This information circular is a revision of I.C. 7238, Danger from Carbon Monoxide in the Home, by L. B. Berger and H. H. Schrenk, issued in April 1943. Each winter brings its toll of deaths and serious accidents from the deadly gas, carbon monoxide. Newspapers carry warnings to the public of the, dangers of this gas, and frequent news items report the finding of persons dead at unconscious from the effects of carbon monoxide, yet these accidents continue. Either these warnings are not read, or they are unheeded by people who feel that, "It can't happen to me." It can happen to you carbon monoxide is no respecter of persons.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Three dimensional (3D) seismic exploration has been carried out since the 1980s in the mining district of China, where it has been playing an indispensable role in the high yield and efficiency of coal mining. Unlike oil exploration, this technique uses high density seismic acquisition technology. In recent years, it has focused on wide azimuth 3D seismic acquisition. Evaluation of geometry moved away from depending on folds, azimuth and offset, toward being based upon a focus-ray beam. Conventionally, coalfield 3D seismic interpretation focused on time domain and was based on kinematics information, which is only suitable for less complex geological conditions. To achieve high-resolution processing for the complex topography and structure in coalfields, data processing shifted from poststack to prestack. The means to improve the accuracy of interpretation for complex geological conditions are 1) integration of multiple attributes and multiple disciplines, 2) combination of seismic and geological data, and 3) comprehensive combination of time domain and depth domain data. With the demand for lithological exploration, high-resolution data processing must also take into account the requirements of preserving amplitude in seismic data. Seismic attributes technology, seismic inversion technology, and dynamic tracking interpretation are key techniques in 3D seismic exploration. They greatly improve interpretation accuracy, produce better results in the goaf space description of the coal-bearing strata and coal seam structure, and effectively reduce exploration costs under complicated geological conditions. Therefore, these techniques will be the primary means employed in the future to deal with coalfield complex structure and lithological exploration.

This paper introduces the Quantified Value-created Process (QVP), a decision-making process for use in mine design and operation. The implementation of the QVP is founded on a ten-step rock engineering design process and a ten-step strategic planning process. The potential of this approach is demonstrated by examples of value created as a result of good research, design, and planning. The QVP is divided into two parts, namely, a ‘planning the process’ section and an ‘implementing the process’ section, and can be considered to be an extension of the risk approach. All costs, direct and indirect, associated with a significant mining decision (design, operational, investment) must be quantified in advance of committing to that decision, to determine the value that will be created in the short, medium, and long terms. Ethical values, associated with health, safety, the environment, public perception, and social aspects, may be difficult to quantify in economic terms, but must nevertheless also be considered. In contrast with a traditional risk approach, the QVP will identify the upside, and allow executives to make more informed strategic decisions. In this context it is essential to actively engage upper management in the decision process.

History In approaching the subject of diamond-drilling practice, a brief history of the origin and development of the diamond drill may be of interest. The use of black diamonds for boring holes in rock was discovered in 1863 by Rudolph Leschot, a French engineer. In 1864, he employed a diamond bit to bore holes for blasting purposes in the construction of the Mont Cenis tunnel between France and Italy. The method was introduced and developed in the United States in the late sixties. The first deep hole was drilled for coal in 1870, near Pottsville, Penn., by M. C. Bullock. This hole was seven hundred and fifty feet deep. It can be inferred that the diamond drill was introduced into Canada in 1871, when a bore-hole, known as Harper's bore-hole, was drilled for coal at Springhill, Cumberland county, Nova Scotia; and at the old Silver Islet mine, near Port Arthur, the diamond drill was in use as early as 1872. Diamond Drills While M. C. Bullock was developing the diamond drill for exploratory work, the Sullivan Machine Company, of Claremont, N.H., was developing the diamond channelers and gadders for quarry work. These quarry machines were very successful until the high price of black diamonds became prohibitive. In 1878, Albert Ball, chief engineer of the Sullivan Machine Company, designed the Sullivan diamond drill so well known today. The first of these drills went to the Lake Superior iron ranges. After 1880, use of the diamond drill expanded rapidly and spread to all parts of the world where exploratory work was being done. In recent years, the type of diamond drill has not changed much, but better material is going into its construction. Steel castings have replaced cast-iron. Ball and roller bearings are now used instead of brasses and babbits. Gun lubrication has eliminated the oil-can. These improvements make a drill that can be operated for long periods without expense for repairs.

"The International Tunnelling and Underground Space Association (ITA) held its 42nd General Assembly along with the 2016 World Tunnel Congress (WTC) April 22-28 in San Francisco, CA. The event was co-organized by the Underground Construction Association of SME (UCA). This was the first time the event has been held in the United States in 20 years.As cities demonstrate strong needs in tunnels and underground spaces for public transportation, subway stations, hydroelectric infrastructures and other underground infrastructure needs, the WTC 2016 provided the 2,319 attendees from around the world a unique occasion to discover the latest state-of-the art innovations in urban planning and underground construction projects in the United States and worldwide.The Congress, held in lieu of the UCA’s North American Tunneling Conference, included an exhibit that attracted 226 exhibitors in 298 booths. The exhibit had on display the latest in technical equipment and services from some of the largest and best companies in the international tunneling industry. The exhibit hall was also the place where participants were able to interact with the authors of the 168 posters and receive answers to various topics directly concerning civil engineers: innovative procurement processes, cost and risk management, contractual methods, BIM technologies and examples of innovative underground infrastructures built worldwide. And the technical program included 353 technical presentations – 185 podium and 168 posters.What is ITA?ITA-AITES is a nonprofit, nongovernmental organization that promotes greater use of underground space as a key instrument in sustainable development. "Condon-Johnson & Associates, Inc. (CJA) with the assistance of design consultant, Brierley Associates, designed and installed a water tight shoring system utilizing several ground improvement techniques jointly. Prior to the installation of the shoring system, a test program consisting of the installation and testing of 7 jet grout columns; 1 soil mix panel; 7 soil anchors and 3 tie downs was performed to verify design assumptions. State-of-the-art quality control measures were employed throughout the project.The shoring system consisted of:•,Deep Soil Mixing (DSM) wall utilizing triple axis tooling to install overlapping columns along the perimeter of the proposed excavation to a treatment depth of 54 feet.•,Installation of steel piles in DSM to provide rigidity to the wall.•,Installation of a jet grout bottom seal, installed from surface to a depth of 58 feet below existing grade.•,Installation of 153 soil anchors, 1-1/4” diameter and 47’-6” length, to tie the jet grout bottom seal to the native soil below to resist hydrostatic uplift, installed from existing grade to a depth of 91 feet.•,Installation of walers and pipe struts for internal bracing•,Installation and stressing of 117 tie downs, 1” diameter and 16 ft length, to tie the pump station mat slab to the jet grout bottom seal, installed from inside the excavated pit.Subsurface conditions at the site consist of uncontrolled fill materials to a depth of 20 feet comprised of sandy silt and clayey sand. The native soils underlying the fill generally consist of soft and compressible lean clay and silt with varying amounts of sands to a depth ranging from approximately 44 to 54 feet below ground surface, which in turn are underlain by dense to very dense silty sand. The groundwater elevation at the site is approximately 8 feet below ground surface.This paper covers the design aspects, construction, and quality control measures undertaken by the geotechnical contractor, with supporting figures.INTRODUCTIONThe owner, County Sanitation District No. 2 of Los Angeles County, designed upgrades to their existing Long Beach Main Pumping Plant Facility. The upgrades consist of a new pump station that includes a reinforced concrete below-grade wet well and dry well, a two-story operations building, two 78-inch diameter pipe inlets, two 42-inch diameter discharge pipelines connecting the existing manifold, two pipe junction structures, two insert-type junction structures, and electrical transformers. This paper focuses on the excavation shoring for the new pump station. The original shoring design concept for the pump station excavation illustrated on the contract plans consisted of pressed in sheet piles, a jet grouted bottom seal with soil anchors, 2 levels of internal bracing, and tie-down anchors to resist hydraulic uplift forces on the buried pump station."

Mine closure planning integrated into life-of-mine (LOM) planning during the operational phase consists of many subcategories. One of these subcategories that needs to be fully integrated into the LOM planning is that of water management planning. Issues arise when mine water management infrastructure, which is required post-mine closure, is only considered closer to the end of LOM. Once the mine is closed monitoring is focused on water quality as an indicator of closure performance, hence inadequate upfront post-mining water management planning or integration into operations will result in environmental harm. The lack of considering post-mining water management is not uncommon in LOM planning. Poor mine site water management and planning are a wide reaching problem for the mining industry. Numerous active mines have been issued with environmental notices and penalties for contaminated water discharge; including issues with abandoned mines now being cared for by governments all over the world.Mitigating sources of water contamination caused by existing and closed mine sites that are leaching pollutants might be impractical, due to inaccessibility to the contaminants or final landform topography. The opportunity to consider and introduce post-closure required mine water management infrastructure is lost once the waste facilities are constructed. Mitigating contamination is made more problematic with mines located in remote areas, serviced by dedicated roads, water supply, electrical supply, etc. It is expected that these services will be decommissioned or drastically reduced, once closure is reached making remedial work much harder. Inadequate planning, to ensure access and easily maintainable water management infrastructure, could lead to erosion and contamination going unnoticed. There are numerous examples of poor mine water management practices in Africa, Asia, Australia, South and North America. In the United States more than 23 000 km of streams are affected by pollution sourced from some 50 000 abandoned or mature mine sites (DeGraff, 2007).CITATION:Boshoff, H J J, ?2014. Since the late 1970s, offshore mining of diamonds has gained more importance and now more than 60 percent of Namibia?s generally high-quality stones come from those offshore deposits, with a steady increase expected for the coming years. Diamonds in Namibia The mining industry is one of the three pillars of the Namibian economic development, along with tourism and fishing. Diamond mining is one of the two main mining activities in Namibia and diamond mining operations are rapidly moving from onshore to offshore deposits. This article provides a comprehensive overview of the current and future situation of the Namibian mining industry, with a special focus on the tasks and new technical developments for the successful excavation of beach and marine diamond deposits. The different deposits and the adequate applied mining technologies and methods are shown in examples.

The feasibility of building a calibrator for the DuPont D-100 audio dosimeter is discussed. The desired specifications are used as guidelines but the possibility of their relaxation is also considered. Commercially available equipment is surveyed and found to not satisfy the guidelines. Several alternate concepts are investigated, and recommendations for further work are presented.

The Pittsburgh Research Laboratory (PRL) of the National Institute for Occupational Safety and Health (NIOSH) has conducted full-scale explosion experiments, to evaluate the strength characteristics of various seal designs, used for safely isolating worked-out areas in underground coal mines. Large-scale explosion tests conducted within the multiple entry section of PRLÆs Lake Lynn Experimental Mine (LLEM) is currently the only accepted test method used by the Mine Safety and Health Administration (MSHA), for deeming a seal design suitable for use in American mines. These explosion tests are labour intensive, expensive to conduct, and can interfere with other critical underground safety and health research programs conducted by NIOSH. The PRL has developed an alternative seal evaluation method, based on a hydrostatic pressure loading concept, that can facilitate the in situ testing of seals in an operating mine. Two chambers within LLEM and one within PRLÆs Safety Research Coal Mine (SRCM) were used for hydrostatic pressure loading various seal designs. The results from these chamber tests compare favourably with those from the large-scale explosion tests in the multiple entries. In addition to testing seal designs at the required 20 psi static pressure level, the chamber test approach also allows for the determination of the sealÆs ultimate design strength. Size-scaling relationships for predicting the strength of seal designs as a function of entry size are also presented.

One of the many challenges in developing new metallurgical processes is choosing the best option for validating process design parameters and establishing the process operating window. Historically this has been achieved by constructing pilot or demonstration plants of considerable size and complexity, to enable integrated operation of all key process units for a significant duration. While this would appear to be the best option for minimizing technical risk, it is often impractical from a perspective of cost, schedule, resources or bulk sample acquisition. Such an undertaking can even provide a false sense of security, as the optics of running a complete scaled-down version of a commercial plant suggest a high level of process understanding, regardless of the challenges encountered during operation. Xstrata Nickel has undertaken numerous process development and improvement projects over the past 15 years to support green field project development and continuous improvement in existing operations. The approaches used have included; computer simulation, cold flow modeling, construction of pilot test units in third party facilities, commercial plant referencing and the more conventional construction of integrated test plants. Regardless of the strategy employed, the keys to success have included; managing stakeholder expectations, involving the commercial plant design engineers in the planning stage, anticipating subsequent data and sample requirements and most importantly, constructing and operating complex demonstration plants only as a last resort. This paper provides examples of various process development approaches used by Xstrata Nickel and discusses the rationale specific to each project.

In order that the reader may not be misled, I feel it necessary to comment upon the title of this paper. "Laws of Rock Behavior," one may take as being part of the all-embracing "Law of Nature" and this, according to the Oxford Dictionary, relates to the principles of conduct recognized as being pleasing to God. I feel it would be presumptuous to erect a code of rock behavior on this basis, so turn to a secondary definition, namely that a "Law" is that which is intrinsically reasonable. It is extremely unlikely that any group of geologists, or workers in rock mechanics would be in complete agreement about what could be taken to be "intrinsically reasonable." Therefore, this paper does not present any deified utterances, but rather deals with a few aspects of rock behavior which I consider to be intrinsically reasonable and with which I hope the reader will concur. The mode of behavior of rocks in the Earth&apos;s crust covers the range from wholly brittle to completely ductile ; with a deformation time scale which may be measured in milliseconds, for some events such as meteoritic impact, or in millions of years for the tectonic deformation involved in mountain building. A complete survey is, of course, outside the scope of an introductory paper, so I must be both brief and selective in my treatment. Any attempt to establish the mode of rock behavior in the Earth&apos;s crust should take into account data and relationships established in laboratory experiments and correlate these with field evidence. Consequently, the first section of this paper presents a brief discussion of the experimental aspects of rock mechanics which are pertinent to the subsequent discussion.

In most States of Australia it is a legislative requirement for underground mines to have a separate means of egress from the normal means of ingress to permit the safe evacuation of workers to the surface in event of an emergency. Such egress may be in the form of a ladderway equipped shaft/underlie or trafficable incline/decline. Additionally, there is a general requirement to have one or more airways, separate to the main shaft or other principal entrance of an underground mine, extending to the lowest level to permit ventilation to all parts of the mine. Fulfilling these requirement is difficult in the development phase of a mine when access underground is achievable by only a shaft or decline in the process by being sunk. What is frequently practiced in Australia is for production to then commence, sometimes in association with continuing development, in the same configuration without the establishment of this separate method of egress to the surface nor additional airway/s with reliance on refuge chambers to cater for back up in any emergency. Not only, could this jeopardise the health and safety of workers in case of an underground emergency it could be construed as a statutory breach of legislation. While recognising that refuge chambers are often used throughout underground operations nowadays their equipping and the criteria for their location needs to be bought into focus. This presentation discusses how such legislative requirement can be met alongside the use and appropriateness of refuge chambers.

The nature, usage and comparatively rare occurrence of ball clay is described. The importance of these white firing plastic clays in ceramics, particularly in the production of industrial sanitary ware and tiles, is emphasized. The US deposits are briefly discussed. What is known about relevant deposits in Brazil, Argentina, Colombia and Venezuela (the four largest ceramic producers in South America) is also described. The ceramic plants have been sited near the centers of population rather than the plastic clay deposits. At present, extraction is probably concentrated on surface residual deposits and lenses which require minimum investment. Native plastic clays are used exclusively in the tile plants, but in sanitary ware, they are often used as extenders of imported ball clays.

The management of mine tailings poses a complex range of potential risks, including physical risks, such as the risk of catastrophic failure of a tailings facility and chemical risks such as the risk of long-term leaching of contaminants into the downstream environment. If these risks materialize, they can lead to a wide range of impacts on people and the environment, including human fatalities and severe impacts on downstream ecosystems, and the costs of recovery borne by industry, communities, governments, and society as a whole can be immense. Tailings facility failures in Canada (2014), Brazil (2015 and 2019), South Africa (2022), and in other locations around the world underscore how high the stakes are when mines produce and manage tailings (WISE Uranium Project 2019). Understandably, many people do not trust the mining industry to manage tailings responsibly. As an industry, it is our responsibility to manage tailings in a manner that is responsible and strives to minimize harm associated with tailings management, including effectively managing both the physical and chemical risks associated with tailings. Standards have an important role to play in responsible tailings management by setting expectations for standards of care and providing a means to measure and report on performance. In jurisdictions with strong legal requirements for tailings management, standards play a complementary role, pushing performance improvements in areas that may be difficult or impossible to address in legal requirements. In jurisdictions where legal requirements are lagging, standards may represent the de facto leading practice and may be the primary tool to help ensure responsible tailings management. Implementation of standards can help companies demonstrate that tailings are being responsibly managed and help build trust and credibility. Most importantly though, standards are important tools to help ensure that the risks associated with tailings are reduced, and that remaining risks are effectively managed. However, we must recognize that conformance with standards can have a down-side – the proverbial two-edged sword. Having standards is vital but there is a risk that implementing standards becomes a “box checking” exercise, with ongoing conformance leading to complacency. We must not allow ourselves to fall into the trap of believing that, just because we are complying with applicable standards and laws, we are therefore doing our best, and that “it can’t happen to us.” In addition, if conformance becomes the goal and the standard does not have mechanisms to drive continual improvement, then once the standard is met, there may be no incentive to improve. The point of complying with standards is not to pass the test. The point is to manage tailings responsibly, prevent catastrophic failures, and minimize harm. The point is to do better, because we must do better. To do that, we need to strive not for conformance, but for excellence in tailings management, driven by continual improvement and a quest to do better.

This Bureau of Mines publication contains analyses and related source data for 271 natural gas samples from 22 States collected during calendar year 1982. All samples were obtained and analyzed as part of Bureau of Mines investigations of the occurrences of helium in natural gases of countries with free market economies. This survey has been conducted since 1917. The analyses published herein were made by the mass spectrometer and chromatograph. The results of the investigation of the occurrences of helium are published annually to make the information available to the petroleum industry and the general public. Twenty-six publications have presented the results of 13,051 gas analyses vide prior to 1982. The first 3 bulletins (1-3)3 contain analyses and related source data on 5,218 gas samples collected from 1917 through 1960; these bulletins have been supplemented annually by information circulars (4-17, 19-24, 26) containing 7,833 analyses of samples collected since 1960.
