"In partnership with California Department of Transportation (Caltrans), Federal Highway Administration (FHWA), and with support from International Association of Foundation Drilling (ADSC), Deep Foundations Institute (DFI) organized a workshop on quality assurance (QA) practices for acceptance of post-grouted drilled shafts in conjunction with DFI’s 40th Annual Conference on Deep Foundations. The 1½-day workshop was held on October 15-16, 2015 at the Oakland Marriott City Center in Oakland, CA.Post-grouting, in the context of this workshop and report, refers to the alternative design and construction of base-grouted drilled shafts wherein a grout distribution system is installed during construction and used to grout the base under pressure after the drilled shaft concrete has gained adequate strength, resulting in a stiffer axial-compression load-displacement relationship and additional possible ground improvement effects.The primary goals of the workshop were to•?Identify and summarize the state of the practice for design, construction, inspection, verification, acceptance criteria, reliability and performance of post-grouting.•?Identify gaps and needs in quality assurance that should be addressed for successful use of post-grouting.This report is structured to follow the workshop program: workshop presentations outlined the state of practice and views from owners, engineers and contractors on acceptance of post grouting methods for drilled shafts; and subsequent workshop discussions attended by a small invited group of participants focused on identifying areas of agreement and disagreement on post-grouting best practices, acceptance criteria, design and verification procedures, and inspection requirements. A partial list of attendees of the open and closed sessions is provided as Appendix A, and copies of presentations are included as Appendix B.FHWA and Caltrans are the primary audiences for this report, and its content will be used to develop a roadmap for deploying the findings into policy and practice. The secondary audience is the broader industry group, and the report content will be disseminated in a variety of formats."

Pressure oxidation is increasingly being used for processing refractory sulphide ores and concentrates because it offers highly effective base metal and precious metal extraction while minimizing elemental sulphur, arsenic & mercury emissions. Yet pressure oxidation processes introduce new safety concerns and process risks as demonstrated by a number of safety incidents over the last 20 years, including overfilling, oxygen-metal fires, nozzle/sparger failures, and uncontrolled pressure relief events. Chemical Process Safety regulations in the United States and Canada mandate that companies constructing or operating chemical process facilities perform a rigorous hazard and risk analysis based on either ANSI/ISA-84.00.01 or IEC 61511 safety standards to determine the level of protection required for each hazard in the plant. This paper describes the application and implementation of three (3) unique safety instrumented systems developed by Hatch for ensuring the safe operation of three different pressure oxidation processes.

This report summarizes the cost of U.S, mining and milling industry compliance with the Environmental Protection Agency (EPA) interim water quality guidelines for waste discharges. Applicable excerpts from Public Law 92-500 are stated and interpreted in conjunction with the guidelines being used for implementation of the law. Over 200 U.S. Army Corps of Engineers applications for discharge permits were used to develop the quality and quantity data base needed to estimate the cost to comply with EPA guidelines, Cost of three basic treatment methods were considered: Aeration, addition of lime, and sedimentation. From the process design, cost-capacity graphs were developed to consider maximum flow rate, maxi lime addition, and average flow rates. The graphical method used in this evaluation should provide a reasonable guide to the effect of waste treatment and, therefore, Public Law 92-500 and corresponding EPA guidelines on the mineral processing industry. The cost of complying with the EPA guidelines will have the effect of prompting the mining industry to minimize the production of waste water and of undertaking some additional treatment, Two case studies are presented to illustrate efforts that might be fruitful for minimizing waste water production and for treatment of combinations of acid mine drainage, mill wastes, and metallurgical process wastes when treatment is deemed necessary.

This paper describes the location, features, qualities, extent, and potential of iron occurrences and some associative resources - limestone and dolomite, coal, petroleum and natural gas, power, and water - in New Mexico. Reconnaissance investigations were made of 118 iron occurrences to determine their extent, attitude, and potential; character samples were analyzed to determine their quality and impurities; and pioneer metallurgical investigations were made on character samples to present their metallurgical problems. Some factors influencing past and potential iron markets are discussed. The most abundant iron minerals are magnetite and hematite, The higher grade deposits are extensive contact-metamorphic and pyrometasomatic replacements in calcareous Paleozoic rocks associated with intrusives. Low-grade deposits of considerable potential are the extensive Cambrian oolitic hematites of sedimentary origin and the Precambrian magnetite-hematite taconite iron formation. Iron occurs also as extensive colluvial and alluvial concentrations. A wide variety of accessory elements are indicated and these pose metallurgical and economic problems.

INTERNATIONAL conferences on safety in mines originated in 1912 when, in response to an invitation issued by the United States, representatives of Austria-Hungary, Belgium, France, and Germany participated in a conference held in Pittsburgh, Pa The record shows that a permanent organization was proposed to be developed through. diplomatic channels. However, the advent of war 2 years later prevented this proposal from coming to fruition. No further cooperation between the Bureau of Mines and a group having: similar interests in a foreign country took place until 1924, when formal cooperation with the Safety in Mines Research Board of Great Britain was established. Interchange of personnel and results of research under that arrangement have resulted in a number of publications in both countries and more rapid advance in knowledge than could have been obtained through the same efforts exerted independently.

"The converting cost modeling software introduced by Ng et al. in 2005 [1] has been extended to consider the reprocessing of reverts and copper scrap. The original model evaluated the operational costs of a Peirce-Smith converter; algebraic relationships had been implemented with arrays (to represent matrices, vectors and scalar ratios). The new software brings flexibility to the underlying thermochemical balances, by using linked-lists instead of arrays. It is now possible to consider the blending of several feeds, including reverts and scrap.The software computes the length of the converting cycle from the Fe and S content of the charge together with the blowing rate; the times required for other operations (charging, skimming and idle time) are also accounted for. The costs of consumables such as flux, fuel, oxygen or compressed air are all evaluated. Downtime, labor, and materials for converter lining repairs are also considered. The software is easily extended to examine the costs of alternative operating strategies or injection technologies such as sonic single pipe injection and sonic shrouded injection. The potential cost benefits of alternative operating procedures and technologies are discussed through example calculations."

Electrical accidents occurring in coal mines from 1975 through 1979 were categorized and analyzed. The data was searched for common electrical hazards, common activities on the part of those injured, and common negligent or careless behavior. The most common electrical hazard was abnormal arcs or sparks, the most common injured activity was handling energized trailing cables, and the most common careless behavior was failure to de-energize circuits before working on them. The accident data was combined with mine production statistics to form a normalized electrical accident rate. It was found that some mines have consistently more lost workdays per man-hour worked than do other mines. Bureau of Mines electrical safety research contract expenditures were reviewed to judge whether the research effort was directed to those areas which are related to the most accidents. In some cases it was not. A method was devised and utilized to estimate the relative cost to the mining industry to implement certain safety measures developed through Bureau research, and also to estimate the potential safety benefits. Some safety changes are recommended, others are not. MSHA regulations were reviewed for effectiveness in preventing electrical accidents, and suggestions were made for discontinuing, clarifying, or otherwise modifying the regulations.

During the cyanide leaching process, gold and silver ores that come from the transition zones, which typically contains low levels of sulfide, may produce one or more of the following reduced sulfur species: sulfide ( HS−), polysulfides ( Sn 2−), thiocyanate ( SCN−), thiosulfate ( S2O3 2−), trithionate ( S3O6 2−), tetrathionate ( S4O6 2−), pentathionate ( S5O6 2−), hexathionate ( S6O6 2−). The actual speciation of the reduced sulfur species in the circuits will primarily depend on the redox potential, pH, cyanide levels, and existing sulfur species in recycled process water, as well as the presence of other reactive metal species. The development of an accurate analytical method for sulfur species in cyanide leaching units of gold ores where sulfur speciation occurs has an important role in explaining the metal recovery deviations. This study presents the speciation of sulfur species and the development of ion chromatography method to analyze reduced sulfur species thiocyanate ( SCN−), thiosulfate ( S2O3 2−), trithionate ( S3O6 2−), tetrathionate ( S4O6 2−) in cyanide leaching circuits for gold ores. The method developed has resulted in precise quantification of target sulfur species in leach solutions. Method development and validation results were illustrated on analytical graphs containing each species’ peak number, retention time (min), type, width (min), area (mAU*s), height (mAU), and area distribution (%) of the peak.

Pipeline or borehole free-fall transfer of material has long been done in support of underground mining. This paper addresses some characteristics and design elements in selecting systems suitable to the transferred media and the mine requirements. Though aspects of overall systems are discussed, principal attention is directed to the slickline/dropline/transfer line and the discharge diverter/rockbox/energy dissipator/boot. These terms are common to these systems, often used interchangeably, and will be used as appropriate to the material, equipment and construction. Following several design sections, fabrication and construction, and system operating and maintenance procedures are discussed.

This report, sponsored by the Bureau of Mines, presents the results of an examination of the safety of the Blind Shaft Borer (BSB) shaft sinking system and an alternative modified BSB design concept in comparison to conventional shaft sinking technology. Detailed study of the two systems was also performed to identify and recommend design changes for optimum safety conditions. A comparison of the BSB systems with conventional shaft sinking was made on the basis of relative risk levels. Risk incorporates both the frequency and the potential consequence of accidents. A safety evaluation of the component operations of the two BSB systems was performed using a Failure Modes and Effects Analysis (FMEA) methodology. The results of this analysis show generally that both BSS systems are safer than conventional shaft sinking. The potential risk of conventional shaft sinking operations could be reduced by over 30%, on the average, with the use of the original BSB system, and by an average of about 10% with the modified BSB. Design change recommendations to further improve the safety of these systems include a better fire suppression system, improved man haulage systems and more compact hydraulics. An operational safety manual was also developed for the modified BSB system.

The fact that coal is a complex organic material and heterogeneous in composition has made its study extremely difficult, particularly in regard to obtaining a fundamental concept of the processes involved in the formation of coke. It has been stated recently that coals are natural polymers and that in a complex reaction such as occurs during carbonization the differences between coals are quantitative and not qualitative. Such being the case, it would be expected, therefore, that a series of coals subjected to exactly the same conditions of coking would produce results that should be related quantitatively to the chemical analyses of the individual coals. In the past, however, this fact was not generally recognized and consequently many empirical test methods were devised for testing coals. The interpretation of the results obtained depended upon comparing a set of data with other data on coals of known characteristics. The testing procedure had to be con- ducted in precisely the same manner each time and not the least requirement in making the interpretation was experience. The test methods varied from those utilizing a gram of coal to the plant-scale tests in which one or more ovens were charged with the test coal. The aim had been principally to find some simple, inexpensive means by which the coke-making and other properties of coal could be predicted. It is not necessary to emphasize the importance of these endeavors, for anyone acquainted with the problems involved in the selection of coals for making coke is well aware of the time and expense involved in making a series of definitive tests.

History attests to the catastrophic consequences that fire, explosion and contaminated, noxious andirrespirable atmospheres can have for workplace health and safety in underground coal mining. It also confirms that fires and explosions present the greatest risk of partial or total loss of underground mines and equipment and that this risk continues to materialise. The design, implementation, operation and maintenance of the mine ventilation system are core controls for managing the hazards associated with these risks, both for preventing them from materialising and for mitigating their consequences if they do materialise. These are most effective when embedded in a risk management approach.

There were 63 fatalities in the California petroleum industry in 1929, the largest number in any year since 1923 and an increase of 34.0 per cent over 1928, when 47 men lost their lives in the drilling, producing, manufacturing, and marketing divisions of the industry. It will be observed from the accompanying chart that after following a consistent decline for 4 years, the fatality curve turned upward in 1923 and advanced sharply in 1929. Although this increase in fatalitics coincided with a period of intensive development it tends to indicate some laxness in the industry during a period of increased hazards. Intensive drilling campaigns are usually started hurriedly and conducted with a great demand for speed, and the result is that accident-prevention work is too often relegated to a position of minor importance. This glossary is published by the Bureau of Mines as a contribution to the mining literature in the belief that it will fill a long-felt need. It contains about 20,000 terms; these include both technical and purely local terms related to metal mining, coal mining., quarrying, petroleum, and natural gas, and metallurgical works; names of useful, important, and common minerals and rocks; and geological terms. It presents in one comprehensive volume the available standard, technical terms relating to the mining and mineral industry, as well as provincialisms that have been or are now in use in English-speaking countries. The glossary also includes many terms relating to ceramics and. the clay industry, glass making, foundry practice, railway and building construction, electrical installation and power-plant equipment, and chemical terms relating to metallurgical practice. Complete lists of terms for each of these allied industries are beyond the scope of this glossary. Paleontological terms, although closely associated with geology, are far removed from mining and metallurgical operations, and for this reason have been omitted.

Traditional mechanised resin rock bolting in hard rock underground mines requires the installation of resin capsules into a pre-drilled hole prior to the rock bolt being spun into a hole to mix the resin and set the bolt in place. In poor rockmass conditions, especially at depth or in highly altered faults and shears, the condition of the holes often deteriorates as the poor ground around it unravels and falls into the hole. This results in partially or fully blocked holes that either damages the resin capsule when they are being inserted into the holes or prevent it entirely until the hole is continuously cleaned of debris. This deterioration of the hole condition, and continuous need to clean and re-drill holes, also increases the diameter of the hole which in turn affects the continuously mechanically coupled anchorage mechanism of the rock bolt to the rockmass as there is simply not enough resin in the hole to fill the annulus between the bolt and the rock. This results in a large increase to critical heading cycle time and, more importantly, results in the rock reinforcement not performing as per design specification and increases the risk of instability. To overcome the quality control issues with resin bolt installation in poor ground, a single pass drill, install, inject and tension self-drilling resin bolt application was designed and implemented for use in such conditions. This system allows the operator to inject a two part polyester resin through a self-drilled hollow bolt to mix and fully encapsulate the rock reinforcement. The self-drilling bolt allows for post injection of the hole after the bolt has been installed into the rock and gives the operator manual or automated control on the amount of resin used in the hole to ensure complete encapsulation and continuously mechanically coupled anchorage to the rock. This allows the reinforcement to work as designed and provide the Geotechnical Engineer extra quality control confidence in these conditions. The implementation of this single pass resin bolting system resulted in significant decreases to the resin bolting cycle time and rock bolt quality control failures. In this paper the design, implementation and embedment into the development cycle will be discussed and the in situ quality control checks used to validate the increase in rock bolt installation quality will be shared. Mineral froth pumping utilising centrifugal slurry pumps is a major engineering challenge for both the end users and pump manufacturers. The traditional approach has been to oversize the pump for pumping mineral froth. Heat stress is a major concern for the health, safety and productivity of miners working in hot and humid underground mines. At greater depth, heat inside underground mines increases due to geothermal gradient, heat released from high-capacity machinery, exposed rock surface, auto-compression of air, human metabolism, etc. Prolonged physical work in thermal environments severely affects the health, safety and efficiency of miners. In order to address this important issue, a systematic literature review (SLR) has been conducted. This SLR aimed to identify the research gap in the area of heat stress effect on underground miners and its control. An extensive literature search was carried out on numerous digital databases, grey literature and conference proceedings to identify relevant research work for this review. Moreover, the impactful studies, methodologies and preventive measures undertaken during the past five decades on the effect of heat stress on underground miners were retrospectively analysed, with a broader aim to explore the need for further research on this pertinent topic. Overall, 571 references were identified from the literature review, out of which 51 research articles addressing the sources of heat, heat stress effects on miners, heat stress indices and control measures for heat stress in underground mines were selected as primary papers for detailed review. Pre-developed fatigue is one of the prime causes of health hazard, safety and productivity loss of miners working in underground high heat stress zones. Hence, development of a better heat stress index integrating personal, environmental parameters and pre-developed fatigue (PDF) of miners is essential.
