mineral resource estimates that provide a basis for further exploration and mine planning, as well as for financial models that ultimately determine the economic viability of a deposit. Traditional, explicit geometric modelling techniques that are utilised for resource modelling by the general mining software packages, such as MineSight«, involve manually digitising interpreted ore outlines that must be carefully linked to create valid geometric solid models. An alternative, implicit modelling method, employed by LeapfrogTM software relies on a fast method of global interpolation, which offers several advantages over traditional modelling techniques. Although previous case studies have documented the relative efficiencies of the two approaches, the accuracy of the two methods has not been previously quantified and compared. This study quantifies the accuracy of solid models created using explicit and implicit methods, and compares the differences between the models. Solid models of the Doris Hinge vein gold deposit (located in the Hope Bay district, Nunavut, Canada) are constructed from preliminary and follow-up drilling data using both modelling techniques. The accuracies of the solid models generated using preliminary drilling data are determined by measuring the 3D distance between the surface of the models and the actual contact location defined by the follow-up drilling data. Additional comparisons based on volume, cross-sectional area and surface differences were made between both phase 1 solid models and solid models constructed from phase 2 drilling information. The overall results of this study find that the MineSight« and LeapfrogTM solid model accuracies are virtually the same, and that their volume, cross-sectional area, and surface differences are comparable. This study concludes that the implicit modelling technique generates accurate solid models that can be used in place of explicit models as ore shells for resource estimation in certain circumstances.

6.4-1. Principles of Operation. The jet-piercing process is a patented thermal process which depends upon a characteristic of the rock which is termed spallability. In its simplest terms, spalling is considered as decrepitation resulting from differential expansion of rock crystals due to thermally induced stresses. The number of individual properties influencing the spalling characteristics of rock are so numerous and their interaction so complex that the only truly reliable way to ascertain the spallability of rocks under thermal shock conditions is to expose them to these conditions. However, laboratory and field work have determined the principal thermal, physical, and mechanical properties of rock which affect spallability (10, 18). The approximate relationship between spallability and these characteristics is as follows: [ ] where To is the critical temperature at which, it is believed, a rock becomes elastic or sufficiently plastic to prevent further spalling action. Thermal diffusivity takes into account the amount of heat used in saturating the medium itself during the migration of a heat front. The rate and degree of dimensional changes which influence the spalling tendency of the rock will depend upon the characteristic of the heat gradient in the rock. The greater the spalling tendency, the higher the piercing rate. Those properties most favorable to a high degree of spallability are: 1. Large linear thermal expansion at temperatures below 700°C. 2. High thermal diffusivity at temperatures below 400°C. 3. Equigranular interlocking structure with little or no fine-grained clay or mica alteration products. 4. No significant amounts of soft, low-melting, or elastic mineral or materials which yield after thermal decomposition.

The process of grinding mill quality assurance brings with it associated truths and myths. When the mill quality assurance process took its first great steps forward in the late 1970s and early 1980s, they were in response to the conditions existing, in our industry, at those times. In the late 1990s, we are faced with different conditions, and problems, and need fresh solutions. Yet the system developed for the early 1980s has produced no new approaches for the late 1990s, becoming instead more hardened and entrenched. This lack of development and actual resistance to research is now costing the industry significant amounts of money in late start-ups and useless scrap loss.

THE time has arrived in our national development when we must have- a definite national- program in the development of our great engineering problems. Our rail and water transport, our water supplies for irrigation, our reclamation, the provision of future' fuel resources, the development and distribution of electrical power, all 'cry out for some broad-visioned national guidance. We must create a national engineering sense of provision for the nation as a whole. If we are to develop this national sense of engineering wand its relations to our great human problems, it must receive the advocacy of such institutions as this. We, together with our sister engineering societies, represent the engineers of the United States. It is our duty as citizens to give voice to those critical matters of national policy which our daily contact with this, the fundamentally constructive profession, illuminates to us. Just as our medical associations voice the necessity of -safeguards to national health; as the bar associations of safeguards to our judiciary, so the engineers should exert themselves in our national engineering policies. We have none; but we need some, or the next generation will face a lower instead of a higher standard of living than ours.

(written discussion ?).¬Your secretary requested a brief description of the Russell Sage Foundation, in order that members of the Institute, if they should desire to avail themselves of it, would know what type of service is offered to them through our institution. The foundation was established in 1907 by the gift of Mrs. Russell Sage of $10,000,000 as a capital fund, the income of which is to be spent for the improvement of social and living conditions in the United States. One of the lines of activity which the foundation has felt would lead toward this end is the development and promotion of community surveys; and because the scope and methods followed in these surveys are very similar to the scope and method of much of the institution's work in general, an idea of its main activities can perhaps be given best by describing one of our surveys. Incidentally, it may be pointed out that antecedent to the survey stand at least two important facts: first, the recognition. that communities are active, not static forces, that they develop, that changes in. material and human relationships occur continuously, and that with them come new social and civic problems which must, in many cases, be diagnosed and prescribed for anew; and second, the reality of scientific advances along lines which make possible at least some measure of solution of the problems. Evidences of these changes are abundant; many of them are found in the rapid growth of cities in the last few decades. The Bureau of Mines conducted beneficiation and smelting studies on siliceous offgrade manganese samples to indicate effective methods of manufacturing silicomanganese from materials that cannot be economically beneficiated to meet General Services Administration (GSA) specifications for stockpiling. Laboratory and pilot-plant flotation tests were conducted on 2 samples, 1 from the waste-tailings dump of the Domestic Manganese & Development Co , Butte, Mont., and the other from the Nettie mine in the Butte-Philipsburg area. Smelting tests were made on the combined concentrates from the pilot-plant flotation tests and on part of the tailings sample The flotation scheme found to be adequate in laboratory tests and later in the pilot-plant tests consisted of flotation by a fuel oil-tall oil emulsion in an acid circuit (pH )-1-.8 to 5.0). The oil-emulsion reagent flocculated the manganese minerals and selectively floated them from the siliceous gangue, In the pilot-plant flotation test on Butte-mill tailings 90.1 percent of the manganese was recovered in a concentrate that analyzed 32.6 percent Mn, 4.8 percent Fe and 28.1 percent Si02. In the test on Nettie mine ore 90.8 percent of the manganese was recovered in a concentrate that analyzed 36 6 percent Mn, 5.5 percent Fe, and 26-4 percent Si02.

Not so many years ago the interest of the average mining engineer in money matters-aside from his pay check or his consulting fees-was confined to the per-ton cost of mining and beneficiating ore and to the amount of capital that would be required to erect the plant necessary to carry out such operations on a prescribed scale. The so-called "business men" assumed that questions of marketing the products, of profits, and of dividends were in their exclusive sphere-matters too intricate or, perhaps, too practical for the mere engineer! In the case of a new enterprise, the promoter shouldered the entire responsibility for raising the necessary funds, and incidentally he got the lion's share of profits. In fact, he frequently profited handsomely even though the mine earned little or nothing. Occasionally the engineer turned promoter or entrepreneur--conspicuous examples are Herbert Hoover, D. C. Jackling, William Braden and F. W. Bradley- but by and large he remained in a sphere of his own. To what extent this condition of affairs has changed is difficult to say. High aluminium steel is a promising advanced steel known for its exception properties and holds significant economic and strategic importance. However, the CaO-Al2O3-SiO2 based slag with high alumina content reacts vigorously with alumina refractories during the smelting process at high temperature, seriously affecting the safe and efficient production of furnaces and resulting in the formation of Al2O3-MgO non-metallic inclusions within the molten steel. This is a bottleneck problem for high-quality steel refining, and the key reaction path during the corrosion remains unclear. The dissolution reaction mechanism of alumina refractories in high alumina CaO-Al2O3-SiO2 slags under a weak static magnetic field was investigated in this work, using high temperature laser confocal microscopy and in situ radiation spectroscopy techniques. The results indicate that the interaction between the high alumina slags and the alumina refractories is governed by free radical reactions. The dissolution of alumina forms AlO and AlO2 radicals. The reaction rate increases with the C/S ratio of the slag due to its low polymerisation. The reaction between non-bridging oxygen and AlO2 free radicals accelerates the generation of AlO and O2ꞏ- radicals. The reaction layer composed of calcium aluminate is formed through the recombination of AlO, Caꞏ+ and O2ꞏ- radicals. The weak static magnetic field induces the Zeeman splitting of free radical pairs and intersystem crossing occurs, promoting the formation of triplet free radical pairs through the hyper-fine coupling effect. The reaction can be significantly inhibited as the triplet free radical pair does not meet Pauli’s incompatibility principle. This discovery perfects the structure theory of molten slags with radicals and provides supplement to the design of the highly slag-resistant refractories, and provides the theoretical basis for the development of external field protection technologies for high-quality steel refining.

The performance of carbon-magnesia brick in electric arc furnaces (EAF) is interpreted in light of their observed optical microstructures. Features at the bricks' hot faces were protruding periclase grains, metal globules, graphite-metal contacts and linear void patterns. These features were com-pared to similar features previously seen from microstructures of basic oxygen furnace (BOF) refractories. Modes of formation were sought; an attempt was made to interpret their influence related to wear mechanisms.

High-grade cobalt concentrates were produced from the complex Blackbird ore with very good recovery in continuous pilot-plant operations in which a low-grade bulk cobaltite-pyrite flotation concentrate was roasted in a multiple-hearth furnace to oxidize the pyrite selectively. The cobaltite was floated from the resulting calcine.

To minimize the equipment operator?s respirable dust exposure in enclosed cabs of mobile equipment by install-ing cab filtration and pressurization systems and by keep-ing doors and windows closed. Background Enclosed cabs are used on mobile equipment to protect operators from health and safety hazards. The primary health concern is overexposure to respirable dust. Examples of such equipment in mining include drills, dozers, haul trucks, and other equipment. When equipment is new, the operator?s dust exposure is normally at acceptable levels. As the equipment ages and gaskets and seals deteriorate, the air quality inside an enclosed cab can reach a point where it is no longer at acceptable levels for the equipment operators. In an effort to improve the air quality in enclosed cabs of older mining equipment, NIOSH has been performing research to retrofit cabs with new filtration and pressurization systems. Over the past few years, this research has shown that retrofit systems can substantially improve the cab?s air quality back to safe and acceptable levels. How-ever, this is only possible when a positive cab pressure is achieved and maintained. Thus, when a cab door or window is opened, the cab pressure drops to zero and the cab filtration and pressurization system is rendered ineffective. During a recent field study, the magnitude of this impact was noted when a drill operator repeatedly opened the cab door throughout the course of the drill cycle. Upon closer examination, it was determined that the cab door was being opened to manually guide the next drill steel into place each time an additional section of steel was needed to drill deeper holes.

The U.S. Bureau of Mines in cooperation with Jim Walter Resources, Inc. (JWR), Brookwood, AL, evaluated the use of sheathed explosive charges in breaking up large stones that can jam the feeder¬breaker or pan conveyor on longwalls and halt operations. Use of the sheathed charge reduced the downtime of the longwall panel to 15 to 30 min as compared with the 1 to 2 h required to drill and shoot the stone. Firing the sheathed charge at the face caused no significant damage to Iongwall equipment. Use of the sheathed charge also represented an improvement in safety by eliminating the exposure of miners to the inherent hazards of the face for extended periods of time. Another benefit, which is difficult to quantify, is the elimination of the temptation to shoot mudcaps or adobes, which is illegal in U.S. underground coal mines.

A new heuristic sublevel mining stope optimizer is presented. The optimizer seeks the best locations and lengths of a series of vertical raises that, together with the blocks linked to each raise, define a mining stope. Five design constraints ? the footwall angle, the hangingwall angle, the number of raises, the maximum distance of a block from a raise, and the minimum width required to move the farthest block towards the raise ? allow the shape of the sub-stopes associated with each raise to be controlled. The optimization is done on the locations and lengths of raises using a genetic algorithm to efficiently sample the parameters? space. For each raise, a local network is defined in cylindrical coordinates around the raise such as to impose the design constraints. A maxflow algorithm on the local network is used to determine the optimal sub-stope for each raise. All substopes are combined to define the global stope for the entire deposit. The best global stope is obtained using a genetic algorithm to find the raise parameters providing the best profit over the entire deposit. Two synthetic cases and one real deposit are used to evaluate the new algorithm and compare the results with the single-raise optimizer. The multiple raises approach leads to significantly improved economics compared with the single-raise stope optimizer, and the dilution is also substantially reduced compared to the single-raise case.
